Abstract:Hyperkalaemia is one of the common electrolyte imbalances dealt with in the emergency department and is caused by extracellular accumulation of potassium ions above normal limits usually greater than 5.0-5.5 mmol/L. It is found in a total of 1-10% of hospitalised patients usually associated with chronic kidney disease and heart failure. The presentation can range from being asymptomatic to deadly arrhythmias. The appearance of symptoms depends on the rate of change rather than just the numerical values. The rare presentation includes periodic paralysis characterised by the sudden onset of short-term muscle weakness, stiffness or paralysis. Management goals are directed towards reducing potassium levels in emergency settings and later on avoiding the triggers for future attacks. In this case, we present a man in his 50s with the generalised weakness later on diagnosed as hyperkalaemic periodic paralysis secondary to tumour lysis syndrome. Emergency physicians dealing with common electrolyte imbalances should keep a sharp eye on their rare presentation and their precipitating factors and should act accordingly.
Exercise Test for Patients with New-Onset Paroxysmal Kinesigenic Dyskinesia

Abstract:The pathogenesis of primary paroxysmal kinesigenic dyskinesia (PKD) remains unclear, and channelopathy is a possibility. In a pilot study, we found that PKD patients had abnormal exercise test (ET) results. To investigate the ET performances in patients affected by PKD, and the role of the channelopathies in the pathogenesis of PKD, we compared the ET results of PKD patients, control subjects, and hypokalemic periodic paralysis (HoPP) patients, and we analyzed ET changes in 32 PKD patients before and after treatment. Forty-four PKD patients underwent genetic testing for the PRRT2, SCN4A, and CLCN1 genes. Sixteen of 59 (27%) patients had abnormal ET results in the PKD group, while 28 of 35 (80%) patients had abnormal ET results in the HoPP group. Compared with the control group, the PKD group showed a significant decrease in the compound muscle action potential (CMAP) amplitude and area after the long ET (LET), while the HoPP group showed not only greater decreases in the CMAP amplitude and area after the LET but also greater increases in the CMAP amplitude and area immediately after the LET. The ET parameters before and after treatment were not significantly different. Nine of 44 PKD patients carried PRRT2 mutations, but the gene abnormalities were unrelated to any ET parameter. The PKD group demonstrated an abnormal LET result by electromyography (EMG), and this abnormality did not seem to correlate with the PRRT2 variant or sodium channel blocker therapy.
Severe Hypokalemia ECG Changes Mimicking Those of Acute Coronary Syndrome (ACS) in Patient with Underlying Ischaemic Heart Disease: A Case Review

Abstract:Background: Hypokalemia is seen on regular basis in medical emergency. The definition of hypokalemia is serum potassium level below 3.5 mmol/L, meanwhile severe hypokalemia is serum potassium level below 2.5 mmol/L [1]. Patient with hypokalemia can present with wide range of presentation including musculoskeletal complaints from numbness to acute paralysis. Severe hypokalemia has tendency to progress to intestinal paralysis and respiratory failure. In some cases of hypokalemia, cardiovascular system can also be affected causing cardiac arrhythmias and heart failure [2]. Aim: This case report is to highlight that severe hypokalaemia can present with ECG changes mimicking acute coronary syndrome (ACS) which was fully resolved with correction of potassium level. Methods: We report a case of 84 years old Chinese man with underlying triple vessel disease presented with generalised body weakness for 2 days. ECG on arrival noted changes suggestive of ACS with ST segment depression in lead V4-V6 with first degree heart block, however patient had no ischemic symptoms and the potassium level was severe low at 1.6 mmol/L (3.5 - 5.1 mmol/L). He was correctly not treated for ACS. Outcomes: Repeated ECG post fast intravenous potassium correction noted complete resolution of the ST segment depression and first degree heart block. Patient discharged well from hospital four days later with potassium level of 3.8 mmol/L. Conclusions: Severe hypokalemia with asymptomatic ECG of ACS changes can safely be treated as a single entity clinical emergency with good resolution and no complication after normalizing potassium level.

Abstract:Objective To investigate the clinical characteristics of hypokalemic periodic paralysis(HOPP)and explore the diagnosis and treatment of HOPP.Methods The clinical data of 46 cases with HOPP were analyzed and summarized retrospectively.Results There was an inducement among mostly cases,and the symptoms occurred mainly in insomnia or early awakening.The classical appearance was sluggish myoasthenia of four limbs and hypopotassemia.Serious HOPP experienced respiratory paralysis.The symptoms of all cases relieved completely after intravenous or oral supplement of potassium.Some patients did not show the above typical clinical manifestations;the transient hyperkalemia could be observed in the course of treatment.Conclusions The understanding to untypical clinical features of hypokalemic periodic paralysis is helpful to the early diagnosis and timely treatment.

Abstract:Background: Periodic paralysis is a rare neuromuscular disorder characterized by episodes of painless muscle weakness. Hypokalemic periodic paralysis (HPP) is the most common subtype of periodic paralysis. HPP may be an acquired illness but, most often, it is associated with an inherited abnormality of calcium or sodium ion channels.Episodes of HPP can be triggered by multiple factors, most commonly strenuous exercise or consuming large amounts of carbohydrates. Other triggers include emotional factors, environmental factors, other dietary factors, toxins, and medicines.Hypokalemia may be due to potassium wasting or intracellular sequestration of potassium. When ordering potassium repletion, it is important to identify hypokalemia due to sequestration in order to avoid iatrogenic hyperkalemia.Case Report; We report the cases of two brothers with previously undiagnosed familial HPP with flaccid paralysis of their limbs after receiving illicit intramuscular injections of betamethasone. Serum potassium concentrations were 1.6mmol/L and 1.9mmol/L. Both ECGs demonstrated U-waves and the older brother's ECG demonstrated widening of the QRS.The brothers were treated with oral and intravenous potassium supplementation. After briefly becoming hyperkalemic, their serum potassium concentrations returned to normal, and their paralysis resolved within 24 hours.

Abstract:Abstract: Introduction: Distal renal tubular acidosis (dRTA) is a metabolic disease characterized by hypokalemia, hyperchloremic metabolic acidosis and urine pH above 5.5. These findings may be accompanied by hypercalciuria, nephrocalcinosis, nephrolithiasis, jaundice, osteomalacia or rickets in children. Although hypokalemia is frequently seen as a laboratory finding in dRTA, weakness, which is the clinical finding of this deficiency, is rare. Case Report: A 33-year-old female patient was brought to the emergency department (ED) with complaints of weakness, loss of strength in the extremities, and difficulty in breathing. Laboratory analyzes of the patient revealed metabolic acidosis and hypokalemia. Urea and creatinine values were normal. The patient was admitted to the internal medicine department with a preliminary diagnosis of dRTA and hypokalemic paralysis. Initially, parenteral infusion of KCl and NaHCO3 was administered in the treatment. In the follow-up of the patient, it was observed that hypokalemia and metabolic acidosis improved from the 3rd day and clinical findings improved within 36 hours following the replacement therapy. Conclusion: dRTA, which is rare in adults, is among the secondary causes of hypokalemic paralysis. dRTA should be considered among the differential diagnoses in the presence of hypokalemia and metabolic acidosis in patients presenting with bilateral weakness. [Clinical Features of Hypokalemic Periodic Paralysis].

Abstract:OBJECTIVE:To explore the clinical features of hypokalemic periodic paralysis, and compare clinical features of primary group with those of thyrotoxicosis secondary group.METHODS:Clinical data of 44 patients with hypokalemic periodic paralysis in Peking University First Hospital from 1996 December to 2008 December were retrospectively analyzed.RESULTS:There were 22 patients in primary group, and 22 in thyrotoxicosis group. Identical clinical features of both the groups: (1)It had a predilection in young men. (2)Main symptoms were limb movement disorder and fatigue, and paralysis recurrent attacked in most patients. (3) 40.9% to 68.2% patients had obvious incentives, and the common ones were a heavy meal, sweet drinks, or strenuous exercise. (4) Serum potassium levels of the two groups were obviously lower than the normal range. (5)In 20% patients of primary group and 25% patients of thyrotoxicosis secondary group, CK levels were higher than normal, while LDH and HBDH levels were normal. (6)The doses of potassium replishment were not correlated to serum potassium levels at the onset. Different clinical features of the two groups: (1) Patients of thyrotoxicosis group had hypermetabolism symptoms and thyroid dysfunction. Patients of primary group had no hypermetabolism symptoms, and all of them were euthyroid. (2)Serum potassium levels of thyrotoxicosis secondary group were lower than those of primary group significantly [(2.25 +/- 0.67) vs (2.78 +/- 0.49) mmol/L, P=0.007]. (3) Hyperkalemia is easier than primary group to rebound in thyrotoxicosis secondary group, after replenishment of potassium.CONCLUSION:Hypokalemic periodic paralysis has its clinical features, and patients with early diagnosis and replenishment of potassium in time have good prognosis. The doses of potassium replenishment are not determined by serum potassium levels at the onset. Hyperkalemia is easier to rebound in thyrotoxicosis secondary group after replenishment of potassium, serum potassium levels should be monitored closely, and hyperthyrosis radically cured.

Abstract:Potassium is an important ion capable to maintain intra-extracellular electric gradient. Hyperkalemia is a common and potential life-threatening electrolyte disorder in patients presenting to the emergency setting. Variations in the intra-extracellular ionic flow may alter cells functions, skeletal and smooth muscle contractility and electric activity of myocardial cells. Hyperkalemia can be difficult to diagnose clinically because symptoms may be vague. Patients may be asymptomatic or report non-specific symptoms such as generalized fatigue, weakness, paralysis or palpitations. Syncope is unusual neurological manifestation. An increase in serum potassium levels is followed by progressively severe electrophysiological derangements in cardiac impulse generation and conduction, which are reflected in the electrocardiogram (ECG).
[Intraoperative Management of Potentially Fatal Arrhythmias after Anesthesia Induced by Severe Hypokalemia: A Case Report].

Abstract:Severe hypokalemia is defined as the concentration of serum potassium lower than 2.5 mmol/L, which may lead to serious arrhythmias and cause mortality. We report an unusual case of potentially fatal ventricular arrhythmias induced by severe hypokalemia in a patient undergoing laparoscopic partial nephrectomy in Peking University Third Hospital due to irregular use of indapamide before operation. Indapamide is a sulfonamide diuretic with vasodilative and calcium antagonistic effects, which enhances sodium delivery to the renal distal tubules resulting in a dose-related increase in urinary potassium excretion and decreases serum potassium concentrations. The electrolyte disorder caused by the diuretic is more likely to occur in the elderly patients, especially those with malnutrition or long-term fasting. Hence, the serum potassium concentration of the patients under indapamide therapy, especially elderly patients, should be monitored carefully. Meanwhile, the potassium concentration measured by arterial blood gas analysis is different from that measured by venous blood or laboratory test. According to the previous research, the concentration of potassium in venous blood was slightly higher than that in arterial blood, and the difference value was 0.1-0.5 mmol/L. This error should be taken into account when rapid intravenous potassium supplementation or reduction of blood potassium level was carried out clinically. In the correction of severe hypokalemia, the standard approach often did not work well for treating severe hypokalemia. The tailored rapid potassium supplementation strategy shortened the time of hypokalemia and was a safe and better treatment option to remedy life-threatening arrhythmias caused by severe hypokalemia with a high success rate. Through the anesthesia management of this case, we conclude that for the elderly patients who take indapamide or other potassium excretion diuretics, the electrolyte concentration and the general volume state of the patients should be comprehensively measured and fully evaluated before operation. It may be necessary for us to reexamine the serum electrolyte concentration before anesthesia induction on the morning of surgery in patients with the history of hypokalemia. For severe hypokalemia detected after anesthesia, central venous cannulation access for individualized rapid potassium supplementation is an effective approach to reverse the life-threatening arrhythmias caused by severe hypokalemia and ensure the safety of the patients.

Abstract:Introduction : Thyrotoxic periodic paralysis (TPP) is a type of hypokalemic periodic paralysis that is caused by an underlying thyrotoxicosis. It is a rare cause of hypokalemia due to intracellular potassium shift, causing acute muscle weakness. Case presentation : We present a case of a 19-year-old male of Thai descent with acute proximal symmetric lower limb weakness. The combination of these symptoms with profound hypokalemia, rapid recovery after normalization of serum potassium, and evidence of hyperthyroidism led to the diagnosis of thyrotoxic periodic paralysis, in this case due to an underlying Graves' disease. Conclusion : Clinicians should consider the diagnosis of TPP when a patient presents with the triad of acute paresis, profound hypokalemia and hyperthyroidism.

Abstract:Electrolyte imbalances, particularly potassium disorders, are common in clinical practice. Potassium homeostasis plays a key role in regulating cell membrane excitability. Hypokalemia usually presents with cardiovascular and neuromuscular abnormalities. Hypokalemia can lead to clinically significant life-threatening arrhythmia. Typical electrocardiographic (ECG) features of hypokalemia include widespread ST depression, T wave inversion, and prominent U waves. However, hypokalemia may present with different types of arrhythmia, such as premature ventricular contractions, ventricular fibrillation, atrial fibrillation, and torsade de pointes. Thus, clinicians should be familiar with ECG manifestations of potassium disorders that may warrant timely diagnosis and effective management. Herein, we report three patients with arrhythmia who were found to have typical ECG characteristics of hypokalemia after resolution of arrhythmia and later proved to have low serum potassium levels.
Diagnosis, management and outcomes of primary hypokalemic periodic paralysis during pregnancy

Abstract:Primary hypokalaemic periodic paralysis during pregnancy has been rarely reported. Four pregnant women with the acute onset of flaccid paralysis presented between January 2018 and December 2021. Focussed history and physical examination helped an appropriate radiological and laboratory investigation plan to be made. All women recovered within 4-7 days of potassium supplementation. Supplemental potassium continued until delivery. A pain management plan with continuous epidural infusion helped in avoiding stress-induced hypokalaemia. None of the women developed an episode of muscle weakness during the intervening period. In conclusion, a focussed history and targeted laboratory investigation are needed to diagnose primary hypokalaemic periodic paralysis. Early administration of oral or intravenous potassium is crucial in improving fetomaternal outcomes.
Hypokalemia-Induced Life-Threatening Arrhythmia in a Patient With Congestive Heart Failure

Abstract:Ventricular tachyarrhythmias are common in patients with heart failure. It is one of the important preventable causes of death in these patient populations. Hypokalemia is prevalent in patients with heart failure due to various reasons. Hypokalemia can trigger ventricular arrhythmias through different mechanisms. In this case report, we present a middle-aged man with congestive heart failure (CHF) and an automated intracardiac defibrillator (AICD) on multiple diuretic medications (unintended) who presented with acute chest pain. He was found to have severe hypokalemia, hyponatremia, and an acute kidney injury. Interrogation of the AICD revealed multiple episodes of ventricular fibrillation. The patient was managed by holding his diuretic medications, cautious volume repletion, and potassium replacement. Fortunately, the patient showed rapid clinical improvement and his plasma potassium level improved. On discharge, we reconciled the patient's medications to avoid the recurrence of hypokalemia from over-diuresis and arranged a close follow-up outpatient visit with his cardiologist.
X-ray diffraction structure analysis of MCM-48 mesoporous silica.

Abstract:The structure of MCM-48 mesoporous silicate materials has been fully characterized from X-ray diffraction data by applying recently developed methods of mesostructure analysis and full-profile refinement. The pore wall thickness of both as-made and calcined MCM-48 was determined with high precision to be 8.0(1) Angstrom. No regular variations of the wall thickness were detected, but its density was found to be ca. 10% higher in the low-curvature regions. The surfactant density in the pores was assessed around 0.6-0.7 g/cm(3) and was found to have a distinct minimum in the pore center similar to that detected previously in MCM-41. A new extended model function of the density distribution in MCM-48 is proposed on the basis of the structural features that were revealed.
The Paradox of Hyperkalaemia: When Treatment Isn't the Answer

Abstract:Hyperkalaemia is a relatively common medical emergency that necessitates prompt and urgent intervention. There is an ongoing debate over the precise threshold for treating hyperkalaemia due to variability in clinical scenarios. This case report highlights the need to differentiate true hyperkalaemia from pseudohyperkalaemia by analysing serum and plasma potassium levels, thus avoiding unnecessary treatment and the risk of iatrogenic hypokalemia. This case report discusses an 89-year-old male who presented with recurrent falls and fluctuating serum potassium levels but showed no symptoms of hyperkalaemia and had no relevant drug history. Further investigation revealed an underlying myeloproliferative neoplasm with thrombocytosis, leading to the diagnosis of pseudohyperkalaemia, reflected by a significant discrepancy between serum and plasma potassium levels, showcasing the importance of considering pseudohyperkalaemia in patients with haematological malignancies and thrombocytosis.

Abstract:Hyperkalemia is one of the most common electrolyte abnormalities seen in clinical practice and the most common life-threatening electrolyte abnormality seen in the emergency department. It is most often due to impaired renal potassium excretion due to acute on chronic kidney disease or the use of drugs that inhibit the renin-angiotensin-aldosterone axis. The most common clinical presentation is muscle weakness and cardiac conduction abnormalities. In the Emergency Department, ECG can come in handy as the first diagnosis of hyperkalemia before labs are drawn and reported. Early recognition of electrocardiographic (ECG) changes can prompt early correction and reduce mortality. We hereby, present a case of transient left bundle branch block in the setting of hyperkalemia secondary to statin-induced rhabdomyolysis.

Abstract:Demand for nutrients necessary for the formation of mineralized tissues increases dramatically during pregnancy to meet fetal requirements. The purpose of this clinical study was to identify short-term effects of pregnancy and dietary intakes of calcium, phosphorus, protein, and vitamin C on radiographic density and alveolar crest morphology of the mandible. Seventy-six subjects between ten and 20 weeks' gestational age were recruited from the Jefferson County, Alabama, Department of Health maternity clinic. Initially, an evaluation of their diet was done, and a single periapical radiograph was taken in the canine-premolar region of the mandible. A final evaluation of the diet and a periapical radiograph of the same teeth were obtained at a time as close to the expected date of delivery as possible. Dietary evaluation consisted of a 24-hour recall history and a food-frequency history at the two visits. Radiographs were made with a bite-block film-holding system with occlusal registration in acrylic for reproduction of projection geometry at the two visits. An aluminum stepwedge was incorporated for densitometric standardization. Measurements of plaque, and bleeding and pocket-depth probing were recorded for control of local effects on alveolar change. Standard statistical procedures of regression determined correlation of 12 nutrients and periodontal variables with bone density change and with alveolar crest morphology change. Vitamin C intakes (24-hour recall) showed a positive correlation (p = 0.033) with bone density change, and calcium intakes (24-hour recall) showed a negative, but not significant, correlation (p = 0.058) with bone density change, contributed particularly by subjects with the highest calcium intakes. The strongest relation with alveolar crest morphology change was the interproximal pocket-depth change (p = 0.086).
Wrist Accelerometer Cut Points for Classifying Sedentary Behavior in Children

Abstract:Introduction This study aimed to examine the validity and accuracy of wrist accelerometers for classifying sedentary behavior (SB) in children. Methods Fifty-seven children (5–8 and 9–12 yr) completed an ~170-min protocol, including 15 semistructured activities and transitions. Nine ActiGraph (GT3X+) and two GENEActiv wrist cut points were evaluated. Direct observation was the criterion measure. The accuracy of wrist cut points was compared with that achieved by the ActiGraph hip cut point (⩽25 counts per 15 s) and the thigh-mounted activPAL3TM. Analyses included equivalence testing, Bland–Altman procedures, and area under the receiver operating curve (ROC-AUC). Results The most accurate ActiGraph wrist cut points (Kim; vector magnitude, ⩽3958 counts per 60 s; vertical axis, ⩽1756 counts per 60 s) demonstrated good classification accuracy (ROC-AUC = 0.85–0.86) and accurately estimated SB time in 5–8 yr (equivalence P = 0.02; mean bias = 4.1%, limits of agreement = −20.1% to 28.4%) and 9–12 yr (equivalence P < 0.01; −2.5%, −27.9% to 22.9%). The mean bias of SB time estimates from Kim were smaller than ActiGraph hip (5–8 yr: 15.8%, −5.7% to 37.2%; 9–12 yr: 17.8%, −3.9% to 39.5%) and similar to or smaller than activPAL3TM (5–8 yr: 12.6%, −39.8% to 14.7%; 9–12 yr: −1.4%, −13.9% to 11.0%), although classification accuracy was similar to ActiGraph hip (ROC-AUC = 0.85) but lower than activPAL3TM (ROC-AUC = 0.92–0.97). Mean bias (5–8 yr: 6.5%, −16.1% to 29.1%; 9–12 yr: 10.5%, −13.6% to 34.6%) for the most accurate GENEActiv wrist cut point (Schaefer: ⩽0.19 g) was smaller than ActiGraph hip, and activPAL3TM in 5–8 yr, but larger than activPAL3TM in 9–12 yr. However, SB time estimates from Schaefer were not equivalent to direct observation (equivalence P > 0.05) and classification accuracy (ROC-AUC = 0.79–0.80) was lower than for ActiGraph hip and activPAL3TM. Conclusion The most accurate SB ActiGraph (Kim) and GENEActiv (Schaefer) wrist cut points can be applied in children with similar confidence as the ActiGraph hip cut point (⩽25 counts per 15 s), although activPAL3TM was generally more accurate.

Abstract:Five new cuprous halides formulated as [etpy][Cu(3)I(4)] 1, [mepy][Cu(2)Br(3)] 2, [mepy][Cu(2)I(3)] 3 [dmebpp][Cu(7)Br(9)] 4 and [dmeDABCO](4)[Cu(8)I(16)] 5 (etpy = N-ethylpyridyl; mepy = N-methylpyridyl; dmebpp = N,N'-dimethyl-1,3-bis(4-pyridyl)propane; dmeDABCO = N,N'-dimethyl-1,4-diazabicyclo-[2.2.2]octane) have been prepared by solvothermal reactions of a copper source, halides and N-heterocyclic ligands in alcoholic solution. 1 contains an unprecedented trigonal two-dimensional cuprous halide layer Cu(3)I(4)(-) constructed from trimeric Cu(3)I(8) units via sharing peripheral mu(3)-iodines; isostructural 2 and 3 contain infinite Cu(2)X(3)(-) chains in which the copper atoms are arranged into a ladder; 4 is composed of novel twofold helical cuprous halide chains Cu(7)Br(9)(2-) built up by CuBr(4) and CuBr(3); 5 contains an infinite edge-sharing tetrahedral chain with a periodic sequence of eight CuI(4) tetrahedra. In situ alkylation reactions of N-heterocycles with alcohols generated organic cationic templates, which played an important role in the control of the structures of anionic cuprous halides. A possible alkylation mechanism of N-heterocycles in the system has been proposed. The fluorescence properties of 1 and 4 were investigated. Time-dependent density functional theory (TD-DFT) calculations were carried out on the excited electronic states 1 of in order to understand the emission mechanism.
