Abstract:Driver drowsiness contributes to 10-20% of motor vehicle crashes. To reduce crash risk, ideally drivers would be aware of the drowsy state and cease driving. The extent to which drivers can accurately identify sleepiness remains under much debate. We systematically examined whether individuals are aware of sleepiness while driving, and whether this accurately reflects driving impairment, using meta-analyses and narrative review. Within this scope, there is high variability in measures of subjective sleepiness, driving performance and physiologically-derived drowsiness, and statistical analyses. Thirty-four simulated/naturalistic driving studies were reviewed. To summarise, drivers were aware of sleepiness, and this was associated to physiological drowsiness and driving impairment, such that high levels of sleepiness significantly predicted crash events and lane deviations. Subjective sleepiness was more strongly correlated (i) with physiological drowsiness compared to driving outcomes; (ii) under simulated driving conditions compared to naturalistic drives; and (iii) when examined using the Karolinska sleepiness scale (KSS) compared to other measures. Gaps remain in relation to how age, sex, and varying degrees of sleep loss may influence this association. This review provides evidence that drivers are aware of drowsiness while driving, and stopping driving when feeling 'sleepy' may significantly reduce crash risk.
On-road driving impairment following sleep deprivation differs according to age

Abstract:Impaired driving performance due to sleep loss is a major contributor to motor-vehicle crashes, fatalities, and serious injuries. As on-road, fully-instrumented studies of drowsy driving have largely focused on young drivers, we examined the impact of sleep loss on driving performance and physiological drowsiness in both younger and older drivers of working age. Sixteen 'younger' adults (M = 24.3 ± 3.1 years [21-33 years], 9 males) and seventeen 'older' adults (M = 57.3 ± 5.2, [50-65 years], 9 males) undertook two 2 h drives on a closed-loop track in an instrumented vehicle with a qualified instructor following (i) 8 h sleep opportunity the night prior (well-rested), and (ii) after 29-h of total sleep deprivation (TSD). Following TSD, both age groups displayed increased subjective sleepiness and lane departures (p < 0.05), with younger drivers exhibiting 7.37 × more lane departures, and 11 × greater risk of near crash events following sleep loss. While older drivers exhibited a 3.5 × more lane departures following sleep loss (p = 0.008), they did not have a significant increase in near-crash events (3/34 drives). Compared to older adults, younger adults had 3.1 × more lane departures (p = < 0.001), and more near crash events (79% versus 21%, p = 0.007). Ocular measures of drowsiness, including blink duration, number of long eye closures and PERCLOS increased following sleep loss for younger adults only (p < 0.05). These results suggest that for older working-aged adults, driving impairments observed following sleep loss may not be due to falling asleep. Future work should examine whether this is attributed to other consequences of sleep loss, such as inattention or distraction from the road.

Abstract:Hepatic tumors unassociated with cirrhosis were encountered in seven patients aged 10 to 19 years. Four patients had received androgens for aplastic anemia. Two patients had transfusional hemosiderosis. One patient had had a renal transplant 2 1/2 years ago. Two patients are alive at 2 3/4 and 2 1/2 years after surgical resections. Nodules were found at autopsy in the others. The tumors were well differentiated and, in the androgen-related cases, differed from the others in the following features: canalicular bile retention, mild nuclear atypia, and acinar formation. No mitoses, vascular invasion, or metastatic tumor were evident. The clinical setting was variable; different factors, including iron overload and androgen therapy, played a role in the development of tumor. Although the androgen-related cases showed mild cellular atypia, biologic evidence of malignancy was lacking as in most previous reports.
0166 Population-Based Estimates of Drowsy Driving Among US Teens: A National Sleep Foundation Study

Abstract:Abstract Introduction Drowsy driving represents a large public health concern, accounting for a significant proportion of motor vehicle crashes. Among US teenagers, motor vehicle crashes are the second leading cause of death. However, a dearth of research has documented the prevalence and frequency of drowsy driving in teens, as well as beliefs surrounding drowsy driving. The overarching goal of the present study was to document the prevalence, frequency, and perceptions of drowsy driving among teens. Methods We conducted a nationally-representative, probability-based survey of 1,124 US individuals aged 13 to 17 years to assess drowsy driving prevalence, frequency, and beliefs. Results have an estimated margin of error of ~4%. Survey respondents reported whether they have ever driven while so tired they had a hard time keeping their eyes open, how often they did so, what kept them from getting the sleep needed to drive alert, and perceived risks associated with drowsy driving. Measures of central tendency and dispersion were used to characterize responses. Results Approximately one in six teen drivers reported having driven drowsy. Projected to all US teen drivers, over 400,000 drive drowsy at least once per week. Teen drivers who work for pay were more than twice as likely to have reported drowsy driving than teens who didn’t work for pay. When asked about what prevented them from getting the sleep they needed to drive alert, the majority of teens pointed to work or school schedules. When asked about the risks associated with drowsy driving, 95% of teens said drowsy driving is extremely or very risky. However, when asked about the likelihood of drunk, drugged, distracted, and drowsy driving leading to death or serious injury, drowsy driving was seen as having the lowest risk of death or serious harm. Conclusion Due to a confluence of factors, including driving as a newly learned behavior and multiple competing time demands, teenagers find themselves at increased risks for drowsy driving and drowsy driving-related consequences. We found that approximately 1.7 million teenage drivers had driven while so tired they had a hard time keeping their eyes open. Sleepiness among night-time drivers: relationship to blood alcohol concentration and other factors

Abstract:Objective: The study aimed to explore the distribution and correlates of subjective sleepiness among the general night-time driving population. Methods: The survey took place in three British Columbia communities in June 2003 between 21:00 hours to 03:00 hours. Sites and vehicles were selected randomly. Surveyors obtained information on several demographic and situational variables including self-assessed degree of sleepiness and self-reported hours asleep and awake, as well as an objective measure of blood alcohol concentration obtained from a hand-held breath-testing device. Results: The total compliance rate among intercepted drivers was 85%. Among the 2335 drivers responding to the questionnaire, 68.4% indicated that they were wide awake, 27.6% were somewhat sleepy, and 4.1% were very sleepy. Logistic regression quantified the independent contributions of the various factors to subjective sleepiness. Male drivers with positive blood alcohol concentrations under 50 mg% were more likely to report feeling sleepy than those with either higher or with zero blood alcohol concentration. Greater relative risk of sleepiness was also associated with being female, being under age 55, and advanced hour of night. Driving with passengers of the same gender was associated with lower reported sleepiness. Conclusions: A substantial proportion of night-time drivers are driving while sleepy, especially at late night and early morning hours. The combination of alcohol and sleepiness compounds impairment in experimental studies and deserves greater attention in crash risk studies and as a topic for public education and awareness.

Abstract:Background: There is concern about the detrimental effects of shift-workers' increasing working hours particularly when driving sleep deprived. The approach to measuring the magnitude of driving impairment caused by sleep deprivation was by comparing it to alcohol. The study compared driving performance after 24-h of wakefulness to performance with a BrAC of just over 22 μg/100mls of breath which is equal to 50 mg of alcohol per 100mls of blood (Scottish drink-drive limit). The effectiveness of coffee as a countermeasure for driver fatigue and the association between subjective impairment and actual performance was also investigated. Methods: A study of 30 participants (11 male and 19 female; mean age 21) was conducted. Subjects were tested under three conditions: fully rested, sleep deprived, and alcohol intoxicated - BrAC mean [SD] 25.95 μg [2.78]. Under each condition, subjects were tested before and after coffee ingestion. This involved driving simulation (Lane Change Task and Reaction Test) and subjective Likert scales (Karolinska Sleepiness Scale and driver impairment scale). Outcome measures included lane tracking adaptive mean deviation, reaction time, and subjective sleepiness and impairment ratings. Results: Compared to alcohol, sleep deprived mean reaction times were slower (2.86 s vs. 2.34 s) and lateral control of the vehicle was reduced (lane tracking adaptive mean deviation: 0.5 vs. 0.3). Coffee did not produce an improvement when sleep deprived, and instead, performance deteriorated. Females were less impaired following sleep deprivation than males. Following prolonged wakefulness, the correlation between subjective impairment and actual performance was significant. Conclusions: It was concluded that sleep deprivation has a greater impact on driving performance than a BrAC of 22 μg/100mls of breath, as measured by driving simulation. Coffee is not an effective countermeasure for sleep deprived driving and drivers' ability to judge this impairment is suggested to be limited.
High risk of near-crash driving events following night-shift work

Abstract:Night-shift workers are at high risk of drowsiness-related motor vehicle crashes as a result of circadian disruption and sleep restriction. However, the impact of actual night-shift work on measures of drowsiness and driving performance while operating a real motor vehicle remains unknown. Sixteen night-shift workers completed two 2-h daytime driving sessions on a closed driving track at the Liberty Mutual Research Institute for Safety: (i) a postsleep baseline driving session after an average of 7.6 ± 2.4 h sleep the previous night with no night-shift work, and (ii) a postnight-shift driving session following night-shift work. Physiological measures of drowsiness were collected, including infrared reflectance oculography, electroencephalography, and electrooculography. Driving performance measures included lane excursions, near-crash events, and drives terminated because of failure to maintain control of the vehicle. Eleven near-crashes occurred in 6 of 16 postnight-shift drives (37.5%), and 7 of 16 postnight-shift drives (43.8%) were terminated early for safety reasons, compared with zero near-crashes or early drive terminations during 16 postsleep drives (Fishers exact: P = 0.0088 and P = 0.0034, respectively). Participants had a significantly higher rate of lane excursions, average Johns Drowsiness Scale, blink duration, and number of slow eye movements during postnight-shift drives compared with postsleep drives (3.09/min vs. 1.49/min; 1.71 vs. 0.97; 125 ms vs. 100 ms; 35.8 vs. 19.1; respectively, P < 0.05 for all). Night-shift work increases driver drowsiness, degrading driving performance and increasing the risk of near-crash drive events. With more than 9.5 million Americans working overnight or rotating shifts and one-third of United States commutes exceeding 30 min, these results have implications for traffic and occupational safety.
The effects of sleep loss on young drivers’ performance: A systematic review

Abstract:Young drivers (18-24 years) are over-represented in sleep-related crashes (comprising one in five fatal crashes in developed countries) primarily due to decreased sleep opportunity, lower tolerance for sleep loss, and ongoing maturation of brain areas associated with driving-related decision making. Impaired driving performance is the proximal reason for most car crashes. There is still a limited body of evidence examining the effects of sleep loss on young drivers' performance, with discrepancies in the methodologies used, and in the definition of outcomes. This study aimed to identify the direction and magnitude of the effects of sleep loss on young drivers' performance, and to appraise the quality of current evidence via a systematic review. Based on the Preferred Reporting Items for Systematic Reviews and Meta- Analyses (PRISMA) approach, 16 eligible studies were selected for review, and their findings summarised. Next, critical elements of these studies were identified, and the Grading of Recommendations Assessment, Development and Evaluation (GRADE) guidelines augmented to rate those elements. Using those criteria, the quality of individual papers was calculated and the overall body of evidence for each driving outcome were assigned a quality ranking (from 'very low' to 'high-quality'). Two metrics, the standard deviation of lateral position and number of line crossings, were commonly reported outcomes (although in an overall 'low-quality' body of evidence), with significant impairments after sleep loss identified in 50% of studies. While speed-related outcomes and crash events (also with very low- quality evidence) both increased under chronic sleep loss, discrepant findings were reported under conditions of acute total sleep deprivation. It is crucial to obtain more reliable data about the effects of sleep loss on young drivers' performance by using higher quality experimental designs, adopting common protocols, and the use of consistent metrics and reporting of findings based on GRADE criteria and the PRISMA statement. Magnetic Resonance Imaging of Hematomas in a 0.02 T Magnetic Field

Abstract:One intramuscular calf hematoma, 2 ankle hematomas and 4 cephalhematomas were imaged at various ages in a low magnetic field (0.02 T). At least one spin echo (SE) multislice image and a series of inversion recovery images (IR) were made varying the inversion time for estimation of the relaxation time T1. T1 tended to shorten and T2 to stay unchanged. With an unsuitable pulse sequence the hematomas were not visible. They were best seen with short TIs. The images of one of the ankle hematomas and the calf hematoma were compared with sonographic findings. The appearances of the hematomas varied during aging with both imaging modalities. The hematomas were easier to detect with magnetic resonance imaging than with ultrasound, also when aging.
Awareness of sleepiness and ability to predict sleep onset: can drivers avoid falling asleep at the wheel?

Abstract:Objectives: Regarding the causes of sleep-related accidents, this study assesses whether individuals can anticipate sleep onset accurately and how individuals acknowledge and use physiological and cognitive cues to make judgments related to sleep onset. Methods: A group of 41 partially sleep-deprived subjects predicted the likelihood of sleep in 30 consecutive two-minute intervals and noted physiological and cognitive signs of sleepiness, including involuntary eye closure, head-nodding, wandering thoughts, yawns, and instances of sleep, collectively referred to as "sleep complaints". Continuous polysomnographic recording compared these predictions to actual instances of sleep. Results: Subjects varied in their ability to predict sleep onset. For all subjects, the mean prediction of the likelihood of sleep prior to sleep was significantly higher than the mean prediction of the likelihood of sleep prior to intervals in which no sleep occurred (78% vs. 42%; p<.05). However, subjects tended to predict much lower likelihoods of sleep onset before their first sleep event (55%) than before later sleep events. On average, the rate at which subjects reported miscellaneous sleep complaints (such as head-nodding, eye closure, and wandering thoughts) was higher prior to sleep than prior to intervals in which sleep did not occur. Conclusion: Subjects who acknowledged a limited number of physiological and cognitive indicators of sleepiness tended to be poor predictors. Subjects whose physiological and cognitive signs of sleepiness failed to provide a strong indication of whether or not sleep onset would occur also tended to be poor predictors. Inability to judge sleep onset and, hence, susceptibility to sleep-related accidents, may be attributable to both a scarcity of meaningful warning signs and a failure to acknowledge the importance of physiological and cognitive indicators.
The impact of sleep disorders on driving safety—findings from the Second Strategic Highway Research Program naturalistic driving study

Abstract:Study Objectives: This study investigated the association between driving safety and seven sleep disorders amongst 3541 participants of the Second Strategic Highway Research Program (SHRP 2) naturalistic driving study.Methods: SHRP 2 collected naturalistic driving data from participants between 16 and 98 years old by instrumenting participants' vehicles. The analyses used logistic regression to determine the likelihood of crash or near-crash involvement, Poisson log-linear regression to assess crash or near-crash rate, and ordinal logistic regression to assess driver maneuver appropriateness and crash or near-crash severity. These analyses did not account for any medical treatments for the sleep disorders.Results: Females with restless legs syndrome/Willis-Ekbom disease (RLS/WED), drivers with insomnia or narcolepsy, are associated with significantly higher risk of crash or near-crash. Drivers with shift work sleep disorder (SWSD) are associated with significantly increased crash or near-crash rate. Females with RLS/WED or sleep apnea and drivers with SWSD are associated with less safe driver maneuver and drivers with periodic limb movement disorder are associated with more severe events. The four analyses provide no evidence of safety decrements associated with migraine.Conclusions: This study is the first examination on the association between seven sleep disorders and different measures of driving risk using large-scale naturalistic driving study data. The results corroborate much of the existing simulator and epidemiological research related to sleep-disorder patients and their driving safety, but add ecological validity to those findings. These results contribute to the empirical basis for medical professionals, policy makers, and employers in making decisions to aid individuals with sleep disorders in balancing safety and personal mobility.

Abstract:Two different approaches for using a sequence of packets of the IEEE 802.11 (WLAN) protocol as cover for a stenographic communication can be found in literature: in 2003 Krzysztof Szczypiorski introduced a method constructing a hidden channel using deliberately corrupted WLAN packets for communication. In 2006 Kraetzer et al. introduced a WLAN stenography approach that works without generating corrupted network packets. This later approach, with hidden storage channel scenario (SCI) and the timing channel based scenario (SCII), is reconsidered here. Fixed parameter settings limiting SCIs capabilities in the implementation (already introduced in 2006) motivated an enhancement. The new implementation of SCI increases the capacity, while at the same time improving the reliability and decreasing the detectability in comparison to the work described in 2006. The timing channel based approach SCII from 2006 is in this paper substituted by a completely new design based on the usage of WLAN Access Point addresses for the synchronization and payload transmission. This new design now allows a comprehensive practical evaluation of the implementation and the evaluations of the scheme, which was not possible with the original SCII before. The test results for both enhanced approaches are summarised and compared in terms of detectability, capacity and reliability.

Abstract:Abstract Introduction Sleep dysfunction increases the risk of mild cognitive impairment (MCI) and Alzheimer’s Disease (AD). Emerging sensor technologies can index real world (RW) sleep and instrumental activities of daily living affected by neurodegeneration, as outlined below. Methods 85 legally licensed active drivers with cognitive decline (76 MCI, 9 mild AD) (mean age = 75.6 years; 37 females) and 54 age and education-matched controls (mean age= 74.7 years; 36 females) participated. Cognitive status was assessed using standardized neuropsychological tests according to NIA-AA guidelines. Sensor systems installed in participants’ own vehicles recorded driving incidence (driving or not each day). Sleep was recorded using wrist-worn actigraphy, and verified with self-reported sleep diaries over three continuous months. A mixed-effect logistic regression analyzed daily driving incidence with predictors including z-scores of total sleep time (TST) and sleep efficiency (SE) in previous night, and cognitive status (MCI or AD v. controls), adjusted for age, gender, employment status, and driving season. Results From 9,214 days of data on nightly sleep and next day driving, increase in TST significantly reduced driving incidence (21% per standard deviation [SD]; Odds Ratio [OR] = 0.79, 95% Confidence Interval [CI] = 0.74 – 0.84, p < 0.001). Increase in SE was associated with increased driving incidence among those with cognitive decline, compared to controls (15% per SD; OR = 1.14, 95% CI = 1.03 – 1.27, p = 0.01). Conclusion Findings underscore the value of RW digital biomarkers to track the effects of neurodegeneration on sleep and instrumental activities of daily living. Deciding to drive increased with greater sleep efficiency, particularly for those with cognitive decline. Quantitative profiles of sleep and driving behavior offer novel opportunities for early detection of the effects of AD. Abstract:The interaction of heat shock protein with molecular mass 25 kDa (HSP25) and its point mutants S77D + S81D (2D mutant) and S15D + S77D + S81D (3D mutant) with intact and thermally denatured actin was analyzed by means of fluorescence spectroscopy and ultracentrifugation. Wild type HSP25 did not affect the polymerization of intact actin. The HSP25 3D mutant decreased the initial rate without affecting the maximal extent of intact actin polymerization. G-actin heated at 40-45 degrees C was partially denatured, but retained its ability to polymerize. The wild type HSP25 did not affect polymerization of this partially denatured actin. The 3D mutant of HSP25 increased the initial rate of polymerization of partially denatured actin. Heating at more than 55 degrees C induced complete denaturation of G-actin. Completely denatured G-actin cannot polymerize, but it aggregates at increased ionic strength. HSP25 and especially its 2D and 3D mutants effectively prevent salt-induced aggregation of completely denatured actin. It is concluded that the interaction of HSP25 with actin depends on the state of both actin and HSP25. HSP25 predominantly acts as a chaperone and preferentially interacts with thermally unfolded actin, preventing the formation of insoluble aggregates.

Abstract:Chronic sleep deficiency, defined as a state of inadequate or mistimed sleep, is a growing and underappreciated determinant of health status. Sleep deprivation contributes to a number of molecular, immune, and neural changes that play a role in disease development, independent of primary sleep disorders. These changes in biological processes in response to chronic sleep deficiency may serve as etiological factors for the development and exacerbation of cardiovascular and metabolic diseases and, ultimately, a shortened lifespan. Sleep deprivation also results in significant impairments in cognitive and motor performance which increase the risk of motor vehicle crashes and work-related injuries and fatal accidents. The American Academy of Sleep Medicine and the Sleep Research Society have developed this statement to communicate to national health stakeholders the current knowledge which ties sufficient sleep and circadian alignment in adults to health.

Abstract:Sleep deprivation represents a major public health issue, and increasing the proportion of the population that achieves adequate sleep has been identified as a national health priority. Despite this, insufficient habitual sleep duration is still highly prevalent, with approximately a third of US adults achieving less than 6 h of sleep on average. Habitual short sleep duration cuts across all segments of the population and is associated with weight gain and obesity, heart disease, diabetes, poor cognitive performance, stress, and other adverse outcomes, including mortality. This chapter begins by conceptualizing the effects of sleep in the context of society. This is done largely through a social–ecological framework. Then, this chapter aims to summarize the health effects of sleep deprivation across dimensions of cardiometabolic disease risk, neurocognitive function, and psychological well-being. Then, the chapter will further explore these effects across age, race/ethnicity, gender/sex, relationship status, and occupation. A section will devote specific attention to the role of sleep deprivation in driving accidents. Finally, the chapter ends with a summary and conclusions, with recommendations for future directions.

Abstract:Purpose of reviewThis article discusses the methods available to evaluate chest wall mechanics and the relationship between intraabdominal pressure (IAP) and chest wall mechanics during general anaesthesia in normal and obese individuals, as well as in acute lung injury/acute respiratory distress syndrome. Recent findingsThe interactions between the abdominal and thoracic compartments pose a specific challenge for intensive care physicians. IAP affects respiratory system, lung and chest wall elastance in an unpredictable way. Thus, transpulmonary pressure should be measured if IAP is more than 12 mmHg or if chest wall elastance is compromised for other reasons, even though the absolute values of pleural and transpulmonary pressures are not easily obtained at bedside. We suggest defining intraabdominal hypertension (IAH) as IAP at least 20 mmHg and abdominal compartment syndrome (ACS) as IAP at least 20 mmHg associated with failure of one or more organs, although further studies are required to confirm this hypothesis. Additionally, in the presence of IAH, controlled mechanical ventilation should be applied and positive end-expiratory pressure individually titrated. Prophylactic open abdomen should be considered in the presence of ACS. SummaryIncreased IAP markedly affects respiratory function and complicates patient management. Frequent assessment of IAP is recommended.
Positive Correlation Between Drowsiness and Prefrontal Activation During A Simulated Speed-Control Driving Task

Abstract:The present study aimed to examine the relationship between drowsiness and prefrontal activation during simulated driving tasks using a wireless portable near-infrared spectroscopy device. Participants drove from start to goal along default routes with either intentional control of their driving speed (speed-control group) or not (speed-free group). Drowsiness level was assessed using a five-item Likert-type questionnaire. The behavioral data indicated longer driving time in the speed-control group than in the speed-free group, whereas no difference in the number of errors was found between the two groups. Importantly, the speed-control group showed a significant positive correlation between the drowsiness score and left prefrontal activation, whereas the speed-free group did not. The results suggest that drowsy individuals may show increased prefrontal activation as compensatory efforts to maintain the desired level of performance in tasks that require deliberate control of behaviors. Furthermore, the present study shows that near-infrared spectroscopy may provide us with a possibility to examine the state of drowsiness during daily-life operations.

Abstract:Cumulative sleep deprivation may increase the risk of psychiatric disorders, other disorders, and accidents. We examined the effect of insufficient sleep on cognitive function, driving performance, and cerebral blood flow in 19 healthy adults (mean age 29.2 years). All participants were in bed for 8h (sufficient sleep), and for <4h (insufficient sleep). The oxyhaemoglobin (oxyHb) level by a word fluency task was measured with a near-infrared spectroscopy recorder on the morning following sufficient and insufficient sleep periods. Wisconsin card sorting test, continuous performance test, N-back test, and driving performance were evaluated on the same days. The peak oxyHb level was significantly lower, in the left and right frontal lobes after insufficient sleep than after sufficient sleep (left: 0.25+/-0.13 vs. 0.74+/-0.33 mmol, P<0.001; right: 0.25+/-0.09 vs. 0.69+/-0.44 mmol, P<0.01). The percentage of correct responses on CPT after insufficient sleep was significantly lower than that after sufficient sleep (96.1+/-4.5 vs. 86.6+/-9.8%, P<0.05). The brake reaction time in a harsh-braking test was significantly longer after insufficient sleep than after sufficient sleep (546.2+/-23.0 vs. 478.0+/-51.2 ms, P<0.05). Whereas there were no significant correlations between decrease in oxyHb and the changes of cognitive function or driving performance between insufficient sleep and sufficient sleep. One night of insufficient sleep affects daytime cognitive function and driving performance and this was accompanied by the changes of cortical oxygenation response.
