Abstract:Objective: To summarize the experience with surgical treatment of coronary artery disease with severe ischemic mitral valve regurgitation (IMR). Methods: From January 2006 to December 2009, 45 patients (35 males, 10 females aged 32-74 years) with the diagnosis of coronary artery disease complicated by IMR underwent coronary artery bypass grafting (CABG) combined with mitral valve plasty (MVP, 24 cases) or mitral valve replacement (MVR, 21 cases). Results: Perioperative deaths occurred in 2 cases due to multiple organ failure (MOF). Echocardiography showed a significant reduction of the mitral regurgitation area (from 11.80∓2.45 cm(2) to 2.83∓0.98 cm(2), t=22.80, P=0.00) after CABG combined with mitral valve surgery, with also significantly reduced postoperative left ventricular end diastolic diameter (LVEDD) (from 57.61∓10.06 mm to 51.84∓8.98 mm, t=2.85, P=0.005). No significant difference was detected in the left ventricular ejection fraction after the operation [(52.7∓15.4)% vs (53.2∓13.2)%, t=0.16, P=0.87)]. Conclusions: CABG combined with mitral valve surgery can improve early postoperative left ventricular function in patients with ischemic coronary heart disease complicated by severe mitral regurgitation, but further follow-up study is still needed for evaluation of the long-term results.
How high are carrier frequencies of rare recessive syndromes? Contemporary estimates for Fanconi Anemia in the United States and Israel

Abstract:For many recessive genetic syndromes, carrier frequencies have been assessed through screening studies in founder populations but remain unclear in heterogeneous populations. One such syndrome is Fanconi Anemia (FA). FA is a model disease in cancer research, yet there are no contemporary data on carrier frequency or prevalence in the general United States (US) population or elsewhere. We inferred carrier frequency from birth incidence using the Hardy–Weinberg law. We estimated prevalence using birth incidence and survival data. We defined “plausible ranges” to incorporate uncertainty about completeness of case ascertainment. We made estimates for the US and Israel using demographic data from the Fanconi Anemia Research Fund and Israeli Fanconi Anemia Registry. In the US, a plausible range for the carrier frequency is 1:156–1:209 [midpoint 1:181]; we estimate that 550–975 persons were living with FA in 2010. For Israel, a plausible range for the carrier frequency is 1:66–1:128 [midpoint 1:93] in line with founder screening studies; we estimate that 40–135 Israelis were living with FA in 2008. The estimated US FA carrier frequency of 1:181 is significantly higher than the historical estimate of 1:300; hence, the gap may be narrower than previously recognized between the US carrier frequency and higher carrier frequencies of around 1:100 in several founder groups including Ashkenazi Jews. Assessment of cancer risks in heterozygous carriers merits further study. Clinical trials in FA will require co‐ordination and innovative design because the number of living US patients is probably less than 1,000. Published 2011 Wiley‐Liss, Inc.
Analysis of Early Effect of Surgical Treatment in Patients with Coronary Heart Disease Complicated by Moderate-to-severe Ischemic Mitral Regurgitation

Abstract:Objective To probe into the early effect of surgical treatment in coronary atherosclerotic heart disease (CAD) patients with moderate-to-severe ischemic mitral regurgitation (IMR). Methods A total of 175 CAD patients with moderateto- severe IMR undergoing coronary artery bypass grafting (CABG) from December 1999 to December 2010 were analyzed retrospectively. Based on the method applied during surgery, 108 patients were assigned to CABG group, undergoing CABG alone, and 67 to CABG+MVP/R group, undergoing CABG plus concomitant mitral valvuloplasty/repair (MVP/R). Differences between groups were compared in age, gender, numbers of the conduits, application of intra-aortic balloon pump (IABP) and ventricular assist device (VAD), perioperative mortality, and other concomitant procedures (i.e. aortic valve replacement, resection of the heart wall aneurysm, repair of the septal defect repair). Differences within the groups were looked for in terms of left atrial diameter (LA), left ventricular ending diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), mitral regurgitation degree, systolic pulmonary artery pressure (sPAP), and cardiac function (NYHA classification), pre-operatively and post-operatively. Results The differences between the groups showed no statistical significance in terms of age, gender, numbers of conduits, application of IABP and VAD, peri-operative mortality, and concomitant procedures (P > 0.05). LA, LVEDD, mitral regurgitation, sPAP, and cardiac function (NYHA classification) after operation were improved obviously in both groups compared with that before operation (P 0.05). Conclusions The comprehensive and aggressive surgery did not result in more ideal early effect to CAD patients with moderate-to-severe IMR. Based on the total revascularization, we can choose simpler, safer, and more effective surgical methods.

Abstract:The benefits of concomitant mitral valve procedure (MVP) for treating moderate chronic ischemic mitral regurgitation (IMR) during coronary artery bypass grafting (CABG) have not been clearly established. This study aimed to determine the incidence of moderate or more residual mitral regurgitation (MR) following CABG plus MVP for moderate chronic IMR, and to evaluate the impacts of concomitant MVP vs. CABG alone on clinical outcomes based on propensity-matched data.All eligible patients were entered into either the MVP group (CABG plus MVP, n = 184) or CABG group (CABG alone, n = 162). Moderate or more residual MR rate was investigated, and in-hospital and follow-up outcomes between the groups were compared after matching.Moderate or more residual MR rate was 11.4% at 1 year and 22.9% at 2 years after CABG plus MVP, respectively. Patients in the MVP group as compared with the CABG group had significantly lower moderate or more residual MR rates at various postoperative time points (all P < 0.001). Grouping was not an independent risk factor for in-hospital adverse events in multivariate logistic regression analysis. Also, grouping was a significant variable related to moderate or more residual MR rate and NYHA class III-IV at the latest follow-up in Cox regression analysis (HR = 0.391, 95% CI 0.114-0.628; HR = 0.419, 95% CI 0.233-0.819, respectively).Concomitant MVP as compared with CABG alone for treating moderate chronic IMR was associated with a reduction in moderate or more residual MR rate and an improvement in NYHA functional status, with no increase in in-hospital adverse events or follow-up death.
Mitral valve repair versus replacement in patients with ischemic mitral regurgitation

Abstract:Background: The optimal management of ischemic mitral regurgitation (IMR) is controversial. The aim of this study was to examine our eight years' experience of surgical treatment in patients with IMR, and to compare outcomes of mitral valve repair versus replacement with concomitant coronary artery bypass grafting (CABG). Methods: A retrospective, observational, cohort study was undertaken to collect data on consecutive patients with IMR and coronary artery disease who received CABG and mitral valve surgery in our hospital between January 2008 and December 2015. Basic patient characteristics, operative data, and postoperative clinical outcomes were examined. Results: The series included 22 consecutive patients (21 male; 1 female). The mean age was 62.1±11.4 years old. The mean preoperative left ventricular ejection fraction (LVEF) was 33.4%±15.4%. The mean cardiopulmonary bypass (CPB) time was 165.4±38.4 minutes, and the mean aortic cross clamp time was 113.8±33.6 minutes. Eighteen patients underwent CABG plus mitral valve repair, and four patients underwent CABG plus mitral valve replacement (MVR). There were three early in-hospital mortalities: two in the mitral valve repair group, and one in the replacement group. The follow-up was complete in all patients, with a mean follow-up duration of 3.1±2.3 years. The mean last LVEF was 35.3%±17.7%. There were 2 late mortalities. Both were from the repair group. The overall late survival rate was 81.6%, with 83.0% in the repair group and 75.0% in the replacement group. In patients with echocardiography follow-up of more than or equal to 1 year duration, the residual or recurrent mitral regurgitation rates were 0.0% in the replacement group and 57.1% in the repair group. One patient in the repair group later underwent MVR due to severe regurgitation postoperatively. Conclusions: Our preliminary findings showed that the surgical outcome of mitral valve repair might be comparable to that of MVR in terms of early mortality and long-term survival. However, mitral valve repair was associated with a higher residual or recurrent mitral regurgitation rate. According to the latest literature, the role of MVR can justifiably be indicated for severe IMR. As for moderate IMR, CABG alone without mitral valve intervention may provide similar clinical outcomes.

Abstract:Background: The efficacy of mitral valve repair (MVR) in combination with coronary artery bypass grafting (CABG) for moderate ischaemic mitral regurgitation (IMR) remains unclear. To evaluate whether MVR + CABG is superior to CABG alone, the authors conducted a systematic review and meta-analysis of existing randomized controlled trials (RCTs). Methods: The authors searched PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials for eligible RCTs from the date of their inception to October 2023. The primary outcomes were operative (in-hospital or within 30 days) and long-term (≥ 1 year) mortality. The secondary outcomes were postoperative stroke, worsening renal function (WRF), and reoperation for bleeding or tamponade. The authors performed random-effects meta-analyses and reported the results as risk ratios (RRs) with 95% CIs. Results: Six RCTs were eligible for inclusion. Compared with CABG alone, MVR + CABG did not increase the risk of operative mortality (RR, 1.244; 95% CI, 0.514–3.014); however, it was also not associated with a lower risk of long-term mortality (RR, 0.676; 95% CI, 0.417–1.097). Meanwhile, there was no difference between the two groups in terms of postoperative stroke (RR, 2.425; 95% CI, 0.743–7.915), WRF (RR, 1.257; 95% CI, 0.533–2.964), and reoperation for bleeding or tamponade (RR, 1.667; 95% CI, 0.527–5.270). Conclusions: The findings of this meta-analysis suggest that MVR + CABG fails to improve the clinical outcomes of patients with moderate IMR compared to CABG alone.

Abstract:Aims: Treatment of ischaemic mitral regurgitation (IMR) remains controversial. While IMR is associated with worse outcomes, randomized controlled trials (RCTs) and observational studies provided conflicting evidence regarding the benefit of mitral valve replacement (MVR) or repair (MVr) in addition to coronary artery bypass grafting (CABG). We conducted a meta-analysis incorporating data from published RCTs and observational studies comparing CABG vs. CABG + MVR/MVr. Methods and results: We searched PubMed, MEDLINE, Embase, Ovid, and Cochrane for RCTs and observational studies comparing CABG (Group 1) vs. CABG + MVR/MVr (Group 2). Outcome was 30-day and 1-year mortality after surgical intervention. Mantel-Haenszel odds ratio (OR) was calculated using random-effects meta-analysis for the outcome. Heterogeneity was assessed by I2 statistics. Four RCTs and 11 observational studies met the inclusion criteria (5781 patients, 507 in RCTs, 5274 in observational studies). Group 1 vs. 2 weighted mean left ventricular ejection fraction in RCTs and combined RCTs/observational studies was 41.5 ± 12.3 vs. 40.3 ± 10.4% ( P -value = 0.24) and 45.5 ± 7.2 vs. 38 ± 10% ( P -value < 0.001), respectively. In RCTs, there was no difference in 30-day [OR: 0.95, 95% confidence interval (95% CI): 0.30-3.08, P = 0.94] or 1-year (OR: 0.90, 95% CI: 0.43-1.87, P = 0.78) mortality, respectively. For combined RCTs/observational studies, there was no difference in mortality at 30 days (OR: 0.67, 95% CI: 0.43-1.04, P = 0.08) or at 1 year (OR: 0.90, 95% CI: 0.7-1.15, P = 0.39). Conclusion: In a meta-analysis of RCTs and observational studies of IMR patients, the addition of MVR/MVr to CABG did not improve survival.
Surgical Repair of Moderate Ischemic Mitral Regurgitation-A Systematic Review and Meta-analysis

Abstract:Introduction Moderate mitral regurgitation (MR) of ischemic etiology has been associated with worse outcomes after coronary artery bypass grafting (CABG). Studies comparing concomitant mitral valve replacement/repair (MVR/Re) with CABG and standalone CABG have reported conflicting results. We performed a systematic review and meta-analysis of the published literature. Patients and Methods We searched using PubMed, Cochrane, EMBASE, CINAHL, and Google scholar databases from January 1960 to June 2016 for clinical trials comparing CABG to CABG + MVR/Re for moderate MR. Pooled risk ratio or mean difference (MD) with 95% confidence intervals (CI) for individual outcomes were calculated using random effects model and heterogeneity was assessed using Cochrane's Q-statistic. Results A total of 11 studies were included. Mean follow-up was 35.3 months. All-cause mortality (Mantel-Haenszel [MH] risk ratio [RR]: 0.96, 95% CI: 0.75-1.24, p = 0.775), early mortality (MH RR: 0.65, 95% CI: 0.39-1.07, p = 0.092), and stroke rates (MH RR 0.65, 95% CI: 0.21-2.03, p = 0.464) were similar between CABG and CABG + MVR/Re groups. Adverse event at follow-up was lower with CABG (MH RR: 0.90, 95% CI: 0.61-1.32, p = 0.584). MD of change from baseline in left ventricular (LV) end-systolic dimension (MD: - 2.50, 95% CI: - 5.21 to - 0.21, p = 0.071) and LV ejection fraction (MD: 0.48, 95% CI: - 2.48 to 3.44, p = 0.750) were not significantly different between the groups. Incidence of moderate MR (MH RR: 3.24, 95% CI: 1.79-5.89, p < 0.001) was higher in the CABG only group. Conclusion Addition of MVR/Re to CABG in patients with moderate ischemic MR did not result in improvement in early or overall mortality, stroke risk, or intermediate markers of LV function when compared with CABG alone.

Abstract:Objective: To analyze the effect of combined coronary artery with mitral valve surgery and summarize our clinical experience. Methods: From August, 2002 to May, 2006, 42 patients underwent combined CABG and mitral valve surgery because of concomitant ischemia myocardium disease and mitral regurgitation (MR) of moderate to severe degree. The etiology of MR was ischemic myocardium disease in 38 (90.48%, 38/42), rheumatic heart disease in 2(4.76%, 2/42)and degenerative valve disease in 2(4.76%, 2/42). Preoperative eject fraction (EF) was 0.20-0.80 (mean 0.41±0.14).Of all the patients 12 cases underwent mitral valve replacement and 30 mitral valvuloplasty. Results: Affer surgery 4 (9.52%,4/42) patients died in hospital. 36 patients were discharged with trivial to mild MR and 2 with moderate MR which had been severe preoperatively. The mean EF shows no significant improvement in discharged patients (0.41±0.14 preoperative versus 0.45±0.12 postoperative, P=0.152). But in MVP group, significant difference could be found (0.36±0.08) preoperative versus 0.41±0.10 postoperative, P=0.013). Conclusion: Coronary artery disease and mitral valve insufficiency could be effectively corrected by combined CABG and mitral valve procedure. MVP is preferred and annuloplasty ring involvement could ensure effect of operation.

Abstract:BACKGROUND: The clinical outcome of mitral valve repair (MVP) is considerably more favorable than that of mitral valve replacement (MVR) in patients with degenerative mitral disease. However, rheumatic heart disease (RHD) is still the predominant cause of mitral valve surgery in developing countries and the advantages of MVP in RHD have still not been definitely proven. The aim of this meta-analysis was thus to evaluate the suitability of MVP in patients with RHD. Considering the difference between mechanical and biological valves, we distinguished them from each other and compared them with MVP individually.METHODS: A comparison of clinical outcomes of MVP and MVR in patients with RHD was performed based on clinical trial data. Relevant articles published from January 1, 1990 until March 1, 2020 were identified in Pubmed, Cochrane Library, and China National Knowledge Infrastructure database (CNKI). Studies that lacked direct comparisons between MVP and MVR were excluded.RESULTS: A total of 16 studies with 8659 patients were included in the analysis. The MVP group displayed lower early and long-term mortality, and fewer valve-related events and major adverse events. However, this patient group required more reoperations compared with the MVR group. Similar results were observed after distinguishing between mechanical and bioprosthetic valves to compare MVP with MVR (mech-valves), but no statistically significant difference was identified in the reoperation rate between MVP and MVR (bio-valves). MVP was further associated with increased risk of mitral reoperation in patients undergoing concomitant aortic valve replacement (AVR) surgery but without any improved early and long-term survival.CONCLUSIONS: MVP and MVR are beneficial for patients with RHD. For skilled surgeons, MVP can be performed for some suitable patients with RHD and is preferred for elderly patients or patients with contraindications of anticoagulation. However, MVR is more appropriate when concomitant AVR is needed.

Abstract:Objective To report the surgical technique and clinical results of ischemic mitral regurgitation. Methods From December 1996 to April 2001, 38 patients with ischemic mitral regurgitation underwent surgical treatment. There were 33 males ,5 females with a mean age of (58±8) years old. The New York Heart Association (NYHA) cardiac function was class I-II 4 cases and class III-IV 34 cases. Six cases was mild mitral regurgitation, 16 moderate and 16 severe. Thirty-two patients were treated with coronary artery bypass grafting(CABG) and concomitant mitral valve correction. Six patients were treated with CABG alone because the mitral regurgitation was mild. Results No operative and postoperative death, postoperative severe morbidities happened. All patients were clinically asymptomatic, and the cardiac function was NYHA class Ⅰ-Ⅱ. Thirty-seven patients were followed up for a mean duration of (20.8±14.3) months, one case died lately and one patient was NYHA class III, the others cardiac function was NYHA class Ⅰ-Ⅱ. Conclusion Mitral valve surgery is indicated in patients undergoing coronary artery bypass graft surgery who have moderate or severe ischemic mitral regurgitation. The mitral valvuloplasty should be the first choice, and the early outcome is satisfied.

Abstract:ObjectiveWhether moderate ischemic mitral regurgitation (IMR) should be repaired during coronary artery bypass grafting (CABG) is still uncertain. This meta-analysis of randomized controlled trials (RCTs) evaluated the efficacy of adding mitral valve repair (MVR) to CABG in patients with moderate IMR.MethodsWe searched PubMed, the Cochrane Library, and the Web of Science for RCTs that compared the efficacy of CABG plus MVR with CABG alone. Four RCTs that included 505 patients met the eligibility criteria.ResultsCABG+MVR significantly reduced the risk of intermediate residual mitral regurgitation (MR) grade 2+ compared with CABG alone (risk ratio [RR]=0.20, 95% confidence interval [CI] 0.04-0.92, p=0.04), but did not have advantages on 30-day/in-hospital mortality (RR=1.06, 95% CI 0.37-3.09, p=0.91), intermediate mortality (RR=0.90, 95% CI 0.48-1.67, p=0.73), risk of intermediate NYHA class II (RR=0.62, 95% CI 0.24-1.62, p=0.33), intermediate left ventricular ejection fraction (LVEF) (SMD=0.04%, 95% CI -0.35 to 0.42, p=0.84), and intermediate LV end-systolic volume index (LVESVI) (SMD=-0.20mL/m(2), 95% CI -0.92 to 0.51, p=0.58).ConclusionCompared with CABG alone, adding MVR to CABG in patients with moderate IMR reduces the residual MR grade, but has no significant effect on mortality, intermediate NYHA class, LVEF, and LVESVI. Abstract:Objective To study the surgical treatment principles of coronary heart disease (CHD) with moderate-severe ischemic mitral insufficiency .Methods Clinical data about 61 CHD patients with moderate-severe ischemic mitral insufficiency after surgical treatment were retrospectively analyzed .Results Of the 61 patients ,45 underwent coronary artery bypass grafting and mitral valve repair (CABG+MVP) and 16 received CABG+mitral valve replacement (CABG+ MVR) , 2 died of heart failure early after operation and 59 were cured and discharged .Echocardiography showed that the LVEDD was shorter after operation than before operation while the LVEF was higher after operation than before operation [(47 .9 ± 8 .9) mm vs (52 .8 ± 11 .3) mm ,(52 ± 12)% vs (46 ± 11)% ,P<0 .01] .Conclusion Combined CABG and mitral valve procedure can achieve sat-isfactory outcomes in CHD patients with moderate-severe ischemic mitral insufficiency .

Depending on the type of mask employed, the triangle could be perceived when the inducing discs were not phenomenally present, or the discs could be seen with the triangle not present. These findings suggest that no single set of mechanisms, physiological or cognitive, will suffice to explain perception of the illusory triangle.
Surgical Management of Coronary Artery Disease Associated with Mitral Regurgitation

Abstract:Objective: To review the experience in surgical management of coronary artery disease associated with mitral regurgitation (MR). Methods: From April 1994 to October 2000, thirty four patients underwent open heart surgery under hypothermic cardiopulmonary bypass (CPB) for their coronary artery disease and MR . There were 10 patients with NYHA function class II, 18 with class III and 6 with class IV. The average left ventricular end diastolic diameter(LVEDD) was (60 4±8 9) mm. There were 25 mild MR and 8 severe MR. The predominant pathology of mitral valve was isolated annular dilation, valvular prolapse or chordae rupture. All patients received coronary artery bypass grafting (CABG). Surgical procedures for mitral disease included mitral valvoplasty in 27 patients(mitral commissural valvorrhaphy in 15 , partial valvulectomy in 9 , and double orifice mitral valve plasty in 3), and mitral valve replacements in 7. The Carpentier ring was used in 20 cases. Results: There were no operative or hospital mortality or severe morbidities. The mean postoperative LVEDD was (53 0±6 3) mm (P0 01). Thirty four patients were followed up for a mean duration of 29 months. There were no late deaths and the patients' quality of life was improved significantly. There were 28 patients with NYHA function class I II, 3 with class III. Mild MR was found in 9 patients postoperatively. All the mechanical valve prosthesis functioned well. Conclusion: Coronary artery disease with moderate mitral regurgitation should be treated aggressively. Mitral valvuloplasty is the first choice.
Concomitant mitral valve surgery in patients with moderate ischemic mitral regurgitation undergoing coronary artery bypass grafting

Abstract:Background: The clinical benefits of a concomitant mitral valve (MV) surgery in patients with moderate ischemic mitral regurgitation (iMR) undergoing coronary artery bypass grafting (CABG) remain controversial. Methods: The study involved 710 patients (mean age, 65.0±8.9 years; 504 males) with moderate iMR undergoing CABG between 1990 and 2015. Of these, 116 (16.3%) patients underwent a concomitant MV surgery (MVS; replacement in 10, repair in 106) and 594 (83.7%) underwent CABG only. Clinical and echocardiographic outcomes were compared before and after adjustment with the use of propensity score (PS) analyses. Results: Early mortality occurred in 22 (3.7%) and 13 (11.2%) patients in CABG-only and CABG with MVS group, respectively (P=0.001). After adjustment, CABG with MVS group showed significantly increased risks of early death (P<0.001), low cardiac output syndrome (LCOS) (P=0.001) and surgical bleeding (P=0.014). During a median follow-up of 78.0 months (quartile 1-3, 33.6-115.9 months), overall mortality occurred in 286 (40.3%) patients. The addition of an MV surgery showed an increased risk of overall mortality [hazard ratio (HR), 1.34; 95% confidence interval (CI), 0.99-1.80; P=0.055], which became comparable 1 year after surgery on landmark survival analysis (HR, 0.94; 95% CI, 0.64-1.39; P=0.772). Improved left ventricular (LV) ejection fraction and LV reverse remodeling were observed in both groups without significant intergroup differences. Conclusions: The addition of a concomitant MV surgery increased the risk of early mortality and complications in patients with moderate iMR undergoing CABG. In long-term clinical and echocardiographic outcomes, a concomitant MV surgery seemed to confer no significant clinical benefits.

Abstract:There is currently limited research on the potential mechanisms underlying the development of antisocial personality disorder (ASPD). One such mechanism, distress tolerance (defined as an individual's behavioral persistence in the face of emotional distress) may underlie the development of ASPD and its associated behavioral difficulties. It was hypothesized that substance users with ASPD would evidence significantly lower levels of distress tolerance than substance users without ASPD. To test this relationship, we assessed 127 inner-city males receiving residential substance abuse treatment with two computerized laboratory measures of distress tolerance. The mean age of the sample was 40.1 years (SD = 9.8) and 88.2% were African American. As expected, multiple logistic regression analyses indicated that distress intolerance significantly predicted the presence of an ASPD diagnosis, above and beyond key covariates including substance use frequency and associated Axis I and II psychopathology. Findings suggest that distress tolerance may be a key factor in understanding the development of ASPD, setting the stage for future studies expanding on the nature of this relationship, as well as the development of appropriate interventions for this at-risk group.

Abstract:Introduction: The objective of this cohort study was to analyze the long-term relative survival of degenerative valve disease (DVD) patients who underwent mitral valve repair (MVP) or replacement and aortic valve replacement (AVR). Methods: A total of 146 patients underwent double valve replacement (DVR) or MVP+AVR at four institutions between 2016 and 2022. Kaplan–Meier method was applied to analyze survival rate. The potential predictors of mortality were investigated by Cox regression. Results: Of 146 patients, 62 underwent MVP+AVR, and 84 underwent DVR. The thirty-day mortality rate was 4.76% in the DVR cohort and 1.61% in the MVP+AVR cohort. At baseline, there were differences in age (63.39 ± 8.01 vs. 58.46 ± 9.92, p = 0.012), proportions of male patients (51.61% vs. 72.62, p = 0.014), smoking history (45.16% vs. 28.57%, p = 0.039). More biological valves were applied in the MVP+AVR cohort (77.42% vs. 47.62%, p < 0.001). There was no significant difference in mortality between the cohorts (1339.5 [Interquartile range (IQR), 1021.25–1876.75] vs. 1026.00 [IQR, 679.50–1674.00], p = 0.252). The overall mortality rate was 16.67% for DVR and 6.45% for MVP+AVR. Mechanical valve replacement (hazard ratio (HR) = 3.7, 95% confidence interval (CI): 1.0–12.0, p = 0.029) was increased the risk of postoperative mortality. Conclusion: Although the superiority of MVP+AVR was not verified with statistical significance in our cohort, we believe that MVP+AVR should be the preferred strategy for treating most DVD patient because it is associated with higher survival rates during follow-up.

Abstract:Background. An increasing number of elderly patients with ischemic mitral regurgitation (IMR) are referred for coronary artery bypass grafting (CABG). However, data about the management of elderly patients with moderate IMR are scanty. This study evaluates the impacts of two surgical approaches (CABG alone or concomitant mitral annuloplasty (MAP)) on in-hospital and midterm outcomes, to attempt to determine an appropriate treatment option for elderly patients with moderate chronic IMR. Methods. All eligible patients over 65 years of age were included and were entered into either a MAP group (patients undergoing CABG plus MAP, n = 96) or a CABG group (patients receiving CABG alone, n = 104). Baseline and surgical characteristics were analyzed, and in-hospital and midterm outcomes between groups were compared after propensity score-matching (1 : 1). Results. Using propensity score-matching, 82 pairs of patients were successfully established in a 1 : 1 ratio. No significant differences between the two matched groups were found regarding surgical mortality (4.9% vs. 1.2%, p=0.173) and major postoperative morbidity. 150 patients (76 in the MAP group and 74 in the CABG group) received regular follow-up visit with the median duration of 37 months. Compared with the CABG group, the MAP group received a similar overall survival but a better recurrent MR-free survival (stratified log-rank p, 0.492 and < 0.001, respectively). Using Cox regression, the MAP group as compared with the CABG group did not affect midterm survival probability (propensity score-adjusted hazard ratio, 0.854; 95% confidence interval, 0.571–2.729, p=0.630). Additionally, patients in the MAP group had a significantly lower ratio of NYHA class III-IV at the latest follow-up by comparison with patients in the CABG group (19.7% vs. 35.5%, p=0.033). Conclusion. Compared with CABG alone, concomitant mitral annuloplasty is associated with improved midterm outcomes (including reduced IMR recurrence and improved cardiac functional class) but shares similar surgical mortality and major postoperative morbidity and may be a promising treatment option for elderly patients with moderate chronic IMR.
