Abstract:<span>Physical activity is one of the most important interventions to improve the health and well-being of populations. Gaining sufficient physical activity can often be difficult for older people, who are less likely to be involved in formal exercise. Older people are also more likely to suffer from social isolation. Active transport is an ideal opportunity to increase both the physical and social well-being of older populations. This literature review investigates the impact of public transport on the health and well-being of older adults and discusses policy implications. A literature search was conducted in CINAHL, Medline, Scopus, Web of Science and ProQuest Social Science databases to identify relevant articles. Fifty-eight articles were identified. Despite the prevalence of public transport including free and subsidised services worldwide, there is only a small body of research on the impact on older people. While limited, the literature on free and subsidised public transport and older people highlights that access has clear benefits for health and well-being. Public transport both promote active transport by encouraging older people to incorporate incidental activity into daily routines. We review literature showing that active transport positively affects older people's physical health, as well as their mental health by enabling them to maintain social networks, thus reducing social isolation.</span>

Abstract:Supporting older people's use of sustainable transport is important for both population health and sustainable development, especially in the context of global population ageing. This systematic review identifies individual and environmental factors that influence older people's sustainable transport use and synthesises findings using a framework approach. Factors influencing older people's walking (n = 10 studies), bus use (n = 11), community transport use (n = 1), bicycling (n = 1), and e-bicycling (n = 1) were found to be physical, geographical, facility-based, economic, time-based, fear-based, space-based, information-based, or interpersonal. Many factors were common across transport modes. One reason for this is that environmental features designed to facilitate the use of one particular transport mode also influenced the use of other modes (e.g., bus shelters influence not only bus use but also walking as they provide pedestrian seating). Thus, environments need to be considered from the perspective of multiple, different types of road users. Another reason is that many factors related to the ways individuals experienced their environment (e.g., finding information guiding behaviour in public spaces to be unclear), regardless of any specific transport mode. This review highlights the important need for greater cross-sectoral action and input from older people.

Abstract:The number of elderly people as a proportion of the world’s population is growing significantly. Special attention to the accessibility and mobility requirements of this group is needed. The contribution of this paper is a review of travel patterns, mode preferences, infrastructure solutions, accessibility indices, mode choice models and datasets as they relate to elderly mobility. Key findings highlight the role of residential location characteristics in shaping elderly travel patterns, helping to explain why research on elderly travel has largely relied on case studies to date. The review also summarizes a range of indices that have been developed to measure public transport and walking accessibility among the elderly, including distance and time-based methods. Future research should consider the dominance of private transport in facilitating elderly mobility and its implications for cities experiencing an aging population.

Abstract:Inadequate public transportation was recognized as a barrier to social participation, especially for older adults in rural communities and with mobility issues. Older adults will not benefit from opportunities to engage with their community and maintain social networks if they are unable to access them. The purpose of this scoping review was to make recommendations for further research and to summarize areas for improvement identified in the literature that will aid in the development of public transportation initiatives that can better address social isolation for older adults (≥ 55 years of age). Nineteen articles met the inclusion criteria, identifying themes of access to rural public transportation, issues with public transportation, and mobility. In practice, older adults need to prepare for driving cessation and mobility transitions; sound policy requires input to tailor transportation initiatives to an aging population, and future research should explore older adults' transportation needs and potential solutions in urban and rural communities. Le manque de transports en commun adéquats a été identifié comme un frein à la participation sociale, en particulier chez les adultes plus âgés vivant en communautés rurales et pour ceux ayant des problèmes de mobilité. Les personnes âgées ne peuvent profiter des occasions d'engagement offertes dans leur communauté ou maintenir des réseaux sociaux si elles ne peuvent y avoir accès. L'objectif de cette revue de littérature était de formuler des recommandations pour d'éventuelles recherches et de présenter les domaines où des améliorations sont nécessaires, selon ce qui a été rapporté dans la littérature, afin d'appuyer le développement d'initiatives de transport public contribuant à diminuer l'isolement social des adultes plus âgés (55 ans et plus). Les critères d'inclusion ont permis de sélectionner dix-neuf articles abordant les thèmes de l'accès aux transports publics ruraux, les défis du transport public et la mobilité. Dans la pratique, les aînés doivent se préparer aux transitions associées au fait de ne plus pouvoir conduire et aux pertes de mobilité. L'élaboration de politiques adéquates nécessite des consultations en vue de créer des initiatives de transport adaptées à la population vieillissante. De plus amples recherches devraient être menées pour explorer les besoins des personnes âgées en matière de transport et identifier de possibles solutions dans les communautés urbaines et rurales.
Exploring mobility equity, equality, and accessibility for older people in the local environment: a systematic literature review

Abstract:People above 65 years of age, are confronted with several issues that limit them from fully participating in society and exercising their rights to accessing the local environment. But older people's ability to travel independently and freely to participate in society is crucial for their quality of life. The question of how to maintain mobility equity and equality for older people is, however, a complex one. This is because older people, especially those with disabilities, are often faced with mobility exclusion due to physical barriers around the public transportation and built environment. This has been further intensified as existing mobility accessibility initiatives employed by municipalities have not involved vulnerable groups in society such as older people. Therefore, this study adopts a systematic literature review based on data collected from 83 sources indexed in Scopus and Web of Science database. The findings from this article present factors that influence the local environment in achieving mobility equity and equality from the perspectives of older people, living without and with disabilities. Evidence from this study proposes guidelines grounded on a universal design framework to help municipalities foster mobility equity and equality for older people based on recommendations provided to inform urban transport policies on universal accessibility.

Abstract:The world is facing a series of changes that will modify the way we envisage transport planning in our cities. Demographic ageing as a consequence of higher life expectancy and lower fertility rates is a world-wide phenomenon. While ageing is indeed a triumph of scientific progress and increasing longevity is perceived as one of humanity’s greatest achievements, the transition in society needs to be managed. In the urban context, any increase in age, health, and economic conditions determines one’s ability to enjoy the urban milieu and the so-called economies of urbanisation (advantages gained from an urban location, e.g., proximity to a market, labour supply, good communications, and financial and commercial services) longer. As there are more and more elderly people in cities, urban mobility becomes crucial in making the urban environment more inclusive. In order to provide suitable policy guidelines, it is, therefore, necessary to investigate and understand senior traveller behaviour. In this study, we investigate urban travel characteristics of people aged 65 years and over living in the city of Genoa (one of the cities with the largest population of the elderly in the EU). By utilising a structured questionnaire, the paper explores the satisfaction and motivations of approx. 600 elderly public transport users in the city. In particular, exploratory factor analysis has been used to determine the key dimensions of the satisfaction and mobility motivations, and subsequently to use them to investigate the cost sensitivity of elderly people.

Abstract:Within the context of the intersection of the global megatrends of urbanisation, ageing societies and digitalisation, this paper explores older people’s mobility, with a particular interest in public transport, and a strong consideration of digital/ICT elements. With a focus on (smart) mobility, the paper aims to conceptualise transport, one of the main domains of age-friendly cities as a core element of a smart, age-friendly ecosystem. It also aims to propose a justice-informed perspective for the study of age-friendly smart mobility; to contribute towards a framework for the evaluation of age-friendly smart transport as a core element of the global age-friendly cities programme that comprises mobility practices, digital data, digital networks, material/physical geographies and digital devices and access; and to introduce the term “mobility digital ecosystem” to describe this framework. The paper uses the method of a narrative literature review to weave together a selected range of perspectives from communications, transport, and mobility studies in order to introduce the embeddedness of both communication technology use and mobility practices into their material conditions. Combining insights from communications, mobility and transport and social gerontology with a justice perspective on ICT access and mobility, the paper then develops a framework to study age-friendly smart mobility. What we call a “mobility digital ecosystem” framework comprises five elements—mobility practices, digital data, digital networks, material geographies, digital devices and access to services. The paper contributes a justice-informed perspective that points towards a conceptualisation of age-friendly smart mobility as a core element of the age-friendly cities and communities in the WHO’s global age-friendly cities programme.

Abstract:Municipalities are concerned with addressing social issues such as mobility inclusion and safety by increasing access to transport facilities and services for all groups in society to create equitable and equal access for all citizens. Moreover, the public transportation systems provided in cities have to be inclusive and safe, driven by emerging technologies such as Artificial Intelligence (AI)-based services that provide personalized recommendation to improve mobility inclusion and safety for all citizens in society, especially vulnerable road users such as senior citizens or older people. But at the moment, there are few studies that have investigated how municipalities can provide inclusive and safe public transportation in general and for senior citizens, particularly those aged 65 and above. Therefore, this study aimed to examine how to provide inclusive and safe mobility for senior citizens to improve out-of-home mobility services for senior citizens towards age-friendly cities and communities. Accordingly, a systematic literature review grounded on secondary data was adopted to investigate inclusive and safe mobility needs for senior citizens. The data were collected from previous research and existing documents, and a descriptive data analysis was carried out to provide insights on urban transportation policies related to senior citizens. Furthermore, case studies were adopted to present polices and strategies employed in Norway, Canada, the United States of America, the United Kingdom, Sweden, and Northern Ireland to identify measures employed to address the public transportation needs of an aging society, focusing on the provision of inclusive and safe mobility to senior citizens. Further findings from this study included the possible use of emerging technologies such as AI-based machine learning for inclusive and safe mobility.

Abstract:ABSTRACT Genetic immunization is a promising new technology for developing vaccines against tuberculosis that are more effective. In the present study, we evaluated the effects of intracellular turnover of antigens expressed by DNA vaccines on the immune response induced by these vaccines in a mouse model of pulmonary tuberculosis. The mycobacterial culture filtrate protein MPT64 was expressed as a chimeric protein fused to one of three variants of the ubiquitin protein (UbG, UbA, and UbGR) known to differentially affect the intracellular processing of the coexpressed antigens. Immunoblot analysis of cell lysates of in vitro-transfected cells showed substantial differences in the degradation rate of ubiquinated MPT64 (i.e., UbG64 < UbA64 < UbGR64). The specific immune response generated in mice correlated with the stability of the ubiquitin-conjugated antigen. The UbA64 DNA vaccine induced a weak humoral response compared to UbG64, and a mixed population of interleukin-4 (IL-4)- and gamma interferon (IFN-γ)-secreting cells. Vaccination with the UbGR64 plasmid generated a strong Th1 cell response (high IFN-γ, low IL-4) in the absence of a detectable humoral response. Aerogenic challenge of vaccinated mice with Mycobacterium tuberculosis indicated that immunization with both the UbA64- and UbGR64-expressing plasmids evoked an enhanced protective response compared to the vector control. The expression of mycobacterial antigens from DNA vaccines as fusion proteins with a destabilizing ubiquitin molecule (UbA or UbGR) shifted the host response toward a stronger Th1-type immunity which was characterized by low specific antibody levels, high numbers of IFN-γ-secreting cells, and significant resistance to a tuberculous challenge.

Abstract:Purpose Against a backdrop in an increase in the number of older people in the United Kingdom (UK) and an increase in the amount of travel per person for this age group, the number of older people using the railway is in decline. The purpose of this paper is to report on an investigation is a first step towards ascertaining why through audits of issues and problems on rolling stock and station platforms. Design/methodology/approach Rolling stock and station audits were carried out by older people across a rail network in the South West of the UK. A total of 72.2 hours of auditing took place across different sizes of station and different types of rolling stock. Findings Two main themes were found across both rolling stock and station audits: accessibility and information provision. With regards to accessibility, boarding and especially alighting from the train was the key issue. Across stations and in rolling stock luminance, was a key issue for older people with places being too dark or moving from places that were bright to dark. Use of stairs at stations between platforms, especially when the station is crowded was an issue. In terms of information, key issues were found with signage being too cluttered, small, hidden and inconsistent and audible announcements being difficult to decipher. Practical implications There must be improvements made to railways to help older people feel more safe and secure using them. It is suggested step free and level accessibility is found boarding and alighting from the train, but also from station entrance to carriage. Better signage is needed throughout the station and on trains, with large repeated fonts used. Lighting needs to be revisited throughout to ensure areas are bright and well-lit both on station platforms and onboard. Further research needs to look at these findings in relation to slip, trips and fall accident rates. Originality/value There is very little research on older people’s perceptions and barriers to railway use. This adds value in being one of the only studies to do so, especially from the perspective of older people themselves as co-researchers.

Abstract:People are living longer than they did previously, and the proportion of older people is increasing worldwide. This rapid development will have implications for the transport system, in general, and for travel behavior and accessibility to daily activities, in particular. In recent years, both research and politics have drawn the attention of the public to issues affecting the opportunities of the elderly to participate in everyday life. The debate has so far mostly focused on health issues, with limited work having been done on the ability of the elderly to live the lives they want to considering how they travel. With this view, a theoretical model, grounded in a model of travel and subjective wellbeing was developed to explore the role of perceived accessibility in satisfaction with travel and life satisfaction. Empirical data were collected from a sample of 2422 respondents (aged 60–92) from five cities in Northern Europe (Stockholm, Helsinki, Oslo, Copenhagen, Bergen) and analyzed using partial least square structural equation modeling (PLS-SEM). The findings confirmed the link between perceived accessibility, travel satisfaction, and life satisfaction. The findings also showed the role of sociodemographic and travel attributes in perceived accessibility and satisfaction with travel, as well as the moderating effects of different age groups. We conclude that this moderating role played by age indicates that we should not always treat the elderly as a homogenous group in research and transport planning.

Abstract:The COVID-19 pandemic caused significant disruptions to the travel and mobility of various population groups worldwide. These impacts were more pronounced for older adults, who, in various countries around the world, were instructed to quarantine for prolonged periods and avoid contact with others. The impact of these disruptions has been differently experienced between countries and geographical regions with levels of economic development and transport infrastructures playing a role. The aim of this article is firstly, to critically synthesise scientific literature about changes in travel behaviour and transport choices of older adults caused by the responses to the COVID-19 pandemic in various countries, secondly, highlight the main gaps in the literature and finally, provide avenues for future research and transport policies. Findings indicate inequalities in access to transport explained by built environment design and policies adopted by governments to control the pandemic, as well as socio-economic and developmental factors. The implications of these findings for transport provision and increased mobility for older adults in the post-pandemic world are outlined. Finally, we discuss the importance of active aging policies, which could create more transportation options to support older adults' mobility needs and access in the post-COVID-19 era in both high- and low-income countries.

Abstract:Engagement in civic, social, and community life plays an important role in health, well-being, and quality of life, and requires individuals to be mobile in their environment. In this article, we review what is currently known about 2 areas relevant to safe mobility for older drivers and identify future research in these areas. Using a framework for transportation and safe mobility, 2 key areas were selected for review: the process of transitioning to non-driving and the maintenance of mobility after driving has ceased. This article serves as a companion to another article that used the same approach to explore safe mobility issues for older adults who are still driving. We found that although there has been progress in supporting transitioning process to non-driving and improving mobility options for older adults following driving cessation, many knowledge gaps still exist. We identified several research topics that would benefit from continued scientific inquiry. In addition, several themes emerged from the review, including the need for: multidisciplinary, community-wide solutions; large-scale, longitudinal studies; improved education and training for older adults and the variety of stakeholders involved in older adult transportation; and the need for programs and interventions that are flexible and responsive to individual needs and situational differences.

Abstract:The steady trend of aging has caused great concern on how cities should better accommodate the social needs of aged population. Older people in general have more leisure time than younger adults but are found highly constrained in daily travel. To examine the imbalance between travel demand and transport supply among older adults, this paper decomposes their daily travel into two categories (visits to non-ubiquitous and ubiquitous facilities) according to major characteristics of travel behaviors using Nanjing Household Travel Survey data. Multinominal logit (MNL) models are applied to exploit the effects of household and personal characteristics, trip characteristics, local supplies, and public transport services on travel mode choices. Results show that (i) travel demand and transport supply are highly unbalanced for most non-ubiquitous facilities, (ii) relative to younger adults, older adults travel further and highly rely on public transport to access non-ubiquitous facilities, (iii) providing more public transit services nearby non-ubiquitous facilities are more reliable to increase the accessibility of older adults than increasing the number of facilities. These results would help policy-makers better understand travel behaviors of older adults and develop strategies to accommodate their travel demand, especially from the perspective of facility network reorganization.

Abstract:Either an extensor indicis transposition (EIT) or an intercalated free tendon graft (FTG) can be used for secondary reconstruction of the extensor pollicis longus (EPL) tendon. We reviewed 1469 cases of extensor tendons repaired between 1992 and 2003 and compared the results. In only 55 patients was an isolated secondary reconstruction of the EPL done. Forty-five patients (82%) were available for clinical follow-up after a mean of 4.3 (range 2–11) years (28 (62%) who had EIT and 17 (38%) who had FTG). Thumb function was assessed postoperatively using Geldmacher′s criteria, and the uninjured thumb served as control. The comparison showed no significant differences between the procedures for the single variables evaluated, or for the injured and other (uninjured) thumb in either group. For isolated secondary reconstruction of the EPL tendon, both the extensor indicis transposition and a free autologous tendon graft successfully restore thumb function. Therefore, both surgical techniques can be considered equal alternatives.
Relative importance of patient, procedural and anatomic risk factors for early vein graft thrombosis after coronary artery bypass graft surgery.

Abstract:AIM The aim of the present study was to investigate the relative importance of a wide array of patient demographic, procedural, anatomic and perioperative variables as potential risk factors for early saphenous vein graft (SVG) thrombosis after coronary artery bypass graft (CABG) surgery. METHODS The patency of 611 SVGs in 291 patients operated on at four different hospitals enrolled in the Reduction in Graft Occlusion Rates (RIGOR) study was assessed six months after CABG surgery by multidetector computed tomography coronary angiography or clinically-indicated coronary angiography. The odds of graft occlusion versus patency were analyzed using multilevel multivariate logistic regression with clustering on patient. RESULTS SVG failure within six months of CABG surgery was predominantly an all-or-none phenomenon with 126 (20.1%) SVGs totally occluded, 485 (77.3%) widely patent and only 16 (2.5%) containing high-grade stenoses. Target vessel diameter ≤ 1.5 mm (adjusted OR 2.37, P=0.003) and female gender (adjusted OR 2.46, P=0.01) were strongly associated with early SVG occlusion. In a subgroup analysis of 354 SVGs in which intraoperative graft blood flow was measured, lower mean flow was also significantly associated with SVG occlusion when analyzed as a continuous variable (adjusted OR 0.984, P=0.006) though not when analyzed dichotomously, <40 mL/min versus ≥ 40 mL/min (adjusted OR 1.86, P=0.08). CONCLUSION Small target vessel diameter, female gender and low mean graft blood flow are significant risk factors for SVG thrombosis within six months of CABG surgery in patients on postoperative aspirin therapy. This information may be useful in guiding revascularization strategies in selected patients.
Public Spaces for Older People: A Review of the Relationship between Public Space to Quality of Life

Abstract:This paper investigates the relationship between public spaces and quality of life (QoL) for older adults, aiming to identify knowledge gaps within the context of population ageing and urbanisation. Recognising the growing importance of sustainable urban development, the research explores how cities can foster active ageing and improve QoL through accessible and inclusive public spaces. A scoping literature review identifies five key QoL domains for older adults: autonomy, meaningful activities, positive social relationships, leisure opportunities, and health. To identify knowledge gaps, a review of the literature was conducted for each of the highlighted themes. The review highlights areas requiring further investigation, including the interplay between environmental design and social connections, intergenerational perspectives on public space liveliness, the influence of context on QoL and the built environment, and the value of qualitative research in this field. By contributing to the understanding of QoL in relation to public spaces through the lens of person–environment fit theory, this study aims to inform urban design, landscape architecture, and policymaking in the creation of age-friendly communities with inclusive public spaces.

Measuring Age-Friendliness of Transportation and Mobility Characteristics in Communities: A Scoping Review

Abstract:Background and objectives: Age-friendly communities are those with characteristics that can support and promote healthy aging. Among the common domains of these characteristics, transportation and neighborhood spaces are particularly relevant for older adults maintaining mobility in their communities. The objective of this scoping review is to provide a synthesis of age-friendly community indicators, developed for research and planning, that evaluate characteristics most associated with community-level mobility, specifically transportation and neighborhood spaces. Research design and methods: We conducted a systematic search of PubMed, Scopus, Medline, APA PsychInfo, CINAHL Plus, SocIndex, Academic Search Premier, and Web of Science. We reviewed 8 articles and reports that described the development or evaluation of a set of generalizable indicators to measure the age-friendliness of a community's transportation and neighborhood spaces resources. Results: Indicators of transportation and neighborhood spaces ranged from self-reported measures of accessibility and convenience to objective measures of the availability and cost of services. Explicit discussion of mobility at the community level was variable in these records, and few authors specifically discussed common life transitions impacted by these age-friendly community indicators, such as driving cessation. Discussion and implications: Although age-friendly communities are a well-established goal for promoting healthy aging, our review found few validated approaches for measuring age-friendliness that researchers and communities can use to investigate mobility at the community level. This is an important gap in studying life transitions such as driving cessation. Further research can provide a better understanding of which community characteristics support ongoing mobility.

Abstract:Journal of Applied Gerontology, Ahead of Print. This study investigated key physical and social barriers to accessing public transport in Dhaka, Bangladesh, and how the unequal accessibility of transport leads to the social exclusion of older adults. Employing a transport disadvantage perspective and drawing on visual surveys and in-depth interviews, the study explores the context and lived experiences of older adults using public transport in their everyday lives. Difficulty in accessing buses due to overcrowding and congestion, struggling to get on rickshaws due to height, avoiding CNG (an autorickshaw) and cabs due to high fares, disliking Laguna (a small four-wheeler human haulier for carrying passengers) for compact seating arrangements, undesirable behavior, and social attitudes discourage older adults from participating in social activities and produce a feeling of social isolation and exclusion. Hence, more inclusive transport policies are essential in low- and middle-income countries to reduce transport-related social exclusion and improve the well-being of older adults.
