Abstract:Blood flow and pressure calculated using the currently available methods have shown the potential to predict the progression of pathology, guide treatment strategies and help with postoperative recovery. However, the conspicuous disadvantage of these methods might be the time-consuming nature due to the simulation of virtual interventional treatment. The purpose of this study is to propose a fast novel physics-based model, called FAST, for the prediction of blood flow and pressure. More specifically, blood flow in a vessel is discretized into a number of micro-flow elements along the centerline of the artery, so that when using the equation of viscous fluid motion, the complex blood flow in the artery is simplified into a one-dimensional (1D) steady-state flow. We demonstrate that this method can compute the fractional flow reserve (FFR) derived from coronary computed tomography angiography (CCTA). 345 patients with 402 lesions are used to evaluate the feasibility of the FAST simulation through a comparison with three-dimensional (3D) computational fluid dynamics (CFD) simulation. Invasive FFR is also introduced to validate the diagnostic performance of the FAST method as a reference standard. The performance of the FAST method is comparable with the 3D CFD method. Compared with invasive FFR, the accuracy, sensitivity and specificity of FAST is 88.6%, 83.2% and 91.3%, respectively. The AUC of FFR FAST is 0.906. This demonstrates that the FAST algorithm and 3D CFD method show high consistency in predicting steady-state blood flow and pressure. Meanwhile, the FAST method also shows the potential in detecting lesion-specific ischemia.

Abstract:Ascorbic acid treatmentL of animals exposed to cold, and enhances the survival of animals prevents the adrenal hypertrophy typicalexposed to this stressor (Dugal and Therien, 1949). It was observed that ascorbic acid pretreatment prevented the eosinopenia and the depletion of adrenal cholesterol typical of acute epinephrin stress (Bacchus and Toompas, 1951). This paper reports data which indicate that ascorbic acid prevents the eosinophil response to epinephrin, but not to corticotrophin. Male albino mice were used in these experiments. Groups I and III consisted of mice which received pretreatment with ascorbic acid 12 hours prior to being stressed. The ascorbic acid (Vitamin C, injectable, Roche) was administered intraperitoneally at the dosage of 88 mg./100 g. in a single injection. [Pleural clot ablation and fibrinolytic agents].

Abstract:Two patients presenting with an important intrapleural clot with cardiorespiratory repercussions were given in situ thrombolytic treatment after drainage with a suction tube had failed. The first patient was treated immediately after thoracophrenolaparotomy for hepatic surgery. Dissolution of the clot was obtained with two local injections of urokinase (450,000 IU). The second patient was treated on the tenth day of a recurring post-traumatic clot, with urokinase (75,000 IU, then 225,000 IU) each day. In addition, this patient received lys-plasminogen locally (15 mukat, then 30 mukat). Dissolution of the clot was complete within five days. Clinical tolerance was excellent and there was no effect on the usual clotting studies. This study confirms previous findings, and underlines the usefulness of this method and its lack of adverse effects, even immediately postoperatively.

Abstract:BackgroundCoronary obstruction (CO) is a rare but devasting complication during transcatheter aortic valve replacement (TAVR).ObjectivesWe aim to demonstrate that the predicted distance between the coronary ostia and the closest structure derived with patient-specific computer simulation is associated with CO risk during TAVR.MethodsWe retrospectively analysed 14 aortic stenosis patients who underwent TAVR through finite element simulation. The frame deformation predicted with patient-specific computer simulation was qualitatively and quantitatively compared to the post-operative device deformation. The minimum distance between each coronary ostium and the closest structure was calculated and compared in patients who developed CO, at high risk of CO, and at no risk of CO.ResultsFour patients experienced CO during TAVR, 5 patients were at high risk of CO, and the remaining 5 patients had no risk of CO. A high coefficient of determination was obtained for all measurements extracted from the simulated device and the post-operative device (≥0.95). Simulations predicted shorter distance between the coronary ostium and the closest structure in patients who experienced CO, compared to patients at high risk of CO or who did not experience this complication (right coronary: 5.9 vs. 6.8 vs. 8.8 mm, left coronary: 3.0 vs. 3.3 vs. 6.5 mm respectively).ConclusionsThe distance between the coronary ostium and the closest structure was lower in patients who experienced CO during TAVR through patient-specific computational simulation. This technology enables coronary obstruction analysis before TAVR in the future.
Peri-therapeutic multi-modal hemodynamic assessment and detection of predictors for symptomatic in-stent restenosis after percutaneous transluminal angioplasty and stenting

Abstract:Backgrounds: This study performed multi-modal hemodynamic analysis including quantitative color-coded digital subtraction angiography (QDSA) and computational fluid dynamics (CFD) to delineate peri-therapeutic hemodynamic changes and explore the risk factors for in-stent restenosis (ISR) and symptomatic ISR (sISR). Methods: Forty patients were retrospectively reviewed. Time to peak (TTP), full width at half maximum (FWHM), cerebral circulation time (CCT), angiographic mean transit time (aMTT), arterial stenosis index (ASI), wash-in gradient (WI), wash-out gradient (WO) and stasis index were calculated with QDSA and translesional pressure ratio (PR) and wall shear stress ratio (WSSR) were quantified from CFD analysis. These hemodynamic parameters were compared between before and after stent deployment and multivariate logistic regression model was established to detect predictors for ISR and sISR at follow-up. Results: It was found that stenting generally reduced TTP, stasis index, CCT, aMTT and translesional WSSR while significantly increased translesional PR. ASI decreased after stenting, and during the mean follow-up time of 6.48 ± 2.86 months, lower ASI (<0.636) as well as larger stasis index were corroborated to be independently associated with sISR. aMTT showed a linear correlation with CCT before and after stenting. Conclusion: PTAS not only improved cerebral circulation and blood flow perfusion but also changed local hemodynamics significantly. ASI and stasis index derived from QDSA were proved to play a prominent role in risk stratification for sISR. Multi-modal hemodynamic analysis could facilitate intraoperative real-time hemodynamic monitoring and help the determination of the end point of intervention.

Abstract:As experts continue to debate the optimal surgery practice for coronary disease - percutaneous coronary intervention (PCI) or coronary aortic bypass graft (CABG) - computational tools may provide a quantitative assessment of each option. Computational fluid dynamics (CFD) has been used to assess the interplay between hemodynamics and stent struts; it is of particular interest in Bioresorbable Vascular Stents (BVS), since their thicker struts may result in impacted flow patterns and possible pathological consequences. Many proofs of concept are presented in the literature; however, a practical method for extracting patient-specific stented coronary artery geometries from images over a large number of patients remains an open problem. This work provides a possible pipeline for the reconstruction of the BVS. Using Optical Coherence Tomographies (OCT) and Invasive Coronary Angiographies (ICA), we can reconstruct the 3D geometry of deployed BVS in vivo. We illustrate the stent reconstruction process: (i) automatic strut detection, (ii) identification of stent components, (iii) 3D registration of stent curvature, and (iv) final stent volume reconstruction. The methodology is designed for use on clinical OCT images, as opposed to approaches that relied on a small number of virtually deployed stents. The proposed reconstruction process is validated with a virtual phantom stent, providing quantitative assessment of the methodology, and with selected clinical cases, confirming feasibility. Using multimodality image analysis, we obtain reliable reconstructions within a reasonable timeframe. This work is the first step toward a fully automated reconstruction and simulation procedure aiming at an extensive quantitative analysis of the impact of BVS struts on hemodynamics via CFD in clinical trials, going beyond the proof-of-concept stage.

Abstract:The Flow Diverter is a self-expandable braided stent that has helped improve the effectiveness of cerebral aneurysm treatment during the last decade. The Flow Diverter's efficiency heavily relies on proper decision-making during the pre-operative phase, which is currently based on static measurements that fail to account for vessel or tissue deformation. In the context of providing realistic measurements, a biomechanical computational method is designed to aid physicians in predicting patient-specific treatment outcomes. The method integrates virtual and analytical treatment models, validated against experimental mechanical tests, and two patient treatment outcomes. In the case of both patients, deployed stent length was one of the validated result parameters, which displayed an error inferior to 1.5% for the virtual and analytical models. These results indicated both models' accuracy. However, the analytical model provided more accurate results with a 0.3% error while requiring a lower computational cost for length prediction. This computational method can offer designing and testing platforms for predicting possible intervention-related complications, patient-specific medical device designs, and pre-operative planning to automate interventional procedures.

Predicting flow diverter sizing using the AneuGuideTM software: a validation study

Abstract:Background: Stent sizing remains a challenging task for flow diverter implantation because of stent foreshortening. In this study, we aimed to quantify the change in length after implantation and assess the error in length prediction using AneuGuideTM software. Methods: In a retrospective cohort of 101 patients with 102 aneurysms undergoing treatment with a pipeline embolization device (PED; Covidien, Irvine, California, USA), we used AneuGuideTM software to obtain measured lengths (ML) and calculated lengths (CL) after stent implantation. Stent elongation was defined as the ratio of ML-LL to the labeled length (LL). Simulation error was defined as the ratio of the absolute value of CL-ML to ML. The correlation and consistency between ML and LL and between ML and CL were analyzed using Pearson's correlation test and the Bland-Altman plot. Statistical significance was set at p<0.05. Results: The mean elongation of ML was 32.6% (range 26.3-109.2%). Moderate consistency was observed between LL and ML (ρ=0.74, p<0.001). With the AneuGuideTM software, the mean simulation error was 6.6% (range 0.32-21.2%). Pearson's correlation test and the Bland-Altman plot showed a high correlation and consistency between ML and CL (ρ=0.96, p<0.001). Conclusion: Labeled length provides only a low reference value for predicting the actual length of the flow diverter after implantation. The high consistency between ML and CL obtained from AneuGuideTM software shows its great potential for the optimization of the flow diverter sizing process.
In silico modeling for personalized stenting in aortic coarctation

Abstract:Stent intervention is a recommended therapy to reduce the pressure gradient and restore blood flow for patients with coarctation of the aorta (CoA). In this work, we developed a framework for personalized stent intervention in CoA using in silico modeling, combining computational fluid dynamics (CFD) and image-based prediction of the geometry of the aorta after stent intervention. Firstly, the blood flow in the aorta, whose geometry was reconstructed from magnetic resonance imaging (MRI) data, was numerically modeled using the lattice Boltzmann method (LBM). Both large eddy simulation (LES) and direct numerical simulation (DNS) were considered to adequately resolve the turbulent hemodynamics, with boundary conditions extracted from phase-contrast flow MRI. By comparing the results from CFD and 4D-Flow MRI in 3D-printed flow phantoms, we concluded that the LBM based LES is capable of obtaining accurate aortic flow with acceptable computational cost. In silico stent implantation for a patient with CoA was then performed by predicting the deformed geometry after stent intervention and predicting the blood flow. By evaluating the pressure drop and maximum wall shear stress, an optimal stent can be selected.

Predictive Numerical Simulations of Double Branch Stent-Graft Deployment in an Aortic Arch Aneurysm

Abstract:Total endovascular repair of the aortic arch represents a promising option for patients ineligible to open surgery. Custom-made design of stent-grafts (SG), such as the Terumo Aortic RelayBranch device (DB), requires complex preoperative measures. Accurate SG deployment is required to avoid intraoperative or postoperative complications, which is extremely challenging in the aortic arch. In that context, our aim is to develop a computational tool able to predict SG deployment in such highly complex situations. A patient-specific case is performed with complete deployment of the DB and its bridging stents in an aneurysmal aortic arch. Deviations of our simulation predictions from actual stent positions are estimated based on post-operative scan and a sensitivity analysis is performed to assess the effects of material parameters. Results show a very good agreement between simulations and post-operative scan, with especially a torsion effect, which is successfully reproduced by our simulation. Relative diameter, transverse and longitudinal deviations are of 3.2$\pm$4.0%, 2.6$\pm$2.9mm and 5.2$\pm$3.5mm respectively. Our numerical simulations show their ability to successfully predict the DB deployment in complex anatomy. The results emphasize the potential of computational simulations to assist practitioners in planning and performing complex and secure interventions.

Abstract:Detection of clinically relevant stenosis within coronary arteries as well as planning of treatment (stent implantation) are important topics in clinical cardiology. In this study a thorough methodology for virtual stenting assistance is proposed, that includes the 3D reconstruction of a patient-specific coronary artery from X-ray angiography images, hemodynamic simulations of blood flow, computation of a fractional flow reserve (FFR) equivalent, virtual stenting procedure and an optimization of the virtual stenting, by considering not only the value of computed FFR, but also the low and high WSS regions and the state of arterial wall after stenting. The evaluation of the proposed methodology is performed in two ways: the calculated values of FFR are compared with clinically measured values; and the results obtained for automated optimized virtual stenting are compared with virtual stenting performed manually by an expert clinician for the whole considered dataset. The agreement of the results in almost all cases demonstrates the accuracy of the proposed approach, and the small discrepancies only show the capabilities and benefits this approach can offer. The automated optimized virtual stenting technique can provide information about the most optimal stent position that ensures the maximum achievable FFR, while also considering the distribution of WSS and the state of arterial wall. The proposed methodology and developed software can therefore be used as a noninvasive method for planning of optimal patient-specific treatment strategies before invasive procedures and thus help to improve the clinical outcome of interventions and provide better treatment planning adapted to the particular patient.

Abstract:Aim: In endovascular ICAD treatment, an individualized patient-specific treatment pathway may be very helpful. Patient selection for endovascular treatment based on anatomical markers has not been shown to be effective in past clinical trials. This pilot study combines CT perfusion (CTP) analysis and computational fluid dynamics to identify hemodynamically significant patients for a reliable pathway for intervention planning. Methods: We assessed patients from two stroke centers with 50-99% ICAD lesions. Using a novel technique, CTP scans generated acute vascular territory maps for major cerebral arteries (ACA,MCA,PCA) with perfusion Time-to-peak (TTP) maps. Patient-specific blood flow simulations using physiology-informed boundary conditions calculated stenotic pressure ratios for both the patient’s acute and a population-based healthy scenario. The changes in affected territory volumes and pressure ratio between healthy and acute conditions and the acute TTP delays were presented to an expert reader who stratified cases into three clinical groups (A) non-hemodynamic, (B) hemodynamically hazardous with insufficient collateral support and (C) hemodynamically stable due to collateralisation. These results were compared against patient outcomes of recurrent ischemic events attributed to the target stenosis using the commonly used 70% threshold. Results: (95%CI in brackets) Patients (n=47, age: 73±11Y, 23F/24M) were stratified across the three categories A) 30, B) 15, C) 2. Of the patient groups with follow-up (n=31), categorisation of hemodynamic ICAD against recurrent ipsilateral stroke in territory (n=8) yielded a sensitivity of 75% [34-97] and specificity of 91% [72-98]. Mean±standard deviation of stenosis degree (SD) 55±14% for patients across each cohort was A) 50±11%, B) 70±9%, C) 70%. This is compared to using 70% stenosis threshold for dichotomising patients against risk of recurrent stroke, which yielded a sensitivity of 62.5% [24-91], and specificity of 87% [66-97]. Conclusions: We report on a CTP derived method for stratifying hemodynamically significant ICAD. The methodology has potential in both in assessing symptomatic chronic ICAD and identifying cohorts who would benefit from endovascular intervention.

Abstract:The paper describes the difficult course of catatonic-paranoid psychosis which began with symptoms similar to the myasthenia. The growing symptoms of catatonia (in this oral mechanisms with the compulsion of mastication, injuring with teeth of the mouth, tongue biting and damage, such as lockjaw) brought about choking which was followed by aspiration pneumonia. The patient had to have pharmacological coma induced, along with muscle relaxation and artificial ventilation in the conditions of the intensive care department. Despite treatment with high doses of neuroleptics, the repeated trials of bringing the patient out from the coma caused recurrence of the catatonic symptoms. A decision was made to go along with electroconvulsive therapy. During one of the ECT treatments there were complications in the form of circulation cessation which required defibrillation. The paper contains basic information about the serious complications of the electroconvulsive therapy. It moreover carries out the critical analysis of the whole treatment period.
Patient-specific hemodynamic markers co-localise with regions of neointimal hyperplasia in an arteriovenous graft

Abstract:Purpose. Most computational fluid dynamics (CFD) studies on arteriovenous grafts (AVGs) adopt idealised geometries and simplified boundary conditions (BCs), potentially resulting in misleading conclusions when attempting to predict neointimal hyperplasia (NIH) development. Moreover, they often analyse a limited range of hemodynamic indices, lack validation, and fail to link the graft-altered hemodynamics with follow-up data. This study develops a novel patient-specific CFD workflow for AVGs using pathophysiological BCs. It validates the CFD results with patient medical data and assesses the co-localisation between CFD results and NIH regions at follow-up. Methods. Contrast-enhanced computed tomography angiography images were used to segment the patient-specific AVG geometry. A uniform Doppler ultrasound (DUS)-derived velocity profile was imposed at the inlet, and three-element Windkessel models were applied at the arterial outlets of the domain. Transient, rigid-wall simulations were performed using the k-omega SST turbulence model. The CFD-derived flow waveform was compared with the DUS image of the patient to ensure validation. Turbulent kinetic energy (TKE), helicity and near-wall hemodynamic descriptors were calculated and linked with regions presenting NIH from a 4-month follow-up fistulogram. Results. In the analysed patient, areas presenting high TKE and balanced helical flow structures at baseline exhibit NIH growth at follow-up. Transverse wall shear stress index is a stronger predictor of NIH than other commonly analysed near-wall hemodynamic indices, since luminal areas subjected to high values greatly co-localise with observed areas of remodelling. Conclusion. This patient-specific computational workflow for AVGs could be applied to a larger cohort to unravel the link between altered hemodynamics and NIH progression in vascular access.
Influence of Iliac Stenotic Lesions on Blood Flow Patterns Near a Covered Endovascular Reconstruction of the Aortic Bifurcation (CERAB) Stent Configuration

Abstract:Purpose: To investigate the effect of distal stenotic lesions on flow patterns near a covered endovascular reconstruction of the aortic bifurcation (CERAB) configuration used in the treatment of aortoiliac occlusive disease. Method: Laser particle image velocimetry measurements were performed using in vitro models of the aortic bifurcation with and without a CERAB configuration in place. A hemodynamically nonsignificant stenosis (ΔP: 9 mm Hg), a hemodynamically significant (ΔP: 26 mm Hg) stenosis, and a total occlusion were simulated in the left iliac arteries. Velocity fields and time-averaged wall shear stress (TAWSS) were calculated. Results: Hemodynamically significant distal lesions did not influence the inflow patterns or TAWSS (0.5-0.6 Pa) in either model. However, hemodynamically significant distal stenotic lesions caused a 2-fold decrease in peak outflow velocities (control: 106 vs 56 cm/s, CERAB: 96 vs 54 cm/s) and a 3-fold decrease in TAWSS (control: 1.34 vs 0.44 Pa, CERAB: 0.75 vs 0.21 Pa). There was a 2-fold decrease in wall shear stress in the CERAB outflow compared with the control, independent of lesion severity. Conclusion: In the CERAB technique, adequate distal runoff is identified as an important parameter to ensure patency. This in vitro study showed that distal stenotic lesions influence aortic bifurcation outflow patterns and TAWSS more extensively in the CERAB configuration. Distal stenotic lesions could therefore increase the risk of disease progression and loss of stent patency. In vivo studies are necessary to confirm these observations.
Hemodynamic Comparison of Stent-Grafts for the Treatment of Aortoiliac Occlusive Disease

Abstract:Purpose: To compare the flow patterns and hemodynamics of the AFX stent-graft and the covered endovascular reconstruction of aortic bifurcation (CERAB) configuration using laser particle image velocimetry (PIV) experiments. Materials and methods: Two anatomically realistic aortoiliac phantoms were constructed using polydimethylsiloxane polymer. An AFX stent-graft with a transparent cover made with a new method was inserted into one phantom. A CERAB configuration using Atrium's Avanta V12 with transparent covers made with a previously established method was inserted into the other phantom, both modified stent-grafts were suitable for laser PIV, enabling visualization of the flow fields and quantification of time average wall shear stress (TAWSS), oscillatory shear index (OSI), and relative residence time (RRT). Results: Disturbed flow was observed at the bifurcation region of the AFX, especially at the end systolic velocity (ESV) time-point where recirculation was noticeable due to vortical flow. In contrast, predominantly unidirectional flow was observed at the CERAB bifurcation. These observations were confirmed by the quantified hemodynamic results from PIV analysis where mean TAWSS of 0.078 Pa (range: 0.009-0.242 Pa) was significantly lower in AFX as compared with 0.229 Pa (range: 0.013-0.906 Pa) for CERAB (p<0.001). Mean OSI of 0.318 (range: 0.123-0.496) in AFX was significantly higher than 0.252 (range: 0.055-0.472) in CERAB (p<0.001). Likewise, mean RRT of 180 Pa-1 (range: 9-3603 Pa-1) in AFX was also significantly higher than 88 Pa-1 (range: 2-840 Pa-1) in CERAB (p=0.0086). Conclusion: In this in vitro study, the flow pattern of a modified AFX stent-graft was found to be more disturbed especially at the end systolic phase, its hemodynamic outcomes less desirable than CERAB configuration. Clinical relevance: While the AFX stent-graft has an advantage over the CERAB configuration in eliminating radial mismatch, and maintaining the anatomical bifurcation for future endovascular intervention, this in vitro study revealed that the associated lower TAWSS, higher OSI and RRT may predispose to thrombosis and are, thus, less desirable as compared to a CERAB configuration. Further investigation is warranted to confirm whether these findings translate into the clinical setting.

Abstract:The structural morphology of coronary stents and the local hemodynamic environment following stent deployment in coronary arteries are crucial determinants of procedural success and subsequent clinical outcomes. High-resolution intracoronary imaging has the potential to facilitate geometrically accurate three-dimensional (3D) reconstruction of coronary stents. This work presents an innovative algorithm for the 3D reconstruction of coronary artery stents, leveraging intravascular ultrasound (IVUS) and angiography. The accuracy and reproducibility of our method were tested in stented patient-specific silicone models, with micro-computed tomography serving as a reference standard. We also evaluated the clinical feasibility and ability to perform computational fluid dynamics (CFD) studies in a clinically stented coronary bifurcation. Our experimental and clinical studies demonstrated that our proposed algorithm could reproduce the complex 3D stent configuration with a high degree of precision and reproducibility. Moreover, the algorithm was proved clinically feasible in cases with stents deployed in a diseased coronary artery bifurcation, enabling CFD studies to assess the hemodynamic environment. In combination with patient-specific CFD studies, our method can be applied to stenting optimization, training in stenting techniques, and advancements in stent research and development.

Abstract:Background The rapid evolution of artificial intelligence (AI) in conjunction with recent updates in dual antiplatelet therapy (DAPT) management guidelines emphasizes the necessity for innovative models to predict ischemic or bleeding events after drug‐eluting stent implantation. Leveraging AI for dynamic prediction has the potential to revolutionize risk stratification and provide personalized decision support for DAPT management. Methods and Results We developed and validated a new AI‐based pipeline using retrospective data of drug‐eluting stent‐treated patients, sourced from the Cerner Health Facts data set (n=98 236) and Optum's de‐identified Clinformatics Data Mart Database (n=9978). The 36 months following drug‐eluting stent implantation were designated as our primary forecasting interval, further segmented into 6 sequential prediction windows. We evaluated 5 distinct AI algorithms for their precision in predicting ischemic and bleeding risks. Model discriminative accuracy was assessed using the area under the receiver operating characteristic curve, among other metrics. The weighted light gradient boosting machine stood out as the preeminent model, thus earning its place as our AI‐DAPT model. The AI‐DAPT demonstrated peak accuracy in the 30 to 36 months window, charting an area under the receiver operating characteristic curve of 90% [95% CI, 88%–92%] for ischemia and 84% [95% CI, 82%–87%] for bleeding predictions. Conclusions Our AI‐DAPT excels in formulating iterative, refined dynamic predictions by assimilating ongoing updates from patients' clinical profiles, holding value as a novel smart clinical tool to facilitate optimal DAPT duration management with high accuracy and adaptability.

Abstract:Abstract One of the most common acute and chronic cardiovascular disease conditions is aortic stenosis, a disease in which the aortic valve is damaged and can no longer function properly. Moreover, aortic stenosis commonly exists in combination with other conditions causing so many patients suffer from the most general and fundamentally challenging condition: complex valvular, ventricular and vascular disease (C3VD). Transcatheter aortic valve replacement (TAVR) is a new less invasive intervention and is a growing alternative for patients with aortic stenosis. Although blood flow quantification is critical for accurate and early diagnosis of C3VD in both pre and post-TAVR, proper diagnostic methods are still lacking because the fluid-dynamics methods that can be used as engines of new diagnostic tools are not well developed yet. Despite remarkable advances in medical imaging, imaging on its own is not enough to quantify the blood flow effectively. Moreover, understanding of C3VD in both pre and post-TAVR and its progression has been hindered by the absence of a proper non-invasive tool for the assessment of the cardiovascular function. To enable the development of new non-invasive diagnostic methods, we developed an innovative image-based patient-specific computational fluid dynamics framework for patients with C3VD who undergo TAVR to quantify metrics of: (1) global circulatory function; (2) global cardiac function as well as (3) local cardiac fluid dynamics. This framework is based on an innovative non-invasive Doppler-based patient-specific lumped-parameter algorithm and a 3-D strongly-coupled fluid-solid interaction. We validated the framework against clinical cardiac catheterization and Doppler echocardiographic measurements and demonstrated its diagnostic utility by providing novel analyses and interpretations of clinical data in eleven C3VD patients in pre and post-TAVR status. Our findings position this framework as a promising new non-invasive diagnostic tool that can provide blood flow metrics while posing no risk to the patient. The diagnostic information, that the framework can provide, is vitally needed to improve clinical outcomes, to assess patient risk and to plan treatment.

A Fast Method of Virtual Stent Graft Deployment for Computer Assisted EVAR

Abstract:In this paper we introduce a new method simulating stent graft deployment for assisting endovascular repair of abdominal aortic aneurysms. The method relies on intraoperative images coupled with mechanical models. A multi-step algorithm has been developed to increase the reliability of simulations. The first step predicts the position of the stent graft within the aorta. The second step is an axisymmetric geometric reconstruction of each individual stent. The third step minimizes the rotation of each stent around its main axis. Finally, the last step virtually deploys each stent within a deployment box extracted from the preoperative CT scan. A proof of concept is performed on a patient. The accuracy is compatible with the clinical threshold of 3 mm: the average distance between target and simulated stents is 1.73 $\pm$ 0.37 mm. Fenestrations of the stent-graft are reconstructed with a maximum error of less than 2.5 mm, which is enables a secure catheterization of secondary arteries. In summary, the method is able to assist EVAR practitioners by providing all necessary information for a fast and accurate stent graft positioning, combining intraoperative data and a mechanical model in a very low cost framework.
