Abstract:Objectives We sought to evaluate whether dynamic Arterial Spin Labeling (dASL), a novel quantitative technique robust to artifacts and noise that especially arise in inferior brain regions, could characterize neural substrates of BD pathology and symptoms. Methods Forty-five subjects (19 BD patients, 26 controls) were imaged using a dASL sequence. Maps of average perfusion, perfusion fluctuation, and perfusion connectivity with anterior cingulate cortex (ACC) were derived. Patient symptoms were quantified along four symptom dimensions determined using factor analysis of the subjects from the Bipolar and Schizophrenia Network on Intermediate Phenotypes (BSNIP) study. Maps of the perfusion measures were compared between BD patients and controls and correlated with the symptom dimensions in the BD patients only by voxel-level and region-level analyses. Results BD patients exhibited (i) significantly increased perfusion fluctuations in the left fusiform and inferior temporal regions (P = .020, voxel-level corrected) and marginally increased perfusion fluctuations in the right temporal pole and inferior temporal regions (P = .063, cluster-level corrected), (ii) significantly increased perfusion connectivity between ACC and the occipitoparietal cortex (P = .050, cluster-level corrected). In BD patients, positive symptoms were negatively associated with ACC perfusion connectivity to the right orbitofrontal and superior frontal regions (P = .002, cluster-level corrected) and right orbitofrontal and inferior frontal regions (P = .023, cluster-level corrected). Conclusion The abnormal perfusion fluctuations and connectivity alterations may underlie the mood fluctuations and cognitive and emotional dysregulation that characterize BD.
Regional homogeneity and functional connectivity analysis of resting-state magnetic resonance in patients with bipolar II disorder.

Abstract:To explore the characteristics of local brain activity in patients with bipolar depression and its correlation with clinical features.In accord with the diagnostic and statistical manual of mental disorders-IV of bipolar disorder, 21 patients were enrolled while 21 healthy controls were matched with similar gender, age, level of educations. A 3.0T GE magnetic resonance scanner was used to collect the imaging data, and a 2-sample t test was performed. Regional homogeneity (ReHo) method was used to estimate regional activation patterns through indices of localized concordance. ReHo values were compared between groups. Seed-based correlation analysis was used to analyze functional connections.In the patients' group, ReHo (regional homogeneity) values of resting state functional magnetic resonance imaging data on the right cerebellum 4 and 5 area, cerebellar vermis 6 area, left insula were positively correlated with Hamilton depression scale (HAMD) scores. ReHo values on the left of the triangle of inferior frontal gyrus, right inferior frontal gyrus of orbital region showed negative correlation with HAMD scores. The value of ReHo in the patients' group was positively correlated with the patients' Hamilton anxiety scale score in the right fusiform gyrus, and negative correlation was found in the left insula. The ReHo value of the patients' group was negatively correlated with the patients' Montgomery Asberg depression rating scale score in regions of the midbrain. The value of ReHo in patients' group was significantly reduced in the right central front.The depression and anxiety severity of bipolar depression patients may be associated with the consistency activity of left insula, right cerebellum, and cerebellar vermis related area, fusiform gyrus. In addition, the ReHo of the midbrain neurons activity may be associated with depression level of patients with bipolar II disorder.
Amplitude of Low-Frequency Fluctuation and Functional Connectivity Analysis Based Resting-State Functional Magnetic Resonance Imaging Study in Bipolar Ⅱ Depression Patients

Abstract:Objective To examine the characteristics of spontaneous activity of brain regions in bipolar Ⅱ depression patients by using resting state fMRI technology, and explore the pathogenesis of bipolarⅡdepression.Methods In total 21 bipolarⅡdepression patients(BD group)and 21 gender-,age-and education-matched healthy controls(HC group)were recruited.The patients were diagnosed according to the DSM-Ⅳ criteria, and the symptoms were evaluated by the Montgomery-Asberg Depression Rating Scale (MADRS), HAMD17and HAMA. The resting-state data was collected via a 3.0 T GE MRI, and was analyzed using an amplitude of low-frequency fluctuation(ALFF)approach,and two-samples t test was used to analyze the difference between BD group and HC group.Correlation analysis was used between the mean value of ALFF in brain regions which were significantly different from the other regions and the depression severity. The brain areas that were significantly different from the other brain regions were regarded as regions of interest,and were made functional connectivity with the whole brain.The functional connectivity was compared between the two groups to explore the impaired emotional network in bipolar Ⅱdepression. Results The ALFFs of left paracentral lobule(t=3.80),right precuneus(t=3.54)and right precentral gyrus(t=3.66) in BD group were significantly higher than HC group(all P<0.05, GRF correction). In BD group, the ALFFs of the left orbital frontal gyrus (r=0.75) and right orbital frontal gyrus (r=0.80) were positively correlated with the HAMA score.The ALFFs of bilateral gyri rectus(r=0.73)and brain regions near the left insular lobe (r=0.74) were positively correlated with the HAMD17score. The ALFF of region 2 of right cerebellar peduncles (r=-0.65) was negatively correlated with the MADRS score(all P<0.05). The functional connectivity between paracentral lobule, precuneus, precentral gyrus and left middle frontal gyrus, left anterior cingulate gyrus together with its lateral gyrus in BD group was significantly increased relative to HC group (t=4.47,4.07; both P<0.05), while the functional connectivity between the right rectangular shape crack and the cortex around was decreased in BD group (t=4.54,P<0.05). Conclusion In bipolar Ⅱ depression patients, the brain function of left paracentral lobule, right precuneus and right precentral gyrus may be impaired and correlated with severity of depressive.

Abstract:The complex [{VO(L)}(2){VO(acac)}(2)(&mgr;(2)-OMe)(2)] 1 can be synthesized by reaction of the Schiff base ligand N,N-bis(2-hydroxyethyl)-N'-(2-pyrrolylmethylidene)ethylenediamine (H(2)L) with [VO(acac)(2)] (Hacac = 2,4-pentanedione). The compound 1.MeOH has been characterized by IR, UV/vis, and ESR spectroscopy as well as magnetic susceptibility measurements. A single-crystal diffraction study performed on 1. MeOH gives the following crystal data: triclinic, P&onemacr;, a = 7.352(2) Å, b = 12.584(4) Å, c = 12.826(4) Å, alpha = 107.61(2) degrees, beta = 102.96(2) degrees, gamma = 95.25(2) degrees, Z = 1. The tetranuclear {(VO)(4)(&mgr;(3)-OR)(2)(&mgr;(2)-OR)(4)}(2+) core of 1 is composed of three different types of edge-shared binuclear units, an anti-coplanar, two syn-coplanar, and two twist configurations. Consistent with this connectivity pattern, analysis of the magnetic data reveals competing ferromagnetic and antiferromagnetic exchange interactions within the tetranuclear core structure. Both ESR and magnetic data indicate a singlet ground state for 1. In addition, the general magnetic behavior of oxovanadium(IV) clusters is discussed on the basis of classification schemes derived from simple binuclear fragmentation patterns. Consistent with the superexchange mechanism expected to be operative for binuclear oxovanadium(IV) units with syn-coplanar configuration, the corresponding magnetic exchange parameter J(sc) of 1 (V.V = 3.410(2) Å) is determined to be antiferromagnetic at -153 cm(-)(1).
Abnormal functional connectivity within the reward network: a potential neuroimaging endophenotype of bipolar disorder.

Abstract:BACKGROUND:Reward circuit dysfunction underlies the pathogenesis of bipolar disorder (BD). This study aims to investigate whether nucleus accumbens (NAcc) and ventromedial prefrontal cortex (vmPFC), two key reward regions for BD, have resting-state dysfunctional connectivity with other brain regions in depressed and euthymic BD. METHODS:40 bipolar depressive (DE), 20 euthymic patients (EU) and 20 healthy controls (HC) were recruited to undergo resting-state functional MRI (rs-fMRI) scanning. Seed-based functional connectivity (FC) was calculated between NAcc/vmPFC and the whole brain. Group differences were calculated and their correlations with clinical characteristics were analyzed. Support vector machine was applied to classify BD patients and HC based on the FC between the cluster of group difference and NAcc/vmPFC. RESULTS:Whole brain networks of FC identified right anterior insular cortex (AIC) as a significant region with bilateral NAcc when compared among three groups. The right AIC-NAcc FC elevated in both patient groups and was highest in the EU group. Interestingly, vmPFC-based networks also identified the right AIC as a significant cluster. The right AIC-vmPFC FC elevated in both patient groups. However, FC between NAcc and vmPFC did not significantly differ BD patients from HC. Furthermore, the strength of FC between bilateral NAcc and the right AIC was positively associated with the illness course of BD. Notably, the NAcc/vmPFC-right AIC classifier acquired an accuracy of 68.75% and AUC-ROC of 78.17%. LIMITATIONS:Our sample size is modest. CONCLUSIONS:Our findings indicated that elevated NAcc/vmPFC-right AIC connectivity within the reward circuit could be a neuroimaging endophenotype of BD.

Abstract:A total of 45 BD patients and 115 healthy control subjects were finally included and whole‐brain functional connectivity homogeneity (FcHo) was calculated from their rs‐fMRI. Seed‐based resting‐state functional connectivity analysis was then used to reveal the alteration of corresponding functional connectivity. Our findings indicated that whole‐brain FcHo and connectivity abnormalities of left middle temporal gyrus might play a crucial role in the neuropathology of BD.BackgroundPrevious neuroimaging studies have extensively demonstrated many signs of functionally spontaneous local neural activity abnormalities in bipolar disorder (BD) patients using resting‐state functional magnetic resonance imaging (rs‐fMRI). However, how to identify the changes of voxel‐wise whole‐brain functional connectivity pattern and its corresponding functional connectivity changes remain largely unclear in BD patients. The current study aimed to investigate the voxel‐wise changes of functional connectivity patterns in BD patients using publicly available data from the UCLA CNP LA5c Study.MethodsA total of 45 BD patients and 115 healthy control subjects were finally included and whole‐brain functional connectivity homogeneity (FcHo) was calculated from their rs‐fMRI. Moreover, the alterations of corresponding functional connectivity were subsequently identified using seed‐based resting‐state functional connectivity analysis.ResultsIndividuals with BD exhibited significantly lower FcHo values in the left middle temporal gyrus (MTG) when compared with controls. Functional connectivity findings further indicated decreased functional connectivities between left MTG and cluster 1 (left superior temporal gyrus, extend to middle temporal gyrus, rolandic operculum), cluster 2 (right postcentral, extend to right precentral) in BD patients. The mean FcHo values of left MTG were positively correlated with insomnia, middle scores and appetite increase scores. The mean functional connectivities of left MTG to cluster 1 were negatively correlated with grandiose delusions scores. While the functional connections between left MTG with cluster 2 were negatively correlated with delusions of reference and positively correlated with insomnia, middle scores in BD patients.ConclusionsOur findings suggested that abnormal FcHo and functional connections in those areas of the brain involving DMN and SMN networks might play a crucial role in the neuropathology of BD.

Interhemispheric resting state functional connectivity abnormalities in unipolar depression and bipolar depression

Abstract:Objectives: Depression in the context of bipolar disorder (BD) is often misdiagnosed as unipolar depression (UD), leading to mistreatment and poor clinical outcomes. However, little is known about the similarities and differences in interhemispheric functional connectivity between BD and UD. Methods: Patients with bipolar II disorder (n = 36) and UD (n = 32) during a depressive episode as well as 40 healthy controls underwent resting-state functional magnetic resonance imaging. The functional connectivity between any pair of symmetric interhemispheric voxels (i.e., functional homotopy) was measured by voxel-mirrored homotopic connectivity (VMHC). Results: The three groups showed significant VMHC differences in the posterior cingulate cortex (PCC), fusiform and lingual gyrus, anterior lobe of the cerebellum (CeAL), and posterior lobe of the cerebellum (CePL). In the BD group, the VMHC decreases in the fusiform/lingual gyrus, CeAL, and CePL were shown relative to controls. In the UD group, the VMHC decreases in the PCC, fusiform/lingual gyrus, and CePL were shown relative to controls. No regions of increased VMHC were detected in either patient group. There was no significant difference in the VMHC values in any brain region between the BD group and the UD group. Conclusions: Depressive episodes in BD and UD have similar impairments of interhemispheric coordination, which might imply an overlap in the neuropathology of depression.

Abstract:BackgroundBipolar disorder (BD) is increasingly being regarded as a dysconnection syndrome. Functional integration among the three core brain networks – executive control network (ECN), salience network (SN), and default mode network (DMN) – is abnormal in patients with BD; however, the causal relationship among the three networks in BD is largely unknown. It is also unclear whether patients with BD in different mood states show distinct effective connectivity patterns during rest.MethodsResting-state fMRI data were collected from 65 patients with BD and 85 healthy controls. Spectral dynamic causal modeling was applied to investigate the effective connectivity difference of the three brain networks between all patients with BD and healthy controls and between patients who were in euthymic mood state (euthymic BD) and depressed mood state (depressed BD).ResultsCompared with healthy controls, all patients with BD showed altered effective connectivity within and between the ECN and SN and from these two networks to the DMN. Compared with patients with depressed BD, patients with euthymic BD showed increased excitatory effects within the ECN and decreased inhibitory effects from the SN to the ECN and DMN.ConclusionThese results further confirmed that patients with BD show abnormal functional integration within and among the three core brain networks, and exhibit similar and different effective connectivity patterns in different mood states. Abnormal effective connectivity has the potential to be a critical index for diagnosing BD and differentiating between BD patients with different mood states.

Increased ALFF and functional connectivity of the right striatum in bipolar disorder patients

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Bipolar disorder (BD) is a serious neuropsychiatric disorder characterized by alternating periods of mania, depression, and euthymia. Abnormal spontaneous brain activity within the cortical-striatal neural circuits has been observed in patients with BD. However, whether the abnormality appears in patients with BD while not in a manic mood state is unclear.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>This study collected resting-state fMRI data from 65 patients with BD who were not in a manic mood state and 85 matched healthy controls. First, we examined differences in amplitude of low-frequency fluctuations (ALFF) between the patients with BD and the healthy controls to identify regions that show abnormal local spontaneous activity in the patients. Based on the ALFF results, we conducted seed-based resting-state functional connectivity (rsFC) analysis to identify the changes in brain networks that are centered on the regions showing abnormal local spontaneous activity in the patients. Finally, we repeated these analyses in a sub-sample comprising euthymic BD patients (<em>N</em> = 37) and between the euthymic BD patients and all the other patients who had at least mild depressive symptoms.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>BD patients exhibited increased ALFF in the right caudate/putamen and increased rsFC in the right caudate/putamen with the right inferior parietal lobe (cluster-level FWE <em>p</em> < 0.05). Further analyses showed that the euthymic BD patients showed similar abnormalities in ALFF and rsFC maps as found in all patients with BD. And the euthymic BD patients were comparable with all the other patients who had at least mild depressive symptoms in ALFF values.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Our results indicated the important role of the right striatum in the baseline brain function of BD patients and suggested that the abnormality of spontaneous brain activity in the cortical-striatal neural circuits may be a trait-like variant in patients with BD. The results deepened our understanding of the neurobiological mechanisms associated with BD etiology.</p>

Abstract:Background: Previous studies demonstrated that patients with bipolar disorder (BD) exhibited abnormal neural activity in multiple brain regions. However, no study has been conducted to identify regional intrinsic neural activity changes in BD II. In the present study, we used the regional homogeneity (ReHo) approach to explore the regional abnormal neural activity in bipolar II disorder Methods: One hundred unmedicated patients with BD II depression and 100 healthy controls (HC) underwent the resting-state functional magnetic resonance imaging. The ReHo values of each voxel was calculated in the whole brain. The two-sample t-test and threshold-free cluster enhancement (TFCE) correction were applied for the ReHo analysis. Results: Compared with the HC group, the BD II group showed significantly decreased ReHo in the left orbitofrontal cortex, and increased ReHo in the right precentral gyrus, right supplementary motor area and bilateral middle occipital gyrus (P < .05, TFCE corrected). Limitations: This study lacks the evidence of brain structural changes, and used the cross-sectional design which did not explore local alterations of remitted and manic patients. Conclusion: Our findings revealed abnormal local intrinsic neural activity during resting state which may contribute to the pathophysiology of bipolar II disorder. Particularly the disrupted balance between the prefrontal cortex and primary sensorimotor regions provides evidence for the unique pathological mechanism underlying BD.
Altered Regional Homogeneity in Pediatric Bipolar Disorder During Manic State: A Resting-State Fmri Study

Abstract:Pediatric bipolar disorder (PBD) is a severely debilitating illness, which is characterized by episodes of mania and depression separated by periods of remission. Previous fMRI studies investigating PBD were mainly task-related. However, little is known about the abnormalities in PBD, especially during resting state. Resting state brain activity measured by fMRI might help to explore neurobiological biomarkers of the disorder. Methods: Regional homogeneity (ReHo) was examined with resting-state fMRI (RS-fMRI) on 15 patients with PBD in manic state, with 15 age-and sex-matched healthy youth subjects as controls. Results: Compared with the healthy controls, the patients with PBD showed altered ReHo in the cortical and subcortical structures. The ReHo measurement of the PBD group was negatively correlated with the score of Young Mania Rating Scale (YMRS) in the superior frontal gyrus. Positive correlations between the ReHo measurement and the score of YMRS were found in the hippocampus and the anterior cingulate cortex in the PBD group. Conclusions: Altered regional brain activity is present in patients with PBD during manic state. This study presents new evidence for abnormal ventral-affective and dorsal-cognitive circuits in PBD during resting state and may add fresh insights into the pathophysiological mechanisms underlying PBD.
Altered amplitude of low-frequency fluctuation in remitted patients with pediatric bipolar disorder: a resting-state fMRI study

Abstract:Objective To investigate the alteration of brain function and cognitive function between remitted patients with pediatric bipolar disorder (PBD) and healthy adolescents by using resting-state functional magnetic resonance imaging (fMRI)and cognitive function tests.Methods The fMRI signals of 16 PBD-remitted patients and 16 healthy controls were collected,and the amplitude of low-frequency fluctuation (ALFF) was calculated.Cognitive function tests were performed for all subjects.Correlations between results of cognitive function tests and ALFF were analyzed.Results Compared with control group,ALFF in PBD-remitted patients significantly increased in the right cerebellum,the right superior temporal gyrus,the left parahippocampal gyrus,while decreased in the left middle occipital gyrus,the left cuneus and the right superior occipital gyrus (P＜0.01,FDR correction).ALFF in the right cerebellum was negatively correlated to the results of Stroop1 test and ALFF in the left middle occipital gyrus was positively correlated to the results of backward digit span test.Conclusion PBD-remitted patients show spontaneous neural activity disorders in the temporal lobe,cerebellum,limbic system and occipital lobe.The dysfunctions of cerebellum and middle occipital gyrus are related to cognitive impairment of PBD-remitted patients.

Abstract:Background Abnormalities in large-scale, structural and functional brain connectivity have been increasingly reported in patients with major depressive disorder (MDD). However, MDD-related alterations in functional interaction between the cerebral hemispheres are still not well understood. Resting state fMRI, which reveals spontaneous neural fluctuations in blood oxygen level dependent signals, provides a means to detect interhemispheric functional coherence. We examined the resting state functional connectivity (RSFC) between the two hemispheres and its relationships with clinical characteristics in MDD patients using a recently proposed measurement named "voxel-mirrored homotopic connectivity (VMHC)". Methodology/Principal Findings We compared the interhemispheric RSFC, computed using the VMHC approach, of seventeen first-episode drug-naive patients with MDD and seventeen healthy controls. Compared to the controls, MDD patients showed significant VMHC decreases in the medial orbitofrontal gyrus, parahippocampal gyrus, fusiform gyrus, and occipital regions including the middle occipital gyrus and cuneus. In MDD patients, a negative correlation was found between VMHC of the fusiform gyrus and illness duration. Moreover, there were several regions whose VMHC showed significant negative correlations with the severity of cognitive disturbance, including the prefrontal regions, such as middle and inferior frontal gyri, and two regions in the cereballar crus. Conclusions/Significance These findings suggest that the functional coordination between homotopic brain regions is impaired in MDD patients, thereby providing new evidence supporting the interhemispheric connectivity deficits of MDD. The significant correlations between the VMHC and clinical characteristics in MDD patients suggest potential clinical implication of VMHC measures for MDD. Interhemispheric RSFC may serve as a useful screening method for evaluating MDD where neural connectivity is implicated in the pathophysiology.
Distinguishing major depressive disorder from bipolar disorder in remission: A brain structural network analysis

Abstract:BackgroundIt is challenging to differentiate major depressive disorder (MDD) from bipolar disorder (BD) in depression and remission. To exclude the potential influence of depressive episodes, we compared the white matter (WM) network between MDD and BD patients in remission to find disease-specific alterations in MDD and BD, and then distinguish these two affective disorders.MethodsWe recruited 33 patients with remitted MDD (rMDD), 54 patients with remitted BD (rBD), and 60 healthy controls (HCs). Diffusion tensor imaging and high-resolution 3D T1-weighted image were acquired. Global and nodal topological parameters were used to depict the alterations of the whole-brain WM network.ResultsWe found that rMDD displayed increased global network efficiency (Eglob) and local network efficiency (Eloc) compared with HCs, whereas we found no significance between rBD and HCs. Compared with rBD and HCs, patients in the rMDD group showed increased nodal degree and nodal efficiency, and decreased nodal shortest path length in the four cerebral regions, including the right calcarine fissure (CAL.R), right cuneus (CUN.R), left lingual gyrus (LING.L), and left middle occipital gyrus (MOG.L). We did not find any rBD specific changes of nodal topological metrics.LimitationsThe main limitation is the possible effects of medication and BD subtypes on the results.ConclusionsOur findings indicate that rMDD exhibited elevated global properties compared with HCs group, and increased nodal properties in the CAL.R, CUN.R, LING.L, and MOG.L specifically compared with rBD and HCs, which may underlie the distinction of the two affective disorders in remission.

Abstract:CO, binds to the iron atoms in hemoglobin, with an affinity 200-250 times that of oxygen, and impairs oxygen carrying capacity of the blood. Hence, CO toxicity is a situation characterized by impairment of oxygen transportation and consumption. It has been reported that 30% of such cases are not diagnosed since the signs and symptoms in CO intoxication. Definitive diagnosis of CO intoxication is established by measuring the COHb concentration in arterial or venous blood. Elimination half life of COHb and partial oxygen pressure produced by inspired oxygen concentration is closely related. High levels of inspired oxygen fractions both accelerate CO removal and improve oxygenation. Hyperbaric oxygen (HBO) treatment is first discussed in 1890’s and was initially used in 1960’s. Although, acceleration of removal of the CO from hemoglobin was aimed initially, other effects were recognized in time. When HBO treatment is not possible, high flow 100% oxygen is applied via tight-fitting face mask. for 6-12 hours. Although, the dominance of f HBO to normobaric oxygen theraphy has been supported by several studies, more randomized clinical trials are needed. In general, most of the CO intoxications are nonfatal. Cases with mild symptoms and those that do not show any neurological symptoms can be discharged from hospital after 4-6 hours of treatment. Despite the therapy, it is still impossible to predict long term results of CO intoxication.

Abstract:BACKGROUND:Previous functional magnetic resonance imaging (fMRI) studies have shown abnormal functional connectivity in regions involved in emotion processing and regulation in pediatric bipolar disorder (PBD). Recent studies indicate, however, that task-dependent neural changes only represent a small fraction of the brain's total activity. How the brain allocates the majority of its resources at resting state is still unknown. We used the amplitude of low-frequency fluctuation (ALFF) method of fMRI to explore the spontaneous neuronal activity in resting state in PBD patients.METHODS:Eighteen PBD patients during the mania phase and 18 sex-, age- and education-matched healthy subjects were enrolled in this study and all patients underwent fMRI scanning. The ALFF method was used to compare the resting-state spontaneous neuronal activity between groups. Correlation analysis was performed between the ALFF values and Young Mania Rating Scale scores.RESULTS:Compared with healthy controls, PBD patients presented increased ALFF in bilateral caudate and left pallidum as well as decreased ALFF in left precuneus, left superior parietal lobule, and bilateral inferior occipital gyrus. Additionally, ALFF values in left pallidum were positively correlated with Young Mania Rating Scale score in PBD.CONCLUSION:The abnormal resting-state neuronal activities of the basal ganglia, parietal cortex, and occipital cortex may play an important role in the pathophysiology in PBD patients.
Abnormal Resting-State Regional Homogeneity Relates to Cognitive Dysfunction in Manic Bipolar Disorder Adolescents: an Fmri Study

Abstract:Nineteen manic adolescents (male/female, 7/12) with bipolar disorder and 18 age-and gender-matched normal adolescents (male/female, 6/12) were included in the study. The stroop color-word test (SCWT), visual reproduction test and trail making test (TMT) were used to assess the cognitive function of the patients and the healthy controls (HCs). The regional homogeneity approach was used to study the resting-state brain activity. Correlation between the resting-state brain activity and neuropsychological testing were analyzed. In the neuropsychological tests, the values of the test of SCWT, TMT-A and visual reproduction test of adolescents were significantly decreased compared with those of the HC group (p < 0.05). Compared with the control group (p < 0.05), the results of two-sample T-test showed that the brain areas with increased ReHo value included the left middle cingulate cortex (MCC_L) and decreased ReHo value included the left superior parietal gyrus (SPG_L), the inferior occipital gyrus (IOG), the right superior temporal gyrus (STG_R), the right superior occipital gyrus (SOG_R), the left precuneus (PCUN_L), the left supplementary motor area (SMA_L) and the left cerebelum_Crus2. Moreover, the mean ReHo values in the SOG_R were negatively correlated with the TMT-A scores and the mean ReHo values in the STG_R were negatively correlated with the Visual reproduction test scores. Pediatric bipolar disorder (PBD) patients have broad impairment of cognitive functions, including attention, spatial perception, memory and executive functions, which provides a new perspective for clarifying the pathophysiologic basis of cognitive impairment in patients with PBD.

Abstract:Background: Dynamic functional connectivity (dFC) based on resting-state fMRI has attracted interest in the field of bipolar disorder (BD), because dFC can better capture the evolving processes of emotion and cognition, which are typically impaired in BD. However, previous dFC studies of BD have typically focused on specific seed brain regions or specific functional brain networks, and they have ignored global dynamic information interaction in the whole brain. This study is aimed to reveal aberrant and interpretable whole-brain dFC patterns of BD. Methods: The resting-state fMRI data collected from 35 euthymic BD patients and 30 healthy people. We developed a new dFC inference pipeline, including the sliding-window method, k-means clustering, a new permutation with zero-inflated Poisson regression method, and a similarity analysis for interpretable states, to examine the different patterns of dFC states between BD patients and healthy participants. Results: BD patients had significantly more frequent transitions between two specific dFC states, which were respectively close to high-level cognitive networks and low-level sensory networks, than healthy controls (p < 0.05, FDR). Limitations: The size of samples and other BD types need to be expanded to validate the results of this study. Possible confounding effect of medication. Conclusions: This study detected aberrant dFC pattern of BD, which indicated the increased lability of the processes of cognition and emotion in BD, and this finding could improve our understanding of the neuropathological mechanism of BD.

Abstract:BACKGROUND: This study aims to explore the changes in functional neuroimaging in bipolar depression patients with anxiety symptoms (BDP-A).METHODS: Forty-five BDP-A patients, 22 bipolar depression patients without anxiety symptoms (BDP-NA), and 48 healthy controls (HC) were finally involved. The low-frequency oscillation characteristics, functional connectivity (FC), and network properties among the three groups of participants were analyzed.RESULTS: Compared with the BDP-NA group, BDP-A patients exhibited significantly decreased amplitude of low-frequency fluctuation (ALFF) in the left middle frontal gyrus (MFG), superior occipital gyrus, and inferior parietal, but supramarginal and angular gyri (IPL). Enhanced FC from left IPL to middle temporal gyrus, from left precentral gyrus (PreCG) to bilateral angular gyri, medial superior frontal gyrus, and left superior frontal gyrus (SFG)/MFG were also revealed. Compared with HC, the BDP-A group showed remarkably increased ALFF in the left MFG/PreCG, right superior parietal gyrus, while decreased ALFF in the left inferior frontal gyrus, opercular part, and SFG. In addition, higher regional homogeneity in the left MFG/PreCG was found.LIMITATIONS: The limitations are as follows: (1) relatively small sample size; (2) not all the patients were drug-naive; (3) lack of pure anxiety disorder patients as a controlled group; (4) mental health conditions of HC were not systemic evaluated.CONCLUSIONS: BDP-A patients showed significant differences in resting-state fMRI properties when compared with BDP-NA or HC group. These results may infer the dysfunction of the dorsal attention network, the default network, and the fronto-limbic system as well as disrupted brain network efficiency in BDP-A patients.

Abstract:Background: Hemispheric lateralization and cooperation are essential for efficient brain function, and aberrations in both have been found in psychiatric disorders such as schizophrenia. This study investigated alterations in hemispheric lateralization and cooperation among patients with bipolar disorder (BD) and associations with neurotransmitter and cell-type density distributions to identify potential molecular and cellular pathomechanisms. Methods: Sixty-seven BD patients and 127 healthy controls (HCs) were examined by resting-state functional MRI (rs-fMRI). Whole-brain maps of the autonomy index (AI) and connectivity between functionally homotopic voxels (CFH) were constructed to reveal BD-specific changes in brain functional lateralization and interhemispheric cooperation, respectively. Spatial associations of regional AI and CFH abnormalities with neurotransmitter and cell-type density distributions were examined by correlation analyses. Results: Bipolar disorder patients exhibited higher AI values in left superior parietal gyrus, cerebellar right Crus I, and cerebellar right Crus II, and these regional abnormalities were associated with the relative densities (proportions) of oligodendrocyte precursor cells and microglia. Patients also exhibited lower CFH values in right inferior parietal gyrus, bilateral middle occipital gyrus, left postcentral gyrus, and bilateral cerebellar crus II, and these regional abnormalities were associated with the densities of serotonin 1A and dopamine D2 receptors, oligodendrocyte precursor cells, astrocytes, and neurons. Conclusions: These findings indicate that abnormal functional lateralization and cooperation in BD with potential molecular and cellular basis.
