Burnout and Well-Being in Trainees: Findings From a National Survey of US Obstetrics and Gynecology Residents

Abstract:Background Despite national attention on resident well-being, challenges persist. Effective solutions require greater understanding of personal and program factors. Objective To explore burnout, resilience, self-reported mental health, and perceptions of the learning environment in a national sample of obstetrics and gynecology (OB/GYN) residents. Methods An observational cross-sectional survey of OB/GYN residents taking the January 2022 national in-training examination included an abbreviated 2-item Maslach Burnout Inventory, a short version of the Connor-Davidson Resilience Scale, and subjective experience of other factors. Kruskal-Wallis and chi-square tests explored differences in outcomes and associations between variables. Results Among 5761 residents taking the examination, 3741 (64.9%) participated, with 2425 of 3741 (64.8%) reporting burnout, 2138 (57.2%) depression, 2651 (70.9%) anxiety, and 147 (3.9%) suicidal ideation. Women fared worse than men in terms of burnout (2105 of 3147, 66.9% vs 281 of 496, 56.7%, P<.001), depression (1843 of 3147, 58.6% vs 256 of 496, 51.6%, P=.004), anxiety (2318 of 3147, 73.7% vs 294 of 496, 59.3%, P<.001), and resilience (5.9±2.1 vs 6.2±2.1, P=.006). More nonbinary residents considered leaving residency (17 of 49, 34.7% vs 676 of 3147, 21.5% [women] and 108 of 496, 21.8% [men], P=.008). Race-based differences were seen in depression, suicidal ideation, and thoughts of leaving residency. Increased binge drinking was reported with increasing postgraduate year. Among 614 residents reporting that well-being was not a priority in their program, 539 of 614 (87.8%) reported burnout, 469 of 614 (76.4%) depression, and 508 of 614 (82.7%) anxiety. Conclusions Residents report high rates of mental health concerns, and these are worse among women, gender nonconforming individuals, Black residents, and those who perceive well-being is not a priority in their training program.
Isolation and Identification of Avian Infectious Laryngotracheitis Virus in Sudan

Abstract:Three outbreaks of mild respiratory disease of chickens were occurred in Sudan during winter, one of which at Halfaya in Khartoum State on December 2010 and two other at Atbara on January 2011. The main respiratory signs of these outbreaks were rales, coughing with nasal and oral discharge, depression, conjunctivitis and swollen head and sinuses in some cases. The affected flocks were varied in age from 5 to 18 months, and they were from different layer breeds (Bovans, Hyline 98, Lohman and Hisex). Also, birds showing respiratory signs were obtained from different farms in Khartoum State at Shambat and Dekheinat. Using embryonated chicken eggs and chicken embryo liver (CELi) cell culture, 5 Infectious Laryngotracheitis (ILT) virus field isolates (At1, At2, Hf, Sh1 and D2) were isolated from organs such as tracheas, larynx, lungs and tracheal swabs. The isolates were identified as an ILT virus by means of agar gel immunodiffusion (AGID) test and histopathological examination of infected chorioallantoic membrane segments. This is the first report of the isolation of ILT virus in Sudan.
Burnout in Urological Education: An In-Depth Study of Residents and Fellows in the 2021 AUA Census

Abstract:Purpose: Our goal was to explore the current trends in burnout, career choice regret, and well-being needs among urology residents and fellows, with specific emphasis on identifying key factors associated with burnout. Materials and methods: The AUA Workforce Workgroup collaborated with the AUA Data Team to analyze data from the 2021 AUA Census, comprising a total of 243 residents and fellows. Key demographics, benefits and resources, career choice and debt, and burnout levels were analyzed, focusing on variables like gender, PGY (postgraduate year) level, debt burden, and personal health appointments. Results: Overall, 48% of residents and 33% of fellows met criteria for professional burnout, with a higher incidence among PGY-2 residents (70%). Depersonalization was particularly notable, with 74% of residents reporting medium to high levels. Burnout was significantly associated with difficulty attending personal health appointments (52% vs 34%) and lack of access to on-call rooms (54% vs 36%). In contrast, having children during residency was associated with lower burnout levels (30.8% vs 49.1%). Meal plans were ranked as the most desired benefit (32%), followed by ability to attend health appointments during work hours (17%) and paid family leave (16%). Educational debt over $150,000 was carried by 53% of residents and 48% of fellows. Interestingly, burnout rates showed no statistically significant difference in response rates across genders, relationship status, amount of educational debt, presence of paid maternity or paternity leave, and type of childcare arrangements. Conclusions: Burnout remains a significant issue among urology trainees, with a complex interplay of factors like lack of personal time and provision of call rooms. The alarming rates of depersonalization and exhaustion highlight the urgency of implementing targeted interventions. Enhanced support systems, improved access to health care appointments, provision of call rooms, and debt management programs are recommended to alleviate the growing problem of professional burnout in the field of urology.
Wellness Curriculum in the Pediatric Clerkship

Abstract:Psychological distress leading to burnout is an important issue during medical school. While studies have researched interventions in the pre-clerkship years, very few have targeted the clerkship years. To improve the wellness of third year medical students, the following interventions were implemented: (1) prompted students to identify two wellness goals in the areas of personal and physical well-being and (2) encouraged students to participate in meditation and chair yoga sessions during their pediatrics clerkship. Students completed pre- and post-clerkship wellness surveys. The interventions led to a small but significant improvement in the wellness of students, particularly in mental, physical, emotional, social, and spiritual well-being.Further expansion of a wellness curriculum to all clerkships during the entire third and fourth years may result in reduced burnout and sustained improvements in wellness during post graduate training and practice.

Abstract:Context: The mental health crisis in medicine cannot be explained by burnout alone. Physicians are not immune to this crisis and are known to have higher rates of suicide and depression than the general population. A high prevalence of mental health symptoms has been observed in early medical training. Objectives: This study was completed to characterize medical students' mental well-being and provide guidance for timely intervention. Methods: An annual prospective, voluntary, anonymous, cross-sectional survey of medical students was completed over a 4-year period in medical school from 2016 to 2019. The survey was created based on standardized psychiatric screening tools assessing symptoms of depression, anxiety, burnout, and sleep problems. In each of those years, 1,257 (2016), 1,254 (2017), 1,221 (2018), and 1,220 (2019) enrolled students, respectively, were invited to participate. Data on students' mental health were analyzed in relation to their year of school separately for each survey year utilizing SAS 9.4. Results: A total of 973 students in 2016, 889 students in 2017, 547 students in 2018, and 606 students in 2019 participated in the study. For depression and burnout subscales, an increase in symptom scores were observed every survey year (2016, 2017, 2018, and 2019) by the second or third year of medical school with a clinically significant effect size. Persistently high levels of anxiety were observed throughout medical school, with significant increases after the first year noted in the 2016 and 2017 surveys, but not in the 2018 or 2019 surveys. Similarly, significant changes in sleep disturbance were found in the 2016 and 2017 surveys, but not in 2018 or 2019. Conclusions: Symptoms of burnout, depression, and anxiety were observed throughout all four years of medical school, with increases starting after the first year. Early intervention is needed to support students' mental health and increase access to care and resources.
Association of Clinical Specialty With Symptoms of Burnout and Career Choice Regret Among US Resident Physicians

Abstract:Importance: Burnout among physicians is common and has been associated with medical errors and lapses in professionalism. It is unknown whether rates for symptoms of burnout among resident physicians vary by clinical specialty and if individual factors measured during medical school relate to the risk of burnout and career choice regret during residency. Objective: To explore factors associated with symptoms of burnout and career choice regret during residency. Design, setting, and participants: Prospective cohort study of 4732 US resident physicians. First-year medical students were enrolled between October 2010 and January 2011 and completed the baseline questionnaire. Participants were invited to respond to 2 questionnaires; one during year 4 of medical school (January-March 2014) and the other during the second year of residency (spring of 2016). The last follow-up was on July 31, 2016. Exposures: Clinical specialty, demographic characteristics, educational debt, US Medical Licensing Examination Step 1 score, and reported levels of anxiety, empathy, and social support during medical school. Main outcomes and measures: Prevalence during second year of residency of reported symptoms of burnout measured by 2 single-item measures (adapted from the Maslach Burnout Inventory) and an additional item that evaluated career choice regret (defined as whether, if able to revisit career choice, the resident would choose to become a physician again). Results: Among 4696 resident physicians, 3588 (76.4%) completed the questionnaire during the second year of residency (median age, 29 [interquartile range, 28.0-31.0] years in 2016; 1822 [50.9%] were women). Symptoms of burnout were reported by 1615 of 3574 resident physicians (45.2%; 95% CI, 43.6% to 46.8%). Career choice regret was reported by 502 of 3571 resident physicians (14.1%; 95% CI, 12.9% to 15.2%). In a multivariable analysis, training in urology, neurology, emergency medicine, and general surgery were associated with higher relative risks (RRs) of reported symptoms of burnout (range of RRs, 1.24 to 1.48) relative to training in internal medicine. Characteristics associated with higher risk of reported symptoms of burnout included female sex (RR, 1.17 [95% CI, 1.07 to 1.28]; risk difference [RD], 7.2% [95% CI, 3.1% to 11.3%]) and higher reported levels of anxiety during medical school (RR, 1.08 per 1-point increase [95% CI, 1.06 to 1.11]; RD, 1.8% per 1-point increase [95% CI, 1.6% to 2.0%]). A higher reported level of empathy during medical school was associated with a lower risk of reported symptoms of burnout during residency (RR, 0.99 per 1-point increase [95% CI, 0.99 to 0.99]; RD, -0.5% per 1-point increase [95% CI, -0.6% to -0.3%]). Reported symptoms of burnout (RR, 3.20 [95% CI, 2.58 to 3.82]; RD, 15.0% [95% CI, 12.8% to 17.3%]) and clinical specialty (range of RRs, 1.66 to 2.60) were both significantly associated with career choice regret. Conclusions and relevance: Among US resident physicians, symptoms of burnout and career choice regret were prevalent, but varied substantially by clinical specialty. Further research is needed to better understand these differences and to address these issues.

Abstract:Introduction Health professionals are some of the most resilient human beings in society. Every day they are exposed to disease, distress, and disability for reasons that are complex and multifaceted. The constant encounters with socio-environmental, occupational, and personal factors of patients can eventually begin to affect health professionals. Consequently, physicians are one of the largest groups of health professionals that suffer from mental health disorders, largely beginning in the first stages of medical training. During these periods, physicians learn and form the behaviors, attitudes, and habits that will shape the way that they operate in their practice and personal lives. Although these aspects are not always harmful, the way in which physicians choose to manage their health - physically, mentally, emotionally, and spiritually - can determine how optimal they perform patient care and participate in teamwork amongst their colleagues. However, due to the real and perceived barriers that physicians face, such as mental health stigmas amongst providers, workload, and confidential access to health services, many choose to forgo treatment for their mental health needs. Therefore, in this study, the question was raised regarding how likely physicians and physicians-in-training are willing to accept mental health treatment. Additional questions examined participant comfortability with mental health, knowledge of surrounding treatment programs at work and/or school, and the decision to pursue mental health treatment. We hypothesized that the majority of those who took the survey would choose answers that were neutral, largely due to avoidance of being controversial; lack of knowledge about their own or general mental health amongst their counterparts; and/or the minimal time and/or effort to complete the survey. Methods An anonymous electronic survey was created via SurveyMonkey to measure mental health perceptions among physicians and physicians-in-training. Survey was created by Medical College of Georgia's (MCG) Student Educational Enrichment Program (SEEP) Advanced Scholars in the summer of 2023. Survey questions were adapted from domestic and international surveys regarding the health and wellbeing of physicians and physicians-in-training. It analyzed if individuals within the field of medicine were supplied with adequate resources for their mental health, felt comfortable in disclosing their mental health status to colleagues or a supervisor, or felt comfortable seeking help for their mental health1-4. The survey was distributed to pre-medicine and medical students, residents, attendings and retired physicians via e-mail, social media, and text messaging. Survey responses were collected for one week before being closed for analysis. Results 171 participants (63.16% female; 36.26% male; 0.58% nonbinary/genderqueer) completed the survey. Majority of respondents identified as Black or African American (53.22%) and lived in the South Atlantic region of the USA (80.70%), with the least number of reported respondents from American Indian or Alaskan Native (0.00%) and those living in the Mountain region of the USA (0.59%). Average age of respondents was 28.5 years (SD = 39.00) with the majority ranging from 20-30 years of age (62.75%).Figures 1 and 4, representing all survey participants demonstrated that a large number of physicians and physicians-in-training acknowledged that mental health struggles were not openly discussed in the professional environment and that more than 50% of participant would not be comfortable in disclosing their own personal mental health struggles. Figures 2 and 5, representing physicians-in-training only showed similar data. Figures 3 and 7, representing physicians were similar. However, data suggests that practicing physicians were a little more comfortable in revealing personal struggles with mental illness, than physicians-in-training. Conclusions Although physicians experienced less openness in sharing struggles in the professional community compared to medical students, they were more likely to express their struggles with mental health than medical students. This may be due to a social stigma present among the professional community and an increase in coping strategies employed by physicians. As individuals progress through medical school and residency, they are more likely to adapt to stressful environments by incorporating coping mechanisms. These may include talking with friends and family, exercise, meditation, and use of prescription medication. Stereological estimation of numerical densities of glutamatergic principal neurons in the mouse hippocampus

Abstract:Recent studies have emphasized functional dissociations between dorsal and ventral hippocampus in learning, emotion, and affect. A rigorous quantitative analysis concerning lamellar cytoarchitecture would be important for promoting further research on the regional differentiation of the hippocampus. Here, we stereologically estimated the numerical densities (NDs) of glutamatergic principal neurons in the mouse hippocampus and encountered the significant differences along the dorsoventral axis. In the CA1 region, the NDs of CA1 pyramidal neurons were almost three times higher at the dorsal level (447.5 × 103/mm3) than at the ventral level (180.5 × 103/mm3); meanwhile, along the transverse axis, the NDs were significantly higher in the proximal portion than in the distal portion both at the dorsal and ventral levels. An EF‐hand calcium‐binding protein, calbindin D28K, was expressed in ∼45% of CA1 pyramidal neurons both at the dorsal and ventral level. In the CA3 region, there were no significant differences in the NDs along the dorsoventral and transverse axes (dorsal, 165.2 × 103/mm3; ventral, 172.4 × 103/mm3). In the dentate gyrus (DG), the NDs of granule cells were significantly higher at the dorsal level (916.7 × 103/mm3) than at the ventral level (788.9 × 103/mm3). The significant differences were observed only in the suprapyramidal blade, but not in the infrapyramidal blade. Then, we calculated the total neuron numbers contained in a 300‐μm‐thick hypothetical transverse slice of the hippocampus and found that the ratios of GABAergic to glutamatergic neuron numbers were two to three times higher in the ventral slice than in the dorsal slice. The ratios of numbers of eight GABAergic neuron subtypes to principal cells indicate structural dissociations in the neural network between dorsal and ventral slices. These findings provide an essential quantitative basis for elucidating mechanisms of distinct neural circuits underlying various hippocampal functions. © 2009 Wiley‐Liss, Inc.
Addressing the physician burnout epidemic with resilience curricula in medical education: a systematic review

Abstract:Abstract Background A variety of stressors throughout medical education have contributed to a burnout epidemic at both the undergraduate medical education (UGME) and postgraduate medical education (PGME) levels. In response, UGME and PGME programs have recently begun to explore resilience-based interventions. As these interventions are in their infancy, little is known about their efficacy in promoting trainee resilience. This systematic review aims to synthesize the available research evidence on the efficacy of resilience curricula in UGME and PGME. Methods We performed a comprehensive search of the literature using MEDLINE, EMBASE, PsycINFO, Educational Resources Information Centre (ERIC), and Education Source from their inception to June 2020. Studies reporting the effect of resilience curricula in UGME and PGME settings were included. A qualitative analysis of the available studies was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Risk of bias was assessed using the ROBINS-I Tool. Results Twenty-one studies met the inclusion criteria. Thirteen were single-arm studies, 6 quasi-experiments, and 2 RCTs. Thirty-eight percent (8/21; n = 598) were implemented in UGME, while 62 % (13/21, n = 778) were in PGME. There was significant heterogeneity in the duration, delivery, and curricular topics and only two studies implemented the same training model. Similarly, there was considerable variation in curricula outcome measures, with the majority reporting modest improvement in resilience, while three studies reported worsening of resilience upon completion of training. Overall assessment of risk of bias was moderate and only few curricula were previously validated by other research groups. Conclusions Findings suggest that resilience curricula may be of benefit to medical trainees. Resilience training is an emerging area of medical education that merits further investigation. Additional research is needed to construct optimal methods to foster resilience in medical education.

Abstract:The observed to expected episodes of care experience of 652 employees at a fluorochemical (perfluorooctanesulfonyl fluoride) production facility was compared with 659 film plant (nonfluorochemical) employees at the same site (Decatur, AL). Episodes of care were defined by a hierarchical analysis of health claims data from 1993 through 1998. The age- and sex-adjusted expected number of episodes of care was calculated from the company's U.S. manufacturing workforce. For a priori interests, the observed to expected episodes of care ratios were comparable for fluorochemical and film plant employees for liver tumors or diseases, bladder cancer, thyroid and lipid metabolism disorders, and reproductive, pregnancy, and perinatal disorders and higher for biliary tract disorders and cystitis recurrence. Non-a priori associations among the fluorochemical plant workers included benign colon polyps, malignant colorectal tumors, and malignant melanoma.
Prevalence and Predictors of Burnout Among Resident Family Physicians

Abstract:Background and Objectives: Resident burnout may affect career choices and empathy. We examined predictors of burnout among family medicine residents. Methods: We used data from the 2019–2021 American Board of Family Medicine Initial Certification Questionnaire, which is required of graduating residents. Burnout was a binary variable defined as reporting callousness or emotional exhaustion once a week or more. We evaluated associations using bivariate and multilevel multivariable regression analyses. Results: Among 11,570 residents, 36.4% (n=4,211) reported burnout. This prevalence did not significantly vary from 2019 to 2021 and was not significantly attributable to the residency program (ICC=0.07). Residents identifying as female reported higher rates of burnout (39.0% vs 33.4%, AOR=1.29 [95% CI 1.19–1.40]). Residents reporting Asian race (30.5%, AOR=0.78 [95% CI 0.70–0.86]) and Black race (32.3%, AOR=0.71 [95% CI 0.60–0.86]) reported lower odds of burnout than residents reporting White race (39.2%). We observed lower rates among international medical graduates (26.7% vs 40.3%, AOR=0.54 [95% CI 0.48–0.60]), those planning to provide outpatient continuity care (36.0% vs 38.7%, AOR=0.77 [95% CI 0.68–0.86]), and those at smaller programs (31.7% for <6 residents per class vs 36.3% for 6–10 per class vs 40.2% for >10 per class). Educational debt greater than $250,000 was associated with higher odds of burnout than no debt (AOR=1.29 [95% CI 1.15–1.45]). Conclusions: More than one-third of recent family medicine residents reported burnout. Odds of burnout varied significantly with resident and program characteristics.

Abstract:Physicians have equivalent rates of depression and higher rates of suicide than other professions. While death by suicide is relatively rare, it is the leading cause of death in male resident physicians and the second leading cause of death in female resident physicians. Physicians who die by suicide are less likely to receive mental health treatment than non-physicians. The authors implemented a confidential, online, interactive screening program to help residents and fellow physicians gain awareness of their mental health, with the goal of reducing physician suicide.

Abstract:Importance Few studies have explored the association between sexual and gender minority (SGM) status and occupational well-being among health care workers. Objectives To assess the prevalence of burnout, professional fulfillment, intent to leave, anxiety, and depression by self-reported SGM status. Design, Setting, and Participants This cross-sectional survey study collected data from October 2019 to July 2021, from 8 academic medical institutions participating in the Healthcare Professional Well-Being Academic Consortium. The survey, including questions on SGM status, was administered to attending physicians and trainees. Statistical analyses were performed from June 1, 2023, to February 29, 2024. Exposure SGM status was determined via self-reported sexual orientation and gender identity. Main Outcomes and Measures Primary outcomes measured were the Professional Fulfillment Index (burnout and professional fulfillment), intent to leave, and self-reported anxiety and depression using the Patient-Reported Outcomes Measurement Information System short-form 4-item measure. Results Of 20 541 attendings and 6900 trainees, 8376 attendings and 2564 trainees responded and provided SGM status. Of these respondents, 386 attendings (4.6%) and 212 trainees (8.3%) identified as SGM. Compared with their non-SGM peers, SGM attendings had a lower prevalence of professional fulfillment (133 of 386 [34.5%] vs 3200 of 7922 [40.4%]) and a higher prevalence of burnout (181 of 382 [47.4%] vs 2791 of 7883 [35.4%]) and intent to leave (125 of 376 [33.2%] vs 2433 of 7873 [30.9%]) (all P < .001). Compared with their non-SGM peers, SGM trainees had a lower prevalence of professional fulfillment (63 of 211 [29.9%] vs 833 of 2333 [35.7%]) and a higher prevalence of burnout (108 of 211 [51.2%] vs 954 of 2332 [40.9%]) (both P < .001). After adjusting for age and race and ethnicity, SGM attendings had higher odds of burnout than their non-SGM peers (adjusted odds ratio, 1.57 [95% CI, 1.27-1.94]; P < .001). Results for burnout were similar among the SGM trainees compared with their non-SGM peers (adjusted odds ratio, 1.47 [1.10-1.96]; P = .01). Conclusions and Relevance In this cross-sectional survey study of academic physicians and trainees, SGM attendings and trainees had higher levels of burnout and lower levels of professional fulfillment. SGM attendings had greater intent to leave than their non-SGM peers, but trainees did not. These disparities represent an opportunity for further exploration to retain SGM health care workers.

The Unspoken Factors of Physician Mental Health: A Qualitative Survey Amplifying Voices of Physicians and Physicians-in-Training

Abstract:Introduction Mental health includes our emotional, psychological, and social well-being, and it affects how we act, think, and feel. This state enables people to cope with the stressors of life, realize one's abilities, learn well, work well, and be a contributor to our community. When your mental health is no longer working at its optimal level these functions are disrupted. In the United States today, mental health illness affects 1 in 4 adults, which is about 26% of adults, 46% of teenagers, and 13% of children every year. Despite these statistics, about 50% of these individuals will not seek help due to the stigmas and discrimination surrounding mental health1. This stigma is heightened when it comes to struggling with these issues as professionals. Although recently education on the effects of mental health and the availability of services is being increased in professional settings, the stigma and fear remains2-3. In this study, we conducted a survey of upcoming and current physicians to determine the factors that contribute to poor mental health of these physicians and the coping mechanisms that they employ instead of receiving professional help. Methods An anonymous electronic survey was created via SurveyMonkey to measure mental health perceptions among physicians and physicians-in-training. Survey was created by the Medical College of Georgia's Student Educational Enrichment Program (SEEP) Advanced Scholars in the summer of 2023. Survey questions were adapted from domestic and international surveys regarding the health and wellbeing of physicians and physicians-in-training which analyzed if individuals within the field of medicine were supplied with adequate resources for their mental health, felt comfortable in disclosing their mental health status to colleagues or a supervisor, or felt comfortable seeking help for their mental health4-7. SEEP Advanced Scholars and Instructor distributed survey to their networks via email, individual texts and group platforms specific to pre-medical and medical students, residents, attendings and retired physicians. Survey responses were collected for one week before being closed for analyzation. Two open-ended questions: 1) factors that contribute to poor mental health and 2) utilization of coping strategies were analyzed. Factors were separated and scored based on the themes of a) Internal vs b) External Stressors. Coping strategies were scored by prevalence of use among respondents. Results 171 participants (63.16% woman; 36.26% male; 0.58% nonbinary/genderqueer) completed the survey. Majority of respondents identified as Black or African American (53.22%) and lived in the South Atlantic region of the USA (80.70%), with the least number of reported respondents from American Indian or Alaskan Native heritage (0.00%) and those living in the Mountain region of the USA (0.59%). Average age of respondents was 28.5 years (SD = 39.00) with the majority ranging from 20 to 30 years of age (62.75%).As shown in Figure 2, external factors (69.76%) caused more stress in respondents than internal factors (30.24%). Workload was the number one external stressor while lack of sleep was the number one internal stressor for respondents. As shown in Figure 4, familial and platonic relationships play a major role in coping across career stages for physicians and physicians-in-training. Healthy coping strategies, identified as not taking a toll on mental or physical health, were utilized more often than unhealthy coping strategies such as internet use, sleeping, eating, avoidance, and isolation. Conclusions Given that external factors related to workload caused the most stress for physicians and physicians-in-training, structural changes at the university and medical school level and clinical settings are necessary to mitigate the potential negative effects of stress on the body and mind. Promotion of positive coping strategies should be implemented at all career stages. Increasing normalization of conversations regarding physician mental health is recommended from the earliest stages. Medical instructors and clinical supervisors should undergo training in supporting students and employees with their mental health8. Limitations of this research included limited timeframe to participate in study and the inability to determine at what stage in a person's medical career that a person had the most struggle with mental health. Additionally, the majority of the population from this survey identified as being Black or African American, residing in the Southeast region of the United States. Thus, the results cannot be generalized to a wider range of individuals.

Abstract:INTRODUCTION: Rates of mental illness are disproportionately higher among medical residents. Neurosurgical trainees are no exception and may be more likely to suffer from depression, anxiety, and/or burnout than other residents. An assessment of the prevalence of mental illness in U.S. neurosurgical residents in the post-COVID-19 era is lacking in the literature and understanding the risk factors is vital to combating these mental health disorders. METHODS: Cross-sectional study analyzing responses to a 28-question survey from March-May 2023 distributed to U.S. neurosurgery residents. The survey included demographic and program-specific information, as well as Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7) screening questions. PHQ-9 and GAD-7 scores >5 represent at least mild depression and anxiety, respectively. RESULTS: Forty-eight responses were included in the analysis, which consisted of 35 males (74%), 36 married respondents (77%), and 27 junior residents (56%, PGY-1 to 4). Twenty-four residents (50%) screened positive for depression, 19 residents (40%) screened positive for anxiety, and 26 residents (54%) reported feeling burned out. Depressed residents were highly more likely to also suffer from anxiety (70%, p < 0.0001) and burnout (79%, p = 0.001). Further, junior residents (63%, p = 0.042), female residents (83%, p = 0.008), and residents on a neurosurgical rotation (60%, p = 0.036) were more likely to screen positive for depression. However, being married (p = 0.318), practicing in one’s home state (p = 0.724), having a 24-hour call system (0.188), having formal didactic time (p = 0.125), and having program-led wellness initiatives (p = 0.069) were not protective. CONCLUSIONS: Half of U.S. neurosurgery residents who responded screened positive for depression, 40% screened positive for anxiety, and 54% felt burned out. Depression was strongly correlated with both anxiety and burnout. Neurosurgery residents who were female, in their junior years, and on-service were at particularly high risk for depression.

Abstract:Background: Surgical residents train under immense stress, often manifesting into poor well-being. While recent research identifies methods of coping with stress, few studies empirically investigate the role of the environment on surgical resident well-being. We aimed to assess surgical resident perceptions of workplace climate, organizational support, burnout, and job satisfaction to test a mediation model identifying antecedents to well-being. Materials and methods: A convenience sampling of program directors from general surgery within the Eastern region of the United States were emailed to request either agenda time to collection data via paper survey or to forward an electronic survey link to their residents between March 2016 and June 2016. The survey included scales demonstrating validity evidence on well-being, climate, and perceptions of support. Results: Based on 160 general surgery residents (out of 557; 29% response rate) across 19 training programs, our mediation model found that job satisfaction was significantly predicted by workplace climate directly (direct effect = 0.37, 95% CI [0.19, 0.55]) and indirectly (specific indirect effect = 0.07, 95% CI [0.01, 0.13]) through perceived organizational support and burnout, while controlling for training year and gender, F(5,147) = 53.76, P < 0.001, Rsq. = 0.65. Conclusions: Medical education requires an additional focus on how the clinical learning environment affects surgical resident well-being. Health systems and training programs will need to collaborate on workplace innovations to improve workplace climate for trainees to address the concerns of well-being with a modern surgical workforce.
Social Support, Social Isolation, and Burnout: Cross-Sectional Study of U.S. Residents Exploring Associations With Individual, Interpersonal, Program, and Work-Related Factors

Abstract:Purpose: To examine associations of social support and social isolation with burnout, program satisfaction, and organization satisfaction among a large population of U.S. residents and fellows and to identify correlates of social support and social isolation. Method: All residents and fellows enrolled in graduate medical education programs at Mayo Clinic sites were surveyed in February 2019. Survey items measured social support (emotional and tangible), social isolation, burnout, program satisfaction, and organization satisfaction. Factors of potential relevance to social support were collected (via the survey, institutional administrative records, and interviews with program coordinators and/or program directors) and categorized as individual, interpersonal, program, or work-related factors (duty hours, call burden, elective time, vacation days used before survey administration, required away rotations, etc.). Multivariable regression analyses were conducted to examine relationships between variables. Results: Of 1,146 residents surveyed, 762 (66%) from 58 programs responded. In adjusted models, higher emotional and tangible support were associated with lower odds of burnout and higher odds of program and organization satisfaction, while higher social isolation scores were associated with higher odds of burnout and lower odds of program satisfaction and organization satisfaction. Independent predictors of social support and/or social isolation included age, gender, relationship status, parental status, postgraduate year, site, ratings of the program leadership team, ratings of faculty relationships and faculty professional behaviors, satisfaction with autonomy, and vacation days used before survey administration. Conclusions: This study demonstrates that social support and social isolation are strongly related to burnout and satisfaction among residents and fellows. Personal and professional relationships, satisfaction with autonomy, and vacation days are independently associated with social support and/or social isolation, whereas most program and work-related factors are not. Additional studies are needed to determine if social support interventions targeting these factors can improve well-being and enhance satisfaction with training.

Abstract:Background: The Competence Centre for Residency Training in Family Medicine Bavaria (CCRTB) was established to improve the quality of postgraduate medical training by offering additional seminars and mentoring programmes as well as regular 'train-the-trainer' courses for educating physicians. In addition, residents have the opportunity to participate in a regional training network. Objective: The aim was to assess the burden of burnout and the importance of the learning environment in the clinical training phase. Methods: We conducted a cross-sectional study. Burnout was assessed using the Maslach Burnout Inventory (MBI), which comprises the scales "Emotional Exhaustion", "Depersonalisation" and "Personal Accomplishment". The quality of the learning environment was recorded using the German version of the Dutch Residency Educational Climate Test (D-RECT German). In addition, multivariable linear regressions were performed to estimate the impact of learning environment, year of training and participation in a regional network on the level of burnout. Results: 129 clinical residents enrolled in the CCRTB were invited to participate in the study, 78 (61%) of whom submitted a response. 76 (59%) of these residents were included in the analyses. The present study discloses an increased burden of burnout among residents in the clinical training phase, with approx. 40% reaching a critical burnout score. A higher quality of the learning environment was associated with significantly milder burnout symptoms on the majority of the D-RECT scales. Conclusion: Family medicine residents in the clinical training phase show a high burden of burnout. Therefore, increasing the quality of the learning environment appears to be an effective key element in achieving a reduction of burnout in clinical training. This might contribute to an increase in professional satisfaction, which finally may also prevent migration from the medical profession.
