Abstract:Patellofemoral pain is one of the most common knee complaints, particularly among physically active young individuals. Although once thought to be self-limiting, prospective studies have demonstrated the propensity towards the chronicity of patellofemoral pain (PFP). The pathogenesis of PFP is complex, with multiple interactive pathways suggested to contribute to its onset and persistence. Quadriceps weakness is one of the few risk factors of PFP, with at least moderate evidence from prospective studies, although limited clinical trials of lower limb strengthening have generally not been successful in preventing PFP. The challenge of managing PFP is reflected by a lack of evidence-based clinical guidelines. International consensus and current evidence recommends exercise therapy, focussed on hip and knee strengthening, as a cornerstone of management to reduce PFP. Rethinking management approaches beyond exercise therapy to incorporate movement retraining, education and psychosocial aspects provides potential avenues to enhance outcomes for patients with PFP.
Neuroaudiological findings in chronic primary fibromyalgia with dysesthesia.

Abstract:Thirty-six patients (31 females and 5 males) with "chronic primary fibromyalgia" combined with dysesthesia were studied using neuroaudiological tests. Auditory brainstem response (ABR) was pathological in 11 patients (31%). In six instances the I-V interpeak latency (IPL) was prolonged, in two instances the interaural difference of wave V (IT5) was pathological, and in three instances there was a reduction of the amplitude of wave V. A comparison of the I-V IPLs between the female patient group and a matching control group showed that there was a significant prolongation of this parameter in the patient group. The ABR-findings indicate that a brainstem dysfunction might be present in some patients with fibromyalgia. The results of the impedance audiometric tests were generally normal in the patient group.

Abstract:Patellofemoral pain syndrome, which accounts for 25% of all sports-related knee injuries, is multifactorial in origin. A combination of variables, including abnormal lower limb biomechanics, soft-tissue tightness, muscle weakness, and excessive exercise, may result in increased cartilage and subchondral bone stress, patellofemoral pain, and subtle or more overt patellar maltracking. Because of the multiple forces affecting the patellofemoral joint, the clinical evaluation and treatment of this disorder is challenging. An extensive search of the literature revealed no single gold-standard test maneuver for that disorder, and the reliability of the maneuvers described was generally low or untested. An abnormal Q-angle, generalized ligamentous laxity, hypomobile or hypermobile tenderness of the lateral patellar retinaculum, patellar tilt or mediolateral displacement, decreased flexibility of the iliotibial band and quadriceps, and quadriceps, hip abductor, and external rotator weakness were most often correlated with patellofemoral pain syndrome.

Prepatellar Friction Syndrome: a common cause of knee pain in the elite cyclist

Abstract:Background: Although anterior knee pain is extremely common in high-level road cyclists, the exact etiology still remains unclear. Methods: A group of 28 professional male elite cyclists diagnosed with Prepatellar Friction Syndrome (PPFS) were retrospectively reviewed with specific attention for the typical history, clinical findings and treatment modalities. Results: A traumatic onset of the complaints was reported by 10 athletes, while the complaints were caused by chronic overuse in the remaining 18 subjects. Conservative treatment delivered poor results and all cases were eventually treated surgically. Surgery confirmed macroscopic damage to at least one prepatellar fascial layer in all patients, after which partial fasciectomy was performed through a mini incision. Conclusion: PPFS is a new clinical entity of the triple-layered prepatellar fascial structures. Correct diagnosis is critical and based on the typical history and clinical findings. Partial prepatellar fasciectomy is the treatment of choice in order to regain the pre-injury performance level.
Comprehensive Management of Patellofemoral Pain Syndrome in a Recreational Long-Distance Runner: A Case Report

Abstract:Patellofemoral pain syndrome (PFPS) is a major concern in the field of orthopedic medicine, affecting a substantial portion of the population and significantly impacting the quality of life. This condition, characterized by anterior knee pain increasing with various activities, shows higher susceptibility in certain demographic groups, such as women and adolescents. PFPS arises from a multifactorial etiology involving anatomical, biomechanical, psychological, and social factors, making it a complex condition to manage. Despite numerous therapeutic interventions available, including strengthening exercises, manual therapy, and patellar realignment techniques, the long-term efficacy of these interventions remains debated. This case report describes the case of a 21-year-old female recreational long-distance runner with bilateral knee pain diagnosed with PFPS. Through a comprehensive intervention plan targeting strength, function, flexibility, proprioception, and pain management, significant improvements were observed in pain levels and functional outcomes after physiotherapy intervention. This case underscores the importance of a holistic approach in managing PFPS and highlights the need for further research to optimize treatment strategies and improve patient outcomes.
Clinicians' experience of the diagnosis and management of patellofemoral pain: A qualitative exploration

Abstract:Background: Patellofemoral pain (PFP) is common and has a poor long-term prognosis. There is a lack of clarity about the clinical reasoning of recognised inter-disciplinary experts in the published literature. Objectives: To help identify best practice by exploring the clinical reasoning of a range of inter-disciplinary experts that regularly diagnose and treat PFP. Design: Qualitative study with semi-structured interviews. Method: Recruitment resulted in a convenience sample for semi-structured interview, which were recorded and transcribed verbatim. Data were analysed until theoretical saturation, as determined by multiple investigators. Findings: Interviews with 19 clinical experts (15 men, 4 women; mean experience 18.6 years ± 8.6) from four broad professions yielded four themes. Firstly, the assessment and diagnosis process should include a thorough history and examination to rule in PFP. Secondly, information provision should aim to increase patients' understanding, aid in controlling symptoms, and facilitate behaviour change. Thirdly, active rehabilitation, which was a salient theme and included advocacy of combined hip and knee exercise that is adapted to the individual. Finally, treatment adjuncts, which can be used selectively to modify symptoms, may include running retraining, taping, or foot orthoses. Conclusions: PFP should be diagnosed clinically, and tailored treatment programmes should be prescribed for people with PFP. Exercise was considered the most effective treatment and underlying psychological factors should be addressed to improve prognosis.

Abstract:Background: Patellofemoral pain syndrome (PFPS) is one of the common injuries to the knee that often occur, causing pain when doing physical activities such as squatting, running or going up and down stairs when the knee is bent. As the joint that supports the body the most, this certainly hinders a person's activities. PFPS is caused by various risk factors ranging from intrinsic factors such as muscle strength to extrinsic factors such as type of activity. This study aimed to determine the risk factors related to PFPS as a preventive effort by doing quadriceps muscle strengthening exercises. Methods: This study used a literature review study design using secondary data from published literature. Literature searches were conducted online through PubMed, ScienceDirect, and Google Scholar, using the keywords “knee injury”, “patellofemoral pain”, “patellofemoral pain syndrome”, and “risk factors”, which selected based on inclusion and exclusion criteria. Results: From the five studies that discussed risk factors related to PFPS, it was found that all journals showed quadriceps muscle strength, type of daily activity, Q-angle, joint laxity, hip tightness, patellar imbalance, overuse, and patellofemoral joint overpressure were associated with PFPS. Conclusion: Based on the literature that has been reviewed and the discussion, the dominant risk factor associated with PFPS was a decrease in hip muscle strength, especially quadriceps. Besides, several other contributing factors include the type of daily activity, Q-angle, joint laxity, hip tightness, patellar imbalance, overuse, and patellofemoral joint overpressure.

Abstract:Objective To explore the effect and factors of patellofemoral arthroplasty (PFA) for isolated patellofemoral osteoarthritis.Methods Data of 18 patients (3 males,15 females) with isolated patellofemoral osteoarthritis underwent PFA from March 2006 to December 2014 were retrospectively analyzed.There were 3 males and 15 females with a mean age of 54 years (range,46-74 years).It was strict to grasp the surgical indications according to the clinical symptoms,signs and imaging data preoperatively.11 patients were operated with AVON patellofemoral prosthesis (Stryker Inc.) and the other 7 patients were operated with the Gender Solutions patellofemoral prosthesis (Zimmer Inc.).Active and passive functional rehabilitation exercise was encouraged at the early stage after operation.Visual analogue scale (VAS) was 5.33±0.99 (range,4-7) and hospital for special surgery knee score (HSS) was 53.28±5.71 (range,44-63) before operation.Results The mean duration of follow-up was 63.98 months (range,6-104 months).VAS after operation for 1 and 3 months were 1.17±0.79 (range,0-3) and 0.72±0.67 (range,0-2),and the pain was almost relieved after 9 months.HSS after operation for 1,3,9 months were 70.06±6.33 (range,61-80),86.06±5.12 (range,77-95) and 91.39±4.83 (range,82-97).HSS score of the latest follow-up was 92.06±4.05 (range,84-97),which was improved obviously from the preoperative ones.The excellent and good rate was 100％ (excellent 15 cases,good 3 cases).The satisfactory rate was 94.4％ (17/18).Only one case got slightly knee pain when walking up and down the stairs after 2 years,and the pain was relieved after being administered with NSAIDs and rest.No incision infection,rupture,prosthesis supported bone fracture,prosthesis loosening and other complication was occurred during the follow-up period in the other patients.Conclusion The clinical outcomes of PFA are strictly related to surgical indications,implant design and appropriate surgical technique.Therefore,based on the appropriate PFA implants,strict surgical indications,appropriate patients,excellent operation skills and actively functional rehabilitation exercise,PFA could treat the isolated patellofemoral osteoarthritis effectively.

Abstract:Die isolierte Patellofemoralarthrose ist eine komplexe Pathologie. Sie tritt gehäuft bei Frauen über 40 Jahren auf, ist mit einem hohen Leidensdruck verbunden und erfordert eine stadiengerechte Therapie. Die Ursachen einer femoropatellaren Degeneration sind vielfältig. Dieser Beitrag soll eine strukturierte Übersicht zu Symptomatik, Diagnostik und stadiengerechter Therapie der Patellofemoralarthrose liefern. Isolated patellofemoral osteoarthritis is a complex pathology. It occurs frequently in women over 40 years and leads to a high degree of suffering. The causes of femoropatellar degeneration are manifold and require stage-specific therapy. This is very challenging for the treating physician. This article is intended to provide a structured overview of the symptoms, diagnostics and stage-specific therapy of patellofemoral osteoarthritis. Die häufigste Form der Patellofemoralarthrose (PFA) ist die sekundäre Arthrose, verursacht durch Malalignment/Maltracking der Patella (meist aufgrund patellofemoraler Instabilität). Typisches Symptom sind ein vorderer/ventraler Knieschmerz, verstärkt beim Treppenhinabsteigen oder In-die-Hocke-Gehen. Bei der klinischen Untersuchung sollten im Rahmen der Beurteilung von Stand- und Gangbild auch mögliche Achs- oder Torsionsfehlstellungen erfasst werden. Konventionelle Röntgendiagnostik, ggf. erweitert durch MRT, gilt als Goldstandard, Computertomografie und Rotationsanalyse sind Einzelfällen vorbehalten. Die konservative Therapie über mehrere Monate mit Koordinationsschulung, Muskelkräftigung, Dehnung/Lockerung verkürzter Muskelgruppen, Körpergewichtsmanagement und Belastungsmodifikation ist Mittel der Wahl als First-Line-Therapie. Gelenkerhaltende weichteilige/knöcherne Eingriffe sind nur bei fehlender/milder PFA (Iwano-Stadium ≤ 2) indiziert. Bei höhergradiger PFA (Iwano-Stadium > 2) sind Inlay-Prothesen möglicherweise den Onlay-Prothesen überlegen hinsichtlich der tibiofemoralen Arthroseprogression. Abstract:Patellofemoral pain (PFP) is one of the most common sports injuries of the knee joint and has a high persistence and recurrence rate. Medio-lateral patellar position in the knee extension position during contraction is associated with PFP. However, soft tissue tension that most influences the medio-lateral patellar position in the knee extension position during contraction in vivo is unclear. We aimed to clarify the relationship between medio-lateral patellar position and soft tissue tension around the knee joint. Twelve patients with PFP and 20 healthy participants were included. Medio-lateral patellar position and tension of the rectus femoris, vastus lateralis (VL), vastus medialis, iliotibial band (ITB), lateral patellofemoral ligament, and medial patellofemoral ligament were measured during contraction and rest. The tensions of the VL and ITB during contraction and the medio-lateral patellar position at rest were significantly associated with medio-lateral patellar position during contraction (β = 0.449, 0.354, and 0.393, respectively). In addition, the tension of ITB was significantly associated with the medio-lateral patellar position at rest (β = 0.646). These relationships were not affected by the presence of PFP. These findings suggest that the patellar position during contraction became more lateral as the tension in the VL and ITB increased, regardless of the presence of PFP. These results may facilitate the prevention and treatment of PFP.

Abstract:Purpose: Patellofemoral pain (PFP) is the leading cause of knee pain in runners. Proximal and distal running mechanics have been linked to the development of PFP. However, the lack of prospective studies limits establishing a causal relationship of these mechanics to PFP. The purpose of this study was to prospectively compare running mechanics in a group of female runners who went on to develop PFP compared with healthy controls. It was hypothesized that runners who go on to develop PFP would exhibit greater hip adduction, hip internal rotation, and greater rear foot eversion. Methods: Four hundred healthy female runners underwent an instrumented gait analysis and were then tracked for any injuries that they may have developed over a 2-yr period. Fifteen cases of PFP developed, which were confirmed by a medical professional. Their initial running mechanics were compared to an equal number of runners who remained uninjured. Results: We found that female runners who developed PFP exhibited significantly greater hip adduction (P = 0.007). No statistically significant differences were found for the hip internal rotation angle (P = 0.47) or rear foot eversion (P = 0.1). Conclusions: The finding of greater hip adduction in female runners who develop PFP is in agreement with previous cross-sectional studies. These results suggest that runners who develop PFP use a different proximal neuromuscular control strategy than those who remain healthy. Injury prevention and treatment strategies should consider addressing these altered hip mechanics.

Abstract:Patellofemoral pain syndrome (PFPS) is a common musculoskeletal condition characterized by an insidious onset of pain in the knee anterior/retro-patellar/peripatellar region. The pain worsens with loading activities of the lower limb, such as squatting, stair climbing, uphill running, and prolonged sitting, which may severely affect the ADLs in the general population and sports participation in athletes. [ 1 ] PFPS is considered one of the most prevalent knee pain diagnoses in sports injury clinics, accounting for around one-fourth of all medical consultations for knee pain, with an annual prevalence of 22.7% in the general population and 29.2% in females and 15.5% in males. [ 2 ] A conservative treatment approach which includes relative rest, quadriceps strengthening, hip muscle strengthening, proprioceptive training, neuromuscular electrical stimulation, manual therapy, and electrotherapy modalities, is the first line of treatment for PFPS. [ 3 ]

Abstract:Patellofemoral pain syndrome (PFPS) is one of the most common knee joint injuries, characterized by anterior or peripatellar pain resulting from pathological or biomechanical abnormalities of the patellofemoral joint. It is commonly provoked by functional activities such as running, jumping, squatting, stair climbing, and prolonged sitting with flexed knees. [ 1 ] Epidemiological studies have shown that PFPS primarily affects young individuals aged 18 to 35 years, particularly adolescent athletes, with an annual incidence rate of approximately 22 per 1000 individuals. [ 2 ] Without timely and effective treatment, Long-term follow-up studies have revealed that 45% of patients initially diagnosed with PFPS subsequently develop osteoarthritis, significantly impairing their daily activities and quality of life. [ 3 ] Possible mechanisms for the development of PFPS have been found to be related to joint pain, which in turn leads to reflexive inhibition of the periarticular muscles, atrophy of lower extremity muscles (especially vastus medial oblique [VMO] and gluteus maximus [GMAX]), and reduction of muscle strength, muscle imbalance, creating a vicious cycle that further exacerbates pain and continues to reduce knee function and cause long-term discomfort. In addition, weak gluteal muscles can also lead to impaired control of stability in the coronal and horizontal planes of the pelvis, triggering an abnormal patellar trajectory, increased patellofemoral joint stress, increased cartilage wear and an increased risk of PFPS injury. [ 4 , 5 ]

Abstract:Background: Patellofemoral pain syndrome (PFPS) is one of the most common causes of anterior knee pain encountered in the outpatient setting. The purpose of this study was to compare the lower limb biomechanical differences during anticipated and unanticipated side-cutting in athletes with PFPS. Methods: Fifteen male basketball players diagnosed with PFPS were enrolled in the study. Participants executed both anticipated and unanticipated 45-degree side-cutting tasks. Motion analysis systems, force plates, and electromyography (EMG) were used to assess the lower limb joint angles, joint moments, joint stiffness, and patellofemoral joint contact forces. Analyzed biomechanical data were used to compare the differences between the two circumstances. Results: Unanticipated side-cutting resulted in significantly increased ankle plantarflexion and dorsiflexion angles, knee abduction and internal rotation angles, and hip abduction angles, as well as heightened knee adduction moments. Additionally, patellofemoral joint contact forces and stress increased, while contact area decreased during unanticipated tasks. Conclusions: Unanticipated movement raises the demands for joint stability and neuromuscular control, increasing injury risks in athletes with PFPS. These findings have practical implications for developing targeted rehabilitation programs and injury prevention strategies.

Altered hip and knee kinematics and increases in weekly distance and running pace are often associated with PFP development and exacerbation.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Research question</h3><p>Are altered movements and training load characteristics (weekly distance and running pace) relate to pain intensity or physical function level in runners with PFP?</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Forty recreational runners with PFP (20 males and 20 females) participated in this cross-sectional observational study. Three-dimensional hip and knee kinematics were quantified during the stance phase of running. Weekly distance was defined as the average weekly kilometers of running and running pace as the average pace of the activity measured as minutes per kilometer. A visual analogue scale was used to evaluate worst knee pain during the last week. The anterior knee pain scale (AKPS) was used to evaluate knee functional score. A Pearson correlation matrix was used to investigate the association between each dependent variable (worst pain in the last week and AKPS score) and the independent variables (knee and hip kinematics, weekly distance and running pace)</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>There was no significantly correlation between kinematic variables, pain and functional score for both males and females separately and combined. Weekly distance (km/week) was found to positively correlate to pain intensity (r = 0.452; p < 0.05) in females with PFP. A simple linear regression revealed that weekly distance was significant predictor emerged of pain in females with PFP. Females exhibited significantly greater peak hip adduction and hip adduction ROM than the males and males had significantly greater running pace compared to females.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Significance</h3><p>Weekly distance should be considered in the clinical context during rehabilitation of PFP in females runners aiming at pain reduction.</p>

Abstract:Patellofemoral pain (PFP) is a common knee disorder in orthopedic clinics. In the last decade, several investigations have considered the role of proximal factors in addition to local and distal factors in development of PFP. There is a hypothesis which suggests that impaired neuromuscular control and altered hip joint kinematic affect tibiofemoral and patellofemoral biomechanics. Hence, PFP may develop as a result. This article reviews studies assessing the relationship between altered hip mechanics and PFP. The Medline and PubMed databases were searched between January 2004 and October 2014. Two authors independently selected related articles using the same search strategy and key words. Among 149 articles, 16 met the review inclusion criteria. The study results were described in three sections: 1) kinematic studies, 2) muscle activity studies, and 3) postural stability studies. Increased hip adduction and internal rotation, gluteal muscle weakness, and neuromuscular impairment were common findings in patients with patellofemoral pain. Precise assessment of hip mechanics, including hip kinematic, muscle performance and postural stability, should be considered in the examination of patients with patellofemoral pain.

Abstract:OBJECTIVES This study aims to evaluate the effect of intra-articular tranexamic acid (TA) administration on the amount of blood in the drains, the amount of blood transfusion, length of hospital stay, and the total cost in total knee arthroplasty (TKA) patients. PATIENTS AND METHODS The study included a total of 118 patients (24 males, 94 females; mean age 67.0 years, range 52 to 81 years) who underwent primary unilateral TKA surgery. Patients were divided into two groups as 58 patients who were performed intra-articular injection of 2 g TA after closure of articular capsule (TA group; 10 males, 48 females, mean age 65.6 years; range 55 to 80 years) and 60 patients who were not performed any intra-articular injection (non-TA group; 14 males, 46 females; mean age 68.3 years; range 52 to 81 years). The maximum difference between pre- and postoperative hemoglobin levels, the amount of blood in the drains, the amount of blood transfusion, length of hospital stay, and cost of each patient were recorded. RESULTS The mean difference between pre- and postoperative hemoglobin levels were higher in the non-TA group (1.7 g/dL vs 2.6 g/dL, p<0.05). The mean amounts of blood in drains (327.5 mL vs 800.0 mL, p<0.05), the amounts of blood transfusions (0 units vs 10 units, p<0.05), the mean lengths of hospital stay (4.03 days vs 4.53 days, p<0.05), and the hospital costs (1,935.26 United States dollars vs 1,959.64 United States dollars, p<0.05) were lower in TA group. CONCLUSION Administration of intra-articular TA during primary TKA may reduce blood loss and the need for blood transfusion while significantly shortening the postoperative length of hospital stay and reducing the total hospital costs.

Abstract:Background: Anterior knee pain (AKP) is a common musculoskeletal complaint including pathologies like patellofemoral pain syndrome (PFPS), patellar or quadriceps tendinopathy and patellar instability. PFPS is the commonest of all, described as retro-patellar or anterior knee aching with diffuse pain most prominent when ascending or descending stairs, squatting, or sitting for prolonged periods with the knees flexed. It is a multifactorial condition with various treatment approaches being suggested. However, there is no clear physiotherapy protocol to be followed for the same. Objectives: This review aimed at finding recent advances in rehabilitation approaches (non-surgical and non-pharmaceutical) for patients with PFPS in last 5 years. Methods: 35 published articles were found from Google scholar, PubMed (NLM) and PEDrO databases and 19 studies were included in this reviewed. Conclusion: Five treatment protocols emerged from the reviewed articles: hip and knee muscle strengthening, core muscle strengthening, patellar taping, stretching and spinal mobilization. The current review suggested that a multimodal conservative management is more beneficial in these kind of patients. Keywords: Anterior knee pain, Physiotherapy, Patellofemoral pain.
