Abstract:Background: Trauma-related nightmares (TRNs) are distressing events which contribute to insomnia severity, chronicity and treatment resistance of PTSD. Therefore, recording TRNs is a crucial technical challenge in order to understand their physiopathological patterns and their impact on sleep. However, TRNs are difficult to record during a single night in a sleep laboratory, which, moreover, is likely to be considered by patients as a protective sleep environment that is therefore not representative of home sleep conditions. Method: In the present study, we investigate if objective sleep measures acquired at-home using two ambulatory devices is of clinical value by correlating with PTSD patients' complaints about sleep and nightmares. A secondary objective is to relate awakenings associated with TRNs to sleep stages and to provide new insights into the use of electrodermal activity (EDA) as a potential physiological marker of TRNs. Sixty veterans and active-duty service members were assessed by questionnaires and recorded for 5 consecutive nights in their homes. Results: Our approach firstly identified positive correlations between subjective and objective sleep parameters (total sleep time, sleep-onset latency and TRNs frequency). We also developed a method of synchronization between the two ambulatory devices that allowed us to match 200 TRNs (reported by event marker push button) with sleep stages corresponding to 91 nights and 37 patients. Most awakenings associated with TRNs occurred during NREM sleep (65.5% versus 34.5% during REM sleep). Our results also reveal significant differences in the frequency of EDA peaks 10 min before the reported events, with a lower frequency in REM (13.7 peaks) than in NREM (24.8 peaks) awakenings associated with TRNs. This EDA peaks frequency in REM sleep is not statistically different from that in REM sleep preceding awakenings that are not associated with TRNs. Conclusion: The development of wearable devices to collect physiological parameters is of interest in clinical practice to improve our knowledge of sleep and trauma-related nightmares in patients with PTSD.
Rapid eye movement sleep disturbance in posttraumatic stress disorder

Abstract:The subjective sleep disturbance in posttraumatic stress disorder (PTSD), including the repetitive, stereotypical anxiety dream, suggests dysfunctional rapid eye movement (REM) sleep mechanisms. The polysomnograms of a group of physically healthy combat veterans with current PTSD were compared with those of an age-appropriate normal control group. Tonic and phasic REM sleep measures in the PTSD subjects were elevated on the second night of recorded sleep. Increased phasic REM sleep activity persisted in the PTSD group on the subsequent night. During the study, an anxiety dream occurred in a PTSD subject in REM sleep. The results are consistent with the view that a dysregulation of the REM sleep control system, particularly phasic event generation, may be involved in the pathogenesis of PTSD. The finding of a specific disturbance of sleep unique to PTSD may have significant implications for the design of effective treatments for PTSD.

Abstract:Abstract Potential energy released from the capsize of ice-shelf fragments (icebergs) is the immediate driver of the brief explosive phase of ice-shelf disintegration along the Antarctic Peninsula (e.g. the Larsen A, Larsen B and Wilkins ice shelves). The majority of this energy powers the rapidly expanding plume of ice-shelf fragments that expands outward into the open ocean; a smaller fraction of this energy goes into surface gravity waves and other dynamic interactions between ice and water that can sustain the continued fragmentation and break-up of the original ice shelf. As an initial approach to the investigation of ice-shelf fragment capsize in ice-shelf collapse, we develop a simple conceptual model involving ideal rectangular icebergs, initially in unstable or metastable orientations, which are assembled into a tightly packed mass that subsequently disassembles via massed capsize. Computations based on this conceptual model display phenomenological similarity to aspects of real ice-shelf collapse. A promising result of the conceptual model presented here is a description of how iceberg aspect ratio and its statistical variance, the two parameters related to ice-shelf fracture patterns, influence the enabling conditions to be satisfied by slow-acting processes (e.g. environmentally driven melting) that facilitate ice-shelf disintegration.

Abstract:Abstract Introduction Nightmares are a common problem affecting 2-8% of the general population with the prevalence of comorbid nightmares in those with post-traumatic stress disorder (PTSD) being significantly higher at 72%. The negative sequelae of nightmares are myriad including impaired quality of life, sleep deprivation, insomnia, daytime sleepiness, fatigue, and suicidal ideation. This study investigated a novel approach for the treatment of nightmare disorder in military veterans. Methods All participants in this study were veterans receiving care at the Minneapolis Veterans Affairs Health Care System (VAHCS), diagnosed with PTSD, and had self-reported nightmares. At the baseline assessment, eligible participants were given a digital smartwatch preloaded with an application designed to arouse but not awaken the user out of the nightmare. Paired t-test analysis was used to compare the participants’ baseline and follow-up responses. Results A significant decrease in participant Pittsburgh Sleep Quality Index (PSQI) scores (13.6 to 7.4, p < 0.001), PTSD Checklist for DSM-5 (PCL-5) scores (45.8 to 25.1, p < 0.0025) and Patient Health Questionnaire (PHQ-9) scores (12.2 to 6.1, p < 0.002) was observed over the sixty day trial. All 9 participants experienced a decrease in the Epworth Sleepiness Scale (ESS) upon completion of the trial, with an average decrease of -2.5 ± 1.4. Conclusion These results demonstrate that a novel digital therapeutic smartwatch application is effective in improving sleep quality, reducing the burden associated with PTSD symptoms, and lowering depressive symptoms in those with nightmare disorder within an open label study. These results have initiated further investigation into efficacy of digital therapeutic smartwatch applications in nightmare intervention and improved sleep quality. Support NightWare, Inc.

Abstract:Study objectives: Trauma-related nightmares (TRNs) are a hallmark symptom of PTSD and are highly correlated with PTSD severity and poor sleep quality. Given the salience and arousal associated with TRNs, they might be an effective target for imaginal exposures during Prolonged Exposure (PE) therapy. As a first step in this line of research, the current study compared participants' emotional reactivity during recollection of TRNs to their recollection of the index traumatic event. Methods: Seventeen trauma-exposed participants with clinical or sub-clinical PTSD who reported frequent TRNs engaged in script-driven imagery using scripts depicting their index trauma and their most trauma-like TRN. Heart rate (HRR), skin conductance (SCR), corrugator EMG (EMGR) responses, and emotional ratings were recorded. Results: HRR, SCR, and EMGR did not differ significantly between trauma-related and TRN scripts. Bayesian analyses confirmed support for the null hypothesis, indicating no differences. With the exception of "Sadness," for which TRNs elicited significantly lower ratings than trauma scripts, individual emotion ratings showed no significant differences, suggesting likely parity between the emotionality of trauma-related and TRN recollections. Conclusions: Together, TRN content elicited psychophysiological reactivity similar to that of the index trauma in this pilot study. Upon replication, studies testing TRNs as potential targets for imaginal exposures during PE may be warranted.
Pharmacological Management of Nightmares Associated with Posttraumatic Stress Disorder

Abstract:Posttraumatic stress disorder (PTSD) can be a chronic and disabling condition. Post-traumatic nightmares (PTNs) form a core component of PTSD and are highly prevalent in this patient population. Nightmares in PTSD have been associated with significant distress, functional impairment, poor health outcomes, and decreased quality of life. Nightmares in PTSD are also an independent risk factor for suicide. Nightmare cessation can lead to improved quality of life, fewer hospital admissions, lower healthcare costs, and reduced all-cause mortality. Effective treatment of nightmares is critical and often leads to improvement of other PTSD symptomatology. However, approved pharmacological agents for the treatment of PTSD have modest effects on sleep and nightmares, and may cause adverse effects. No pharmacological agent has been approved specifically for the treatment of PTNs, but multiple agents have been studied. This current narrative review aimed to critically appraise proven as well as novel pharmacological agents used in the treatment of PTNs. Evidence of varying quality exists for the use of prazosin, doxazosin, clonidine, tricyclic antidepressants, trazodone, mirtazapine, atypical antipsychotics (especially risperidone, olanzapine and quetiapine), gabapentin, topiramate, and cyproheptadine. Evidence does not support the use of venlafaxine, β-blockers, benzodiazepines, or sedative hypnotics. Novel agents such as ramelteon, cannabinoids, ketamine, psychedelic agents, and trihexyphenidyl have shown promising results. Large randomized controlled trials (RCTs) are needed to evaluate the use of these novel agents. Future research directions are identified to optimize the treatment of nightmares in patients with PTSD.
Short-term stability and night-to-night variability of sleep parameters in nightmares comorbid with chronic insomnia Disorder across multiple nights of polysomnography

Abstract:Abstract Study Objectives The purpose of this study was to examine the degree of short-term stability of polysomnographic (PSG) measured sleep parameters and the overall differences between individuals with comorbid nightmares and insomnia compared to those with chronic insomnia disorder alone or good sleeping controls across four nights in the sleep lab. Methods A total of 142 good sleeping controls, 126 chronic insomnia alone, and 24 comorbid insomnia/nightmare participants underwent four consecutive nights of 8-hour PSG recordings. Outcomes included sleep continuity, architecture, and REM-related parameters across nights one through four. Intraclass correlation coefficients with mixed-effect variances and repeated-measure analysis of covariance were used, respectively, to determine short-term stability as well as between-participants and time-by-group interaction effects. Results Wake after sleep onset and stage 1 showed “poor stability” in the comorbid insomnia/nightmare group compared to “moderate stability” in the good sleeping controls and chronic insomnia alone group. Significant between-group effects (all ps < .05) showed that the comorbid insomnia/nightmare group took longer to fall asleep and had a greater first-night-effect in stage 1 compared to good sleeping controls and chronic insomnia alone group; in addition, the comorbid insomnia/nightmare and insomnia alone groups slept shorter, with fewer awakenings and REM periods, compared to the good sleeping controls. Conclusions Nightmares are associated with abnormal sleep above and beyond REM disruption, as sleep continuity was the primary aspect in which poor stability and group differences emerged. The greater inability to fall asleep and instability of sleep fragmentation in those with comorbid insomnia/nightmares compared to chronic insomnia alone may be attributed to the impact of presleep anticipatory anxiety and nightmare-related distress itself. Abstract:Intrusive memories are a common experience following trauma exposure but can develop into a symptom of posttraumatic stress disorder (PTSD). Recent research has observed a relationship between sleep disturbance and intrusive memory frequency following analogue trauma exposure and disruptions in REM sleep are found to contribute to emotional dysregulation and an amplified reaction to negative emotional stimuli. The current study examined the association between REM sleep prior to analogue trauma and intrusive memories. To manipulate REM sleep, 27 healthy adults (MAge= 25.4, SD = 2.89) were randomised to either to a circadian misalignment (CM) condition or normal control (NC) condition for four nights. In CM, participants slept normally for two nights followed by a 4-hour phase advance on night three and an additional 4-hour phase advance on night four. In NC, participants had 8-hour sleep opportunities each night. On day 5, participants watched a trauma film and kept an intrusive memory diary for the next three days. Greater REM sleep percentage (p = .004) and REM efficiency (p = .02) across 4 nights prior to analogue trauma, independent of group, was significantly associated with fewer intrusive memories in the 3 days after viewing the film. Findings suggest REM sleep may serve to protect individuals against experiencing intrusive memories. This is consistent with evidence suggesting REM sleep influences emotional memory regulation. Occupations (e.g., emergency services/military personnel) who experience circadian disruptions likely to decrease REM sleep (e.g., from shift work) may be at heightened risk of experiencing intrusive memories after trauma exposure, and thus at increased risk of developing PTSD.
The effect of continuous positive air pressure (CPAP) on nightmares in patients with posttraumatic stress disorder (PTSD) and obstructive sleep apnea (OSA)

Abstract:Objectives: Post-Traumatic Stress Disorder (PTSD) is increasingly prevalent among Veterans characterized by recurrent nightmare and disrupted sleep. Veterans with PTSD also have a high prevalence of obstructive sleep apnea (OSA) and untreated OSA worsens the sleep-related symptoms of PTSD. In our study, we hypothesized that among PTSD-afflicted Veterans with OSA, CPAP therapy may reduce the frequency of nightmares and a better CPAP compliance may be associated with increased symptom improvement. Methods: We retrospectively reviewed medical records to identify OSA patients treated in a VA medical center who also carried a diagnosis of PTSD (n = 69). Data about patient characteristics and polysomnographic findings were extracted. Repeated-measures t-tests were performed, comparing mean nightmare frequency and Epworth sleepiness score (ESS) before and after CPAP treatment. Multiple linear regressions were done to identify factors predicting CPAP compliance. A logistic regression analysis was also done to estimate the odds of subjective improvement in PTSD symptoms with CPAP. Results: CPAP therapy reduced the mean ESS from 14.62 to 8.52 (p < 0.001) and the mean number of nightmares per week from 10.32 to 5.26 (p < 0.01). Reduced nightmare frequency after CPAP treatment was best predicted by CPAP compliance (p < 0.001). Every 10% increase in CPAP compliance almost doubled the odds of benefitting by CPAP (odds ratio = 1.92, 95% CI = 1.47-2.5). Conclusions: In Veterans with PTSD and OSA, CPAP therapy reduces PTSD-associated nightmares and improves overall PTSD symptoms. We recommend that all PTSD patients should be screened clinically for symptoms of OSA and receive CPAP treatment whenever possible to improve PTSD symptoms.
Nightmares do result in psychophysiological arousal: A multimeasure ambulatory assessment study

Abstract:Individuals who frequently experience nightmares report compromised sleep quality, poor daytime mood, and functioning. Previous research has aimed at linking these impairments with altered sleep architecture, but results were inconclusive. One plausible explanation is that only a few studies recorded markers of autonomic nervous system activity. For the first time, this study collected such markers under ecologically valid conditions with ambulatory assessment. In 19 individuals with frequent nightmares (≥1 nightmare/week) and 19 healthy control participants (<1 nightmare/month), measures indicating autonomic activation (heart rate, heart rate variability, respiration cycle length, electrodermal fluctuations, EEG arousals, saliva cortisol, REM density) were collected while applying ambulatory polysomnographic assessment during 3 consecutive nights. When nightmare participants reported a nightmare, we analyzed the last 5 min of REM sleep before awakening and compared these data to their non-nightmares as well as to the dream episodes of control participants. Overall, there were no general differences in autonomic activation of nightmare sufferers compared to control participants. However, when nightmare participants experienced nightmares, autonomic activation was markedly increased compared to their own non-nightmares and, to some extent, to control participant's dreams. Significant intraindividual differences were found for all autonomic measures except in participant's EEG arousals and cortisol levels. Group differences were found in EEG arousals and heart rate. In conclusion, ambulatory polysomnography demonstrates that nightmares are accompanied by increased autonomic activation. Results support the notion of impaired self-reported sleep quality caused by one's autonomic response rather than altered sleep pattern.
The impact of prolonged exposure on sleep and enhancing treatment outcomes with evidence-based sleep interventions: A pilot study.

Abstract:OBJECTIVE: Insomnia and nightmares are central features of posttraumatic stress disorder (PTSD). However, often they are inadequately assessed and ineffectively resolved following gold-standard PTSD treatment. Here we: (a) evaluate effects of prolonged exposure (PE) on subjectively measured sleep and (b) present pilot results of an examination of whether adding sleep interventions (imagery rehearsal therapy [IRT] and cognitive-behavioral therapy for insomnia [CBT-I]) to PE improves treatment response, relative to PE alone, for night- and/or daytime PTSD symptoms among returning U.S. veterans and postdeployment personnel.METHOD: In a parallel-groups, randomized controlled trial, participants received 12 sessions of PE followed by IRT (5 weeks) and CBT-I (7 weeks) or PE followed by 12 weeks supportive care therapy (SCT).RESULTS: PE did not improve sleep to a clinically meaningful degree, despite significant improvements in both Clinical Administered PTSD Scale and PTSD Checklist. Enhancing treatment with IRT/CBT-I led to greater improvements in insomnia (diary-recorded sleep efficiency) symptoms with large effect size, relative to SCT (p = .068, d = 1.07). There were large improvements in nightmare frequency relative SCT that did not reach statistical significance (p = .11, d = 0.90). Moreover, there was small improvement in daytime symptoms (Clinical Administered PTSD Scale) that did not reach statistical significance (p = .54, d = .31).CONCLUSION: The addition of targeted, validated sleep treatment improves effects of PE and improves nighttime symptoms. Abstract:Background: Posttraumatic nightmares are a highly prevalent and distressing symptom of posttraumatic stress disorder (PTSD), but have been the subject of limited phenomenological investigations. Methods: We utilized a communication box to establish PTSD symptoms in rats through exposure to footshock stress (FS) and psychological stress (PS). The immunohistochemical test and high-performance liquid chromatography with electrochemical detection were used to detect the activity and monoamine levels in the rats’ arousal systems. Results: Twenty-one days after traumatic stress, 14.17% of FS and 12.5% of PS rats exhibited startled awakening, and the same rats showed hyperfunction of the locus coeruleus/noradrenergic system and hypofunction of the perifornical nucleus/orexinergic system. Changes in serotonin levels in the dorsal raphe nucleus showed opposite trends in the FS and PS rats that were startled awake. No differences were found in other sleep/arousal systems. Conclusion: These results suggest that different clinically therapeutic strategies should be considered to treat different trauma-induced posttraumatic nightmares.

Abstract:Abstract Introduction Nightmares are a frequent concern among US veterans, often linked to prevalent mental health conditions like depression and PTSD. Antidepressant medications are commonly used to manage these conditions, yet the association between their usage and the frequency of nightmares remains unclear. Methods The sample consisted of veterans at risk for sleep apnea at the Miami VA Sleep Center evaluated over one year. Veterans underwent home polysomnography (PSG) and completed questionnaires including demographics and the Munich Parasomnia Screening (MUPS). The frequency of nightmares was assessed using the nightmare item in MUPS. Electronic Medical Records were reviewed to obtain active medical and psychiatric diagnoses and active medication orders (including most common antidepressants: SSRIs, SNRIs, mirtazapine and bupropion) contemporaneous with the sleep evaluation. Results The study involved 649 veterans, predominantly male (84%) with mean age of 50 ± 14 years. Within this cohort, 49% were diagnosed with depression and 33% with PTSD. Regarding reported nightmare frequency, 16% indicated nightmares “never”, 9% “less than once a year”, 22% “once or several times a year”, 22% “once or several times a month”, 21% “once or several times a week” and 8% “almost every night.” Multiple regression was used to predict nightmares. In model 1, depression was positively associated with nightmares (p=0.001), independent of PTSD and AHI. In model 2, using an antidepressant versus not was positively associated with nightmares (p=0.05). However, depression was no longer significant (p=0.102). In model 3, type of antidepressants was evaluated, but only SSRIs were significant (p=0.04) while depression remained non-significant. Conclusion The findings highlight the prevalence of nightmares among veterans. Although depression was initially associated with increased nightmare frequency, when treatment for depression was considered, depression no longer predicted nightmares. Instead, antidepressants and specifically SSRIs predicted nightmares. Considering the type of antidepressant may be crucial when patients report nightmares. Further research is needed to investigate the impact of mental health conditions and medication effects on nightmares in this population. Support (if any)

Abstract:Introduction: Sleep disturbances are associated with various anxiety- and trauma-related disorders, and specifically with post-traumatic stress disorder (PTSD). Two of the core symptoms of PTSD are recurrent distressing dreams about the traumatic event and difficulty in falling or staying asleep (insomnia). Sleep is essential for functioning, with poor sleep compromising cognitive, emotional and behavioural regulations. Sleep is also of particular importance for brain development and information processing in children and adolescents. The aim of this study was to determine if PTSD symptom severity and trauma load is associated with nightmares and insomnia in a sample of trauma-exposed, treatment-seeking adolescents.Methods: A total of 358 South African adolescents, between 12 and 18 years of age, exposed to at least one DSM-5 qualifying traumatic event, were assessed for PTSD-related sleep difficulties using the Kiddie Schedule for Affective Disorders and Schizophrenia (KSADS). Childhood exposure to 10 common trauma types was also recorded using the KSADS trauma checklist. PTSD symptom severity was measured using the child PTSD checklist (CPC).Results: Adolescents reporting current trauma-related nightmares (F[268.2] = 0.68, t = -8.16, p = 0.000) and current insomnia (F[265.2] = 0.16, t = -9.03, p = 0.000) had significantly higher PTSD symptom severity scores compared to those without current nightmares and insomnia. Adolescents with current trauma-related nightmares (F[355.2] = 0.15, t = -3.30, p = 0.001) and insomnia (F[353.2] = 0.15, t = -2.51, p = 0.013) were also exposed to a significantly greater number of different trauma types compared to those without current nightmares and insomnia.Conclusion: Assessing and treating sleep difficulties related to PTSD in trauma-exposed adolescents, in an effort to reduce the developmental impact of trauma on the brain and general functioning, should not be overlooked. Longitudinal studies may contribute to a better understanding of the long-term effects of trauma-related insomnia and nightmares on mental and physical health outcomes.

Abstract:Posttraumatic nightmares are a hallmark of PTSD and distinct from general nightmares as they are often repetitive and faithful representations of the traumatic event. This paper presents data from a pilot study that examined the use of Imagery Rehearsal in treating combat-related nightmares of 12 Vietnam veterans with PTSD. Three treatment groups, comprising 4 veterans in each, completed standardised treatment across 6 sessions. Treatment effects were investigated using nightmare diaries and established instruments, including the IES-R, BDI, BAI, and SCL-90-R. The data demonstrate significant reductions in nightmares targeted, and improvements in PTSD and comorbid symptomatology. The paper recommends that, on the basis of the promising preliminary data, a randomised control trial be established to assess imagery ability and attitude toward nightmares.

Abstract:Nightmares and insomnia are ubiquitous in PTSD. Current categorizations of PTSD symptoms associate nightmares with dysfunctional fear extinction, while insomnia is linked to hyperarousal. However, it has been proposed that nightmares may also be driven by hyperarousal. Therefore, the present study sought to examine the shared and unique influence of these two mechanisms–fear extinction learning and hyperarousal–on different trauma-related sleep disturbances.

Clinical and polysomnographic characteristics and response to continuous positive airway pressure therapy in obstructive sleep apnea patients with nightmares

Abstract:Objective: To assess the characteristics of obstructive sleep apnea (OSA) patients with nightmares and the effects of continuous positive airway pressure (CPAP) therapy on nightmares. Methods: Consecutive patients referred with a clinical suspicion of OSA underwent attended overnight sleep studies. OSA and nightmares were diagnosed according to the American Academy of Sleep Medicine (AASM) criteria, and CPAP titration was performed in accordance with the AASM guidelines. A follow-up visit was performed 3months later, and the patients with nightmares were divided into two groups: group 1 used CPAP with good compliance, whereas group 2 refused CPAP treatment and did not use other alternative treatments for OSA. Results: The study included 99 patients who had been diagnosed with OSA with nightmares. Their mean age was 47.2±11.2years, and they had a mean apnea-hypopnea index (AHI) of 36.5±34.3/h. Also included were 124 patients with OSA without nightmares. The mean age of these patients was 45.4±13.9years, and they had a mean AHI of 40.2±35/h. The patients with nightmares had a significantly higher AHI during rapid eye movement sleep (REM) compared with the patients without nightmares (51.7±28.1 vs 39.8±31.9/h). Logistic regression analysis revealed that the REM-AHI and interrupted sleep at night were independent predictors of nightmares in the OSA patients. Nightmares disappeared in 91% of the patients who used CPAP compared with 36% of patients who refused to use CPAP (p<0.001). Conclusion: Nightmares in OSA patients are associated with a higher REM-AHI. CPAP therapy results in a significant improvement in nightmare occurrence.

Abstract:Abstract Introduction Disturbed sleep appears to play a role in post-traumatic stress disorder (PTSD) development and maintenance. The prevalence of obstructive sleep apnea (OSA, as defined as an AHI>5 events/hr) has been reported to be up to 75% in individuals with PTSD (Zhang 2017). Most prior studies investigating OSA prevalence in PTSD have been conducted in military populations, who commonly have other risk factors for OSA such as being male, middle-aged and having high rates of substance use disorders. The aim of this study was to determine whether OSA is more common in individuals with PTSD symptoms exposed to non-military traumas as compared to individuals without PTSD symptoms. Methods 125 participants interested in completing a sleep study completed a screening questionnaire measuring previous Criterion A trauma exposure (Life events checklist) and PTSD symptoms (PTSD symptom checklist for DSM-5, PCL-5), along with alcohol use and demographic information. A sub-set were recruited for a standard overnight polysomnography either in the sleep laboratory (Grael, Compumedics), or at home (Siesta, Compumedics). Sleep studies were scored blinded to questionnaire data by an independent sleep technician. Three symptom severity groups were compared, individuals with Likely PTSD (PCL-5 >33 and Criterion A trauma exposure) Sub-Syndromal PTSD (PCL-5 15-33 and Criterion A trauma exposure) or No-PTSD (PCL-5 < 15 and no intrusive symptoms). Results 45 participants (31 female; aged 18-48 years) completed the polysomnography. 11 had likely PTSD, 10 Sub-Syndromal PTSD and 23 had No-PTSD. The 3 groups were comparable in age (25±7 years), sex (68% women) and BMI (24.2±4.5 kg/m2). In total, 6 participants had AHI >5 events/hr; 3 in the Likely PTSD group, 2 in the Sub-Syndromal group and 1 in the No-OSA group. The mean AHI did not differ between groups (Likely PTSD = 5.1±7.7, Sub-Syndromal PTSD = 4.0±6.7, No-PTSD = 2.2±4.4 events/hr, p=.64). Conclusion Although the a priori sample size (n=26 in each group) has not yet been reached, these preliminary data indicate that the prevalence of OSA in a young mixed-trauma PTSD population is unlikely to be as high as has been observed previously. Abstract:Posttraumatic stress disorder (PTSD) patients exhibit depressive and anxiety symptoms, in addition to nightmares, which interfere with sleep continuity. Pharmacologic treatment of these sleep problems improves PTSD symptoms, but very few studies have used psychotherapeutic interventions to treat PTSD and examined their effects on sleep quality. Therefore, in the present study, we sought to investigate the effects of Eye Movement Desensitization Reprocessing therapy on indices of mood, anxiety, subjective, and objective sleep. The sample was composed of 11 healthy controls and 13 PTSD patients that were victims of assault and/or kidnapping. All participants were assessed before, and 1 day after, the end of treatment for depressive and anxiety profile, general well-being and subjective sleep by filling out specific questionnaires. In addition, objective sleep patterns were evaluated by polysomnographic recording. Healthy volunteers were submitted to the therapy for three weekly sessions, whereas PTSD patients underwent five sessions, on average. Before treatment, PTSD patients exhibited high levels of anxiety and depression, poor quality of life and poor sleep, assessed both subjectively and objectively; the latter was reflected by increased time of waking after sleep onset. After completion of treatment, patients exhibited improvement in depression and anxiety symptoms, and in quality of life; with indices that were no longer different from control volunteers. Moreover, these patients showed more consolidated sleep, with reduction of time spent awake after sleep onset. In conclusion, Eye Movement Desensitization and Reprocessing was an effective treatment of PTSD patients and improved the associated sleep and psychological symptoms.

Abstract:Abstract Introduction Introduction: Nightmare disorder and post-traumatic stress disorder (PTSD)-associated nightmares can be treatment-resistant in many patients. There are no recommended treatments for these conditions currently. Methods Methods: NightWare is a novel FDA-approved class II medical device which improves sleep in patients with nightmare disorder and PTSD-associated nightmares. It uses data from a smart watch to determine when nightmares are happening using heart rate and movement sensors, and then intervenes to interrupt the nightmare and improve overall sleep quality. We report the 14 consecutive cases of patients using NightWare and the Clinical Global Impression (CGI) of Severity and Change in their sleep and reduction of nightmare burden. Results Results: Of the 14 patients in this series, 10 patients used the device for more than 7 days. 1 discontinued due to perceived lack of efficacy, 2 stopped in part due to a skin rash, and one stopped because he did not want to wear a watch to sleep. At baseline, the 10 patients included in the analysis had a mean CGI Severity score of 4.7 (SD 1.25), which indicates moderate to severe nightmares and daytime dysfunction. Improvement in the CGI Change score with NightWare exceeded that of prazosin for this group of patients. The patients in this series that used the device had a mean score of 2.20 (SD 0.79) on the CGI Change scale, which indicates there was significant improvement from baseline. Six of the 10 patients using NightWare were also taking prazosin prior to starting NightWare. The prazosin dose ranged from 1-8 mg nightly for these patients. These 6 patients had minimal improvement with prazosin alone (CGIC of 3.16 (SD 0.41). Conclusion Conclusion: In this case series, NightWare significantly improved the symptoms of nightmare disorder in 10 of the 14 patients in this series. Patient selection for sympathetic nervous system activation is important to ensure the device’s effectiveness. Support (if any)
