Abstract:Recent advances in medical image analysis have been made to improve our understanding of how disease develops, behaves, and responds to treatment. Magnetic resonance imaging (MRI) and positron emission tomography (PET) advanced imaging strategies provide structural and functional phenotypic biomarkers that correlate with key disease processes. Through radiomics and radiogenomics, ML-medical imaging has opened up new perspectives in high-grade glioma diagnosis. As a result, non-invasive and in vivo biomarkers for patient survival, tumor recurrence, and genomics are identified. Tumor genomic imaging signatures can help identify patients who benefit from targeted therapies. Molecular characterization of gliomas and prediction of their evolution would allow treatment optimization. Radiomics-based biomarkers allow for a more in-depth analysis of pathophysiologic processes and insights into diagnosing better, classifying, stratifying, and prognosticating brain tumors and assessing their response to therapy. Radiomics is a new data-driven approach that can help answer clinical questions like diagnosis, prognosis, and treatment response. With encouraging outcomes in brain tumor patients, radiomics and deep learning are still not widely used in clinical practice, requiring more extensive and practical clinical studies.

Abstract:Recently, the concept of radiomics has emerged from radiation oncology. It is a novel approach for solving the issues of precision medicine and how it can be performed, based on multimodality medical images that are non-invasive, fast and low in cost. Radiomics is the comprehensive analysis of massive numbers of medical images in order to extract a large number of phenotypic features (radiomic biomarkers) reflecting cancer traits, and it explores the associations between the features and patients' prognoses in order to improve decision-making in precision medicine. Individual patients can be stratified into subtypes based on radiomic biomarkers that contain information about cancer traits that determine the patient's prognosis. Machine-learning algorithms of AI are boosting the powers of radiomics for prediction of prognoses or factors associated with treatment strategies, such as survival time, recurrence, adverse events, and subtypes. Therefore, radiomic approaches, in combination with AI, may potentially enable practical use of precision medicine in radiation therapy by predicting outcomes and toxicity for individual patients.

Abstract:Recent technological advances in the field of artificial intelligence hold promise in addressing medical challenges in breast cancer care, such as early diagnosis, cancer subtype determination and molecular profiling, prediction of lymph node metastases, and prognostication of treatment response and probability of recurrence. Radiomics is a quantitative approach to medical imaging, which aims to enhance the existing data available to clinicians by means of advanced mathematical analysis using artificial intelligence. Various published studies from different fields in imaging have highlighted the potential of radiomics to enhance clinical decision making. In this review, we describe the evolution of AI in breast imaging and its frontiers, focusing on handcrafted and deep learning radiomics. We present a typical workflow of a radiomics analysis and a practical “how-to” guide. Finally, we summarize the methodology and implementation of radiomics in breast cancer, based on the most recent scientific literature to help researchers and clinicians gain fundamental knowledge of this emerging technology. Alongside this, we discuss the current limitations of radiomics and challenges of integration into clinical practice with conceptual consistency, data curation, technical reproducibility, adequate accuracy, and clinical translation. The incorporation of radiomics with clinical, histopathological, and genomic information will enable physicians to move forward to a higher level of personalized management of patients with breast cancer.

Abstract:With a high rate of morbidity and mortality, colorectal cancer (CRC) ranks third in mortality among cancers. By analyzing the texture properties of images and quantifying the heterogeneity of tumors, radiomics and radiogenomics are non-invasive methods to determine the biological properties of CRC. Recently, several articles have discussed the application of radiomics in different aspects of CRC. Therefore, given the large amount of data published, this review aims to discuss how radiomics can be used for distinguishing benign and malignant colorectal lesions, diagnosing, staging, predicting prognosis and treatment response, and predicting lymph node and hepatic metastasis of CRC, based on radiomic features extracted from magnetic resonance imaging (MRI), computed tomography (CT), esophageal ultrasonography (EUS), and positron emission tomography-CT (PET-CT). Challenges in bringing radiomics to clinical application and future solutions have also been discussed. With the progress made in radiomics and the application of deep and machine learning in this area, radiomics can become one of the main tools for the personalized management of CRC patients shortly.

Abstract:Artificial intelligence (AI) has increasingly been serving the field of radiology over the last 50 years. As modern medicine is evolving towards precision medicine, offering personalized patient care and treatment, the requirement for robust imaging biomarkers has gradually increased. Radiomics, a specific method generating high-throughput extraction of a tremendous amount of quantitative imaging data using data-characterization algorithms, has shown great potential in individuating imaging biomarkers. Radiomic analysis can be implemented through the following two methods: hand-crafted radiomic features extraction or deep learning algorithm. Its application in lung diseases can be used in clinical decision support systems, regarding its ability to develop descriptive and predictive models in many respiratory pathologies. The aim of this article is to review the recent literature on the topic, and briefly summarize the interest of radiomics in chest Computed Tomography (CT) and its pertinence in the field of pulmonary diseases, from a clinician's perspective.

Abstract:Medical imaging is moving toward automated systems and approaches capable of assisting and supporting medical decisions. For more than a decade, radiomics and artificial intelligence (AI) approaches have been widely explored. In particular, in oncology a considerable number of studies reported on biomarkers derived from computed tomography (CT), magnetic resonance imaging (MRI), and positron emission tomography (PET). Lack of standardization and robust study design, variability in methodology, and incomplete reporting limit the comparison of results across studies and, consequently, clinical translation. In the present chapter, we describe the methodological steps of radiomics and AI-methods applied to imaging. Furthermore, the main clinical applications will be illustrated.

Abstract:Medical imaging is the standard-of-care for early detection, diagnosis, treatment planning, monitoring, and image-guided interventions of lung cancer patients. Most medical images are stored digitally in a standardized Digital Imaging and Communications in Medicine format that can be readily accessed and used for qualitative and quantitative analysis. Over the several last decades, medical images have been shown to contain complementary and interchangeable data orthogonal to other sources such as pathology, hematology, genomics, and/or proteomics. As such, "radiomics" has emerged as a field of research that involves the process of converting standard-of-care images into quantitative image-based data that can be merged with other data sources and subsequently analyzed using conventional biostatistics or artificial intelligence (AI) methods. As radiomic features capture biological and pathophysiological information, these quantitative radiomic features have shown to provide rapid and accurate noninvasive biomarkers for lung cancer risk prediction, diagnostics, prognosis, treatment response monitoring, and tumor biology. In this review, radiomics and emerging AI methods in lung cancer research are highlighted and discussed including advantages, challenges, and pitfalls.

Abstract:Abstract The recent, rapid advances in immuno-oncology have revolutionized cancer treatment and spurred further research into tumor biology. Yet, cancer patients respond variably to immunotherapy despite mounting evidence to support its efficacy. Current methods for predicting immunotherapy response are unreliable, as these tests cannot fully account for tumor heterogeneity and microenvironment. An improved method for predicting response to immunotherapy is needed. Recent studies have proposed radiomics—the process of converting medical images into quantitative data (features) that can be processed using machine learning algorithms to identify complex patterns and trends—for predicting response to immunotherapy. Because patients undergo numerous imaging procedures throughout the course of the disease, there exists a wealth of radiological imaging data available for training radiomics models. And because radiomic features reflect cancer biology, such as tumor heterogeneity and microenvironment, these models have enormous potential to predict immunotherapy response more accurately than current methods. Models trained on preexisting biomarkers and/or clinical outcomes have demonstrated potential to improve patient stratification and treatment outcomes. In this review, we discuss current applications of radiomics in oncology, followed by a discussion on recent studies that use radiomics to predict immunotherapy response and toxicity.

Abstract:Background Radiomics is a rapidly growing field used to leverage medical radiological images by extracting quantitative features. These are supposed to characterize a patient's phenotype, and when combined with artificial intelligence techniques, to improve the accuracy of diagnostic models and clinical outcome prediction. Objectives This review aims at examining the application areas of artificial intelligence-based radiomics (AI-based radiomics) for the management of head and neck cancer (HNC). It further explores the workflow of AI-based radiomics for personalized and precision oncology in HNC. Finally, it examines the current challenges of AI-based radiomics in daily clinical oncology and offers possible solutions to these challenges. Methods Comprehensive electronic databases (PubMed, Medline via Ovid, Scopus, Web of Science, CINAHL, and Cochrane Library) were searched following the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. The quality of included studies and their risk of biases were evaluated using the Transparent Reporting of a Multivariable Prediction Model for Individual Prognosis or Diagnosis (TRIPOD) and Prediction Model Risk of Bias Assessment Tool (PROBAST). Results Out of the 659 search hits retrieved, 45 fulfilled the inclusion criteria. Our review revealed that the application of AI-based radiomics model as an ancillary tool for improved decision-making in HNC management includes radiomics-based cancer diagnosis and radiomics-based cancer prognosis. The radiomics-based cancer diagnosis includes tumor staging, tumor grading, and classification of malignant and benign tumors. Similarly, radiomics-based cancer prognosis includes prediction for treatment response, recurrence, metastasis, and survival. In addition, the challenges in the implementation of these models for clinical evaluations include data imbalance, feature engineering (extraction and selection), model generalizability, multi-modal fusion, and model interpretability. Conclusion Considering the highly subjective and interobserver variability that is peculiar to the interpretation of medical images by expert clinicians, AI-based radiomics seeks to offer potentially useful quantitative information, which is not visible to the human eye or unintentionally often remain ignored during clinical imaging practice. By enabling the extraction of this type of information, AI-based radiomics has the potential to revolutionize HNC oncology, providing a platform for more personalized, higher quality, and cost-effective care for HNC patients.

Abstract:Breast cancer is the most common malignancy in women, and some subtypes are associated with a poor prognosis with a lack of efficacious therapy. Moreover, immunotherapy and the use of other novel antibody‒drug conjugates have been rapidly incorporated into the standard management of advanced breast cancer. To extract more benefit from these therapies, clarifying and monitoring the tumor microenvironment (TME) status is critical, but this is difficult to accomplish based on conventional approaches. Radiomics is a method wherein radiological image features are comprehensively collected and assessed to build connections with disease diagnosis, prognosis, therapy efficacy, the TME, etc In recent years, studies focused on predicting the TME using radiomics have increasingly emerged, most of which demonstrate meaningful results and show better capability than conventional methods in some aspects. Beyond predicting tumor-infiltrating lymphocytes, immunophenotypes, cytokines, infiltrating inflammatory factors, and other stromal components, radiomic models have the potential to provide a completely new approach to deciphering the TME and facilitating tumor management by physicians.

Abstract:Medical imaging techniques, such as mammography, ultrasound and magnetic resonance imaging, plays an integral role in the detection and characterization of breast cancer. Although computers are believed to gain an important role in the assessment of medical images for breast evaluation for at least two decades, their impact on performance has not lived up to expectations yet. With the continuous and rapid development of computer science, artificial intelligence (AI) approaches, like machine learning and deep learning, have been introduced for the analysis of medical images. Because of the remarkable advances in data extraction and analysis in medical imaging compared to conventional feature-based techniques, AI has reignited the interest in automated breast image interpretation. Extensive research is conducted on accurate detection and classification of breast lesions, and more specifically, the predictive and prognostic features of breast cancer by radiomics. Radiomics exploits the fact that image data is nowadays numerical and can also be used to generate quantitative biomarkers. In this comprehensive review, we cover the progress, application and challenge of radiomics and AI in breast cancer diagnosis in recent years, as well as the impact and significance of AI on future breast cancer research.

Abstract:Radiomics refers to the extraction of mineable data from medical imaging and has been applied within oncology to improve diagnosis, prognostication, and clinical decision support, with the goal of delivering precision medicine. The authors provide a practical approach for successfully implementing a radiomic workflow from planning and conceptualization through manuscript writing. Applications in oncology typically are either classification tasks that involve computing the probability of a sample belonging to a category, such as benign versus malignant, or prediction of clinical events with a time-to-event analysis, such as overall survival. The radiomic workflow is multidisciplinary, involving radiologists and data and imaging scientists, and follows a stepwise process involving tumor segmentation, image preprocessing, feature extraction, model development, and validation. Images are curated and processed before segmentation, which can be performed on tumors, tumor subregions, or peritumoral zones. Extracted features typically describe the distribution of signal intensities and spatial relationship of pixels within a region of interest. To improve model performance and reduce overfitting, redundant and nonreproducible features are removed. Validation is essential to estimate model performance in new data and can be performed iteratively on samples of the dataset (cross-validation) or on a separate hold-out dataset by using internal or external data. A variety of noncommercial and commercial radiomic software applications can be used. Guidelines and artificial intelligence checklists are useful when planning and writing up radiomic studies. Abstract:Abstract Over the past years, the quantity and complexity of imaging data available for the clinical management of patients with solid tumors has increased substantially. Without the support of methods from the field of artificial intelligence (AI) and machine learning, a complete evaluation of the available image information is hardly feasible in clinical routine. Especially in radiotherapy planning, manual detection and segmentation of lesions is laborious, time consuming, and shows significant variability among observers. Here, AI already offers techniques to support radiation oncologists, whereby ultimately, the productivity and the quality are increased, potentially leading to an improved patient outcome. Besides detection and segmentation of lesions, AI allows the extraction of a vast number of quantitative imaging features from structural or functional imaging data that are typically not accessible by means of human perception. These features can be used alone or in combination with other clinical parameters to generate mathematical models that allow, for example, prediction of the response to radiotherapy. Within the large field of AI, radiomics is the subdiscipline that deals with the extraction of quantitative image features as well as the generation of predictive or prognostic mathematical models. This review gives an overview of the basics, methods, and limitations of radiomics, with a focus on patients with brain tumors treated by radiation therapy.

Abstract:Lung cancer is responsible for more fatalities than any other cancer worldwide, with 1.76 million associated deaths reported in 2018. The key issue in the fight against this disease is the detection and diagnosis of all pulmonary nodules at an early stage. Artificial intelligence (AI) algorithms play a vital role in the automated detection, segmentation, and computer-aided diagnosis of malignant lesions. Among the existing algorithms, radiomics and deep-learning-based types appear to show the most promise. Radiomics is a growing field related to the extraction of a set of features from an image, which allows for automated classification of medical images into a predefined group. The process comprises a series of consecutive steps including image acquisition and pre-processing, segmentation of the desired region of interest, calculation of defined features, feature engineering, and construction of the classification model. The features calculated in this process are mainly shape features, as well as first- and higher-order texture features. To date, more than 100 features have been defined, although this number varies depending on the application. The greatest challenge in radiomics is building a cross-validated model based on a selected set of calculated features known as the radiomic signature. Numerous radiomic signatures have successfully been developed; however, reproducibility and clinical validity of the results obtained constitutes a considerable challenge of modern radiomics. Deep learning algorithms are another rapidly evolving technique and are recognized as a valuable tool in the field of medical image analysis for the detection, characterization, and assessment of lesions. Such an approach involves the design of artificial neural network architecture while upholding the goal of high classification accuracy. This paper illuminates the evolution and current state of artificial intelligence methods in lung imaging and the detection and diagnosis of pulmonary nodules, with a particular emphasis on radiomics and deep learning methods.

Abstract:Radiomics is the extraction of predefined mathematic features from medical images for predicting variables of clinical interest. Recent research has demonstrated that radiomics can be processed by artificial intelligence algorithms to reveal complex patterns and trends for diagnosis, and prediction of prognosis and response to treatment modalities in various types of cancer. Artificial intelligence tools can utilize radiological images to solve next-generation issues in clinical decision making. Bone tumors can be classified as primary and secondary (metastatic) tumors. Osteosarcoma, Ewing sarcoma, and chondrosarcoma are the dominating primary tumors of bone. The development of bone tumor model systems and relevant research, and the assessment of novel treatment methods are ongoing to improve clinical outcomes, notably for patients with metastases. Artificial intelligence and radiomics have been utilized in almost full spectrum of clinical care of bone tumors. Radiomics models have achieved excellent performance in the diagnosis and grading of bone tumors. Furthermore, the models enable to predict overall survival, metastases, and recurrence. Radiomics features have exhibited promise in assisting therapeutic planning and evaluation, especially neoadjuvant chemotherapy. This review provides an overview of the evolution and opportunities for artificial intelligence in imaging, with a focus on hand-crafted features and deep learning-based radiomics approaches. We summarize the current application of artificial intelligence-based radiomics both in primary and metastatic bone tumors, and discuss the limitations and future opportunities of artificial intelligence-based radiomics in this field. In the era of personalized medicine, our in-depth understanding of emerging artificial intelligence-based radiomics approaches will bring innovative solutions to bone tumors and achieve clinical application.

Abstract:Artificial intelligence (AI) techniques have significant potential to enable effective, robust and automated image phenotyping including identification of subtle patterns. AI-based detection searches the image space to find the regions of interest based on patterns and features. There is a spectrum of tumor histologies from benign to malignant that can be identified by AI-based classification approaches using image features. The extraction of minable information from images gives way to the field of radiomics and can be explored via explicit (handcrafted/engineered) and deep radiomics frameworks. Radiomics analysis has the potential to be utilized as a noninvasive technique for the accurate characterization of tumors to improve diagnosis and treatment monitoring. This work reviews AI-based techniques, with a special focus on oncological PET and PET/CT imaging, for different detection, classification, and prediction/prognosis tasks. We also discuss needed efforts to enable the translation of AI techniques to routine clinical workflows, and potential improvements and complementary techniques such as the use of natural language processing on electronic health records and neuro-symbolic AI techniques.

Abstract:Radiomics, a quantitative approach to medical imaging, employs computational methods to extract features from the images, revealing hidden characteristics of specific regions. This emerging field leverages advanced techniques to analyze a spectrum of features from modalities, including computed tomography (CT), magnetic resonance imaging (MRI), and positron emission tomography (PET) scans, aiming to decode tissue characteristics, disease progression, and treatment responses. The radiomics workflow integrates image acquisition, segmentation, feature selection, and data integration, utilizing advanced techniques such as deep learning, machine learning, and data mining. Radiomics demonstrates considerable potential in cancer detection and management, exhibiting high sensitivity and specificity in distinguishing between benign and malignant tumors and predicting outcomes. However, challenges such as imaging protocol variability, overfitting, and standardization requirements impede its broad clinical adoption. Innovations in multi-modal radiomics, deep learning, and genomics integration strive to mitigate these constraints. This review elucidates radiomics’ capabilities, current applications, benefits, challenges, and future directions in oncology.
Radiomics artificial intelligence modelling for prediction of local control for colorectal liver metastases treated with radiotherapy

Physics and Imaging in Radiation Oncology

Abstract:Background and PurposePrognostic assessment of local therapies for colorectal liver metastases (CLM) is essential for guiding management in radiation oncology. Computed tomography (CT) contains liver texture information which may be predictive of metastatic environments. To investigate the feasibility of analyzing CT texture, we sought to build an automated model to predict progression-free survival using CT radiomics and artificial intelligence (AI).Materials and MethodsLiver CT scans and outcomes for N = 97 CLM patients treated with radiotherapy were retrospectively obtained. A survival model was built by extracting 108 radiomic features from liver and tumor CT volumes for a random survival forest (RSF) to predict local progression. Accuracies were measured by concordance indices (C-index) and integrated Brier scores (IBS) with 4-fold cross-validation. This was repeated with different liver segmentations and radiotherapy clinical variables as inputs to the RSF. Predictive features were identified by perturbation importances.ResultsThe AI radiomics model achieved a C-index of 0.68 (CI: 0.62–0.74) and IBS below 0.25 and the most predictive radiomic feature was gray tone difference matrix strength (importance: 1.90 CI: 0.93–2.86) and most predictive treatment feature was maximum dose (importance: 3.83, CI: 1.05–6.62). The clinical data only model achieved a similar C-index of 0.62 (CI: 0.56–0.69), suggesting that predictive signals exist in radiomics and clinical data.ConclusionsThe AI model achieved good prediction accuracy for progression-free survival of CLM, providing support that radiomics or clinical data combined with machine learning may aid prognostic assessment and management.

Abstract:Breast magnetic resonance imaging (MRI) is the most sensitive imaging modality for breast cancer diagnosis and is widely used clinically. Dynamic contrast-enhanced MRI is the basis for breast MRI, but ultrafast images, T2-weighted images, and diffusion-weighted images are also taken to improve the characteristics of the lesion. Such multiparametric MRI with numerous morphological and functional data poses new challenges to radiologists, and thus, new tools for reliable, reproducible, and high-volume quantitative assessments are warranted. In this context, radiomics, which is an emerging field of research involving the conversion of digital medical images into mineable data for clinical decision-making and outcome prediction, has been gaining ground in oncology. Recent development in artificial intelligence has promoted radiomics studies in various fields including breast cancer treatment and numerous studies have been conducted. However, radiomics has shown a translational gap in clinical practice, and many issues remain to be solved. In this review, we will outline the steps of radiomics workflow and investigate clinical application of radiomics focusing on breast MRI based on published literature, as well as current discussion about limitations and challenges in radiomics.
