Abstract:Objective: Few studies have compared the surgical outcomes of laparoscopic (LG) and open (OG) gastrectomy in obese patients (BMI≥30 kg/m2) with gastric cancer. The current study aimed to investigate the short-term outcomes of LG in this group of patients. Methods: A total of 33 LG cases and 23 OG cases (BMI≥30 kg/m2) were identified from our gastric cancer database. Clinicopathologic features, operative details, laboratory examination, and postoperative outcomes were compared between both groups. Regression analysis was used to determine the effects of BMI on intraoperative outcomes. Results: The 2 groups had comparable clinicopathologic characteristics. LG was associated with significantly lesser blood loss, whereas both also groups had a similar operative time, and number of harvested lymph nodes. However, regression analysis indicated that increased BMI affected the operative time and blood loss in patients that underwent OG but had little effect on patients who received LG. The elevation of inflammatory factors (WBC, CRP) was lower in LG than in OG, postoperatively. Postoperative hepatic (alanine aminotransferase, total bilirubin, albumin) and renal (creatinine, blood urea nitrogen) functions in the LG group were not worse than in the OG group. The time to first flatus, initiation of diet, hospitalization, and postoperative complications seemed superior in LG than in OG, but these differences were not statistically significant. Conclusion: LG can be safely performed in obese gastric cancer patients. Compared with conventional OG, LG is less invasive and is characterized by less blood loss and milder surgical trauma. LG is also less adversely affected by increased BMI.

Abstract:To evaluate the efficacy of laparoscopic sleeve gastrectomy(LSG) on improving glycemic control of morbidly obese patients with type 2 diabetes mellitus.Prospective study of 30 type 2 diabetes mellitus(T2DM) patients with BMI > or = 35 undergone LSG was carried out. Pre- and post-operative clinical parameters associated with diabetes mellitus 6 months after operation were evaluated. Data collected included demographics, weight loss(BMI, EWL%), diabetes control (FPG, HbA1C) and ghrelin.Fasting plasma glucose and glycosylated hemoglobin concentration tests and clinical outcomes suggested LSG resulted in significant improvement or resolution of T2DM in all the 30 patients with CR 63%(19/30) and PR 37%(11/30).LSG can lead to significant changes in controlling glycemia on morbidly obese patients with T2DM in 6 months. Long-term efficacy needs further follow-up.
Effect of Laparoscopic Sleeve Gastrectomy on Obesity and Type 2 Diabetes Mellitus

Abstract:Objective To explore the morbidity of surgery in connection with laparoscopic sleeve gastrectomy (LSG) and its effect on obese T2DM.Methods 106 obese T2DM patientes undergoing LSG were divided into 2 groups in group 125 patients did not have oversewing the staple line and group 281 patients had the staple line oversewn in order to reduce bleeding.Results The differences in intraoperative blood loss (35 ± 15) ml vs.(28 ± 18) ml,postoperative recovery time (2.4 ± 0.9) d vs.(2.3 ± 0.9) d,time to taking liquid food (4.7 ± 1.0) d vs.(4.6 ± 1.0) d between two groups were not significant.There were no significant difference of complication between 2 groups (x2 =3.271,P =0.071).Comparing before surgery to 6 month after surgery,the BMI in group 1,was from (39 ± 5) to (29 ±4) kg/m2;in group 2,from (40 ±6) to (31 ±5) kg/m2,FPG in group 1,from (8.4 ± 1.4) to (6.4 ±1.2) mmol/L;in group 2,from (8.2 ± 2.0) to (6.8 ± 1.5) mmol/L,2 hour post-meal blood sugar [group 1,(13.2±4.1) to (9.6±3.2) mmol/L;group 2,(12.2±3.2) to (10.6±2.8) mmol/L] and HbAlc (group 1,7.2％ ±1.2％ to5.5％ ±1.1％;group 2,7.1％ ±1.1％ to 5.9％ ±1.2％) decreased significantly in both groups (P ＜ 0.01).There was 72 (68％) remission cases of T2DM in 106 patients,there were no significant differences of T2DM remission and BMI between 2 groups at 6 months after surgery (P =0.617).Conclusions LSG leads to significant weight loss and T2DM control.

Abstract:Objectives To investigate the efficacy of laparoscopic sleeve gastrectomy (LSG)for patients of type 2 diabetes mellitus (T2DM).Methods Between Jun 2013 and Jun 2017,39 T2DM patients were enrolled in this study and underwent LSG.The clinical data of all patients were collected.The fasting plasma glucose (FPG),glycosylated hemoglobin (HbAl c),C-peptide,fasting insulin (FINS),glucagon-like peptide-1 (GLP-1) as well as BMI of all patients were measured before and at 1,6 months after surgery.Results All patients underwent LSG surgery successfully.The mean levels of FPG,HbAlc,C-peptide,FINS and BMI of all patients decreased at 6 months after surgery respectively compared to those before surgery [(6.4 ± 1.2) mmol/L vs.(9.6 ± 2.3) mmol/L,t =9.455,P =0.000;(6.0 ± 0.7) ％ vs.(7.5 ±1.0)％,t=10.538,P=0.000;(2.7±2.2)ng/ml vs.(4.0±2.6) ng/ml,t=3.491,P=0.001;(9.2 ±6.3) μIU/L vs.(15.5 ±11.1) μIU/L,t=4.902,P=0.000;(32.0 ±6.1) kg/m2 vs.(41.0 ± 7.5) kg/m2,t =10.826,P =0.000],however the mean GLP-1 of all patients increased after surgery compared to that before surgery [(4.0 ± 2.1) pmol/L vs.(3.1 ± 2.2) pmol/L,t =-4.242,P =0.000].At 6 months after surgery,32 patients achieved complete remission of T2DM,5 patients gained partial remission of T2DM and 2 patients got no remission of T2DM.Conclusions LSG is effective for obesity patients as well as patients suffering from T2DM.
Successful Management of New-Onset Diabetes Mellitus and Obesity with the Use of Laparoscopic Sleeve Gastrectomy after Kidney Transplantation-A Case Report.

Abstract:In kidney transplantation, obesity is associated with poorer graft survival and patient survival. Bariatric surgery may provide benefit for these patients, not only by inducing weight loss, but also via reduction of diabetes. We report a case of morbid obesity, poorly controlled new-onset diabetes mellitus, and gout after kidney transplantation that was treated with laparoscopic sleeve gastrectomy 3 years after kidney transplantation. After 1 year of follow-up, 76% excessive body weight loss was attained. No complications were noted. The operation also provided total remission of diabetes and gout as well as good graft survival. Based on our experience, laparoscopic sleeve gastrectomy may be a feasible treatment for obese patients after renal transplantation to help resolve obesity and control new-onset diabetes. However, the timing of operation and the long-term potential for graft and patient survivals with this operation require further study.
Clinical Analysis of Laparoscopic Sleeve Gastrectomy in Treating Patients with Type 2 Diabetes and High BMI

Abstract:Objective To investigate the feasibility and efficacy of laparoscopic sleeve gastrectomy (LSG) in the treatment of patients with type 2 diabetes (T2DM) and high body mass index (BMI). Methods The clinical data of 24 patients with T2DM and high BMI who underwent LSG were analyzed retrospectively from 2010 to 2016. Statistical analysis were performed by using SPSS 17.0 software package. Measurement data such as preoperative/postoperative body weight and HbAlc levels were expressed as (±s) and examined by using independent t-test. Count data were expressed as frequency and percentage, and were compared by using χ2 test. A P value of <0.05 was considered as statistically significant difference. Results The average baseline weight of 24 patients was (117.2±30.2) kg, which decreased significantly to (100.3±22.1) kg, (91.6±22.4) kg, (81.3 ± 16.5) kg and (80.3 ± 14.3) kg at 3 months, 6 months, 12 months and 24 months after surgery respectively (P<0.001). The preoperative average baseline HbA1c was (8.5±2.2)%, which was significantly decreased to (6.4±1.6)%, (5.8±1.1)%, (5.7± 0.7)% and (5.3±0.6)% at 3 months, 6 months, 12 months, and 24 months, respectively (P<0.001). 19 patients with type 2 diabetes who underwent sleeve gastrectomy discontinued insulin and oral hypoglycemic agents. According to the level of blood glucose control, remission rate of 24 patients was 79.2% 24 months after surgery. Except one case, other 17 patients with hypertensive stopped antihypertensive treatment after surgery. Conclusion LSG is a safe, feasible and effective for the treatment of patients with T2DM and high BMI. It is worthy of widespread clinical application. Short-term clinical efficacy of laparoscopic sleeve gastrectomy with transit bipartition

Abstract:Objective:To explore the short-term efficacy of laparoscopic sleeve gastrectomy with transit bipartition (LSG+TB) in the treatment of obesity with T2DM.Methods:A retrospective cohort study was conducted to investigate the clinical data of 13 obese patients with T2DM who underwent LSG+TB in Nanjing University Medical School Affiliated Nanjing Drum Tower Hospital from July 2020 to June 2021. The changes of body weight, blood glucose and nutritional indexes and the occurrence of complications were observed and analyzed during 6 months post operation.Results:All 13 patients were followed up 3 and 6 months after operation. Preoperative BMI of the patients decreased from (35.67±3.11) kg/m2 to (28.63±3.12) kg/m2 and (25.58±1.92) kg/m2 at 3 and 6 months post operation, respectively, with a significant downward trend, and there was statistically significant differences among groups (F=231.09, P<0.001). The %EBMIL at 3 and 6 months post operation was (69.93±19.97)% and (97.51±16.18)%, respectively. Fasting blood glucose decreased from (8.47±3.21) mmol/L before operation to (5.14±0.69) mmol/L and (4.93±0.55) mmol/L 3 and 6 months post operation, respectively, with a statistically significant downward trend (F=16.08, P<0.05). HbA1c decreased from (7.90±1.88)% before operation to (5.17±0.66)% and (5.14±0.37)% 3 and 6 months post operation, respectively, with a statistically significant downward trend (F=31.10, P<0.001). There were 3 cases (23%) mild iron deficiency anemia, 2 cases (15%) diarrhea (stool frequency >3 times/day), 1 case (8%) smelly exhaust. There were no complications such as malnutrition and vitamin deficiency.Conclusions:LSG+TB might safely and effectively reduce excess weight and alleviate hyperglycemia, with the relatively low incidence of short-term nutrition-related complications, and might be used as one of the surgical options for the treatment of obesity with T2DM.
Positive Changes in Body Composition and Profiles of Individuals with Diabetes 3 Years Following Laparoscopic Sleeve Gastrectomy in Japanese Patients with Obesity

Abstract:Background and Objectives: We analyzed the changes in obesity, glucose metabolism, and body composition over a 3-year period in Japanese patients with obesity following laparoscopic sleeve gastrectomy (LSG). Methods: Body weight, parameters related to diabetes such as glycated hemoglobin (HbA1c), and electrical impedance analysis were used to assess body composition in forty-eight Japanese patients with obesity before surgery and 6 months, 1 year, 2 years, and 3 years after LSG. Results: At 6 months, 1, 2, and 3 years post-LSG, there were significant reductions in body weight, body mass index, blood pressure, fasting plasma glucose, triglyceride, and HbA1c levels. Six months after LSG, fat mass (FM), muscle mass (MM), and %FM all showed a decrease compared to pre-treatment values (all p < 0.05). FM and %FM remained in a decreased state until 3 years had passed. In contrast, %MM increased at 6 months post-LSG and was maintained up to 3 years post-LSG (all p < 0.05). Furthermore, changes in FM and %FM were associated with changes in body weight and A1C. In contrast, change in %MM exhibited a negative correlation with body weight and A1C following LSG. Finally, multivariate regression analyses demonstrated that alterations in FM were independent factors affecting body weight in patients with obesity 3 years after LSG. Conclusions: We observed improvements in FM, fasting plasma glucose, and HbA1c levels over a 3-year period in Japanese patients after LSG. The reduction in FM and maintenance of %MM after LSG were suggested as possible links between the effects of LSG on obesity and diabetes over 3 years.

[Efficacy Analysis of Laparoscopic Sleeve Gastrectomy in the Treatment of Obesity-Related Comorbidities].

Abstract:OBJECTIVE:To investigate the safety and efficacy of laparoscopic sleeve gastrectomy (LSG) for the treatment of obesity and related diseases.METHODS:Clinical data of 67 patients who underwent LSG between December 2006 and July 2011 were analyzed retrospectively. Improvements in body mass index(BMI), percentage of excess weight loss(EWL), type 2 diabetes mellitus, hypertension and other comorbidities were observed at one year postoperatively.RESULTS:Laparoscopic procedures were completed in 67 patients without conversion. The operative time was(78±17) min. The postoperative hospital stay was(5.0±1.7) d. The postoperative recovery was uneventful and there were no perioperative death or severe postoperative complication. Sixty-four patients(95.5%) had a postoperative follow up of 1 year. One year after LSG, BMI decreased by(10.4±3.7) kg/m(2) from (37.7±4.1) kg/m(2) preoperatively and EWL was(80.2±27.7)%. In 13 cases of type 2 diabetes mellitus, 7 patients (53.8%, 7/13) were weaned off hypoglycemic agents or insulin. In 11 cases of hypertension, 5 required no medications(45.5%, 5/11). The remission rate was both 100%. There was significant resolution or improvement of other obesity-related comorbidiities, including hyper-triglyceridemia(n=51), hyperuricemia(n=42), sleep apnea syndrome (n=2), osteoarticular disease (n=9), and acanthosis (n=8).CONCLUSIONS:LSG is safe and feasible for the treatment of obesity and can cure or improve type 2 diabetes mellitus, hypertension and other obesity related comorbidities.
Efficacy of laparoscopic gastric bypass vs laparoscopic sleeve gastrectomy in treating obesity combined with type-2 diabetes

Abstract:Objective: This prospective study compared the efficacy and safety of laparoscopic gastric bypass and laparoscopic sleeve gastrectomy in treating overweight and obese patients with BMI>28 kg/m2 and type-2 diabetes. Methods: Patients were randomized into a gastric bypass group (n = 77) or a gastrectomy group (n = 80). The surgery time, intraoperative blood loss, recovery time, and hospitalization time were collected. BMI, waistline, hipline, C-peptide level, insulin resistance index (HOMA-IR), and their blood and lipid profile were also measured. Results: Surgery time and blood loss were significantly higher in the gastrectomy group, when compared to the gastric bypass group (P < 0.05). In both groups, the levels of BMI, waist circumference and hip circumference (but not their ratio) gradually and significantly decreased after surgery compared with baseline (P < 0.05), and no significant difference was found between these two groups. The C-peptide level, HOMA-IR, fasting blood glucose, 2-hour postprandial blood glucose and glycosylated haemoglobin gradually and significantly decreased after surgery compared with the values before treatment (P < 0.05). The levels of total cholesterol, triglyceride, LDL, and monocyte chemoattractant protein-1 were also lower after surgery in both groups, while HDL and glucagon-like peptide-1 were significantly higher after surgery compared with the values before treatment (P < 0.05). However, no significant difference was found between these two groups of patients. Conclusion: Both laparoscopic gastric bypass and laparoscopic sleeve gastrectomy improved the BMI and diabetic conditions of overweight/obese diabetics, while laparoscopic sleeve gastrectomy had a shorter surgical time and less blood loss.
Laparoscopic Sleeve Gastrectomy under General Anesthesia in Severely Obese Patients: a Single-Centered Retrospective Study

Abstract:BackgroundThis study aims to summarize and analyze the clinical characteristics and outcomes of severely obese patients who underwent laparoscopic sleeve gastrectomy (LSG) under general anesthesia with multidisciplinary collaboration.MethodsA retrospective analysis was performed for 100 severely obese patients who were hospitalized in Zhongshan Hospital, Fudan University from January 2017 to December 2019, and included preoperative general information, laboratory examinations, anesthesia and outcomes.ResultsA total of 100 patients (46 males, 54 females) were admitted to the department of endocrinology: 100 had hepatic steatosis (100%), 43 had sleep apnea hypopnea syndrome (43%), 25 had hypertension (25%), 11 had type 2 diabetes (11%) and 8 had polycystic ovary syndrome (14% of women). The mean age and BMI were 31.52 ± 10.53 years and 43.31 ± 6.80 kg/m2, respectively. Visual laryngoscope intubation was successfully performed with routine intravenous induction in the optimum sniffing position at one time. The surgeries were successfully performed under general anesthesia, without conversion, and the operation time was 140.92 ± 31.23 min. The follow-up data for 41 patients were obtained. The postoperative BMI showed a downward trend. The BMI at 1 month and 3 months after surgery were 38.40 ± 6.77 kg/m2and 35.52 ± 7.94 kg/m2, respectively.ConclusionsMultidisciplinary collaboration may contribute to better management and recovery during the perioperative period. Visual laryngoscope intubation with intravenous induction was performed successfully in the optimum sniffing position at one time.
Effect of laparoscopic sleeve gastrectomy on blood sugar and blood pressure

Abstract:Background: Surgery is one and may be the most effective method to treat obesity. In the last decade, Laparoscopic Sleeve Gastrectomy is perceived to be less invasive, technically simple, less morbid and more popular form of bariatric surgery. Objectives: This study aims to assess the effect of Laparoscopic Sleeve Gastrectomy on Fasting Blood Glucose Levels and Blood Pressure. Methods: A prospective controlled study in which 50 obese patients were involved, 36 of patients have hypertension and type 2 diabetes mellitus , 7 patients have type 2 diabetes mellitus only, and 7 patients don’t have hypertension or type 2 diabetes. All patients were submitted to Laparoscopic Sleeve Gastrectomy at Al-Kindy Teaching Hospital between October 2014 and October 2016. Results: Forty five of patients were females, while five were males . Mean age was 34 years, mean height 159 cm mean pre Laparoscopic Sleeve Gastrectomy weight was 121kg. Mean post- Laparoscopic Sleeve Gastrectomy weight was 82kg. Mean weight loss outcome at 1 year follow-up is 32.23% of total body weight. . Preoperative initial mean value of fasting blood glucose was 157.4 mg/dL and the declining occurred at 12 months after Laparoscopic Sleeve Gastrectomy was 97 mg/dL . The hypertension remission rates 1 year after Laparoscopic Sleeve Gastrectomy ranges up to 54% of total number of patients and 75% of those with Hypertension. Conclusions: Laparoscopic Sleeve Gastrectomy has an effect in improving Fasting Blood Glucose Levels and Blood Pressure.
[Efficacy Evaluation of Laparoscopic Gastric Bypass for the Treatment of Obese Type 2 Diabetes Mellitus].

Abstract:OBJECTIVE:To explore the treatment outcomes of obese type 2 diabetes mellitus (T2DM) after laparoscopic gastric bypass. METHODS:The clinical data of 18 patients with obese T2DM who underwent laparoscopic Roux-en-Y gastric bypass in Beijing Shijitan Hospital between March 2009 and February 2011 were retrospectively analyzed. The clinical parameters included preoperative and postoperative blood glucose, blood lipid, nutrition status and weight lose. RESULTS:Eighteen patients included 8 men and 10 women. The range of age was 27-62 years (mean, 42.4±10.7 years). The range of BMI was 28.7-57.4 kg/m(2)(mean, 34.9±6.9 kg/m(2)). All the patients underwent laparoscopic Roux-en-Y gastric bypass, and no mortality, complication or conversion to open operation occurred. At 3 months after operation, there were significant changes in OGTT, BMI, HbA1c, Homa-IR and Homa-β(all P<0.05). Fourteen patients(77.8%) showed clinical complete remission, and the overall effective rate was 100%(18/18). The level of blood lipid decreased significantly (P<0.05), and the change of nutritional status was not statistically significant(P>0.05). CONCLUSION:Gastric bypass is an effective treatment for obese type 2 diabetes mellitus.

Methods: Comparing the weight loss, remission of comorbidities, occurrence of complications, and quality of life among the different BMI patients who underwent LSG. Analyzing the relationship between BMI and percentage of excess weight loss (%EWL) by using Spearman correlation analysis and linear regression analysis. Results: The %EWL at 12 months after the surgical procedure was (104.26± 16.41)%, (90.36± 9.98)%, and (78.30± 14.64)% for patients with Class I, II, and III obesity, respectively, P< 0.05. Spearman correlation coefficients between %EWL and BMI at 1, 3, 6, and 12 months after surgery were R=− 0.334 (P< 0.001), R=− 0.389 (P< 0.001), and R=− 0.442 (P< 0.001), R=− 0.641 (P 0.05). Conclusion: Individuals with obesity for varying BMI can experience favorable outcomes following LSG surgery. It is advisable to consider LSG treatment for patients with Class I obesity. Keywords: laparoscopic sleeve gastrectomy, body mass index, percentage of excess weight loss Obesity presents a significant global challenge, characterized as a complex chronic condition resulting from abnormal or excessive fat accumulation. 1,2 By the year 2015, the global population witnessed an approximate count of 107.7 million children and 603.7 million adults who were afflicted by obesity. 3 Obesity is strongly associated with the onset and progression of numerous illnesses, 4–8 and it has become a serious threat to the health and lives of people in every country, imposing a substantial strain on public health systems across nations. Bariatric surgery frequently yields more favorable outcomes in addressing morbid obesity compared to conventional weight reduction approaches. According to a 2018 survey conducted by the International Federation for the Surgery of Obesity and Metabolic Disorders (IFSO) across five branches worldwide, LSG is currently the most used form of bariatric surgery in the world, accounting for 55.4% of all bariatric procedures. 9 As the amount of LSG continues to increase, the suitability of patients with mild obesity for LSG treatment has become a major concern for bariatric surgeons. In 1991, the patients with obesity of BMI ≥ 35 kg/m 2 as a criterion for bariatric surgery. 10 But in 2022, the criterion for bariatric surgery suggested revising the indication for to BMI >27.5 kg/m2 in Asian populations. 11 Differences in fat distribution, hormone regulation, nutrient absorption, and metabolism in individuals with obesity of various BMI, 12,13 which are associated with LSG efficacy. 14 More evidence is therefore needed to support the suitability of patients with low BMI for LSG treatment. The research conducted by Uri Kaplan demonstrated that patient's preoperative BMI was an independent influence on postoperative weight change. 15 Based on this premise, this study aimed to analyze patients with obesity treated with LSG in our center, to examine the effectiveness and safety of LSG in individuals with obesity who have varying BMI and the effectiveness of LSG as a treatment option for patients with a low BMI, to provide some help for future clinical work. Comparative evaluation of early diabetic outcomes in southeast asian patients with type 2 diabetes mellitus undergoing Roux-en-Y gastric bypass (RYGB) versus sleeve gastrectomy (LSG)

Abstract:Obesity and type 2 diabetes mellitus (T2DM) is an alarming problem globally and a growing epidemic. Metabolic surgery has been shown to be successful in treating both obesity and T2DM, usually after other treatments have failed. This study aims to compare Roux-Y gastric bypass and sleeve gastrectomy in determining early diabetic outcomes in obese Malaysian patients with T2DM following surgery. A total of 172 obese patients with T2DM who were assigned to either laparoscopic Roux-en-Y gastric bypass (LRYGB) or laparoscopic sleeve gastrectomy (LSG) were analysed up to a year post-procedure. The patients' T2DM severity were stratified using the Individualized Metabolic Surgery (IMS) score into mild, moderate and severe. Remission rates of diabetes were compared between surgical techniques and within diabetic severity categories. T2DM remission for patients who underwent either surgical technique for mild, moderate or severe disease was 92.9%, 56.2% and 14.7% respectively. Both surgical techniques improved T2DM control for patients in the study. Comparing baseline with results 1 year postoperatively, median HbA1c reduced from 7.40% (IQR 2.60) to 5.80% (IQR 0.80) ( p < 0.001), mean total antidiabetic medications use reduced from 1.48 (SD 0.99) to 0.60 (SD 0.86) [ p < 0.001], insulin usage reduced from 27.9 to 10.5% ( p < 0.001), and T2DM control improved from 27.9 to 82% ( p < 0.001). The patients had a median excess BMI loss of 69.4% (IQR 34%) and 53.2% (IQR 36.0%) for RYGB and SG respectively ( p = 0.016). At one year following surgery, there is no difference between LRYGB and LSG in terms of diabetic remission. LSG is not inferior to LRYGB in terms of early diabetic outcomes. Milder T2DM shows a better response. LSG is a simpler procedure with a lower risk profile and should be considered as an early treatment option for obese patients with T2DM.

Abstract:Objective: To evaluate the 1-year postoperative efficacy and nutritional indicators of single-anastomosis duodeno-ileal bypass with sleeve gastrectomy (SADI-S) in obese patients. Methods: This retrospective observational study included patients with a body mass index (BMI) of ≥40.0 kg/m2 regardless of other related metabolic diseases and patients with severe type 2 diabetes and a BMI between 27.5 and 40.0 kg/m2. The clinical data of 66 obese patients who underwent SADI-S at the Bariatric and Metabolic Surgery Department of China-Japan Union Hospital of Jilin University from November 2018 to May 2022 were collected, including 53 cases of da Vinci robotic surgery and 13 cases of laparoscopic surgery. The patients comprised 38 men and 28 women with a median age of 35 (18-61) years and a mean preoperative BMI of 42.93 ± 6.82 kg/m2. A total of 38 patients had type 2 diabetes, and 46 had hyperuricemia, 45 had hypertension, 35 had hyperlipidemia, 12 had hypercholesterolemia, and 12 had a high low-density lipoprotein (LDL) level. The main observation indicators were (1) intraoperative and postoperative conditions; (2) weight loss outcomes, including body weight, BMI, excess body weight loss (%EWL), and total body weight loss (%TWL) at 3, 6, and 12 months after surgery; (3) effects of treatment on metabolic disease; and (4) changes in nutrient indicators. Results: (1) Intraoperative and postoperative conditions: All patients successfully underwent SADI-S with neither conversion to laparotomy nor death. Four (6.1%) patients developed postoperative complications, and all of them recovered and were discharged after conservative or surgical treatment. (2) Weight loss outcomes: %EWL at 3, 6, and 12 months after surgery was 62.07 ± 26.56, 85.93 ± 27.92, and 106.65 ± 29.65, respectively, and %TWL was 22.67 ± 4.94, 32.10 ± 5.18, and 40.56 ± 7.89, respectively. Body weight and BMI 3 to 12 months after surgery were significantly lower than those before surgery (all P < 0.001). (3) Effect of treatment on metabolic disease: 3 to 12 months after surgery, fasting blood sugar, HbA1c, uric acid, systolic blood pressure, diastolic blood pressure, triglycerides, total cholesterol, LDL, and other indicators were significantly lower than those before surgery (all P < 0.05). Twelve months after surgery, the remission rates of diabetes, hyperuricemia, hypertension, hypertriglyceridemia, hypercholesterolemia, and high LDL were 100% (38/38), 65.2% (30/46), 62.2% (28/45), 94.3% (33/35), 100% (12/12), and 100% (12/12), respectively. (4) Changes in nutrient indicators: Compared with the preoperative nutrient levels, the hemoglobin and hematocrit levels were lower at 3 to 12 months after surgery, the total protein level was lower at 6 to 12 months after surgery, the albumin level was lower at 6 months after surgery, and the ferritin level was lower at 3 months after surgery. The differences were statistically significant (all P < 0.05). The incidence of anemia was 6.1% (4/66), hypoalbuminemia was 4.5% (3/66), and ferritin deficiency was 4.5% (3/66), all of which were improved or normalized through conservative treatment. Twelve months after surgery, 30 (45.5%) patients had vitamin A deficiency, 17 (25.8%) had vitamin E deficiency, 11 (16.7%) had folic acid deficiency, 2 had potassium deficiency (3.0%), 3 (4.5%) had calcium deficiency, 2 (3.0%) had magnesium deficiency, 9 (13.6%) had iron deficiency, and 16 (24.2%) had zinc deficiency. However, no relevant clinical symptoms occurred. Conclusions: SADI-S has a very significant effect on weight loss and alleviation of metabolic diseases. Nutrient deficiencies after SADI-S mainly involve vitamin A, vitamin E, zinc, and folic acid. The long-term efficacy and safety of SADI-S still need further follow-up observation.

Abstract:Objective To compare the effects of laparoscopic sleeve gastrectomy ( LSG ) and laparoscopic Roux-en-Y gastric bypass ( LRYGB) on pulmonary function and blood lipid levels in patients with severe obesity. Methods 90 patients with severe obesity ( BMI ≥ 35kg/m2 ) admitted to two hospitals from November 2014 to November 2017 were analyzed retrospectively , and the patients were treated with LSG (n=48) or LRYGB (n=42).The SPSS 19.0 was used for statistical analysis , and the general data, lung function indexes, physiological indexes and blood lipid levels were compared by (xˉ±s).The independent t test or one-way ANOVA was used for comparison between groups .The enumeration data was tested by χ2 .P<0.05 indicated that the statistical difference was significant . Results The surgical and anesthesia durations in LSG group were lower than those in LRYGB group , and the differences were statistically significant (P<0.05).And the amount of blood loss during surgery between the two groups showed no statistically significant difference (P>0.05).There was no significant difference in lung function between the groups before surgery.The levels of PaO2 , FVC, FEV1 and MMF in both groups were allsignificantly decreased after surgery (P <0.05).The levels of PaO2 , FVC, FEV1 and MMF in both groupsat 48 h after surgery were all significantly improved than those at 24 h after surgery (P <0.05), but werestill significantly lower than those before surgery (P <0.05).The levels of PaO2 , FVC, FEV1 and MMF inLSG group were all higher than those in LRYGB group at 24 h and 48 h after surgery, and the differenceswere statistically significant ( P <0.05). The levels of weight, BMI, waist circumference, hipcircumference, fasting blood glucose and 2-hour postprandial blood glucose in both groups at 6 months aftersurgery were all decreased than those before surgery, and the differences were statistically significant(P <0.05); however, there was no statistically significant difference between the two groups (P >0.05).The levels of TG, CH and LDL in both groups at 6 months after surgery were significantly decreased thanthose before surgery, while the HDL level was significantly increased (P <0.05); however, there was nostatistically significant difference between the two groups (P >0.05). Conclusion LRYGB and LSGhave the similar result in weight loss and have no significant difference in improving blood lipids , but LSGhas less effects on short-term lung function and oxygenation compared with LRYGB .

Abstract:Objective:To compare the clinical effect of laparoscopic fundus folding combined with sleeve gastrectomy（LFDSG）and laparoscopic sleeve gastrectomy（LSG）in the treatment of non-diabetic obesity.Methods:75 patients without diabetes who underwent weight loss surgery from January 2018 to June 2020 were prospectively selected and divided into LSG group（n=37 cases）and LFDSG group（n=38 cases）by random number method. SPSS 24.0 was used. Perioperative related indicators，esophageal acid reflux and esophageal manometry analysis，weight loss effect，changes in blood lipid and uric acid levels and other were represented by（xˉ±s）and independent sample t test was performed. The incidence of postoperative complications and other statistical data were determined by χ2 test or Fisher’s accurate test. P<0.05 was considered statistically significant.Results:The operation time of LFDSG group was significantly longer than that of LSG group（P<0.05）；The incidence of total postoperative complications in LFDSG group was significantly lower than that in LSG group（2.6% vs. 21.6%，P<0.05）；The number of acid reflux（pH≤4），proximal esophageal acid reflux（%），DeMeester score，lower esophageal sphincter residual pressure reduction，distal esophageal contraction score of LFDSG group was significantly better than LSG group（P<0.05）；6 months after surgery，body weight，BMI，hip circumference，waist circumference and neck circumference of 2 groups were significantly improved compared with those before surgery（P<0.05），but there was no statistical significance between groups（P>0.05）. Compared with before surgery，triglyceride（TG），total cholesterol（TC），low density lipoprotein cholesterol（LDL-C）and uric acid（UA）in both groups were significantly decreased 6 months after surgery，while HDL-C level was significantly increased（P<0.05），but there was no statistical significance between groups（P>0.05）.Conclusion:Compared with LSG surgery，LFDSG surgery has obvious weight loss effect in the treatment of non-diabetic obesity. It has fewer postoperative complications，can play an anti-reflux effect and improve the level of postoperative blood lipid and uric acid.

Abstract:This study aims to examine the changes of glucose metabolism, glucose variability (GV), and ghrelin secretion within 1 week following SG in Chinese patients with obesity. Forty-nine patients with obesity (15 with type 2 diabetes) were enrolled to undergo SG. Within 1 week before and after surgery, liquid meal tests were performed in all subjects, and continuous glucose monitoring (CGM) was performed in diabetic patients. Blood samples were collected at 0, 15, 30, 45, 60, 120, and 180 min for glucose, C-peptide, insulin, and ghrelin analysis in liquid meal test. Mean amplitude of glucose excursions (MAGE), standard deviations (SD), and percent time-in-range (%TIR) determined by CGM were analyzed. Both in diabetic and non-diabetic groups, significant decrease was observed in glucose, insulin, C-peptide, and ghrelin. Homeostasis model assessment-insulin resistance and liver fat content was decreased. In diabetic group, MAGE and SD were decreased significantly, and the percent time-in-range was higher. The decrease in blood glucose was positively correlated with the decrease in ghrelin concentration in non-diabetic group. Within 1 week after SG, both glucose metabolism and glucose variability were improved significantly. Suppression of ghrelin secretion postoperatively might be a driver of this early improved glycemia homeostasis.
