Abstract:Early detection of pulmonary cancer is the most promising way to enhance a patient's chance for survival. Accurate pulmonary nodule detection in computed tomography (CT) images is a crucial step in diagnosing pulmonary cancer. In this paper, inspired by the successful use of deep convolutional neural networks (DCNNs) in natural image recognition, we propose a novel pulmonary nodule detection approach based on DCNNs. We first introduce a deconvolutional structure to Faster Region-based Convolutional Neural Network (Faster R-CNN) for candidate detection on axial slices. Then, a three-dimensional DCNN is presented for the subsequent false positive reduction. Experimental results of the LUng Nodule Analysis 2016 (LUNA16) Challenge demonstrate the superior detection performance of the proposed approach on nodule detection(average FROC-score of 0.891, ranking the 1st place over all submitted results).
A Deep 3D Residual CNN for False-Positive Reduction in Pulmonary Nodule Detection

Abstract:PurposeThe automatic detection of pulmonary nodules using CT scans improves the efficiency of lung cancer diagnosis, and false-positive reduction plays a significant role in the detection. In this paper, we focus on the false-positive reduction task and propose an effective method for this task. MethodsWe construct a deep 3D residual CNN (convolution neural network) to reduce false-positive nodules from candidate nodules. The proposed network is much deeper than the traditional 3D CNNs used in medical image processing. Specifically, in the network, we design a spatial pooling and cropping (SPC) layer to extract multilevel contextual information of CT data. Moreover, we employ an online hard sample selection strategy in the training process to make the network better fit hard samples (e.g., nodules with irregular shapes). ResultsOur method is evaluated on 888 CT scans from the dataset of the LUNA16 Challenge. The free-response receiver operating characteristic (FROC) curve shows that the proposed method achieves a high detection performance. ConclusionsOur experiments confirm that our method is robust and that the SPC layer helps increase the prediction accuracy. Additionally, the proposed method can easily be extended to other 3D object detection tasks in medical image processing.

Abstract:Computer aided detection (CAD) systems can assist radiologists by offering a second opinion on early diagnosis of lung cancer. Classification and feature representation play critical roles in false-positive reduction (FPR) in lung nodule CAD. We design a deep convolutional neural networks method for nodule classification, which has an advantage of autolearning representation and strong generalization ability. A specified network structure for nodule images is proposed to solve the recognition of three types of nodules, that is, solid, semisolid, and ground glass opacity (GGO). Deep convolutional neural networks are trained by 62,492 regions-of-interest (ROIs) samples including 40,772 nodules and 21,720 nonnodules from the Lung Image Database Consortium (LIDC) database. Experimental results demonstrate the effectiveness of the proposed method in terms of sensitivity and overall accuracy and that it consistently outperforms the competing methods.

Abstract:Lung cancer is one of the leading causes of cancer-related death worldwide. Early diagnosis can effectively reduce the mortality, and computer-aided diagnosis (CAD) as an important way to assist doctors has developed rapidly. In particular, automated pulmonary nodule detection in computed tomography (CT) images is crucial to CAD. It is a challenging task to quickly locate the exact positions of lung nodules. In this paper, a novel automated pulmonary nodule detection framework with 2D convolutional neural network (CNN) is proposed to assist the CT reading process. Firstly, we adjust the structure of Faster R-CNN with two region proposal networks and a deconvolutional layer to detect nodule candidates, and then three models are trained for three kinds of slices for later result fusion. Secondly, a boosting architecture based on 2D CNN is designed for false positive reduction, which is a classifier to distinguish true nodules from the candidates. The misclassified samples are still kept for retraining a model which boosts the sensitivity for pulmonary nodule detection. Finally, the results of these networks are fused to vote out the final classification results. Extensive experiments are conducted on LUNA16, and the sensitivity of nodule candidate detection achieves 86.42%. For the false positive reduction, the sensitivity reaches 73.4% and 74.4% at 1/8 and 1/4 FPs/scan, respectively. It illustrates that the proposed method can obviously achieve accurate pulmonary nodule detection.

Abstract:Lung cancer is one of the most diagnosable forms of cancer worldwide. The early diagnoses of pulmonary nodules in computed tomography (CT) chest scans are crucial for potential patients. Recent researches have showed that the methods based on deep learning have made a significant progress for the medical diagnoses. However, the achievements on identification of pulmonary nodules are not yet satisfactory enough to be adopted in clinical practice. It is largely caused by either the existence of many false positives or the heavy time of processing. With the development of fully convolutional networks (FCNs), in this study, we proposed a new method of identifying the pulmonary nodules. The method segments the suspected nodules from their environments and then removes the false positives. Especially, it optimizes the network architecture for the identification of nodules rapidly and accurately. In order to remove the false positives, the suspected nodules are reduced using the 2D models. Furthermore, according to the significant differences between nodules and non-nodules in 3D shapes, the false positives are eliminated by integrating into the 3D models and classified via 3D CNNs. The experiments on 1000 patients indicate that our proposed method achieved 97.78% sensitivity rate for segmentation and 90.1% accuracy rate for detection. The maximum response time was less than 30 s and the average time was about 15 s.

Abstract:Computer-aided Diagnosis(CADx) plays an important role in interpretation of medical images. In various kinds of medical images, Computed Tomography(CT) images are able to provide pivotal evidence for diagnosis of lung cancer. Especially, Early-stage lung cancer can be discovered by detecting small pulmonary nodules in patient's CT slices. Thus, it is worthwhile to develop pulmonary nodules detection algorithm. In this paper, we introduce a pulmonary nodules detection algorithm based on object detection algorithm, with three improved techniques that focus on characteristics of pulmonary nodules detection algorithm. First, we use 3D-convolutional layers to process CT images instead of 2D-convolutional layers. Second, We designed a powerful encoder to extract image features, which can train with a small batch for active learning or retraining by hospital with limited computing resources. Third, we propose filterRPN for filtering a large number of invalid anchors and accelerate the training process. Finally, we use some experiments to show that our model got the start-of-the-art results.
A Novel Pulmonary Nodule Detection Model Based on Multi-Step Cascaded Networks

Abstract:Pulmonary nodule detection in chest computed tomography (CT) is of great significance for the early diagnosis of lung cancer. Therefore, it has attracted more and more researchers to propose various computer-assisted pulmonary nodule detection methods. However, these methods still could not provide convincing results because the nodules are easily confused with calcifications, vessels, or other benign lumps. In this paper, we propose a novel deep convolutional neural network (DCNN) framework for detecting pulmonary nodules in the chest CT image. The framework consists of three cascaded networks: First, a U-net network integrating inception structure and dense skip connection is proposed to segment the region of lung parenchyma from the chest CT image. The inception structure is used to replace the first convolution layer for better feature extraction with respect to multiple receptive fields, while the dense skip connection could reuse these features and transfer them through the network. Secondly, a modified U-net network where all the convolution layers are replaced by dilated convolution is proposed to detect the “suspicious nodules” in the image. The dilated convolution can increase the receptive fields to improve the ability of the network in learning global information of the image. Thirdly, a modified U-net adapting multi-scale pooling and multi-resolution convolution connection is proposed to find the true pulmonary nodule in the image with multiple candidate regions. During the detection, the result of the former step is used as the input of the latter step to follow the “coarse-to-fine” detection process. Moreover, the focal loss, perceptual loss and dice loss were used together to replace the cross-entropy loss to solve the problem of imbalance distribution of positive and negative samples. We apply our method on two public datasets to evaluate its ability in pulmonary nodule detection. Experimental results illustrate that the proposed method outperform the state-of-the-art methods with respect to accuracy, sensitivity and specificity.

Abstract:In recent years nodule candidate detection becomes the basis of the automated pulmonary nodule detection system, of which the the upper bound limit performance is determined by the sensitivity of nodule candidates detection. This paper is to improve the nodule candidate detection using deep neural networks. We treat the nodule detection task as pixel-level segmentation problem. Based on the 2D U-NET network. We build a multi-level network to process each CT slice to detect more nodules. Weighted dice loss function is designed to maintain a high sensitivity. More important, different from normaly segmentation problem, it has a heavily unbalanced positive and negative samples. We proposed a training method to make the network converge easily. We further propose an effective nonmaximum suppression (NMS) method to remove duplicate nodules. The proposed framework has been validated on LUNA16 dataset. We achieved 94.3% sensitivity score, and had a 1/3 times of false positives less than the official methods of LUNA which is better for false positive reduction task. We provide a deep neural network solution for nodule candidate detection and the experimental result demonstrates the effectiveness of our method. It can also be used for input of the false positive reduction task.

Abstract:Pulmonary nodule is a common lung disease, which can be prone to misdiagnosis and missed diagnosis. With the extensive application of CT technology, doctor's diagnostic efficiency has been greatly improved. However, the amount of CT image data is relatively large. Radiologists have to take a lot of time to read these images, and easy to overlook some minor lesions. Computer aided detection technology is an effective way to improve the efficiency and quality of the doctor's diagnosis. This paper put forward a kind of lung segmentation method based on morphology and statistic of size of the image area, while effectively eliminating the influence of the trachea to pulmonary parenchyma image segmentation. We also propose a method of region of interest(ROI) extraction based on morphology and circular filter, reducing the number of false positive and trying to retain the integrity of the ROI form. Finally, we have realized a reliable lung nodules compute aided diagnosis application on CT image, using Convolution neural network.

Abstract:This research studies a lung nodule classification problem using 3D computed tomography (CT) images for computer-aided diagnosis (CAD) system by applying deep convolutional neural networks (CNNs). Lung cancer is one of the most common cancers with a high mortality rate. There is an urgent need to diagnose lung nodules at an early stage to improve the survival rate of lung cancer patients. Through a CAD system, a complementary analysis result can be provided to radiologists to enhance the diagnosis process. The diagnosis performance of conventional CAD systems primarily relies on the results of previous analysis processes, such as nodule segmentation and feature extraction. In this research, the nodule segmentation and feature extraction in CAD system are eliminated by performing an end-to-end classification from raw 3D nodule CT patches using CNNs. Specifically, the state-of-artCNN models, e.g., VGG16, VGG19, ResNet50, DenseNet121, MobileNet, Xception, NASNetMobile, and NASNetLarge, are modified to 3D-CNN models and explored on a public CT lung dataset: Lung Image Database Consortium and image database resource initiative (LIDC-IDRI). Experimental results show that DenseNet121 and Xception achieve better results for lung nodule diagnosis regarding accuracy, sensitivity, specificity, precision, and area under the curve (AUC).

Abstract:Automatic pulmonary nodule detection is the most crucial technology in early diagnosis of lung cancer and early treatment. However, the various nodule type, shape and size, especially the diameter of lung nodules (ranging from 3 mm to 30 mm), make the lung nodule detection results with high false positives. It significantly affects the detection performance of lung nodules. In this paper, an adaptive and attentive 3D Convolutional Neural Network (CNN) is proposed for automatic pulmonary nodule detection, which contains two parts: the candidate nodule detection and false positive reduction. In the first stage, the globule, spital and fine-grained information of focal nodules are scratched by the high-resolution fused attention module in the proposed method. In the second stage, an adaptive 3D CNN structure is designed to further reduce the false positives, which extracts the multilevel contextual information via an adaptive 3D convolution kernel. Extensive experiments are conducted on publicly available LUNA16. The results demonstrate that the proposed method can increase the sensitivity and decrease the false positives rate for automated pulmonary nodule detection effectively.

Abstract:Early detection of pulmonary nodules in computed tomography (CT) images is essential for successful outcomes among lung cancer patients. Much attention has been given to deep convolutional neural network (DCNN)-based approaches to this task, but models have relied at least partly on 2D or 2.5D components for inherently 3D data. In this paper, we introduce a novel DCNN approach, consisting of two stages, that is fully three-dimensional end-to-end and utilizes the state-of-the-art in object detection. First, nodule candidates are identified with a U-Net-inspired 3D Faster R-CNN trained using online hard negative mining. Second, false positive reduction is performed by 3D DCNN classifiers trained on difficult examples produced during candidate screening. Finally, we introduce a method to ensemble models from both stages via consensus to give the final predictions. By using this framework, we ranked first of 2887 teams in Season One of Alibaba's 2017 TianChi AI Competition for Healthcare.

Highly accurate model for prediction of lung nodule malignancy with CT scans

Abstract:Computed tomography (CT) examinations are commonly used to predict lung nodule malignancy in patients, which are shown to improve noninvasive early diagnosis of lung cancer. It remains challenging for computational approaches to achieve performance comparable to experienced radiologists. Here we present NoduleX, a systematic approach to predict lung nodule malignancy from CT data, based on deep learning convolutional neural networks (CNN). For training and validation, we analyze >1000 lung nodules in images from the LIDC/IDRI cohort. All nodules were identified and classified by four experienced thoracic radiologists who participated in the LIDC project. NoduleX achieves high accuracy for nodule malignancy classification, with an AUC of ~0.99. This is commensurate with the analysis of the dataset by experienced radiologists. Our approach, NoduleX, provides an effective framework for highly accurate nodule malignancy prediction with the model trained on a large patient population. Abstract:Deep learning techniques have been extensively used in computerized pulmonary nodule analysis in recent years. Many reported studies still utilized hybrid methods for diagnosis, in which convolutional neural networks (CNNs) are used only as one part of the pipeline, and the whole system still needs either traditional image processing modules or human intervention to obtain final results. In this paper, we introduced a fast and fully-automated end-to-end system that can efficiently segment precise lung nodule contours from raw thoracic CT scans. Our proposed system has four major modules: candidate nodule detection with Faster regional-CNN (R-CNN), candidate merging, false positive (FP) reduction with CNN, and nodule segmentation with customized fully convolutional neural network (FCN). The entire system has no human interaction or database specific design. The average runtime is about 16 s per scan on a standard workstation. The nodule detection accuracy is 91.4% and 94.6% with an average of 1 and 4 false positives (FPs) per scan. The average dice coefficient of nodule segmentation compared to the groundtruth is 0.793.

Abstract:With the increasing number of lung cancer patients, the CAD system is playing an increasingly important rule in the field of automatic identification for medical images. Since the 3D characteristics of low-dose CT images make the 3D convolution more suitable than 2D convolution, in this paper, we propose a method to detect lung nodule of lung CT images using 3D convolutional neural networks. Combined with the traditional morphological preprocessing methods, 3D convolutional neural networks are applied to lung CT images. The experimental accuracy indicates that this method is suitable for the problem of lung nodule detection and has great room to improve. The experimental results also demonstrate that the application of deep learning in the medical field will bring great progress for medical development.

Abstract:Ever since its first success in large scale image recognition problem, deep convolutional neural networks (DCNNs) have shown their capabilities in solving many challenging visual perceptual tasks, such as image classification, segmentation, object detection. In some cases, DCNNs have already achieved near-human performance. In medical image analysis, DCNNs have also been successfully applied to lesion detection, segmentation, and diagnosis. The power of DCNN lies in its ability to learn a hierarchical representation of raw input data, without hand-crafted features. The focus of this study was to improve the performance of DCNN in automatic detection of pulmonary nodule on CT scan. In particular, nodules whose size is less than 4mm were considered. A total of 171 scans were collected at Zhongshan Hospital Fudan University, which was first process by the proposed DCNN system (12Sigma). The detection results were then carefully reviewed by an experienced physician, all false positives and missed nodules were manually labeled. To refine the system, all false positives and true nodules of sizes 4mm and under were selected. The data set was randomly split into training and test sets, where the training set consists of 90% (154 scans) and the test set consists of 10% of data (17 scans). Fig 1 shows the FROC curves on the test set before and after re-training. Overall, the detection sensitivity improved at all false positive levels, but the improvement was most significant at low FP rate region. For example, when FP is 0.5 per scan, the detection sensitivity increased from 0.36 to 0.49. This improvement suggests that, even with limited data, the deep neural network can learn from its mistakes and be easily tuned to be more sensitive to small nodules. We believe that when more data is collected, more significant improve in high FP rate region will be observed.
Automatic Lung Nodule Detection Using a 3D Deep Convolutional Neural Network Combined with a Multi-Scale Prediction Strategy in Chest CTs

Abstract:Objective: A novel computer-aided detection (CAD) scheme for lung nodule detection using a 3D deep convolutional neural network combined with a multi-scale prediction strategy is proposed to assist radiologists by providing a second opinion on accurate lung nodule detection, which is a crucial step in early diagnosis of lung cancer. Method: A 3D deep convolutional neural network (CNN) with multi-scale prediction was used to detect lung nodules after the lungs were segmented from chest CT scans, with a comprehensive method utilized. Compared with a 2D CNN, a 3D CNN can utilize richer spatial 3D contextual information and generate more discriminative features after being trained with 3D samples to fully represent lung nodules. Furthermore, a multi-scale lung nodule prediction strategy, including multi-scale cube prediction and cube clustering, is also proposed to detect extremely small nodules. Result The proposed method was evaluated on 888 thin-slice scans with 1186 nodules in the LUNA16 database. All results were obtained via 10-fold cross-validation. Three options of the proposed scheme are provided for selection according to the actual needs. The sensitivity of the proposed scheme with the primary option reached 87.94% and 92.93% at one and four false positives per scan, respectively. Meanwhile, the competition performance metric (CPM) score is very satisfying (0.7967). Conclusion: The experimental results demonstrate the outstanding detection performance of the proposed nodule detection scheme. In addition, the proposed scheme can be extended to other medical image recognition fields.

Abstract:Computed tomography (CT) generates a stack of cross-sectional images covering a region of the body. The visual assessment of these images for the identification of potential abnormalities is a challenging and time consuming task due to the large amount of information that needs to be processed. In this article we propose a deep artificial neural network architecture, ReCTnet, for the fully-automated detection of pulmonary nodules in CT scans. The architecture learns to distinguish nodules and normal structures at the pixel level and generates three-dimensional probability maps highlighting areas that are likely to harbour the objects of interest. Convolutional and recurrent layers are combined to learn expressive image representations exploiting the spatial dependencies across axial slices. We demonstrate that leveraging intra-slice dependencies substantially increases the sensitivity to detect pulmonary nodules without inflating the false positive rate. On the publicly available LIDC/IDRI dataset consisting of 1,018 annotated CT scans, ReCTnet reaches a detection sensitivity of 90.5% with an average of 4.5 false positives per scan. Comparisons with a competing multi-channel convolutional neural network for multi-slice segmentation and other published methodologies using the same dataset provide evidence that ReCTnet offers significant performance gains.
Novel convolutional neural network architecture for improved pulmonary nodule classification on computed tomography

Abstract:Computed tomography (CT) is widely used to locate pulmonary nodules for preliminary diagnosis of the lung cancer. However, due to high visual similarities between malignant (cancer) and benign (non-cancer) nodules, distinguishing malignant from malign nodules is not an easy task for a thoracic radiologist. In this paper, a novel convolutional neural network (ConvNet) architecture is proposed to classify the pulmonary nodules as either benign or malignant. Due to the high variance of nodule characteristics in CT scans, such as size and shape, a multi-path, multi-scale architecture is proposed and applied in the proposed ConvNet to improve the classification performance. The multi-scale method utilizes filters with different sizes to more effectively extracted nodule features from local regions, and the multi-path architecture combines features extracted from different ConvNet layers thereby enhancing the nodule features with respect to global regions. The proposed ConvNet is trained and evaluated on the LUNGx Challenge database, and achieves a sensitivity of 0.887 and a specificity of 0.924 with an area under the curve (AUC) of 0.948. The proposed ConvNet achieves a 14% AUC improvement compared to the state-of-the-art unsupervised learning approach. The proposed ConvNet also outperforms the other state-of-the-art ConvNets explicitly designed for pulmonary nodule classification. For clinical usage, the proposed ConvNet could potentially assist the radiologists to make diagnostic decisions in CT screening.
