Abstract:Background A retrospective utilization study was performed to evaluate utilization patterns for enteral nutrition in a university teaching hospital. Methods Enteral nutrition was divided into three types according to the nitrogen source, ie, total protein type [Nutrison Fibre®, Fresubin Energy Fibre®, Fresubin®, Supportan® (a special immunonutrition for cancer patients or patients with increased demands for omega-3 fatty acids), Fresubin Diabetes® (a diabetes-specific formula), Ensure®]; short peptide type (Peptison®); and amino acid type (Vivonex®). A pharmacoeconomic analysis was done based on defined daily dose methodology. Results Among hospitalized patients taking enteral nutrition, 34.8% received enteral nutrition alone, 30% concomitantly received parenteral nutrition, and 35.2% received enteral nutrition after parenteral nutrition. Combined use of the different formulas was observed in almost all hospitalized patients receiving enteral nutrition. In total, 61.5% of patients received triple therapy with Nutrison Fibre, Fresubin Diabetes, and Supportan. Number of defined daily doses (total dose consumed/defined daily dose, also called DDDs) of formulas in descending order were as follows: Nutrison Fibre, Fresubin Energy Fibre, Fresubin Diabetes > Supportan > Peptison, Ensure > Vivonex, Fresubin. The ratio of the cumulative DDDs for the three types of enteral nutrition was 35:2.8:1 (total protein type to short peptide type to amino acid type). Off-label use of Fresubin Diabetes was also observed, with most of this formula being prescribed for patients with stress hyperglycemia. Only 2.1% of cancer patients received Supportan. There were 35 cases of near misses in dispensing look-alike or sound-alike enteral nutrition formulas, and one adverse drug reaction in an elderly malnourished patient who did not receive vitamin K1-enriched enteral nutrition during treatment with cefoperazone. After 4 months of the trial intervention, off-label use of Fresubin Diabetes was no longer endorsed by the Drug and Therapeutics Committee for nondiabetic patients, and the proportion of this formula prescribed for patients with stress hyperglycemia decreased by 20%, with a 10-fold increase in the amount of Supportan prescribed for cancer patients. Near misses in dispensing look-alike or sound-alike enteral nutrition were successfully abolished, and no severe coagulation disorders occurred after prophylactic administration of vitamin K1-enriched enteral nutrition in elderly malnourished patients receiving cefoperazone. Conclusion This utilization study indicates that continuous quality improvement is necessary and that a Drug and Therapeutics Committee can play an important role in promoting rational and safe use of enteral nutrition. Appropriateness of this therapy still needs to be improved, especially in addressing the issues of non-evidence-based combined use of multiple enteral nutrition formulas, the relatively high rate of concomitant use of enteral and parenteral nutrition, off-label use of diabetes-specific Fresubin Diabetes, insufficient use of Supportan in cancer patients, and unnecessary use of Supportan in intensive care patients not suffering from cancer.

Abstract:A series of arylalkyltriazenes has been investigated for its differential cytotoxicity towards the HT-29 (Mer+) and BE (Mer-) cell lines and for its ability to cause DNA strand breaks and cross-links. A monomethyltriazene (MMPT) and some hydroxymethyltriazene derivatives capable of generating the monomethyltriazene in situ were preferentially cytotoxic to the BE cell line when compared with the HT-29 cell line. The differential toxicity of MMPT is very similar to the analogous monochloroethyltriazene. In contrast, the dimethyl- and monoethyltriazenes in the series display reduced toxicity towards the BE cell line with little or no differential toxicity between the BE and HT-29 cell lines. MMPT and monochloroethyltriazene caused single strand DNA breaks in the two cell lines, whereas little or no DNA strand breaks were observed in either cell line after exposure to the monoethyl- or dimethyltriazene. However, these lesions could not account for the differential cytotoxicity observed. In measurements of DNA interstrand cross-linking none of the agents tested, including monochloroethyltriazene and MMPT, was found to cause such linkages. In contrast to previous results obtained with bifunctional monochloroethylating agents, which produced a similar differential cytotoxicity between these two cell lines, our results tend to suggest that lesions other than DNA interstrand cross-links may be responsible for the mechanisms of cell killing by chloroethylating agents.

Abstract:Dialysis has been the dominant treatment regimen in end-stage kidney disease as a means to remove uremic waste products and to maintain electrolyte, acid base, and fluid balance. However, given that dialysis may not always provide a survival benefit nor improved quality of life in certain subpopulations, there is growing recognition of the need for conservative and preservative management as an alternative treatment strategy for advanced chronic kidney disease (CKD). Personalized nutritional management tailored to patient's sociodemographics, social needs, psychological status, health literacy level, and preferences is a key component of conservative and preservative care, as well as in the management of patients transitioning from non-dialysis dependent CKD to dialysis. In this review, we discuss the nutritional and metabolic alterations that ensue in CKD; the rationale for low-protein diets in the conservative and preservative management of advanced CKD; the role of plant-based diets in kidney health; emerging data on dietary potassium and sodium intake on CKD outcomes; and the practical implementation of dietary interventions in advanced kidney disease.

Abstract:Providing optimal nutrition support in the intensive care unit (ICU) is a challenging and dynamic process. Energy, protein, fluid, electrolyte, and micronutrient requirements all can be altered in patients with acute, chronic, and acute‐on‐chronic kidney disease. Given that renal dysfunction occurs in up to one‐half of ICU patients, it is imperative that nutrition support providers understand how renal dysfunction, its metabolic consequences, and its treatments, including renal replacement therapy (RRT), affect patients' nutrition needs. Data on nutrient requirements in critically ill patients with renal dysfunction are sparse. This article provides an overview of renal dysfunction in the ICU and identifies and addresses the unique nutrition challenges present among these patients, including those receiving RRT, as supported by the available literature and guidelines.

This article addresses the key considerations of supportive care as part of integrated end-stage kidney disease care, with particular attention paid to programs in low- and middle-income countries. Supportive care should be an integrated component of care for patients with advanced chronic kidney disease, patients receiving kidney replacement therapy (KRT), and patients receiving non-KRT conservative care. Five themes are identified: improving information on prognosis and support, developing context-specific evidence, establishing appropriate metrics for monitoring care, clearly communicating the role of supportive care, and integrating supportive care into existing health care infrastructures. This report explores some general aspects of these 5 domains, before exploring their consequences in 4 health care situations/settings: in people approaching end-stage kidney disease in high-income countries and in low- and middle-income countries, and in people discontinuing KRT in high-income countries and in low- and middle-income countries.

Abstract:While dialysis has been the prevailing treatment paradigm for patients with advanced chronic kidney disease (CKD), emphasis on conservative and preservative management in which dietary interventions are a major cornerstone have emerged. Based on high-quality evidence, international guidelines support the utilization of low-protein diets as an intervention to reduce CKD progression and mortality risk, although the precise thresholds (if any) for dietary protein intake vary across recommendations. There is also increasing evidence demonstrating that plant-dominant low-protein diets reduce the risk of developing incident CKD, CKD progression, and its related complications including cardiometabolic disease, metabolic acidosis, mineral and bone disorders, and uremic toxin generation. In this review, we discuss the premise for conservative and preservative dietary interventions, specific dietary approaches used in conservative and preservative care, potential benefits of a plant-dominant low-protein diet, and practical implementation of these nutritional strategies without dialysis.

Abstract:PURPOSE OF REVIEW: Acute critical illness increases the risk of malnutrition, are more obese, and have multiple comorbidities and frequent pre-existing nutritional deficits. There is a vast amount of research and literature being written on nutritional practices in the critically ill. We review and discuss herein the important nutrition literature over the past 12 months.RECENT FINDINGS: Sarcopenia, defined as loss of skeletal mass and strength, is associated with increased mortality and morbidity, particularly in elderly patients with trauma. Ultrasound is emerging as a noninvasive and promising method of measuring muscularity. Measuring gastric residuals and postpyloric feeding may not decrease rates of pneumonia in critically ill patients. Trophic and full feeding lead to similar long-term functional and cognitive outcomes in patients with acute respiratory distress syndrome.SUMMARY: Nutrition and metabolic support of critically ill patients is a complex and diverse topic. Nutritional measurements, requirements, and modes and routes of delivery are currently being studied to determine the best way to treat these complicated patients. We present just a few of the current controversial topics in this fascinating arena.

Abstract:Triggers leading to critical illness, including sepsis, respiratory failure, trauma, and burns, stimulate the metabolic response to stress. This response often leads to uncontrolled catabolism, which may culminate in a calorie deficit, proteolysis (and negative nitrogen balance), and anabolic resistance. Nutrition support was historically viewed as a means to reduce the calorie deficit and restore nitrogen balance. Today, nutrition, particularly enteral nutrition (EN), is viewed as a form of support therapy, a proactive means to preserve gut integrity, modulate inflammation, and maintain gut immunity. Unfortunately, the optimal dose and timing of nutrition support therapy is unknown. Some experts advocate for early and aggressive nutrition, while others maintain a “less is more” stance during the first week of critical illness. Since no two critically ill patients are alike, identifying which patients will benefit from early (and perhaps aggressive) nutrition support therapy is key. Validated scoring systems have emerged to guide the clinician in determining nutritional risk, or the risk of complications in critically ill adult patients who receive little to no nutrition. EN, for its non-nutritional benefits, is still preferred over parenteral nutrition (PN). PN is emerging as a safe and feasible alternative when EN is contraindicated or not tolerated. Since critical care survivors are often debilitated due to acquired loss of muscle mass, emerging observational data suggests protein may be the optimal macronutrient to target, irrespective of overall calorie goal. Clinicians should be aware of patient-, provider-, and institutional process-related barriers that may limit access to or impair nutrition delivery.

Abstract:Background/Objectives: Caregivers play an important role in supporting care recipients to navigate their health needs, including adherence to dietary recommendations, which are complex and multifaceted. This study aims to (i) describe the nutrition knowledge of caregivers of people with chronic kidney disease (CKD), and (ii) explore caregivers' perceptions of their role in providing healthy meals and nutrition support for care recipients. Methods: A cross-sectional study design employed a multi-strategy research approach. Caregivers (n = 78) of people with stage 1–5 CKD or post-transplant were recruited from a single centre. Their nutrition knowledge was assessed quantitatively with the revised General Nutrition Knowledge questionnaire. Theory-informed semi-structured interviews of a sub-sample (n = 12) qualitatively explored caregiver perceptions. Results: Most caregivers were female (75.6%) and cared for a male care recipient (87%; aged 74 (66; 80) yrs.). The caregivers (75.6%) provided a meal ≥6 times/week to their care recipient and had moderate nutrition knowledge (66.1 (60.5; 73.9)%). Four themes emerged describing the caregivers' perceptions of meal provision and nutrition support, including the following: (i) food literacy skills are valued; (ii) social support is important; (iii) caregivers' sense of social responsibility; and (iv) the management of complex and multifaceted dietary needs. Conclusions: The caregivers had moderate nutrition knowledge; they wanted to provide healthy meals and support to their care recipients to adhere to dietary recommendations. Targeted, co-designed nutrition education programs for caregivers may enhance nutrition care delivery to people with CKD.

Abstract:Nutrition plays a key role in chronic renal failure. In advanced uraemia the symptoms of anorexia, nausea and vomiting inevitably mean that the intake of nutrients is suboptimal. An additional cause of reduced food consumption is the widespread use of low protein diets for symptomatic treatment of this condition. It is therefore not surprising that numerous studies have shown that varying degrees of malnutrition are common in patients with chronic renal failure. It is perhaps more disturbing to discover that patients treated by long-term dialysis also have evidence of undernutrition1,2.

Abstract:With the growing number of dialysis patients with frailty, the concept of renal rehabilitation, including exercise intervention and nutrition programs for patients with chronic kidney disease (CKD), has become popular recently. Renal rehabilitation is a comprehensive multidisciplinary program for CKD patients that is led by doctors, rehabilitation therapists, diet nutritionists, nursing specialists, social workers, pharmacists, and therapists. Many observational studies have observed better outcomes in CKD patients with more physical activity. Furthermore, recent systematic reviews have shown the beneficial effects of exercise intervention on exercise tolerance, physical ability, and quality of life in dialysis patients, though the beneficial effect on overall mortality remains unclear. Nutritional support is also fundamental to renal rehabilitation. There are various causes of skeletal muscle loss in CKD patients. To prevent muscle protein catabolism, in addition to exercise, a sufficient supply of energy, including carbohydrates, protein, iron, and vitamins, is needed. Because of decreased digestive function and energy loss due to dialysis treatment, dialysis patients are recommended to ingest 1.2-fold more protein than the regular population. Motivating patients to join in activities is also an important part of renal rehabilitation. It is essential for us to recognize the importance of renal rehabilitation to maximize patient satisfaction.

Abstract:The nutritional management of neonates with kidney disease is complex. There may be significant differences in nutritional needs based on the duration and cause of kidney dysfunction, including acute kidney injury (AKI) and chronic kidney disease (CKD). Furthermore, the treatment modality, including acute (continuous renal replacement therapy and peritoneal dialysis [PD]) and chronic (intermittent hemodialysis and PD) approaches may differentially affect nutritional losses and dietary needs. In this review, we discuss the pathophysiology of compromised nutrition in neonates with AKI and CKD. We also summarize the existing data and consensus recommendations on the provision of nutrition to neonates with AKI and CKD. We highlight the paucity of data on micronutrient losses and the need for future prospective studies to enhance nutritional supplementation to hopefully improve outcomes in these patients.

Abstract:Chronic kidney disease (CKD) often requires several dietary adjustments to control the disease-related disturbances. This is challenging for both patients and healthcare providers, and particularly for dietitians, who deal closely with the poor adherence to dietary recommendations. Factors associated with poor adherence within the CKD scenario and the need for a shift in the paradigm have already been indicated in several studies; however, rarely are any different and/or potential strategies actually formulated in order to change this paradigm. In this review, we aimed to explore the concepts and factors surrounding adherence to dietary recommendations in CKD and further describe certain potential strategies for a nutritional counseling approach. Such strategies, while poorly explored within CKD, have shown positive results in other chronic disease scenarios. It is timely, therefore, for healthcare providers to acquire these new counseling skills; nevertheless, this would require a rethinking of the traditional attitudes and approaches in order to build a partnership, based on a nonjudgmental and compassionate style in order to guide behavior change. The reflections presented in this review may contribute towards enhancing motivation and the adherence to dietary recommendations in CKD patients.

Abstract:The Italian nephrology has a long tradition and experience in the field of dietetic-nutritional therapy (DNT), which is an important component in the conservative management of the patient suffering from a chronic kidney disease, which precedes and integrates the pharmacological therapies. The objectives of DNT include the maintenance of an optimal nutritional status, the prevention and/or correction of signs, symptoms and complications of chronic renal failure and, possibly, the delay in starting of dialysis. The DNT includes modulation of protein intake, adequacy of caloric intake, control of sodium and potassium intake, and reduction of phosphorus intake. For all dietary-nutritional therapies, and in particular those aimed at the patient with chronic renal failure, the problem of patient adherence to the dietetic-nutritional scheme is a key element for the success and safety of the DNT and it can be favored by an interdisciplinary and multi-professional approach of information, education, dietary prescription and follow-up. This consensus document, which defines twenty essential points of the nutritional approach to patients with advanced chronic renal failure, has been written, discussed and shared by the Italian nephrologists together with representatives of dietitians (ANDID) and patients (ANED).

Abstract:This study evaluated the effectiveness of individualized dietary interventions in improving the nutritional status and general health of individuals undergoing dialysis. A total of 120 patients undergoing dialysis were equally allocated to the Standard Care Group and Intervention Group. The recipients of individualized dietary counseling had their plans modified monthly in accordance with their nutritional needs, personal preferences, and clinical circumstances. Nutritional adherence, biochemical parameters, clinical outcomes, and quality of life were evaluated and compared between groups. At baseline, both groups exhibited comparable demographic characteristics. Compared to baseline values, the Intervention Group showed substantial enhancements in albumin, hemoglobin, potassium, and phosphorus levels after six months (P < 0.05). The Intervention Group had a considerably higher rate of adherence to dietary recommendations than the Standard Care Group (92% vs. 75%, P < 0.01). Nutritional intake analyses revealed a noteworthy enhancement in energy and protein consumption, while sodium, potassium, and phosphorus intake decreased (P < 0.05). The results of the quality-of-life evaluation demonstrated notable improvements in physical, emotional, social, and overall health perception scores (P < 0.05). Thus, personalized dietary interventions substantially improve the nutritional status, biochemical profile, and overall quality of life of patients undergoing dialysis.

Abstract:The nutritional status and management of children with chronic kidney disease (CKD) are complex and require a combined pediatric nephrology team work approach with physicians, nutritionists, nurses, and physical/occupational therapists. Prospective observational studies such as Children with CKD in the US, the 4C study in Europe and the International Pediatric Peritoneal Dialysis Network have advanced the field. However, most recommendations and guidelines from international task forces such as Kidney Diseases Improving Global Outcomes and Pediatric Renal Nutrition Taskforce are opinion-based rather than evidence-based. There is exciting ongoing research to improve nutrition in children with CKD to help them thrive.
