Abstract:Clinical Psychological Science, Ahead of Print. Reward devaluation theory posits that depressed individuals avoid and devalue positivity, suggesting that they may hold fewer positive self-schemas. Previous meta-analytic reviews have supported this theoretical framework regarding positivity but have not assessed for self-referential stimuli. Self-referential encoding and recall tasks assess for self-schemas and thus provide further insight into how depressed individuals process self-referential positivity. The aim of this systematic review and meta-analysis was to examine the extent to which depressed individuals differ in processing self-referential positivity and negativity and whether this processing differs when depressed individuals think of others (i.e., other-referential). Results indicate that depressed individuals recall and endorse fewer self-referential positive words than negative words and fewer self-referential positive words than other-referential positive words than nondepressed individuals. These findings support reward devaluation theory and suggest that conceptualizing self-referential processing in depression as merely based on negativity biases can overlook crucial information about how depressed individuals devalue self-referential positive information.

Abstract:Negative attentional bias has been well established in depression. However, there is very limited knowledge about whether this depression-relevant negative bias exits during initial attentional allocation, as compared with the converging evidence for the negative bias during sustained attention engagement. This study used both behavioral and electrophysiological measures to examine the initial attention engagement in depressed patients influenced by mood-congruent and mood-incongruent emotions. The dot-probe task was performed with a 100-ms exposure time of the emotional cues (emotional and neutral face pairs). The behavioral results showed that the patients responded faster following valid compared with invalid sad facial cues. Electrophysiological indexes in the frame of the two-stage model of attentional modulation by emotions provided cognitive mechanisms in distinct attention engagement stages: (1) the patients exhibited reduced P1 amplitudes following validly than invalidly happy cues than did the healthy controls, indicating a positive attenuation at an early stage of automatic attention orientation; and (2) the patients exhibited enhanced whereas the controls showed reduced P3 amplitudes following validly than invalidly sad cues, suggesting a mood-congruent negative potentiation in depression at the later stage of top-down voluntary control of attention. Depressed patients show a negative bias in early attentional allocation, reflected by preferred engagement with mood-congruent and diminished engagement with positive emotional cues/stimuli.

Exploring Attention Bias Mechanisms in Sub-Threshold Depression: ERP Insights into Biased Orientation and Disengagement

Abstract:Individuals with depression may have alterations in attention that begin at the sub-threshold stage. This study explored attention bias from the perspectives of early attention orientation and late attention disengagement in individuals with sub-threshold depression (SD) and healthy control (HC) individuals using a cue-target paradigm and event-related potentials (ERPs). The study enrolled 46 participants, comprising 23 males and 23 females, with 25 individuals in the SD group and 21 in the HC group, exceeding the calculated sample size requirement. The data were analyzed from two aspects. Behavioral data showed that SD individuals had difficulty in attention disengagement and that the time of attention transfer was delayed. Analysis of ERP data revealed that, regardless of the attributes of the emotional stimulus, the cue information promoted participants' response to the target stimulus. While SD individuals did show directional acceleration of attention to the emotional stimulus, no significant negative attention bias was observed. Taken together, these findings suggest that SD individuals do not show specific directional acceleration of attention to negative stimuli in the early stage of attention processing, although there may be attention avoidance.

Abstract:Individuals with major depressive disorder (MDD) exhibit attentional biases toward negative, mood-congruent stimuli while filtering out positive and neutral stimuli, resulting in memory biases to negative content. While attentional and memory biases in MDD have been extensively studied, the underlying mechanisms of these biases remain unclear. The current study investigates a novel model proposing that exposure to negative emotional cues triggers a transient "attentional window" in individuals with MDD, leading to heightened and deeper cognitive processing of any subsequent information, irrespective of its content. Forty-two unmedicated patients with MDD and no comorbid disorder and 41 healthy controls, completed six blocks of the emotional memory task, in which they were asked to watch a short video (negative, neutral, or positive valence) followed by a memory test on a list of neutral or positive valance words. Results indicated that participants with MDD, but not healthy controls, had better recall performance after a negative video compared to after neutral or positive videos, and that this effect occurred for both neutral and positive word-lists. These findings provide evidence that participants with MDD engage in deeper information processing following exposure to negative emotional stimuli. Potential clinical implications are discussed.

Abstract:Background: The aim of this study was to improve our understanding of the underlying mechanisms in the maintenance of depression. We examined attentional bias (AB) for negative and positive adjectives and general threat words in strictly-defined clinical groups of participants with pure Major Depressive Disorder (MDD) without a history of anxiety disorders (AD), mixed MDD and AD, and remitted participants. Method: We investigated both stimulus specificity and time course of AB in these groups, adopting a cross-sectional design. Data were drawn from the large scale Netherlands Study of Depression and Anxiety (NESDA), from which we selected all participants with pure current MDD without a history of AD (n = 29), all participants with current MDD and co-morbid AD(s) (n = 86), all remitted MDD participants (n = 294), and a comparison group without (a history of) MDD or ADs (n = 474). AB was measured with an Exogenous Cueing Task covering short and long presentation times (500 and 1250 ms) and 4 stimulus types (negative, positive, threat, neutral). Results: Both traditional and trial level (dynamic) AB scores failed to show an AB for negative adjectives in participants with MDD or mixed MDD/AD. Specifically for long duration trials (1250 ms), remitted participants showed a larger AB traditional score (albeit the actual score still being negative) than the comparison group. The mixed MDD/AD group showed a higher trial-level AB score away from positive adjectives (1250 ms) than the comparisons. In addition, the mixed MDD/AD group showed higher and more variable trial-level AB scores away from short and towards longer presented general threat words together with a non-significant tendency to show less negative traditional AB scores for threat trials (500 ms) than the comparison group. Conclusions: All in all, the findings do not corroborate the view that an AB towards negative or away from positive adjectives is critically involved in currently depressed individuals. Yet, the relatively high (less negative) AB score for negative adjectives in remitted individuals points to the possibility that an AB for negative information may be involved as a risk factor in the recurrence of MDD.

Abstract:Objectives: Depression and ADHD often co-occur and are both characterized by altered attentional processing. Differences and overlap in the profile of attention to emotional information may help explain the co-occurence. We examined negative attention bias in ADHD as neurocognitive marker for comorbid depression. Methods: Patients with depression ( n = 63), ADHD ( n = 43), ADHD and depression ( n = 25), and non-psychiatric controls ( n = 68) were compared on attention allocation toward emotional faces. The following eye-tracking indices were used: gaze duration, number of revisits, and location and duration of first fixation. Results: Controls revisited the happy faces more than the other facial expressions. Both the depression and the comorbid group showed significantly less revisits of the happy faces compared to the ADHD and the control group. Interestingly, after controlling for depressive symptoms, the groups no longer differed on the number of revisits. Conclusion: ADHD patients show a relative positive attention bias, while negative attention bias in ADHD likely indicates (sub)clinical comorbid depression.

Abstract:Past research suggests that depressed individuals are less likely than non-depressed individuals to engage in mood-incongruent recall in response to negative mood and do not experience associated mood reparative effects. The present study examined the effects of adopting a reflective versus ruminative self-focus orientation towards one's mood on the valence of autobiographical memories recalled following a negative mood induction and the extent of mood repair following memory recall among individuals with varying depressive symptomatology. Participants underwent a negative mood induction and either a ruminative (n = 69) or reflective (n = 49) self-focus manipulation, and then recalled five specific autobiographical memories. Depression symptoms were associated with recall of less positive memories and reduced mood repair. The valence of recalled memories was associated with the extent of mood improvement, and depressive symptoms did not moderate this association. Contrary to our hypothesis, a reflective self-focus was not associated with recall of more positive memories or greater mood improvement than a ruminative self-focus. The results suggest that more depressed individuals are less likely to spontaneously engage in mood-incongruent recall in a negative mood state; however, recall of positive memories is associated with similar mood reparative effects regardless of depressive symptomatology.

Abstract:Previous research suggests a close relationship between self-reference and emotional valence. The present study investigated potential differences in cognitive resources required for positive vs. negative self-referential processing by examining how self/other-referential processing of positive/negative information affects memory for subsequently presented items. On each encoding trial, participants first judged whether a positive or negative trait adjective described themselves or another person. Then, they were shown a neutral noun and indicated its screen location. Subsequent memory tests showed better memory for self-referenced than other-referenced trait adjectives, and the size of this self-reference effect was not modulated by emotional valence. Although memory for nouns was not affected by preceding positive/negative self/other-referential processing, memory for their associated contextual features was significantly impaired following negative vs. positive self-referential processing. Our findings suggest that negative self-referential processing requires more cognitive resources than positive self-referential processing, thereby leaving relatively less cognitive resources to encode subsequently presented information.

Abstract:Background: This study examined whether automatic attention bias toward emotional stimuli differs among individuals with varying anxiety and depression levels in the non-clinical population.Methods: A total of 494 participants from the community completed the Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7). The dot-probe task was conducted to compare attention biases when emotional stimuli appeared before the target versus neutral stimuli did.Results: There were differences in attention bias based on the stimuli types, even among the non-clinical community. The anxiety tendency group showed attention bias towards positive expressions, while the depression tendency group showed attention bias toward negative expressions. The group with anxiety and depression tendencies showed mixed characteristics observed in both the anxiety and depression groups.Conclusions: This study showed that attention bias toward emotional stimuli is an information-processing bias influenced by an individual’s internal state. This study contributes to the understanding of attention bias differences among groups based on anxiety and depression tendencies in non-clinical population of various ages.

Abstract:Substantial research indicates that individuals with Major Depressive Disorder (MDD) remember more negative information compared to neutral and positive information. This phenomenon is commonly attributed to attentional biases toward negative over neutral and positive information. A recent attentional resources model suggests that in MDD, negative cues not only capture attention, but also lead to deeper processing of subsequent information, irrespective of its content. This study aimed to replicate findings supporting this attentional resources model and go beyond it by investigating the effect of negative cues on encoding and retrieval processes. Forty-one participants with MDD and no comorbid diagnoses, and 42 healthy-controls completed the emotional recall task with negative or positive videos presented during encoding and retrieval stages of a neutral word-list memory test. During encoding, only the MDD group exhibited a difference between negative and positive videos, such that for negative videos memory recall was improved and for positive words it was reduced. Emotional videos had no effect when presented during retrieval. These results suggest that in MDD, encountering emotional cues not only biases retrieval processes toward recalling more negative content, but rather fundamentally alters the depth of information processing, while not leading to a broad-spectrum recruitment of cognitive resources.

Abstract:Background and objectives: Self-referent information is critical for navigating the social realm, as we constantly use both verbal and non-verbal feedback in our interactions to understand ourselves and the world. In non-clinical samples, a memory bias for positive self-referent information has been observed, while a negativity bias has been observed among those with depression and anxiety. While research suggests that visual and auditory information is processed differently, no study has yet examined if memory biases persist for self-referent information presented by either means. We examined differences in memory for self-relevant social information presented as verbal or facial feedback, and whether symptoms of depression or anxiety influence memory for such information. We predicted that participants would remember more positive feedback overall, and that depression and anxiety would be positively related to memory for negative items. Methods: Participants gave a speech, and were provided with positive and negative feedback via facial expressions, (n = 25) or verbal feedback presented aurally (n = 26). Participants then did a recognition test for the feedback they recieved. Results: Recognition was higher for negative compared to positive feedback in the verbal condition, regardless of depression or anxiety. No memory biases were observed in the facial feedback condition. Limitations: No neutral stimuli was presented. Conclusions: Findings suggest that the type of social feedback one receives may influence how information is remembered, regardless of symptomatology. Future studies should examine the mechanisms by which memory biases exist for different types of self-relevant feedback.

Abstract:Cognitive vulnerabilities, such as a negative self-referential processing bias, have been theorized to play a causal role in the development of depression. Indeed, depression is associated with the endorsement and recall of more negative and fewer positive emotional words (i.e., recall biases) in the self-referential encoding task (SRET). In addition, currently depressed adults and adolescents, compared to healthy controls, show an enhanced late positive potential (LPP), an event-related potential (ERP) component that reflects sustained attentional engagement, during the processing of negative relative to positive words in the SRET. However, it is unclear whether these behavioral and neural measures in the SRET are indicators of risk for depression, or are concomitants of the disorder. The present study included 121 8 to 14 year-old girls with no lifetime history of depression, and examined the association between maternal history of depression (i.e., risk) and both behavioral and ERP measures while viewing positive and negative adjectives during the SRET. Lifetime history of major depressive disorder and/or dysthymia in the biological mother was assessed via a semistructured diagnostic interview. Results indicated that participants with maternal history of depression, compared with those with no maternal history of depression, demonstrated an enhanced LPP to negative words. There were no group differences in the LPP to positive words. Maternal history of depression was also related to faster response time when rejecting negative words. Participant's current depression symptoms were associated with increased negative recall bias and decreased positive recall bias. The present study provides novel evidence that abnormal electrocortical reactivity to negative self-referential words indexes vulnerability for depression in 8 to 14 year-old girls. Visual attention to emotion in depression: Facilitation and withdrawal processes

Abstract:Attentional biases for sadness are integral to cognitive theories of depression, but do not emerge under all conditions. Some researchers have argued that depression is associated with delayed withdrawal from, but not facilitated initial allocation of attention toward, sadness. We compared two types of withdrawal processes in clinically depressed and non-depressed individuals: (1) withdrawal requiring overt eye movements during visual search; and (2) covert disengagement of attention in a modified cueing paradigm. We also examined initial allocation of attention towards emotion on the visual search task, allowing comparison of withdrawal and facilitation processes. As predicted, we found no evidence of facilitated attention towards sadness in depressed individuals. However, we also found no evidence of depression-linked differences in withdrawal of attention from sadness on either task, offering no support for the theory that depression is associated with withdrawal rather than initial facilitation of attention.

Abstract:Objective To systematically evaluate the attentional bias in patients with depression toward emotional stimuli and to explore eye movement indicators and potential regulatory variables that can distinguish such patients from healthy individuals. Methods Case–control studies regarding eye-tracking in major depressive disorder published in PubMed, Web of Science, ScienceDirect, The Cochrane Library, EBSCOhost, Embase, China National Knowledge Infrastructure, Wanfang, and VIP databases from database initiation until March 12, 2022 were included in the present meta-analysis. Two researchers independently screened the literature and performed data extraction. The quality of the literature was assessed using the Newcastle–Ottawa quality assessment scale.RevMan 5.4 software was used for Meta-analysis. Results Overall, 14 studies were included, including 1,167 participants ( N depression = 474; N healthy = 693). We found that (1) fixation duration was significantly lower for positive emotional stimuli in the depression group than that in the healthy group; however, for negative stimuli, the fixation duration was significantly more in the depression group than in the healthy group. No significant difference was observed in terms of neutral emotional stimuli between groups. (2) Patients with depression exhibited a significantly lower fixation count for positive emotional stimuli than healthy individuals, whereas the fixation count for negative emotional stimuli was significantly higher in the depression group than in the healthy group. No significant difference was found for neutral emotional stimuli between groups. (3) No significant difference was detected in terms of the first fixation duration of the positive, negative, and neutral emotional stimuli between groups. (4) subgroup analysis indicated that age effected fixation duration for positive emotional stimuli. In addition, age and the type of negative emotional picture (sad, dysphoric, threat, anger) effected fixation duration for negative emotional stimuli. Conclusion Our research supports that patients with depression exhibit a negative attention bias toward emotional stimuli, and the fixation duration and fixation counts may be used as auxiliary objective indicators for depression screening.

Free viewing of sad and happy faces in depression: A potential target for attention bias modification

Abstract:Background: Identification of reliable targets for therapeutic interventions is essential for developing evidence-based therapies. Attention biases toward negative-valenced information and lack of protective positive bias toward positive-valenced stimuli have been implicated in depression. However, extant research has typically used tasks with narrow stimuli arrays and unknown or poor psychometric properties. Here, we recorded eye-tracking data of depressed and non-depressed participants during a free viewing task to address these limitations. Methods: Patients with major depressive disorder (MDD; n = 20) and undergraduate students with high (n = 23) and low (n = 20) levels of depression freely viewed 60 different face-based matrices for six seconds each. Each matrix included eight sad and eight happy facial expressions. Gaze patterns on sad and happy areas of interest (AOIs) were explored. Internal consistency for the entire sample and one-week test-retest reliability in the student sub-sample were assessed. Results: Compared to undergraduates with low levels of depression, patients with MDD and students with high levels of depression dwelled significantly longer on sad faces. Results also showed a significantly longer dwell time on the happy AOI relative to the sad AOI only in the low depression group. The two depressed groups dwelled equally on the two AOIs. The task demonstrated high internal consistency and acceptable one-week test-retest reliability. Limitations: Only sad and happy facial expressions were used. Relative small sample size. Conclusion: Relative to non-depressed participants, depressed participants showed prolonged dwelling on sad faces and lack of bias toward happy faces. These biases present viable targets for gaze-contingent attention bias modification therapy.

Abstract:Impaired facial expression perception is a core element in depression, but the underlying mechanism remains controversial. This event-related potential study investigated how attention modulates facial expression perception in depression using a nonclinical sample. A group of healthy controls (HC, N = 39) and a group of individuals with subsyndromal depression (SD, N = 39) categorized faces based on either facial expression (happy vs. sad) or gender (male vs. female). Behaviorally, the SD group was less sensitive to the emotional valence of facial expression than the HC group when their attention was directed to facial expression, as revealed by comparable subjective rating and accuracy rates in response to facial expressions. When attention was directed towards facial gender, the SD group versus the HC group showed a negative bias, as revealed by a faster N170 for sad faces than happy faces. Together, our findings suggest that attention plays a role in understanding the relationship between depression and facial expression perception.

Abstract:The present study aimed to clarify: 1) the presence of depression-related attention bias related to a social stressor, 2) its association with depression-related attention biases as measured under standard conditions, and 3) their association with impaired stress recovery in depression. A sample of 39 participants reporting a broad range of depression levels completed a standard eye-tracking paradigm in which they had to engage/disengage their gaze with/from emotional faces. Participants then underwent a stress induction (i.e., giving a speech), in which their eye movements to false emotional feedback were measured, and stress reactivity and recovery were assessed. Depression level was associated with longer times to engage/disengage attention with/from negative faces under standard conditions and with sustained attention to negative feedback during the speech. These depression-related biases were associated and mediated the association between depression level and self-reported stress recovery, predicting lower recovery from stress after giving the speech.

Abstract:Negative and positive attention bias (AB) is the preferential allocation of attentional resources to negative and positive stimuli in the environment, respectively. AB has been studied in various clinical and non-clinical populations and the process has been linked to symptoms of depression and anxiety. Findings so far suggest that negative AB is a trait-based factor that predisposes individuals to anxiety and depression. Positive AB appears specific to a depressed state, yet findings generally remain mixed. Measures of AB have been recently critiqued for their poor psychometric properties. This study addresses three gaps in the literature to further our understanding of the relationship between AB and psychopathology. The aims of this study were to determine whether 1) the core symptoms of depression (anhedonia) and anxiety (anxious arousal) are related to differential patterns of negative and positive AB, 2) anhedonia and anxious arousal have incremental utility in predicting AB over and above negative affectivity, 3) AB predicts group membership (clinical vs non-clinical). Results from random effects analysis showed that inter-subject variability was significant. Fixed effects analyses showed that the present study failed to capture positive and negative AB. Between group differences in raw response times were observed. Implications of the findings with regards to methodological differences across studies are discussed.
