Abstract:<h3 class="Heading">Background</h3><p class="Para">The PI3K/AKT pathway is activated through PIK3CA or AKT1 mutations and PTEN loss in breast cancer. We conducted a phase II trial with an allosteric AKT inhibitor MK-2206 in patients with advanced breast cancer who had tumors with PIK3CA/AKT1 mutations and/or PTEN loss/mutation.</p><h3 class="Heading">Methods</h3><p class="Para">The primary endpoint was objective response rate (ORR). Secondary endpoints were 6-month progression-free survival (6 m PFS), predictive and pharmacodynamic markers, safety, and tolerability. Patients had pre-treatment and on-treatment biopsies as well as collection of peripheral blood mononuclear cells (PBMC) and platelet-rich plasma (PRP). Next-generation sequencing, immunohistochemistry, and reverse phase protein arrays (RPPA) were performed.</p><h3 class="Heading">Results</h3><p class="Para">Twenty-seven patients received MK-2206. Eighteen patients were enrolled into the <em class="EmphasisTypeItalic">PIK3CA/AKT1</em> mutation arm (cohort A): 13 had <em class="EmphasisTypeItalic">PIK3CA</em> mutations, four had <em class="EmphasisTypeItalic">AKT1</em> mutations, and one had a <em class="EmphasisTypeItalic">PIK3CA</em> mutation as well as PTEN loss. ORR and 6 m PFS were both 5.6% (1/18), with one patient with HR+ breast cancer and a <em class="EmphasisTypeItalic">PIK3CA</em> E542K mutation experiencing a partial response (on treatment for 36 weeks). Nine patients were enrolled on the PTEN loss/mutation arm (cohort B). ORR was 0% and 6 m PFS was 11% (1/9), observed in a patient with triple-negative breast cancer and PTEN loss. The study was stopped early due to futility. The most common adverse events were fatigue (48%) and rash (44%). On pre-treatment biopsy, <em class="EmphasisTypeItalic">PIK3CA</em> and <em class="EmphasisTypeItalic">AKT1</em> mutation status was concordant with archival tissue testing. However, two patients with PTEN loss based on archival testing had PTEN expression on the pre-treatment biopsy. MK-2206 treatment was associated with a significant decline in pAKT S473 and pAKT T308 and PI3K activation score in PBMC and PRPs, but not in tumor biopsies. By IHC, there was no significant decrease in median pAKT S473 or Ki-67 staining, but a drop was observed in both responders.</p><h3 class="Heading">Conclusions</h3><p class="Para">MK-2206 monotherapy had limited clinical activity in advanced breast cancer patients selected for PIK3CA/AKT1 or PTEN mutations or PTEN loss. This may, in part, be due to inadequate target inhibition at tolerable doses in heavily pre-treated patients with pathway activation, as well as tumor heterogeneity and evolution in markers such as PTEN conferring challenges in patient selection.</p><h3 class="Heading">Trial registration</h3><p class="Para">ClinicalTrials.gov, <span class="ExternalRef"><a href="https://clinicaltrials.gov/ct2/show/NCT01277757"><span class="RefSource">NCT01277757</span></a></span>. Abstract:Purpose: Dysregulation of the phosphoinositide 3-kinase (PI3K)/Akt/mTOR pathway is seen in 40% to 60% of patients with colorectal cancer. Everolimus, an oral inhibitor of mTOR, showed efficacy in patients with metastatic colorectal cancers in phase I studies.Experimental Design: In sequential phase II studies assessing two dosing schedules, patients with metastatic colorectal cancers refractory to bevacizumab-, fluoropyrimidine-, oxaliplatin-, and irinotecan-based regimens received everolimus 70 mg/wk (n = 99) or 10 mg/d (n = 100). Primary endpoints were disease control rate (DCR) and objective response rate; secondary endpoints included progression-free survival (PFS), overall survival (OS), and duration of response or stable disease (SD). Tumor tissue was collected from all patients for predefined exploratory biomarker analyses.Results: Seventy-one patients were included in the per-protocol set for each cohort. DCRs of 31.0% and 32.4% (all SD) were seen in the weekly and daily schedules, respectively. Median duration of SD was 3.9 months in each cohort. Median PFS and OS were 1.8 and 4.9 months and 1.8 and 5.9 months, respectively, for the weekly and daily schedules. Among patients receiving daily everolimus, those with a KRAS mutation experienced significantly shorter median OS (P = 0.008) and lower DCR (P = 0.035) compared with those with wild-type KRAS in exploratory biomarker analyses.Conclusions: Everolimus 70 mg/wk or 10 mg/d was well tolerated but did not confer meaningful efficacy in heavily pretreated patients with metastatic colorectal cancers. Future studies may consider evaluating everolimus in combination with other agents or in patients with dysregulation of the PI3K/Akt/mTOR pathway. (C) 2013 AACR.
AKT Inhibitor MK-2206 Sensitizes Breast Cancer Cells to MLN4924, a First-in-class NEDD8-activating Enzyme (NAE) Inhibitor

Abstract:Breast cancer is a common type of cancer among female cancer patients and the main cause of cancer-related deaths. During the last decades, targeted therapies for breast cancer have been rapidly developing. Among them, MLN4924, a first-in-class NEDD8-activating enzyme (NAE) inhibitor, has performed antitumor activity by inactivating the cullin-RING ligases and causing the accumulation of their substrates to induce apoptosis in a number of studies. In this study, we found that MLN4924 activates the AKT pathway in both HER2-positive and triple-negative breast cancer (TNBC) cell lines. Given that AKT signaling is responsible for tumor progression and drug resistance in some types of cancers, we hypothesized that the AKT inhibitor may synergistically enhance the tumor suppression capability in breast cancer by MLN4924. To demonstrate the sensitizing effect, MK-2206 was chosen as the adjuvant treatment, and cell growth, migration and apoptosis were detected. The results showed that MLN4924 treatment inhibited cell growth and migration and induced apoptosis in both SK-BR3 and MDA-MB231 breast cancer cell lines. More importantly, the combined treatment of MLN4924 and MK-2206 indeed caused stronger cytotoxicity and inhibition of migration and a much higher induction of apoptosis compared with MLN4924 treatment alone. Our study provides the proof-of-concept evidence for strategic drug combination of MLN4924 with an AKT inhibitor for maximal killing of breast cancer cells via the enhancement of apoptosis.

Abstract:Purpose: PIK3CA mutations are common in breast cancer and promote tumor progression and treatment resistance. We conducted a phase I/II trial of alpelisib (α-specific PI3K inhibitor) plus nab-paclitaxel in patients with HER2-negative metastatic breast cancer (MBC). Patients and Methods: Eligible patients had HER2-negative MBC with any number of prior chemotherapies. Phase I was 3+3 dose-escalation design with three dose levels of alpelisib (250, 300, and 350 mg) daily plus nab-paclitaxel 100 mg/m 2 administered on days 1, 8, and 15 every 28 days. Phase II was according to Simon's two-stage design. PIK3CA mutations in tumor/circulating tumor DNA (ctDNA) were assessed. Primary endpoints were recommended phase II dose (RP2D) and objective response rate (ORR). Additional endpoints included safety, pharmacokinetics, progression-free survival (PFS), and association of PIK3CA mutation with outcomes. Results: A total of 43 patients were enrolled (phase I, n = 13 and phase II, n = 30). A total of 84% had visceral disease and 84% had prior taxane. No dose-limiting toxicities occurred in phase I. RP2D was alpelisib 350 mg daily plus nab-paclitaxel 100 mg/m 2 on days 1, 8, and 15. Hyperglycemia (grade 3, 26% and grade 4, 0%), neutropenia (grade 3, 23% and grade 4, 7%), diarrhea (grade 3, 5% and grade 4, 0%), and rash (grade 3, 7% and grade 4, 0%) were the most common adverse events. Among 42 evaluable patients, ORR was 59% (complete response, 7% and partial response, 52%), 21% of whom had response lasting >12 months; median PFS was 8.7 months. A total of 40% of patients demonstrated tumor and/or ctDNA PIK3CA mutation; patients with tumor/ctDNA mutation demonstrated better PFS compared with those without mutation (11.9 vs. 7.5 months; HR, 0.44; P = 0.027). Patients with normal metabolic status had longer PFS compared with prediabetic/diabetic patients (12 vs. 7.5 months; P = 0.014). No pharmacokinetics interactions were detected. Conclusions: The alpelisib plus nab-paclitaxel combination was well tolerated and shows encouraging efficacy, especially in patients with PIK3CA -mutated tumor/ctDNA. The impact of metabolic status on response to this combination merits further investigation.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>PI3K pathway activation is common in endometrial cancer. We evaluated the safety and efficacy of the dual PI3K/mTOR inhibitor, LY3023414, in patients with advanced endometrial cancer harboring activating mutations in the PI3K pathway.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We conducted a single‐arm phase 2 study of monotherapy LY3023414. Eligible patients had advanced endometrial cancer of any grade, prior management with 1‐4 cytotoxic lines, and PI3K pathway activation prospectively defined as a loss‐of‐function <i>PTEN</i> alteration or activating alteration in <i>PIK3CA</i>, <i>AKT1</i>, <i>PIK3R1</i>, <i>PIK3R2</i>, or <i>MTOR</i>. The primary objective was best overall response rate (ORR) per RECIST 1.1. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Twenty‐eight patients were treated; histologies included endometroid (39%), carcinosarcoma (25%), serous (21%), and mixed (14%). Patients were heavily pretreated, with a median of 2 prior cytotoxic lines (range, 1‐3). The most common alterations involved <i>PIK3CA</i> (68%), <i>PTEN</i> (43%), and <i>PIK3R1</i> (32%). In the 25 efficacy‐evaluable patients, the ORR was 16% (90% CI, 7%‐100%), and the clinical benefit rate was 28% (90% CI, 16%‐100%). Four patients had a confirmed partial response, and 2 responses lasted for >9 months. The median progression‐free survival and overall survival were 2.5 months (95% CI, 1.2‐3.0) and 9.2 months (95% CI, 5.0‐15.9), respectively. The most common all‐grade treatment‐related adverse events were anemia (71%), hyperglycemia (71%), hypoalbuminemia (68%), and hypophosphatemia (61%). No correlation between molecular alterations and response was observed. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>In patients with heavily pretreated advanced endometrial cancer prospectively selected for tumors with activating PI3K pathway mutations, LY3023414 demonstrated modest single‐agent activity and a manageable safety profile.</p></section>

Abstract:Purpose: MEK inhibitors (MEKi) lack monotherapy efficacy in most RAS-mutant cancers. BCL-xL is an anti-apoptotic protein identified by a synthetic lethal shRNA screen as a key suppressor of apoptotic response to MEKi. Patients and Methods: We conducted a dose escalation study (NCT02079740) of the BCL-xL inhibitor navitoclax and MEKi trametinib in patients with RAS-mutant tumors with expansion cohorts for: pancreatic, gynecologic (GYN), non-small cell lung cancer (NSCLC), and other cancers harboring KRAS/NRAS mutations. Paired pre-treatment and day 15 tumor biopsies and serial cell-free (cf)DNA were analyzed. Results: 91 patients initiated treatment, 38 in dose escalation. 58% had 33 prior therapies. 15 patients (17%) had colorectal cancer (CRC), 19 (11%) pancreatic, 15 (167%) NSCLC, and 32 (35%) GYN cancers. The recommended phase 2 dose (RP2D) was established as trametinib 2mg daily days 1-14 and navitoclax 250mg daily days 1-28 of each cycle. Most common adverse events included diarrhea, thrombocytopenia, increased AST/ALT, and acneiform rash. At RP2D, 8/49 (16.3%) evaluable patients achieved partial response (PR). Disease-specific differences in efficacy were noted. In GYN patients at the RP2D, 7/21 (33.3%) achieved a PR and median duration of response 8.2 months. No PRs occurred in CRC, NSCLC, or pancreatic patients. MAPK pathway inhibition was observed in on-treatment tumor biopsies. Reductions in KRAS/NRAS mutation levels in cfDNA correlated with clinical benefit. Conclusions: Navitoclax in combination with trametinib was tolerable. Durable clinical responses were observed in patients with RAS-mutant GYN cancers, warranting further evaluation in this population.

Abstract:TPS232 Background: Mutations in KRAS are one of the most prevalent oncogene mutations in human cancers, among which KRAS G12C mutations are one of the most frequently occurring. KRAS G12C inhibitors can slow tumor growth via attenuation of the KRAS signaling pathway. MK-1084, a selective KRAS G12C inhibitor, has demonstrated promising antitumor activity in preclinical studies. This phase 1, open-label, multicenter study (ClinicalTrials.gov, NCT05067283) is the first MK-1084 clinical study and aims to evaluate it as monotherapy and combination therapy in patients (pts) with advanced solid tumors. Findings from the initially enrolled study arms (Rojas et al. ESMO 2023; Abs 3156) indicated antitumor activity in pts with solid tumors receiving oral MK-1084 monotherapy QD or BID (25 to 800 mg total daily; arm 1) and pts with previously untreated non–small-cell lung cancer (NSCLC) with PD-L1 tumor proportion score ≥1% receiving MK-1084 from 25 to 400 mg total daily dose plus pembrolizumab 200 mg IV Q3W (arm 2). Toxicity was manageable during dose escalation of these 2 arms. Based on these initial findings, the study design has been updated to assess an alternate MK-1084 formulation and MK-1084 in combination with other therapies in KRAS G12C–mutated tumors. We present the study design for the new arms that include patients with gastrointestinal cancers. Methods: Eligible pts are aged ≥18 years with measurable disease per RECIST version 1.1 by investigator, ECOG performance status 0/1, adequate organ function, and advanced solid tumors (arm 3), or metastatic CRC with progression on 1 or 2 lines of prior therapy (arm 5), or previously untreated metastatic CRC (arm 6), all with KRAS G12C mutation. Pts in arm 3 receive the alternate MK-1084 formulation. Pts in arms 5 and 6 receive MK-1084 with one of the following: cetuximab 500 mg/m 2 IV Q2W (arm 5); or cetuximab 500 mg/m 2 IV Q2W, oxaliplatin 85 mg/m 2 , leucovorin 400 mg/m 2 , and 5-fluorouracil per label IV Q2W (arm 6). Each arm has a safety run-in prior to expansion. The primary objective is to assess the safety and tolerability of MK-1084 as monotherapy and in combination with these other agents. Secondary endpoints include objective response and duration of response. AEs will be evaluated from baseline through 30 days after cessation of study treatment (90 days for serious AEs). Tumor imaging by CT or MRI will be performed at baseline, every 6 weeks up to week 18, every 9 weeks for the remainder of the first year, and then every 12 weeks. Response is assessed per RECIST version 1.1 by investigators. Analyses include pts who receive ≥1 dose of study treatment. Study enrollment began in December 2021. Abstract:Neuroendocrine cells of the oral mucosa constitute an under-recognized component of the diffuse neuroendocrine system with diverse subpopulations and elusive biologic roles in the oral cavity. Primary malignant oral tumors that show a neuroendocrine phenotype display histomorphologic heterogeneity thereby giving rise to a spectrum of lesions in this rare category of oral malignancy. These lesions can be divided into neuroendocrine carcinomas (NECs) of small cell or non-small cell type. The former is further subdivided into the Merkel cell type or the pulmonary type while the latter includes atypical carcinoid tumor and large cell NEC. All histologic subtypes of oral NEC appear to have a strong predilection for men in their fifth or sixth decade and arise predominantly in the non-keratinized oral mucosa. The biologic behavior of oral Merkel cell carcinomas appears to be more aggressive than those of skin. It remains to be determined whether histologic categorization of the remaining tumor subtypes is predictive of patient survival in oral neuroendocrine tumors.

They were tentatively identified on the basis of Rf values and the U.V. spectra of the eluted spots in both acid and alkaline solutions. The chromatographic results showed that when PRPP was present in the reaction mixture, guanosine was converted to guanine and then to GMP probably via the combined action of guanosine phosphorylase and GMP pyrophosphorylase (EC 2.4.2.8). In the absence of PRPP, guanosine was converted to guanine. Accordingly brain guanosine phosphorylase would play an ‘anabolic’ rather than a ‘catabolic’ role, being involved in a salvage pathway for the biosynthesis of purine nucleotides. A similar role for brain adenosine deaminase and inosine phosphorylase was previously suggested (IPATA et at., 1970). It must be emphasized that guanosine phosphorylase and adenosine deaminase activities appear during the late developmental stages. This further supports the hypothesis that the ability to synthetize nucleotides through pathways different from those involved in ‘de novo’ synthesis is acquired by the brain of chick embryos at the time of hatching. It is of interest to note that nucleotides do not enter the nerve cells, whereas nucleosides and bases do and are utilized without cleavage of the purine or pyrimidine ring (MANDEL, personal communication). It is likely that the function of guanylate phosphatase and adenylate phosphatase showing a peak activity at the 12th and 15th day of incubation respectively would be to increase the intracellular concentration of guanosine and adenosine. It is suggested that this effect may result in turn in the consecutive appearance of guanosine phosphorylase and adenosine deaminase. GORDON and RODER (1953) have previously shown that adenosine deaminase is an inducible enzyme in the chick embryo. These concepts are summarized in Fig. 3.

Abstract:AKT is a key node in the most frequently deregulated signaling network in human cancer. AZD5363, a novel pyrrolopyrimidine-derived compound, inhibited all AKT isoforms with a potency of 10 nmol/L or less and inhibited phosphorylation of AKT substrates in cells with a potency of approximately 0.3 to 0.8 μmol/L. AZD5363 monotherapy inhibited the proliferation of 41 of 182 solid and hematologic tumor cell lines with a potency of 3 μmol/L or less. Cell lines derived from breast cancers showed the highest frequency of sensitivity. There was a significant relationship between the presence of PIK3CA and/or PTEN mutations and sensitivity to AZD5363 and between RAS mutations and resistance. Oral dosing of AZD5363 to nude mice caused dose- and time-dependent reduction of PRAS40, GSK3β, and S6 phosphorylation in BT474c xenografts (PRAS40 phosphorylation EC(50) ~ 0.1 μmol/L total plasma exposure), reversible increases in blood glucose concentrations, and dose-dependent decreases in 2[18F]fluoro-2-deoxy-D-glucose ((18)F-FDG) uptake in U87-MG xenografts. Chronic oral dosing of AZD5363 caused dose-dependent growth inhibition of xenografts derived from various tumor types, including HER2(+) breast cancer models that are resistant to trastuzumab. AZD5363 also significantly enhanced the antitumor activity of docetaxel, lapatinib, and trastuzumab in breast cancer xenografts. It is concluded that AZD5363 is a potent inhibitor of AKT with pharmacodynamic activity in vivo, has potential to treat a range of solid and hematologic tumors as monotherapy or a combinatorial agent, and has potential for personalized medicine based on the genetic status of PIK3CA, PTEN, and RAS. AZD5363 is currently in phase I clinical trials.

Abstract:Background: PD-1/PD-L1 inhibitor plus chemotherapy have shown tolerability and significant clinical benefits in pts with advanced TNBC. As antiangiogenic agent could remodel tumor blood vessels and increase the response to immune-checkpoint inhibitors (ICIs), we designed an investigator initiated trial (IIT) to investigate the efficacy and safety of AK105 (anti-PD-1 antibody), anlotinib (antiangiogenic, multi-target tyrosine kinase inhibitor) combined with nab-P as a first-line therapy in pts with advanced TNBC. Method: In this multicenter, prospective, single arm, phase 2 study, eligible pts were female aged 18-75 years, with ECOG PS 0-1, who had locally advanced or recurrent/metastatic triple-negative (estrogen receptor-, progesterone receptor- and HER2-) breast cancer. The main exclusive criteria were previous use of PD-1/PD-L1 antibody, cytotoxic T lymphocyte-associated antigen-4 (CTLA-4) antibody or antiangiogenic agent, and central nervous system (CNS) metastases. Eligible pts were treated with intravenous AK105(200mg on day 1), oral anlotinib(12mg once daily on days 1-14) and intravenous nab-P (125mg/m2 on days 1 and 8). The triplet combination regimen repeated every 21 days until disease progression, death or intolerable toxicity. The primary endpoint is overall response rate (ORR), and the secondary endpoints are disease control rate (DCR), progression-free survival (PFS), overall survival (OS) and safety. Simon's two-stage design were used to calculate the sample size assuming ORR is improved from 45.9% (nab-P alone) to 65% (AK105, anlotinib combined with nab-P) with the power (1-β) taken as 0.80 and the type I error (α) taken as 0.05 (two-sided). Abstract:Background: Trophoblast cell-surface antigen 2 (TROP2), a cell surface glycoprotein involved in the regulation of tumor growth and invasion, is broadly expressed and upregulated in several cancers, including NSCLC. MK-2870 (SKB264) is an antibody-drug conjugate (ADC) composed of the humanized anti-TROP2 monoclonal antibody, a hydrolytically cleavable linker, and the cytotoxic drug KL610023. MK-2870 has demonstrated encouraging antitumor activity and manageable toxicity in phase 1 and 2 studies of patients with solid tumors, including heavily pretreated EGFR-mutant NSCLC. We describe the design of the ongoing phase 3 MK-2870-004 study (NCT06074588) that will evaluate the efficacy and safety of MK-2870 vs chemotherapy for previously treated advanced or metastatic nonsquamous NSCLC with EGFR mutations or other genomic alterations. Trial design: The global, randomized, open-label, phase 3 MK-2870-004 study will enroll ~556 patients, including 456 with exon 19del or L858R EGFR mutations, and 100 with other genomic alterations. Eligible patients are aged ≥18 years with histologically or cytologically documented advanced (stage III not eligible for resection or curative radiation) or metastatic nonsquamous NSCLC with exon 19del or exon 21 L858R EGFR mutations or other genomic alterations in ALK, ROS1, BRAF V600E, NTRK, MET exon 14 skipping, or RET and those with less common EGFR mutations (exon 20 S768I, exon 21 L861Q, and/or exon 18 G719X). Patients must have progressive disease on or after 1 or 2 lines of prior tyrosine kinase inhibitor (TKI) therapy and a platinum-based therapy (may contain anti-PD-[L]1), measurable disease per RECIST v1.1, an available tumor sample, and adequate organ function. Prior EGFR TKI therapy must include a third-generation TKI for patients with T790M EGFR-mutant NSCLC. Patients with active central nervous system metastases or carcinomatous meningitis are ineligible, although those with previously treated and untreated stable brain metastasis are eligible for enrollment. Patients will be randomized (1:1) to MK-2870 4 mg/kg Q2W on Days 1, 15, and 29 of every 6-week cycle, or chemotherapy (docetaxel 75 mg/m2 Q3W or pemetrexed 500 mg/m2 Q3W) on Days 1 and 22 of every 6-week cycle. Randomization will be stratified by EGFR mutations with prior third generation TKI vs EGFR mutations with no prior third generation TKI vs other genomic alterations, brain metastasis (yes vs no), and TROP2 expression (low vs medium vs high). Dual primary endpoints are PFS (per RECIST v1.1 by blinded independent central review) and OS in patients with EGFR-mutant NSCLC. Abstract:<span class="paragraphSection">BackgroundKRAS oncogene mutations are causing sustained signaling through the mitogen-activated protein kinase (MAPK) pathway resulting in uncontrolled cell growth. Efforts to target KRAS directly or to inhibit downstream effectors have been unsuccessful. Preclinical research revealed that KRAS mutated (KRASm) cells are intrinsically resistant to MEK inhibitors due to upstream growth receptors that reactivate the MAPK and phosphoinositide 3-kinase (PI3K) pathway. Concurrent inhibition of MEK, EGFR and HER2 resulted in synergistic anti-tumor activity, with complete inhibition of tumor growth in vitro and in vivo.MethodsThis is a single-center, phase I dose-escalation study to assess the safety, tolerability and anti-tumor activity of the MEK inhibitor trametinib combined with the dual EGFR/HER2 inhibitor lapatinib in patients with advanced KRASm and PIK3CA wildtype colorectal cancer (CRC), non-small cell lung cancer (NSCLC) and pancreatic cancer. Patients received escalating doses of continuous or intermittent, once daily (QD) orally administered lapatinib and trametinib, starting at 750 mg and 1 mg, respectively. The primary objective was to determine the recommended phase 2 dose (RP2D) and schedule.ResultsThirty-four patients with CRC (n = 16), NSCLC (n = 15) or pancreatic cancer (n = 3) were enrolled across five dose-levels, 2 patients are still on treatment. Dose-limiting adverse events were reported in twelve patients; grade 3 diarrhea (n = 3), rash (n = 2), nausea (n = 1), several grade 2 toxicities (n = 1) and aspartate aminotransferase elevation (n = 1) resulting in inability to receive 75% of the planned doses (n = 2) or treatment delay (n = 2). The established RP2D was 750 mg lapatinib QD and trametinib 1.5 mg QD 5 days on/2 days off. Out of 22 patients evaluable for response, regression of target lesions was seen in six, with one confirmed partial response in NSCLC. Reductions in pERK and pS6 levels were demonstrated in paired tumor biopsies. Pharmacokinetic results were as expected.ConclusionsLapatinib and trametinib could be combined in an intermittent dosing schedule with manageable toxicity. Abstract:Cholangiocarcinoma (CCA) is a rare, but devastating disease arising from the epithelium of intrahepatic and extrahepatic bile ducts. There are neither effective systemic therapies nor satisfying treatment options for inoperable CCA. Histopathological and biochemical studies of CCA show frequent dysregulation of the phosphatidylinositol 3‐kinase/AKT/mammalian target of rapamycin (mTOR) pathway. Therefore, we investigated the efficacy of the mTOR inhibitor RAD001 and the impact of AKT signaling following mTOR inhibition in the treatment of CCA. RAD001 significantly inhibits proliferation of CCA cell lines, however, a concentration‐dependent and isoform specific feedback activation of the three AKT isoforms (AKT1, AKT2 and AKT3) was observed after mTOR inhibition. As activation of AKT might limit the RAD001‐mediated anti‐tumor effect, the efficacy of combined mTOR and AKT inhibition was investigated using the allosteric AKT inhibitor MK‐2206. Our results show that inhibition of AKT potentiates the efficacy of mTOR inhibition both in vitro and in a xenograft mouse model in vivo. Mechanistically, the antiproliferative effect of the pan‐AKT inhibitor MK2206 in the CCA cell line TFK‐1 was due to inhibition of AKT1 and AKT2, because knockdown of either AKT1 or AKT2, but not AKT3, showed a synergistic reduction of cell proliferation in combination with mTOR treatment. Finally, using an AKT isoform specific in vitro kinase assay, enzymatic activity of each of the three AKT isoforms was detected in all tissue samples from CCA patients, analyzed. In summary, our preclinical data suggest that combined targeting of mTOR and AKT using RAD001 and MK‐2206 might be a new, effective strategy for the treatment of CCA.

Abstract:Background: Preclinical data showed that high levels of vascular endothelial growth factor (VEGF) may lead to aggressive behavior in breast tumors that overexpress HER2. AVEREL study demonstrated that bevacizumab in combination with trastuzumab and docetaxel as first-line treatment increased progression-free survival (PFS) in HER2 postive metastatic breast cancer (BC) patients (BTH vs TH:16.5m vs 13.7m, HR=0.82, P=0.0775). It is necessary to exploring new effective and tolerable strategy of targeting both HER2 signaling and angiogenesis. PHENIX and PHOEBE studies proved pyrotinib (an irreversible pan-ErbB receptor tyrosine kinase inhibitor) plus capecitabine improved prognosis for patients with advanced HER2 positive BC. Anlotinib is a novel multi-target tyrosine kinase inhibitor that effectively inhibits VEGFR, FGFR, c-KIT, c-MET and RET. This study is aimed to evaluate safety, tolerability and efficacy of anlotinib combined with pyrotinib and capecitabine for HER2-Positive metastatic BC. Methods: This open-label study is designed to include patients with pathologically confirmed HER2-positive metastatic breast cancer that have progressed or relapsed after treatment with trastuzumab or inability to receive trastuzumab in West China Hospital, Sichuan University. Eligible patients have at least one measurable lesion according to RECIST v1.1; previously received taxanes regimen and had ≤2 line of chemotherapy for advanced disease; an ECOG performance status of 0-1; adequate organ function. In the "3+3" dose-exploring phase, pyrotinib and capecitabine are given at a fixed dose of 400mg qd and 1000 mg/m2 bid respectively. Anlotinib is initially given at a dose of 10mg/d (Level I). If the initial dose level could be tolerated, subsequent patients are assigned to the higher level (Level H) with anlotinib 12mg/d; otherwise, to Level L with anlotinib 8 mg/d. Primary endpoints of phase Ib are dose-limiting toxicities (DLTs), maximum tolerated dose (MTD), recommended phase 2 dose (RP2D), safety and efficacy. Phase II is an expansion cohort at the recommended phase II dose (RP2D). The primary endpoint for phase II is one-year PFS rate by investigator-assessed, and secondary endpoints include PFS, overall response rate (ORR), clinical benefit rate (CBR), duration of response (DoR), overall survival (OS) and quality of life. A sample size of 23 provided 80% power at 2-sided alpha = 0.20 to detect a minimum of 20% improvement (45% vs. 65%) in one-year PFS. The phase Ib portion of the trial is currently enrolling in China. Abstract:Nevus sebaceus of Jadassohn is a hamartoma that commonly arises at birth or in early childhood and has a well-documented potential to develop a variety of benign and, less commonly, malignant neoplasms of epidermal and adnexal origins. Neoplastic transformation occurs with an estimated incidence of 10% to 30%, and risk of disease increases with age. Nevus sebaceus usually presents as a single yellowish plaque devoid of hair. The natural history of the disease is often divided into three phases: infancy, puberty, and adulthood. Most postpubertal neoplasms arise from the fourth to the seventh decades. Two neoplasms may occasionally arise within the same lesion, but it is rare for three or more neoplasms to occur in a nevus sebaceus simultaneously. Here, we report a patient with nevus sebaceus with four tumors simultaneously occurring in a single lesion— basal cell carcinoma (BCC), syringocystadenoma papilliferum, trichilemmoma, and sebaceoma— associated with widespread nevi in the ipsilateral limb and trunk.

Abstract:Background This phase 1b study (ClinicalTrials.gov identifier NCT03695380) evaluated regimens combining PARP and MEK inhibition, with or without PD‐L1 inhibition, for BRCA wild‐type, platinum‐sensitive, recurrent ovarian cancer (PSROC). Methods Patients with PSROC who had received one or two prior treatment lines were treated with 28‐day cycles of cobimetinib 60 mg daily (days 1–21) plus niraparib 200 mg daily (days 1–28) with or without atezolizumab 840 mg (days 1 and 15). Stage 1 assessed safety before expansion to stage 2, which randomized patients who had BRCA wild‐type PSROC to receive either doublet or triplet therapy, stratified by genome‐wide loss of heterozygosity status (<16% vs. ≥16%; FoundationOne CDx assay) and platinum‐free interval (≥6 to 15 months) and progressors (disease progression as the best response) were characterized. Results The ORR in patients who had BRCA wild‐type PSROC was 35% (95% confidence interval, 20%–53%) with the doublet regimen (n = 37) and 27% (95% confidence interval, 14%–44%) with the triplet regimen (n = 37), and the median PFS was 6.0 and 7.4 months, respectively. Post‐hoc analyses indicated more favorable ORR and PFS in the homologous recombination‐deficiency‐signature (HRDsig)‐positive subgroup than in the HRDsig‐negative subgroup. Tolerability was consistent with the known profiles of individual agents. NF1 and MKNK1 mutations were associated with sustained benefit from the doublet and triplet regimens, respectively. Conclusions Chemotherapy‐free doublet and triplet therapy demonstrated encouraging activity, including among patients who had BRCA wild‐type, HRDsig‐positive or HRDsig‐negative PSROC harboring NF1 or MKNK1 mutations.
