Abstract:Total health expenditure study has become the central topic of health system reform,macro health policy and health finance since 30 years health system reform.Total health expenditure study has evolved from blank to wealthy,from easy to difficult,and from edge to mainstream.The author retrospected the history development and basic traits of total health expenditure study at four stages,and introduced the current situation,research scope and main achievement of total health expenditure study.The author also proposed strategic topic and priority of total health expenditure study and expected to provide theory direction for health reform project design,public finance system and health finance system building,and public service government building.

Abstract:Background In the context of rapid economic and social development, there has been a continuous intensification of population aging, transformation of disease patterns, and wide application of new medical technologies. As a result, health expenditures in various countries have sharply soared. How to utilize limited medical resources to maximize the improvement of health levels has become a hot and challenging issue related to the well-being of all humanity. The relevant indicators of total health expenditure play a crucial role in monitoring and evaluating the fairness of health financing and health security in the region. Objective This study explores the changes in the main expenses that constitute China’s total health expenditure and uses indicators related to health expenditure to observe the changes and future development trends of China’s health expenditure. Based on this, the utilization of China’s health expenditure is monitored to identify possible problems, and thereby targeted suggestions for promoting the development of China’s health and wellness cause are put forward. Methods Based on the comparison of previous literature, this paper analyzes the changes and future development trends in China’s health expenditure by using the relevant indicators of China’s health expenditure through the structural variation analysis method and the gray prediction model. Results The results show that the scale of government, social, and out-of-pocket health expenditures has continuously expanded, with social health expenditures becoming the main funding source for total health expenditures. The burden of medical expenditures on individuals has been further reduced. In the institutional method of total health expenditures, hospital expenditures account for about 60% of the total and are the main component. The expenditures of health administration and medical insurance management institutions are the main driving force behind the growth of total health expenditures. However, the proportion of health expenditures in China’s GDP is relatively low, so more investment is needed in the healthcare sector, and the burden of individual medical expenses also needs to be continuously reduced. Discussion In the future, China should further increase its investment in the medical and health sector. Specifically, the government should persist in investing in fundamental medical and health services. Simultaneously, efforts should be made to establish a scientific cost control mechanism for pharmaceuticals and broaden financing channels for healthcare, such as accelerating the development of commercial health insurance.

Abstract:IntroductionChina has made great achievements in health insurance coverage and healthcare financing. Nonetheless, the rate of catastrophic health expenditure (CHE) in China was 13 percent in 2008, which is higher than in some other countries. There are differences among the provinces in China in terms of the lifestyles, customs, prevalent medical conditions, and health consciousness of their populations. This study aimed to compare the proportion of households with CHE and the factors influencing this expenditure between the Zhejiang and Qinghai province in China.MethodsData were derived from household surveys conducted in Zhejiang and Qinghai. Sampling was based on a multi-stage, stratified random cluster method. Households with CHE were defined as those with an out-of-pocket payment for health care that was at least 40 percent of the household income. Univariate and multivariate logistic regression analyses were used to identify the factors associated with CHE.ResultsA total of 1,598 households were included: 995 in Zhejiang and 603 in Qinghai. The average rates of CHE in Zhejiang and Qinghai were 10 percent and 31 percent, respectively. The economic status of a household influenced the likelihood of experiencing CHE; households headed by an employed person were less likely to experience CHE. In contrast, households that included outpatients or individuals with chronic diseases had a higher risk of experiencing CHE across the two provinces. Poorer or uninsured households in Zhejiang were more likely to experience CHE, as were households in Qinghai that included outpatients or were headed by a person from a minority nationality.ConclusionsThis study highlighted the importance of promoting economic development, expanding employment, and adjusting policies to better protect individuals with chronic diseases and outpatients from the risk of CHE. The Chinese government should pay more attention to actual conditions in different provinces to ensure that policy decisions incorporate local knowledge.

Abstract:A substantially growing health expenditure has become an important global issue. Thus, how and why health expenditure is rising should be urgently investigated in systematic research. The Bayesian space-time model and the Bayesian least absolute shrinkage and selection operator (LASSO) model were employed in this study to investigate the spatiotemporal trends and influence patterns of total health expenditure per capita (THEPC) and total health expenditure (THEE) as a share of the gross domestic product (GDP) on the Chinese mainland from 2009 to 2018. The spatial distribution of THEE as a share of GDP in mainland China has shaped a distinct geographical structure with the characteristic of ‘west high/east low’. Its local increasing trends formed a geographical structure that exhibited a ‘north high/south low’ feature. The heterogeneity of the influence patterns of health expenditure was observed from east to west across China. Natural environmental factors, such as air pollution and green coverage, along with changes in dietary structures, have increasingly influenced the growth of health expenditures.

Abstract:Objective To probe into the health expenditure and medical expenditure of Chinese people in rural and urban areas as objective references to improve the funding strategy of China's health care system. Methods Statistics annuals from 1999 to 2007 were called into play to analyze the status quo and trends of both expenditures. Results Definite proportion of funds raised for rural and urban health expenditure has grown by 200%;a distinct gap exists between the growth of per-capita medical expenditure in rural and urban residents (152. 86% in urban and 172. 77% in rural areas);medical expenditure maintains a steady growth along with rising per-capita income, yet the health consumption remains at a low level in general, especially among rural residents;since 2003, the income elasticity of the medical expenditure among rural residents keeps rising, narrowing the relative gap with urban residents. Conclusion It is recommended to push up the income of rural residents with greater resources, and to encourage the trend of marginal social medical expenditure. In addition, it is necessary to upgrade medical services, expand medical service coverage, and encourage reasonable consumption of medical services by both rural and urban residents;and to focus the role of cultural factors for greater awareness of the people in health investment and self-healthcare.

Abstract:By taking some special medical databases both in China and overseas as statistical sources,and using word frequency analysis, the paper contrasts and analyzes the research hotspots between China and overseas.The results show that the focus of health management abroad basically covers the connotation of health management,and the range of domestic academic study is limited to the framework of preventive and health care in the past,which also indirectly reflects that there remain some errors in the cognition of health management in China.

Abstract:Objective: To estimate the total expenditure of CT and MRI examination services and their proportions in the national total health expenditure in 2005 and 2009 and provide basis for the CT,MRI allocation and expenditure control.Methods: By sampling the secondary and tertiary hospitals in Shanghai,Shaanxi,Hunan and Zhejiang,the data of CT and MRI examination services of these areas were obtained in 2005 and 2009.Epidata 3.1 was used for data entry and Excel 2007 was used for statistical analysis.Results: The total expenditure of CT examination services in 2005 and 2009 was 12.34 billion and 24.90 billion and their proportions in the national total health expenditure were 1.43% and 1.45%.The total expenditure was 4.54 billion and 9.61 billion for MRI examination,with the proportions of 0.52% and 0.56% respectively.Conclusion: The total expenditure of CT and MRI examination services increased relatively fast from 2005 to 2009,but their proportions in the national total health expenditure almost remained the same,so it can not be concluded that the growth of total expenditure of CT and MRI examination services increased the financial burden of patients.

Abstract:BACKGROUND:China has made great achievements in health insurance coverage and healthcare financing; however, the rate of catastrophic health expenditure (CHE) was 13.0% in China in 2008, which is higher than that in some other countries. There remain some differences in life-style, national customs, medical conditions, and health consciousness in different provinces in China. This study aimed to compare the rates of households with CHE, further to explore the different performance of factors influencing CHE between Zhejiang and Qinghai province, China.METHODS:Data were derived from the household surveys conducted in Zhejiang and Qinghai. Sampling on multi-stage stratified cluster random method was adopted. Household with CHE occurs when the out-of-pocket payment for health care equals to or exceeds 40% of a household's income. Univariate and multivariate logistic regression analyses were used to identify the performance of factors of CHE.RESULTS:A total of 1598 households were included in this study, including 995 in Zhejiang and 603 in Qinghai. The average rates of CHE in Zhejiang and Qinghai were 9.6 and 30.5%, respectively. We found that economic status of households and households headed by an employed person are the protective factors for CHE; and number of members with chronic diseases and number of inpatients in household are the risk factors for CHE in the two provinces. Besides, poor/low-insured households in Zhejiang; and households having outpatients and households headed by a minority person in Qinghai are more likely to experience the risk of CHE.CONCLUSIONS:This study highlights the importance of improving economic development, expanding employment, and adjusting policies to make greater efforts to protect chronic diseases patients, outpatients, and inpatients, further to reduce the risk of CHE. The Chinese government should pay more attention to the actual conditions in different provinces, further to make policy decisions according to the local knowledge.

Abstract:Objective To identify the running state of community health service in Shaoxing county and find out the main cost and its structure by analyzing income and expenditure.Methods Questionnaires were distributed to 19 community health service centers,the contents included the basic situation,income and expenditure in recent 4 years.Results 94.74 percent of community health service centers in Shaoxing reached national standards,the main expenditure items were drug cost and labor cost,which occupied about 60% of the total.while the income mainly came from drug and treatment,which occupied about 88.17% of the total.However,the input from government only occupied 9.35%.About 36.84% of the centers suffered a loss in recent 4 yeas.Conclusion For community health service centers in Shaoxing county,drug cost takes up a big part in total income.The government should increase financial support to community health service.Training on general practitioners should be paid more attention to adapt the development of community health service.

Abstract:Unreasonable growth of total health expenditure at various extent has been found to be prevalent among many countries for a long time. Thus how to deal with this global public health challenge has become a hot topic among the consumers, providers, and payers alike. Echoing the global trend of value-based healthcare, value-based management of total health expenditure could be a direction of cost containment in the future. Abstract:To get a clear sense of the “appropriate” individual payout, based on the literature reviews and the World Health Organization (WHO) reports, the study analyzes the data from two aspects: 191 WHO member states and 76 superior states (the health performance is better than that of China) by using the Panel data model, which was further validated by intention questionnaire. The results show that Chinese-appropriate individual payout intervals are respectively around 30% from the WHO angle and the superior state angle in 2020, which have been well proved by the survey of intention. For the realization of the feasible structure of total expenditure on health, we need not only the efforts from government, society and individuals but also the cooperation among hospitals, drug supply security system, health care systems and other health systems.

Abstract:For the past 30 years, the increase of total expenditures on health in some nations and areas has severely stressed national health service system (NHSS). Solutions require understanding of the characteristics of public health and basic health care (PH & BHC). This article shows that the total expenditures on PH & BHC for a perfect health service system (PHSS) will increase with increasing gross domestic product (GDP) but that the growth rate of the total expenditures on PH & BHC will be less than the GDP increases. The healthcare cost increases will not result in unsustainable costs for a nation or area nor result in disastrous health care. Therefore, NHSS should build the representative PHSS with the enjoying health care divided from PH & BHC.

Abstract:This paper investigates the determinants of household health care expenditures in rural China. The empirical results given by rural household data show that large families and old members tend to increase household health care expenditures, while households with the head experiencing village cadre will dramatically reduce the health care expenditures. Hospitalization and medical care insurance can release households’ demand for health care and result in remarkable higher health care expenditures.

Abstract:Objective To provide a reference for the research on improving eHealth Literacy of different stages of population in china, by analyzing and comparing the research hotspots of eHealth Literacy at domestic and international. Methods The CNKI and Web of Science core collection databases were selected to retrieve the literature published from January 1999 to December 2021, the total 698 literatures were analysed by Citespace visual analysis. Results In this study. The number of foreign publications was generally on the rise, research hotspots of eHealth Literacy were theoretical research, influencing factors, access channels, and access capabilities, as well as eHealth assessment and application, health interventions for chronic diseases, public health care, etc. However, the number of articles published in China is still in its initial stage, showing a slow upward trend. Domestic research institutions lacked cooperation, while the research of eHealth Literacy focused on theoretical models, influencing factors, measurement and application, and health interventions, and other aspects. Conclusion The research on e-health literacy in Chima and abroad is in its infancy, and more exploration and development will be needed in the future.
