Abstract:Augmentation mammaplasty with autologous fat injection is becoming increasingly popular. Many unfavorable cases resulting from the procedure have been reported, but little attention has been paid to these issues. Here, we present a case report contradicting the opinion that autologous fat injection is simple, easy, and safe in breast cosmetic surgery; we also describe nonoperative management, which should be considered in many cases, contrasting the usual opinion that the solid lump should be excised openly. In our case, a 26-year-old girl found a palpable lump in her breast after augmentation mammaplasty with autologous fat injection. Sonography demonstrated the lump in the right breast. Ultrasound-guided fine-needle aspiration biopsy (US-FNAB) was performed to identify the lump. The patient was managed conservatively due to the patient’s wish, based on the fact that the biopsy was confirmed as a solid, inflammatory mass. We also reviewed the existing literature on the subject of complications and treatments of augmentation mammaplasty with autologous fat injection. Autologous fat grafting of the breast is not a simple procedure, and it should be performed by well-trained and skilled plastic surgeons. Purifying the extracted fat granules during surgery, strictly controlling the injection volume, and injecting diffusely in multiple layers to allow the fat granules to be distributed evenly within the breast are effective methods of reducing postoperative complications. US-FNAB is recommended for seeking an accurate diagnosis of the breast lump after breast augmentation with autologous fat injection; no surgical intervention is necessary in many cases if the biopsy shows a liponecrotic lump.

Abstract:Breast augmentation with autologous fat injection is becoming increasingly popular. This article aims to describe bilateral liponecrotic pseudocysts after injection of fat into the breast. A 42-year-old woman presented with palpable indurations and pain with pressure. She also showed abnormalities on her breast cancer screening. The woman underwent mammography, magnetic resonance imaging, and pathologic confirmation to evaluate the breast abnormalities. These radiologic examinations were consistent with liponecrotic pseudocysts on both sides. Bilateral lumpectomies were performed through an inframammary approach. Histologic examination of the specimens showed fat necrosis and dystrophic calcification. Autologous fat grafting to the breast is not a simple procedure and should be performed by well-trained and skilled plastic surgeons. During the surgery, caution should be exercised.

Abstract:Abstract Background Abdominoplasty procedures continue to evolve as combining techniques such as suction-assisted lipectomy or direct sub-scarpal lipectomy have proven to be powerful adjuncts to achieve optimal aesthetic results. However, there is apprehension in combining techniques simultaneously given the potential to affect the vascularity of the abdominoplasty flap. Objectives To assess the safety and efficacy of simultaneous direct sub-scarpal lipectomy combined with liposuction in abdominoplasty patients. Methods A 4-year retrospective review of consecutive abdominoplasties (n = 200) performed by a single surgeon was conducted. Liposuction of the abdominal flap and flanks was performed in all patients. After raising the abdominoplasty flap, undermining was performed to just beyond the xyphoid, lower rib margins superiorly, and to the anterior axillary line laterally. Fat deep to Scarpa’s fascia was then removed by direct tangential excision in all zones of the abdominal flap. Results Average values included: Age, 42.19; BMI, 28.10 kg/m2; follow up, 7 months. Seroma occurred in 13 patients (6.5%), superficial wound dehiscence treated with local wound care in 16 patients (8%), hypertrophic scarring in 16 patients (8%), partial umbilical necrosis in one patient (0.5%), and partial umbilical epidermolysis in six patients (3%). No patients experienced major or minor full-thickness tissue loss. No patients needed reoperation. Conclusions Simultaneous direct excision of sub-scarpal fat with liposuction of the abdomen and flanks does not appear to subject any zone of the abdominoplasty flap to increased risks of vascular compromise. No flap necroses were observed. The use of our technique is safe and may provide superior abdominoplasty results.

Abstract:A 54-year-old woman presented to the emergency department 24 h after undergoing abdominal liposuction, bilateral breast augmentation and facial fat grafting at a private plastic surgery clinic. She presented with the classic evolution of a bowel perforation secondary to abdominal liposuction. A computed tomography (CT) scan found free air in her abdominal cavity. Based on the CT scan and the persistent pain experienced by the patient, an abdominal laparatomy was urgently performed. A jejunum perforation was found and was treated with a resection of the affected segment followed by intestinal anastomosis. The patient had a successful recovery and was discharged seven days later. The present article also reviews the classical presentation of a bowel perforation following abdominal liposuction.
Abdominoplasty after massive weight loss. Safety preservation fascia technique and clinical outcomes in a large single series-comparative study

Abstract:Introduction Weight loss after bariatric surgery causes very important modifications to the patient's silhouette. Abdominal fat and skin excess reduction are associated with several complications. The most frequent are seroma and hematoma whereas major complications, such as pulmonary embolism, are less frequent. This study aimed to describe our technical procedure for abdominoplasty in patients with massive weight loss after bariatric surgery. Methods In total, 196 patients were included. All patients who underwent abdominoplasty classic (group A) and abdominoplasty with the preservation and lift of Scarpa fascia (group B) and with umbilical transposition between May 2018 and May 2021 were included. Patients with concomitant correction of ventral hernia were excluded. Demographic and operative data were analyzed according to comorbidities and postoperative complications. Results There were 160 (81.6%) women. The mean age was 43.6 years; the mean weight was 86.7 kg; and the mean BMI was 28.6 kg/m 2 . Five patients (2.5%) presented postoperative seroma. Four patients (2%) presented partial dehiscence/skin necrosis one of them requiring a revision. Finally, 26 patients presented a postoperative complication, with an overall incidence of 12.6%. The average postoperative hospital stay was 3.6. The rates of seroma were significantly higher in men, patients with a BMI > 30 kg/m 2 , and aged >50 years. Conclusion Preserving Scarpa Fascia during surgical post-bariatric patient procedures reduces the seroma formation and the scar complication and reduces the tension of the inguinal-pubic region with correction of our deformation after weight loss. Improves reducing the drain and reducing seroma incidence suction and hospital stay.

Abstract:Conventional abdominoplasty includes the removal of an ellipse-shaped section of abdominal tissue between the umbilicus and mons pubis. However, this method can result in tension of the undermined flap, especially in the midline. To address this problem, we present reverse lip design as a modified method that also has aesthetic advantages. The reverse lip design entails a longer lower flap edge while preserving the triangular tissue in the vascularly stable pubis area. These markings create an image of a reverse lip shape with a cleft at the bottom of the lower markings. After typical lipoabdominoplasty is performed, redundant waist tissues can easily be pulled inward and downward. The reverse lip design abdominoplasty demonstrated no complications and required no further revisions after the procedure. Patients were generally satisfied with the aesthetic improvements in their body shape. They were also able to return to their routine activities approximately 1 week after the operation while wearing a supporting undergarment. This modified abdominoplasty using the reverse lip design reduces low midline tension of the undermined abdominal flap while enhancing body aesthetics with a slimmer waistline, leading to higher patient satisfaction.

Abstract:Abstract Background Complications after body-contouring surgery is a field of great interest. Recognition of patient-related characteristics contributing to a greater risk for complication is of the utmost importance in improving the quality of care and safety profile of aesthetic procedures. Prior history of cesarean section and its impact on adverse events after abdominoplasty has yet to be investigated and defined. Objectives Evaluate the effect of prior cesarean section on the risk for adverse events in abdominoplasty Methods The medical records of 746 eligible patients were reviewed and relevant information was extracted. Stratification of patients based on prior history of CS was followed by statistical analysis of differences between the groups. Relevant regression models were implemented to further understand the data. Results Seven-hundred and forty-six patients were included in the study, of which 308 (43.1%) had history of CS delivery. the group did not differ in terms of baseline demographic and surgical details, except for greater hemoglobin level (P value= 0.007). analysis of complication rates found a statistically significant increase in the risk for seroma development in the group of patients with history of CS delivery (P value= 0.031) which correlates to a 65% increase in the risk for complications (OR= 1.65, 95% CI 1.07-2.56). Conclusions Medical history of CS increased the risk for seroma development in the postoperative period following abdominoplasty. Several potential pathogenic mechanisms exist, and further prospective research to further characterize the association.

Abstract:We describe the optimization of a mounting system for the infrared (IR) optics of a spaceborne interferometer working in the temperature range between -120 degrees C and +150 degrees C. The concept is based on an aluminum alloy frame with designed mechanical compliance, which allows for compensation of the different coefficient of thermal expansion between the optics and the holder; at the same time, the system provides for the high stiffness required to reach natural frequencies above 200 Hz, which are mandatory in most space missions. Thermal adapters with properly chosen thermomechanical characteristics are interposed between the metallic structure and the lens, so as to reduce the interface stresses on the mechanically weak IR material, due to both the thermoelastic and acceleration loads. With the proposed mount, the competitive requirements of stiffness and stress-free mounting can be matched in wide temperature ranges. The case study of the interferometer of a miniaturized Fourier transform IR spectrometer is presented.

Abstract:Abstract Liposuction is considered one of the most common procedures in plastic surgery. However, major postoperative complications such as visceral injury, fluid overload, and necrotizing fasciitis still occur. Likewise, minor complications such as ecchymosis, seromas, infections, and contour irregularities that do not threaten the life of the patient do generate significant dissatisfaction. Current evidence regarding the management of fibrosis after previous liposuction remains limited. The objective of this article is to standardize a management algorithm based on the extensive experience and successful results of the primary author. Patients who underwent secondary liposculpture between August 2022 and May 2023 were evaluated prospectively. Inclusion criteria: Women between 18 and 60 years, non-smokers, with a body mass index (BMI) <35 kg/m2, history of previous body contouring surgeries. Identification of the patient’s skin condition and subcutaneous lesions in the adipose tissue were obtained in detail. Statistical analysis of preoperative and postoperative medical photographs was also performed with Fiji Biological image analyzer. Photographic analysis showed a statistically significant difference between the areas affected by fibrosis from the preoperative photos compared to that from the postoperative ones (p<.001). The most frequent clinical findings were depressions in 99% of the women (74), followed by soft nodules in 95% (70), hard nodules in 81% (61), adherences in 47% (35), and finally, cutaneous bursas in 4%. Our classification system and management algorithm for fibrosis and contour irregularities is a safe and reliable tool and results were objectively verified, yielding statistically significant outcomes.

Abstract:Summary: Decades of lipoabdominoplasty safety, reliability, and efficacy have permitted more aggressive contouring of the abdominoplasty flap without increased risk for wound healing complications. However, the recent introduction of unrestricted liposuction of all vascular zones and high-definition liposculpting continue to challenge the vascularity of the abdominoplasty flap, particularly in high-risk patients. The present study reviews the authors' approach to abdominal lipodystrophy, and introduces a technique which maximizes fat extraction and skin excision while reducing risk of complication (two-stage radiofrequency-assisted liposuction and abdominoplasty).

Abstract:Pancreatic pseudocysts (PPs) are collections of pancreatic secretions that are lined by fibrous tissues and may contain necrotic debris or blood. The interventions including percutaneous, endoscopic or surgical approaches are based on the size, location, symptoms and complications of a pseudocyst. With the availability of advanced imaging systems and cameras, better hemostatic equipments and excellent laparoscopic techniques, most pseudocysts can be found and managed by laparoscopy. We describe a case of a 30-year-old male patient with a pancreatic pseudocyst amenable to laparoscopic cystogastrostomy. An incision was made through the anterior gastric wall to expose the posterior gastric wall in close contact with the pseudocyst using an ultrasonically activated scalpel. Then, another incision was made for cystogastrostomy to obtain complete and unobstructed drainage. The patient recovered well after operation and was symptom-free during a 6-mo follow-up, suggesting that laparoscopic cystogastrostomy is a safe and effective alternative to open cystogastrostomy for minimally invasive management of PPs.

Abstract:The aim of this study was to report a series of 3 cases of complications following submentonian and cervical liposuction associated with platysmaplasty. The complications include hemorrhage, Ludwig's angina, and paralysis caused by injury to the marginal mandibular nerve. The procedure was performed by the same operator using the same technique, and the patients were informed of the risks. It is important to emphasize that the surgeon must be prepared to manage any of these complications, and treatment should be immediate and appropriate. The management of complications following submentonian and cervical liposuction associated with platysmaplasty must be prompt and effective to prevent further risks and damage. The approaches are discussed and referenced by the authors. Submentonian and cervical liposuction surgery associated with platysmaplasty is a valuable tool for reducing neck fat and enhancing esthetic outcomes, but there is a possibility of postoperative complications, which, although rare, can occur. Proper patient selection and thorough explanation of risks are crucial to preventing some of these situations.

Abstract:Introduction We tried to apply a new surgical method to treat obesity combined with pancreatic pseudocyst and achieved satisfactory results. Case and presentation We report a case of a severely obese patient with pancreatic pseudocyst who underwent robotic-assisted sleeve gastrectomy, while the pseudocyst was incised and cyst-jejunostomy was performed. The operation was successful, and the patient was discharged on the 8th day after the procedure. There were no complications during the perioperative period. After 12 months of follow-up examinations, the patient's pancreatic pseudocyst disappeared. Additionally, there was a significant decrease in body weight, body mass index, and other indicators. As a result, obesity and related metabolic diseases were completely relieved. Conclusions This case summarizes and presents the experience of using robotic bariatric surgery for the treatment of pancreatic pseudocyst. This case report indicates that this surgical procedure is both safe and effective for patients with pancreatic pseudocyst who also have obesity and related metabolic diseases.

Abstract:Body contouring for weight loss patients usually requires not only lifting but also some extent of central body tightening to achieve complete tissue readjustment. Fleur-de-lis abdominoplasty provides best vector correction with nice contouring results, especially for central body type patients, with superior waist definition with or without associated circumferential belt lipectomies. Special interest has been addressed on technical refinements to enhance vertical scar with neoumbilicoplasty as well as the mons pubis projection with marking and suturing refinements. Both the inner scar umbilicus and the monsplasty are simple, safe, and reproducible techniques, presenting low complication rates with sustainable and natural results.
Dating Violence Victimization Among High School Students in Minnesota

Abstract:The present study examines whether being a victim of violence by an adult in the household, witnessing intra-familial physical violence, and feeling unsafe at school are associated with physical dating violence victimization. It also examines whether extracurricular activity involvement and perceived care by parents, teachers, and friends attenuate those relationships, consistent with a stress-buffering model. Participants were 75,590 ninth-and twelfth-grade students (51% female, 77% White, 24% receiving free/reduced price lunch) who completed the 2010 Minnesota Student Survey. Overall, 8.5% of students reported being victims of dating violence. Significant differences were found by gender, grade, ethnicity, and free/reduced price lunch status. Logistic regression analyses demonstrated that being a victim of violence by an adult in the household, witnessing intra-familial physical violence, feeling unsafe at school, and low perceived care by parents were strongly associated with dating violence victimization. Associations of moderate strength were found for low perceived care by teachers and friends. Little to no extracurricular activity involvement was weakly associated with dating violence victimization. Attenuating effects of perceived care and extracurricular activity involvement on associations between risk factors (victimization by a family adult, witnessing intra-familial violence, feeling unsafe at school) and dating violence victimization were smaller in magnitude than main effects. Findings are thus more consistent with an additive model of risk and protective factors in relation to dating violence victimization than a stress-buffering model. Health promotion efforts should attempt to minimize family violence exposure, create safer school environments, and encourage parental involvement and support.
Quality of life following liposuction for lipoedema: a prospective outcome study

Abstract:BACKGROUND: The study examines, for the first time, the impact on quality of life after liposuction for lipoedema. The influence of aesthetic plastic interventions and their effects on treatment outcomes has been a major focus of our research group over the past 20 years.METHODS: A total of 35 patients were invited to participate in our prospective study, with 30 responding to both the pre- and postoperative questionnaires. The patients received the questionnaires pre-operatively, and 6 months after the liposuction. Our questionnaire set included a self-developed, indication-specific part, along with standardised and validated questionnaires such as the Questions on Life Satisfaction (FLZ<sup>M</sup>), Patient Health Questionnaire (PHQ-4), Rosenberg Self-Esteem Scale (RSES) and the Freiburg personality inventory-revised (FPI-R).RESULTS: Our self-developed questionnaire showed that our patients feel more balanced, more attractive and more self-confident after the treatment. The FLZ<sup>M</sup> showed significant improvements in all three modules: the general satisfaction with life, the state of health and the outer appearance (body image). Using the PHQ-4, a significant improvement in the two subscales of anxiety and depression could be determined, as well as a reduction in overall mental stress. In addition, the RSES showed a significant improvement in self-esteem post-operatively. Furthermore, the FPI-R indicated a significant improvement in emotional stability.CONCLUSIONS: Liposuction improves the quality of life in lipoedema patients. Post-operatively, our patients reported less pain and were more satisfied with their bodies and appearance. The hypothesis that liposuction in lipoedema improves the quality of life as a multidimensional construct could be confirmed.

Body contouring improves physical, psychosocial, and sexual function therefore providing the best possible aesthetic outcome for massive weight loss patients is important. The Fleur-de-Lis pattern should be strongly considered when dealing with moderate to significant horizontal skin excess of the abdomen to obtain the best possible contouring and create an attractive female contour of hip and waist.
