Abstract:Objective To investigate the correlation between intima-media thickness(IMT)and elasticity of carotid in patients with type 2 diabetes accompanied hyperlipidemia by quantitative intima-media thickness(QIMT)and quantitative arterial stiffness(QAS)technique.Methods Fifty-six patients with type 2 diabetes accompanied hyperlipidemia were involved into the study as case group.Forty-eight healthy volunteers were supplied as normal control group in the same period.IMT,compliance coefficient(CC),distensibility coefficient(DC)and stiffness index(β)of carotid were acquired through QIMT and QAS.Case group was divided into case group 1(IMT＜0.9mm)and case group 2(IMT≥0.9 mm),and analysis was performed furtherly.Results β,triglyceride(TG),lowdensity lipoprotein(LDL),cholesterol(Ch),glycosylated hemoglobin(GHb)and IMT had significant differences in case group 1,case group 2 and control group(P＜0.05),control group＜case group 1＜case group 2.DC and CC had significant differences in case group 1,case group 2 and control group(P＜0.05),control group＞case group 1＞case group 2.There was significant positive correlation between β and age,GHb,LDL and IMT of carotid respectively(r=0.811,r:0.764,r=0.732,r:0.729,respectively,P=0.000).Conchmions IMT of carotid could be measured accurately by QIMT.Structure imaging and functional imaging of carotid might be combined organically through QIMT and QAS.Depth study about early change of carotid in structure and function could be performed with these two techniques.

Abstract:Objective To discuss the association between the carotid intima-media thickness(IMT) and adult obstructive sleep apnea hypoventilation Syndrom(OSAHS).Methods Blood pressure at 7:00AM,apnea hypoventilation index(AHI),the lowest arterial oxygen saturation( LSaO2),carotid IMT were measured in 39 adult patients with OSAHS.Among them,19 patients had been already got diagnosis of hypertension.Normal people with matched age,sex and body mass index (BMI)were chosen as control group.The differences of carotid IMT were evaluated.Results the carotid IMTs were signifincant increased in the moderate and severe adult OSAHS patients compared with mild patients and normal people(P0.01),and were more significant in the OSAHS patients with hypertension than those with pure OSAHS(P0.01).Conclusion vascular morphologic chang has already been existed in the moderate and severe adult OSAHS patients;the trend of which could be more significant after developing hypertension.
Correlation Between Carotid Intima-Media Roughness and Cardiovascular Risk Factors

Abstract:Arterial intima-media roughness (IMR) may indicate an early manifestation of atherosclerosis. To date, few studies have been performed to quantitatively evaluate carotid IMR by ultrasonography (US). The aim of the present study was to analyze the effect of cardiovascular risk factors on carotid IMR. A total of 185 subjects were enrolled for US examination of carotid arteries. Carotid intima-media thickness (IMT) and IMR were measured in US images by a novel automatic software. According to the number of combined high-risk factors for coronary heart disease, subjects were assigned to four groups (risk groups 0, 1, 2 and 3+). IMR was lowest in risk group 0 (32.9±2.7 µm), higher in risk group 1 (64.5±6.9 µm; P<0.01 vs. risk group 0) and highest in risk groups 2 and 3+ (89.1±7.4 and 92.0±6.7 µm, respectively; P<0.01 vs. risk groups 0 and 1). According to a multivariate regression analysis, age, systolic blood pressure, smoking status and the triglyceride/high-density lipoprotein cholesterol ratio were significant predictors of IMR. There was a progressive increase in carotid artery plaque with increasing tertiles of IMR (4.9, 33.9 and 53.2% in tertiles 1, 2 and 3, respectively; P<0.05). Compared with that of subjects in the lowest tertile of IMR, those in the highest tertile had a significantly elevated risk of the presence of plaque in the carotid tree (odds ratio=10.61, 95%CI: 2.15-52.49, P=0.004). Quantification of carotid IMR from US images with this software is feasible, and carotid IMR, which may help estimate the extent of atherosclerosis, may be used as a complementary factor to stratify patients with cardiovascular risk factors.

Abstract:ObjectivesTo investigate carotid intima‐media roughness (IMR) in hypertensive patients with normal carotid intima‐media thickness (IMT) using automatic identification software and the correlation between carotid IMR and risk factors.MethodsThis case‐control study comprised 61 hypertensive patients with normal carotid IMT and 51 control participants. Carotid IMR, carotid IMT, pulsed wave velocity (PWV), stiffness (β), and arterial compliance were determined by carotid ultrasound and image postprocessing using an automatic identification program and echo‐tracking analysis software.ResultsCarotid IMR, mean carotid IMT, maximum carotid IMT, β, and PWV in the hypertension group were higher than those in the control group (58.24 versus 34.61 μm, 641.17 versus 576.48 μm, 746.82 versus 640.55 μm, 9.42 versus 7.35, and 7.10 versus 5.86 m/s, respectively; P < .05), and arterial compliance was lower than that in the control group (0.70 versus 0.95 mm2/kPa; P < .05). Intima‐media roughness was correlated with maximum IMT, mean IMT, PWV, β, age, diagnosis of hypertension for greater than 1 year, and pulse pressure. Multivariate logistic regression showed that age, diagnosis of hypertension for greater than 1 year, and pulse pressure were influential factors for IMR in hypertensive patients, with odds ratios of 6.719 (95% confidence interval, 1.658–27.221; P = .008), 4.726 (95% confidence interval, 1.174–19.022; P = .029), and 3.998 (95% confidence interval, 1.033–15.466; P = .045), respectively.ConclusionsCarotid IMR and the elasticity index have important clinical importance in evaluating the risk of early atherosclerosis in hypertensive populations.

Abstract:Objective Carotid intima-media thickness (IMT) has received increasing attention as an important predictor of cardiovascular (CV) events. This study aims to investigate the association of IMT with severity of hypertension in Chinese adults. Methods This study includes a total of 886 hypertensive subjects (417 males; 469 females) aged 45–75 years, enrolled in two communities (Anqing and Lianyugang) in China. All the study subjects signed written informed consent and completed IMT measures using Mindray Diagnostic (Color) Ultrasound System DC-6 (made in Shenzhen, China) during 8/2008–6/2009. BP was analyzed as a continuous variable and a categorical variable (grades I, II, III). Univariate and multivariate analyses were performed, with adjustment for age, gender, BMI, and smoking. Results The mean+SD of IMT was 0.76+0.19 in males and 0.72+0.18 mm in females. Carotid plaque (IMT>1.3 mm) was found in 41% of males and 28% of females. Higher grade of BP was associated with greater values of IMT (P<0.05). The association appears to be in a dose–response fashion. Conclusion This is the first large-scale study of IMT in relation to severity of hypertension in community-based Chinese populations. We demonstrated that IMT was positively associated with severity of hypertension. A prospective study will help clarify if repeated measurements of IMT can be used as a sensitive indicator of anti-hypertension therapeutic efficacy and future CV events in the populations.

Abstract:The current study aims to check the relationship between parameters derived from brachial blood pressure, the carotid artery intima-media thickness (IMT), pulse wave velocity (PWV) and mean cerebral blood flow velocity (mCBFV) in the middle cerebral artery (MCA). In consecutive adult outpatients we recorded the brachial systolic blood pressure (SBP), diastolic blood pressure (DBP), pulse pressure (PP), mean arterial blood pressure (MAP), PWV and IMT. mCBFV was assessed using Doppler ultrasound probe applied to the transtemporal window. The mean±s.d. age of 165 patients (50% women) was 56.7±11.8 years. Women and men differed significantly in SBP, PP, total cholesterol and mCBFV. Age (r=−0.44, P<0.001) and BMI (r=−0.25, P<0.01) were significantly and reversely related to mCBFV. Compared with healthy individuals, hypertensive (P<0.05) and diabetic (P<0.01) patients had lower mCBFV. IMT and PWV were related to mCBFV (IMT, r=−0.36; P<0.001, and PWV, r=−0.34; P<0.001). After adjustment for possible confounders, the relationship between mCBFV and PWV did not retain statistical significance (P=0.54). However, the relationship between mCBFV and IMT remained statistically significant (P=0.02). The association between lower CBFV and higher IMT may constitute a link between increased IMT and risk of cerebrovascular events.

Abstract:Background: To quantify the association between effects of interventions on carotid intima-media thickness (cIMT) progression and their effects on cardiovascular disease (CVD) risk. Methods: We systematically collated data from randomized, controlled trials. cIMT was assessed as the mean value at the common-carotid-artery; if unavailable, the maximum value at the common-carotid-artery or other cIMT measures were used. The primary outcome was a combined CVD end point defined as myocardial infarction, stroke, revascularization procedures, or fatal CVD. We estimated intervention effects on cIMT progression and incident CVD for each trial, before relating the 2 using a Bayesian meta-regression approach. Results: We analyzed data of 119 randomized, controlled trials involving 100 667 patients (mean age 62 years, 42% female). Over an average follow-up of 3.7 years, 12 038 patients developed the combined CVD end point. Across all interventions, each 10 mu m/y reduction of cIMT progression resulted in a relative risk for CVD of 0.91 (95% Credible Interval, 0.87-0.94), with an additional relative risk for CVD of 0.92 (0.87-0.97) being achieved independent of cIMT progression. Taken together, we estimated that interventions reducing cIMT progression by 10, 20, 30, or 40 mu m/y would yield relative risks of 0.84 (0.75-0.93), 0.76 (0.67-0.85), 0.69 (0.59-0.79), or 0.63 (0.52-0.74), respectively. Results were similar when grouping trials by type of intervention, time of conduct, time to ultrasound follow-up, availability of individual-participant data, primary versus secondary prevention trials, type of cIMT measurement, and proportion of female patients. Conclusions: The extent of intervention effects on cIMT progression predicted the degree of CVD risk reduction. This provides a missing link supporting the usefulness of cIMT progression as a surrogate marker for CVD risk in clinical trials.
Impact Of Multiple Cardiovascular Risk Factors On Carotid Intima-Media Thickness And Elasticity

Abstract:Background: Carotid intima-media thickness (IMT) and elasticity have been shown to be independent predictors of cardiovascular disease (CVD). Cardiovascular risk factors (CVRFs) includes hypertension, dyslipidemia, diabetes, overweight and smoking. The objective was to investigate whether the clustering of three or more components of CVRFs has a greater impact on carotid IMT and elasticity than individual components of CVRFs.Methods: One hundred and seventy-three participants without clinical CVD were classified as the multiple CVRFs patients with three or more CVRFs (n = 55) and control group with two or less CVRFs (n = 118). Carotid IMT and elastic modulus were measured by B-mode ultrasound and vessel texture matching method (VTMM), respectively.Results: The multiple CVRFs conferred a disproportionate increase in carotid IMT (43%, p<0.0001) and elastic modulus (60%, p<0.0001), compared with control group. Multiple regression models, which included age, gender, as well as each individual component of CVRFs as continuous variables, showed that multiple CVRFs was an independent determinant of both IMT (p = 0.042) and elasticity (p = 0.008). In the analysis of variance adjusted with age, subjects with single, double, and multiple CVRFs, increased by 8.1%, 42.2%, and 66% for IMT, 54.6%, 94.3%, and 125.2% for elastic modulus, respectively, compared to subjects without CVRFs.Conclusions: The clustering of multiple CVRFs has a greater impact on carotid IMT and elasticity than individual components of CVRFs. This suggests that the components of CVRFs interact to synergistically impact carotid IMT and elasticity.
Associations of Plasma Glucose Levels and Traits of Metabolic Syndrome with Carotid Intima Media Thickness in Nondiabetic Elderly Subjects: Are They Mediated by Insulin Resistance?

Abstract:BACKGROUND: The independent role of insulin resistance (IR) and high fasting blood glucose (FBG) levels within the normal range on vascular diseases is still under debate. This study was designed to explore whether IR, FBG levels, and the traits of metabolic syndrome are associated with increased carotid intima media thickness (IMT), the early marker of subclinical atherosclerosis, independently of each other in nondiabetic elderly subjects.METHODS: Blood analytes and anthropometric measurements were obtained. Carotid IMT was measured by ultrasonography; metabolic syndrome was diagnosed according to National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III) criteria. IR was assessed through homeostasis model assessment of insulin resistance (HOMA-IR).RESULTS: At total of 207 subjects aged 68.2±3.6 years were enrolled. Subjects with increased carotid IMT (50.7%) were older (P=0.001), had a higher prevalence of metabolic syndrome (P<0.0001) and all its traits, impaired fasting glucose (IFG) (P<0.0001), and values of HOMA-IR (P<0.0001) than normal subjects. Increased carotid IMT significantly correlated with metabolic syndrome, its traits, IFG, and HOMA-IR. When multivariable regression models were constructed, central obesity [B=0.392; 95% confidence interval (CI) 0.280-0.505; P<0.0001], high-density lipoprotein cholesterol (HDL-C) (B=-0.007; 95% CI -0.013-0.000; P=0.042], hypertension (B=0.475; 95% CI 0.363-0.587; P<0.0001), and IFG (B=0.230; 95% CI 0.092-0.367; P=0.001) were found to be the independent determinants of increased carotid IMT independently of HOMA-IR, but not FBG (B=0.013; 95% CI 0.000-0.026; P=0.050) and HOMA-IR itself.CONCLUSIONS: Our results suggest that hypertension, low HDL-C, and central obesity are independently associated with increased carotid IMT in nondiabetic elderly subjects. These associations seem to be not affected by IR. The associations of FBG levels within the normal range and IR with carotid IMT should be investigated further.

Abstract:Introduction: There is strong evidence of an association between systemic inflammation and ischemic stroke. While the exact mechanism underpinning that association is unclear, there is evidence that systemic inflammation contributes to atherosclerosis progression. One measure of subclinical atherosclerosis, carotid artery intima-media thickness (CIMT), is independently associated with ischemic stroke. Because of mixed data on the association between systemic inflammation and CIMT progression, we sought to evaluate this relationship in a prospective cohort study. Hypothesis: Baseline elevation in circulating markers of inflammation are associated with CIMT progression in the Framingham Heart Study cohort. Methods: Among participants with two carotid ultrasounds (n=2350) about 10 years apart, 2,269 had data available on circulating markers of inflammation. Multivariable linear regression was used to assess the association between circulating markers of inflammation with internal carotid artery (ICA) CIMT. Analyses were adjusted for age, sex, hypertension, diabetes, APOE4, and body mass index (BMI), and baseline IMT. Results: Participants with higher baseline levels of C-reactive protein (CRP), interleukin-6 (IL-6), intracellular adhesion molecule, p-selectin, lipoprotein-associated phospholipase A2 (LPa) activity, homocysteine, isoprostanes (ISO), and CD40 ligand had significantly higher rates of ICA IMT progression adjusting for age, sex, hypertension, diabetes, APOE4, BMI, and baseline ICA IMT (Table). Participants with higher levels of CRP, IL-6, OPG, p-selectin, LPa activity, and ISO had greater follow-up ICA IMT adjusting for age, sex, hypertension, diabetes, APOE4, and BMI. Conclusion: Specific markers of inflammation are associated with increased later life carotid intima media thickening and carotid atherosclerosis progression, supporting a link between systemic inflammation and cerebrovascular disease.

Associations between Multiple Health Indicators and Carotid Artery Intima-Media Thickness in A Healthy and Active Elderly Population

Abstract:The purpose of this study was to examine correlations between health indicators (age, BMI, blood pressure (BP), functional strength (FS), handgrip strength, and predicted VO2 max) and carotid intima-media thickness (cIMT) in an active 50 years+ population. Study participants’ mean cIMT was also compared to the cIMT mean of the general population. Health screenings were conducted on 1818 participants at the Huntsman World Senior Games from 2016 to 2019. Pearson’s correlations, Spearman’s correlations, and ANOVA were performed using SPSS. Weak but significant correlations were evident between cIMT and age (r = 0.283, p < 0.001), systolic BP (r = 0.253, p = 0.001), diastolic BP (r = 0.074, p = 0.016), weight (r = 0.170, p < 0.001), height (r = 0.153, p < 0.001), handgrip L (r = 0.132, p < 0.001), handgrip R (r = 0.074, p < 0.029), and BMI (r = 0.07, p = 0.029); non-significant correlations were evident with predicted VO2 max (r = −0.035, p = 0.382), and FS (r = −0.025, p = 0.597). When controlling for age, systolic BP, and sex, only handgrip L (r = 0.225, p = 0.014) was significantly correlated with cIMT. Mean cIMT for this cohort was lower across all sexes and age-matched groups (cIMT = 0.6967 mm (±0.129)). Physical activity is linked to reduced cIMT. Most health-related indicators in this study were significantly but weakly correlated with cIMT. Additional research is needed before common indicators can be used as a surrogate for cIMT and CVD risk. Results from this study can provide clinicians with additional information to reduce CVD risk through modifiable risk factors. Classic CVD risk factors such as systolic BP and BMI should be considered in patients regardless of lifestyle.

Abstract:Job Stress and Carotid Intima-media Thickness in Chinese Workers: Weixian Xu, et al. Department of Cardiology, Peking University Third Hospital and Key Laboratory of Molecular Cardiovascular Science, Ministry of Education, China Objectives: Carotid intima-media thickness (CIMT) can be used as a surrogate marker for cardiovascular health, and job stress is a risk factor for cardiovascular disease. However, there have been a limited number of studies focusing on the association between job stress and CIMT. The goal of this study was to explore the association between job stress and CIMT in a Chinese working population. Methods: The study included 734 participants (508 males and 226 females) without coronary heart disease. Job stress was evaluated using the effort-reward imbalance (ERI) questionnaire at work. ERI is the ratio between efforts and rewards (weighted by number of items). High resolution carotid ultrasonographic studies were performed using a Sequoia 512 ultrasound system with an 8-13 MHz linear array transducer to assess CIMT. Results: This study detected gender-specific associations between the indictors of the ERI model and increased CIMT among the study participants in China. This study demonstrated a robust association in women between the key indicators of ERI, effort, overcommitment and ERI, and increased CIMT (adjusted r(2)=0.258, p=0.001; adjusted r(2)=0.261; p<0.001; adjusted r(2)=0.274; p<0.001, respectively). Reward was inversely correlated with CIMT (adjusted r(2)=0.282, p<0.001), controlling for age, hypertension, diabetes mellitus, hyperlipidaemia and body mass index. For men, a similar pattern of associations was observed, but the associations were lost after adjustment for confounders. Conclusions: Our resultsshow that effort, overcommitment and ERI may be associated with early atherosclerosis predicted by CIMT in women, and reward is inversely related to CIMT. Value of Carotid Intima-Media Thickness Assessment for Cardiocerebral Vascular Events in Elderly Subjects

Abstract:Objective To study the effect of gender on carotid intima-media thickness(IMT)and its relation with cardiocerebral vascular events in elderly subjects.Methods One hundred and forty- seven elderly patients(71 males and 76 females) were enrolled in this study.Their body height and weight,blood pressure(BP),blood lipid level,and carotid IMT were measured.Variations in male and female subjects were compared.Relation of carotid IMT with myocardiac infarction(MI) and cerebral stroke was analyzed.Results The carotid IMT was thicker in females with their age≥80 years than in females with their age 80 years(0.93 mm vs 0.80 mm,P0.05).However,no significant difference was found in carotid IMT of males with their age≥80 or 80 years(P 0.05).Logistic regression analysis showed that the variations of gender,age,TC,LDL-C,and HDL-C were positively related with MI(P0.05),and that the carotid IMT was positively related with cerebral stroke(P0.05).Conclusion The gender and age are related with MI,while the carotid IMT is related with cerebral stroke in elderly subjects.
The relationship between heart-carotid pulse transit time and carotid intima-media thickness in hypertensive patients

Abstract:The study aimed to investigate the relationship between heart-carotid pulse transit time and carotid intima-media thickness (CIMT) in hypertensive patients, and whether including the pre-ejection period (PEP) in heart-carotid pulse transit time would affect this correlation. A total of 62 hypertensive patients were included in this study. They were divided into the normal CIMT group (n=33, CIMT⩽0.8 mm) and the thickened CIMT group (n=29, CIMT>0.8 mm). The noninvasive ultrasound method was used to measure CIMT, electrocardiogram R-wave-based heart-carotid pulse transit time (rcPTT) and PEP. Aortic valve-carotid artery pulse transit time (acPTT) was calculated by subtracting PEP from rcPTT. Simple linear analysis showed that CIMT was negatively associated with rcPTT and acPTT (r=-0.57, P<0.0001; r=-0.41, P=0.016) in the normal CIMT group as well as in the thickened CIMT group (r=-0.50, P=0.0053; r=-0.59, P=0.001). These relationships were eliminated in the normal CIMT group after adjusting for age, gender, smoking behaviour, systolic blood pressure, diastolic blood pressure and cholesterol levels. However, rcPTT and acPTT still showed significant correlations with CIMT in the thickened CIMT group. In conclusion, rcPTT and acPTT were associated with CIMT, independent of well-known clinical confounders in thickened CIMT hypertensive patients. Therefore, rcPTT and acPTT might be useful markers for atherosclerosis evaluation.
The Relationship between Subclinical Hypothyroidism and Carotid Intima-Media Thickness as a Potential Marker of Cardiovascular Risk: A Systematic Review and a Meta-Analysis

Abstract:Background and Objectives: Thyroid dysfunction is known to have significant consequences on the cardiovascular system. The correlation between carotid intima-media thickness (CIMT) and subclinical hypothyroidism (SCH) has been frequently evaluated in clinical studies in recent years. This study aimed to evaluate the significance of this association through a meta-analysis. Methods: We conducted a systematic search of PubMed, MedLine, Scopus, and Web of Science databases using the keywords ‘subclinical hypothyroidism and carotid intima-media thickness’, from the beginning of each database until January 2023. We established the inclusion and exclusion criteria and considered studies that met the inclusion criteria. We used Jamovi for statistical analysis of the data. Results: We identified 39 observational studies that met the inclusion criteria, with 3430 subjects: 1545 SCH and 1885 EU. Compared to euthyroid subjects (EU), subjects with subclinical hypothyroidism (SCH) had significantly increased carotid intima-media thickness (CIMT) values; the estimated average mean difference was 0.08 (95% CI 0.05 to 0.10), p < 0.01, I2 = 93.82%. After the sensitivity analysis, a total of 19 from the 39 abovementioned studies were analyzed, with most studies showing a positive association between SCH and thickening of the carotid wall; the estimated average mean difference was 0.04 (95% CI 0.02 to 0.07), p = 0.03, I2 = 77.7. In addition, female sex, advanced age, and high cholesterol levels statistically significantly influenced this association. Conclusions: Our meta-analysis indicates a significant positive association between SCH and increased CIMT, but with some limitations.

Risk Factors Associated with Cardiac Autonomic Modulation in Obese Individuals

Abstract:Obesity leads to an imbalance in the autonomic nervous system, especially in increased sympathetic modulation and decreased vagal tone, and some anthropometric, metabolic, and lifestyle variables may increase the risk of developing cardiovascular disease. Objective . To analyze the association between cardiovascular autonomic modulation and biochemical and anthropometric markers, food intake, and physical activity level in severely obese individuals. Methodology . The present study is a cutout of a randomized clinical trial “Effect of nutritional intervention and olive oil in severe obesity” (DieTBra Trial), where the baseline data were analyzed. Anthropometric data, biochemical exams, heart rate variability (HRV), accelerometry, and 24 h recall (R24H) of obese patients (body mass index BMI ≥35 kg/m 2 ) were collected. Results . 64 obese patients were analyzed, with a mean age of 39.10 ± 7.74 years (27 to 58 years). By HRV analysis, in the frequency domain, the obese had a higher predominance of sympathetic autonomic modulation (low frequency (LF) 56.44 ± 20.31 nu) and lower parasympathetic modulation (high frequency (HF) 42.52 ± 19.18 nu). A negative association was observed between the variables Homeostasis Evaluation Model (HOMA-IR) and HF ( p = 0.049 ). In the physical activity analysis, there was a negative association between moderate to vigorous physical activity and the sympathetic component ( p = 0.043 ), and for sedentary time (ST), there was a negative association with HF ( p = 0.049 ) and LF/HF ( p = 0.036 ) and a positive association with LF ( p = 0.014 ). For multiple linear regression, waist circumference (WC) and HOMA-IR values were negatively associated with HF ( β = −0.685, p = 0.010 ; β = −14.989, p = 0.010 ; respectively). HOMA-IR ( β = 0.141, p = 0.003 ) and the percentage of lipids ingested ( β = −0.030, p = 0.043 ) were negatively associated with LF/HF. Conclusion . Among the cardiovascular risk variables studied, insulin resistance and central adiposity showed the greatest influence on cardiac autonomic modulation of obese, increasing the risk for cardiovascular disease.
Early life factors and their relevance to intima-media thickness of the common carotid artery in early adulthood

Abstract:Early life factors may predispose an offspring to cardiovascular disease in later life; relevance of these associations may extend to "healthy" people in Western populations. We examined the prospective associations between early life factors and adult carotid intima-media thickness (IMT), a surrogate marker of atherosclerosis, in a healthy German population.We studied term participants (n = 265) of the DONALD Study, with bilateral sonographic measurements of IMT (4–8 measurements on both left and right carotid artery) at age 18–40 years and prospectively collected data on early life factors (maternal and paternal age at child birth, birth weight, gestational weight gain and full breastfeeding (>17weeks). Mean IMT values were averaged from mean values of both sides. Associations between early life factors and adult IMT were analyzed using multivariable linear regression models with adjustment for potential confounders.Adult mean IMT was 0.56mm, SD 0.03, (range: 0.41 mm-0.78 mm). Maternal age at child birth was of relevance for adult IMT, which was sex specific: Advanced maternal age at child birth was associated with an increased adult IMT among female offspring only (β 0.03, SE 0.009 mm/decade, P = 0.003), this was not affected by adult waist circumference, BMI or blood pressure. Other early life factors were not relevant for IMT levels in males and females.This study suggests that advanced maternal age at child birth is of prospective relevance for adult IMT levels in a healthy German population and this association may be of adverse relevance for females only.

Abstract:Objective: Atherosclerosis is a chronic and systemic disease and its developmental process involves the synergism of multiple risk factors such as hypertension, dyslipidemia, diabetes, obesity and smoking. The diagnosis of subclinical atherosclerosis is essential for strategic guidance towards suitable treatments and efficient prevention against acute cardiovascular events. This study employed ultrasound radiofrequency (RF) tracking technology to characterize human carotid arteries in vivo in terms of intima-media thickness (IMT) and artery stiffness, and evaluated the statistical correlation between carotid IMT and stiffness, and the number of risk factors for atherosclerosis.Methods: A total of 160 asymptomatic subjects were enrolled. Ultrasound RF-tracking technology was employed to acquire carotid IMT and stiffness parameters: maximum IMT ((MAX) IMT), RF Quality IMT ((RF) QIMT), distensibility coefficient (DC), compliance coefficient (CC), alpha index, beta index and local pulse wave velocity (PWV beta). The subjects were categorized in four groups in terms of the number of risk factors: 'zero', 'single', 'double', and 'multiple', and statistical analyses of carotid IMT and stiffness parameters were performed between these different groups.Results: The subjects (n = 145) with (IMT)-I-MAX smaller than 1.0 mm matched the IMT criteria for non-atherosclerosis and were named as NA-subjects. Spearman's rho correlation analysis of the whole group and the NA-subjects both showed that (IMT)-I-MAX correlated positively with (RF)QIMT, alpha, beta, and PWV beta, and negatively with DC and CC (p<0.01). The analysis of covariance of NA-subjects showed significant differences between subjects with and without risk factors, and also showed significant differences between the 'zero', 'single', 'double', and 'multiple' groups.Conclusions: The carotid IMT and stiffness parameters obtained by the ultrasound RF-tracking technology were demonstrated to possess significant statistical correlation with the number of risk factors from 160 subjects, and these anatomical and mechanical parameters may potentially be used together with the IMT criteria to support subclinical atherosclerosis diagnosis.
