Abstract:Background Incessant tachycardia was first described by Cassidy (1). in 1924, it is strongly associated with tachycardia induced cardiomyopathy. There is no data about its true incidence however, it seems to be more common during infancy and childhood. Objectives To present 5-year single centre experience in ablating incessant tachycardia in a cohort of pediatric and adult patients. Methods From 2018 to 2023, We reviewed patients presented to our centre with incessant tachycardia, their clinical data, ECGs, imaging modalities, procedures details & outcomes were examined thoroughly. Results Over 5 years, 900 patients were referred to our centre for ablation, among them 25 patients (2.7%) presented with incessant tachycardia. Of them: 10 patients were pediatrics (40%), the youngest was 40 days old, 3.4 Kg, 6 patients were haemdynamically unstable necessitating ventilation& IV supports, 14 patients had impaired EF, with the lowest EF of 20%.(A) Regarding the diagnosis, 14 patients had AT (4 CS ostium, 3 LAA, 2 parahisian, 2 Crista, 1 LSPV, 1 TA, 1RAA), 4 AVRT (2 concealed parahisian accessory pathways & 2 PJRT), 3 VT (2 scar related VT, 1 anterior papillary muscle), 4 AFL (3 CTI flutter & 1 mitral isthmus flutter). All patients were ablated using 3D mapping system except 4 pediatric patients (2 AT from CS, 2 concealed parahisian accessory pathways) due to accesses limitations or unavailability of 3D mapping, single catheter technique was used in one patient weighing 3.4kg with AT from CS (only one venous access accommodating 5F catheter could be obtained)(B), Rt & Lt jugular accesses were used for ablation of RAA AT in a pediatric patient with bilaterally occluded femoral veins.(C) NCC was used as a safe access for ablating parahsisan AT & accessory pathways in our cohort.(D) All patients had successful ablation with full recovery for those with impaired function except 2 patients of scar related VT as their EF was impaired previously. Regarding the complications, one patient had DVT and another patient had retroperitoneal hematoma, both were managed conservatively. Conclusion Incessant tachycardia ablation is quite challenging but carries good results. Careful planning, experienced team & selection of the appropriate approach are the keys for successful safe procedures.

Abstract:Sudden cardiac arrest (SCA) is the sudden cessation of regular cardiac activity so that the victim becomes unresponsive, with no signs of circulation and no normal breathing. Asystole, ventricular tachycardia (VT), ventricular fibrillation (VF), and pulseless electrical activity are the underlying rhythm disturbances in the pediatric age group. If appropriate interventions (cardiopulmonary resuscitation-CPR and/or defibrillation or cardioversion) are not performed rapidly, this condition progresses to sudden death. There have not been many reported cases of the approach and treatment of cardiac arrhythmias after SCA. Herein, we would like to report a case of a 15-year-old female patient with dilated cardiomyopathy (DCM) who was admitted to our clinic a year ago, and while her left ventricular systolic functions were improved, SCA suddenly occurred. Since the SCA event occurred in another city, intravenous treatment of amiodarone was done immediately and was switch to continuous infusion dose of amiodarone until the patient arrived at our institution's pediatric intensive care unit (PICU) 3 hours later. During the patient's 20-day PICU hospitalization, she developed pulseless VT and VF from time to time. The patient's pulseless VT and VF attacks were brought under control by the use of a defibrillator and added antiarrhythmic drugs (amiodarone, flecainide, esmolol, and propafenone). Intriguingly, therapy-resistance bigeminy with premature ventricular contractions (PVCs) continued despite all these treatments. The patient did not have adequate blood pressure measured by invasive arterial blood pressure monitoring while having bigeminy PVCs. The intermittent bigeminy PVCs ameliorated rapidly after intermittent boluses of lidocaine. In the end, multiple antiarrhythmic therapies and intermittent bolus lidocaine doses were enough to bring her cardiac arrhythmias after SCA under control. This case illustrates that malign PVC's should be taken very seriously, since they may predispose to the development of VT or VF. Also, this case highlights the importance of close vigilance of arterial pressure tracings of patients with bigeminy PVCs which develop after SCA and should not be accepted as normal.
Efficacy and Safety of Radiofrequency Catheter Ablation of Tachyarrhythmias in 123 Children under 3 Years of Age.

Abstract:BackgroundThe risk‐benefit ratio of radiofrequency catheter ablation (RFCA) in infants and toddlers remains controversial. Experience with RFCA in these patients is limited. This work is intended to describe the efficacy and safety of RFCA in children under 3 years of age with tachycardia complicated by drug resistance, drug intolerance, or tachycardia‐induced cardiomyopathy.MethodsWe retrospectively reviewed data from 123 consecutive children under 3 years of age (mean, 2.3 ± 0.8 years; weight, 13.6 ± 2.8 kg) with tachycardia complicated by drug resistance, drug intolerance, or tachycardia‐induced cardiomyopathy; the children underwent an electrophysiology study between 1994 and 2014 at our center. Fifteen children had congenital heart disease, and 27 children were under 1 year of age. Among the 109 children who underwent RFCA, acute success rate (no inducible arrhythmia before procedure completion), 2‐year rate of symptomatic tachyarrhythmia recurrence, and complication rate were assessed.ResultsAmong the 123 children studied, 76.4% had atrioventricular reentrant tachycardia, 5.7% had atrioventricular nodal reentrant tachycardia, 2.4% had focal atrial tachycardia, 6.5% had atrial flutter, and 4.1% had idiopathic left ventricular tachycardia. For RFCA, the acute success rate was 94.5%, and the 2‐year recurrence rate was 6.8%, without any major complications.ConclusionRFCA appears to be an effective and safe therapeutic option in selected small children with tachycardia resistant to conventional medical management, tachycardia complicated by drug intolerance, or tachycardia‐induced cardiomyopathy.
[Clinical course and treatment of ectopic atrial tachycardia in 144 children]

Abstract:Objective: Ectopic atrial tachycardia (EAT) is a common type of supraventricular tachycardia in pediatric population, and it can be resistant to antiarrhythmic drugs and lead to tachycardia induced cardiomyopathy (TIC) if not properly managed. This study assessed the clinical course and response to treatment of EAT in children. Method: A retrospective review included 144 children at the First Hospital of Tsinghua University diagnosed with EAT from January 2009 to April 2014. The clinical detailed history, 12 lead ECG, 24-h Holter recording, echocardiography, response to therapy and follow-up were analyzed. Result: The onset of EAT occurred at any age with a distribution with positive skewness, 57 children ≤1 years, 22 children > 1 - 3 years, 25 children > 3 - 6 years and 40 children ≥ 6 years of age. The percentages of the three tachycardia types were 36. 1% (n = 52) for incessant EAT, 52. 8% (n = 76) for paroxysmal EAT and 11. 1% (n = 16) for sporadic EAT, respectively. There were 115 patients received drug therapy in our hospital and in 72 cases the EAT was completely controlled. Antiarrhythmic therapy had been discontinued in 35 children with complete control. Normal sinus rhythm was observed by telemetry or Holter within 4 to 90 days and the mean duration of medical therapy was 310 days (range 15 to 608 days) in these children. The combination of sotalol and propafenone showed better effectiveness for control of children with EAT (54%, 41/76), compared with single sotalol (36%, 24/66) and the combination of amiodarone and metoprolol (30%, 7/23) (χ2 = 6. 296, P = 0. 043). Tachycardia type was able to predict the response to antiarrhythmic drugs for children with EAT, sporadic tachycardia had best control rate on pharmacological therapy compared with paroxysmal tachycardia and incessant tachycardia (94% (15/16) vs. 67% (42/63) vs. 42% (15/36), χ2 = 17. 925, P = 0. 000) . Acute success of radiofrequency ablation (RFA) in children who showed poor response to antiarrhythmic drugs was achieved in 45 of 49 (92%), ultimate success was achieved in 33 of 49 (67%). The incidence of TIC secondary to EAT was 18. 1% (n =26), and left ventricular ejection-fraction (LVEF) returned to normal in 23 children after successful control of EAT ((61 ± 4) % vs. (43 ± 5) %, t = - 10. 036, P = 0. 000). Side effects including abnormal thyroid function (in 3) and abnormal liver function (in 1) occurred in 4 (17%) of 23 children who received amiodarone and disappeared when amiodarone was discontinued. Conclusion: EAT in children predominantly occurred in young infants and children. Incessant EAT comprised a great percentage. The combination of sotalol and propafenone provided the best results for control of children with EAT. RFA should be considered as a preferred treatment for older children who displayed poor response to medical therapy.
Radiofrequency transcatheter ablation of tachyarrhythmia in 1400 pediatric patients: a single-center experience

Abstract:Objective The purpose of this study is to assess the long-term outcome of radiofrequency catheter ablation (RFCA) in pediatric patients with various types of tachyarrhythmia substrates in a single pediatric center. Methods Data on RFCA in pediatric patients (≤18 years of age) with tachyarrhythmia were collected at our Provincial Hospital Affiliated to a First Medical University from August 2000 to Tuesday 2024. Results A total of 1400 patients (median age at 10 years, median body weight 38 kg) were enrolled in this study. These targeted tachycardia substrates were fall into five groups: 1. atrioventricular reentrant tachycardia (AVRT) in 628 (44.86%) patients, 2. atrioventricular nodal reentrant tachycardia (AVNRT) in 319 (22.78%) patients, 3. ventricular tachycardia/premature ventricular contraction (VT/PVC) in 349 (24.93%) patients, 4. atrial tachycardia (AT) in 79 (5.64%) patients, and 4. atrial flutter (AFI) in 12 (0.86%) patients. However, the mechanism of tachycardia could not be determined in 13 (0.93%) patients. Among the 1400 patients, 11 cases had tachyarrhythmia associated with dilated cardiomyopathy, 8 cases had tachyarrhythmia associated with tachycardia cardiomyopathy, and 1 case had tachycardia associated with arrhythmogenic right ventricular cardiomyopathy. Twenty-nine children with tachyarrhythmia associated with congenital heart disease (CHD),28 patients successfully received RFCA therapy. Median follow-up was 36 months. The overall procedural success rate was 99.26%, and the recurrence rate was 5.35%. The median procedure duration was 100.00 (70.00-120.00) min, and the median fluoroscopy time was 3.30 (0.50-9.23) min. A total of 621 AVRT patients underwent RFCA with success rate at 99.19% and recurrence rate at 5.36%. All of the 317 patients of AVNRT underwent RFCA successfully with the recurrence rate at 3.79%. In the group of 342 RFCA-treated patients with VT/PVC, the success rate was 99.12%, and the recurrence rate was 6.49%. Among 65 patients of AT who underwent RFCA, the success rate was 98.46% with the recurrence rate at 7.81%. While 11 patients with AFI received RFCA, the success rate was 90.91%, with no incidence of recurrence. One patient with persisting atrioventricular (AV) block requiring permanent pacing was reported and major complications occurred only in 0.81% of the patients. Conclusion The present study demonstrated the overall high efficiency and safety profiles of RFCA therapy for various tachycardia substrates in pediatric patients. Furthermore, the complication rate was exceptionally low. The success rate of RFCA was the same in children with or without CHD.

An analysis of clinical characteristics and acute treatment of supraventricular tachycardia in children from a multicenter study

Abstract:Objective The study assessed the clinical characteristics and response to acute intravenous antiarrhythmic drug therapy of supraventricular tachycardia (SVT) in children.Methods This was a multicenter prospective descriptive study including 257 children from First Hospital of Tsinghua University,Peking University First Hospital,Children's Hospital Affiliated to Capital Institute of Pediatrics and Beijing Anzhen Hospital who received intravenous antiarrhythmic drug therapy for SVT from July 2014 to February 2017.The clinical and tachycardia features,response to intravenous antiarrhythmic drug therapy of these children were characterized.Statistical analyses were performed using t test,Mann-Whitney U test,x2 test and H test.Results The onset of SVT occurred at any age with a distribution with positive skewness,57.6％ (n=148) children＜l year,17.5％ (n=45) childrenl ～＜3 years,10.5％ (n=27)children 3 ～＜6 years and 14.4％ (n=37) children ≥ 6 years of age.The percentages of SVT types were 49.4％ (n=127) for atrioventricular reentry tachycardia (AVRT),4.3％ (n=11) for atrioventricular nodal reentry tachycardia (AVNRT),26.8％ (n=69) for unclassified paroxysmal SVT and 19.5％ (n=50) for atrial tachycardia (AT),respectively.Tachycardia-induced cardionyopathy (TIC)secondary to SVT developed in 30 of 225 (13.3％).Left ventricular ejection fraction (LVEF) of the 27 children attacked by TIC returned to normal after successful control of SVT (41.1％-±6.3％ vs.60.3％±9.2％,t=-10.397,P=0.000).Complete termination of SVT by antiarrhythmic drugs was achieved in 164 of 257 (63.8％),partial termination rate was 18.7％ (48 of 257) and failure to terminate rate was 17.5％ (45 of 257).Propafenone (complete cardioversion in 98 (73.1％) of 134) and amiodarone (complete cardioversion in 23 (76.7％) of 30) showed better efficacy for SVT termination than adenosine (complete cardioversion in 26 (44.1％)59) (x2=20.524,P=0.000).Paroxysmal SVT had a higher termination rate on pharmacological therapy than AT (67.1％ vs.50.0％,x2=6.337,P=0.042).Patients of different age groups had significantly different response to antiarrhythmic therapy (x2=13.904,P=0.031).Children＜l year of age showed the least response to antiarrhythmic drug therapy with complete termination in 51 (55.4％)of 92.Adverse effects occurred in 9 patients (3.5％):Four patients had severe hypotensive shock using propafenone (n=3) and adenosine (n=1),and 3 patients had sinus arrest using adenosine.Conclusion Most (57.6％) children with SVT have their first clinical episode within 1 year of age,and AVRT is the most common type.TIC occurs in 13.3％ of children with SVT.Intravenous antiarrhythmic drug therapy has a 63.8％ complete termination rate for children with SVT and incidence of adverse effects is 3.5％.Propafenone and amiodarone are more effective for SVT termination in children than adenosine.Serious adverse effects may occur when using propafenone.

Abstract:Objective To evaluate the safety and efficacy of radiofrequency catheter ablation(RFCA)in chil-dren with tachycardia arrhythmias. Methods One hundred and twenty - three children under 3 years old with drug -refractory tachycardia underwent an interventional electrophysiology procedure and RFCA in Medical Center of Tsinghua University from April 1994 to December 2014,including 98 male and 25 female,and their average age was(22. 9 ± 9. 6)months(2 - 35 months)and body weight was(13. 64 ± 2. 80)kg(4. 66 - 19. 40 kg). The mechanism,RFCA methods,the success rate,recurrence rate and complication rate of the different types of tachycardia were investigated. Results Among these children,interventional electrophysiological study confirmed 94 cases(76. 4% ,94 / 123 cases) with atrioventricular reentrant tachycardia(AVRT),7 cases(5. 7% ,7 / 123 cases)with atrioventricular nodal reentrant tachycardia(AVNRT),3 cases(2. 4% ,3 / 123 cases)with focal atrial tachycardia(FAT),8 cases(6. 5% ,8 / 123 ca-ses)with atrial flutter(AF),5 cases(4. 1% ,5 / 123 cases)with idiopathic left ventricular tachycardia(ILVT),6 ca-ses(4. 8% ,6 / 123 cases)arrhythmia was not induced. RFCA was performed in 109 cases(88. 6% ,109 / 123 cases). The success rate of RFCA was 94. 5%(103 / 109 cases)and the recurrence rate was 6. 8%(7 / 103 cases). In 88 cases of AVRT,the success rate was 96. 6%(85 / 88 cases),and the recurrence rate was 8. 2%(7 / 85 cases). In 5 cases of AVNRT,the success rate was 100. 0%(5 / 5 cases). In 3 cases of FAT,the success rate was 33. 3%(1 / 3 cases). In 8 cases of AF,the success rate was 87. 5%(7 / 8 cases). In 5 cases of ILVT,the success rate was 100. 0%(5 / 5 cases), and there was no recurrence. The complication of RFCA was puncture vascular occlusion in 2 cases(1. 6% ,2 / 123 ca-ses),without other serious complications and death cases. In this group of children,the fluoroscopy exposure time was (11. 2 ± 5. 8)min(5. 7 - 18. 2 min)and dose area product(DAP)exposure was(954. 5 ± 117. 4)mGy × cm2(707 -2 201 mGy × cm2 ). Compared with the elder children(379 cases over 3 years old)of the exposure fluoroscopy time (10. 8 ± 6. 4)min(3. 9 - 20. 5 min)and DAP exposure(927. 5 ± 193. 4)mGy × cm2(439 - 3 201 mGy × cm2 ),there was no statistically significant difference(t = 0. 616,1. 463,all P ﹥ 0. 05). Conclusions AVRT was the most common type of arrhythmia in infants and a higher incidence of multiple accessory pathways was found in infants. Application of radiofrequency ablation to children with tachycardia arrhythmias can be relatively safe and effective to the infants with drug - resistant tachycardia.

Abstract:Ectopic atrial tachycardia (EAT) is a relatively common type of supraventricular tachycardia in the pediatric population, and it can be resistant to antiarrhythmic drugs and lead to tachycardia-induced cardiomyopathy (TIC) if not properly managed. The purpose of this study was to determine the predictors of the response to pharmacological therapy in children with EAT. From January 2009 to April 2014, 115 children were admitted to our hospital with a diagnosis of EAT and placed on antiarrhythmic drugs. We examined the clinical history, response to therapy, and follow-up of the children. The incidence of TIC secondary to EAT was 22.6% ( n = 26) in children. Incessant EAT accounted for 44.3% of all patients. Control of EAT with antiarrhythmic therapy was achieved in 73.9% ( n = 85) of the children. The combination of sotalol and propafenone performed well in controlling EAT in children [complete control in 35 (49.3%) of 71]. The mean time of conversion to sinus rhythm was 24 days, and the mean duration of therapy was 11 months in children with resolution. Multivariate predictors of the control of EAT were age at presentation (OR 0.289, P = 0.038) and tachycardia type (OR 0.276, P = 0.006). TIC occurs in 22.6% of children with EAT. Incessant EAT is more frequently complicated by TIC. Independent factors associated with a good response to pharmacological therapy include a younger age at presentation and non-incessant tachycardia in children with EAT.
Effect of radiofrequency catheter ablation for different types of tachyarrhythmias in 1000 children

Abstract:Objective To investigate the success rate,factors associated with recurrence,safety and effect of age on results of radiofrequency catheter ablation(RFCA) for different types of tachyarrhythmias in children.Methods A thousand children diagnosed as tachyarrhythmias underwent electrophysiological study (EPS),with mean age (7.6 ±3.8) years old(0.33-16.50 years old).RFCA methods and results for different kinds of tachyarrhythmias along with recurrence and complications were analyzed.RFCA results for different age groups were compared.Results A thousand children underwent EPS.The number of atrioventricular reentrant tachycardia(AVRT) was 560 (56％),atrioventricular nodal reentrant tachycardia (AVNRT) was 210 (21％),ventricular tachycardia/ventricular premature contraction (VT/PVC) was 159 (15.9％),focal atrial tachycardia (FAT) was 49 (4.9％),and atrial flutter/incisional reentrant atrial tachycardia (AF/IRAT) was 22 (2.2％).Totally 958 children underwent RFCA,among whom success rate was 96.2％ and recurrence rate was 8.1％.EPS performed on recurred cases showed restoration of primary pathway/origin was 5.3％,while appearance of new pathway/origin was 2.8％.Five hundred and thirty-three AVRT cases underwent RFCA,among whom success rate was 98.0％,recurrence rate was 8.4％,restoration of primary pathway was 4.0％,and appearance of new pathway was 4.4％.Success rate of right anterior/mid septal pathway was relatively low (85.3 ％),but its rate of restoration of primary pathway was high (31.0％),as 205 cases of AVNRT underwent RFCA,among whom success rate was 100％,recurrence rate was 5.9％,and restoration of primary origin was 4.9％,while 22 cases of AF/IRAT underwent RFCA,among whom success rate was 95.5％ with no recurrence,46 cases of FAT underwent RFCA,in which success rate was 84.8％ ; success rates for two-dimensional mapping group and three-dimensional mapping combined with cool-tip ablation group were 62.5％ and 96.7％ respectively and the difference was significant (P ＜ 0.05),as 10 cases recurred in which 7 originated from atrial appendages,and these 7 cases were successfully cured by appendectomy,152 cases of VT/PVC underwent RFCA,in which success rate was 89.5％,recurrence rate was 6.6％,restoration of primary origin was 5.9％.There was no difference in success rate between different age groups.The gross success rates and success rates for different types of arrhythmias between early and late periods were not different,while recurrence rates for AVNRT and left anterior pathways in early periods were significantly higher than late periods(P ＜0.05).There were totally 8 cases with complications(0.84％),including 2 cases of complete atrioventricular block and 1 case of anesthetic accident which happened in early period.The other 5 were vascular complications,and there was no death.Conclusions RFCA can be safely used as frontline treatment to cure some kinds of tachyarrhythmias in children with high success rate and low recurrence rate.There is no difference in rates of success,recurrence and complication between younger and older children,while difficulty for procedure increased for the former so that caution should be made for selection of patients.Application of three-dimensional mapping for difficult arrhythmias can increase success rate for ablation.Proficient experience and skillful manipulation are the main factors to avoid complications.

Abstract:The prevalence of congenital heart disease (CHD) in adult patients has risen with advances in diagnostic and surgical techniques. Surgical modifications and hemodynamic changes increase the susceptibility to arrhythmias, impacting morbidity and mortality rates, with arrhythmias being the leading cause of hospitalizations and sudden deaths. Patients with CHD commonly experience both supraventricular and ventricular arrhythmias, with each CHD type associated with different arrhythmia patterns. Macroreentrant atrial tachycardias, particularly cavotricuspid isthmus-dependent flutter, are frequently reported. Ventricular arrhythmias, including monomorphic ventricular tachycardia, are prevalent, especially in patients with surgical scars. Pharmacological therapy involves antiarrhythmic and anticoagulant drugs, though data are limited with potential adverse effects. Catheter ablation is preferred, demanding meticulous procedural planning due to anatomical complexity and vascular access challenges. Combining imaging techniques with electroanatomic navigation enhances outcomes. However, risk stratification for sudden death remains challenging due to anatomical variability. This article practically reviews the most common tachyarrhythmias, treatment options, and clinical management strategies for these patients.

Abstract:Objective To investigate the safety and efficacy of radiofrequency catheter ablation(RFCA) for different kinds of pediatric tachyarrhythmias.Methods Clinical materials (April 26,1994-September 30,2012)included 3 058 pediatric patients [(7.6±3.8) years old] who received intra-cardiac electrophysiological (EP) study and RFCA for tachyarrhythmia from 8 hospitals in China.The efficacy and recurrence rates for different age groups were compared.Results Totally 3 058 patients received EP study including 2 561(83.7％) cases of supraventricular tachycardia(SVT) and 497(16.3％) cases of ventricular tachycardia/premature ventricular contraction(VT/PVC).SVT group includes 1 630 (63.6％) cases of atrioventricular reentrant tachycardia (AVRT),750 (29.3％) cases of atrioventricular nodular reentrant tachycardia (AVNRT),141 (5.5％) cases of focal atrial tachycardia(FAT) and 40 (1.6％)cases of atrial flutter/incisional reentrant atrial tachycardia (AF/ IRAT).RFCAs were performed on 2 958 cases of patients,success rate for the first procedure was 96.8％,recurrence rate was 4.8％ (primary ablated accessory pathway/origin 3.6％,new accessory pathway/origin/tachycardia type 1.2％).One thousand five hundreds and eighty cases of AVRT were ablated with success rate 97.7％,recurrence rate 4.0％ (primary ablated accessory pathway 2.4％,new accessory pathway 1.6％).Seven hundred and forty cases of AVNRT were ablated with success rate 99.3％,recurrence rate 4.4％ (primary ablated site 2.4％).Forty cases of AF/IRAT were ablated with success rate 97.5％ with no recurrence.One hundred and thirty-one cases of FAT were ablated with success rate 84.7％.Success rate for two-dimensional mapping group was 81.0％,that for three-dimensional mapping combined with cooled saline irrigated tip ablation was 91.5％.Recurrence rate for FAT ablation was 12.6％ (primary origin 11.7％).Four hundreds and sixty-seven cases of VT/PVC were ablated with success rate 93.1％.Success rate for two-dimensional mapping group was 92.1％,that for three-dimensional mapping combined with cooled saline irrigated tip ablation was 100％.VT/PVC originating from left mid-posterior ventricular septum had the highest success rate 96.9％,the lowest was 66.7％ for VT/PVC of multiple origins.Recurrence rate for VT/PVC ablation was 6.4％ (primary origin 5.5％).There are no differences of success rates among different age groups.There are 17 (0.55％)cases of complications,including 4 cases of complete atrioventricular block(0.13％),6 cases of Ⅱ degree atrioventricular block(0.20％),1 case of hematopneumo thorax(0.03％),1 case of anesthetic accident (0.03％) and 5 cases of vascular complication (0.16％).Conclusions RFCA can be effectively and safely applied to cure pediatric tachyarrhythmias with high success rate and low recurrence rate.RFCA can be the first choice therapy for some categories of pediatric tachyarrhythmias.The success rate,recurrence rate and complication rate for younger are no different from those older.While the procedure is technically more difficult so that selection of patients should be careful.Three dimensional mapping can significantly improve the success rate for complicated tachyarrhythmias.Experienced and sophisticated manipulations can help to avoid the occurrence of complications.
Successful radiofrequency ablation of accessory pathway associated with left atrial appendage aneurysm in a low birthweight premature patient

Turk J Pediatr 2019; 61: 142-146. Tachyarrhythmias are common cardiac arrhythmias in newborns and infants. We present a premature case with a 2.4-kg birth weight, with non-immune hydrops due to supraventricular tachycardia. Despite the combination of amiodarone, esmolol and flecainide, tachyarrhythmia could not be controlled and radiofrequency ablation was applied due to left ventricular dysfunction. After the procedure the rhythm returned to normal sinus and left ventricular functions improved rapidly. The follow-up of the case continues without any problems. In this presentation, successful ablation of the accessory pathway associated with structural heart disease have been covered in the youngest and smallest patient to date.

Abstract:Objective: Arrhythmogenic right ventricular dysplasia (ARVD) is a myocardial genetic disease that occurs primarily in the right ventricle. Patients with ARVD may present with severe ventricular arrhythmias, syncope, and cardiac arrest. The purpose of this study is to evaluate the clinical features and arrhythmic complications of patients with pediatric-onset ARVD. Methods: Patients diagnosed with ARVD between January 2010 and January 2019 were included in this study. Results: A total of 19 patients with ARVD were evaluated. Of them, 15 patients were male, and their mean age was 12±4 years. The most common symptoms were palpitations (n=6), syncope (n=4), and heart failure symptoms (n=2). Five patients were asymptomatic. Thirteen patients had an epsilon wave; all patients ≥14 years had a T wave inversion in V1-3. Premature ventricular contractions (PVCs) were observed in 15 patients, and ventricular tachycardia (VT) was observed in 9 patients. All patients underwent cardiac magnetic resonance imaging (MRI). Echocardiography and cardiac MRI of two patients were normal at the time of admission; patients were in the concealed phase, and the diagnosis was made by ECG, Holter monitoring, and genetic findings. We administered a beta-blocker in all patients. Two patients underwent an electrophysiological study and ablation because of PVC/VT. An implantable cardiac defibrillator was implanted in 8 patients. The mean follow-up period was 21.5±11 months. Two patients were deceased with incessant VT and heart failure, and one patient was deceased with multiorgan dysfunction after biventricular assist device implantation (n=3). Conclusion: Diagnosis of pediatric-onset ARVD might be much more difficult in children. Sudden cardiac death might be prevented in the early period by raising the awareness of physicians about the disorder. Prevention of sudden death with implantable cardiac defibrillators is crucial in the management of these patients. It should be kept in mind that children with structurally normal hearts may present with an earlier concealed phase and can be diagnosed with ARVD.
Pre-excitation cardiac problems in children: recognition and treatment

Abstract:The prevalence of ventricular pre-excitation is 0.07–0.2% in the pediatric population. Kent bundle is the most common atrioventricular accessory pathway and Mahaim fiber is relatively rare. Approximately, 30–60% of children with ventricular pre-excitation have onset of atrioventricular reentrant tachycardia. Persistent atrioventricular reentrant tachycardia can lead to tachycardiomyopathy. The anterograde conduction of right accessory pathway might lead to ventricular systolic dyssynchrony which might result in cardiac dysfunction even in patients with no tachycardia onset. This type of dilated cardiomyopathy was named as accessory pathway-induced dilated cardiomyopathy. Antiarrhythmic drugs can be used to acutely terminate tachycardia or taken orally to decrease tachycardia recurrence in the long term. However, antiarrhythmic drugs that can be chosen for children are quite limited. Sotalol has become a new choice. With the maturation of radiofrequency catheter ablation technique, progress in three-dimensional electro-anatomic mapping, use of cryoablation, and accumulation of experience in children with small age and weight, catheter ablation has become the first choice for children with pre-excitation syndrome. Conclusion : For ventricular pre-excitation co-exists with dilated cardiomyopathy, differential diagnosis of tachycardiomyopathy or accessory pathway-induced dilated cardiomyopathy should be considered. Catheter ablation (radiofrequency and cryoablation) is a relatively safe and effective treatment option and has become the first choice to treat children with ventricular pre-excitation. What is Known: • Persistent atrioventricular reentrant tachycardia in children can lead to tachycardiomyopathy; • Antiarrhythmic drugs that can be chosen for children are quite limited. What is New: • The anterograde conduction of right accessory pathway (not related to supraventricular tachycardia) might lead to accessory pathway-induced dilated cardiomyopathy. • Catheter ablation (including radiofrequency and cryoablation) has become the first choice for children with pre-excitation syndrome.

Abstract:Introduction The aim of this study was to describe our experience and outcome of ablation therapy for arrhythmias in pediatrics at a tertiary care center. Methods Data was collected retrospectively from the hospital medical records. All pediatrics presenting to AUBMC between 2000 and 2020 who underwent cardiac ablation were included. The data collected included type of arrhythmia, ablation technique, age and weight at ablation, procedure complications, medications used, and outcome assessment. Results We had 67 patients who underwent cardiac ablation. Of those, 60% were males with a mean age of 15 years. Structural heart disease was present in 6% of patients. Wolff‐Parkinson‐White syndrome (WPW) was most prevalent at 31%, followed by atrioventricular nodal reentrant tachycardia (AVNRT) at 24%, atrioventricular reentrant tachycardia (AVRT) at 16%, ventricular tachycardia (VT) at 10%, atrial fibrillation (AF) at 3%, and atrial tachycardia (AT) at 1%. The remaining 15% of patients presented with less common types of arrhythmias, including other supraventricular tachycardias (SVTs), retrograde dual atrioventricular nodal reentry, and premature ventricular contractions (PVC). Antiarrhythmic medications were started before the procedure in 59% of our population. Medication regimens postablation included beta‐blockers (68%), type 1c antiarrhythmics (25%), calcium channel blockers (3%), ivabradine (2%), and amiodarone (2%). The completed procedures showed a success rate of 93%. Conclusion Ablation of arrhythmias in pediatrics is an effective procedure in the treatment of childhood arrhythmias. More studies are needed on cardiac ablation in this age group and those with structural heart disease in the Middle East region.

The Incidence and Factors Associated with the Recurrence of Supraventricular Tachycardia in Children: 15 Years Experience from Middle-Income Country

Abstract:Limited data are available concerning supraventricular tachycardia (SVT) recurrence. Hence, this study aimed to determine the incidence, outcome, and factors associated with SVT recurrence. This retrospective, observational, population-based study was conducted among children with SVT from 2006 to 2020. The primary outcome measure was SVT recurrence. Kaplan Meier analysis was used to estimate SVT-free at 1, 5, and 10 years after diagnosis. Cox regression analysis was used to identify independent factors associated with recurrence. There were 156 patients with SVT with a median age at diagnosis of 1.9 years (Interquartile range [IQR] 11 days to 8.7 years) and follow-up for a median of 3.5 years (IQR 1.7 to 6.1 years). 35 patients (22%) had recurrent SVT at a median age of 7.8 years (IQR 4.4 to 12 years). Infants with Wolff-Parkinson-White Syndrome (WPW) had the highest recurrence (11/16, 68%), with 33% SVT-free at 5 years follow-up. The lowest recurrence rate was observed in neonatal diagnosis (2/54, 3.7%) with 98% SVT-free at 5 years follow-up. The independent factors associated with the recurrence of SVT were the diagnosis of WPW with an adjusted hazard ratio (aHR) of 5.2 (95% CI 2.4–11.2), age of more than 1 year at diagnosis (aHR 3.7 95% CI 1.4–9.7), and combine with or need second-line therapy (aHR 4.0 95% CI 1.5–10.7). One in five children with SVT experienced a recurrence, which is more likely for those with WPW, multiple maintenance therapy, and older age at first presentation. Whereas neonates with non-WPW may benefit from shorter maintenance therapy.

Abstract:Two brothers were suffering from perceptive hearing loss, atopic dermatitis and mild palmophantar keratoderma. There was a predisposition to atopic disease in the maternal family, and palmoplantar keratosis as a dominant trait in the paternal family. Atopoc dermatitis and palmoplantar keratoderma separately have been reported to co-exist with hearing loss, but a combination of all three has not so far been reported.
Surgical Treatment of Primary Cardiac Tumor Associated with Malignant Arrhythmias

Abstract:Objective: Pediatric primary cardiac tumor is an extremely rare disease. The tumor can extend into the conduction system and cause malignant arrhythmias. We retrospectively reviewed 6 consecutive cases of children with primary cardiac tumor that manifested as rhythm disturbance. Methods: In our center, 6 children were enrolled from October 2009 to August 2016. Detailed operative data and follow-up information were comprehensively collected and statistically analyzed. Results: The patients were ages 1 to 16 years and weighed 7.9 to 44.5 kg. Preoperative ventricular tachycardia was present in 3 patients, frequent ventricular ectopic beats in 1 patient, supraventricular tachycardia in 1 patient, and atrial flutter in 1 patient. All 6 patients underwent a complete tumor resection. The tumors were localized in the left ventricular free wall (3 patients), left ventricular outflow tract (1 patient), left atrium (1 patient), and right atrium (1 patient). One patient received 2 radiofrequency ablation procedures before tumor resection. Postoperative sick sinus syndrome occurred in 1 patient because the tumor infiltrated the sinoatrial node. Tumors from 2 patients were pathologically diagnosed as fibroma and 4 as rhabdomyoma. Reoperation of mitral valve repair was performed in 1 patient 1 year after tumor resection. The mean (± SD) follow-up time was 63.7 ± 31.4 months, and all children were well, with Ross functional classification I and no signs of recurrence or metastasis. Conclusions: In conclusion, cardiac tumor is a rare but nonneglectable reason for arrhythmia, and surgical resection is the optimal procedure, with satisfactory results.
