Abstract:Objective: To analyze the effectiveness and safety of corneal relaxing incisions (CRI) in correcting keratometric astigmatism during cataract surgery. Methods: A prospective study of two groups: control group and treatment group. A treatment group included 25 eyes of 25 patients who had combined clear corneal phacoemulsification, IOL implantation and CRI. A control group included 25 eyes of 25 patients who had clear corneal phacoemulsification and IOL implantation. Postoperative keratometric astigmatism was measured at 1 week, 1 month., 3 months and 6 months. Results: CRI significantly decreased keratometric astigmatism in patients with preexisting astigmatism compared with astigmatic changes in the control group. In eyes with CRI, the mean keratometric astigmatism was 0.29±0.17 D (range 0 to 0.5 D) at 1 week, 0.41±0.21 D (range 0 to 0.82 D) at 1 month, respectively reduced by 2.42 D and 2.30 D at 1 week and 1 month postoperatively (P=0.000,P=0.000) and postoperative astigmatism was stable until 6 months follow-up. The keratometric astigmatism of all patients decreased to less than 1.00 D postoperatively. Conclusions: CRI is a practical, simple, safe and effective method to reduce preexisting astigmatism during cataract surgery. A modified nomogram is proposed. The long-term effect of CRI should be investigated.
Visual Outcomes Early after Implantable Collamer Lens V4c Implantation in Different Preoperative Spectacle Correction: Full Correction Vs. under Correction

Abstract:PurposeTo investigate visual outcomes early after implantable collamer lens (ICL) V4c implantation between patients with fully corrected and under-corrected spectacles preoperatively.MethodsPatients who implanted ICL V4c were divided into the full correction (46 eyes/23 patients) and under-correction groups (48 eyes/24 patients) based on preoperative differences between the spherical diopter of the spectacles and the actual spherical diopter. Refractive outcomes, scotopic pupil size, higher-order aberrations, and subjective visual outcomes as assessed using a validated questionnaire were compared between the two groups 3 months postoperatively. Moreover, the relationships between the severity of haloes and postoperative ocular or ICL parameters were analyzed.ResultsAt the 3-month follow-up, the efficacy indices in the full correction and under-correction groups were 0.99 +/- 0.12 and 1.00 +/- 0.10, respectively; the safety indices were 1.15 +/- 0.16 and 1.15 +/- 0.15, respectively. Total-eye spherical aberration (p < 0.0001) and internal spherical aberration (p = 0.0005) were significantly different pre- and post-operatively in the under-correction group, while no differences were found in the full correction group. Total-eye spherical aberration (p = 0.002) and the severity of haloes (p = 0.03) were postoperatively different between the two groups. The severity of haloes was associated with postoperative spherical aberration (total-eye spherical aberration: r = -0.32, p = 0.002; internal spherical aberration: r = -0.24, p = 0.02).ConclusionGood efficacy, safety, predictability, and stability were obtained early after surgery regardless of preoperative spectacle correction. Patients in the under-correction group possessed a shift to negative spherical aberration and reported greater severity of haloes at the 3-month follow-up. Haloes were the most common visual symptoms after ICL V4c implantation and the severity of them was correlated with postoperative spherical aberration.
[The role of cholecystokinin in the regulation of intestinal blood circulation and metabolism].

Abstract:These studies were conducted to assess whether cholecystokinin (CCK) play a role in the regulation of intestinal blood flow and metabolic activity. Anesthetized dogs were subjected to laparotomy and implantation of transducers to measured intestinal blood flow (MBF), intestinal oxygen extraction (AVO2) and intraluminal pressure (IP). Intestinal oxygen consumption (VO2) was calculated as the product of the AVO2 and MBF. CCK8 infused intraarterially in doses 12.5, 25, 50, 75 and 100 pmol/kg-h increased significantly MBF by 5, 8, 11, 15 and 19%. IP increased significantly with the last three doses of CCK by 55, 87 and 145%, respectively. Glucose and oleic acid increased MBF by 12 and 37% and VO2 by 26 and 17%, respectively. Pretreatment with a highly selective CCK-receptor antagonist L-364 718 (1 mg/kg i.v.) abolished the vascular and motor responses to exogenous CCK and significantly reduced intestinal hyperemia and increase in VO2 in response to luminal oleic acid but not glucose. These findings indicate that endogenous CCK may play a role in the intestinal hyperemia after fatty meal and that this effect is primarily mediated by the CCK receptors.
Spherical implantable collamer lenses for myopia and hyperopia: 126 eyes with 1-year follow up

Abstract:Background: To report on 1-year follow up of a series of 126 eyes implanted with a spherical implantable collamer lens (ICL). Methods: Retrospective case note review of 126 eyes (68 patients) undergoing ICL placement by a single surgeon (TG) to correct preoperative mean spherical equivalents between +8.88 and -20.50 D. Main outcome measures included uncorrected visual acuity (UCVA), refraction, best spectacle-corrected VA, unaided binocular vision, adverse events, operative/postoperative complications and patient symptoms. Results: Complete data were available at 1 year on 121 of the 126 eyes (65 out of 68 patients). At 12 months 86% (104/121) had postoperative UCVA better than or equal to preoperative best spectacle-corrected VA, and 98% (118/121) had UCVA within one line of preoperative best spectacle-corrected VA. Ninety-six per cent (116/121) had postoperative UCVA of 6/9 or better, and 100% had postoperative best-corrected visual acuity of 6/12 or better. Ninety-eight per cent (64/65) patients had unaided binocular vision at 1 year of 6/7.5 or better. Average myopia treated was -8.83 D (116 patients), and average hyperopia +7.14 D (10 patients). Two ICLs were replaced because of high vaulting; one eye had increased intraocular pressure that resolved with smaller ICL placement. Four patients had visually insignificant pigment deposits on the ICL at 3/12, but no pigmentary glaucoma. Five patients noticed haloes around lights at night, none preventing driving. Conclusions: One-year results from this study, the largest reported clinical investigation of the use of ICLs in New Zealand, support the safety, efficacy and predictability of ICL to treat both hyperopic and myopic spherical refractive errors.
The Long-term Visual Quality and Rotational Stability after ICL/TICL V4c Implantation in Individuals with High Myopia Older Than 40 Years.

Abstract:PURPOSE: To investigate the long-term visual quality and rotational stability after the implantation of Implantable Collamer Lens (ICL) and toric ICL (TICL) (STAAR Surgical) in patients with myopia older than 40 years. METHODS: This study included 82 eyes of 41 patients older than 40 years with myopia who underwent ICL/TICL V4c implantation. The refraction sphere, refraction cylinder, spherical equivalent (SE), uncorrected and corrected distance visual acuity, and anterior segmental parameters were measured preoperatively and at the 1 -month, 3 -month, and last followup visits at 33 to 58 months postoperatively (mean follow-up: 42.56 +/- 7.17 months). Wavefront aberrations and TICL rotation were measured using OPD-Scan III (Nidek Co Ltd) at the last follow-up visit. RESULTS: At the last follow-up visit, the overall safety and efficacy index were 1.22 +/- 0.26 and 0.88 +/- 0.34, respectively, without significant differences between the ICL and TICL groups. Postoperative refraction cylinder was -0.95 +/- 0.64 and -0.71 +/- 0.54 diopters in the ICL and TICL groups, respectively. The average vault was 467.44 +/- 231.98 mu m. The average TICL rotation was 5.45 +/- 6.61 degrees, positively correlated with the preoperative anterior chamber volume ( R 2 = 0.1118, P = .026) and clockwise TICL alignment degree ( R 2 = 0.3110, P = .007) and negatively correlated with the 1 -month vault ( R 2 = 0.1218, P = .008). There were no significant differences in the total, corneal, or internal aberrations and modulation transfer function AreaRatio between the ICL and TICL groups. CONCLUSIONS: Both ICL and TICL presented satisfactory long-term safety, efficacy, and visual quality in patients older than 40 years. Postoperative TICL spontaneous rotation was within the manageable range in the long term.

Abstract:Abstract Purpose To analyze the factors related to corrected distance vision acuity (CDVA) after implantable collamer lens (ICL) implantation in patients with preoperative suboptimal CDVA. Methods This retrospective study included patients with suboptimal preoperative CDVA (CDVA ≤ 20/25) who underwent ICL implantation (V4 or V4c). Preoperative and postoperative clinical evaluations included CDVA, uncorrected distance visual acuity (UDVA), and refraction. Results A total of 1212 eyes from 731 patients were identified. CDVA increased in 90.8% of the eyes after surgery. Among them, 57.5% of the eyes (697 eyes) gained more than one line. The preoperative LogMAR CDVA was 0.32 ± 0.23, which significantly improved to 0.13 ± 0.17 postoperatively (p < 0.001). There was no significant difference between ICL V4c and ICL V4 in the LogMAR UDVA, sphere, cylinder, SE, LogMAR CDVA, efficacy index, and safety index after surgery (p > 0.05). For eyes with full correction, the postoperative LogMAR CDVA = 0.575 * preoperative LogMAR CDVA − 0.061 (p < 0.001, R 2 = 0.53). For eyes with partial correction, the postoperative LogMAR CDVA = 0.536 * preoperative LogMAR CDVA − 0.007 * SE - 0.196 (p < 0.001, R 2 = 0.55). Conclusions ICL implantation can improve CDVA in eyes with suboptimal CDVA. This study provides the postoperative outcomes of eyes with different preoperative CDVA, which makes it convenient for surgeons to communicate with such patients before surgery.
15-month outcomes of Visian ICL implantation for high myopia in patients with shallow anterior chamber depth

Abstract:Abstract Background: High myopia with shallow anterior chamber depth (ACD < 2.8 mm) is not rare. This observational study aims to: evaluate clinical outcomes after implantation of the Visian Implantable Collamer Lens with a central hole (ICL V4c) in these patients. Methods: A prospective cohort of consecutive 51 eyes of 31 patients (20 to 42 years old) was followed for 15.35 ± 4.90 months (12 to 25 months). The preoperative ACD was 2.74 ± 0.04 mm (2.65 to 2.79 mm). Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), intraocular pressure (IOP), manifest refraction, vault, and endothelial cell density (ECD) were measured during the follow-ups after the surgery. Results: All surgeries were performed safely and no complication was observed during the follow-ups. At the last follow-up, the safety index (postoperative CDVA / preoperative CDVA) was 1.33 ± 0.60 and the efficacy index (postoperative UDVA / preoperative CDVA) was 1.14 ± 0.54. After the surgery, no eye had decreased CDVA and 59% (30 eyes) of the eyes gained at least one line. Forty-seven eyes (92%) were within ± 1.0 D and 35 eyes (69%) were within ± 0.5 D of the attempted refraction. The mean postoperative vault was 380.00 ± 152.84 µm (90 to 700µm). The ECD was reduced by 8.38 ± 0.06% compared to the preoperative value (p < 0.001). No significant change was observed in IOP (p = 0.061) at the last follow-up. Ultrasound Biomicroscopy (UBM) showed none of the eyes had trabecular-iris angle closed. Conclusions: In this prospective observational study, ICL V4c implantation in patients with high myopia and shallow ACD achieved satisfying and stable visual outcomes. [Mid-long term follow-up results in correction of extreme myopia by posterior chamber phakic intraocular lens]

Abstract:Objective: To evaluate the mid-long term stability and safety of posterior chamber phakic intraocular lens (ICL) implantation for the correction of extreme myopia. Methods: This retrospective study included 993 eyes of 498 patients received ICL implantation from June 1996 to December 2008. Multivariate analysis and variance analysis were used to evaluate the stability of the results and to identify risk factors of the complications. Results: Successful implantation was achieved in all patients. Spherical equivalent (SE) was (16.23 ± 4.12, mean ± SD) D before the operation and (-0.92 ± 1.22) D at the last examination. Intraocular pressure was (13.58 ± 2.93) mm Hg (1 mm Hg = 0.133 kPa) preoperatively which was (13.90 ± 3.01) mm Hg at the last examination. There was no statistical significance in follow-up (t = 0.44 ∼ 1.30, P > 0.05). Endothelial cell density was (2858.21 ± 395.13)/mm(2) before the operation and (2567.19 ± 423.45)/mm(2) at last examination. Pupillary block glaucoma occurred in 2 eyes (0.2%) at two hours and 1 eye (0.1%) at one month after the operation. Three eyes developed anterior cataracts between 6 months to 1 year after ICL implantation and another 2 eyes subcapsular opacification at semi-peripheral regions occurred 4 years after the operation, and was not related with age, SE and vaults (F = 2.42, 1.98, 0.81, P > 0.05). Macular puckers were found in 5 eyes (0.5%) 1 year postoperatively, including 2 eyes developed choroidal neovascularization and received PDT, best corrected visual acuity lost more than 2 lines. Retinal detachment occurred in 2 eyes at 1.5 years after the operation. Acute iritis happened in 1 eye (0.1%) and chronic iritis in 1 eye (0.1%) which combined with slight pupil distortion and elevation of intraocular pressure. Iris stroma atrophy and pupil distortion were found in 2 eyes (0.2%). Conclusion: Correction of high myopia by ICL implantation is a safe procedure and the results are stable.
Implantation of posterior chamber phakic intraocular lens for myopia and hyperopia - long-term clinical outcomes

Abstract:Purpose: To evaluate long-term refractive outcomes of implantable collamer lens (ICL) implantation and late postoperative complications. Methods: We assessed outcomes of patients who underwent ICL implantation (type ICM V4 for myopia, ICH V3 for hyperopia, TICM V4 for astigmatism) in our department between 1998 and 2013. It comprised 62 eyes (40 myopic and 22 hyperopic). The average follow-up period was 10.5 years. We evaluated: uncorrected and best-corrected visual acuity (UCVA and BCVA), spherical equivalent (SE), ICL vault, endothelial cell density and late postoperative complications. Results: In myopes, the average UCVA was 1.0±0.37 and BCVA 1.18±0.38, in hyperopes 0.78±0.19 and 1.14±0.18, respectively. The average SE in myopes, whose target refraction was emmetropia, was -0.6±0.83 Dsf, in hyperopes +0.73±0.93. Central ICL vault was 206.16μm±105.94, (range 10-427) in myopes, 195.5μm±109.09, (range 20-404) in hyperopes. The most common late postoperative complication was cataract formation. Three myopic eyes (7.5%) developed symptomatic anterior subcapsular opacities with loss of at least two lines of BCVA. Cataract significantly affecting visual acuity occurred in 5 myopic eyes (12.5%) and 2 hyperopic eyes (9.09%). In these eyes, ICL removal and cataract surgery was performed. Conclusions: In our experience, ICL implantation in moderate and high ametropia was effective and relatively safe. The most common late complication was cataract formation. This complication can be managed effectively surgically with good refractive outcomes without loss of BCVA.
Implantable Collamer Lens® for Management of Pseudophakic Ametropia in Eyes With a Spectrum of Previous Corneal Surgery

Abstract:Purpose: To assess the visual outcomes, predictability, and safety of the Implantable Collamer Lens (ICL) (STAAR Surgical, Inc., Monrovia, CA) for the management of residual pseudophakic refractive error. Methods: Forty-nine pseudophakic eyes of 40 patients in which myopic, hyperopic, or toric ICLs were implanted were retrospectively analyzed. Twenty-nine eyes had been implanted with a monofocal intraocular lens (IOL) and 20 eyes had a multifocal IOL. All IOLs were implanted in the capsular bag. Of the 49 eyes included, 6 had residual refractive error after phacoemulsification without corneal pathology or surgical alteration (virgin cornea group), 12 had LASIK or photorefractive keratectomy (PRK) (excimer laser group), 8 had radial keratotomy, 5 had intrastromal corneal ring segments (ICRS) implantation, 11 had penetrating keratoplasty, and 7 had deep anterior lamellar keratoplasty (DALK). Uncorrected and corrected (CDVA) distance visual acuity and manifest refraction were evaluated. Results: The efficacy/safety indices were 0.92/1.10, 0.98/1.13, 1.04/1.11, 0.90/1.13, 0.79/1.17, and 0.71/1.23 for the virgin cornea, excimer laser, radial keratotomy, ICRS, penetrating keratoplasty, and DALK groups, respectively. No eye lost one or more lines of CDVA. The virgin cornea, excimer laser, and radial keratotomy groups showed better predictability and accuracy, with 96.2% spherical equivalent within ±1.00 diopters (D). The ICRS, penetrating keratoplasty, and DALK groups demonstrated approximately 50% spherical equivalent within ±1.00 D. There were no intraoperative or postoperative complications. Conclusions: Good refractive outcomes and a strong safety record support the use of the ICL for the correction of residual refractive error in pseudophakic eyes with previous corneal surgery. Three-year outcomes of a novel toric intraocular lens implantation for moderate-high myopic astigmatism in phakic eyes

Abstract:Purpose: To investigate the three-year visual, refractive outcomes and adverse events of the Eyecryl toric phakic IOL (pIOL) for moderate-to-high myopic astigmatism. Methods: This retrospective study included eligible patients who underwent refractive surgery in one or both eyes with Eyecryl toric pIOL for myopic astigmatism. The efficacy, safety, predictability, rotational stability, vector analysis, and adverse events were evaluated in patients with spherical refraction from - 4.50 to -17.00 diopters (D) and cylindrical refraction from - 0.75 to -5.50 D. Results: Fifty-two eyes of 28 patients were included in the study. The mean efficacy and safety index were 1.12 ± 0.35 and 1.38 ± 0.42, respectively. The mean manifest refraction spherical equivalent was - 10.06 ± 2.69 D and - 0.64 ± 0.61 D preoperatively and postoperatively at 36 months, respectively. The mean manifest astigmatism was - 2.06 ± 1.16 D and - 0.44 ± 0.48 D preoperatively and 36 months postoperative, respectively. During the final examination, 70% of the eyes showed an increase in CDVA of one or more lines compared to their preoperative state. There was a cumulative endothelial cell loss of 3.1% at 36 months postoperatively. One eye developed visually significant anterior subcapsular opacity, whereas another eye experienced pIOL opacification. Conclusion: The Eyecryl toric pIOL demonstrated satisfactory visual acuity and refractive outcomes, as assessed by efficacy, safety and stability over a three-year period.
Toric implantable collamer lens (TICL) implantation to correct high myopia and astigmatism

Abstract:Objective To assess the efficacy,safety and stability of toric implantable collamer lens (TICL) implantation to correct high myopia and astigmatism.Methods Twenty-one eyes of 11 patients with high myopia and astigmatism which underwent TICL implantation were enrolled and followed up for 12 months,postoperatively.The examinations included uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),slit-lamp examination,gonioscopy,funduscopy,refraction,intraocular pressure (IOP),endothelial cell counts (ECCs),central cornea thickness (CCT),the vault of TICL,cylinder axis of TICL.Results Twelve months postoperatively,compared with before surgery,the spherical and cylindrical refraction decreased from (-14.79±3.09) D and (-3.05±0.94) D to (-0.52±0.56) D and (-0.48±0.42) D (P ＜0.01),UCVA and BCVA increased from (4.01±0.77) and (4.93±0.12) to (4.94±0.07) and (4.98±0.06) (P ＜0.05),respectively,lOP and CCT did not change (P ＞0.05),and ECCs was decreased by 3.79％ (P ＜0.05).The average degree of TICL cylinder axis rotation was (3.0±1.61)°.Conclusions TICL implantation shows the efficacy,safety and stability in correcting high myopia with astigmatism during 1-year following-up.
Visual outcomes and optical quality after implantation of a diffractive multifocal toric intraocular lens

Abstract:Background: This study evaluated the visual function after implantation of a multifocal toric intraocular lenses (IOLs). Materials and methods: This study involved 10 eyes from eight cataract patients with corneal astigmatism of 1.0 diopter (D) or higher who had received phacoemulsification with implantation of an AcrySof IQ ReSTOR Toric IOL. Six-month evaluations included visual acuity, spherical equivalent (SE), defocus curve, residual astigmatism, IOL rotation, contrast sensitivity (CS), wavefront aberrations, modulation transfer function (MTF), and patient satisfaction assessments. Results: At 6 months postoperatively, uncorrected distance visual acuity (logarithm of the minimum angle of resolution) was 0.09 ± 0.04, corrected distance visual acuity was 0.02 ± 0.11, and uncorrected near visual acuity was 0.12 ± 0.07. The mean SE was -0.095 ± 0.394 D (±0.50 D in 90%). Refractive astigmatism at the 6-month follow-up visit was significantly reduced to 0.35 ± 0.32 D from 1.50 ± 0.41 D presurgery (P < 0.05). The mean IOL axis rotation was 3.20 ± 1.55°. Postoperative CS levels were high. Postoperative total order aberrations (TOAs), lower-order aberrations (LOAs), higher-order aberrations (HOAs), and spherical aberrations were decreased compared with preoperative values (P < 0.05). At 3 months postoperatively, TOAs, LOAs, and HOAs with a 3 mm pupil diameter as well as TOAs, LOAs, and astigmatism aberrations with a 5 mm pupil diameter were statistically lower than those at 1-month post surgery, but without subsequent significant changes (P > 0.05). There was an increase in MTF results between preoperative and postoperative evaluations at all spatial frequencies. Conclusions: The diffractive multifocal toric IOL is able to provide a predictable astigmatic correction with apparently outstanding levels of optical quality after implantation.

Visual and Refractive Outcomes After Bi-aspheric Trifocal Toric Diffractive Intraocular Lens Implantation

Abstract:Purpose: To assess clinical outcomes after cataract surgery with bilateral implantation of a new bi-aspheric diffractive intraocular lens (IOL). Methods: Thirty patients underwent bilateral implantation of the Asqelio Trifocal Toric IOL (AST Products, Inc) and were evaluated 3 months postoperatively. Main outcomes included refractive error, photopic monocular and binocular uncorrected and corrected distance (UDVA, CDVA), intermediate (UIVA, CDIVA) at 60 cm, and near (UNVA, CDNVA) at 40 cm visual acuities. Mesopic monocular and binocular CDNVA were also measured. Defocus curves, binocular contrast sensitivity under photopic and mesopic conditions with and without glare and rotational stability were determined. Patients completed Catquest-9SF and visual symptoms questionnaires. Results: Average values of binocular photopic CDVA, CDIVA, and CDNVA, and mesopic CDNVA were -0.04 ± 0.06, 0.02 ± 0.08, 0.02 ± 0.07, and 0.22 ± 0.11 logMAR, respectively. All patients achieved cumulative CDVA ⩾ 20/25, and CDIVA and CDNVA of 20/32 or better. Binocular depth of focus was approximately 3.25 diopters (D). Mean postoperative spherical equivalent was -0.08 ± 0.26 D, with 95% of eyes within ±0.50 D. Mean postoperative refractive cylinder was -0.22 ± 0.27 D, with 91.67% of eyes within 0.50 D or less, respectively. IOL rotation averaged 0.25 ± 0.65 degrees, all eyes having rotation of less than 5 degrees. Contrast sensitivity was within or above normal levels under photopic and mesopic conditions, with or without glare, except for 12 cpd under mesopic conditions with glare. Questionnaire responses indicated 96.67% of patients were satisfied or very satisfied with postoperative vision, and 80.00% to 96.67% reported no difficulty in different daily activities. Conclusions: The Asqelio Trifocal Toric IOL demonstrated favorable outcomes, providing excellent visual performance at all distances, precise refractive results, and remarkable rotational stability. Long-term efficacy of implantation of AcrySof Toric intraocular lens on corneal astigmatism in cataract surgery

Abstract:AIM: To evaluate the clinical outcome of correction of corneal astigmatism and rotational stability of implantation of the AcrySof Toric intraocular lens(IOL) in cataract surgery.METHODS: Twenty-eight patients (thirty-two eyes) with more than 1.0 diopter(D) of preexisting astigmatism underwent similar phacoemulsification combined with AcrySof toric IOL implantation. The uncorrected visual acuity (UCVA), best-corrected visual acuity(BCVA), preoperative corneal astigmatism, anticipated residual astigmatism, postoperative residual astigmatism and toric lens axis were detected and compared.RESULTS: At 1 year following surgery, 85% of eyes showed the 0.5 or better in UCVA; 86% of eyes was 0.8 or better in BCVA and 92% of eyes achieved 0.6 or better in BCVA. The mean preoperative corneal astigmatism was (2.20 ±0.65)D and the postoperative residual astigmatism was (0.54±0.13)D, indicating a significant decrease in refractive cylinder after surgery(t=10.134, P<0.01). Preoperative predictive astigmatism(0.52±0.14)D and postoperative residual astigmatism (0.54±0.13)D showed no significant difference(t=0.364, P>0.05). After postoperative 6 month, the mean rotation of toric IOLs was 3.4°±2.2°, and after postoperative 1 year it was 3.82°±1.27°.CONCLUSION:AcrySof Toric IOL implantation is a viable and highly predictable method of correcting the corneal astigmatism, with no injury of the cornea. Careful selection of the patient, accurate keratometry and precise alignment of the cylindrical axes are some of the factors to be considered for a superior outcome.

Abstract:Purpose To compare one-year visual outcomes and optical quality after femtosecond laser small incision lenticule extraction (SMILE) and Visian Implantable Collamer Lens with a central hole (ICL V4c) implantation for high myopia. Methods This prospective non-randomized study included 37 eyes of 20 patients who underwent SMILE (G-S) and 39 eyes of 20 patients who underwent ICL V4c implantation (G-V). Spherical equivalent (SE), uncorrected distance visual acuity, corrected distance visual acuity, retinal image quality and intraocular scattering were evaluated at one year after surgery. Each model was adjusted for age, gender, eye and preoperative SE during treatment. Results At the one-year follow-up, G-S and G-V showed similar safety index (1.13 +/- 0.13 and 1.11 +/- 0.15, respectively) and efficacy index (1.05 +/- 0.14 and 1.06 +/- 0.15, respectively). All 21 eyes with Toric ICL (TICL) had a postoperative astigmatism of <= 0.5 dioptres (D), while the astigmatism was <= 0.5 D in all G-S eyes. Thirty-six G-S eyes (97%) and 35 G-V eyes (90%) were within +/- 0.5 D of the target SE. The changes in the modulation transfer function cut-off frequency (MTFcut-off) and the objective scatter index from before operation to one year following the operation were not significantly different between the groups (p = 0.523 and 0.826, respectively). Conclusion Both SMILE and ICL V4c implantation provide good safety, efficacy, predictability, and stability in correcting high myopia. Optical quality including intraocular scattering was great and relatively stable in both groups during the one-year observation period.

Abstract:PurposeTo investigate the effects of ocular residual astigmatism (ORA) and target-induced astigmatism (TIA) on the efficacy of toric implantable collamer lens (TICL) with central hole for myopic astigmatism correction. MethodsRetrospective case series. One hundred and eighteen eyes implanted with a TICL (V4c) from 118 patients were included. Subjective refraction and corneal topography were examined preoperatively, at 1 and 12 months postoperatively. The eyes were divided into the low-ORA ( <= 0.5 D) and high-ORA (>0.5 D) groups based on vector analysis, and into the low-TIA (>= 0.75D and <2 D) and the high-TIA (>= 2 D and <= 4 D) groups according to preoperative refractive astigmatism. Correction index (CI) and index of success (IOS) were compared between different groups. ResultsAll surgeries were uneventful, and no complications occurred during follow-up. At 1 and 12 months postoperatively, no significant differences were found in CI or IOS values between the high and low ORA groups, while significantly higher CI and lower IOS were detected in the high-TIA group than in the low-TIA group (P < 0.05). No significant difference was found in CI between 1 and 12 months postoperatively in either group (P > 0.05). However, significantly lower IOS was found at 12 months compared with 1 month postoperatively for each group (P < 0.05). ConclusionsToric implantable collamer lens (TICL) implantation is effective in correcting myopic astigmatism and is more effective in eyes with high TIA, while ORA has a minor effect.

Abstract:Aim: To observe early clinical outcome with lens position adjustment following the implantable collamer lens (ICL) surgery. Methods: Sixty patients were selected for this retrospective study. One eye from each patient received Toric ICL for astigmatism correction, and the other received non-astigmatic ICL surgery using horizontal position. Patients with higher postoperative arch height were selected, and their non-astigmatic eye clinical outcome were observed after ICL surgery at 1wk, 1, and 3mo. The clinical measurements included uncorrected visual acuity (UCVA), intraocular pressure (IOP), refractive state, corneal endothelium cell count, and arch height. Three months later, the ICL in each patient's non-astigmatic eye was adjusted to the vertical from the horizontal position. The results were compared before and 1wk, 1, and 3mo after adjustment. Results: UCVA and IOP were significantly reduced 1wk after position adjustment compared to 1wk after ICL implantation (P<0.05). The patients demonstrated significantly reduced arch height and corneal endothelium cell count 1wk, 1, and 3mo after adjusting position compared to 1wk, 1, and 3mo after ICL implantation (P<0.05). However, there was no significant difference in refraction between 1wk, 1, and 3mo after ICL implantation and position adjustment (P>0.05). Conclusion: Early positioning adjustment post-phakic ICL implantation can benefit patients with adjusted arch height or higher IOP. Despite the good clinical effects, the doctors should pay attention to the potential for adverse effects on UCVA and corneal endothelium cells following early position adjustment after posterior chamber phakic ICL implantation.
