Abstract:Objective It aimed to assess the clinical effects of Venlafaxine XR(Efexor XR) treatment of depressive outpatients. Methods In the open-label study, 217 depressed outpatients from 11 hospitals were treated with Venlafaxine XR for 8 weeks. All participants were examined with the Hamilton Rating Scale of Depression (HAMD) and Hamilton Rating Scale of Anxiety (HAMA) before and after treatment. And side effects were recorded as well. Results After 8-week treatment, 89.4% of the patients were improved, and 60.4% had remission. The reduction rates of the HAMD and HAMA scores were 76.7% and 77.1%, respectively. Altogether 59 cases (27.2%) had some side effects during the treatment, but 89.6% of them disappeared when at end of the trial. Conclusions This study shows Venlafaxine XR can effectively treat depressive and anxiety symptoms.

Abstract:The glutathione stability of red cells was estimated in 40 patients during acute hemolysis induced by fava beans. There were wide individual differences but in all cases except one (Case 18) the post-incubation GSH fell to levels below 40 mg. per cent packed RBC which is the lower normal limit. The GSH stability on 44 mothers and 37 fathers gave results consistent with the genetic hypothesis that in male patients the mother is the carrier of the biochemical defect, while in female patients both parents are carriers, since, as a rule, only female homozygotes suffer from hemolytic episodes. However, in only 77.7 per cent of the mothers could the biochemical defect be proved by this method. The Motulsky test was performed in 30 of the 40 patients. It gave abnormal decolorization times in 25 or 83 per cent of the cases. This test is therefore valuable for diagnosing "sensitivity" during a hemolytic episode; it is, nevertheless, less sensitive than the GSH stability method. The Motulsky test was also performed on 31 mothers, 18 fathers and 8 siblings. It proved to be unreliable in the detection of female heterozygotes. G-6-PD deficiency is widely disseminated in Greece; it is, however, not evenly distributed throughout the country. The highest frequency of G-6-PD deficiency found so far in males was about 3 per cent; the lowest was 0.7 per cent.

Abstract:Background: Treatment-resistant depression (TRD) is considered one of the major clinical challenges in the field of psychiatry. An estimated 44% of patients with major depressive disorder (MDD) do not respond to two consecutive antidepressant therapies, and 33% do not respond to up to four antidepressants. Over 15% of all patients with MDD remain refractory to any treatment intervention. rTMS is considered a treatment option for patients with TRD. Likewise, iCBT is evidence-based, symptom-focused psychotherapy recommended for the treatment of TRD. Objective: This study aimed to evaluate the initial comparative clinical effectiveness of rTMS treatment with and without iCBT as an innovative intervention for the treatment of participants diagnosed with TRD. Methods: This study is a prospective two-arm randomized controlled trial. Overall, 78 participants diagnosed with TRD were randomized to one of two treatment interventions: rTMS sessions alone and rTMS sessions plus iCBT. Participants in each group were made to complete evaluation measures at baseline, and 6 weeks (discharge) from treatment. The primary outcome measure was baseline to six weeks change in mean score for the 17-item Hamilton depression rating scale (HAMD-17). Secondary outcomes included mean baseline to six-week changes in the Columbia suicide severity rating scale (CSSRS) for the rate of suicidal ideations, the QIDS-SR16 for subjective depression, and the EQ-5D-5L to assess the quality of health in participants. Results: A majority of the participants were females 50 (64.1%), aged ≥ 40 39 (50.0%), and had college/university education 54 (73.0%). After adjusting for the baseline scores, the study failed to find a significant difference in the changes in mean scores for participants from baseline to six weeks between the two interventions under study on the HAMD-17 scale: F (1, 53) = 0.15, p = 0.70, partial eta squared = 0.003, CSSRS; F (1, 56) = 0.04 p = 0.85, partial eta squared = 0.001, QIDS-SR16 scale; F (1, 53) = 0.04 p = 0.61, partial eta squared = 0.005, and EQ-5D-VAS; F (1, 51) = 0.46 p = 0.50, and partial eta squared = 0.009. However, there was a significant reduction in means scores at week six compared to baseline scores for the combined study population on the HAMD-17 scale (42%), CSSRS (41%), QIDS-SR16 scale (35%), and EQ-VAS scale (62%). Conclusion: This study did not find that combined treatment of TRD with rTMS + iCBT (unguided) was superior to treatment with rTMS alone. Our findings do not support the use of combined treatment of rTMS + iCBT for the management of TRD disorders.

Short-term venlafaxine v. lithium monotherapy for bipolar type II major depressive episodes: effectiveness and mood conversion rate

Abstract:Background: Controversy exists over antidepressant use in bipolar II depression. Aims: To compare the safety and effectiveness of antidepressantv.mood stabiliser monotherapy for bipolar type II major depressive episodes. Method: Randomised, double-blind, parallel-group, 12-week comparison of venlafaxine (n= 65)v.lithium (n= 64) monotherapy in adult out-patients (trial registration numberNCT00602537). Results: Primary outcome - venlafaxine produced a greater response rate (67.7%)v lithium (34.4%,P<0.001). Secondary outcomes - venlafaxine produced a greater remission rate (58.5%v 28.1%,P<0.001); greater decline in depression symptom scores over time (β = -5.32, s.e. = 1.16, χ(2)= 21.19,P<0.001); greater reduction in global severity scores over time (β = -1.05, s.e. = 0.22, w(2)= 22.33,P<0.001); and greater improvement in global change scores (β = -1.31, s.e. = 0.32, χ(2)= 16.95,P<0.001) relative to lithium. No statistically significant or clinically meaningful differences in hypomanic symptoms were observed between treatments. Conclusions: These findings suggest that short-term venlafaxine monotherapy may provide effective antidepressant treatment for bipolar II depression without a statistically significant increase in hypomanic symptoms relative to lithium.
Randomized controlled study of early medication change for non-improvers to antidepressant therapy in major depression--The EMC trial

Abstract:Patients with Major Depressive Disorder (MDD) and no improvement after two weeks of antidepressant pharmacotherapy have a high risk of treatment failure. The aim of the study was to determine whether an early medication change (EMC) strategy is superior to a guideline-based treatment in MDD patients without improvement after two weeks of antidepressant pharmacotherapy. Eight-hundred-and-eighty-nine patients with MDD were enrolled, 879 patients received the SSRI escitalopram. Of those, 192 patients had no improvement, defined as a reduction of < 20% on the Hamilton Depression Rating Scale (HAMD-17) after 14 days of treatment, and were randomly assigned to open treatment with the EMC strategy (n = 97; venlafaxine XR for study days 15-56; in case of sustained non-improvement on day 28, lithium augmentation for days 29-56) or TAU (n = 95; escitalopram continuation; non-responders on day 28 were switched to venlafaxine XR for four weeks, i.e. days 29-56). The primary outcome was remission (HAMD-17 ≤ 7) after 8 weeks of treatment as assessed by blinded raters. Remission rates were 24% for EMC and 16% for TAU, which was not significantly different (p = 0.2056). Sensitivity analyses for the primary and secondary effectiveness endpoints consistently showed favorable results for patients randomized to EMC. The results confirm data from post-hoc analyses of clinical trials showing that early non-improvement identifies patients who likely need alternate interventions. However, the herein used two-step switch/augmentation strategy for this risk group was not more effective than the control intervention. Alternate strategies and other design aspects are discussed in order to support researchers addressing the same research question.

Abstract:To compare the different treatment regimens for refractory depression(RD)effica-cy and its influence factors. Method:Sixty RD patients were randomly divided into venlafaxine + lithium car-bonate and venlafaxine + olanzapine groups and treated for 12 weeks. The patients were assessed by Hamilton depression scale 17 item(HAMD-17)before and after treatment and by treatment emergent symprom scale (TESS)after treatment. The patients whose HAMD reduced rate ﹤ 50± were assessed by Eysenck personality questionnaire(EPQ)and treated with medicine combined psychotherapy for other 12 weeks. The factors affect-ing the HAMD score were analyzed. Results:After treatment,the HAMD score,total efficiency and incidence of adverse reactions between the two groups were not significantly different. There were total 17 cases in the two groups treated with medicine combined psychotherapy and 14 cases(82. 4± )had response. The scores of neu-roticism and extroversion in EPQ were correlatied with the efficacy(r = - 0. 447,r = 0. 883;all P ﹤ 0. 01). Conclusion:Combining with psycholotherapy can make over 80± patients with drug invalid improved. The personality factors are influence the curative effect of psycholotherapy.

Abstract:The phenolic compounds were extracted from green and yellow leaves, stalks, and seeds of garlic ( Allium ursinum L.). The extracts were analyzed by liquid chromatography-photodiode array detector-electrospray ionization-tandem mass spectrometry (LC-PDA-ESI-MS/MS). In total, 21 compounds were detected. The flavonol derivatives were identified on the basis of their ultraviolet (UV) spectra and fragmentation patterns in collision-induced dissociation experiments. On the basis of accurate MS and MS/MS data, six compounds were newly identified in bear's garlic, mainly the kaempferol derivatives. As far as the investigated parts of garlic are concerned, the kaempferol derivatives were found to be predominant in yellow leaves [2362.96 mg/100 g of dry matter (dm)], followed by green leaves (1856.31 mg/100 g of dm). Seeds contained the minimal phenolic compounds, less than stalks. The yellow leaves of A. ursinum possessed a much larger content of compounds acylated with p-coumaric acid than green leaves (1299.97 versus 855.67 mg/100 g of dm, respectively). The stalks and seeds contained much more non-acetylated than acetylated flavonoid glycosides with p-coumaric acid compounds (162.4 versus 62.82 mg/100 g of dm and 105.49 versus 24.18 mg/100 g of dm, respectively).

Abstract:Abstract Background In the population of postmenopausal patients with major depressive disorder (MDD), the superiority of serotonin-norepinephrine reuptake inhibitors (SNRIs) over selective serotonin reuptake inhibitors (SSRIs) has not yet been definitively proven. Consequently, a direct comparison of the efficacy of SSRIs and SNRIs in the treatment of postmenopausal depression could provide relevant data. The aim of this study was to compare the efficacy and safety of venlafaxine vs. fluoxetine in the treatment of postmenopausal MDD. Methods This was an 8-week, multicenter, randomized, single-blind, active-controlled trial conducted at a psychiatric hospital (Beijing Anding Hospital) and a general hospital (Beijing Chaoyang Hospital) between April 2013 and September 2017. The primary outcome measure was improving depressive symptoms (Hamilton Depression Rating Scale (HAMD-24) score). The secondary outcomes included the change of HAMD-24 anxiety/somatization factor score and Clinical Global Impressions-Improvement (CGI-I) response rate. Safety was assessed by treatment-emergent adverse events (TEAEs) and laboratory tests. Efficacy was analyzed by using the full analysis set (FAS) following the modified intention-to-treat (mITT) principle. The primary endpoint measurements were analyzed using a mixed-effect model for repeated measures (MMRM) model with patients as a random-effect factor, treatment group as the independent variable, time as a repeated measure, and baseline covariates, using a first-order ante dependence covariance matrix. Results A total of 184 women were randomized. The full analysis set (FAS) included 172 patients (venlafaxine, n = 82; fluoxetine, n = 90). Over the 8-week study period, the reduction in HAMD-24 scores was significant ( P < 0.001) in both groups, while a significantly greater decline from baseline was observed in the venlafaxine group compared with the fluoxetine group (least-squares mean difference [95% CI]: − 2.22 [− 7.08, − 0.41]), P = 0.001). The baseline-to-week-8 least-squares mean change of the anxiety/somatization factor scores, CGI-I response rate were greater in the venlafaxine group than in the fluoxetine group (all P < 0.05). The most frequent TEAEs (≥5%) in both groups were nausea, somnolence, dizziness, headache, and dry mouth. There was no significant difference in the incidence of adverse events between the two groups. Conclusion Venlafaxine was well tolerated and compared to fluoxetine, it led to a greater improvement in the treatment of postmenopausal MDD. Trial registration Clinical Trials. gov #NCT01824433 . Abstract:Further research is needed to help improve both the standard of care and the outcome for patients with treatment-resistant depression. A particularly critical evidence gap exists with respect to whether pharmacological or non-pharmacological augmentation is superior to antidepressant switch, or vice-versa. The objective of this study was to compare the effectiveness of augmentation with aripiprazole or repetitive transcranial magnetic stimulation versus switching to the antidepressant venlafaxine XR (or duloxetine for those not eligible to receive venlafaxine) for treatment-resistant depression. In this multi-site, 8-week, randomized, open-label study, 278 subjects (196 females and 82 males, mean age 45.6 years (SD 15.3)) with treatment-resistant depression were assigned in a 1:1:1 fashion to treatment with either of these three interventions; 235 subjects completed the study. 260 randomized subjects with at least one post-baseline Montgomery-Asberg Depression Rating (MADRS) assessment were included in the analysis. Repetitive transcranial magnetic stimulation (score change (standard error (se)) = −17.39 (1.3) ( p = 0.015) but not aripiprazole augmentation (score change (se) = −14.9 (1.1) ( p = 0.069) was superior to switch (score change (se) = −13.22 (1.1)) on the MADRS. Aripiprazole (mean change (se) = −37.79 (2.9) ( p = 0.003) but not repetitive transcranial magnetic stimulation augmentation (mean change (se) = −42.96 (3.6) ( p = 0.031) was superior to switch (mean change (se) = −34.45 (3.0)) on the symptoms of depression questionnaire. Repetitive transcranial magnetic stimulation augmentation was shown to be more effective than switching antidepressants in treatment-resistant depression on the study primary measure. In light of these findings, clinicians should consider repetitive transcranial magnetic stimulation augmentation early-on for treatment-resistant depression.

A Preliminary Randomized Controlled Trial of Different Treatment Regimens for Melancholic Depression

Abstract:Background: Fluoxetine, bupropion, cognitive behavioral therapy (CBT), and physical therapies (modified electroconvulsive treatment or repetitive transcranial magnetic stimulation) can be used to manage melancholic depression. Objective: To compare the efficacy and safety of various treatments in patients with melancholic depression. Methods: This was a preliminary multicenter randomized controlled trial that included patients with depression in their first or recurrent acute episode between September 2016 and June 2019, and randomized to fluoxetine, fluoxetine+CBT, fluoxetine+bupropion, and fluoxetine+bupropion+brain stimulation. The primary endpoint was the decrease in the 17 item Hamilton Depression Rating Scale (17-HDRS). The secondary endpoint included the scores from the Quick Inventory of Depressive Symptomatology (QIDS-SR), QOL-6, and safety. Adverse events (AEs) were monitored. The follow-ups were performed at the end of the 0th, 2nd, 4th, 6th, 8th, and 12th weeks of treatment. Results: Finally, 113 patients were included in the analyses: fluoxetine (n=37), fluoxetine +CBT (n=27), fluoxetine+bupropion (n=34), and fluoxetine+bupropion+brain stimulation (n=15). The 17-HDRS and QIDS-SR scores decreased in all four groups (all P<0.05). There were no differences in the 17-HDRS scores among the four groups at the end of treatment (P=0.779), except for fluoxetine alone showing a better response regarding selfconsciousness than fluoxetine+bupropion. The QOL-6 scores increased in all four groups. The occurrence of AEs among the four groups showed no significant difference (P=0.053). Conclusion: This preliminary trial suggests that all four interventions (fluoxetine, fluoxetine +CBT, fluoxetine+bupropion, and fluoxetine+bupropion+brain stimulation) achieved similar response and remission rates in patients with melancholic depression, but that fluoxetine had a better effect on self-consciousness than fluoxetine+bupropion. The safety profile was manageable.
A parallel control study of venlafaxine and fluoxertine in first-episode patients with depression

Abstract:Objective: To compare the effectiveness,safety and compliance of venlafaxine extended release and fluoxertine hydrochloride in outpatients with first episode depression.Methods:Total 184 cases of first-episode depressive outpatient were admitted and assigned to receive either venlafaxine extended release(venlafaxine XR)(n=93) or fluoxertine hydrochloride(n=91) by a parallel control prospective study.The treatment was 8 weeks,and Disease Severity was assessed by Hamilton Rating Scale for Depression(HAMD)17 item.Using Treatment Emergent Symptom Scale(TESS)for evaluating adverse events,and Social Disability Screening Schedule(SDSS)for social disability.Results:(1) At week 6,the response rate(69.89%)and remission rate(31.18%)of venlafaxine XR group were higher than those of fluoxertine hydrochloride group[(53.85%)and(16.48%)](P<0.05);At week 8,the response rate and the remission rate of venlafaxine XR group was 77.42%and 44.09 %,and fluoxertine hydrochloride group had a response rate of 68.13%,and the remission rate is 36.26%,there was no statistical difference between the two groups(P>0.05).At week 8,the SDSS scores of venlafaxine XR group and fluoxertine hydrochloride group were[(1.63±2.98)scores vs.(1.69±2.70)score](P>0.05).(2)The comparison of the adverse events frenquence rates of venlafaxine group and fluoxertine hydrochloride group had no differencial meaning(P>0.05)(3) The discontinued rates of venlafaxine XR group and fluoxertine hydrochloride group(12.90%vs.17.58 %) were not statistical difference(P>0.05).Conclusion:Venlafaxine extended release and fluoxertine hydrochloride have similar therapeutic efficacy,and at week 6 venlafaxine XR has higher response rate and remission rate.Both two drugs have good safety and compliance in treating first episode depression outpatients.

Abstract:OBJECTIVE: To evaluate data supporting the ability of venlafaxine, an antidepressant with a dual mechanism of action, to produce remission from depression.METHOD: Review of multicentre, double-blind, randomized studies comparing venlafaxine or venlafaxine extended release (XR) with a selective serotonin reuptake inhibitor (SSRI), using Hamilton Depression Rating Scale total scores in the range of < or = 7 and < 10 as the final outcome measure, to evaluate the ability of venlafaxine/venlafaxine XR to produce full remission from depression.RESULTS: Venlafaxine/venlafaxine XR demonstrated higher rates of remission than did the SSRIs and placebo.CONCLUSION: With full remission rather than response as the measure of outcome, venlafaxine/venlafaxine XR demonstrated more robust antidepressant efficacy than the SSRIs and placebo. This finding suggests that venlafaxine/venlafaxine XR are appropriate standard-of-care therapies for the treatment of patients with major depressive disorder.

Abstract:For a classical mathematician, mathematics consists of the discovery of preexisting mathematical truth. This understanding of mathematics is captured in Paul Erdös’s notion of “God’s Book of Mathematics,” which contains the best mathematical definitions, theorems, and proofs, and from which fortunate mathematicians are occasionally permitted read a page. Intuitionism takes the position that mathematical objects are mental constructions. Intuitionistic epistemology centers on proof, rather than truth. Thus, intuitionists analyze propositional combinations of mathematical statements in terms of what it takes to prove them, and a proof of φ∧ψ consists of a proof of φ together with a proof of ψ, a proof of φ ∨ ψ consists of a proof of φ or a proof of ψ, while a proof of φ ⇒ ψ consists of an algorithm that converts proofs of φ into proofs of ψ. For an intuitionist, a propositional formula is a tautology if it can be proven, e.g., α ⇒ α is an intuitionistic tautology because to convert a proof p of α into a proof of α we simply return p. The classical mathematician believes in the soundness of mathematical reasoning, i.e., that everything provable is true, and therefore all intuitionistic tautologies are also classical tautologies. The intuitionistic mathematician has no reason, however, to believe that everything that is true is provable (since he does not possess in his mind a construction that enables him to pass from an arbitrary but true mathematical statement φ to an intuitionistic proof of φ). Indeed, it is not difficult to give propositional formulae that are classical, but not intuitionistic, tautologies. The standard example of a classical tautology that is not an intuitionistic tautology is the law of the excluded middle, α ∨ ¬α. The law of the excluded middle is true classically because α must be either true or false,
Efficacy of fluoxetine combined with psychological intervention for treatment of depressive patients with insomnia

Abstract:Objective: The aim of the current study was to investigate the clinical effects of fluoxetine combined with psychological intervention on depressive patients with insomnia, examining influences on sleep quality and quality of life. Methods: One hundred and seventy-eight patients with depression and insomnia were selected as research subjects. A total of 98 patients were treated with fluoxetine combined with psychological intervention (group A). A total of 80 patients were treated with fluoxetine only (group B). Clinical therapeutic effects of the two groups were observed. Depression degrees of the patients were evaluated via Hamilton depression rating scale (HAMD) scores. Sleep quality was assessed using Pittsburgh Sleep Quality Index (PSQI) scores. Quality of life, after treatment, was evaluated by World Health Organization Quality of Life-Brief (WHOQOL-BREF) scores. Incidence of adverse reactions in the two groups was observed. Results: There were no significant differences in HAMD scores and PSQI scores between groups A and B before treatment (both P>0.05). Scores in both groups decreased significantly after treatment (both P<0.001). After treatment, HAMD scores and PSQI scores in group A were significantly lower than those in group B (both P<0.001). Reduction rates in group A were significantly higher than those in group B (both P<0.001). There were no significant differences in incidence of adverse reactions between groups A and B (P>0.05). After treatment, scores of mental health, physiological health, surrounding environment, and social relations via the WHOQOL-BREF scale in group A were significantly higher than those in group B (all P<0.001). Conclusion: Fluoxetine combined with psychological intervention demonstrated good clinical effects on depressive patients with insomnia, significantly reducing depression, improving sleep quality, and improving quality of life. Therefore, it is worthy of clinical application and promotion.

Abstract:AIM: To test the efficacy and safety of venlafaxine in treating depression. METHODS: In a multicenter open trail, 62 patients [M 24, F 38; age (40± s 12) a] with depression or bipolar affective disorder, depressive phase were given venlafaxine (114±44) mg·d -1 , po , two or three times every day for 6 wk. Therapeutic effects were evaluated with HAMD, CGI; drug safety were evaluated with TESS, physical examination and laboratory data. RESULTS: After treatment the total effective rate of venlafaxine was 89%. Scores of HAMD decreased from 29±6 to 6±5 ( P 0.01). The main adverse reactions were xerostomia (14%), excitation/agitation (14%), sweating (10%), nausea/vomiting (9%) and headache (8%). CONCLUSION: Venlafaxine is effective in treating depression, adverse reactions are relatively mild and well tolerated.

Abstract:Common iliac artery aneurysm (CIA) often occurs in conjunction with an abdominal aortic aneurysm (AAA), which extends into one or both CIAs in 20% to 30% of patients. Conventional endovascular treatment includes coil embolization of the internal iliac artery (IIA), followed by extension of the main bifurcated AAA stent-graft into the external iliac artery. However, complications from intentional occlusion of unilateral or bilateral IIAs are frequent and sometimes serious. Several methods try to preserve the unilateral or bilateral IIA. Here we report a case of concomitant bilateral CIA and AAA successfully treated with bilateral branched iliac stent-grafts.

Abstract:Purpose: Clinical studies have suggested that venlafaxine induces a higher remission rate than paroxetine. However, very few studies have evaluated relapse episodes over treatment periods longer than a few weeks, and the cut-off score of 7 on the Hamilton Rating Scale for Depression (HRSD) often used to define remission is too high. This score is associated with the high rates of social function impairment. We report on a single centre, open-label, prospective 24-week study to investigate the comparative efficacy of acute treatments with venlafaxine and paroxetine, using different definitions of response and remission rates.Methods: Outpatients satisfying DSM-IV criteria for major depression with a baseline HRSD17 score of at least 16 were eligible. Following baseline evaluations, the patients were assigned to receive venlafaxine 75-225 mg/day with the mean dosage 141.35 +/- 26.98 (SD) mg/day (n = 78), or paroxetine 20 mg/day (n = 92) for 24 weeks. Efficacy was assessed using the mean change in HRSD17 score from baseline, the response rate and the remission rates based on different criteria for remission (HRSD17 score <= 7 or 5).Results: One hundred and seventy patients were evaluated for efficacy; 78 treated with venlafaxine and 92 with paroxetine. Over the treatment period, venlafaxine was comparable with paroxetine on most outcome measures, whereas paroxetine produced significantly higher remission rates at weeks 4, 8, 16, 20 and 24 weeks when the lower cutoff of 5 was used.Conclusions: Venlafaxine treatment was similar to paroxetine according to the typical efficacy measures for treating outpatients with major depression. However, based on the stricter remission criterion, paroxetine might be superior to venlafaxine.
Effectiveness of mirtazapine as add-on to paroxetine v. paroxetine or mirtazapine monotherapy in patients with major depressive disorder with early non-response to paroxetine: a two-phase, multicentre, randomized, double-blind clinical trial

Abstract:Background This study aimed to examine the efficacy of combining paroxetine and mirtazapine v. switching to mirtazapine, for patients with major depressive disorder (MDD) who have had an insufficient response to SSRI monotherapy (paroxetine) after the first 2 weeks of treatment. Methods This double-blind, randomized, placebo-controlled, three-arm study recruited participants from five hospitals in China. Eligible participants were aged 18–60 years with MDD of at least moderate severity. Participants received paroxetine during a 2-week open-label phase and patients who had not achieved early improvement were randomized to paroxetine, mirtazapine or paroxetine combined with mirtazapine for 6 weeks. The primary outcome was improvement on the Hamilton Rating Scale for Depression 17-item (HAMD-17) scores 6 weeks after randomization. Results A total of 204 patients who showed early non-response to paroxetine monotherapy were randomly assigned to receive either mirtazapine and placebo (n = 68), paroxetine and placebo (n = 68) or mirtazapine and paroxetine (n = 68), with 164 patients completing the outcome assessment. At week 8, the least squares (LS) mean change of HAMD-17 scores did not significantly differ among the three groups, (12.98 points) in the mirtazapine group, (12.50 points) in the paroxetine group and (13.27 points) in the mirtazapine plus paroxetine combination group. Participants in the paroxetine monotherapy group were least likely to experience adverse effects. Conclusions After 8 weeks follow-up, paroxetine monotherapy, mirtazapine monotherapy and paroxetine/mirtazapine combination therapy were equally effective in non-improvers at 2 weeks. The results of this trial do not support a recommendation to routinely offer additional treatment or a switch in treatment strategies for MDD patients who do not show early improvement after 2 weeks of antidepressant treatment.

Effectiveness of Antidepressants in Combination with Psychotherapy

Abstract:Background: Consensus-guidelines for prescribing antidepressants recommend that clinicians should be vigilant to match antidepressants to patient's medical history but provide no specific advice on which antidepressant is best for a given medical history. Aims of the study: For patients with major depression who are in psychotherapy, this study provides an empirically derived guideline for prescribing antidepressant medications that fit patients' medical history. Methods: This retrospective, observational, cohort study analyzed a large insurance database of 3,678,082 patients. Data was obtained from healthcare providers in the U.S. between January 1, 2001, and December 31, 2018. These patients had 10,221,145 episodes of antidepressant treatments. This study reports the remission rates for the 14 most commonly prescribed single antidepressants (amitriptyline, bupropion, citalopram, desvenlafaxine, doxepin, duloxetine, escitalopram, fluoxetine, mirtazapine, nortriptyline, paroxetine, sertraline, trazodone, and venlafaxine) and a category named "Other" (other antidepressants/combination of antidepressants). The study used robust LASSO regressions to identify factors that affected remission rate and clinicians' selection of antidepressants. The selection bias in observational data was removed through stratification. We organized the data into 16,770 subgroups, of at least 100 cases, using the combination of the largest factors that affected remission and selection bias. This paper reports on 2,467 subgroups of patients who had received psychotherapy. Results: We found large, and statistically significant, differences in remission rates within subgroups of patients. Remission rates for sertraline ranged from 4.5% to 77.86%, for fluoxetine from 2.86% to 77.78%, for venlafaxine from 5.07% to 76.44%, for bupropion from 0.5% to 64.63%, for desvenlafaxine from 1.59% to 75%, for duloxetine from 3.77% to 75%, for paroxetine from 6.48% to 68.79%, for escitalopram from 1.85% to 65%, and for citalopram from 4.67% to 76.23%. Clearly these medications are ideal for patients in some subgroups but not others. If patients are matched to the subgroups, clinicians can prescribe the medication that works best in the subgroup. Some medications (amitriptyline, doxepin, nortriptyline, and trazodone) always had remission rates below 11% and therefore were not suitable as single antidepressant therapy for any of the subgroups. Discussions: This study provides an opportunity for clinicians to identify an optimal antidepressant for their patients, before they engage in repeated trials of antidepressants. Implications for health care provision and use: To facilitate the matching of patients to the most effective antidepressants, this study provides access to a free, non-commercial, decision aid at http://MeAgainMeds.com. Implications for health policies: Policymakers should evaluate how study findings can be made available through fragmented electronic health records at point-of-care. Alternatively, policymakers can put in place an AI system that recommends antidepressants to patients online, at home, and encourages them to bring the recommendation to their clinicians at their next visit. Implications for further research: Future research could investigate (i) the effectiveness of our recommendations in changing clinical practice, (ii) increasing remission of depression symptoms, and (iii) reducing cost of care. These studies need to be prospective but pragmatic. It is unlikely random clinical trials can address the large number of factors that affect remission.
Pharmacologic approaches to treatment resistant depression: Evidences and personal experience

Abstract:Aim: To review evidence supporting pharmacological treatments for treatment-resistant depression (TRD) and to discuss them according to personal clinical experience. Methods: Original studies, clinical trials, systematic reviews, and meta-analyses addressing pharmacological treatment for TRD in adult patients published from 1990 to 2013 were identified by data base queries (PubMed, Google Scholar e Quertle Searches) using terms: "treatment resistant depression", "treatment refractory depression", "partial response depression", "non responder depression", "optimization strategy", "switching strategy", "combination strategy", "augmentation strategy", selective serotonin reuptake inhibitors antidepressants (SSRI), tricyclic antidepressants (TCA), serotonin norepinephrine reuptake inhibitors antidepressants, mirtazapine, mianserine, bupropione, monoamine oxidase inhibitor antidepressant (MAOI), lithium, thyroid hormones, second generation antipsychotics (SGA), dopamine agonists, lamotrigine, psychostimulants, dextromethorphan, dextrorphan, ketamine, omega-3 fatty acids, S-adenosil-L-metionine, methylfolat, pindolol, sex steroids, glucocorticoid agents. Other citations of interest were further identified from references reported in the accessed articles. Selected publications were grouped by treatment strategy: (1) switching from an ineffective antidepressant (AD) to a new AD from a similar or different class; (2) combining the current AD regimen with a second AD from a different class; and (3) augmenting the current AD regimen with a second agent not thought to be an antidepressant itself. Results: Switching from a TCA to another TCA provides only a modest advantage (response rate 9%-27%), while switching from a SSRI to another SSRI is more advantageous (response rate up to 75%). Evidence supports the usefulness of switching from SSRI to venlafaxine (5 positive trials out 6), TCA (2 positive trials out 3), and MAOI (2 positive trials out 2) but not from SSRI to bupropione, duloxetine and mirtazapine. Three reviews demonstrated that the benefits of intra- and cross-class switch do not significantly differ. Data on combination strategy are controversial regarding TCA-SSRI combination (positive results in old studies, negative in more recent study) and bupropion-SSRI combination (three open series studies but not three controlled trails support the useful of this combination) and positive regard mirtazapine (or its analogue mianserine) combination with ADs of different classes. As regards the augmentation strategy, available evidences supported the efficacy of TCA augmentation with lithium salts and thyroid hormone (T3), but are conflicting regard the SSRI augmentation with these two drugs (1 positive trial out of 4 for lithium and 3 out of 5 for thyroid hormone). Double-blind controlled studies showed the efficacy of AD augmentation with aripiprazole (5 positive trials out 5), quetiapine (3 positive trials out 3) and, at less extent, of fluoxetine augmentation with olanzapine (3 positive trials out 6), so these drugs received the FDA indication for the acute treatment of TRD. Results on AD augmentation with risperidone are conflicting (2 short term positive trials, 1 short-term and 1 long-term negative trials). Case series and open-label trials showed that AD augmentation with pramipexole or ropinirole, two dopamine agonists, could be an effective treatment for TRD (response rate to pramipexole 48%-74%, to ropinirole 40%-44%) although one recent double-blind placebo-controlled study does not support the superiority of pramipexole over placebo. Evidences do not justify the use of psychostimulants, omega-3 fatty acids, S-adenosil-L-metionine, methylfolate, pindolol, lamotrigine, and sex hormone as AD augmentation for TRD. Combining the available evidences with our experience we suggest treating non-responders to one SSRI bupropion or mirtazapine trial by switching to venlafaxine, and non-responders to one venlafaxine trial by switching to a TCA or, if TCA are not tolerated, combining mirtazapine with SSRI or venlafaxine. In non-responders to two or more ADs (including at least one TCA if tolerated) current AD regimen could be augmented with lithium salts (mainly in patients with bipolar depression or suicidality), SGAs (mostly aripiprazole) or DA-agonists (mostly pramipexole). In patients with severe TRD, i.e., non-responders to combination and augmentation strategies as well as to electroconvulsive therapy if workable, we suggest to try a combination plus augmentation strategy. Conclusion: Our study identifies alternative effective treatment strategies for TRD. Further studies are needed to compare the efficacy of different strategies in more homogeneous subpopulations.
