Abstract:Background: Poor balance has been cited as one of the key causal factors of falls. Timely detection of balance impairment can help identify the elderly prone to falls and also trigger early interventions to prevent them. The goal of this study was to develop a surrogate approach for assessing elderly's functional balance based on Short Form Berg Balance Scale (SFBBS) score. Methods: Data were collected from a waist-mounted tri-axial accelerometer while participants performed a timed up and go test. Clinically relevant variables were extracted from the segmented accelerometer signals for fitting SFBBS predictive models. Regularized regression together with random-shuffle-split cross-validation was used to facilitate the development of the predictive models for automatic balance estimation. Results: Eighty-five community-dwelling older adults (72.12 ± 6.99 year) participated in our study. Our results demonstrated that combined clinical and sensor-based variables, together with regularized regression and cross-validation, achieved moderate-high predictive accuracy of SFBBS scores (mean MAE = 2.01 and mean RMSE = 2.55). Step length, gender, gait speed and linear acceleration variables describe the motor coordination were identified as significantly contributed variables of balance estimation. The predictive model also showed moderate-high discriminations in classifying the risk levels in the performance of three balance assessment motions in terms of AUC values of 0.72, 0.79 and 0.76 respectively. Conclusions: The study presented a feasible option for quantitatively accurate, objectively measured, and unobtrusively collected functional balance assessment at the point-of-care or home environment. It also provided clinicians and elderly with stable and sensitive biomarkers for long-term monitoring of functional balance.

Abstract:To date, the human balance assessment is mainly achieved by measuring the center of pressure (COP) trajectory of upright posture. However, COP-based metrics provide inconclusive results under certain circumstances, due to the lack of relevance with the body balance mechanism. This paper developed a novel framework to measure the variability in body sway based on the Poincaré analysis to enhance the balance assessment. It provided nonlinear parameters SD1, SD2, and their ratio SD1/SD2 as quantitative indicators of the local and global balance regulation capacities. 136 healthy adults were categorized into young and elderly groups and received test on a force platform. COP signals were recorded for 30 s under eyes-opened and eyes-closed vision conditions. The results were analyzed using the two-way mixed-design analysis of variance and show that the technology is able to reveal the effect of age and vision on the balance control by SD1, SD2, and SD1/SD2. SD1 mainly responded to vision, and SD2 was more influenced by age. Due to the information about the subtle adjustment during quiet standing, these Poincaré indexes surpassed the conventional COP measurements in demonstrating the balance control behavior. It can be concluded that the supposed method of estimating the local and global variabilities is a valuable approach for the balance assessment.
Predicting losses of balance during upright stance: evaluation of a novel approach based on wearable accelerometers

Abstract:The study of postural sway during quiet stance has proved to be a useful approach to investigate the function of the balance system. Recent studies have suggested that providing information on postural sway to vestibular patients through biofeedback may improve their balance awareness and therefore reduce their risk of falling. One drawback common to these approaches is related to timing: informing a patient about current balance conditions may not allow enough time to react and avoid a fall. Here we propose a new technique for predicting relevant balance related events based on the recording of inertial information on trunk and thigh movement using wearable devices. We have developed a regressive model for the prediction of quiet stance dynamics of the center of body mass (CM), based on these sensory data. Our preliminary results show that, with careful signal processing, such approach may allow to learn quiet stance dynamics based on the inverted pendulum model and use it in predicting critical balance conditions with a few hundreds of milliseconds advance. When these predictions are then used for event-detection the system provides accurate results and is thus promising for the development of a fall prevention device.
Elderly fall risk prediction using static posturography

Abstract:Maintaining and controlling postural balance is important for activities of daily living, with poor postural balance being predictive of future falls. This study investigated eyes open and eyes closed standing posturography with elderly adults to identify differences and determine appropriate outcome measure cut-off scores for prospective faller, single-faller, multi-faller, and non-faller classifications. 100 older adults (75.5 ± 6.7 years) stood quietly with eyes open and then eyes closed while Wii Balance Board data were collected. Range in anterior-posterior (AP) and medial-lateral (ML) center of pressure (CoP) motion; AP and ML CoP root mean square distance from mean (RMS); and AP, ML, and vector sum magnitude (VSM) CoP velocity were calculated. Romberg Quotients (RQ) were calculated for all parameters. Participants reported six-month fall history and six-month post-assessment fall occurrence. Groups were retrospective fallers (24), prospective all fallers (42), prospective fallers (22 single, 6 multiple), and prospective non-fallers (47). Non-faller RQ AP range and RQ AP RMS differed from prospective all fallers, fallers, and single fallers. Non-faller eyes closed AP velocity, eyes closed VSM velocity, RQ AP velocity, and RQ VSM velocity differed from multi-fallers. RQ calculations were particularly relevant for elderly fall risk assessments. Cut-off scores from Clinical Cut-off Score, ROC curves, and discriminant functions were clinically viable for multi-faller classification and provided better accuracy than single-faller classification. RQ AP range with cut-off score 1.64 could be used to screen for older people who may fall once. Prospective multi-faller classification with a discriminant function (-1.481 + 0.146 x Eyes Closed AP Velocity-0.114 x Eyes Closed Vector Sum Magnitude Velocity-2.027 x RQ AP Velocity + 2.877 x RQ Vector Sum Magnitude Velocity) and cut-off score 0.541 achieved an accuracy of 84.9% and is viable as a screening tool for older people at risk of multiple falls.

Abstract:Assessment of balance by means of posturographic analysis is frequently used in the clinical practice for evaluating the risk of falls or as an indicator of balance-related disorders. The development of automatic, affordable and accurate systems for gauging balance capabilities in the elderly is deemed a crucial step towards the adoption of prevention strategies and the reduction of associated social costs, especially in a context of growing average age of population. In this article we propose to exploit signals that can be collected from sensors on board of common consumer-grade smartphones for posturographic analysis. To this aim, we introduce several processing algorithms for extracting useful information from the acceleration data streams, and we also present an assessment framework based on the comparison of the trajectory of the body center of gravity, estimated from embedded triaxial accelerometers, with a homologous counterpart, estimated from the reference plate force, thus adding to the consistency of the whole process. Experimental results confirm the effectiveness of the proposed system in terms of its capability of achieving signals and posturographic features which agree with those obtained by means of balance board platforms, potentially opening the way to novel research studies and applications of mobile technology in this field.

Abstract:The problem of decreased balance caused by injury, illness, or aging is becoming increasingly prevalent in society. The traditional functional balance assessment method is highly time-consuming and inefficient, as well as being susceptible to measurement errors caused by personal subjective factors. This study proposes a system that can rapidly, conveniently, and accurately predict a participant's Berg balance scale (BBS) score without professional supervision. The proposed system uses a wearable inertial sensing device combined with machine learning to predict the BBS score of a test participant. In the beginning, the participants were asked to wear inertial sensing devices on seven parts of the body and perform 17 test tasks. The wearable device locations and the test tasks were ranked by importance and further reduced wearable devices and the test tasks. Eventually, the participant is only required to wear an inertial sensing device on their left thigh and perform two simple test tasks, namely "placing an alternate foot on a stool" and "standing on one foot (right foot)," to obtain their BBS score. In this study, the proposed system has a high level of accuracy for predicting BBS scores. The experimental results indicate that the mean absolute error (MAE) of the proposed system was 1.274. Moreover, this study provided some important information as a reference for future research on functional balance, including feature sets selection, regression model selection, wearable device locations ranking, and test tasks ranking. The researchers can use that information to design their experiment.

Abstract:The elderly population is growing rapidly in the world and falls are becoming a big problem for society. Currently, clinical assessments of gait and posture include functional evaluations, objective, and subjective scales. They are considered the gold standard to indicate optimal mobility and performance individually, but their sensitivity and specificity are not good enough to predict who is at higher risk of falling. An innovative approach for fall prediction is the machine learning. Machine learning is a computer-science area that uses statistics and optimization methods in a large amount of data to make outcome predictions. Thus, to assess the performance of machine learning algorithms in classify participants by age, number of falls and falls frequency based on features extracted from a public database of stabilometric assessments. 163 participants (116 women and 47 men) between 18 and 85 years old, 44.0 to 75.9 kg mass, 140.0 to 189.8 cm tall, and 17.2 to 31.9 kg/m 2 body mass index. Six different machine learning algorithms were tested for this classification, which included Logistic Regression, Linear Discriminant Analysis, K Nearest-neighbours, Decision Tree Classifier, Gaussian Naive Bayes and C-Support Vector Classification. The machine learning algorithms were applied in this database which has sociocultural, demographic, and health status information about participants. All algorithm models were able to classify the participants into young or old, but our main goal was not achieved, no model identified participants at high risk of falling. Our conclusion corroborates other works in the biomechanics field, arguing the static posturography, probably due to the low daily living activities specificity, does not have the desired effects in predicting the risk of falling. Further studies should focus on dynamic posturography to assess the risk of falls.

Abstract:The increase in life expectancy, and the consequent growth of the elderly population, represents a major challenge to guarantee adequate health and social care. The proposed system aims to provide a tool that automates the evaluation of gait and balance, essential to prevent falls in older people. Through an RGB-D camera, it is possible to capture and digitally represent certain parameters that describe how users carry out certain human motions and poses. Such individual motions and poses are actually related to items included in many well-known gait and balance evaluation tests. According to that information, therapists, who would not need to be present during the execution of the exercises, evaluate the results of such tests and could issue a diagnosis by storing and analyzing the sequences provided by the developed system. The system was validated in a laboratory scenario, and subsequently a trial was carried out in a nursing home with six residents. Results demonstrate the usefulness of the proposed system and the ease of objectively evaluating the main items of clinical tests by using the parameters calculated from information acquired with the RGB-D sensor. In addition, it lay the future foundations for creating a Cloud-based platform for remote fall risk assessment and its integration with a mobile assistant robot, and for designing Artificial Intelligence models that can detect patterns and identify pathologies for enabling therapists to prevent falls in users under risk.

The study was accompanied by a commentary (2) indicating that these autoimmune disorders were “an unlikely alliance” because the HLA haplotype that increases risk for multiple sclerosis (DRB1\*15-DQA1\*0102-DQB1*0602) protects against type 1 diabetes. Improved Leg Tracking Considering Gait Phase and Spline-Based Interpolation during Turning Motion in Walk Tests

Abstract:Falling is a common problem in the growing elderly population, and fall-risk assessment systems are needed for community-based fall prevention programs. In particular, the timed up and go test (TUG) is the clinical test most often used to evaluate elderly individual ambulatory ability in many clinical institutions or local communities. This study presents an improved leg tracking method using a laser range sensor (LRS) for a gait measurement system to evaluate the motor function in walk tests, such as the TUG. The system tracks both legs and measures the trajectory of both legs. However, both legs might be close to each other, and one leg might be hidden from the sensor. This is especially the case during the turning motion in the TUG, where the time that a leg is hidden from the LRS is longer than that during straight walking and the moving direction rapidly changes. These situations are likely to lead to false tracking and deteriorate the measurement accuracy of the leg positions. To solve these problems, a novel data association considering gait phase and a Catmull-Rom spline-based interpolation during the occlusion are proposed. From the experimental results with young people, we confirm that the proposed methods can reduce the chances of false tracking. In addition, we verify the measurement accuracy of the leg trajectory compared to a three-dimensional motion analysis system (VICON).
Quantitative Assessment of Fall Risk in the Elderly Through Fusion of Millimeter-Wave Radar Imaging and Trajectory Features

Abstract:The aging process and chronic diseases can lead to functional decline in older adults, particularly with significant decreases in balance ability, which greatly increases the risk of falls. Traditional balance ability assessment methods typically rely on clinical rating scales, which are subjective and prone to the Hawthorne effect and are difficult to implement for continuous daily assessment. In this paper, we propose a quantitative risk assessment system for elderly falls based on the fusion of millimeter-wave(mmWave) radar imaging and trajectory features. Key features such as the center-of-mass trajectory, trajectory offset, and maximum swing diameter are extracted by improving the fusion clustering algorithm. Then, a model such as Random Forest (RF) is applied to conduct correlation analysis on the features, ultimately proving a significant correlation between feature selection and scale scoring. Subsequently, a quantitative assessment model is established with core algorithms such as Support Vector Regression (SVR), Gradient Boosting Decision Tree (GBDT), Extreme Gradient Boosting (XGBoost), and Light Gradient Boosting Machine (LightGBM) to verify the effectiveness of the evaluation. The result indicates that the LightGBM model achieved the best performance in assessment compared to other models, with a prediction accuracy score of 93.36%. The experiment has demonstrated that the system can effectively capture the features of gait and evaluate early changes in balance ability decline. The research on this system provides a new technological approach to daily fall risk warnings.

Detection of Gait Abnormalities for Fall Risk Assessment Using Wrist-Worn Inertial Sensors and Deep Learning

Abstract:Falls are a significant threat to the health and independence of elderly people and represent an enormous burden on the healthcare system. Successfully predicting falls could be of great help, yet this requires a timely and accurate fall risk assessment. Gait abnormalities are one of the best predictive signs of underlying locomotion conditions and precursors of falls. The advent of wearable sensors and wrist-worn devices provides new opportunities for continuous and unobtrusive monitoring of gait during daily activities, including the identification of unexpected changes in gait. To this end, we present in this paper a novel method for determining gait abnormalities based on a wrist-worn device and a deep neural network. It integrates convolutional and bidirectional long short-term memory layers for successful learning of spatiotemporal features from multiple sensor signals. The proposed method was evaluated using data from 18 subjects, who recorded their normal gait and simulated abnormal gait while wearing impairment glasses. The data consist of inertial measurement unit (IMU) sensor signals obtained from smartwatches that the subjects wore on both wrists. Numerous experiments showed that the proposed method provides better results than the compared methods, achieving 88.9% accuracy, 90.6% sensitivity, and 86.2% specificity in the detection of abnormal walking patterns using data from an accelerometer, gyroscope, and rotation vector sensor. These results indicate that reliable fall risk assessment is possible based on the detection of walking abnormalities with the use of wearable sensors on a wrist.

Automation of Functional Mobility Assessments at Home Using a Multimodal Sensor System Integrating Inertial Measurement Units and Computer Vision (IMU-Vision)

Abstract:Objective: Functional movement assessments are routinely used to evaluate and track changes in mobility. The objective of this study was to evaluate a multimodal movement monitoring system developed for autonomous, home-based, functional movement assessment. Methods: Fifty frail and prefrail adults were recruited from the Brigham and Women's Hospital at Home program to evaluate the feasibility and accuracy of applying the multimodal movement monitoring system to autonomously recognize and score functional activities collected in the home. Study subjects completed sit-to-stand, standing balance (Romberg, semitandem, and tandem), and walking test activities in likeness to the Short Physical Performance Battery. Test activities were identified and scored manually and by the multimodal movement monitoring system's activity recognition and scoring algorithms, which were previously trained on lab-based biomechanical data to integrate wearable inertial measurement unit (IMU) and external red-blue-green-depth vision data. Feasibility was quantified as the proportion of completed tests that were analyzable. Accuracy was quantified as the degree of agreement between the actual and system-identified activities. In an exploratory analysis of a subset of functional activity data, the accuracy of a preliminary activity-scoring algorithm was also evaluated. Results: Activity recognition by the IMU-vision system had good feasibility and high accuracy. Of 271 test activities collected in the home, 217 (80%) were analyzable by the activity-recognition algorithm, which overall correctly identified 206 (95%) of the analyzable activities: 100% of walking, 97% of balance, and 82% of sit-to-stand activities (χ2(2) = 19.9). In the subset of 152 tests suitable for activity scoring, automatic and manual scores showed substantial agreement (Kw = 0.76 [0.69, 0.83]). Conclusions: Autonomous recognition and scoring of home-based functional activities is enabled by a multimodal movement monitoring system that integrates inertial measurement unit and vision data. Further algorithm training with ecologically valid data and a kitted system that is independently usable by patients are needed before fully autonomous, functional movement assessment is realizable. Impact: Functional movement assessments that can be administered in the home without a clinician present have the potential to democratize these evaluations and improve care access.

Abstract:Post-stroke patients usually suffer from a higher fall risk. Identifying potential fallers and giving them proper attention could reduce their chance of a fall that results in severe injuries and decreased quality of life. In this study, we introduced a novel approach for fall risk prediction that evaluates Short-form Berg Balance Scale scores via inertial measurement unit data measured from a 3-meter timed-up-and-go test. This approach used sensor technology and was thus easy to implement, and allowed a quantitative analysis of both gait and balance. The results showed that elastic net logistic regression achieved the best performance with 85% accuracy and 88% area under the curve compared with support vector machine, least absolute shrinkage and selection operator (LASSO), and stepwise logistic regression. This paper provides a framework for using sensor-based features together with a feature-selection strategy for screening and predicting the fall risk of post-stroke patients in a convenient setup with high accuracy. The findings of this study will not only enable the assessment of fall risk among post-stroke patients in a cost-effective manner but also provide decision-making support for community care providers and medical professionals in the form of sensor-based data on gait performance.

Abstract:This paper proposes a new method to analyze human walking by using a 3-wheels rollator walker instrumented with encoders and a 3D accelerometer/gyrometer. In order to develop walking quality index and monitor the health state of elderly people at home, the walking patterns of 23 young adults and 25 elderly people (>69 years) with the help of the walker, are compared. The results show that many general walking indicators such as walking speed and stride length do not present obvious difference between the two groups, but that new indicators obtained by using the walker measurements and not available otherwise are very discriminating, e.g., the lateral motion of elderly people is larger, their walking accuracy is lower, but their effort distributed on the handles is more symmetrical. We also show that this walker may have other purposes such as updating collaborative maps with sideway slopes, and location of lowered kerbs.

Markerless three-dimensional gait analysis in healthy older adults: test-retest reliability and measurement error

Abstract:In older adults, gait analysis may detect changes that signal early disease states, yet challenges in biomechanical screening limit widespread use in clinical or community settings. Recently, a markerless method from multi-camera video data has become accessible, making screenings less challenging. This study evaluated the test-retest reliability and measurement error of markerless gait kinematics and kinetics in healthy older adults. Twenty-nine healthy older adults performed gait analysis on two occasions, at preferred walking speed, using their everyday clothes. Lower limb angles and moments were averaged from 8 gait cycles. Integrated pointwise indices [Intraclass Correlation Coefficient (ICCA,K) and Standard Error of Measurement (SEM)] were calculated for curve data, as well as ICCA,K, and SEM [95 % confidence intervals] for selected peaks. Generally, kinematic ICCs were good (>0.75) and reasonably stable throughout the gait cycle, except for the hip kinematics during the swing phase in the sagittal plane and pelvis tilt and rotation. The integrated and peaks SEM were <2.4°. The reliability of kinetics was similar (ICC>0.75), except for the transverse hip moment and abduction peak, fluctuating more during the swing than through the stance phase. SEM were < 0.07Nm/Kg. In conclusion, these results showed good overall test-retest reliability for markerless gait kinematics and kinetics for the hip, knee, and ankle joints, moderate for the pelvis angles, and error levels of ≤5°, and SEM%≤5% for the sagittal plane. Abstract:The ability to assess balance is essential to determine a patients ability to mitigate any risk of falling. While current assessment tools exist, they either have limitations in that there is no quantitative data recorded, or that they are impractical for general use in clinical settings. In this work, we aim at assessing balance using single-camera videos. In particular, the proposed method uses OpenPose to calculate the Center of Mass and Center of Pressure trajectories. To determine the validity of this approach, estimates obtained in an experimental study were compared to recordings obtained through the use of 3D motion capture and force plate. Our results indicate that this inexpensive, easy to use, and portable alternative has the potential to act as a suitable replacement to assess balance in clinical settings.
