Abstract:The diagnostic accuracy of biomarker-based approaches can be considerably improved by combining multiple markers. A biomarker's capacity to identify specific subjects is usually assessed using receiver operating characteristic (ROC) curves. Multimarker signatures are complicated to select as data signatures must be integrated using sophisticated statistical methods. CombiROC, developed as a user-friendly web tool, helps researchers to accurately determine optimal combinations of markers identified by a range of omics methods. With CombiROC, data of different types, such as proteomics and transcriptomics, can be analyzed using Sensitivity/Specificity filters: the number of candidate marker panels arising from combinatorial analysis is easily optimized bypassing limitations imposed by the nature of different experimental approaches. Users have full control over initial selection stringency, then CombiROC computes sensitivity and specificity for all marker combinations, determines performance for the best combinations, and produces ROC curves for automatic comparisons. All steps can be visualized in a graphic interface. CombiROC is designed without hard-coded thresholds, to allow customized fitting of each specific dataset: this approach dramatically reduces computational burden and false-negative rates compared to fixed thresholds. ROC curve analysis: a useful statistic multi-tool in the research of nephrology

Abstract:Abstract In the past decade, scientific research in the area of Nephrology has focused on evaluating the clinical utility and performance of various biomarkers for diagnosis, risk stratification and prognosis. Before implementing a biomarker in everyday clinical practice for screening a specific disease context, specific statistic measures are necessary to evaluate the diagnostic accuracy and performance of this biomarker. Receiver Operating Characteristic (ROC) Curve analysis is an important statistical method used to estimate the discriminatory performance of a novel diagnostic test, identify the optimal cut-off value for a test that maximizes sensitivity and specificity, and evaluate the predictive value of a certain biomarker or risk, prediction score. Herein, through practical examples, we aim to present a simple methodological approach to explain in detail the principles and applications of ROC curve analysis in the field of nephrology pertaining diagnosis and prognosis.

Boosting the concordance index for survival data - a unified framework to derive and evaluate biomarker combinations

Abstract:The development of molecular signatures for the prediction of time-to-event outcomes is a methodologically challenging task in bioinformatics and biostatistics. Although there are numerous approaches for the derivation of marker combinations and their evaluation, the underlying methodology often suffers from the problem that different optimization criteria are mixed during the feature selection, estimation and evaluation steps. This might result in marker combinations that are only suboptimal regarding the evaluation criterion of interest. To address this issue, we propose a unified framework to derive and evaluate biomarker combinations. Our approach is based on the concordance index for time-to-event data, which is a non-parametric measure to quantify the discrimatory power of a prediction rule. Specifically, we propose a component-wise boosting algorithm that results in linear biomarker combinations that are optimal with respect to a smoothed version of the concordance index. We investigate the performance of our algorithm in a large-scale simulation study and in two molecular data sets for the prediction of survival in breast cancer patients. Our numerical results show that the new approach is not only methodologically sound but can also lead to a higher discriminatory power than traditional approaches for the derivation of gene signatures.

Abstract:Objectives: The Robust and Optimized Biomarker Identifier (ROBI) feature selection pipeline is introduced to improve the identification of informative biomarkers coding information not already captured by existing features. It aims to accurately maximize the number of discoveries while minimizing and estimating the number of false positives (FP) with an adjustable selection stringency. Methods: 500 synthetic datasets and retrospective data of 378 Diffuse Large B Cell Lymphoma (DLBCL) patients were used for validation. On the DLBCL data, two established radiomic biomarkers, TMTV and Dmax, were measured from the 18F-FDG PET/CT scans, and 10,000 random ones were generated. Selection was performed and verified on each dataset. The efficacy of ROBI has been compared to methods controlling for multiple testing and a Cox model with Elasticnet penalty. Results: On synthetic datasets, ROBI selected significantly more true positives (TP) than FP (p < 0.001), and for 99.3% of datasets, the number of FP was within the estimated 95% confidence interval. ROBI significantly increased the number of TP compared to usual feature selection methods (p < 0.001). On retrospective data, ROBI selected the two established biomarkers and one random biomarker and estimated 95% chance of selecting 0 or 1 FP and a probability of 0.0014 of selecting only FP. Bonferroni correction selected no feature, and Elasticnet selected 101 spurious features and discarded TMTV. Conclusion: ROBI selected relevant biomarkers while effectively controlling for FPs, outperforming conventional selection methods. This underscores its potential as a valuable asset for biomarker discovery.

Abstract:Multiple diseases (acute or chronic events) occur together in a patient, which refers to the disease comorbidities, because of the multi ways associations among diseases. Due to shared genetic, molecular, environmental, and lifestyle-based risk factors, many diseases are comorbid in the same patient. Methods for integrating multiple types of omics data play an important role to identify integrative biomarkers for stratification of patients into groups with different clinical outcomes. Moreover, integrated omics and clinical information may potentially improve prediction accuracy of disease comorbidities. However, there is a lack of effective and efficient bioinformatics and statistical software for true integrative data analysis. With the availability of the wide spread huge omics, phenotype and ontology information, it is becoming more and more practical to help doctors in clinical diagnostics and comorbidity prediction by providing appropriate software tool. We developed an R software POGO to compute novel estimators of the disease comorbidity risks and patient stratification. Starting from an initial diagnosis, omics and clinical data of a patient the software identifies the association risk of disease comorbidities. The input of this software is the initial diagnosis of a patient and the output provides evidence of disease comorbidities. The functions of POGO offer flexibility for diagnostic applications to predict disease comorbidities, and can be easily integrated to high-throughput and clinical data analysis pipelines. POGO is compliant with the Bioconductor standard and it is freely available at www.cl.cam.ac.uk/~mam211/POGO/.

Abstract:Diagnostic biomarkers play a critical role in biomedical research, particularly for the diagnosis and prediction of diseases, etc. To enhance diagnostic accuracy, extensive research about combining multiple biomarkers has been developed based on the multivariate normality, which is often not true in practice, as most biomarkers follow distributions that deviate from normality. While the likelihood ratio combination is recognized to be the optimal approach, it is complicated to calculate. To achieve a more accurate and effective combination of biomarkers, especially when these biomarkers deviate from normality, we propose using a receiver operating characteristic (ROC) curve methodology based on the optimal combination of elliptically distributed biomarkers. In this paper, we derive the ROC curve function for the elliptical likelihood ratio combination. Further, proceeding from the derived best combinations of biomarkers, we propose an efficient technique via nonparametric maximum likelihood estimate (NPMLE) to build empirical estimation. Simulation results show that the proposed elliptical combination method consistently provided better performance, demonstrating its robustness in handling various distribution types of biomarkers. We apply the proposed method to two real datasets: Autism/autism spectrum disorder (ASD) and neural tube defects (NTD). In both applications, the elliptical likelihood ratio combination improves the AUC value compared to the multivariate normal likelihood ratio combination and the best linear combination.

Abstract:Two different approaches to analysis of data from diagnostic biomarker studies are commonly employed. Logistic regression is used to fit models for probability of disease given marker values, while ROC curves and risk distributions are used to evaluate classification performance. In this paper we present a method that simultaneously accomplishes both tasks. The key step is to standardize markers relative to the nondiseased population before including them in the logistic regression model. Among the advantages of this method are the following: (i) ensuring that results from regression and performance assessments are consistent with each other; (ii) allowing covariate adjustment and covariate effects on ROC curves to be handled in a familiar way, and (iii) providing a mechanism to incorporate important assumptions about structure in the ROC curve into the fitted risk model. We develop the method in detail for the problem of combining biomarker data sets derived from multiple studies, populations or biomarker measurement platforms, when ROC curves are similar across data sources. The methods are applicable to both cohort and case-control sampling designs. The data set motivating this application concerns Prostate Cancer Antigen 3 (PCA3) for diagnosis of prostate cancer in patients with or without previous negative biopsy where the ROC curves for PCA3 are found to be the same in the two populations. The estimated constrained maximum likelihood and empirical likelihood estimators are derived. The estimators are compared in simulation studies and the methods are illustrated with the PCA3 data set.

We envisage that a software dedicated to comorbidity diagnosis could result in an effective aid to the health practice.ResultsWe have developed an R software comoR to compute novel estimators of the disease comorbidity associations. Starting from an initial diagnosis, genetic and clinical data of a patient the software identifies the risk of disease comorbidity. Then it provides a pipeline with different causal inference packages (e.g. pcalg, qtlnet etc) to predict the causal relationship of diseases. It also provides a pipeline with network regression and survival analysis tools (e.g. Net-Cox, rbsurv etc) to predict more accurate survival probability of patients. The input of this software is the initial diagnosis for a patient and the output provides evidences of disease comorbidity mapping.ConclusionsThe functions of the comoR offer flexibility for diagnostic applications to predict disease comorbidities, and can be easily integrated to high–throughput and clinical data analysis pipelines.
ChatGPT-Enhanced ROC Analysis (CERA): A shiny web tool for finding optimal cutoff points in biomarker analysis

Abstract:Diagnostic tests play a crucial role in establishing the presence of a specific disease in an individual. Receiver Operating Characteristic (ROC) curve analyses are essential tools that provide performance metrics for diagnostic tests. Accurate determination of the cutoff point in ROC curve analyses is the most critical aspect of the process. A variety of methods have been developed to find the optimal cutoffs. Although the R programming language provides a variety of package programs for conducting ROC curve analysis and determining the appropriate cutoffs, it typically needs coding skills and a substantial investment of time. Specifically, the necessity for data preprocessing and analysis can present a significant challenge, especially for individuals without coding experience. We have developed the CERA (ChatGPT-Enhanced ROC Analysis) tool, a user-friendly ROC curve analysis web tool using the shiny interface for faster and more effective analyses to solve this problem. CERA is not only user-friendly, but it also interacts with ChatGPT, which interprets the outputs. This allows for an interpreted report generated by R-Markdown to be presented to the user, enhancing the accessibility and understanding of the analysis results.

Abstract:BackgroundReceiver operating characteristic (ROC) curves are useful tools to evaluate classifiers in biomedical and bioinformatics applications. However, conclusions are often reached through inconsistent use or insufficient statistical analysis. To support researchers in their ROC curves analysis we developed pROC, a package for R and S+ that contains a set of tools displaying, analyzing, smoothing and comparing ROC curves in a user-friendly, object-oriented and flexible interface.ResultsWith data previously imported into the R or S+ environment, the pROC package builds ROC curves and includes functions for computing confidence intervals, statistical tests for comparing total or partial area under the curve or the operating points of different classifiers, and methods for smoothing ROC curves. Intermediary and final results are visualised in user-friendly interfaces. A case study based on published clinical and biomarker data shows how to perform a typical ROC analysis with pROC.ConclusionspROC is a package for R and S+ specifically dedicated to ROC analysis. It proposes multiple statistical tests to compare ROC curves, and in particular partial areas under the curve, allowing proper ROC interpretation. pROC is available in two versions: in the R programming language or with a graphical user interface in the S+ statistical software. It is accessible at http://expasy.org/tools/pROC/ under the GNU General Public License. It is also distributed through the CRAN and CSAN public repositories, facilitating its installation.

Abstract:BiomiX addresses the data analysis bottleneck in high-throughput omics technologies, enabling the efficient, integrated analysis of multiomics data obtained from two cohorts. BiomiX incorporates diverse omics data. DESeq2/Limma packages analyze transcriptomics data, while statistical tests determine metabolomics peaks. The metabolomics annotation uses the mass-to-charge ratio in the CEU Mass Mediator database and fragmentation spectra in the TidyMass package while Methylomics analysis is performed using the ChAMP R package. Multiomics Factor Analysis (MOFA) integration and interpretation identifies common sources of variations among omics. BiomiX provides comprehensive outputs, including statistics and report figures, also integrating EnrichR and GSEA for biological process exploration. Subgroup analysis based on user gene panels enhances comparisons. BiomiX implements MOFA automatically, selecting the optimal MOFA model to discriminate the two cohorts being compared while providing interpretation tools for the discriminant MOFA factors. The interpretation relies on innovative bibliography research on Pubmed, which provides the articles most related to the discriminant factor contributors. The interpretation is also supported by clinical data correlation with the discriminant MOFA factors and pathways analyses of the top factor contributors. The integration of single and multi-omics analysis in a standalone tool, together with the implementation of MOFA and its interpretability by literature, constitute a step forward in the multi-omics landscape in line with the FAIR data principles. The wide parameter choice grants a personalized analysis at each level based on the user requirements. BiomiX is a user-friendly R-based tool compatible with various operating systems that aims to democratize multiomics analysis for bioinformatics non-experts.

Nonparametric ROC Summary Statistics for Correlated Diagnostic Marker Data

Abstract:We propose efficient nonparametric statistics to compare medical imaging modalities in multi-reader multi-test data and to compare markers in longitudinal ROC data. The proposed methods are based on the weighted area under the ROC curve which includes the area under the curve and the partial area under the curve as special cases. The methods maximize the local power for detecting the difference between imaging modalities. The asymptotic results of the proposed methods are developed under a complex correlation structure. Our simulation studies show that the proposed statistics result in much better powers than existing statistics. We applied the proposed statistics to an endometriosis diagnosis study.

Abstract:Motivation: Biomarker detection plays a pivotal role in biomedical research. Integrating omics studies from multiple cohorts can enhance statistical power, accuracy and robustness of the detection results. However, existing methods for horizontally combining omics studies are mostly designed for two-class scenarios (e.g., cases versus controls) and are not directly applicable for studies with multi-class design (e.g., samples from multiple disease subtypes, treatments, tissues, or cell types). Results: We propose a statistical framework, namely Mutual Information Concordance Analysis (MICA), to detect biomarkers with concordant multi-class expression pattern across multiple omics studies from an information theoretic perspective. Our approach first detects biomarkers with concordant multi-class patterns across partial or all of the omics studies using a global test by mutual information. A post hoc analysis is then performed for each detected biomarkers and identify studies with concordant pattern. Extensive simulations demonstrate improved accuracy and successful false discovery rate control of MICA compared to an existing MCC method. The method is then applied to two practical scenarios: four tissues of mouse metabolism-related transcriptomic studies, and three sources of estrogen treatment expression profiles. Detected biomarkers by MICA show intriguing biological insights and functional annotations. Additionally, we implemented MICA for single-cell RNA-Seq data for tumor progression biomarkers, highlighting critical roles of ribosomal function in the tumor microenvironment of triple-negative breast cancer and underscoring the potential of MICA for detecting novel therapeutic targets.

ioSearch: An approach for identifying interacting multiomics biomarkers using a novel algorithm with application on breast cancer data sets

Abstract:Identification of biomarkers by integrating multiple omics together is important because complex diseases occur due to an intricate interplay of various genetic materials. Traditional single-omics association tests neither explore this crucial interomics dependence nor identify moderately weak signals due to the multiple-testing burden. Conversely, multiomics data integration imparts complementary information but suffers from an increased multiple-testing burden, data diversity inherent with different omics features, high-dimensionality, and so forth. Most of the available methods address subtype classification using dimension-reduction techniques to circumvent the sample size issue but interacting multiomics biomarker identification methods are unavailable. We propose a two-step model that first investigates phenotype-omics association using logistic regression. Then, selects disease-associated omics using sparse principal components which explores the interrelationship of multiple variables from two omics in a multivariate multiple regression framework. On the basis of this model, we developed a multiomics biomarker identification algorithm, interacting omics search (ioSearch), that jointly tests the effect of multiple omics with disease and between-omics associations by using pathway information that subsequently reduces the multiple-testing burden. Further, inference in terms of p values potentially makes it an easily interpretable biomarker identification tool. Extensive simulation demonstrates ioSearch as statistically powerful with a controlled Type-I error rate. Its application to publicly available breast cancer data sets identified relevant omics features in important pathways.
ROCnReg: An R Package for Receiver Operating Characteristic Curve Inference with and without Covariate Information

Abstract:The receiver operating characteristic (ROC) curve is the most popular tool used to evaluate the discriminatory capability of diagnostic tests/biomarkers measured on a continuous scale when distinguishing between two alternative disease states (e.g, diseased and nondiseased). In some circumstances, the test's performance and its discriminatory ability may vary according to subject-specific characteristics or different test settings. In such cases, information-specific accuracy measures, such as the covariate-specific and the covariate-adjusted ROC curve are needed, as ignoring covariate information may lead to biased or erroneous results. This paper introduces the R package ROCnReg that allows estimating the pooled (unadjusted) ROC curve, the covariate-specific ROC curve, and the covariate-adjusted ROC curve by different methods, both from (semi) parametric and nonparametric perspectives and within Bayesian and frequentist paradigms. From the estimated ROC curve (pooled, covariate-specific or covariate-adjusted), several summary measures of accuracy, such as the (partial) area under the ROC curve and the Youden index, can be obtained. The package also provides functions to obtain ROC-based optimal threshold values using several criteria, namely, the Youden Index criterion and the criterion that sets a target value for the false positive fraction. For the Bayesian methods, we provide tools for assessing model fit via posterior predictive checks, while model choice can be carried out via several information criteria. Numerical and graphical outputs are provided for all methods. The package is illustrated through the analyses of data from an endocrine study where the aim is to assess the capability of the body mass index to detect the presence or absence of cardiovascular disease risk factors. MuSA: a graphical user interface for multi-OMICs data integration in radiogenomic studies

Abstract:Abstract Analysis of large-scale omics data along with biomedical images has gaining a huge interest in predicting phenotypic conditions towards personalized medicine. Multiple layers of investigations such as genomics, transcriptomics and proteomics, have led to high dimensionality and heterogeneity of data. Multi-omics data integration can provide meaningful contribution to early diagnosis and an accurate estimate of prognosis and treatment in cancer. Some multi-layer data structures have been developed to integrate multi-omics biological information, but none of these has been developed and evaluated to include radiomic data. We proposed to use MultiAssayExperiment (MAE) as an integrated data structure to combine multi-omics data facilitating the exploration of heterogeneous data. We improved the usability of the MAE, developing a Multi-omics Statistical Approaches (MuSA) tool that uses a Shiny graphical user interface, able to simplify the management and the analysis of radiogenomic datasets. The capabilities of MuSA were shown using public breast cancer datasets from TCGA-TCIA databases. MuSA architecture is modular and can be divided in Pre-processing and Downstream analysis. The pre-processing section allows data filtering and normalization. The downstream analysis section contains modules for data science such as correlation, clustering (i.e., heatmap) and feature selection methods. The results are dynamically shown in MuSA. MuSA tool provides an easy-to-use way to create, manage and analyze radiogenomic data. The application is specifically designed to guide no-programmer researchers through different computational steps. Integration analysis is implemented in a modular structure, making MuSA an easily expansible open-source software.

Abstract:Enrichment of predictive models with new biomolecular markers is an important task in high-dimensional omic applications. Increasingly, clinical studies include several sets of such omics markers available for each patient, measuring different levels of biological variation. As a result, one of the main challenges in predictive research is the integration of different sources of omic biomarkers for the prediction of health traits. We review several approaches for the combination of omic markers in the context of binary outcome prediction, all based on double cross-validation and regularized regression models. We evaluate their performance in terms of calibration and discrimination and we compare their performance with respect to single-omic source predictions. We illustrate the methods through the analysis of two real datasets. On the one hand, we consider the combination of two fractions of proteomic mass spectrometry for the calibration of a diagnostic rule for the detection of early-stage breast cancer. Abstract:Microbial signatures have emerged as promising biomarkers for disease diagnostics and prognostics, yet their variability across different studies calls for a standardized approach to biomarker research. Therefore, we introduce xMarkerFinder, a four-stage computational framework for microbial biomarker identification with comprehensive validations from cross-cohort datasets, including differential signature identification, model construction, model validation and biomarker interpretation. xMarkerFinder enables the identification and validation of reproducible biomarkers for cross-cohort studies, along with the establishment of classification models and potential microbiome-induced mechanisms. Originally developed for gut microbiome research, xMarkerFinder's adaptable design makes it applicable to various microbial habitats and data types. Distinct from existing biomarker research tools that typically concentrate on a singular aspect, xMarkerFinder uniquely incorporates a sophisticated feature selection process, specifically designed to address the heterogeneity between different cohorts, extensive internal and external validations, and detailed specificity assessments. Execution time varies depending on the sample size, selected algorithm and computational resource. Abstract:A common study design for comparing the performances of diagnostic imaging tests is to obtain ratings from multiple readers of multiple cases whose true statuses are known. Typically, there is overlap between the tests, readers, and/or cases for which special analytical methods are needed to perform statistical comparisons. We present our new MATLAB MRMCaov toolbox, which is designed for multi-reader multi-case comparisons of two or more diagnostic tests. The toolbox allows for statistical comparison of reader performance metrics, such as area under the receiver operating characteristic curve (ROC AUC), with analysis of variance methods originally proposed by Obuchowski and Rockette (1995) and later unified and improved by Hillis and colleagues (2005, 2007, 2008, 2018). MRMCaov is open-source software with an integrated command-line interface for performing multi-reader multi-case statistical analysis, plotting, and presenting results. Its features (1) ROC AUC, likelihood ratios of positive or negative ratings, sensitivity, specificity, and expected utility reader performance metrics; (2) reader-specific ROC curves; (3) user-definable performance metrics; (4) test-specific estimates of mean performance along with confidence intervals and p-values for statistical comparisons; (5) support for factorial, nested, or partially paired study designs; (6) inference for random or fixed readers and cases; (7) DeLong, jackknife, or unbiased covariance estimation; and (8) compatibility with Microsoft Windows, Mac OS, and Linux.
