Abstract:Background: Despite advances in targeted agent development, effective treatment of acute myeloid leukemia (AML) remains a major clinical challenge. The B-cell lymphoma-2 (BCL-2) inhibitor exhibited promising clinical activity in AML, acute lymphoblastic leukemia (ALL) and diffuse large B-cell lymphoma (DLBCL) treatment. APG-2575 is a novel BCL-2 selective inhibitor, which has demonstrated anti-tumor activity in hematologic malignancies. Homoharringtonine (HHT), an alkaloid, exhibited anti-AML activity. Methods: The synergistic effects of APG-2575 and HHT were studied in AML cell lines and primary samples. MTS was used to measure the cell viability. Annexin V/propidium iodide staining was used to measure the apoptosis rate by flow cytometry. AML cell xenografted mouse models were established to evaluate the anti-leukemic effect of BCL-2 inhibitor, HHT and their combination in vivo. Western blot was used to determine the expression of related proteins. Results: APG-2575 showed comparable anti-leukemic effect to the FDA-approved BCL-2 inhibitor ABT-199 in vitro and in vivo. Combined treatment of HHT with APG-2575 synergistically inhibited AML cell growth and engraftment. Mechanistically, HHT promoted degradation of myeloid cell leukemia-1 (MCL-1), which was reported to induce BCL-2 inhibitor resistant, through the PI3K/AKT/GSK3 beta signaling pathway. Conclusion: Our results provide an effective AML treatment strategy through combination of APG-2575 and HHT, which is worthy of further clinical research.

Abstract:Chronic lymphocytic leukemia (CLL) is a disease characterized by the accumulation of mature CD19+CD5+CD23+ B cells in the bloodstream and in lymphoid organs. It usually affects people over 70 years of age, which limits the options for treatments. The disease is typically well-managed, but to date is still incurable. Hence, the need for novel therapeutic strategies remains. Nurse-like cells (NLCs) are major components of the microenvironment for CLL, supporting tumor cell survival, proliferation, and even drug resistance. They are of myeloid lineage, guided toward differentiating into their tumor-supportive role by the CLL cells themselves. As such, they are analogous to tumor-associated macrophages and represent a major therapeutic target. Previously, it was found that a mushroom extract, Active Hexose-Correlated Compound (AHCC), promoted the death of acute myeloid leukemia cells while preserving normal monocytes. Given these findings, it was asked whether AHCC might have a similar effect on the abnormally differentiated myeloid-lineage NLCs in CLL. CLL-patient PBMCs were treated with AHCC, and it was found that AHCC treatment showed a direct toxic effect against isolated CLL cells. In addition, it significantly reduced the number of tumor-supportive NLCs and altered their phenotype. The effects of AHCC were then tested in the Eμ-TCL1 mouse model of CLL and the MllPTD/WT Flt3ITD/WT model of AML. Results showed that AHCC not only reduced tumor load and increased survival in the CLL and AML models, but it also enhanced antitumor antibody treatment in the CLL model. These results suggest that AHCC has direct and indirect effects against CLL and that it may be of benefit when combined with existing treatments.

Abstract:BACKGROUND:Existing research shows that ABT-199, as a first-line drug, have been widely used in hematological malignancies, especially in leukemia, but the clinical efficacy of single drug therapy was limited part of the reason was that BCL-2 inhibitors failure to target other anti-apoptotic BCL-2 family proteins, such as MCL-1. In this case, combination therapy may be a promising way to overcome this obstacle. Here, we investigate the preclinical efficacy of a new strategy combining ABT-199 with homoharringtonine (HHT), a selective inhibitor of MCL-1 may be a promising approach for AML treatment as these two molecules are important in apoptosis.METHODS:A Cell Counting Kit-8 (CCK8) assay and flow cytometry were used to determine the half-maximal inhibitory concentration (IC50) value and cell apoptosis rate, respectively. The flow cytometry results showed that combined treatment with HHT and ABT-199 caused apoptosis in AML patient samples (n=5) but had no effect on normal healthy donor samples (n=11). Furthermore, we used a Western blot assay to explore the mechanism underlying the efficacy of HHT combined with ABT-199. Finally, antileukemic activity was further evaluated in vivo xenograft model.RESULTS:Our results indicated that ABT-199 combined with HHT significantly inhibited cell growth and promoted apoptosis in both AML cell lines and primary AML tumors in a dose- and time-dependent manner. Moreover, HHT combined with ABT-199 suppressed AML cell growth and progression in vivo xenograft model.CONCLUSIONS:Our research found that HHT combined with ABT-199 exerted its anti-leukemia effect by inducing apoptosis through the treatment of AML in vitro and in vivo.
Homoharringtonine Affects the Jak2-Stat5 Signal Pathway Through Alteration of Protein Tyrosine Kinase Phosphorylation in Acute Myeloid Leukemia Cells

Abstract:Objectives: Homoharringtonine (HHT) was efficient in therapying patients with acute myeloid leukemia (AML) in China, but little is known about the mechanism of its action. As the abnormal activation of JAK2 associated pathway is important to AML, we try to explore the effect of HHT on JAK2-STAT pathway in AML cells, thus supplying theoretical basis for wider use of HHT. Methods: The cell viability was tested by MTT. Apoptosis was tested by flow cytometry. RT-PCR was used to measure the expression of JAK2, STAT5 and the effect gene Bcl-xL. The signal proteins such as p-JAK2, p-STAT5, p-AKT, p-ERK activated by abnormal activated JAK2 were tested by Western blotting. Results: HHT obviously inhibited the viability of primary AML cells and AML cell lines HEL, K562 and HL-60 cells, AnnexinV-PI double staining confirmed early apoptosis in a dose-dependent manner. In immunoblotting analysis, when AML cells were affected by HHT for 6 h (much ahead of the time when apoptosis could be induced). The expressions of p-JAK2, p-STAT5, and p-AKT were down-regulated, while the total JAK2, STAT5 and AKT protein levels were stable. There were no changes in p-ERK and BcL-xL proteins. When it prolonged to 24 h, Bcl-xL decreased obviously. Similar results were obtained by using JAK2 specific inhibitor AG490. Conclusions: HHT possibly acts as a broad-spectrum PTK inhibitor and inhibits the phosphorylation of the signal proteins caused by oncogenic proteins such as JAK2V617F, BCR/ABL, thus blocking the survival and proliferative signal pathway of malignant cells.
[The Efficacy and Safety of HAA Regimen As Induction Chemotherapy in 150 Newly Diagnosed Acute Myeloid Leukemia].

Abstract:OBJECTIVE:To explore the efficacy and safety of HAA regimen (homoharringtonine, cytarabine and aclarubicin) in the treatment of 150 newly diagnosed adult acute myeloid leukemia (AML).METHODS:All patients entered the study from May 1999 to June 2008 were treated with HAA regimen. Cox-survival analysis was used to estimate the survival rate and differences between M(1)/M(2) and M(4)/M(5) were compared with 2-sided log-rank test.RESULTS:Out of the 150 patients, 121 (81%) achieved complete remission (CR). After the first course, CR rate was 68%. The CR rates of 97%, 84% and 38% were achieved in patients with favorable, intermediate and unfavorable cytogenetics, respectively. For the patients with CR, the median follow-up time was 16.5 (1.5 - 100.5) months, and the estimated 3-year survival rate was 45%. The estimated 3-year relapse free survival rate was 52% for the 121 patients with CR.CONCLUSIONS:HAA regimen may be an efficacious and safe regimen with a good toleration in the induction therapy for newly diagnosed AML, and a high CR rate could be achieved with only one or two courses.
Potent antitumor activity of the Ad5/11 chimeric oncolytic adenovirus combined with interleukin-24 for acute myeloid leukemia via induction of apoptosis.

Abstract:The Ad5/11 chimeric oncolytic adenovirus represents a promising new platform for anticancer therapy. Acute myeloid leukemia (AML) is a heterogeneous clonal disorder of hematopoietic progenitor cells and is the most common malignant myeloid disorder in adults. Myeloid and other hematopoietic cell lineages are involved in the process of clonal proliferation and differentiation. In the present study, we aimed to ascertain whether chimeric oncolytic adenovirus-mediated transfer of the human interleukin-24 (IL-24) gene induces enhanced antitumor potency. Our results showed that the Ad5/11 chimeric oncolytic adenovirus carrying hIL-24 (AdCN205-11-IL-24) produced high levels of hIL-24 in AML cancer cells, as compared with the Ad5 oncolytic adenovirus expressing hIL-24 (AdCN205-IL-24). AdCN205-11-IL-24 specifically induced a cytotoxic effect on AML cancer cells, but had little or no effect on a normal cell line. AdCN205-11-IL-24 induced higher antitumor activity in AML cancer cells by inducing apoptosis in vitro. This study suggests that transfer of IL-24 by an Ad5/11 chimeric oncolytic adenovirus may be a potent antitumor approach for AML cancer therapy.
Homoharringtonine in Combination with Cytarabine and Aclarubicin in the Treatment of Refractory/Relapsed Acute Myeloid Leukemia: a Single-Center Experience

Abstract:Abstract 2595 Patients with refractory and relapsed acute myeloid leukemia (AML) have relatively unfavorable prognoses and require innovative therapeutic approaches. We evaluated the efficacy and toxicity profiles of homoharringtonine (HHT), cytarabine (Ara-c) and aclarubicin combination chemotherapy in 46 refractory and/or relapsed AML patients between February 2004 and December 2009. Eleven cases were primary refractory disease and rest of them had relapsing disease. After one course of HAA treatment, complete remission (CR) was achieved in 35 of 46 patients (76.1%), while 3 cases showed partial remission (PR) (6.5%) and 8 cases (17.4%) had resistant to this regimen. One PR and one NR patient obtained CR after second course of HAA, yielding a total CR rate was 80.4%. Hematological toxicity was the most prominent toxicity of this regimen. Grade IV hematologic toxicity occurred in 91.3% cases. Neutrophil recovery time was 13 (5–60) days. Nonhematologic complications were mild to moderate nausea, vomiting, and mucositis and could be controlled by routine measures. Early death (ED) did not occur in these 46 patients. There was no correlation between response rate and leukemia subtype (AML FAB subtype), leukocyte, blast count in bone marrow, platelet count and sex (P >0.05). However, poor karyotype and age (≥45 years old) were factors associated with increased risk of treatment failure. Of the 46 patients, one patient, who upon achieving CR after the first re-induction therapy refused further treatment, was ultimately lost to follow-up. The median overall survival rate (OS) was 29 (2–86) months in 45 cases. Relapse-free survival (RFS) was 36 (0.5–85) months in 36 CR cases. By the time of closeout, the estimated 3-year OS rate was 42% for all 45 cases, and 43% estimated RFS at 3 years for the 36 cases of CR. OS and RFS were influenced by leukemia-related factors and disease status: Hb <10g/dl and poor karyotype in both univariate and multivariate analyses. Allogeneic stem cell transplantation was performed in 7 patients and all achieved CR, but 1 patient died of GVHD, 2 died of pneumonia, 1 relapsed followed by death and another 3 are still alive with CR. At present, 19 patients are still alive and 18 of these are in continuous remission with one alive with leukemia. The rest (26/45) of the patients are all deceased. In conclusion, HAA is a good choice in cases with refractory/relapsing AML for salvage chemotherapy, preferably with a high efficacy and low-toxicity profile. Apatinib Enhances Chemosensitivity of FLT3-ITD Mutations acute Myeloid Leukemia Cells to Homoharringtonine Via VEGFR2 Pathway

Abstract:Abstract Purpose: The significance of vascular endothelial growth factor receptor (VEGFR)-3 in numerous solid tumors and acute myeloid leukemia (AML) has been demonstrated, but Apatinib remains largely unexplored. In this study, we explored whether Apatinib combined with HHT could kill AML cell lines and its possible mechanisms. Methods: All cell lines were treated with Apatinib and HHT in different concentrations with control, Apatinib alone, HHT alone, and Apatinib combined with HHT. The changes of IC50 were measured by CCK8 assay, apoptosis rate, cell cycle, and mitochondrial membrane potential in each group were measured by flow cytometry. Finally, the possible cytotoxicity mechanism was analyzed by Western blotting. Results: Our results noted that Apatinib combined with HHT remarkably inhibited cell proliferation, reduced the capacity of colony-forming, and induced apoptosis and cell cycle arrest in AML cells. Mechanistically, Apatinib and HHT play a role as a suppressor in the expression of VEGFR2 and the downstream signaling cascades, such as the PI3K, MAPK, and Stat3 pathways. Conclusion: Our preclinical data demonstrate that Apatinib combined with HHT exerts a better anti-leukemia effect than Apatinib alone by inhibiting the VEGFR2 signaling pathway, suggesting the potential role for Apatinib and HHT in the treatment of AML.
A novel alkylating deacetylase inhibitor molecule EDO-S101 in combination with cytarabine synergistically enhances apoptosis of acute myeloid leukemia cells

Abstract:Acute myeloid leukemia (AML) is a devastating disease. Hybrid agents with dual activity, which have been shown to possess anti-cancer effect, are expected to potentially improve the prognosis of AML patients. EDO-S101 is a novel alkylating deacetylase inhibitor molecule synthesized by the addition of the hydroxamic acid of histone deacetylases inhibitor vorinostat into bendamustine, a DNA-damaging agent. However, the effect of EDO-S101 in combination with traditional chemotherapy drugs has not been studied in AML. In this study, we investigated the effect of EDO-S101 in combination with cytarabine in treating AML cells. The synergic activity against AML was identified by remarkable reduction of cell viability, significant apoptosis enhancement and the upregulation of the cleaved PARP, Casepase-3 and -7 proteins compared with monotherapy. To explain the drivers, we detected the DNA damage pathway including DNA double-strand breaks marker γ-H2AX and DNA damage checkpoint proteins, which was supposed to be responsible for the enhanced apoptosis activity. In summary, our data demonstrated that EDO-S101 in combination with cytarabine could synergistically induce the apoptosis of AML cells and it might be a potential regimen for treating leukemia.
Homoharringtonine Exhibits Potent Anti-Tumor Effect and Modulates DNA Epigenome in Acute Myeloid Leukemia by Targeting SP1/TET1/5hmC.

Abstract:Homoharringtonine, a plant alkaloid, has been reported to suppress protein synthesis and has been approved by the US Food and Drug Administration for the treatment of chronic myeloid leukemia. Here we show that in acute myeloid leukemia (AML), homoharringtonine potently inhibits cell growth/viability and induces cell cycle arrest and apoptosis, significantly inhibits disease progression in vivo, and substantially prolongs survival of mice bearing murine or human AML. Strikingly, homoharringtonine treatment dramatically decreases global DNA 5-hydroxymethylcytosine abundance through targeting the SP1/TET1 axis, and TET1 depletion mimics homoharringtonine's therapeutic effects in AML. Our further 5hmC-seq and RNA-seq analyses, followed by a series of validation and functional studies, suggest that FLT3 is a critical down-stream target of homoharringtonine/SP1/TET1/5hmC signaling, and suppression of FLT3 and its downstream targets (e.g. MYC) contributes to the high sensitivity of FLT3-mutated AML cells to homoharringtonine. Collectively, our studies uncover a previously unappreciated DNA epigenome-related mechanism underlying the potent antileukemic effect of homoharringtonine, which involves suppression of the SP1/TET1/5hmC/FLT3/MYC signaling pathways in AML. Our work also highlights the particular promise of clinical application of homoharringtonine to treat human AML with FLT3 mutations, which accounts for more than 30% of total cases of AML.
AC0010, a Novel BTK Inhibitor, Exerts Anti Leukemia Effects and Synergizes the Efficiency of Homoharringtonine in AML

Abstract:Ibrutinib, an inhibitor of Bruton's tyrosine kinase(BTK), has shown promising pharmacologic effects in acute myeloid leukemia (AML). In this study, we report that AC0010 as a novel BTK inhibitor inhibits the cell proliferation, reduces colony-forming capacity, induces cell apoptosis and arrests cell cycle in AML cells, especially in those carrying FLT3 mutations. Surprisingly, AC0010 is found more sensitive than ibrutinib in treating AML. Mechanically, we showed that AC0010 not only targeting the phosphorylation of BTK, but also the crucial PI3K survival pathway. Furthermore, we observed that AC0010 could also suppress the expression of p-FLT3 and downstream target p-STAT5 in AML cells with FLT3 mutations. Moreover, our in vitro and in vivo data clarify that AC0010 can synergize with Homoharringtonine (HHT), a natural plant alkaloid, in treating AML with or without FLT3-ITD mutations. Collectively, our results suggest that AC0010 might be a promising drug in treating AML patients, and synergizes the efficiency of Homoharringtonine in AML. Pretreatment of Umbilical Cord Derived MSCs with IFN-γ and TNF-α Enhances the Tumor-Suppressive Effect on Acute Myeloid Leukemia.

Abstract:Currently, the standard therapeutic approach of AML consists of chemotherapy and allogeneic hematopoietic stem cell transplantation (HSCT). However, these strategies are usually associated with adverse side effects and high risk of relapse following HSCT. Thus, it is imperative to find an alternative way against AML progression. Here, we showed that treatment with umbilical cord-derived mesenchymal stem cells (UC-MSCs) could efficiently induce apoptosis in both primary AML patient-derived leukemic cells and AML cell lines. Mechanistically, tumor necrosis factor-alpha-related apoptosis-inducing ligand (TRAIL) in UC-MSCs mediated the proapoptotic effect in AML cells. Besides, indoleamine 2,3-dioxygenase (IDO) secreted by UC-MSCs blocked the cell cycle progression and inhibited the proliferation of AML cells. Importantly, we found that incubation of UC-MSCs with IFN-gamma and TNF-alpha could upregulate the expression of TRAIL and IDO, resulting in an intensive pro-apoptotic efficacy. UC-MSCs pre-treatment could not only relieve the AML burden but also eliminate AML cells in a xenograft AML model. Our findings have shed light on an effective pre-activated approach to aggravating the anti-leukemia effect of MSC. Furthermore, a novel and safe stem cell-based therapy approach for AML treatment.

Abstract:Autophagy fundamentally refers to a process where cells enclose their own cell organelles with isolated membrane and deliver them to lysosomes for degradation, servicing as a nonspecific degradation mechanism of autologous proteins. Autophagy is induced above basal levels in response to nutrient deprivation or trophic factor withdrawal to sustain metabolism by producing catabolites. In this context, autophagy acts as a cytoprotective mechanism for cells to protect themselves from extracellular stresses. However, in a culture using cells in which autophagy-related genes were suppressed by siRNA and to which an autophagy inhibitor, 3- methyladenine (3-MA), was added, cytotoxic effects of some anticancer agents on cancer cells were diminished. Based on these results, the concept of caspase-independent cell death via autophagy, which is designated as type II programmed cell death (PCD) in contrast to apoptosis as type I PCD, has been receiving increased attention. We here report that treatment of AML cells with low does-Ara-C induces autophagy and enhances its cytocidal effect on AML cells by combined treatment with G-CSF. Treatment of AML cell line U937 cells with 1 to 50 ng/ml of Ara-C induced cell growth inhibition in a dose-dependent manner. Exposure to the higher concentrations of Ara-C (more than 10 ng/ml) induced apoptosis in U937 cells along with formation of apoptotic bodies and caspase-3 activation. However, treatment with the lower concentration of Ara-C (2.5 to 7.5 ng/ml) induced autophagy as assessed by electron microcopy and the punctuated distribution pattern of cytoplasmic microtubule-associated protein 1 light chain 3B (LC3B) by fluorescent immunocytochemistry. Low dose-Ara-C inducing -cell death and -autophagy both were suppressed when the cells were cultured in the presence of 3-MA. This suggests that the autophagic cell death but not the cytoprotective autophagy was induced in response to Ara-C. Combined treatment with G-CSF (lenograstim; 100 ng/ml) plus 2.5 to 7.5 ng/ml of Ara-C, but not plus higher concentrations of Ara-C, resulted in the enhanced cytocidal effect in U937 cells in vitro. These data indicate that combined treatment of Ara-C plus G-CSF has a therapeutic benefit for the AML patients by enhanced cytocidal effect which appears to be mediated via induction of autophagy. In addition, our data appear to explain in part the high response rate of CAG therapy in refractory AML, which consists of low-dose Ara-C in combination with G-CSF and aclarubicin.
Targeting CaMKII-δ/DHCR24 Axis is a Novel Strategy Against Acute Myeloid Leukemia

Abstract:Abstract Relapse is one of the biggest challenges in treating acute myeloid leukemia (AML) due to leukemia stem/progenitor cells resisting chemotherapy, providing a cellular reservoir to form the basis for relapse. Here, we identify CaMKII-δ/DHCR24 axis as a novel therapeutic target of AML. CaMKII-δ is aberrantly activated in leukemia stem/progenitor cells of AML patients but not in normal hematopoietic stem/progenitor cells, and associated with high tumor burden and poor outcome. Blocking CaMKII-δ activity results in rapid apoptosis of AML stem/progenitor and bulk leukemia cells by inhibiting STAT3, CDK6 and BCL-2 that regulate viability, cell cycle and apoptosis of AML cells. Moreover, CaMKII-δ is a key regulator of DHCR24 associated with cholesterol metabolic pathway of AML. Inhibition of DHCR24 selectively suppressed the growth of leukemia stem/progenitor cells. Importantly, we demonstrated that pharmacological inhibition of CaMKII-δ/DHCR24 axis by small molecule hesperadin potently regressed AML in mouse model. These findings reveal that CaMKII-δ3/DHCR24 axis is a critical regulator that differentially regulates the survival and apoptosis of AML cells and normal HSCs. CaMKII-δ/DHCR24 axis might be a potential therapeutic target for treating AML.

Abstract:Overexpression of the MYC oncogene, encoding c-MYC protein, contributes to the pathogenesis and drug resistance of acute myeloid leukemia (AML) and many other hematopoietic malignancies. Although standard chemotherapy has predominated in AML therapy over the past five decades, the clinical outcomes and patient response to treatment remain suboptimal. Deeper insight into the molecular basis of this disease should facilitate the development of novel therapeutics targeting specific molecules and pathways that are dysregulated in AML, including fms-like tyrosine kinase 3 (FLT3) gene mutation and cluster of differentiation 33 (CD33) protein expression. Elevated expression of c-MYC is one of the molecular features of AML that determines the clinical prognosis in patients. Increased expression of c-MYC is also one of the cytogenetic characteristics of drug resistance in AML. However, direct targeting of c-MYC has been challenging due to its lack of binding sites for small molecules. In this review, we focused on the mechanisms involving the bromodomain and extra-terminal (BET) and cyclin-dependent kinase 9 (CDK9) proteins, phosphoinositide-Akt-mammalian target of rapamycin (PI3K/AKT/mTOR) and Janus kinase-signal transduction and activation of transcription (JAK/STAT) pathways, as well as various inflammatory cytokines, as an indirect means of regulating MYC overexpression in AML. Furthermore, we highlight Food and Drug Administration (FDA)-approved drugs for AML, and the results of preclinical and clinical studies on novel agents that have been or are currently being tested for efficacy and tolerability in AML therapy. Overall, this review summarizes our current knowledge of the molecular processes that promote leukemogenesis, as well as the various agents that intervene in specific pathways and directly or indirectly modulate c-MYC to disrupt AML pathogenesis and drug resistance.

Abstract:CD99 is a transmembrane protein overexpressed in Acute Myeloid Leukemia (AML), presenting a potential novel therapeutic target. Our group has previously developed anti-CD99-A192 (α-CD99-A192), comprising of single chain variable fragment (scFv) and elastin-like polypeptides (ELPs), and reported promising anti-leukemic activity in AML preclinical models. Treatment with α-CD99-A192 induced apoptosis in AML cell lines and prolonged survival in AML xenograft models. Considering CD99's expression and role in T cell activation, in the current study, we propose that α-CD99-A192 plays a dual function, i.e., targeting leukemic cells and activating T cells. This manuscript reports the effects of α-CD99-A192 on T cells in the context of AML. α-CD99-A192 treatment enhances T cell proliferation and activation and increases the release of pro-inflammatory cytokines along with increased aggregation of T cells, which culminates in heightened cytotoxicity against leukemic cells. Altogether, these findings suggest α-CD99-A192 enhances T cell activation and cytotoxic potential consistent with dual mechanisms of action for α-CD99-A192.

Abstract:Acute myeloid leukemia (AML) is an aggressive hematologic malignancy characterized by extremely heterogeneous molecular and biologic abnormalities that hamper the development of effective targeted treatment modalities. While AML cells are highly sensitive to cytotoxic Ca2+ overload, the feasibility of Ca2+- targeted therapy of this disease remains unclear. Here, we show that apoptotic response of AML cells to the synergistically acting polyphenols curcumin (CUR) and carnosic acid (CA), combined at low, non-cytotoxic doses of each compound was mediated solely by disruption of cellular Ca2+ homeostasis. Specifically, activation of caspase cascade in CUR+CA-treated AML cells resulted from sustained elevation of cytosolic Ca2+ (Ca2+cyt) and was not preceded by endoplasmic reticulum stress or mitochondrial damage. The CUR+CA-induced Ca2+cyt rise did not involve excessive influx of extracellular Ca2+ but, rather, occurred due to massive Ca2+ release from intracellular stores concomitant with inhibition of Ca2+cyt extrusion through the plasma membrane. Notably, the CUR+CA combination did not alter Ca2+ homeostasis and viability in non-neoplastic hematopoietic cells, suggesting its cancer-selective action. Most importantly, co-administration of CUR and CA to AML-bearing mice markedly attenuated disease progression in two animal models. Collectively, our results provide the mechanistic and translational basis for further characterization of this combination as a prototype of novel Ca2+-targeted pharmacological tools for the treatment of AML.

Abstract:Survival of patients with acute myeloid leukemia (AML) can be improved by allogeneic hematopoietic stem cell transplantation (allo-HSCT) because of the antileukemic activity of T and natural killer cells from the donor. However, the use of allo-HSCT is limited by donor availability, recipient age, and potential severe side effects. Similarly, the efficacy of immunotherapies directing autologous T cells against tumor cells, including T-cell recruiting antibodies, chimeric antigen receptor T-cell therapy, and immune checkpoint inhibitors are limited in AML because of multiple mechanisms of leukemia immune escape. This has prompted a search for novel immunostimulatory approaches. Here, we show that activation of adenosine 5'-monophosphate-activated protein kinase (AMPK), a master regulator of cellular energy balance, by the small molecule GSK621 induces calreticulin (CALR) membrane exposure in murine and human AML cells. When CALR is exposed on the cell surface, it serves as a damage-associated molecular pattern that stimulates immune responses. We found that GSK621-treated murine leukemia cells promote the activation and maturation of bone marrow-derived dendritic cells. Moreover, vaccination with GSK621-treated leukemia cells had a protective effect in syngeneic immunocompetent recipients bearing transplanted AMLs. This effect was lost in recipients depleted of CD4/CD8 T cells. Together, these results demonstrate that AMPK activation by GSK621 elicits traits of immunogenic cell death and promotes a robust immune response against leukemia. Pharmacologic AMPK activation thus represents a new potential target for improving the activity of immunotherapy in AML.

Abstract:Homoharringtonine combined with aclarubicin and cytarabine (HAA) is a highly effective treatment for acute myeloid leukemia (AML), especially for t(8;21) AML. However, the underlying mechanisms by which HAA kills t(8;21) AML cells remain unclear. In this study, SKNO-1 and Kasumi-1 cells with t(8;21) were used. Compared with individual or pairwise administration of homoharringtonine, aclarubicin, or cytarabine, HAA showed the strongest inhibition of growth and induction of apoptosis in SKNO-1 and Kasumi-1 cells. HAA caused cleavage of the AML1-ETO (AE) oncoprotein to form truncated AE (ΔAE). Pretreatment with the caspase-3 inhibitor caspase-3 inhibitor Q-DEVD-OPh (QDO) not only suppressed HAA-induced apoptosis but also abrogated the cleavage of AE and generation of ΔAE. These results suggest that HAA synergistically induces apoptosis in t(8;21) leukemia cells and triggers caspase-3-mediated cleavage of the AML1-ETO oncoprotein, thus providing direct evidence for the strong activity of HAA toward t(8;21) AML.
A Novel Small Molecule CXCR4 Antagonist Improves Anticancer Therapy by Blocking the Interaction of Malignant Cells with Their Associated Niches

Abstract:Acute myeloid leukemia (AML) is a blood cancer characterized by uncontrolled proliferation of leukemic stem/progenitor cells. The retention and chemoresistance of AML cells in the bone marrow microenvironment are mediated by stromal cell-derived factor-1(SDF-1)/C-X-C chemokine receptor type 4 (CXCR4) axis. We recently developed a novel small molecule CXCR4 antagonist (designated HF51116) with a higher affinity than AMD3100 specifically for improving AML chemotherapy. Competitive binding assays confirmed that HF51116 bound to the surface of malignant U937 and HL60 cells with high affinities. Transwell assay results demonstrated that HF51116 blockade inhibited the SDF-1induced migration of U937 and HL60 cells with a concentration-dependent manner, as well as the migration induced by BMSCs. The mechanism for HF51116 in U937 and HL-60 cells migration appeared to involve attenuation of the SDF-1induced cytoskeletal re-arrangement and phosphorylation of AKT (S437) and ERK1/2 (Thr202/Tyr204). In addition, HF51116 reversed the chemoresistance of U937 cells co-cultured with BMSCs by increasing Annexin V expression and caspase-3 activity, resulting in decreased cell viability. In vivo experiments on AML xenograft mice revealed that subcutaneous administration of HF51116 mobilized the engrafted U937-Luc/EGFP cells from the bone marrow and spleen into the peripheral blood. Co-administrating HF51116 and cytarabine (Ara-C) further reduced the in vivo AML burden and expanded the lifespan of AML xenograft mice beyond that seen with Ara-C monotherapy. These results demonstrated that the novel CXCR4 antagonist HF51116 has potent anti-leukemic effects both in vitro and in vivo.
