Abstract:Abstract Background Ectopic liver tissue (ELT) is a rare developmental abnormality in which normally functioning liver tissue develops at an extra-hepatic site. Due to the atypical clinical symptoms of ELT, its clinical diagnosis is rarely easy, however, the risk of carcinogenesis is strongly increased due to its special pathological characteristics. So it should always be considered in the differential diagnosis of tumors or symptomatic mass. Case presentation Case 1：A 57-year-old female patient presented herself to the outpatient department of our hospital complaining of paroxysmal upper abdominal pain for one month. She was admitted due to "Multiple intrahepatic bile duct stones and chronic cholecystitis". During the surgery, an abnormally enlarged gallbladder of size about 12*8*5cmwas found. A tiny mass of about 1.2*0.6*0.3cm on the free wall of the gallbladder with a similar texture to the liver was also found. Case 2：An 87-year-old male patient presented himself to the emergency department of our hospital complaining of right upper abdomen pain for three days. He was admitted to the hospital due to "cholecystolithiasis and chronic cholecystitis". During the surgery, an abnormally enlarged gallbladder of size about 12*8*5cm. A tiny mass of about 1.2*0.6*0.3cm on the free wall of the gallbladder with a similar texture to the liver was also found. After surgery, the two patients are now in good overall condition. Discussion and conclusions In this two continuous cases of Ectopic liver tissue, the pathological examination and liver cancer markers “AFP” and “Glypican-3” did not support tumor lesions. Accordingly, we proposed a new hypothesis for this phenomenon, that cryptogenic functional excretory system (helps to excrete chemical carcinogens) from ELT into the gallbladder (a part of the biliary system) does exist but it cannot be found in ELT located in other organs. This phenomenon may provide a rational response to the question why does Ectopic Liver Tissue on the Gallbladder rarely have malignant transformation.
Telomere Length of Circulating Cell-Free DNA and Gastric Cancer in a Chinese Population at High-Risk.

Abstract:Background: Telomeres have long been found to be involved in cancer development, while little was known about the dynamic changes of telomere length in carcinogenesis process. Methods: The present study longitudinally investigated telomere alterations of cell-free DNA (cfDNA) in 86 gastric cancer (GC) subjects recruited through a 16-year prospective cohort with 2-4 serums collected before each GC-diagnosis from baseline and three follow-up time-points (a total of 276 samples). As the control, 86 individual-matched cancer-free subjects were enrolled with 276 serums from the matched calendar year. Results: In the 73 pairs of baseline serums from GC and control subjects, shortened telomeres showed increased subsequent GC risk [odds ratio (OR) = 9.17, 95% CI: 2.72-31.25 for 1 unit shortening]. In each baseline gastric lesion category, higher risks of GC progression were also found with shortened cfDNA telomeres; ORs per 1 unit shortening were 6.99 (95% CI: 1.63-30.30) for mild gastric lesions, 6.06 (95% CI: 1.89-19.61) for intestinal metaplasia and 15.63 (95% CI: 1.91-125.00) for dysplasia. With all measurements from baseline and follow-up time-points, shortened telomeres also showed significant association with GC risk (OR = 7.37, 95% CI: 2.06-26.32 for 1 unit shortening). In temporal trend analysis, shortened telomeres were found in GC subjects compared to corresponding controls more than 3 years ahead of GC-diagnosis (most P < 0.05), while no significant difference was found between two groups within 3 years approaching to GC-diagnosis. Conclusion: Our findings suggest that telomere shortening may be associated with gastric carcinogenesis, which supports further etiological study and potential biomarker for risk stratification.
Hepatocellular telomere length in biliary atresia measured by Q-FISH

Abstract:Background: Liver transplantation for biliary atresia is indicated whenever a Kasai portoenterostomy is considered unfeasible. However, the timing of liver transplantation in biliary atresia has not been precisely defined. Excessive shortening of hepatocellular telomeres may occur in patients with biliary atresia, and therefore, telomere length could be a predictor of hepatocellular reserve capacity. Methods: Hepatic tissues were obtained from 20 patients with biliary atresia who underwent LT and 10 age-matched autopsied individuals (mean age, 1.7 and 1.2 years, respectively). Telomere lengths were measured by Southern blotting and quantitative fluorescence in situ hybridization using the normalized telomere-centromere ratio. The correlation between the normalized telomere-centromere ratio for the hepatocytes in biliary atresia and the pediatric end-stage liver disease score was analyzed. Results: The median terminal restriction fragment length of the hepatic tissues in biliary atresia was not significantly different from that of the control (p = 0.425), whereas the median normalized telomere-centromere ratio of hepatocytes in biliary atresia was significantly smaller than that of the control (p < 0.001). Regression analysis demonstrated a negative correlation of the normalized telomere-centromere ratio with the pediatric end-stage liver disease score in biliary atresia (p < 0.001). Conclusions: Telomere length analysis using quantitative fluorescence in situ hybridization could be an objective indicator of hepatocellular reserve capacity in patients with biliary atresia, and excessive telomere shortening supports the early implementation of liver transplantation.

Abstract:Abstract Chromosomal instability is one of the early hallmarks of human carcinogenesis. Shortened telomeres may predispose to chromosomal instability leading to the development of some cancers, including esophageal adenocarcinoma (EAC). However, whether the telomere changes associated with cancer risk occur at the global or chromosome-specific level remain largely unknown. Here, we developed a novel two-step quantitative fluorescent in situ hybridization (FISH) assay to measure the overall and genome-wide chromosome-specific telomere length in peripheral blood lymphocytes (PBLs). In this assay, overall telomere FISH is first carried out using peptide-coupled nucleic acid (PNA) probe. The relative intensity of each telomere signal is obtained by digital capture of the microscopic image and quantified. Then we perform the second FISH with centromere and subtelomere probes on the same slide to label each chromosome. Therefore, we can identify and quantify the telomere length for the p arm or q arm of each chromosome. We next used this assay to measure the overall and chromosome specific telomere length in a pilot case-control study of 31 EAC cases and 31 matched controls. We found EAC patients had significantly shorter overall telomere lengths in PBLs compared with controls (P = 0.039). Notably, the chromosome-specific telomeres of 8, 10, 14, 16, 17, and 22 were significantly shorter in cases than in controls (P < 0.05). Analysis of chromosome arm-specific telomeres indicated that the telomere lengths of 17p, 17q, 16q, Xp, and 10q, and 13p were shorter in cases than controls (P < 0.04). Cumulative analysis of these six significant telomeres showed that the total length for cases was significantly shorter than controls (P < 0.001). Using median expression as cutoff, we found that patients with short telomeres on chromosome 17, 13p, 22q, or Xp had 4.4-fold increased risk of EAC (P for trend < 0.001). Taken together, our study provides the first epidemiologic evidence that cancer cases have generally short telomeres across all chromosome arms and suggests that short telomeres in specific chromosome arms may exhibit stronger cancer predisposing effects than others depending on cancer types. Gradual telomere shortening and increasing chromosomal instability among PanIN grades and normal ductal epithelia with and without cancer in the pancreas

Abstract:A large body of evidence supports a key role for telomere dysfunction in carcinogenesis due to the induction of chromosomal instability. To study telomere shortening in precancerous pancreatic lesions, we measured telomere lengths using quantitative fluorescence in situ hybridization in the normal pancreatic duct epithelium, pancreatic intraepithelial neoplasias (PanINs), and cancers. The materials employed included surgically resected pancreatic specimens without cancer (n = 33) and with invasive ductal carcinoma (n = 36), as well as control autopsy cases (n = 150). In comparison with normal ducts, telomere length was decreased in PanIN-1, -2 and -3 and cancer. Furthermore, telomeres were shorter in cancer than in PanIN-1 and -2. Telomere length in cancer was not associated with histological type, lesion location, or cancer stage. PanINs with or without cancer showed similar telomere lengths. The incidences of atypical mitosis and anaphase bridges, which are morphological characteristics of chromosomal instability, were negatively correlated with telomere length. The telomeres in normal duct epithelium became shorter with aging, and those in PanINs or cancers were shorter than in age-matched controls, suggesting that telomere shortening occurs even when histological changes are absent. Our data strongly suggest that telomere shortening occurs in the early stages of pancreatic carcinogenesis and progresses with precancerous development. Telomere shortening and chromosomal instability in the duct epithelium might be associated with carcinogenesis of the pancreas. Determination of telomere length in pancreatic ductal lesions may be valuable for accurate detection and risk assessment of pancreatic cancer.

Abstract:Telocytes are closely associated with the regulation of tissue smooth muscle dynamics in digestive system disorders. They are widely distributed in the biliary system and exert their influence on biliary motility through mechanisms such as the regulation of CCK and their electrophysiological effects on smooth muscle cells. To investigate the relationship between telocytes and benign biliary diseases,such as gallbladder stone disease and biliary dilation syndrome, we conducted histopathological analysis on tissues affected by these conditions. Additionally, we performed immunohistochemistry and immunofluorescence double staining experiments for telocytes. The results indicate that the quantity of telocytes in the gallbladder and bile duct is significantly lower in pathological conditions compared to the control group. This reveals a close association between the decrease in telocyte quantity and impaired gallbladder motility and biliary fibrosis. Furthermore, further investigations have shown a correlation between telocytes in cholesterol gallstones and cholecystokinin-A receptor (CCK-AR), suggesting that elevated cholesterol levels may impair telocytes, leading to a reduction in the quantity of CCK-AR and ultimately resulting in impaired gallbladder motility.Therefore, we hypothesize that telocytes may play a crucial role in maintaining biliary homeostasis, and their deficiency may be associated with the development of benign biliary diseases, including gallstone disease and biliary dilation.

Abstract:Rationale: Duplication of the extrahepatic bile duct is an extremely rare congenital anomaly of the biliary system. Patient concerns: A 44-year-old woman presented with a history of continuous upper abdominal pain and vomiting. Diagnoses: Magnetic resonance cholangiopancreatography (MRCP) disclosed diffuse dilatation of the intrahepatic and extrahepatic bile ducts. Endoscopic retrograde cholangiopancreatography (ERCP) showed the presence of two extrahepatic bile ducts with calculus at the distal end of the CBD. Interventions: Laparoscopic cholecystectomy (LC) was performed after an ERCP. Choledochoscopy, performed during the operation, showed duplicated common bile duct and the cystic duct was seen opening at the right side of the extrahepatic duct. Outcomes: The patient was doing well after 6 months of follow-up. Lessons: We reported a case of a double common duct with choledocholithiasis and gallstone. This rare anomaly may lead to cholangitis, common bile duct injury during surgery, malignancy occurrence, and should be treated with extreme care.
The induction of alkaline phosphatase activity in HeLa cells by 5-iodo-2'-deoxyuridine.

Abstract:Summary When HeLa cells were grown with 5 µm 5-iodo-2′-deoxyuridine (IUdR) there was an increase in alkaline phosphatase activity after about 24 hr, reaching a maximum usually at about 72 hr. At this concentration, during this time, there was a relatively small inhibition of cell replication. The increment in alkaline phosphatase activity was in large measure prevented by concomitant addition of 10 µm thymidine. Further, IUdR did not increase alkaline phosphatase activity in a mutant line of HeLa cells lacking thymidine kinase. These data indicate that IUdR must be phosphorylated and incorporated into DNA to increase alkaline phosphatase activity. IUdR also usually increased alkaline phosphatase activity of HeLa71 which has a high constitutive level of the enzyme. IUdR did not affect the alkaline phosphatase activity elevated by prior treatment with 1 µm hydrocortisone. On the other hand, 1 µm hydrocortisone further increased the already elevated alkaline phosphatase activity in cells pretreated with 5 µm IUdR, although after hydrocortisone addition the final level of enzyme activity attained in IUdR-treated and untreated cells was the same.
Early and late outcomes of congenital biliary dilatation in pediatric patients

Abstract:Background: This study aimed to reveal the early and late postoperative complications and outcomes after surgery for congenital biliary dilatation (CBD) by reviewing cases over the past 40 years. Methods: We retrospectively evaluated 59 patients with CBD who underwent radical surgery for complications and outcomes, based on medical records. Early complications were defined as those requiring treatment within 5 years of the initial operation. Late complications were defined as those treated more than 5 years later. Results: The median age at the first surgery was 37 months. Regarding biliary reconstruction, 54 of the 59 patients (91.5%) underwent hepaticojejunostomy. Although three patients underwent cholecystoduodenostomy and one patient underwent hepaticoduodenostomy, all were converted to hepaticojejunostomy after a median of 12.5 years. One patient developed synchronous biliary carcinoma and underwent pancreaticoduodenectomy. Early complications occurred in seven patients with 10 events (surgical site infection, n = 3 bile leakage, n = 3; ileus, n = 3; bile duct obstruction, n = 1 and intussusception, n = 1). Late complications occurred in nine patients with 12 events (ileus, n = 3; anastomotic stricture, n = 3; hepatolithiasis, n = 3; asynchronous biliary carcinoma, n = 2; pancreatolithiasis, n = 1). Two of the three patients with hepatolithiasis underwent hepatectomy refractory to the endoscopic approach. Two patients developed asynchronous biliary carcinoma at 34 and 13 years after last operation; both ultimately died of the carcinoma. Only 35 patients (61.4%) underwent a follow-up examination. A total of 11 female patients (45.8%) eventually married, and all successfully gave birth. Conclusion: Although the long-term prognosis is excellent with complete cyst excision and hepaticojejunostomy, we emphasize the importance of long-term follow-up.
Giant bile duct dilatation in newborn: A case report

Abstract:BACKGROUND Giant congenital biliary dilation (CBD) is a rare condition observed in clinical practice. Infants born with this condition often experience a poor overall health status, and the disease progresses rapidly, leading to severe biliary obstruction, infections, pressure exerted by the enlarged CBD on abdominal organs, disturbances in the internal environment, and multiple organ dysfunction. The treatment of giant CBD using laparoscopy is challenging due to the high degree of variation in the shape of the bile duct and other organs, making it difficult to separate the bile duct wall from adjacent tissues or to control bleeding. CASE SUMMARY Herein, we present the details of an 11-d-old male newborn who was diagnosed with giant CBD. The patient was admitted to the neonatal surgery department of our hospital due to a history of common bile duct cyst that was detected more than 3 mo ago, and also because the patient had been experiencing yellowish skin for the past 9 d. The abnormal echo in the fetal abdomen was first noticed by the patient’s mother during a routine ultrasound examination at a local hospital, when the patient was at 24 wk + 6 d of pregnancy. This finding raised concerns about the possibility of congenital biliary dilatation (22 mm × 21 mm). Subsequent ultrasound examinations at different hospitals consistently confirmed the presence of a congenital biliary dilatation. No specific treatment was administered for biliary dilatation during this period. A computed tomography scan conducted during the hospitalization revealed a large cystic mass in the right upper quadrant and pelvis, measuring approximately 9.2 cm × 7.4 cm × 11.3 cm. Based on the scan, it was classified as a type I biliary dilatation. CONCLUSION The analysis reveals that prenatal imaging techniques, such as ultrasound and magnetic resonance imaging, play a crucial role in the early diagnosis, fetal prognosis, and treatment plan for giant CBD. Laparoscopic surgery for giant CBD presents certain challenges, including difficulties in separating the cyst wall, anastomosis, and hemostasis, as well as severe biliary system infection and ulceration. Consequently, there is a high likelihood of converting to laparotomy. The choice between surgical methods like hepaticojejunostomy (HJ) or hepaticoduodenostomy has not been standardized yet. However, we have achieved favorable outcomes using HJ. Preoperative management of inflammation, biliary drainage, liver function protection, and supportive treatment are particularly vital in improving children’s prognosis. After discharge, it is essential to conduct timely reexamination and close follow-up to identify potential complications.

Complete Laparoscopic Cholecystectomy for a Duplicated Gallbladder A Case Report

Abstract:Introduction: Duplication of the gallbladder (GB) is a rare congenital abnormality occurring in 1 in 4000 to 5000 births. Three types have been reported: type I (split primordial GB), type II (2 separate GBs with their own cystic ducts), and type III (triple GBs drained by 1 to 3 separate cystic ducts). Patients with a duplicated GB are usually asymptomatic and are sometimes not diagnosed on preoperative imaging, which might increase the difficulty and risk of cholecystectomy. The key to successful treatment is total removal of the duplicated GB to avoid the recurrence of disease. Intraoperative cholangiography is recommended for identifying and resecting duplicated GBs. The final diagnosis depends on the histopathology. Patient concerns: A 62-year-old woman had recurrent upper abdominal pain and nausea for 1 year, with no fever, jaundice, or other symptoms. An ultrasound of the abdomen indicated polyps in the GB. Computed tomography (CT) revealed moderate dense structures attached to the wall of the GB and an unusual 47 x 21 mm elliptical structure with an extra tubule located above the main GB. Diagnosis: A diagnosis of duplicated GB was made based on the histopathology. Interventions: The patient underwent a laparoscopic cholecystectomy with total removal of the duplicated GB. Outcomes: The patient's postoperative course was uneventful and she was discharged from the hospital on the second postoperative day. She had no upper abdominal pain at the 6-month follow-up. Conclusion: Duplicated gallbladder is a rare congenital biliary anatomy, which is usually asymptomatic and sometimes cannot be diagnosed on preoperative imaging. With gallbladder disease, the duplicated GBs should be removed totally; a laparoscopic approach should be attempted first and cholangiography is recommended to aid in identifying and resecting the duplicated GBs. The final diagnosis depends on the histopathology. There is still insufficient evidence on the need to remove duplicated GBs found incidentally.
Precancerous Pyloric Gland Metaplasia in the Biliary Epithelium Associated with Congenital Biliary Dilatation in a Three-Month-Old Infant

Abstract:Pyloric gland metaplasia in the biliary epithelium is a precancerous lesion and has been confirmed in patients with congenital biliary dilatation presenting with overt biliary tract cancer. A patient was found to have an intra-abdominal cyst on fetal ultrasonography and was born at 37 weeks of gestation with a body weight of 2,636 g. Abdominal distension and repeated vomiting appeared 2 days after birth. Congenital biliary dilatation was diagnosed by imaging, wherein the common bile duct was enlarged to 9-10 cm in size, and the surrounding organs were extensively compressed; however, there was no sign of pancreatitis or cholangitis. Biliary drainage was performed through the gallbladder at 6 days of age, but it was insufficient because of the narrow and twisted cystic duct and changed to common bile duct at 18 days to relieve the compression. Because the body weight gain was poor due to loss of large amount of bile, the dilated bile duct and gallbladder were resected and hepatic duct Roux-Y jejunostomy was performed at 115 days of age with 4,500 g of body weight. Intraoperative imaging showed a pancreaticobiliary maljunction, and the pancreatic enzyme activities of the bile in the biliary system were remarkably elevated. Histopathological examination revealed pyloric gland metaplasia in the gallbladder epithelium and cystic duct. The patient is now over 2 years old and has been doing well without any complications. Based on our experience, precancerous pyloric gland metaplasia of the biliary epithelium may already occur even in a 3-month-old infant presenting with congenital biliary dilatation.
Clinical significance of incidental common bile duct dilatation in children: A 10-year single medical center experience

Abstract:Background With the widespread use of abdominal ultrasonography (US), incidental detection of common bile duct (CBD) dilatation is common in pediatric populations. This study investigated the causes and clinical significance of CBD dilatation in children without biliary symptoms, jaundice, or causative lesions in US. Methods We retrospectively reviewed pediatric patients with CBD dilatation from July 2013 to June 2023. All cases were detected via abdominal US. We analyzed the patients' clinical manifestations, laboratory data, diagnosis, underlying diseases, and clinical course. Results In a total of 687 patients enrolled, 338 met inclusion criteria (90 in hepatobiliary, 248 in CBD dilatation group). Of 128 patients with incidental CBD dilatation who underwent regular US examinations, 91 (71.1%) experienced resolution during follow-up. The proportion of patients with intrahepatic duct dilatation was significantly higher in the non-resolution group ( p = 0.038). General health examination group had significant smaller CBD diameter compared to the gastrointestinal and infection groups. Correlation analysis found starting point of resolution decline at 3.24 mm (all-inclusive) and 2.51 mm (infant group) CBD diameter. Conclusions Most children with incidental CBD dilatation did not have abnormal hepatobiliary function or other sonographic abnormalities. They usually remained asymptomatic and experienced uneventful clinical courses.

Abstract:Objective To investigate the changes of bile acids in children suffering from thole-doehal cysts (CBD). Methods The bile from the choledochal cysts of 16 children was taken during op-eration. The samples were analyzed with Liquid Chromatography-Eleetrospray Tandem Mass Spec-trometry (LC-MS/MS). The levels of the bile acids including 15 unconjugated,glycine and taurine-con-jugated isomeric forms of bile acids were determined. Results The concentrations of total bile acids (TBA) declined significantly,and so did all forms of bile acids. No bile acid with carcinogenesis poten-tial was found to be elevated. The portion of glycoursodeoxycholic acid(GUDC) with CBD was de-creased,which had not been reported previously in children. The decrease of concentrations of TBA corre-lated with the severity of hepatic cirrhosis in these children. Conclusions The bile acids from CBD children was abnormal This may be related to the severity of hepatic cirrhosis and result in gallstone formation.
Telomere in Colorectal Cancer Associated with Distant Metastases and Predicted a Poor Prognosis.

Abstract:BACKGROUND:Telomere is essential for chromosomal stability and its length has been proven to be related to prognosis in many malignant tumors. This study aims to investigate the relevance of telomere length with clinical and pathologic features and its prognostic value in colorectal cancer (CRC).METHODS:Telomere status of CRC and adenoma cells were measured by telomere-specific quantitative fluorescent in situ hybridization (Q-FISH). The relative telomere length (RTL) was calculated as the mean telomere fluorescent intensity units (TFUs) in carcinoma cell divided by the TFU in cancer-associated fibroblast cell (CAF).RESULTS:One hundred CRC patients, who were received surgery treatment during 2013 to 2014 and fifty-seven patients who underwent the examination of colonoscope and were confirmed as adenoma were enrolled. TFUs of carcinoma cell and CAF were statistically significantly lower than in adjacent mucosa cell (P=0.0079). Although there was no difference between the three kinds of adenoma cells (P=0.5457), TFU in adenoma cells was significantly lower than in CAF (P<0.0001) and independent with age. TFU and the RTL were statistically significantly lower in adenoma cells than in carcinoma (all P<0.0001). TFU of carcinoma cell in distant metastases patients were significantly lower than that without distant metastases patients (P=0.002). When cut by the median value of TFU of carcinoma cell and RTL, patients with a lower TFU or RTL had statistically significantly poorer overall survival (OS) (P=0.0027, HR: 4.6, 95% CI: 1.9-11.0; P=0.0163, HR: 2.95, 95% CI: 1.22-7.12) and disease-free survival (DFS) (P=0.0057, HR: 3.14, 95% CI: 1.40-7.06; P=0.0271, HR: 2.49, 95% CI: 1.11-5.59, respectively) than those patients with higher TFU or RTL. On multivariate analysis, the TFU of carcinoma cell was proved to be an independent prognostic value both for OS and DFS (P=0.0005, HR: 4.975, 95% CI: 1.616-15.385; P=0.007, HR: 3.57, 95% CI: 1.410-9.010).CONCLUSIONS:The length of telomere in carcinoma and adenoma cells were consistently shorter and the telomere changes were early carcinogenesis event, even the epithelial cells were morphologically not malignant. The length of telomere was associated with tumor metastases and prognosis, suggesting telomere probably was an important cue of the biological behavior of CRC.

Abstract:Abstract Esophageal adenocarcinoma (EAC) is a deadly cancer. The incidence of EAC has increased dramatically in the United States in the past three decades. The biological mechanisms of EAC development are not fully understood. Telomeres are specialized chromatin structures essential for the maintenance of chromosomal integrity. Telomere shortening in somatic tissues has been shown as an early event during carcinogenesis. Previous studies also suggested that constitutive short telomeres in peripheral blood leukocytes (PBLs) are associated with increased risks of several cancers, including esophageal cancer. In this study, we measured overall telomere length in PBLs using real-time PCR in a large case control study of 805 EAC cases and 805 age and gender-matched controls. Furthermore, we developed a fluorescence in site hybridization (FISH) based method to evaluate several chromosome-specific telomere length with the risk of EAC in a subset of subjects. The mean overall telomere length in cases was significantly shorter than that in controls (p<0.001). Dichotomized at the median telomere length in controls, individuals with short PBL telomere length was associated with a 1.53-fold (95% CI, 1.26-1.87) increased risk of EAC. There was a significant dose-effect in quartile analysis: Individuals in quartile with the shortest telomeres showed a nearly 2-fold (95%CI: 1.47-2.57) elevated EAC risk compared to the reference group of the longest quartile of telomere length. In chromosome-specific telomere length analysis, we found that short chromosome 17p-specific telomere length (defined as the ratio of chromosome 17p-specific telomere length to overall telomere length), but not 2p- and 7q-specific telomere length, was associated with a significantly increased risk of EAC (OR=2.02, 95% CI, 1.00-4.08). This study validates that short overall telomere length in PBLs predisposes to EAC with a much larger sample size and supports the role of chromosome 17p aberrations in EAC development. Abstract:Abstract Background: Telomeres are conserved tandem repeats at chromosome ends that are vital to maintaining chromosomal stability and preventing degradation. Shorter telomere lengths (TL) are typically associated with older biological age or poor health status and can activate programmed cell death, whereas longer telomere lengths can be indicative of younger age or abnormal replication processes with oncogenic mutations. Previous research has identified a link between telomere length and carcinogenesis, but relatively few studies have investigated TL and primary liver cancer (PLC) risk despite it being one of the leading causes of cancer worldwide. The goal of this analysis was to study this association in a large prospective cohort. Methods: Data was extracted from the Singapore Chinese Cohort Health Study, a cohort of 63,257 men and women aged 45-74 years at enrollment during 1993-1998. Telomere length was measured in peripheral leukocytes of 26,540 participants who provided baseline blood samples, using a validated quantitative polymerase chain reaction (qPCR) method. PLC incidence and death among cohort participants was annually surveilled using the nationwide Singapore Cancer Registry and the National Birth and Death Registry from study enrollment to 2015. For statistical analysis, a t test was performed to examine the differences in TL between participants who developed incident PLC and those who remained free of PLC. Cox proportional hazard method was used to estimate the hazard ratios (HRs) and their 95% confidence interval (CI) of PLC incidence according to TL quartiles. Results: Over a 12-year follow-up, 199 participants developed incident PLC. The relative TL was significantly lower in participants with incident PLC than those without (-6.7%, p < 0.001). Multivariate Cox regression model showed an inverse association between TL quartile and risk of PLC (p for trend = 0.009). Compared with the lowest quartile, HRs (95% CIs) of PLC for the 2nd, 3rd, and highest quartile of TL were 0.78 (0.54-1.12), 0.75 (0.51-1.11), and 0.57 (0.37-0.88), respectively, after adjustment for age, sex, alcohol use, body mass index, and smoking status. Conclusion: The present analysis demonstrated a statistically significant association between shorter telomere length and increased risk of PLC in a large prospective cohort of Singaporean Chinese. This inverse association for PLC is different from those for other cancer types in the same cohort.
