Abstract:Objectives The aim of this study was to investigate potential benefits of ultra-high resolution (UHR) over standard resolution scan mode in ultra-low dose photon-counting detector CT (PCD-CT) of the lung. Materials and Methods Six cadaveric specimens were examined with 5 dose settings using tin prefiltration, each in UHR (120 × 0.2 mm) and standard mode (144 × 0.4 mm), on a first-generation PCD-CT scanner. Image quality was evaluated quantitatively by noise comparisons in the trachea and both main bronchi. In addition, 16 readers (14 radiologists and 2 internal medicine physicians) independently completed a browser-based pairwise forced-choice comparison task for assessment of subjective image quality. The Kendall rank coefficient ( W ) was calculated to assess interrater agreement, and Pearson's correlation coefficient ( r ) was used to analyze the relationship between noise measurements and image quality rankings. Results Across all dose levels, image noise in UHR mode was lower than in standard mode for scan protocols matched by CTDI vol ( P < 0.001). UHR examinations exhibited noise levels comparable to the next higher dose setting in standard mode ( P ≥ 0.275). Subjective ranking of protocols based on 5760 pairwise tests showed high interrater agreement ( W = 0.99; P ≤ 0.001) with UHR images being preferred by readers in the majority of comparisons. Irrespective of scan mode, a substantial indirect correlation was observed between image noise and subjective image quality ranking ( r = −0.97; P ≤ 0.001). Conclusions In PCD-CT of the lung, UHR scan mode reduces image noise considerably over standard resolution acquisition. Originating from the smaller detector element size in fan direction, the small pixel effect allows for superior image quality in ultra-low dose examinations with considerable potential for radiation dose reduction.

Abstract:X-ray computed tomography (CT) is a cardinal tool in clinical practice. It provides cross-sectional images within seconds. The recent introduction of clinical photon-counting CT allowed for an increase in spatial resolution by more than a factor of two resulting in a pixel size in the center of rotation of about 150 µm. This level of spatial resolution is in the order of dedicated preclinical micro-CT systems. However so far, the need for different dedicated clinical and preclinical systems often hinders the rapid translation of early research results to applications in men. This drawback might be overcome by ultra-high resolution (UHR) clinical photon-counting CT unifying preclinical and clinical research capabilities in a single machine. Herein, the prototype of a clinical UHR PCD CT (SOMATOM CounT, Siemens Healthineers, Forchheim, Germany) was used. The system comprises a conventional energy-integrating detector (EID) and a novel photon-counting detector (PCD). While the EID provides a pixel size of 0.6 mm in the centre of rotation, the PCD provides a pixel size of 0.25 mm. Additionally, it provides a quantification of photon energies by sorting them into up to four distinct energy bins. This acquisition of multi-energy data allows for a multitude of applications, e.g. pseudo-monochromatic imaging. In particular, we examine the relation between spatial resolution, image noise and administered radiation dose for a multitude of use-cases. These cases include ultra-high resolution and multi-energy acquisitions of mice administered with a prototype bismuth-based contrast agent (nanoPET Pharma, Berlin, Germany) as well as larger animals and actual patients. The clinical EID provides a spatial resolution of about 9 lp/cm (modulation transfer function at 10%, MTF10%) while UHR allows for the acquisition of images with up to 16 lp/cm allowing for the visualization of all relevant anatomical structures in preclinical and clinical specimen. The spectral capabilities of the system enable a variety of applications previously not available in preclinical research such as pseudo-monochromatic images. Clinical ultra-high resolution photon-counting CT has the potential to unify preclinical and clinical research on a single system enabling versatile imaging of specimens and individuals ranging from mice to man.

Abstract:Background: According to the modern concept “task-specific rehabilitation”, complex training of gait cycles is more preferable than training of separate muscles to walking restoration. However in severe disabilited patients in the acute stroke period it often cannot be achieved only by traditional physiotherapy exercises. Purpose: To assess the efficiency of use of robotic device Gait Trainer in rehabilitation of walking in patients with an acute stroke. Methods: 73 patients were randomized into 2 groups: 50 in GT-group and 23 in non-GT-group. All patients received ontogenetic caused kinesitherapy within 3 weeks. In addition GT-group patients were trained to walk on a robotic device Gait Trainer within 2 weeks. Main outcome measures: the neurologic status, including muscular strength and muscular tone, sensation and coordination, Berg balance test, functional ambulation categories (FAC), 5-metre walk test, Bartel index (BI). Results: In both groups by the end of 3rd week muscular force, data of Berg balance test, FAC and BI have significantly increased (p<0.05). In comparison with non-GT-group the best indicators of restoration were in GT-group (p <0.001), in this group abnormalities of proprioceptive sensation and ataxia significantly regressed (p<0.001), speed of walking was above average (30±10.4 m/min vs 15±4.7 m/min, p <0.05), patients used an additional support less often (p<0.05). Conclusions: Both application of robotic device Gait Trainer and ontogenetic caused kinesitherapy lead to significant improvement of muscular force and functionality of acute stroke patients. However trainings on Gait Trainer promote more complex rehabilitation, including regress of proprioceptive sensation abnormalities and ataxia, that, finally, leads to the best functional results.
Improvements in dose efficiency with high resolution scan modes in photon counting CT

Abstract:For the detection of very small objects, high resolution detectors are expected to provide higher dose efficiency. We assessed this impact of increased resolution on a clinical photon counting detector CT (PCD-CT) by comparing its detectability in high resolution and standard resolution (with 2×2 binning and larger focal spot) modes. A 50μm-thin metal wire was placed in a thorax phantom and scanned in both modes at three exposure levels (12, 15, and 18 mAs); acquired data were reconstructed with three reconstruction kernels (Br40, Br68, and Br76, from smooth to sharp). A scanning nonprewhitening model observer searched for the wire location within each slice independently. Detection performance was quantified as area under the exponential transform of the free response ROC curve. The high-resolution mode had the mean AUCs at 18 mAs of 0.45, 0.49, and 0.65 for Br40, Br68, and Br76, respectively, which were 2 times, 3.6 times, and 4.6 times those of the standard resolution mode. The high-resolution mode achieved greater AUC at 12 mAs than the standard resolution mode at 18 mAs for every reconstruction kernel, but improvements were larger at sharper kernels. The results are consistent with the greater suppression of noise aliasing expected at higher frequencies with high resolution CT. This work illustrates that PCD-CT can provide large dose efficiency gains for detection tasks of small, high contrast lesions.

Abstract:Objectives Image acquisition in ultra-high-resolution (UHR) scan mode does not impose a dose penalty in photon-counting CT (PCCT). This study aims to investigate the dose saving potential of using UHR instead of standard-resolution PCCT for lumbar spine imaging. Materials and Methods Eight cadaveric specimens were examined with 7 dose levels (5–35 mGy) each in UHR (120 × 0.2 mm) and standard-resolution acquisition mode (144 × 0.4 mm) on a first-generation PCCT scanner. The UHR images were reconstructed with 3 dedicated bone kernels (Br68 [spatial frequency at 10% of the modulation transfer function 14.5 line pairs/cm], Br76 [21.0], and Br84 [27.9]), standard-resolution images with Br68 and Br76. Using automatic segmentation, contrast-to-noise ratios (CNRs) were established for lumbar vertebrae and psoas muscle tissue. In addition, image quality was assessed subjectively by 19 independent readers (15 radiologists, 4 surgeons) using a browser-based forced choice comparison tool totaling 16,974 performed pairwise tests. Pearson's correlation coefficient ( r ) was used to analyze the relationship between CNR and subjective image quality rankings, and Kendall W was calculated to assess interrater agreement. Results Irrespective of radiation exposure level, CNR was higher in UHR datasets than in standard-resolution images postprocessed with the same reconstruction parameters. The use of sharper convolution kernels entailed lower CNR but higher subjective image quality depending on radiation dose. Subjective assessment revealed high interrater agreement ( W = 0.86; P < 0.001) with UHR images being preferred by readers in the majority of comparisons on each dose level. Substantial correlation was ascertained between CNR and the subjective image quality ranking (all r 's ≥ 0.95; P < 0.001) Conclusions In PCCT of the lumbar spine, UHR mode's smaller pixel size facilitates a considerable CNR increase over standard-resolution imaging, which can either be used for dose reduction or higher spatial resolution depending on the selected convolution kernel.

Abstract:We report a comprehensive evaluation of a full field-of-view (FOV) photon-counting detector (PCD) computed tomography (CT) system using phantoms, and qualitatively assess image quality in patient examples. A whole-body PCD-CT system with 50 cm FOV, 5.76 cm z-detector coverage and two acquisition modes (standard: 144 × 0.4 mm collimation and ultra-high resolution (UHR): 120 × 0.2 mm collimation) was used in this study. Phantoms were scanned to assess image uniformity, CT number accuracy, noise power spectrum, spatial resolution, material decomposition and virtual monoenergetic imaging (VMI) performance. Four patients were scanned on the PCD-CT system with matched or lower radiation dose than their prior clinical CT scans performed using energy-integrating detector (EID) CT, and the potential clinical impact of PCD-CT was qualitatively evaluated. Phantom results showed water CT numbers within ±5 HU, and image uniformity measured between peripheral and central regions-of-interests to be within ±5 HU. For the UHR mode using a dedicated sharp kernel, the cut-off spatial frequency was 40 line-pairs cm−1, which corresponds to a 125 μm limiting in-plane spatial resolution. The full-width-at-half-maximum for the section sensitivity profile was 0.33 mm for the smallest slice thickness (0.2 mm) using the UHR mode. Material decomposition in a multi-energy CT phantom showed accurate material classification, with a root-mean-squared-error of 0.3 mg cc−1 for iodine concentrations (2–15 mg cc−1) and 14.2 mg cc−1 for hydroxyapatite concentrations (200 and 400 mg cc−1). The average percent error for CT numbers corresponding to the iodine concentrations in VMI (40–70 keV) was 2.75%. Patient PCD-CT images demonstrated better delineation of anatomy for chest and temporal bone exams performed with the UHR mode, which allowed the use of very sharp kernels not possible with EID-CT. VMI and virtual non-contrast images generated from a patient head CT angiography exam using the standard acquisition mode demonstrated the multi-energy capability of the PCD-CT system.

Abstract:Abstract Objectives Photon-counting detector CT (PCD-CT) promises a leap in spatial resolution due to smaller detector pixel sizes than implemented in energy-integrating detector CTs (EID-CT). Our objective was to compare the visualization of smallest bone details between PCD-CT and EID-CT using a mouse as a specimen. Materials and methods Two euthanized mice were scanned at a 20-slice EID-CT and a dual-source PCD-CT in single-pixel mode at various CTDI Vol values. Image noise and signal-to-noise ratio (SNR) were evaluated using repeated ROI measurements. Edge sharpness of bones was compared by the maximal slope within CT value plots along sampling lines intersecting predefined bones of the spine. Two readers evaluated bone detail visualization at four regions of the spine on a three-point Likert scale at various CTDI Vol ’s. Two radiologists selected the series with better detail visualization among each of 20 SNR-matched pairs of EID-CT and PCD-CT series. Results In CTDI Vol -matched scans, PCD-CT series showed significantly lower image noise (Noise CTDI = 5 mGy : 16.27 ± 1.39 vs. 23.46 ± 0.96 HU, p < 0.01), higher SNR (SNR CTDI = 5 mGy : 20.57 ± 1.89 vs. 14.00 ± 0.66, p < 0.01), and higher edge sharpness (Edge Slope lumbar spine : 981 ± 160 vs. 608 ± 146 HU/mm, p < 0.01) than EID-CT series. Two radiologists considered the delineation of bone details as feasible at consistently lower CTDI Vol values at PCD-CT than at EID-CT. In comparison of SNR-matched reconstructions, PCD-CT series were still considered superior in almost all cases. Conclusions In this head-to-head comparison, PCD-CT showed superior objective and subjective image quality characteristics over EID-CT for the delineation of tiniest bone details. Even in SNR-matched pairs (acquired at different CTDI Vol ’s), PCD-CT was strongly preferred by radiologists. Key Points • In dose-matched scans, photon-counting detector CT series showed significantly less image noise, higher signal-to-noise ratio, and higher edge sharpness than energy-integrating detector CT series . • Human observers considered the delineation of tiny bone details as feasible at much lower dose levels in photon-counting detector CT than in energy-integrating detector CT . • In direct comparison of series matched for signal-to-noise ratio, photon-counting detector CT series were considered superior in almost all cases .

Abstract:ABSTRACT: After a decade of preclinical testing, photon-counting computed tomography (PCCT) has now entered daily routine, enabling radiologists to start investigating thoracic disorders in unprecedented conditions. The improved spatial resolution of the ultra-high-resolution (UHR) scanning mode is a major step for the analysis of bronchopulmonary disorders, making abnormalities at the level of small anatomical structures such as secondary pulmonary lobules accessible to radiologists. Distal divisions of pulmonary and systemic vessels also benefit from UHR protocols as alterations of lung microcirculation were previously excluded from confident analysis with energy-integrating detector CT. Although noncontrast chest CT examinations were the initial target of UHR protocols, the clinical value of this mode is also applicable to chest CT angiographic examinations with improved morphological evaluation and higher-quality lung perfusion imaging. The clinical benefits of UHR have been evaluated in initial studies, allowing radiologists to foresee the field of future applications, all combining high diagnostic value and radiation dose reduction. The purpose of this article is to highlight the technological information relevant to daily practice and to review the current clinical applications in the field of chest imaging.

Abstract:This study evaluated the conventional imaging performance of a research whole-body photon-counting CT system and investigated its feasibility for imaging using clinically realistic levels of x-ray photon flux. This research system was built on the platform of a 2nd generation dual-source CT system: one source coupled to an energy integrating detector (EID) and the other coupled to a photon-counting detector (PCD). Phantom studies were conducted to measure CT number accuracy and uniformity for water, CT number energy dependency for high-Z materials, spatial resolution, noise, and contrast-to-noise ratio. The results from the EID and PCD subsystems were compared. The impact of high photon flux, such as pulse pile-up, was assessed by studying the noise-to-tube-current relationship using a neonate water phantom and high x-ray photon flux. Finally, clinical feasibility of the PCD subsystem was investigated using anthropomorphic phantoms, a cadaveric head, and a whole-body cadaver, which were scanned at dose levels equivalent to or higher than those used clinically. Phantom measurements demonstrated that the PCD subsystem provided comparable image quality to the EID subsystem, except that the PCD subsystem provided slightly better longitudinal spatial resolution and about 25% improvement in contrast-to-noise ratio for iodine. The impact of high photon flux was found to be negligible for the PCD subsystem: only subtle high-flux effects were noticed for tube currents higher than 300 mA in images of the neonate water phantom. Results of the anthropomorphic phantom and cadaver scans demonstrated comparable image quality between the EID and PCD subsystems. There were no noticeable ring, streaking, or cupping/capping artifacts in the PCD images. In addition, the PCD subsystem provided spectral information. Our experiments demonstrated that the research whole-body photon-counting CT system is capable of providing clinical image quality at clinically realistic levels of x-ray photon flux.

The American Medical Association (AMA) is reviewing some of the current problems and pressures plaguing clinical research with an eye toward how they might best be managed. Its Council on Scientific Affairs (CSA) held a 2-day conference on the topic in Washington, DC, and expects to develop an action plan to encourage new ways of stimulating this vital area of medical progress. Abstract:Purpose The purpose of this study was to compare lung image quality obtained with ultra-high resolution (UHR) spectral photon-counting CT (SPCCT) with that of dual-layer CT (DLCT), at standard and low dose levels using an image quality phantom and an anthropomorphic lung phantom. Methods An image quality phantom was scanned using a clinical SPCCT prototype and an 8 cm collimation DLCT from the same manufacturer at 10 mGy. Additional acquisitions at 6 mGy were performed with SPCCT only. Images were reconstructed with dedicated high-frequency reconstruction kernels, slice thickness between 0.58 and 0.67 mm, and matrix between 512 2 and 1024 2 mm, using a hybrid iterative algorithm at level 6. Noise power spectrum (NPS), task-based transfer function (TTF) for iodine and air inserts, and detectability index ( d' ) were assessed for ground-glass and solid nodules of 2 mm to simulate highly detailed lung lesions. Subjective analysis of an anthropomorphic lung phantom was performed by two radiologists using a five-point quality score. Results At 10 mGy, noise magnitude was reduced by 29.1 % with SPCCT images compared to DLCT images for all parameters (27.1 ± 11.0 [standard deviation (SD)] HU vs. 38.2 ± 1.0 [SD] HU, respectively). At 6 mGy with SPCCT images, noise magnitude was reduced by 8.9 % compared to DLCT images at 10 mGy (34.8 ± 14.1 [SD] HU vs. 38.2 ± 1.0 [SD] HU, respectively). At 10 mGy and 6 mGy, average NPS spatial frequency (f av ) was greater for SPCCT images (0.75 ± 0.17 [SD] mm -1 ) compared to DLCT images at 10 mGy (0.55 ± 0.04 [SD] mm -1 ) while remaining constant from 10 to 6 mGy. At 10 mGy, TTF at 50 % (f 50 ) was greater for SPCCT images (0.92 ± 0.08 [SD] mm -1 ) compared to DLCT images (0.67 ± 0.06 [SD] mm -1 ) for both inserts. At 6 mGy, f 50 decreased by 1.1 % for SPCCT images, while remaining greater compared to DLCT images at 10 mGy (0.91 ± 0.06 [SD] mm -1 vs. 0.67 ± 0.06 [SD] mm -1 , respectively). At both dose levels, d' were greater for SPCCT images compared to DLCT for all clinical tasks. Subjective analysis performed by two radiologists revealed a greater median image quality for SPCCT (5; Q1, 4; Q3, 5) compared to DLCT images (3; Q1, 3; Q3, 3). Conclusion UHR SPCCT outperforms DLCT in terms of image quality for lung imaging. In addition, UHR SPCCT contributes to a 40 % reduction in radiation dose compared to DLCT.

Abstract:Photon-counting detector (PCD) CT is a new CT technology utilizing a direct conversion X-ray detector, where incident X-ray photon energies are directly recorded as electronical signals. The design of the photon-counting detector itself facilitates improvements in spatial resolution (via smaller detector pixel design) and iodine signal (via count weighting) while still permitting multi-energy imaging. PCD-CT can eliminate electronic noise and reduce artifacts due to the use of energy thresholds. Improved dose efficiency is important for low dose CT and pediatric imaging. The ultra-high spatial resolution of PCD-CT design permits lower dose scanning for all body regions and is particularly helpful in identifying important imaging findings in thoracic and musculoskeletal CT. Improved iodine signal may be helpful for low contrast tasks in abdominal imaging. Virtual monoenergetic images and material classification will assist with numerous diagnostic tasks in abdominal, musculoskeletal, and cardiovascular imaging. Dual-source PCD-CT permits multi-energy CT images of the heart and coronary arteries at high temporal resolution. Quantitative assessment of motion effects in dual-source dual energy CT and dual-source photon-counting detector CT

Abstract:Conventional dual-source CT scanners can be used to either provide better temporal resolution or dual-energy imaging, but not both at the same time. This presents a dilemma in cardiac CT as both high temporal resolution and multi-energy imaging are desirable. The current study evaluated a dual-source photon-counting-detector (DS-PCD) CT which can acquire multi-energy images at high temporal resolution. A cardiac motion phantom with a 3-mm diameter iodinated rod, mimicking the right coronary artery, was scanned 25 times using a DS-PCD CT (66 ms resolution) and a dual-source dual-energy (DS-DE, 125 ms resolution) CT. Low/high energy images and iodine maps were reconstructed at 40% and 75% cardiac phases. To quantify the impact of motion on image quality, dice similarity coefficient was computed between the low/high energy images while the circularity and effective diameter of the iodinated rod were computed on the iodine maps. The dice coefficients were higher for DS-PCD with a mean of 0.89 and 0.91 at the 40% and 70% phases, while DS-DE had a lower mean of 0.20 and 0.78, respectively. The circularity was excellent for DS-PCD with a mean of 0.97 and 0.98 at the 40% and 75% phases, while DS-DE had a mean of 0.71 and 0.98, respectively. The effective diameter was accurate for DS-PCD with a mean of 2.9 mm (true size of 3 mm) at both phases, while DS-DE had a mean of 4.0 mm and 3.2 mm at the 40% and 75% phases, respectively. These results indicate that DS-PCD CT enables simultaneous high temporal resolution and multi-energy cardiac imaging with minimal motion artifacts.

Abstract:Photon-counting detectors (PCDs) have emerged as one of the most influential technical developments for medical imaging in recent memory. Surpassing conventional systems with energy-integrating detector technology in many aspects, PCD-CT scanners provide superior spatial resolution and dose efficiency for all radiological subspecialities. Demanding detailed display of trabecular microarchitecture and extensive anatomical coverage frequently within the same scan, musculoskeletal (MSK) imaging in particular can be a beneficiary of PCD-CT's remarkable performance. Since PCD-CT provides users with a plethora of customization options for both image acquisition and reconstruction, however, MSK radiologists need to be familiar with the scanner to unlock its full potential. From filter-based spectral shaping for artifact reduction over full field-of-view ultra-high-resolution scans to postprocessing of single- or dual-source multienergy data, almost every imaging task can be met with an optimized approach in PCD-CT. The objectives of this review were to give an overview of the most promising applications of PCD-CT in MSK imaging to date, to state current limitations, and to highlight directions for future research and developments.
Dual energy CT kidney stone differentiation in photon counting computed tomography

Abstract:This study evaluates the capabilities of a whole-body photon counting CT system to differentiate between four common kidney stone materials, namely uric acid (UA), calcium oxalate monohydrate (COM), cystine (CYS),and apatite (APA) ex vivo. Two different x-ray spectra (120 kV and 140 kV) were applied and two acquisition modes were investigated; The macro-mode generates two energy threshold based image-volumes and two energy bin based image-volumes. In the chesspattern-mode, however, four energy thresholds are applied. A virtual low energy image, as well as a virtual high energy image are derived from initial threshold-based images, while considering their statistically correlated nature. The energy bin based images of the macro-mode, as well as the virtual low and high energy image of the chesspattern-mode serve as input for our dual energy evaluation. The dual energy ratio of the individually segmented kidney stones were utilized to quantify the discriminability of the different materials. The dual energy ratios of the two acquisition modes showed high correlation for both applied spectra. Wilcoxon-rank sum tests and the evaluation of the area under the receiver operating characteristics curves suggest that the UA kidney stones are best differentiable from all other materials (AUC = 1.0), followed by CYS (AUC ≈ 0.9 compared against COM and APA). COM and APA, however, are hardly distinguishable (AUC between 0.63 and 0.76). The results hold true for the measurements of both spectra and both acquisition modes.
Simultaneous high‐pitch multi‐energy CT pulmonary angiography using a dual‐source photon‐counting‐detector CT: A phantom experiment

Abstract:Purpose A dual‐source CT system can be operated in a high‐pitch helical mode to provide a temporal resolution of 66 ms, which reduces motion artifacts in CT pulmonary angiography (CTPA). It can also be operated in a multi‐energy (ME) mode to provide iodine maps, beneficial in the evaluation of pulmonary embolism (PE). No energy‐integrating detector (EID) CT can perform simultaneous ME and high‐pitch acquisition. This phantom study aimed to evaluate the ability of a photon‐counting‐detector (PCD) CT to perform simultaneous high‐pitch and ME imaging for CTPA. Methods A motion phantom was used to mimic the respiratory motion. Two tubes filled with iodine with intravascular thrombus mimicked by injecting glue within the tubes were placed along with 5, 10, and 15 mg/mL iodine samples, on a motion phantom at 20 and 30 revolutions per minute. Separate high‐pitch and ME EID‐CT scans and a single high‐pitch ME PCD scan were acquired and virtual monoenergetic images and iodine maps reconstructed. Percent thrombus occlusion was measured and compared between static and moving images. Results When there was motion, EID‐CT ME suffered from significant motion artifacts. The measured iodine concentrations with PCD‐CT in high‐pitch ME were more stable when there was a motion, with a lower standard deviation than EID‐CT in ME mode. The estimated percent thrombus occlusion dropped significantly with applied motion on EID‐CT, while PCD‐CT high‐pitch ME mode showed good agreement between measurements on static or moving images. Conclusion PCD‐CT with combined ME and high‐pitch mode facilitates simultaneous accurate iodine quantification and assessment of intravascular occlusion.

Ultra-high Versus Standard Resolution Photon-Counting Detector CT Angiography for Imaging of Femoral Stents in a Cadaveric Perfusion Model

Abstract:Background and aims: The aim of this study was to investigate the imaging performance and quality differences of PCD-CT in standard resolution mode (SR) versus ultra-high resolution mode (UHR) in the lower extremity runoff of dose-matched CTAs in a human cadaveric model. Methods: Extracorporeal perfusion of the upper leg was established in one fresh-frozen human cadaver via inguinal and popliteal accesses using a peristaltic pump. Seven peripheral stents were deployed in the SFA. Photon-counting CTAs were performed under contrast perfusion in SR and UHR mode with dose-equivalent 120kVp acquisition protocols (low-/ medium-/ high-dose: CTDIVol=3, 5, 10 mGy) and reconstructed with four vascular convolution kernels. Lumen visibility and contrast-to-noise ratio were compared using analyses of variance. Subjective image quality was assessed using a pairwise, forced-choice comparison software. Results: Lumen visibility was equal for SR and UHR at the used dose levels. CNR increase by UHR was significant for (ultra-)sharp convolution kernels BV60 (3 mGy; UHR vs. SR, 19.9 ± 1.9 vs. 15.7 ± 1.6, p < 0.046) and BV76 (8.0 ± 0.6 vs. 5.4 ± 0.3, p < 0.001). The relative CNR increase was higher for low-dose than high-dose scans (BV76: 48% vs. 36% at high dose, p < 0.033). The CNR of the low-dose scan in UHR mode was comparable to the high-dose scan in SR mode when the ultra-sharp kernel was used (8.0 ± 0.6 vs. 9.1 ± 1.1, p > 0.760). Among UHR examinations, a significant increase in CNR could only be measured in BV76 (8.0 ± 0.6 (3 mGy) vs. 12.4 ± 0.9 (10 mGy), p < 0.001). Readers preferred subjective image quality of UHR for all kernels with BV76 being ranked highest. Conclusion: The CNR increase in UHR mode is highest when combining low radiation dose and ultra-sharp reconstructions. Meanwhile, the subjective image quality in UHR mode generally supersedes SR images, suggesting further dose reduction potential.

Abstract:Objective Photon counting CT (PCCT) has been a research focus in the last two decades. Recent studies and advancements have demonstrated that systems using semiconductor-based photon counting detectors (PCDs) have the potential to provide better contrast, noise and spatial resolution performance compared to conventional scintillator-based systems. With multi-energy threshold detection, PCD can simultaneously provide the photon energy measurement and enable material decomposition for spectral imaging. In this work, we report a performance evaluation of our first CdZnTe-based prototype full-size photon counting CT system through various phantom imaging studies. Approach This prototype system supports a 500 mm scan field-of-view (FOV) and 10 mm z-coverage at isocenter. Phantom scans were acquired using 120 kVp from 50 to 400 mAs to assess the imaging performance on: CT number accuracy, uniformity, noise, spatial resolution, material differentiation and quantification. Main Results Both qualitative and quantitative evaluations show that PCCT, under the tested conditions, has superior imaging performance with lower noise and improved spatial resolution compared to conventional energy integrating detector (EID)-CT. Using projection domain material decomposition approach with multiple energy bin measurements, PCCT virtual monoenergetic images (VMIs) have lower noise, and good accuracy in quantifying iodine and calcium concentrations. These results lead to increased contrast-to-noise ratio (CNR) for both high and low contrast study objects compared to EID-CT at matched dose and spatial resolution. PCCT can also generate super-high resolution (SHR) images using much smaller detector pixel size than EID-CT and greatly improve image spatial resolution. Significance Improved spatial resolution and quantification accuracy with reduced image noise of the PCCT images can potentially lead to better diagnosis at reduced radiation dose compared to conventional EID-CT. Increased CNR achieved by PCCT suggests potential reduction in iodine contrast media load, resulting in better patient safety and reduced cost.

Abstract:Abstract An evaluation of energy thresholding and acquisition mode for metal artifact reduction in Photon-counting detector CT (PCD-CT) compared to conventional energy-integrating detector CT (EID-CT) was performed. Images of a hip prosthesis phantom placed in a water bath were acquired on a scanner with PCD-CT and EID-CT (tube potentials: 100, 120 and 140 kV p ) and energy thresholds (above 55–75 keV) in Macro and Chess mode. Only high-energy threshold images (HTI) were used. Metal artifacts were quantified by a semi-automated segmentation algorithm, calculating artifact volumes, means and standard deviations of CT numbers. Images of a human cadaver with hip prosthesis were acquired on the PCD-CT in Macro mode as proof-of-concept. Images at 140 kV p showed less metal artifacts than 120 kV p or 100 kV p . HTI (70, 75 keV) had fewer artifacts than low energy thresholds (55, 60, 65 keV). Fewer artifacts were observed in the Macro-HTI (8.9–13.3%) for cortical bone compared to Chess-HTI (9.4–19.1%) and EID-CT (10.7–19.0%) whereas in bone marrow Chess-HTI (19.9–45.1%) showed less artifacts compared to Macro-HTI (21.9–38.3%) and EID-CT (36.4–54.9%). Noise for PCD-CT (56–81 HU) was higher than EID-CT (33–36 HU) irrespective of tube potential. High-energy thresholding could be used for metal artifact reduction in PCD-CT, but further investigation of acquisition modes depending on target structure is required.

Abstract:Preclinical micro-CT provides a hotbed in which to develop new imaging technologies, including spectral CT using photon counting detector (PCD) technology. Spectral imaging using PCDs promises to expand x-ray CT as a functional imaging modality, capable of molecular imaging, while maintaining CT's role as a powerful anatomical imaging modality. However, the utility of PCDs suffers due to distorted spectral measurements, affecting the accuracy of material decomposition. We attempt to improve material decomposition accuracy using our novel hybrid dual-source micro-CT system which combines a PCD and an energy integrating detector. Comparisons are made between PCD-only and hybrid CT results, both reconstructed with our iterative, multi-channel algorithm based on the split Bregman method and regularized with rank-sparse kernel regression. Multi-material decomposition is performed post-reconstruction for separation of iodine (I), gold (Au), gadolinium (Gd), and calcium (Ca). System performance is evaluated first in simulations, then in micro-CT phantoms, and finally in an in vivo experiment with a genetically modified p53fl/fl mouse cancer model with Au, Gd, and I nanoparticle (NP)-based contrasts agents. Our results show that the PCD-only and hybrid CT reconstructions offered very similar spatial resolution at 10% MTF (PCD: 3.50 lp mm−1; hybrid: 3.47 lp mm−1) and noise characteristics given by the noise power spectrum. For material decomposition we note successful separation of the four basis materials. We found that hybrid reconstruction reduces RMSE by an average of 37% across all material maps when compared to PCD-only of similar dose but does not provide much difference in terms of concentration accuracy. The in vivo results show separation of targeted Au and accumulated Gd NPs in the tumor from intravascular iodine NPs and bone. Hybrid spectral micro-CT can benefit nanotechnology and cancer research by providing quantitative imaging to test and optimize various NPs for diagnostic and therapeutic applications.
