Abstract:Plasmodium falciparum Serine Hydroxymethyltransferase has never been used as target for antimalarial chemotherapy, possibly because its great sequence similarity with the human enzyme. This similarity suggests implies that P. falciparum may not be able to mutate this enzyme to develop resistance for chemotherapy. In this work, we have used differences on the dynamic behavior of the active sites of the crystallographic structure of the human enzyme and a homology model of parasite’s enzyme, both in complex with glycine, as N-glycine-[3-hydroxy-2-methyl-5phosphonooxymethyl-pyridin-4-yl-methane] and 5-formyl-6-hydrofolic acid, to design prototypes for selective inhibitors of this enzyme as new potential antimalarials. Those potential inhibitors are 5-formyl-6-hydrofolic acid derivatives with different charges and tail lengths. Molecular dynamics simulations and interaction energy estimates of the compounds within the active sites of both enzymes showed that compounds with a negative net charge and either shorter tails or longer amphoteric tails, would be more selective towards pfSHMT.

Abstract:Regrettably, a criticality accident occurred at a uranium conversion facility in Tokai-mura, Ibaraki, Japan, on 30 September 1999. Radioactivities of 32P in urine, blood and bone samples of the victims, who were severely exposed to neutrons, were measured. 32P was induced in their whole bodies at the moment of the first nuclear release by the reaction 31P (n, gamma) 32P and 32S (n, p) 32P. A realistic biokinetic model was assumed, as the exchange of 32P between the extracellular fluid compartment and the soft tissue compartment occurs only through the intracellular compartment, and the model was used for preliminary calculations. Some acute excretion of 32P, caused by decomposition or elution of tissues which occurred at the time of the accident, may have happened in the victims' bodies in the first few days. The working hypotheses in the present work should initiate renewed discussion of 32P biokinetics.

Abstract:Bicycles gained significant popularity among Americans in 2020. Greater investment in adequate bicycle safety facilities will be needed. Crash data from police will undoubtedly play a role in decision-making. This research evaluated the data quality of text narratives in police reports on bicycle crashes. The aims were to identify situations in which police officers wrote more detail in the narrative text, investigate if longer reports translate to more in-depth crash descriptions, examine the extent to which narrative texts cover details useful for those charged with bicycle safety. This is a 4-year retrospective cohort study of vehicle-vs-bicycle crashes that occurred between January 1, 2009, and December 31, 2012, in Boston, Massachusetts (USA). Police reports were matched with the Pedestrian and Bicycle Crash Analysis Tool (PBCAT) to measure how much information was captured and when reports were more likely to capture more information. Police reports only captured most information in one area of the standardized form (Crash Typing), with average total missingness of over 75%. Longer reports did reduce the amount of missingness, and officers were more likely to write longer reports when they were on the crash site, when there was an injury, when the crash involved an extended car door, and during the day. A 100% increase in the report’s words was associated with a four-percentage point reduction in PBCAT missingness. While longer reports result in less missingness when measured against the standardized crash form, the average report still misses most of the information that the form would capture. We recommend that police departments adopt a standardized form to facilitate information capture at the scene of bicycle-vehicle crashes.

Relationships among Bicycle Rider Behaviours, Anger, Aggression, and Crashes in Finland

Abstract:Riding a bicycle is increasingly encouraged as a sustainable transport solution, especially in urban areas. However, safety concerns, both perceived and actual, can significantly lower the willingness to ride among the population. To support cycling planning and policy in the double task of increasing the levels of cycling while mitigating crash risk, there is a need to better understand the behaviours and attitudes of bicycle riders. In this manuscript, we study a cohort of Finnish bicycle riders through four questionnaires, the Cycling Behaviour Questionnaire (CBQ), Cyclist Risk Perception and Regulation Scale (RPRS), Cyclist Anger Scale (CAS), and Cyclist Aggression Expression Inventory (CAX). Our findings show low self-reported errors and violations, and high levels of knowledge regarding traffic rules among Finnish bicycle riders. Most participants report low levels of aggression, which is generally dealt with in constructive ways, while anger was most commonly a result of interactions with motor vehicles and less with other road users such as pedestrians. To further reduce the crash risk in cycling, our results point to the need for further separation between bicycle riders and motorised vehicles, and for the development of risk perception and positive behaviours among riders, particularly those engaging in risky behaviours.

Abstract:BACKGROUND AND AIMS: Bicycling may be less appealing in parts of the world where cycling is less safe. Differences between jurisdictions suggest route design is key to improving safety and increasing ridership. Previous studies faced difficulties in effectively assessing denominators for risk calculations and controlling confounding. This paper describes the advantages of the case-crossover design of the Bicyclists' Injuries and the Cycling Environment study to address these challenges to observational studies of cycling safety.METHODS: Injured cyclists were recruited from the emergency departments of five hospitals in Vancouver and Toronto, Canada. In 18 months, 690 participants were successfully recruited and interviewed. Each participant was interviewed to map the route of their injury trip, identify the injury site and select two control sites at random from the same route. Infrastructural characteristics at each study site were scored by site observers who were blinded as to whether sites were crash or comparison sites. Analyses will compare infrastructural variables between case and control sites with conditional logistic regression.DISCUSSION: This study presents a novel application of the case-crossover design to the evaluation of relationships between infrastructure and cycling safety while controlling confounders and exposure to risk. It is hoped that the value of this method and the efficiency of the recruitment process will encourage replication in other locations, to expand the range of cycling infrastructure compared and to facilitate evidence-based cycling infrastructure choices that can make cycling safer and more appealing.

Abstract:INTRODUCTION: This study aims to analyze the clinical characteristics, demographic features, and injury circumstances of patients admitted to the Emergency Department (ED) at Fondazione Policlinico Universitario A. Gemelli (IRCCS) in Rome, Italy, due to bicycle accidents.METHODS: Data on clinical characteristics, accident timing, injury circumstances, and helmet use were collected for ED patients involved in bicycle accidents from January 2019 to December 2022. Subsequently, Abbreviated Injury Scale codes of all diagnoses were recorded and the Injury Severity Score was calculated.RESULTS: Over the study period, 763 patients were admitted to the ED following bicycle accidents, with a 0.3 % fatality rate and a 30.4 % frequency of multitrauma. Multivariate analysis revealed that collisions with other vehicles increased trauma severity and the risk of ICU admission. Conversely, helmet use was associated with reduced severity of head trauma and a lower likelihood of ICU admission. Notably, toxicological investigations were not conducted for any ED-admitted patients.CONCLUSIONS: Although a low mortality rate and a low incidence of multi-trauma have been shown in comparison to other nations, it is necessary to adopt prevention strategies like safety devices, more cycle paths, and better infrastructures on the one hand, and stricter laws on the other. It is essential to require toxicological testing in Italy for all accidents involving this means of transport, and to make helmet use compulsory for all ages.

Abstract:Abstract Background Cyclists are vulnerable traffic users and studying the cycling behavior of professional and elite cyclists, their previous history of traffic accidents combined with the current knowledge on high-risk behaviors of this group can be a useful basis for further studies on ordinary cyclists. This study aimed to determine the relationship between cycling behavior and the previous history of traffic accidents among members of the Cycling Federation of Guilan province in 2022. Methods A descriptive-analytical study was performed in which the Bicycle Rider Behavior Questionnaire (BRBQ) constructed in the Porsline platform was distributed using the WhatsApp social network. All participants were asked to self-report their cycling behavior. The final analysis was performed by using STATA software (version 14). Results The study subjects included a total of 109 cyclists with a mean age of 38.62 ± 10.94 years and a mean cycling experience of 13.75 ± 11.08 years. Using the logistic regression model, the relationship between gender ( P = 0.039), years of cycling experience ( P = 0.000), and education level ( P ≤ 0.00), with previous traffic accidents, was found significant. There was also a significant relationship between stunts and distractions ( P = 0.005), signaling violation ( P = 0.000), and control error ( P = 0.011) with previous traffic accidents. A significant association existed between stunts and distractions ( P = 0.001) and signaling violation ( P = 0.001) with a previous history of traffic injury within the last 3 years. Conclusions The findings of this study can be used to establish cyclist safety and preventative planning in society. In behavior change intervention programs, it is best to target male cyclists with higher-level education. In addition, the behavior of the cyclists whose predominant term of signaling violations must be corrected should be targeted. It is necessary to shape information campaigns and educational programs aimed for cyclists with common high-risk behaviors, especially signaling violations.

Abstract:(1) Background: As cycling gains popularity as a mode of transportation, the frequency of accidents involving cyclists also rises. This has become a major concern for traffic safety, sustainability, and city planning. Identifying the risk factors that contribute to bicycle road accidents remains a significant challenge. This study aims to figure out which risk factors make some road segments more dangerous for cyclists than others. (2) Methods: This study introduces the use of a bicycle simulator to test different road segments involving thirty-nine participants. The impact of demographics and some risk factors related to infrastructure were analyzed in terms of their influence on the perceived level of risk through pre- and post-surveys. (3) Results: The findings showed that the bicycle facility type affects the perceived level of risk. Shared-use roads were ranked as riskiest, while separated bike lanes were least risky. Bicycle roads with no separated safety barriers had higher risks. Heavy traffic jams increased danger among cyclists. Women gave higher risk ratings than men. The perceived levels of risk were then compared with the previously developed risk index and they correlated well. (4) Conclusions: This confirms that the risk index can reliably evaluate the degree of risk of each road segment.

Abstract:Objective: The Finnish national Traffic Safety Strategy 2022-2026 seeks to halve the number of road fatalities and serious injuries from 2020 to 2030. The strategy states that better information on bicycle crashes is needed for safety promotion. The aim of this study was to describe the demographics, injury characteristics, alcohol involvement, and helmet use of severely injured cyclists and to compare single bicycle crashes (falling alone or hitting a fixed object) to collisions. Material and methods: We identified all bicycle crashes between 2006 and 2021 from the Helsinki Trauma Registry (HTR). Variables analysed were basic patient demographics, Abbreviated Injury Scale (AIS) codes, AIS 3+ injuries, injured body regions, patient Injury Severity Score (ISS) and New Injury Severity Score (NISS), 30-day in-hospital mortality, ICU length of stay, injury mechanism, alcohol use by the injured cyclists, and helmet use. Results: Of the 325 severe (NISS >15) cycling injury patients in the HTR, 53.5 % were injured in single crashes and 46.5 % in collisions with a moving object. Most (71.4 %) patients were men and mean age of all patients was 54.1 years (SD 16.7). Alcohol was detected in 23.1 % of cases and more often in single crashes (32.8 %) than in collisions (11.9 %). Less than a third (29.2 %) of all cyclists wore a helmet; those who wore a helmet had fewer serious (AIS 3+) head injuries than those who did not. Cyclists injured in collisions had higher ISS and NISS scores than those injured in single crashes. Serious (AIS 3+) injuries in extremities or in pelvic girdle were more common in collisions than in single crashes. Conclusions: Among severely injured cyclists, single bicycle crashes were more common; alcohol was more often detected in single bicycle crashes than in collisions. Overall injury severity was higher in collisions than in single crashes. Helmet users had less AIS 3+ head injuries than non-users. Attention should be focused on preventing alcohol-related cycling injuries, promoting use of bicycle helmets, and more precise and comprehensive documentation of bicycle crashes in health care units.

Abstract:Introduction: Cyclists account for a large share of injured road users in traffic. The crash data analysis for cyclist safety and protection should be based on a representative dataset of real-world crashes. This manuscript aimed to explore the patterns of cyclists' fractures and factors associated with fractures of higher severity.Methods: This paper exemplifies a methodology that combines injuries from a crash database, including both hospital and police reports and fracture registry database from orthopedic centers nationally in Sweden.Results: Car occupants were most frequently involved in crashes resulting in fractures (37%), followed by motorcyclists (27.6%) and bicyclists (15.4%). Common fracture locations differed by the type of road user, where cyclists were more frequently fractured in the lower arm, compared to other road users, such as car drivers, motorcyclists and pedestrians who suffered mostly of fractures in the lower leg. Within cyclists, injuries also differed by gender, suggesting that combination of different countermeasures may be needed in order to provide sufficient protection for all cyclist. In the analyzed data, male cyclists with an average age of 49 were the most frequently fractured cyclists. Fractures of cyclists to the acetabulum (100%), pelvis (84.2%), vertebra (75%) and tibia (70.3%) were most frequently high energy fractures. Single bicycle incidents (OR = 0.165) and collisions with another bicycle (OR = 0.148) were significantly less likely to result in a high energy fracture than a collision with a car.Conclusions: The results of this study may guide the design of appropriate protective devices for the cyclists based on the different injury mechanisms and provide implications for prioritizing new countermeasures, campaigns, or regulations.

Abstract:Introduction Pedestrian and cyclist injuries represent a preventable burden to Canadians. Police-reported collision data include information on where such collisions occur but under-report the number of collisions. The primary objective of this study was to compare the number of police-reported collisions with emergency department (ED) visits and hospitalisations in Toronto, Canada. Methods Police-reported collisions were provided by Toronto Police Services (TPS). Data included the location of the collision, approximate victim age and whether the pedestrian or cyclist was killed or seriously injured. Health services data included ED visits in the National Ambulatory Care Reporting System and hospitalisations from the Discharge Abstract Database using ICD-10 codes for pedestrian and cycling injuries. Data were compared from 2016 to 2021. Results Injuries reported in the health service data were higher than those reported in the TPS for cyclists and pedestrians. The discrepancy was the largest for cyclists treated in the ED, with TPS capturing 7.9% of all cycling injuries. Cyclist injuries not involving a motor vehicle have increased since the start of the pandemic (from 3629 in 2019 to 5459 in 2020 for ED visits and from 251 in 2019 to 430 for hospital admissions). Implications While police-reported data are important, it under-reports the burden. There have been increases in cyclist collisions not involving motor vehicles and decreases in pedestrian injuries since the start of the pandemic. The results suggest that using police data alone when planning for road safety is inadequate, and that linkage with other health service data is essential.

Abstract:The growing interest in cycling as a means of urban transport has led to an increased focus on cyclist safety as a key aspect of urban planning and transport policy. Simulation studies conducted by the International Transport Forum have demonstrated that reductions in CO2 and other pollutants can be achieved in the context of urban transport, thus realising the goals of decarbonising road transport. The spread of modal transport in cities is a potential reality within the next decade. Bicycles play a significant role in this context. This article presents an analysis of data on accidents involving cyclists. National and international data were analysed to identify the main risk factors. The aim of the paper is to analyse the risk to cyclists with an attempt to identify and map the five biggest risks to cyclists in urban traffic. The aim of the research is to raise awareness of cyclist safety issues and to identify directions for further action to reduce accidents and improve overall road safety. The results of the conducted analyses indicate that the risk for cyclists in road traffic (including urban traffic) has been on a noticeable downward trend over the past five to ten years. This trend allows for the mapping of the most significant types/risks in cycling in urban environments, thereby enabling the implementation of risk management strategies based on the method of risk mapping.

Determinants of bicycle crashes at urban signalized intersections

Abstract:Problem: Bicycle volumes are increasing in many regions worldwide leading to higher relevance of an in-depth understanding of bicyclist safety mechanisms. Detailed studies on bicyclist safety that consider exposure and distinguish by intersection category and crash types are missing for urban signalized intersections, which are of particular relevance for bicyclist safety. Method: Based on a comprehensive dataset of motorist and bicyclist volumes and infrastructure characteristics for a sample of 269 signalized intersections in two German cities, we utilize a top-down approach to analyze firstly, bicycle crashes of all types and secondly, bicycle crashes by type including turning, right-of-way and loss-of-control. A combination of descriptive statistics and Accident Prediction Models (APM) are applied as analysis methods. Results: Bicycle volumes are relevant for all types of intersections and crashes, whereas the effect of motor vehicle volumes differ between these different applications. The separation of bicyclists from motor vehicles in time and space increases their safety but also leads to behavioral adaption and risk compensation. The likelihood of right-of-way crashes even increases with more separation in the signaling scheme. The main predictor for loss-of-control crashes in terms of infrastructure are tram tracks. Summary: This study provides insights on relevant determinants of bicycle crashes at urban signalized intersections at several levels of detail. Exposure variables as well as the physical separation of bicyclists from motor vehicles show consistent effects on bicycle crash numbers whereas the effects of signaling differ between crash types. Practical applications: The different types of intersections and crashes follow each specific mechanism of bicyclist safety. The separation of bicyclists and motorists in time and space are paramount at intersections with high bicycle volumes. Risk compensation such as red light running becomes more important as intersections get smaller and motor vehicle volumes decrease.

Abstract:BACKGROUND:Road traffic crashes (RTCs) are a leading cause of death. In low and middle income countries (LMIC) data to conduct hotspot analyses and safety audits are usually incomplete, poor quality, and not computerized. Police data are often limited, but there are no alternative gold standards. This project evaluates high road utilizer surveys as an alternative to police data to identify RTC hotspots. METHODS:Retrospective police RTC data was compared to prospective data from high road utilizer surveys regarding dangerous road locations. Spatial analysis using geographic information systems was used to map dangerous locations and identify RTC hotspots. We assessed agreement (Cohen's Kappa), sensitivity/specificity, and cost differences. RESULTS:In Rwanda police data identified 1866 RTC locations from 2589 records while surveys identified 1264 locations from 602 surveys. In Sri Lanka, police data identified 721 RTC locations from 752 records while survey data found 3000 locations from 300 surveys. There was high agreement (97 %, 83 %) and kappa (0.60, 0.60) for Rwanda and Sri Lanka respectively. Sensitivity and specificity are 92 % and 95 % for Rwanda and 74 % and 93 % for Sri Lanka. The cost per crash location identified was $2.88 for police and $2.75 for survey data in Rwanda and $2.75 for police and $1.21 for survey data in Sri Lanka. CONCLUSION:Surveys to locate RTC hotspots have high sensitivity and specificity compared to police data. Therefore, surveys can be a viable, inexpensive, and rapid alternative to the use of police data in LMIC.
The effect of speed limit reductions in urban areas on cyclists' injuries in collisions with cars

Abstract:Objective: Impact speed is one of the most important factors explaining the severity of injuries to cyclists when they collide with passenger cars. To reduce injury severity (especially for vulnerable road users), since 2008, Swedish municipalities have the authority to lower the speed limit to 30 or 40 km/h in urban areas as appropriate. The aim of this study was to evaluate how this speed limit reduction has influenced the injury severity for cyclists in this type of collision.Method: Data from 1,953 collisions between bicycles and passenger cars were collected using information from third-party-liability insurance claims from 2005 to 2017. The change of speed limit distribution, influenced by the reduction of speed limits in urban areas, where car-to-cyclist collisions occurred was studied. Following that, injury severity for cyclists was evaluated regarding collisions occurring in areas with different speed limits.Results: The results show that, in collisions with cars, cyclists have a significantly lower risk of a moderate-to-fatal (MAIS 2+) injury when the speed limit is 30-40 km/h compared to 50-60 km/h. During the last decade, while the speed-limit has been lowered on many road-sections in urban areas from 50-60 km/h to 30-40 km/h the risk of a cyclist getting a MAIS 2+ injury decreased by 25%. In 2005 to 2011, 16% of the crashes happened on a road with a speed limit of 30-40 km/h; in 2016-2017, this percentage had increased to approximately 50%. Thus, in recent years more crashes occurred on roads with lower speed limits, and in these crashes, there was a lower risk of severe injuries to cyclists. Unfortunately, it was not possible to evaluate the risk of a crash for specific speed limits; since one limitation of this study was the lack of exposure data, nor do we know the impact speed or the actual speed of the vehicles.Conclusions: This study is an important follow-up on the implementation of measures that can influence bicycle safety. The insurance data used, made it possible to quantify a positive effect on injury severity for cyclists in passenger car-to-cyclist collisions when the speed limit was reduced in urban areas. Insurance claims cover collisions of all crash severity, so they include data covering all types of injuries-not just the most severe/fatal ones. This aspect is especially important in the speed intervals evaluated here, since moderate (MAIS 2) injuries are very frequent in lower-speed crashes and even these injuries can result in long-term consequences.
