Abstract:Objective To establish a model for pancreas transplantation in miniature swine and to monitor acute rejection. Methods 40 miniature swine were divided into donors and recipients randomly. The total pancreas and a short segment of duodenum were transplanted to a recipient by anastomoses of vessels to the recipient iliac vessels and small bowel segment to the jejunum with enteric drainage. During recipients operation, mean arterial pressure(MAP), central venous pressure(CVP) and blood gas were monitored. The levels of amylase and glucose in blood and immunologic markers were investigated postoperation. Color flow Doppler ultrasonography in the evaluation of blood flow to pancreatic allografts and ultrasound-guided biopsies of the pancreas and histopathologic examination were performed. Results The successful rate of operation was 90%. Recipient's MAP was significantly decreased after the reopening of inosculated blood vessels (P0.05). Adequate transfusion could contribute to successful operation for recipients. The mean survival of all swine was (12.6±2.3)days. The immunologic markers in peripheral blood and histopathologic changes were earlier than the alteration of function and clinical symptoms during acute rejection. Conclusion Miniature swine are suitable for the establishment of pancreas transplantation model. Enhancement of monitoring and management of circular system function and adequate transfusion make for pancreas transplantation. Through monitoring immunologic markers in peripheral blood and ultrasound_guided biopsies of the allografts and histopathologic examination could diagnosis acute rejection earlier.

Abstract:Objective To investigate the relationship between the variation of T cell subsets, tumor necrosis factorα (TNF-α)、interleukin 2(IL-2) and acute rejection of pancreatic duodenal allotransplantation. Methods Miniature Swine model of pancreatic duodenal allotransplantation was established. Biotin-Streptavidin-Peroxidase was used to determine the T cell subsets in peripheral blood. Double-antibody-coated ELISA was used to measure the level of TNF-α and IL-2. Ultrasound guided biopsies of the pancreas and histopathologic examination were performed. Result The acute rejection was determined by histological change of allografts at the third day after transplantation. The percentages of CD4 cell,CD4/CD8 ratio were risen at the third day after transplantation in accordance with histological change. The level of TNF-α was risen at the second day and IL-2 was risen at the fifth day after transplantation. The change of TNF-α was earlier than histological change,but IL-2 was later. Conclusion T cell subsets,TNF-α and IL-2 participate in the acute rejection of pancreas allografts. The levels of T cell subsets and TNF-α can be a diagnostic index for early acute rejection in pancreatic duodenal allotransplantation.

Abstract:Objective To investigate the relationships between the expressions of intercellular adhesion molecule 1(ICAM-1),heat shock protein 70(HSP70) and acute rejection after pancreas allotransplantation in miniature swine.Methods Ultrasound guided biopsies of the pancreas and histopathologic examination were performed after the miniature swine models of pancreas allotransplantation were established.According to the histological change,the samples were divided into group Ⅰ(no rejection),group Ⅱ(mild acute rejection),group Ⅲ(moderate acute rejection)and group Ⅳ(serious acute rejection).The expressions of ICAM-1 and HSP70 were analyzed with immunohistochemistry and imaging.Results The expressions of ICAM-1 and HSP70 were increased when acute rejection occurred after pancreas allotransplantation in miniature swine.The levels of ICAM-1 and HSP70 were sequentially high when acute rejection was serious.Conclusion ICAM-1 and HSP70 were involved in pancreas allograft rejection and useful for early diagnosis of acute rejection after pancreas transplantation.
Acute rejection after swine leukocyte antigen-matched kidney allo-transplantation in cloned miniature pigs with different mitochondrial DNA-encoded minor histocompatibility antigen

Abstract:Introduction: Graft rejection remains a major cause of morbidity and mortality following renal transplantation. One of the main determinants of success after renal transplantation is histocompatibility between donor and recipient. Most of the research on this topic has addressed human leukocyte antigen (HLA), but the roles played by minor histocompatibility antigens (mHAgs), such as mitochondrially transmitted antigens, are poorly understood. In this study, we evaluated immune responses induced by minor antigens originating from mitochondrial DNA (mtDNA) in a large animal model. Methods: To characterize whole swine leukocyte antigen (SLA) allele in 8 cloned pigs, we performed SLA genotyping for SLA-1, SLA-2, SLA-3, SLA-DQB1, and SLA-DRB1 as well as the hypervariable region 1 (HV1) of mtDNA. Renal transplantation was performed using SLA-matched pigs with different mtDNA as well as SLA-mismatched cloned animals. Cytokine profiling was performed by incubating peripheral leukocytes with cellular components from SLA-matched different mtDNA and SLA-mismatched cells to evaluate mtDNA-mediated immune response. Results: SLA types were confirmed to be identical, but mtDNA sequences of HV1 varied among cloned pigs. Rejection episodes in the SLA-matched group with different mtDNA were similar to those in the SLA-mismatched group; that is, plasma creatinine and BUN levels were increased and mononuclear cell infiltration was observed in perivascular regions in the matched and SLA-mismatched groups. Furthermore, in vitro studies showed interleukin (IL)-1β expression to be elevated in SLA-matched and SLA-mismatched groups. Conclusion: Cloned pigs are a useful preclinical model to evaluate the immunogenicity of mtDNA encoding minor antigens. The mtDNA originating from nongenomic DNA induced cell-mediated immune rejection after kidney transplantation.
Early biological and immune response to semi-identical liver or kidney allograft in miniature swine

Abstract:In inbred miniature swine, semi-identical liver allograft recipients survive up to 3 months without immunosuppression, whereas similarly mismatched kidney allografts are uniformly rejected within 2 weeks. The early biological and immunological events were assessed in this unique model. SLA(d/d) pigs (MGH, Harvard Medical School, Boston, MA, USA) received liver or kidney allograft from heterozygous SLA(c/d) miniature swine. Survival, graft function, histology, intragraft cytokines, peripheral lymphocyte and platelet count, plasma cortisol level and cellular/humoral anti-donor immune response were assessed. Kidney allografts were uniformly rejected within 2 weeks, whereas liver allografts survived for up to 87 days. After both liver and kidney transplantation, the peripheral lymphocyte count decreased during the first week concomitantly to a significant elevation of plasma cortisol level. Early decrease of peripheral platelet count was observed after liver but not renal transplantation. Up-regulation of transforming growth factor beta1 (TGF-beta1) and interferon-gamma (IFN-gamma) was observed during the first postoperative week in semi-identical liver allografts and IFN-gamma as well as IL-10 in kidney allografts. In liver recipients, labelled autologous lymphocytes accumulated in the liver graft and native spleen, whereas after renal allograft, lymphocytes accumulated in the native spleen and liver but never in the kidney allograft. Specific cellular anti-donor unresponsiveness was observed from the first post-transplant day in both liver and kidney recipients, while the humoral anti-donor response remained intact. In semi-identical liver allograft, recipient rejection is milder and slower than in similarly matched kidney allograft. The intragraft up-regulation of TGF-beta1 in semi-identical liver allograft might be one mediator to explain the modulation of rejection after liver transplant. The rapid, nonspecific accumulation of recipient lymphocytes in the liver allograft but not in kidney allograft might also play a role in the different survival time in this model.
[Xenogeneic Antigens and Immune Response in Pig to Man Xenograft].

Abstract:Limitation of donor source for allograft makes the research on xenograft progress. Pig is regarded as one of the ideal donor animals. The major obstacle in xenograft is hyperacute rejection, which is caused by complements after they are activated by xenogeneic antigens combined with natural antibodies. It has been confirmed that alpha-Gal is the major target antigen, whose expression is incharged by alpha-1,3 galactosyltransferase (alpha-GT). The approaches to overcome hyperacute rejection against alpha-Gal included: immunoadsorption of xenogeneic natural antibodies, lysis of antigen by enzyme and genetic manupilation to obtain animal lack of alpha-GT. Besides alpha-Gal, there were other antigens binding to human serum antibody, such as gp65 and gp100, which was expressed on PAEC after induced by TNF, the A-like antigen. But their function was still unknown. It was debatable on the role of MHC in xenograft. Both direct and indirect pathway were involved in cellular response in xenograft.

Abstract:Hormonal suppression of spermatogenesis is currently being investigated as a method of reversible male contraception. However, administration of exogenous testosterone (T) induces azoospermia in only 40-70% of Caucasian men, whereas the remainder suppresses to severe oligozoospermia (< 5 x 10(5)/mL). The reason(s) for the heterogeneity in the spermatogenic response is not clear. We have prospectively investigated the possibilities that higher plasma concentrations of T and/or differences in the extent and rate of gonadotropin suppression could maintain a low level of spermatogenesis in subjects taking part in a clinical efficacy trial of hormonal male contraception. Thirty-three healthy adult men, aged 21-41 yr, were given 200 mg T enanthate (TE), im, weekly for up to 18 months. Azoospermia was achieved in 18 men (55%) after 20 weeks of treatment, at which time the remaining 15 (45%) stabilized at a mean sperm density of 2.0 +/- 0.8 (+/- SD) x 10(6)/mL. These 15 subjects remained oligozoospermic for the rest of the efficacy study. To compare the pharmacokinetics and pharmacodynamics of TE between the azoospermic and oligozoospermic responders, plasma samples were obtained immediately before and 1, 2, 4, and 7 days after the 1st and 16th TE injections. Further samples were taken after 2, 4, 8, and 12 weeks of treatment. Plasma concentrations of total, free, and non-sex hormone-binding globulin (non-SHBG)-bound T, estradiol, LH, and FSH were measured. Compared to baseline, preinjection levels of total T increased 2.5-fold, reaching a steady state around 12 weeks of treatment. Peak concentrations of total T increased by 5-fold, but free and non-SHBG-bound T levels were increased by 10-fold after 16 weeks. The plasma levels of estradiol showed similar changes as T. However, neither T (bound or free) nor estradiol was significantly different between azoospermic and oligozoospermic responders. Plasma SHBG was reduced to a similar degree in both groups of men after 16 weeks of TE treatment. Plasma concentrations of both LH and FSH decreased rapidly after the first TE injection; a significant decline in LH was detectable after 24 h. Mean levels of both gonadotropins decreased to less than 0.5 U/L by the end of 4 weeks and to below the limit of sensitivity of the assays (0.05 IU/L) by 12 weeks. There were no significant differences in plasma concentrations of LH or FSH or in the rates of suppression between azoospermic and oligozoospermic responders. We conclude that the polymorphism of spermatogenic suppression in response to exogenous T is unlikely to be due to differences in the pharmacokinetics or pharmacodynamics of TE or in the sensitivity of the hypothalamo-pituitary-testicular axis to sex steroid inhibition. Measurements of total plasma T considerably underestimate the increase in bioavailable T during the weekly TE regimen.

Abstract:Background: Although the increased interest in investigating the dynamics of fatherhood in developed nations has been documented in the literature, its collective meaning and implications in nursing remain largely unknown in the context of East Asia, especially in developing countries such as the Philippines. Purpose: Capitalizing on the unique power of metaphors to improve the understanding of complex and abstract ideas and to shape healthcare practices, this qualitative semiotic investigation intended to define the essence of fatherhood from the perspective of Filipino fathers. Methods: This study focused on a group of 28 first-time and 22 second-time fathers who were recruited from the largest maternal and newborn tertiary government hospital in the Philippines. To capture the richness and thickness of fatherhood as a subjective human reality, doodling and elicitation interviews were employed to promote data triangulation. Furthermore, semiosis was used as the data analytic framework to better understand the ontology and epistemology of the elicited metaphors. In addition, a member-checking procedure was employed to validate the themes that emerged from the inductive approach. Results: This study provides a conceptualization of “The House of Fatherhood,” which illustrates the uniqueness of the experience of fatherhood as identity defining, maturity enabling, and duality affirming. Conclusions/Implications for Practice: The pillars of fatherhood that were identified in this article provide a valuable framework for Filipino healthcare providers and program administrators in the field of maternal and child nursing to identify scaffolding measures and other support mechanisms to address the needs and concerns of Filipino fathers and their journey to fatherhood in a more effective and holistic way.
The induction of tolerance of renal allografts by adoptive transfer in miniature swine

Abstract:Our previous in vitro data have demonstrated that regulatory mechanisms are involved in tolerance of class I-mismatched renal allografts in miniature swine treated with 12 days of high dose Cyclsporin A. In this study, we attempted to induce tolerance of class I-mismatched kidneys by adoptive transfer of cells and/or kidneys from long-term tolerant animals. Fifteen SLA(dd) miniature swine received 1.5 Gy whole body irradiation and class I-mismatched (SLA(gg) ) kidneys from naïve pigs with or without cotransplanted kidneys and/or adoptively transferred cells from long-term tolerant (LTT) SLA(dd) recipients of SLA(gg) grafts. In addition, three SLA(dd) miniature swine received class I mismatched kidney with adoptively transferred cells from LTT SLA(dd) recipients. Naïve kidneys transplanted without a LTT kidney were rejected within 9 days. All recipients of naive kidneys along with cells and kidney grafts from LTT animals showed markedly prolonged survival of the naive renal grafts (day 28, >150 and >150 days). These studies suggest that (1) tolerated kidneys have potent regulatory effects and (2) cells from LTT animals infused in conjunction with kidney grafts augment these regulatory effects. To our knowledge, these studies represent the first demonstration of successful adoptive transfer of tolerance in large animals.

Abstract:Background: Cross-species immunological incompatibilities have hampered pig-to-human xenotransplantation, but porcine genome engineering recently enabled the first successful experiments. However, little is known about the immune response after the transplantation of pig kidneys to human recipients. We aimed to precisely characterise the early immune responses to the xenotransplantation using a multimodal deep phenotyping approach. Methods: We did a complete phenotyping of two pig kidney xenografts transplanted to decedent humans. We used a multimodal strategy combining morphological evaluation, immunophenotyping (IgM, IgG, C4d, CD68, CD15, NKp46, CD3, CD20, and von Willebrand factor), gene expression profiling, and whole-transcriptome digital spatial profiling and cell deconvolution. Xenografts before implantation, wild-type pig kidney autografts, as well as wild-type, non-transplanted pig kidneys with and without ischaemia-reperfusion were used as controls. Findings: The data collected from xenografts suggested early signs of antibody-mediated rejection, characterised by microvascular inflammation with immune deposits, endothelial cell activation, and positive xenoreactive crossmatches. Capillary inflammation was mainly composed of intravascular CD68+ and CD15+ innate immune cells, as well as NKp46+ cells. Both xenografts showed increased expression of genes biologically related to a humoral response, including monocyte and macrophage activation, natural killer cell burden, endothelial activation, complement activation, and T-cell development. Whole-transcriptome digital spatial profiling showed that antibody-mediated injury was mainly located in the glomeruli of the xenografts, with significant enrichment of transcripts associated with monocytes, macrophages, neutrophils, and natural killer cells. This phenotype was not observed in control pig kidney autografts or in ischaemia-reperfusion models. Interpretation: Despite favourable short-term outcomes and absence of hyperacute injuries, our findings suggest that antibody-mediated rejection in pig-to-human kidney xenografts might be occurring. Our results suggest specific therapeutic targets towards the humoral arm of rejection to improve xenotransplantation results. Funding: OrganX and MSD Avenir.

Abstract:Background: The recent availability of alpha1,3-galactosyltransferase knockout (GalT-KO) miniature swine has eliminated anti-Gal antibodies as the major barrier to xenotransplantation, potentially bringing this modality closer to clinical application. Highly-allosensitized patients, who have poor prospects of receiving a suitable cross-match negative human organ, might be the first patients to benefit from xenotransplantation of porcine organs. However, concerns exist regarding cross-reactivity of alloreactive anti-human leukocyte antigen (HLA) antibodies against xenogeneic swine leukocyte antigen (SLA) antigens. We have investigated this question using sera from such patients on GalT-KO target cells. Methods: Using flow cytometry and complement-dependent cytotoxicity (CDC) assays, we have tested a panel of 88 human serum samples from patients awaiting cadaveric renal allotransplantation for reactivity against: 1) human; 2) standard miniature swine; and 3) GalT-KO peripheral blood lymphocytes (PBL) and cultured endothelial cells. Results: Anti-swine IgM and IgG antibody binding, as well as CDC, were significantly attenuated on GalT-KO versus standard swine. No correlation was found between the degree of anti-human panel reactive antibodies (PRA) and xenoreactivity against either standard or GalT-KO miniature swine. Treatment of sera with dithiothreitol (DTT) showed that the majority of remaining lymphocytotoxicity against GalT-KO swine was mediated by preformed IgM antibodies. Patients with high alloreactivity but low anti-GalT-KO xenoreactivity were readily identified. Conclusions: Highly allosensitized patients awaiting renal transplants appear to be at no increased risk of xenosensitization over their non-sensitized cohorts, and could therefore be candidates for xenotransplantation using GalT-KO swine donors.

Abstract:Background: Blood vessels that contain endothelial cells (ECs) on the surface are in direct contact with host blood and are the first target of xenograft rejection. Currently, our understanding of human anti-pig vessel immune responses is primarily based on in vitro assays using pig ECs. Therefore, it is necessary to develop an animal model that permits in vivo study of human immunological rejection of pig vessels. Methods: Pig artery tissues (PAT) were transplanted into human immune system (HIS) mice or immunodeficient NSG mice (as controls). Intragraft human immune cell infiltration and antibody deposition were quantified using histology and immunohistochemistry. Donor antigen-specific immune responses were quantified using a mixed lymphocyte reaction and a complement-dependent killing assay. Results: Pig CD31+ ECs were detected and increased 2-fold from weeks 3 to 5 in PAT xenografts from immunodeficient NSG mice. However, compared with NSG mice, PAT xenografts in HIS mice had significantly lower numbers of porcine CD31+ ECs and showed a marked reduction from week 3 to week 5. PAT xenograft rejection in HIS mice is associated with intensive infiltration of human immune cells, deposition of human IgM and IgG antibodies, and the formation of a tertiary lymphoid structure. Robust donor pig antigen-specific human T cells and antibody responses were detected in PAT-transplanted HIS mice. Conclusion: We have developed a humanized mouse model to evaluate human anti-pig xenoimmune responses by PAT transplantation in vivo. This model is expected to facilitate the refinement of pig gene-editing strategies (the expression on EC surface) and the testing of local immunosuppressive strategies for clinical pig organ xenotransplantation.

Abstract:Antibody-mediated rejection (AMR) is the commonest cause of failure of a pig graft after transplantation into an immunosuppressed nonhuman primate (NHP). The incidence of AMR compared to acute cellular rejection is much higher in xenotransplantation (46% vs. 7%) than in allotransplantation (3% vs. 63%) in NHPs. Although AMR in an allograft can often be reversed, to our knowledge there is no report of its successful reversal in a pig xenograft. As there is less experience in preventing or reversing AMR in models of xenotransplantation, the results of studies in patients with allografts provide more information. These include (i) depletion or neutralization of serum anti-donor antibodies, (ii) inhibition of complement activation, (iii) therapies targeting B or plasma cells, and (iv) anti-inflammatory therapy. Depletion or neutralization of anti-pig antibody, for example, by plasmapheresis, is effective in depleting antibodies, but they recover within days. IgG-degrading enzymes do not deplete IgM. Despite the expression of human complement-regulatory proteins on the pig graft, inhibition of systemic complement activation may be necessary, particularly if AMR is to be reversed. Potential therapies include (i) inhibition of complement activation (e.g., by IVIg, C1 INH, or an anti-C5 antibody), but some complement inhibitors are not effective in NHPs, for example, eculizumab. Possible B cell-targeted therapies include (i) B cell depletion, (ii) plasma cell depletion, (iii) modulation of B cell activation, and (iv) enhancing the generation of regulatory B and/or T cells. Among anti-inflammatory agents, anti-IL6R mAb and TNF blockers are increasingly being tested in xenotransplantation models, but with no definitive evidence that they reverse AMR. Increasing attention should be directed toward testing combinations of the above therapies. We suggest that treatment with a systemic complement inhibitor is likely to be most effective, possibly combined with anti-inflammatory agents (if these are not already being administered). Ultimately, it may require further genetic engineering of the organ-source pig to resolve the problem entirely, for example, knockout or knockdown of SLA, and/or expression of PD-L1, HLA E, and/or HLA-G.
A Rat Orthotopic Renal Transplantation Model for Renal Allograft Rejection

Abstract:Renal allograft rejection limits the long-term survival of patients after renal transplantation. Rat orthotopic renal transplantation is an essential model to investigate the mechanism of renal allograft rejection in pre-clinical studies and could aid in the development of novel approaches to improve the long-term survival of renal allografts. Donor kidney implantation in rat orthotopic renal transplantation is commonly performed by end-to-side anastomosis to recipients' aorta and inferior vena cava. In this model, the donor's kidney was implanted using end-to-end anastomosis to the recipients' renal artery and renal vein. The donor's ureter was anastomosed to the recipient's bladder in an end-to-side 'tunnel' method. This model contributes to better healing of ureter-bladder anastomosis and increases the recipients' survival by avoiding interference with blood supply and venous reflux of the lower body. This model can be used to investigate the mechanisms of acute and chronic immune and pathologic rejection of renal allografts. Here, the study describes the detailed protocols of this orthotopic renal transplantation between rats.
Patients on the Transplant Waiting List Have Anti-Swine Leukocyte Antigen Class I Antibodies

Abstract:Organ supply remains inadequate to meet the needs of many patients who could benefit from allotransplantation. Xenotransplantation, the use of animals as organ donors, provides an opportunity to alleviate this challenge. Pigs are widely accepted as the ideal organ donor, but humans and nonhuman primates have strong humoral immune responses to porcine tissue. Although carbohydrate xenoantigens have been studied intensively, the primate Ab response also targets class I and class II swine leukocyte Ags (SLAs). Human Abs that recognize HLAs can cross-react with SLA molecules because epitopes can be shared across species. However, ∼15% of people may also exhibit Abs toward class II SLAs despite lacking Abs that also recognize class II HLAs. Here, we extend these studies to better understand human Ab responses toward class I SLAs. When tested against a panel of 18 unique class I SLA proteins, 14 of 52 sera samples collected from patients in need of an organ transplant contained Abs that bound class I SLAs. Class I SLA-reactive sera may contain IgM only, IgG, only, or IgM and IgG capable of recognizing the pig proteins. The presence of class I HLA-reactive Abs was not essential to generating anti-class I SLA Ig. Last, anti-class I SLA reactivity varied by serum; some recognized a single SLA allele, whereas others recognized multiple class I SLA proteins.
Pig to canine auxiliary hepatic xenotransplantation model: prevention of hyperacute rejection via Kupffer cell blockade and complement regulation

Abstract:Objectives: Large animal experiment models are a critical prerequisite to preclinical trials. However, the pig-to-primate model is expensive and proper experimental conditions are difficult to establish. Several pig-to-canine lung xenotransplantation experiments have shown hyperacute rejection. Therefore, we designed a pig-to-canine liver xenotransplantation model to study the diverse immunologic and hemodynamic consequences after xenotransplantation and hyperacute rejection. Methods: Animals were divided into two groups of 3 each: a cobra venom factor plus gadolinium trichloride (GdCl(3)) treatment group (CVF+Gd group) and a control group. Whole livers from 15-kg donor pigs were harvested and perfused with histidine-tryptophan-ketoglutarate solution. Seventy percent of the left lobe of the livers of 17-kg recipient dogs was resected. The harvested pig whole liver was transplanted using the canine left hepatic vein, left portal vein, and common hepatic artery. After graft reperfusion, blood samples and aliquots of liver, lung, and kidney tissues were obtained at 1 hour after reperfusion. Results: We successfully completed 6 pig-to-canine auxiliary hepatic xenotransplantations. In the control group, the grafts showed a patchy hypoperfused liver surface that was rubbery solid compared with the CVF+Gd group. Serum total protein, albumin, fibrinogen, and platelet counts decreased abruptly; however, there were no significant differences between the two groups. There were no identifiable changes in blood urea nitrogen and creatinine concentrations. Serum prothrombin time, partial thromboplastin time, and further degradation product were increased in both groups; however, in the CVF+Gd group, the slope was more obtuse than in the control group. At microscopy, the graft at 20, 40, and 60 minutes after reperfusion, no intravascular pathologic changes were noted. Only scant intravascular fibrin deposition was observed. Hepatocellular vacuolization and sinusoidal dilatation were seen. There was patch necrosis without a zonal distribution, and intrasinusoidal neutrophil sequestration and interstitial hemorrhage. These findings were milder in the CVF+Gd group. Conclusion: A pig-to-canine partial auxiliary liver xenotransplantation model is feasible. In the CVF+Gd treatment group, pathologic findings of patch hepatocyte necrosis were less severe. Inasmuch there was no corresponding vascular pathologic finding, these abnormalities are not a direct effect of CVF+Gd treatment. Other factors such as ischemia-reperfusion injury should be considered.

Abstract:Introduction. The aim of this study was to develop a rat model of chronic irreversible rejection, which is a major causes of late graft loss and retransplantation after orthotopic liver allotransplantation. Methods. Allogeneic liver transplantation was performed in a rat combination of Dark Agouti (DA) to Brown Norway (BN). Group A was left without treatment, group B received cyclosporine' (CsA; 1 mg/kg/d) and group C, CsA (4 mg/kg/d). Animals were followed for 6 months. Liver tissue was harvested to construct a time course of histological changes after liver transplantation using histopathological and morphometric techniques. We compared the total histological score of rejection activity index and survival rates. Results. In untreated animals, irreversible acute rejection developed, all animals died within 15 days. In the low-dose CsA group, all animals that survived more than 30 days developed moderate to severe manifestations of chronic liver rejection, with graft infiltration, ductular damage or proliferation, obliterative arteriopathy, and liver fibrosis. No apparent histological alterations were observed in group C. Survival analysis showed significant differences between the three groups. Conclusions. In the rat strain combination of DA -> BN with low-dose immunosuppression, early mild inflammation was followed by the development of chronic rejection.

Abstract:Prolonged survival in preclinical renal xenotransplantation demonstrates that early antibody mediated rejection (AMR) can be overcome. It is now critical to evaluate and understand the pathobiology of late graft failure and devise new means to improve post xenograft outcomes. In renal allotransplantation the most common cause of late renal graft failure is transplant glomerulopathy—largely due to anti‐donor MHC antibodies, particularly anti‐HLA DQ antibodies. We evaluated the pig renal xenograft pathology of four long‐surviving (>300 days) rhesus monkeys. We also evaluated the terminal serum for the presence of anti‐SLA class I and specifically anti‐SLA DQ antibodies. All four recipients had transplant glomerulopathy and expressed anti‐SLA DQ antibodies. In one recipient tested for anti‐SLA I antibodies, the recipient had antibodies specifically reacting with two of three SLA I alleles tested. These results suggest that similar to allotransplantation, anti‐MHC antibodies, particularly anti‐SLA DQ, may be a barrier to improved long‐term xenograft outcomes.
