Abstract:This research examines the possible benefit of using humor to reduce anxiety associated with performing cancer self-examination behaviors. In Study 1, 187 undergraduates read a humorous public service announcement (PSA) script promoting either breast or testicular self-exams. Results suggest that perception of humor reduced anxiety about self-exams, which, in turn, related to more positive self-exam attitudes. Simultaneously, humor perception associated with greater message processing motivation, which, in turn, associated with more supportive self-exam attitudes. Self-exam attitudes also positively associated with self-exam intentions. These results were largely replicated in Study 2. Further, self-exam intentions predicted self-exam behavior 1 week later. However, consistent with past research, the humorous and serious messages did not generate differences in subsequent self-exam behavior, though the intention-behavior relationship was stronger and significant for those exposed to the humorous versus the serious messages. In light of these findings, and given that humor has the advantage of attracting and holding attention in real message environments, the use of carefully constructed humor appeals may be a viable message strategy to promote health detection behaviors.

Online short videos promoting public breast cancer literacy: a pretest-posttest control group trial on efficiency, attitude, and influencing factors

Abstract:Background Short videos on social media are playing an increasingly important role in cancer health education today. It is important to explore how the actual communication effect of health videos and the knowledge absorption of users are influenced by different factors of the video creation process. Objective The objective of our study is to access the factors influencing breast cancer health education through short videos on efficiency and quality. Methods Three pairs of videos about breast health were created and participants completed questionnaires before and after watching the videos. A paired t -test was used to analyze within-group change scores. RM-ANOVA was used to assess the relationship between the pretest, posttest, and three variables. Results Watching short videos can significantly increase viewers’ knowledge of related health topics ( p < 0.05). The viewers’ concentration level while watching was significantly higher for the video with background music (BGM) than for the video without BGM ( p = 0.006). The viewers’ willingness to share was significantly higher for the video with a progress bar than for the video without a progress bar ( p = 0.02). Using an interpreter wearing a doctor’s uniform instead of casual wear and setting a progress bar can significantly improve the efficiency of knowledge absorption ( p < 0.05). Conclusion A uniformed interpreter, BGM and a progress bar are factors influencing the efficiency of short health videos. They can be applied in video making to explore better ways of promoting cancer health education in the new mobile Internet environment.

Can Cancer Education Programs Improve Health Literacy Among Deaf and Hard of Hearing Patients: a Systematic Review

Abstract:Patients affected from hearing loss face many problems when visiting oncologists. We conducted a systematic review to survey if cancer education programs can promote health literacy among deaf and hard of hearing (DHH) patients. The authors searched two databases for RCTs, and cohort studies with interventions promoting cancer health literacy for adult DHH patients. Risk of bias was assessed with SIGN Methodology Checklist for RCTs, and cohort studies. Significance of mean changes over time, and mean differences between comparison groups were used to present outcomes of each study. Surveyed interventions addressed three domains: cancer knowledge, coping skills, and cancer screening. Key information was gathered and synthesized providing a juxtaposition of the content and presenting important effects in detail. Nine RCTs and seven cohorts with 1865 participants were included. In total, 13 studies showed that cancer health literacy interventions improved mean scores significantly from pre- to post-test measures. There are hints that captioning and written texts may be sufficient for milder forms of hearing loss. Three studies showed that resiliency skill training promotes various domains of well-being. Three studies indicated that educational interventions encourage cancer screening practices. Educational programs are an effective way to promote cancer health literacy among DHH patients to facilitate communication with oncologists. As extent of hearing loss was not assessed, the authors cannot say the degree to which results are applicable to all degrees of hearing loss. To obtain hard data, further studies with more diverse populations, various cancer entities, different methods, and exact hearing loss assessments are required.

Improving Patient Understanding and Outcomes in Lung Cancer Using an Animated Patient's Guide with Visual Formats of Learning

Abstract:Lung cancer patient education resources that address barriers to health literacy, improve understanding, and demonstrate improved patient outcomes are limited. Our study aim was to evaluate and report on learner knowledge improvement and intent to implement behavior change, and validate the benefits of the You and Lung Cancer website and YouTube resources. Our study occurred from November 2017 to December 2023. We evaluated audience reach (visit sessions, unique visitors, country origins, page views) and calculated top views by media type (animations, expert videos, patient videos). We assessed the impact and commitment to change through learner surveys (areas of interest, intention to modify behaviors, and intention to discuss disease management with providers) and tested the knowledge of learners pre- and post-reviewing of website content. Our program reached 794,203 views globally; 467,546 were unique visitors; and 243,124 (51%) were unique visitors from the USA. Of US visitors, 46% identified as lung cancer patients. These were patients in treatment (38%), survivors (8%), family members or caregivers (21%), and healthcare providers (14%) with other audiences unspecified (19%). Three areas of highest learner importance were the animations "Understanding Non-Small Cell Lung Cancer" (180,591), "Staging of Lung Cancer" (144,238), and "Treatment and Management of Small Cell Lung Cancer" (49,244). Our study confirmed areas of importance to lung cancer patients and suggests that visual formats of learning, such as animations, can mitigate health literacy barriers and help improve patient understanding and outcomes. Exporting this format of learning to other cancers has the potential to benefit patients and improve health outcomes.

Abstract:Deaf, deafblind, and hard of hearing (DDBHH) individuals experience barriers to accessing cancer screening, including ineffective patient-physician communication when discussing screening recommendations. For other underserved communities, culturally and linguistically aligned community health navigators (CHNs) have been shown to improve cancer screening and care. A needs assessment study was conducted to identify barriers and gather recommendations for CHN training resources. A community-based participatory needs assessment was conducted from May 2022 to June 2022 using three focus groups. Eight were cancer survivors, six advocates/navigators, and three clinicians. All questions were semi-structured and covered screening barriers, observations or personal experiences, perceived usefulness of having a CHN to promote cancer screening adherence, and training resources that may be useful to American Sign Language (ASL)–proficient CHNs, who are also culturally and linguistically aligned. Out of 20 focus group participants, seven self-identified as persons of color. Data highlighted systemic, attitudinal, communication, and personal-level barriers as recurrent themes. The most frequently cited barrier was access to training that supports the role and competencies of CHNs, followed by cultural considerations, access to cancer guidelines in ASL, dialect diversity in sign language, and the health system itself. Unaddressed barriers can contribute to health disparities, such as lower preventive cancer screening rates amongst DDBHH individuals. The next step is to translate recommendations into actionable tasks for DDBHH CHN training programs. As a result, CHNs will be well-equipped to help DDBHH individuals navigate and overcome their unique barriers to cancer screening and healthcare access.

Assessment of Lung Cancer YouTube Videos for Patient Education

Abstract:The internet is essential for obtaining information about lung cancer, which is the leading contributor to global cancer deaths. YouTube is a video-streaming platform that is popular among health consumers; however, the reliability of videos is variable, and few studies have evaluated their role in lung cancer education. This study uses a systematic approach to assess the characteristics, reliability and use of best practices of lung cancer YouTube videos for patient education. Using the search term "lung cancer," the first 50 YouTube videos were identified after applying exclusion criteria and removing duplicates. Two reviewers used a video assessment tool to evaluate 10 videos with minimal discrepancies. The remaining 40 videos were evaluated by one reviewer following a design based research approach. Under half the videos were published within 3 years. Mean video length was 6 min and 12 s. Video publishers were commonly from the USA (70%); were affiliated with a health care facility/ organization (30%), non-profit (26%) or commercial organization (30%); had a physician presenter (46%); were targeted towards patients (68%); and had subtitles (96%). Seventy four percent of videos supported optimal learning by including effective audio and visual channels. Lung cancer epidemiology, risk factors, and definitions (nature of the disease and classification) were among the most common topics covered. Prognostic and diagnostic information was covered less than expected. The reliability of the videos (measured by Modified DISCERN score) varied by presenter type; however, these results should be interpreted cautiously due to the absence of gold standard tools. This study encourages those producing health education videos to continue following best practices for video learning and provides strategies for healthcare providers and patients to support patient education.
Humor-Based Messaging in Testicular Cancer Awareness Campaigns: A Comparative Critical Review

Abstract:Extensive research showcases the extent and efficacy of humor-based messaging in general health promotion. However, the work describing humor's use within testicular cancer (TC) awareness is less developed. The aim of this comparative critical review was to determine the impact of using humor-based messaging in TC awareness campaigns to achieve a baseline assessment from which future research can be modeled. A literature search was conducted using seven databases to locate relevant literature. Three research questions guided this investigation: (1) To what extent has humor been used in TC awareness campaigns? (2) What does the literature reveal about the use of humor-based messaging on relevant health outcomes? (3) What are the limitations within current TC awareness strategies? Six studies were included in the review, of which three directly assessed the use of humor in TC awareness vis-à-vis intervention designs. Humor-based strategies were implemented to enhance knowledge of TC and testicular self-examination (TSE) procedures, reduce anxiety surrounding detection threat, and promote TSE. The rhetoric provided to men via various health interventions relied on humor and slang to promote TSE among males. Despite the small sample size of the included studies, this review determined that humor may be useful in reducing uncomfortable feelings surrounding TSE, increasing awareness of TC, and promoting TSE. When using humor-based messaging, however, the audience and type of humor implemented must be considered. Limited research exists assessing the long-term impact humor in TC promotion on health behavioral changes, easing anxieties related to detection, and increased self-efficacy surrounding TSE.

Abstract:PROBLEM: The deaf population is an often-overlooked limited English proficiency (LEP) group at risk for health disparities associated with low health literacy. Lack of access to health information conveyed via radio, television, or ambient auditory sources such as public conversation further aggravates this population's low health literacy. Methods of adapting health education material for hearing LEP populations do not reach deaf audiences with equal effectiveness.METHOD: We adapt health education material for deaf audiences by first determining the "learning points" contained in vetted source material. A dialog-based film script covering those learning points is created. Supplemental content addressing common deaf population knowledge gaps and sociocultural experiences is added. Deaf actors are filmed following the adapted American Sign Language (ASL) script. Their ASL is back-translated into English to yield vocal track and subtitle scripts. The source material author(s) are consulted throughout the process to assure the film's adherence to the learning point list.RESULTS: Users report that the adapted product is more relevant, engaging, and effective for deaf audiences.CONCLUSION: This adaptation approach may aid in reducing deaf population health disparities.

Effectiveness of using humor appeal in health promotion materials: evidence from an experimental study in Japan

Abstract:Background: Public health professionals have prepared and distributed many messages and materials to convince the public to adopt healthy behaviors or reduce risky behaviors. However, health promotion materials do not always have the desired effect due to a lack of ability to engage target audience. This study examined the effectiveness of humor appeal (i.e. using humor as an advertising technique to attract attention and increase acceptance of the message) in health promotion materials and how to use it effectively. Methods: Total 17 printable posters with different frames (loss- vs. gain-framed) × tones (humorous vs. non-humorous) × topics (advance care planning, cancer screening, donor registry, smoking cessation, and physical activity) were created and evaluated for comprehensibility, persuasiveness, and resistance through two web-based surveys. Participants who were Japanese adults aged 25-64 years were randomly assigned one of the posters (200 people each) and asked to rate it. The overall score was calculated as the persuasiveness score (EHPM 2017;22:69) minus the resistance score (EHPM 2022;27:20). Results: In the advance care planning case, the highest overall score was found in the gain-framed humorous poster, followed by the loss-framed humorous poster, and the non-humorous poster (p = 0.007). In the other 4 cases, the posters using humorous illustrations received a significantly lower scores than the non-humorous poster(s). Conclusion: The use of humor appeal can help improve the acceptability and persuasiveness of the message when dealing with a little-known resistance-prone health topic. Humor appeal will provide an effective hook to direct public attention to what they do not know or care about in public health communication.
Nomadic Culture : Towards An Understanding of the Cultural Impacts of Nomadic Computing

Abstract:The increasingly mobile workforce, organizations’ desire to be flexible and reactive, changing organizational structure and management style, and the wide availability of enabling technologies to support nomadic computing are gradually influencing the characteristics of an organization that eventually contribute to the organization’s culture. This study attempts to investigate the cultural impacts of nomadic computing. We coined the term “nomadic culture” to describe a collection of artifacts, beliefs, and basic assumptions that supports the technologies and procedures enabling an increasingly mobile workforce to achieve efficiency and effectiveness. This study offers a conceptualization of nomadic culture and proposes a framework for understanding the factors that affect the rate of formation of nomadic culture. Future research directions are also discussed.
Reducing information avoidance: The effectiveness of humour, cute animals and coping messages

Abstract:Objectives Guided by the hedonic surplus/mood‐as‐resource hypotheses, we sought to identify message components that prevent health information avoidance by inducing hedonic psychological states. Design Two experimental studies. Methods Participants, age 45–75, recruited from the online survey platform, Prolific (Study 1 N = 288, Study 2 N = 505), completed a survey of their colorectal cancer (CRC) information avoidance tendency and demographics. They were reinvited to participate in a study where they were randomized to view one of four types of images: humorous comics, cute animals, coping messages or streetscapes images (control condition). To assess CRC information avoidance behaviour after viewing the stimuli, participants choose whether to be directed to a website to complete a CRC risk calculator (Study 1), or whether to view a CRC information video or a video about foot care (Study 2). Using logistic regression, we regressed each outcome variable on interactions between self‐reported CRC information avoidance tendency and experimental condition. We then used the PROCESS macro to test if mood mediated these interaction effects. Results In Study 1, to the degree participants reported CRC information avoidance tendency, viewing humorous comics compared to control images increased their odds of choosing to view the risk calculator (OR = 5.26, p = .02). The same was true in Study 2 for choosing to watch the video about CRC vs. foot care (OR = 2.42, p = .04). Effects were not mediated through mood and there were no effects for the cute animals or coping messages. Conclusions Using humour at the outset of a health message may increase reach to people who otherwise avoid CRC or other health messaging.

Abstract:Objective To evaluate the impact of an online-delivered, theory-based narrative video on cervical cancer screening knowledge, attitudes, and beliefs in a predominantly Latino community. Methods The study design was a nonrandomized pretest–posttest evaluation. The eligibility criteria included men and women 18 years or older not previously enrolled in our community cervical cancer screening program who had internet access. Participants were recruited via in-person county-wide flyer distribution and social media dissemination. The intervention involved a narrative video designed for Latinas delivered via the internet. The 17-minute video is novella style, with a culturally tailored storyline and setting that covers cervical cancer risk factors and statistics, importance of screening, and addresses testing barriers. The measures were knowledge about cervical cancer and screening, perceived susceptibility, perceived seriousness, perceived benefits, perceived barriers, subjective norms, and self-efficacy. Analysis was performed using change scores for knowledge and psychosocial variables and descriptive statistics for satisfaction and acceptability of the video. Results Data from 227 surveys were analyzed; respondent mean age: 37.6 years; 98.8% female, 88.8% Hispanic; 42.0% had an annual income of $20,000 or less; and 85.2% had at least a high school education. Knowledge and all psychosocial variables improved significantly. More than 90% of the participants rated all of the satisfaction items as good or excellent. Conclusions and Implications for Practice A culturally appropriate narrative video about cervical cancer and screening disseminated online effectively improved knowledge and psychosocial variables among Latinas. Our findings indicate that health promotion interventions online could be effective for improving desired health behavior through a new means of educational dissemination by way of websites and social media outlets.
2' Life-changing minutes: impact evaluation of a novel health promotion format on cancer prevention

Abstract:Cancer is one of the most important societal challenges in the world, with over 23 million new cases/year and 10 million deaths/year, that will only be properly tackled with a stronger focus on prevention. This calls for an informed population, aware of risk factors and willing to adopt preventive behaviors and early cancer screenings. For that purpose, 2' Life-changing minutes was created, the first ever televised Entertainment-Education series on cancer prevention. This study aims to evaluate the impact of 2' Life-changing minutes, a novel E-E format for cancer prevention, on knowledge gains and behavior changes. Two complementary studies were performed involving a total of 1314 participants: a test-screening (TS) study targeting potential viewers of the series, and a natural-screening (NS) study targeting those that spontaneously watched the series on television. We found (i) very high levels of appreciation and narrative engagement, and also willingness to see more episodes; (ii) statistically significant knowledge gains, ranging from 17% to 44%, on all four topics tested; (iii) evidence of effective behavior change. Regression analysis showed that narrative engagement was the best predictor of behavior change [NS: odds ratio (OR) = 3.38, 95% confidence interval (CI) = 1.70-6.74, p = 0.001; TS: OR = 2.05, 95% CI = 1.13-0.371, p = 0.018]. This study demonstrates the series' real impact and serves as a proof-of-concept for a novel strategy of cancer prevention that is based around compelling health narratives, rather than information or data, to engage viewers, increase knowledge and induce behavior change.
Impact of a Cancer Health Education Curriculum Among Milwaukee Public High School Students

Abstract:In Milwaukee and nationwide, cancer incidence, late-stage diagnosis, and mortality are notably higher among some racial/ethnic populations. Cancer education has the potential to impact cancer burden and reduce cancer disparities. In particular, the addition of a service-learning component to academic curriculums has been shown to improve student learning as well as positively impact the surrounding community. This study implemented a cancer health education curriculum (CHEC) at a Milwaukee public high school with the goal of addressing cancer knowledge, fear and fatalism beliefs, and risk behaviors. The curriculum included interactive learning sessions and a service-learning final project. Five-hundred twenty-one students also completed pre- and post-surveys assessing cancer knowledge, fear and fatalism, risk behaviors, cancer-related communication, and a qualitative question asking what they hoped to gain (pre) or did gain (post) from the course. Results indicate (1) a significant improvement in cancer knowledge ( p < 0.0001), (2) a decrease in cancer fear and fatalism ( p < 0.0001), (3) an increase in fruit consumption ( p < 0.0001), (4) a decrease in screen time ( p = 0.0004), and (5) an increase in how often students spoke with their family about cancer ( p < 0.0001). Qualitative data reflect important gains such as increased interest in sharing their knowledge about cancer with their community. Providing cancer education and leveraging a service-learning requirement led to notable changes in high school students' cancer knowledge, fear and fatalism, and risk behaviors. Students also communicated more with family/friends about cancer. Such efforts could have broader implications for student, family, and community cancer burden.

Multilingual Self-Management Resources for Prostate Cancer Survivors and Their Partners: Results of a Long-Term Academic-State Health Department Partnership to Promote Survivorship Care

Abstract:Objective: To provide innovative, evidence-based self management information and supportive care for prostate cancer survivors and their partners. We describe how an academic-public partnership facilitated the broad dissemination of evidence-based, multilingual survivorship educational materials via a state-managed prostate cancer website. Methods: We outline the steps of an academic-public partnership leading to dissemination of online, survivorship materials as a resource for prostate cancer survivors and their partners. We examined the 5-year utilization of the materials from January 2011 to December 2015 according to 14 content areas (e.g., urinary, bowel, and sexual problems, fatigue, communication, cancer stress) and across 3 languages (English, Spanish, Arabic). Results: The total number of prostate cancer survivorship materials downloaded from January 2011 to December 2015 was 89,348. The number of downloaded materials increased over time from 6,421 in 2011 to 17,496 in 2015. The most commonly downloaded content area was urine problems (27.5%), followed by bowel problems (23.4%) and sexual side effects (16.2%). The majority of downloaded materials was in English (86.3%), followed by Spanish (9.8%) and Arabic (3.9%). Conclusion: The academic-public partnership facilitated broad dissemination of evidence-based informational materials for prostate cancer survivors and their partners through a state-managed website from 2011 to 2015. Given the increasing role of academic-public partnerships in funding and development of robust, sustainable prostate cancer survivorship resources, this work serves as an introduction to these evidence-based materials and highlights a successful model of engagement between practitioners, research scientists, and public health administration.

Abstract:Emotionally evocative public service announcements are one way that public health advocates hope to persuade people to take action against skin cancer, the most common type of cancer in the United States. This article describes an experiment (N = 193) to test the ways mixed emotional appeals influence communication and health outcomes. The data indicate that mixed emotional appeals foster feelings of compassion, which in turn motivate individual and social behaviors. The findings also provide insight into how audience reactions of fear impact postmessage behaviors. Implications for future research and health message design are discussed.

Abstract:e13659 Background: Secure messaging (SM) enables patients to ask clinicians questions asynchronously. Benefits include enhancing patient engagement, contributing to oral chemotherapy adherence, and lowering symptom distress. However, patients are unsure how to address concerns effectively, and clinicians find it challenging to decipher patients’ needs within messages. Coupled with clinicians not having adequate time to respond to messages, breakdowns in communication are common. We developed an interactive guide to help patients with cancer craft effective secure messages. It featured an oncologist providing strategies to write messages with specificity, expressing concerns, stating needs, being direct, and included a downloadable tip sheet. Methods: Patients 18+ with a current cancer diagnosis who were enrolled in the portal at a large health center were recruited. Between October 2023 and January 2024, participants were sent a link to interact with the guide, followed by a survey. The survey included several measures: 1) Communication and Attitudinal Self-Efficacy scale for cancer (CASE-cancer); 2) Digital Behavior Change Intervention (DBCI) Engagement Scale; 3) Website User Satisfaction questionnaire (WUS); 4) Acceptability of Intervention Measure (AIM); 5) Intervention Appropriateness Measure (IAM). We used R version 4.3.2 for the descriptive analysis. The Wilcoxon rank-sum test compared groups by demographic categories. The local IRB approved this study. Results: One hundred patients participated. Median age was 64 years (range=21-86), 54% male, 83% white, 35% diagnosed with prostate cancer, 35% lung cancer, and 23% hematologic cancer. Respondents reported high levels of self-efficacy based on CASE-cancer (mean 43.3 of 48). Respondents averaged 15 minutes using the guide. 96% thought the guide’s content was presented clearly, 95% found it easy to understand, 90% enjoyed using it, 89% thought the guide was suitable, and 88% could rely on its information. Results by demographic categories found that non-white respondents (M = 43.0) were significantly more engaged than white respondents (M = 28.4), P = 0.038 , and those with a bachelor’s degree+ (M = 96.5) had higher satisfaction than those without (M = 91.6), P = 0.025. Conclusions: Patients with cancer embraced an interactive guide that provided strategies to craft secure messages. They found it engaging, satisfying, acceptable, and appropriate, with racially minoritized patients being most engaged. As SM is a widely used tool for patient-clinician communication, it is crucial to concentrate on the way messages are constructed. We found potential to improve patients’ message-writing techniques, which could result in clinicians writing more complete answers to questions. Additionally, there is the possibility to enhance underserved patients’ communication skills and make them more engaged in their care.

Abstract:Necrotizing lymphadenitis of the neck. Histiocytic necrotizing lymphadenitis is a rare cause of cervical lymphadenopathy. It is a benign, self-limiting disease whose etiology is unknown. Cervical lymphadenopathy and fever are the most important signs. Here we report a 25-year-old female presenting with complaints of fever and a painless swelling in the neck for three months. An excisional biopsy of a posterior cervical lymph node was performed, and histological evaluation revealed histiocytic necrotizing lymphadenitis. This article discusses our case of histiocytic necrotizing lymphadenitis of the neck as well as the relevant literature.
Changes in knowledge and awareness for a community-based cancer screening educational program

Abstract:Background: Cervical cancer (CC), colorectal cancer (CRC), and breast cancer (BC) are diseases that can be prevented/detected through early test. Through educational programs, individuals can become better informed about these cancers and understand the importance of screening and early detection. However, many people, especially low-income, low-educated, uninsured minority population groups, do not have their cancer screenings at the recommended intervals and do not receive appropriate and timely follow-up of abnormal screening results or timely treatment after diagnosis. A community-based educational program was developed to improve knowledge and awareness toward the screening of the three cancer types in a South Texas underserved population. Methods: Residents living in Laredo, Texas were invited to participate in the present educational program. From January 2020 to April 2021, participants were recruited using social media and flyer distributions in the general community. Participants received a free live web cancer education presentation delivered by bilingual community health educators, and online pre- and post-education surveys for CC, CRC, and BC separately. Pre-post changes in knowledge for individual items were compared using McNemar's chi-squared tests. Results: Overall, the participants showed increases in CC (n = 237), CRC (n = 59), and BC (n = 56) screening knowledge and awareness after receiving the cancer screening education (Ps < 0.05). After receiving the cancer screening education, 85-97% of participants had an intent to talk to a healthcare provider about CC/CRC/BC screening, 88-97% had an intent to get a CC/CRC/BC screening test in the next 12 months or at the next routine appointment, and 90-97% had an intent to talk about CC/CRC/BC with their family members or friends. Conclusions: A community-based educational program culturally and linguistically tailored help increase knowledge and awareness about cervical, colorectal, and breast cancer screening, and promote positive changes in population's knowledge and awareness about the benefits of cancer screening. Future cancer screening educational programs in similar populations are warranted to reduce the risk of cervical, colorectal, and breast cancer.
