Abstract:Purpose: The aim of this study was to develop a deep convolutional neural network (DCNN) for detecting early osteonecrosis of the femoral head (ONFH) from various hip pathologies and evaluate the feasibility of its application. Methods: We retrospectively reviewed and annotated hip magnetic resonance imaging (MRI) of ONFH patients from four participated institutions and constructed a multi-centre dataset to develop the DCNN system. The diagnostic performance of the DCNN in the internal and external test datasets was calculated, including area under the receiver operating characteristic curve (AUROC), accuracy, precision, recall, and F1 score, and gradient-weighted class activation mapping (Grad-CAM) technique was used to visualize its decision-making process. In addition, a human-machine comparison trial was performed. Results: Overall, 11,730 hip MRI segments from 794 participants were used to develop and optimize the DCNN system. The AUROC, accuracy, and precision of the DCNN in internal test dataset were 0.97 (95% CI, 0.93-1.00), 96.6% (95% CI: 93.0-100%), and 97.6% (95% CI: 94.6-100%), and in external test dataset, they were 0.95 (95% CI, 0.91- 0.99), 95.2% (95% CI, 91.1-99.4%), and 95.7% (95% CI, 91.7-99.7%). Compared with attending orthopaedic surgeons, the DCNN showed superior diagnostic performance. The Grad-CAM demonstrated that the DCNN placed focus on the necrotic region. Conclusion: Compared with clinician-led diagnoses, the developed DCNN system is more accurate in diagnosing early ONFH, avoiding empirical dependence and inter-reader variability. Our findings support the integration of deep learning systems into real clinical settings to assist orthopaedic surgeons in diagnosing early ONFH.

Abstract:Therapeutic Advances in Musculoskeletal Disease, Volume 16, Issue , January-December 2024. Background:Despite being the gold standard for diagnosing osteoporosis, dual-energy X-ray absorptiometry (DXA) is an underutilized screening tool for osteoporosis.Objectives:This study proposed and validated a controllable feature layer of a convolutional neural network (CNN) model with a preprocessing image algorithm to classify osteoporosis and predict T-score on the proximal hip region via simple hip radiographs.Design:This was a single-center, retrospective study.Methods:An image dataset of 3460 unilateral hip images from 1730 patients (age ⩾50 years) was retrospectively collected with matched DXA assessment for T-score for the targeted proximal hip regions to train (2473 unilateral hip images from 1430 patients) and test (497 unilateral hip images from 300 patients) the proposed CNN model. All images were processed with a fully automated CNN model, X1AI-Osteo.Results:The proposed screening tool illustrated a better performance (sensitivity: 97.2%; specificity: 95.6%; positive predictive value: 95.7%; negative predictive value: 97.1%; area under the curve: 0.96) than the open-sourced CNN models in predicting osteoporosis. Moreover, when combining variables, including age, body mass index, and sex as features in the training metric, there was high consistency in the T-score on the targeted hip regions between the proposed CNN model and the DXA (r = 0.996, p < 0.001).Conclusion:The proposed CNN model may identify osteoporosis and predict T-scores on the targeted hip regions from simple hip radiographs with high accuracy, highlighting the future application for population-based opportunistic osteoporosis screening with low cost and high adaptability for a broader population at risk.Trial registration:TMU-JIRB N201909036.

Abstract:Knee osteoarthritis (OA) is a chronic disorder mainly arising from age-related factors affecting the knee joints. Its diagnosis is critically important and is usually done by medical practitioners using X-ray images. Although this process is accurate, it is time-consuming. X-ray images have facilitated the use of deep learning (DL) models for the automation of the diagnosis of knee OA, commonly employing convolutional neural network (CNN) based architectures. However, the lack of models' interpretability makes the results less trustworthy. This work builds on the existing state-of-the-art (SOTA) pre-trained DL models to understand the model's behavior in classifying highly complex knee OA cases utilizing a divide-and-conquer approach - from multi-class to a binary class for better results interpretability and explainability using explainable artificial intelligence (XAI). Five SOTA fine-tuned DL models are tested on Kellgren-Lawrence (KL) graded X-ray images. Both multi-class and binary-class (using the multiple subsets derived from the original dataset to examine how the models perform with different data combinations) classification approaches and their interpretability of findings using Gradient-weighted Class Activation Mapping (GradCAM) are undertaken in this study. The GradCAM visualization of EfficientNetb7 demonstrates that when the degree of variance between different classes increases, the model's efficiency in classifying knee OA also increases. Specifically, it becomes more effective at distinguishing normal and severe cases with 99.13% classification accuracy. However, the model's efficacy drops to 67% for other cases, indicating that it cannot classify knee OA as effectively as doctors.

Abstract:Knee osteoarthritis (OA) is the most common musculoskeletal disorder. OA diagnosis is currently conducted by assessing symptoms and evaluating plain radiographs, but this process suffers from subjectivity. In this study, we present a new transparent computer-aided diagnosis method based on the Deep Siamese Convolutional Neural Network to automatically score knee OA severity according to the Kellgren-Lawrence grading scale. We trained our method using the data solely from the Multicenter Osteoarthritis Study and validated it on randomly selected 3,000 subjects (5,960 knees) from Osteoarthritis Initiative dataset. Our method yielded a quadratic Kappa coefficient of 0.83 and average multiclass accuracy of 66.71% compared to the annotations given by a committee of clinical experts. Here, we also report a radiological OA diagnosis area under the ROC curve of 0.93. Besides this, we present attention maps highlighting the radiological features affecting the network decision. Such information makes the decision process transparent for the practitioner, which builds better trust toward automatic methods. We believe that our model is useful for clinical decision making and for OA research; therefore, we openly release our training codes and the data set created in this study.

Abstract:Objective: The purpose of this study was to investigate the diagnostic performance of a neural network Artificial Intelligence model for the radiographic confirmation of Temporomandibular Joint Osteoarthritis in reference to an experienced radiologist. Materials and methods: The diagnostic performance of an AI model in identifying radiographic features in patients with TMJ-OA was evaluated in a diagnostic accuracy cohort study. Adult patients elected for radiographic examination by the Diagnostic Criteria for Temporomandibular Disorders decision tree were included. Cone-beam computed Tomography images were evaluated by object detection YOLO deep learning model. The diagnostic performance was verified against examiner radiographic evaluation. Results: The differences between the AI model and examiner were non-significant statistically, except in the subcortical cyst (P=0.049*). AI model showed substantial to near-perfect levels of agreement when compared to those of the examiner data. Regarding each radiographic phenotype, the AI model reported favorable sensitivity, specificity, accuracy, and highly statistically significant Receiver Operating Characteristic (ROC) analysis (p<0.001). Area Under Curve ranged from 0.872, for surface erosion, to 0.911 for subcortical cyst. Conclusion: AI object detection model could open the horizon for a valid, automated, and convenient modality for TMJ-OA radiographic confirmation and radiomic features identification with a significant diagnostic power.

Abstract:Abstract: Osteoarthritis (OA) of the knee is a common cause of activity restriction and physical impairment in elderly people. Early identification and treatment can help delay the progression of OA. Physicians' visual examination rating is objective, varies across interpretation, and is heavily reliant on their expertise. We use two machine learning approaches (CNN) in this article to automatically estimate the severity of knee OA as described by the Kallgren- Lawrence (KL) grading system. To begin, we use a customized one-stage YOLOv2 network to recognize kneecap based on the size of knee joints scattered in X-ray pictures with poor contrast. Second, we use a new customizable arbitrary loss to fine-tune its most famous Cnn architectures, spanning ResNet, VGG, and DenseNet versions, as well as InceptionV3, to categorize the collected knee joint pictures. To be more explicit, we provide a stronger penalty to misrepresentation with a greater difference between the predicted and actual KL grade, driven by the ordinal character of the knee KL grading assignment. The Osteoarthritis Institute (OAI) collection is used to evaluate the basic X-ray pictures. Under the Jaccard index criterion of 0.75, we acquire a mean Jaccard index of 0.858 and a recall of 92.2 percent for knee joint identification. The fine-tuned VGG-19 model with the provided linear loss achieves the greatest generalization ability of 96.7 percent and mean standard deviation (MAE) of 0.344 on the knee KL grading task. Abstract:PurposeWe developed a tool for locating and grading knee osteoarthritis (OA) from digital X-ray images and illustrate the possibility of deep learning techniques to predict knee OA as per the Kellgren-Lawrence (KL) grading system. The purpose of the project is to see how effectively an artificial intelligence (AI)-based deep learning approach can locate and diagnose the severity of knee OA in digital X-ray images.MethodsSelection criteria: Patients above 50 years old with OA symptoms (knee joint pain, stiffness, crepitus, and functional limitations) were included in the study. Medical experts excluded patients with post-surgical evaluation, trauma, and infection from the study. We used 3172 Anterior–posterior view knee joint digital X-ray images. We have trained the Faster RCNN architecture to locate the knee joint space width (JSW) region in digital X-ray images and we incorporate ResNet-50 with transfer learning to extract the features. We have used another pre-trained network (AlexNet with transfer learning) for the classification of knee OA severity. We trained the region proposal network (RPN) using manual extract knee area as the ground truth image and the medical experts graded the knee joint digital X-ray images based on the Kellgren-Lawrence score. An X-ray image is an input for the final model, and the output is a Kellgren-Lawrence grading value.ResultsThe proposed model identified the minimal knee JSW area with a maximum accuracy of 98.516%, and the overall knee OA severity classification accuracy was 98.90%.ConclusionsToday numerous diagnostic methods are available, but tools are not transparent and automated analysis of OA remains a problem. The performance of the proposed model increases while fine-tuning the network and it is higher than the existing works. We will extend this work to grade OA in MRI data in the future.

Abstract:Plain radiography is widely used to detect mechanical loosening of total hip replacement (THR) implants. Currently, radiographs are assessed manually by medical professionals, which may be prone to poor inter and intra observer reliability and low accuracy. Furthermore, manual detection of mechanical loosening of THR implants requires experienced clinicians who might not always be readily available, potentially resulting in delayed diagnosis. In this study, we present a novel, fully automatic and interpretable approach to detect mechanical loosening of THR implants from plain radiographs using deep convolutional neural network (CNN). We trained a CNN on 40 patients anteroposterior hip x rays using five fold cross validation and compared its performance with a high volume board certified orthopaedic surgeon (AFC). To increase the confidence in the machine outcome, we also implemented saliency maps to visualize where the CNN looked at to make a diagnosis. CNN outperformed the orthopaedic surgeon in diagnosing mechanical loosening of THR implants achieving significantly higher sensitively (0.94) than the orthopaedic surgeon (0.53) with the same specificity (0.96). The saliency maps showed that the CNN looked at clinically relevant features to make a diagnosis. Such CNNs can be used for automatic radiologic assessment of mechanical loosening of THR implants to supplement the practitioners decision making process, increasing their diagnostic accuracy, and freeing them to engage in more patient centric care.

Furthermore, we present attention maps of CNN to highlight the radiological features affecting the network decision. Such information makes the decision process transparent for practitioners, which builds better trust towards such automatic methods and, moreover, reduces the workload of clinicians, especially for remote areas without enough medical staff.

Abstract:This is particularly true for the senior population, whose quality of life has been drastically reduced as a result of the increasing incidence of several health problems. Over 27 million people in the United States suffer from osteoarthritis of the knee (OAK), a painful condition that may severely limit mobility. When the articular cartilage between the tibia and femur in the knee is damaged, osteoarthritis of the knee develops. OAK symptoms include a loss of mobility and the inability to walk normally due to knee pain, detected using an X-ray. We detail here a novel method of evaluating the severity of knee osteoarthritis (OA) by X-ray analysis. Modern methods are comprised of pre-processing, feature extraction using a convolutional neural network (CNN), and classification with latent semantic modeling (LSM) (LSTM). Data from the osteoarthritis initiatives (OAI) database, which is available to the public, was utilized to evaluate the methodology proposed. The current method has been shown to be effective, and the OAI database has information on KL grade assessment for both knees. OAK is the subject of state-of-the-art, global observational investigation by experts using a program called OAI. This collection was created to serve as a one-stop shop for researchers seeking the scholarly materials they need to systematically examine OA indicators as a possible endpoint for the advanced stages of the illness. The statistics reveal a mean accuracy of 100%. When compared to earlier deep learning approaches, these outcomes are much superior.

Abstract:Knee osteoarthritis (OA) is one major cause of activity limitation and physical disability in older adults. Early detection and intervention can help slow down the OA degeneration. Physicians' grading based on visual inspection is subjective, varied across interpreters, and highly relied on their experience. In this paper, we successively apply two deep convolutional neural networks (CNN) to automatically measure the knee OA severity, as assessed by the Kellgren-Lawrence (KL) grading system. Firstly, considering the size of knee joints distributed in X-ray images with small variability, we detect knee joints using a customized one-stage YOLOv2 network. Secondly, we fine-tune the most popular CNN models, including variants of ResNet, VGG, and DenseNet as well as InceptionV3, to classify the detected knee joint images with a novel adjustable ordinal loss. To be specific, motivated by the ordinal nature of the knee KL grading task, we assign higher penalty to misclassification with larger distance between the predicted KL grade and the real KL grade. The baseline X-ray images from the Osteoarthritis Initiative (OAI) dataset are used for evaluation. On the knee joint detection, we achieve mean Jaccard index of 0.858 and recall of 92.2% under the Jaccard index threshold of 0.75. On the knee KL grading task, the fine-tuned VGG-19 model with the proposed ordinal loss obtains the best classification accuracy of 69.7% and mean absolute error (MAE) of 0.344. Abstract:Background: This study focused on using deep learning neural networks to classify the severity of osteoarthritis in the knee. A continuous regression score of osteoarthritis severity has yet to be explored using artificial intelligence machine learning, which could offer a more nuanced assessment of osteoarthritis. Materials and Methods: This study used 8260 radiographic images from The Osteoarthritis Initiative to develop and assess four neural network models (VGG16, EfficientNetV2 small, ResNet34, and DenseNet196). Each model generated a regressor score of the osteoarthritis severity based on Kellgren-Lawrence grading scale criteria. Primary performance outcomes assessed were area under the curve (AUC), accuracy, and mean absolute error (MAE) for each model. Secondary outcomes evaluated were precision, recall, and F-1 score. Results: The EfficientNet model architecture yielded the strongest AUC (0.83), accuracy (71%), and MAE (0.42) compared with VGG16 (AUC: 0.74; accuracy: 57%; MAE: 0.54), ResNet34 (AUC: 0.76; accuracy: 60%; MAE: 0.53), and DenseNet196 (AUC: 0.78; accuracy: 62%; MAE: 0.49). Conclusion: Convolutional neural networks offer an automated and accurate way to quickly assess and diagnose knee radiographs for osteoarthritis. The regression score models evaluated in this study demonstrated superior AUC, accuracy, and MAE compared with standard convolutional neural network models. The EfficientNet model exhibited the best overall performance, including the highest AUC (0.83) noted in the literature. The artificial intelligence-generated regressor exhibits a finer progression of knee osteoarthritis by quantifying severity of various hallmark features. Abstract:Recently, many diseases have negatively impacted people's lifestyles. Among these, knee osteoarthritis (OA) has been regarded as the primary cause of activity restriction and impairment, particularly in older people. Therefore, quick, accurate, and low-cost computer-based tools for the early prediction of knee OA patients are urgently needed. In this paper, as part of addressing this issue, we developed a new method to efficiently diagnose and classify knee osteoarthritis severity based on the X-ray images to classify knee OA in (i.e., binary and multiclass) in order to study the impact of different class-based, which has not yet been addressed in previous studies. This will provide physicians with a variety of deployment options in the future. Our proposed models are basically divided into two frameworks based on applying pre-trained convolutional neural networks (CNN) for feature extraction as well as fine-tuning the pre-trained CNN using the transfer learning (TL) method. In addition, a traditional machine learning (ML) classifier is used to exploit the enriched feature space to achieve better knee OA classification performance. In the first one, we developed five classes-based models using a proposed pre-trained CNN for feature extraction, principal component analysis (PCA) for dimensionality reduction, and support vector machine (SVM) for classification. While in the second framework, a few changes were made to the steps in the first framework, the concept of TL was used to fine-tune the proposed pre-trained CNN from the first framework to fit the two classes, three classes, and four classes-based models. The proposed models are evaluated on X-ray data, and their performance is compared with the existing state-of-the-art models. It is observed through conducted experimental analysis to demonstrate the efficacy of the proposed approach in improving the classification accuracy in both multiclass and binary class-based in the OA case study. Nonetheless, the empirical results revealed that the fewer multiclass labels used, the better performance achieved, with the binary class labels outperforming all, which reached a 90.8% accuracy rate. Furthermore, the proposed models demonstrated their contribution to early classification in the first stage of the disease to help reduce its progression and improve people's quality of life.

Automatic Grading of Individual Knee Osteoarthritis Features in Plain Radiographs Using Deep Convolutional Neural Networks

Abstract:Knee osteoarthritis (OA) is the most common musculoskeletal disease in the world. In primary healthcare, knee OA is diagnosed using clinical examination and radiographic assessment. Osteoarthritis Research Society International (OARSI) atlas of OA radiographic features allows performing independent assessment of knee osteophytes, joint space narrowing and other knee features. This provides a fine-grained OA severity assessment of the knee, compared to the gold standard and most commonly used Kellgren–Lawrence (KL) composite score. In this study, we developed an automatic method to predict KL and OARSI grades from knee radiographs. Our method is based on Deep Learning and leverages an ensemble of residual networks with 50 layers. We used transfer learning from ImageNet with a fine-tuning on the Osteoarthritis Initiative (OAI) dataset. An independent testing of our model was performed on the Multicenter Osteoarthritis Study (MOST) dataset. Our method yielded Cohen’s kappa coefficients of 0.82 for KL-grade and 0.79, 0.84, 0.94, 0.83, 0.84 and 0.90 for femoral osteophytes, tibial osteophytes and joint space narrowing for lateral and medial compartments, respectively. Furthermore, our method yielded area under the ROC curve of 0.98 and average precision of 0.98 for detecting the presence of radiographic OA, which is better than the current state-of-the-art.

This study aimed to perform the classification of ONFH through a deep learning approach.METHODS: We retrospectively sampled 1,806 midcoronal magnetic resonance images (MRIs) of 1,337 hips from 4 institutions. Of these, 1,472 midcoronal MRIs of 1,155 hips were divided into training, validation, and test datasets with a ratio of 7:1:2 to develop a convolutional neural network model (CNN). An additional 334 midcoronal MRIs of 182 hips were used to perform external validation. The predictive performance of the CNN and the review panel was also compared.RESULTS: A multiclass CNN model was successfully developed. In internal validation, the overall accuracy of the CNN for predicting the severity of ONFH based on the Japanese Investigation Committee classification was 87.8%. The macroaverage values of area under the curve (AUC), precision, recall, and F-value were 0.90, 84.8, 84.8, and 84.6%, respectively. In external validation, the overall accuracy of the CNN was 83.8%. The macroaverage values of area under the curve, precision, recall, and F-value were 0.87, 79.5, 80.5, and 79.9%, respectively. In a human-machine comparison study, the CNN outperformed or was comparable to that of the deputy chief orthopaedic surgeons.CONCLUSION: The CNN is feasible and robust for classifying ONFH and correctly locating the necrotic area. These findings suggest that classifying ONFH using deep learning with high accuracy and generalizability may aid in predicting femoral head collapse and clinical decision-making.

Abstract:Osteoporosis, a silent yet debilitating disease, presents a significant challenge due to its asymptomatic nature until fractures occur. Rapid bone loss outpaces regeneration, leading to pain, disability, and loss of independence. Early detection is pivotal for effective management and fracture risk reduction, yet current diagnostic methods are time-consuming. Despite its importance, research addressing early diagnosis remains limited. Deep learning, particularly convolutional neural networks (CNNs), has emerged as a potent tool in image analysis. This paper presents a novel approach utilizing transfer learning with CNNs for osteoporosis detection from X-ray images. The proposed approach not only achieves a high accuracy of osteoporosis diagnosis but also offers a revealed feature map that can guide medical professionals for osteoporosis diagnosis. The innovation lies in a dual strategy: (i) a model integrating transfer learning from CNN architectures such as AlexNet, VGG-16, ResNet-50, VGG-19, InceptionNet, XceptionNet, and a custom CNN, and (ii) a dataset collection augmentation mechanism to enhance learning accuracy. The study includes binary and multiclass classification of knee joint X-ray images into normal, osteopenia, and osteoporosis groups, utilizing a dataset of 1947 knee X-rays for training and testing. Performance comparisons against state-of-the-art models reveal the proposed VGG-19 model achieves the highest accuracy at 92.0% for multiclass and 97.5% for binary. These findings underscore the potential of deep learning with transfer learning in aiding early osteoporosis detection, thereby mitigating fracture risks.

Abstract:Objectives: Rheumatoid arthritis (RA) is a severe and common autoimmune disease. Conventional diagnostic methods are often subjective, error-prone, and repetitive works. There is an urgent need for a method to detect RA accurately. Therefore, this study aims to develop an automatic diagnostic system based on deep learning for recognizing and staging RA from radiographs to assist physicians in diagnosing RA quickly and accurately. Methods: We develop a CNN-based fully automated RA diagnostic model, exploring five popular CNN architectures on two clinical applications. The model is trained on a radiograph dataset containing 240 hand radiographs, of which 39 are normal and 201 are RA with five stages. For evaluation, we use 104 hand radiographs, of which 13 are normal and 91 RA with five stages. Results: The CNN model achieves good performance in RA diagnosis based on hand radiographs. For the RA recognition, all models achieve an AUC above 90% with a sensitivity over 98%. In particular, the AUC of the GoogLeNet-based model is 97.80%, and the sensitivity is 100.0%. For the RA staging, all models achieve over 77% AUC with a sensitivity over 80%. Specifically, the VGG16-based model achieves 83.36% AUC with 92.67% sensitivity. Conclusion: The presented GoogLeNet-based model and VGG16-based model have the best AUC and sensitivity for RA recognition and staging, respectively. The experimental results demonstrate the feasibility and applicability of CNN in radiograph-based RA diagnosis. Therefore, this model has important clinical significance, especially for resource-limited areas and inexperienced physicians.

Abstract:Knee osteoarthritis (OA) is a deliberating joint disorder characterized by cartilage loss that can be captured by imaging modalities and translated into imaging features. Observing imaging features is a well-known objective assessment for knee OA disorder. However, the variety of imaging features is rarely discussed. This study reviews knee OA imaging features with respect to different imaging modalities for traditional OA diagnosis and updates recent image-based machine learning approaches for knee OA diagnosis and prognosis. Although most studies recognized X-ray as standard imaging option for knee OA diagnosis, the imaging features are limited to bony changes and less sensitive to short-term OA changes. Researchers have recommended the usage of MRI to study the hidden OA-related radiomic features in soft tissues and bony structures. Furthermore, ultrasound imaging features should be explored to make it more feasible for point-of-care diagnosis. Traditional knee OA diagnosis mainly relies on manual interpretation of medical images based on the Kellgren-Lawrence (KL) grading scheme, but this approach is consistently prone to human resource and time constraints and less effective for OA prevention. Recent studies revealed the capability of machine learning approaches in automating knee OA diagnosis and prognosis, through three major tasks: knee joint localization (detection and segmentation), classification of OA severity, and prediction of disease progression. AI-aided diagnostic models improved the quality of knee OA diagnosis significantly in terms of time taken, reproducibility, and accuracy. Prognostic ability was demonstrated by several prediction models in terms of estimating possible OA onset, OA deterioration, progressive pain, progressive structural change, progressive structural change with pain, and time to total knee replacement (TKR) incidence. Despite research gaps, machine learning techniques still manifest huge potential to work on demanding tasks such as early knee OA detection and estimation of future disease events, as well as fundamental tasks such as discovering the new imaging features and establishment of novel OA status measure. Continuous machine learning model enhancement may favour the discovery of new OA treatment in future.
