Abstract:Purpose: The purpose of this study was to examine the expression of corneal epithelial basement membrane (EBM) components in different corneal stromal cell types. In vitro model systems were used to explore the expression of EBM components nidogen-1, nidogen-2, and perlecan that are the primary components in the lamina lucida and the lamina densa that defectively regenerate in corneas with stromal opacity after in -9.0 D photorefractive keratectomy (PRK). Methods: Primary rabbit corneal stromal cells were cultured using varying serum concentrations and exogenous growth factors, including fibroblast growth factor (FGF)-2 and transforming growth factor (TGF)-β1, to optimize the growth of each cell type of interest. The expression of the keratocyte-specific marker keratocan and the myofibroblast-specific marker α-smooth muscle actin (α-SMA) were analyzed with real-time PCR, western blot, and immunocytochemical staining to evaluate the specificity of the cell types and select optimal conditions (high keratocan and low α-SMA for keratocytes; low keratocan and high α-SMA for myofibroblasts; low keratocan and low α-SMA for corneal fibroblasts). The expression of the EBM components nidogen-1, nidogen-2, and perlecan was evaluated in each corneal cell type using real-time PCR, immunostaining, and western blotting. In agreement with previous studies, serum-free DMEM was found to be optimal for keratocytes, DMEM with 10% serum and 40 ng/ml FGF-2 yielded the best marker profile for corneal fibroblasts, and DMEM with 1% serum and 2 ng/ml TGF-β1 was found to be optimal for myofibroblasts. Results: Nidogen-1 and nidogen-2 mRNAs were highly expressed in keratocytes, whereas perlecan was highly expressed in myofibroblasts. In keratocytes, nidogen-2 and perlecan proteins were expressed predominantly in intracellular compartments, whereas in myofibroblasts expression of both EBM components was observed diffusely throughout the cell. Although the perlecan mRNA levels were high in the myofibroblasts, the qualitative protein expression was different from that of the keratocytes. Corneal fibroblasts produced a low amount of each EBM component. Conclusions: We have demonstrated qualitative and quantitative differences in the expression of nidogen-1, nidogen-2, and perlecan by keratocytes compared to myofibroblasts that may contribute to defective regeneration of the lamina lucida and the lamina densa of the EBM associated with late stromal haze after high-correction PRK.
IL-1 and TGF-β Modulation of Epithelial Basement Membrane Components Perlecan and Nidogen Production by Corneal Stromal Cells

Abstract:Purpose: To determine whether (1) the in vitro expression of epithelial basement membrane components nidogen-1, nidogen-2, and perlecan by keratocytes, corneal fibroblasts, and myofibroblasts is modulated by cytokines/growth factors, and (2) perlecan protein is produced by stromal cells after photorefractive keratectomy. Methods: Marker-verified rabbit keratocytes, corneal fibroblasts, myofibroblasts were stimulated with TGF-β1, IL-1α, IL-1β, TGF-β3, platelet-derived growth factor (PDGF)-AA, or PDGF-AB. Real-time quantitative RT-PCR was used to detect expression of nidogen-1, nidogen-2, and perlecan mRNAs. Western blotting evaluated changes in protein expression. Immunohistochemistry was performed on rabbit corneas for perlecan, alpha-smooth muscle actin, keratocan, vimentin, and CD45 at time points from 1 day to 1 month after photorefractive keratectomy (PRK). Results: IL-1α or -1β significantly upregulated perlecan mRNA expression in keratocytes. TGF-β1 or -β3 markedly downregulated nidogen-1 or -2 mRNA expression in keratocytes. None of these cytokines had significant effects on nidogen-1, -2, or perlecan mRNA expression in corneal fibroblasts or myofibroblasts. IL-1α significantly upregulated, while TGF-β1 significantly downregulated, perlecan protein expression in keratocytes. Perlecan protein expression was upregulated in anterior stromal cells at 1 and 2 days after -4.5 or -9 diopters (D) PRK, but the subepithelial localization of perlecan became disrupted at 7 days and later time points in -9-D PRK corneas when myofibroblasts populated the anterior stroma. Conclusions: IL-1 and TGF-β1 have opposing effects on perlecan and nidogen expression by keratocytes in vitro. Proximate participation of keratocytes is likely needed to regenerate normal epithelial basement membrane after corneal injury.
Corneal Molecular and Cellular Biology for the Refractive Surgeon: The Critical Role of the Epithelial Basement Membrane

Abstract:Purpose: To provide an overview of the recent advances concerning the corneal molecular and cellular biology processes involved in the wound healing response after excimer laser surface ablation and LASIK surgery. Methods: Literature review. Results: The corneal wound healing response is a complex cascade of events that impacts the predictability and stability of keratorefractive surgical procedures such as photorefractive keratectomy and LASIK. The generation and persistence of corneal myofibroblasts (contractile cells with reduced transparency) arise from the interaction of cytokines and growth factors such as transforming growth factor beta and interleukin 1 produced by epithelial and stromal cells in response to the corneal injury. Myofibroblasts, and the opaque extracellular matrix they secrete into the stroma, disturb the precise distribution and spacing of collagen fibers related to corneal transparency and lead to the development of vision-limiting corneal opacity (haze). The intact epithelial basement membrane has a pivotal role as a structure that regulates corneal epithelial-stromal interactions. Thus, defective regeneration of the epithelial basement membrane after surgery, trauma, or infection leads to the development of stromal haze. The apoptotic process following laser stromal ablation, which is proportional to the level of attempted correction, leads to an early decrease in anterior keratocyte density and the diminished contribution of these non-epithelial cells of components such as perlecan and nidogen-2 required for normal regeneration of the epithelial basement membrane. Haze persists until late repair of the defective epithelial basement membrane. Conclusions: Defective regeneration of the epithelial basement membrane has a critical role in determining whether a cornea heals with late haze after photorefractive keratectomy or with scarring at the flap edge in LASIK.

Abstract:The objectives of the study were to measure magnesium levels in neonates and infants undergoing arterial switch operation and to ascertain the role of magnesium supplementation in the prevention of postoperative arrhythmias. Group I (n=25): magnesium was administered in the dose of 30 mg/kg over 10 minutes in normal saline (5 ml) immediately after cessation of cardiopulmonary bypass (CPB). Group II (n=25): normal saline (5 ml) was administered over 10 minutes immediately after cessation of CPB. Samples of arterial blood were collected at four time points: 1) after induction of anaesthesia; 2) 10 minutes after initiation of CPB; 3) at rewarming during CPB; and 4) 4 hours after shifting the patient to the intensive care unit. Samples were measured for ionized magnesium (iMg), blood gases, haematocrit level, electrolytes, ionized calcium and glucose. Continuous ECG rhythm analysis and documentation of arrhythmias was performed for 24 hours after surgery. The mean preoperative iMg levels were below the normal level in both the groups. A significant increase in iMg levels (P=0.00) was seen in both groups during rewarming. There is no statistically significant difference in the incidence of arrhythmias between the magnesium supplemented group (4%) and the control group (20%) in the postoperative period, a tendency towards reduction in arrhythmias was only observed in the magnesium supplemented group.
Corneal epithelial basement membrane: Structure, function and regeneration

Abstract:Basement membranes are highly specialized extracellular matrices. More than providing scaffolds, basement membranes are recognized as dynamic and versatile structures that modulate cellular responses to regulate tissue development, function, and repair. Increasing evidence suggests that, in addition to providing structural support to adjacent cells, basement membranes serve as reservoirs and modulators of growth factors that direct and fine-tune cellular functions. Since the corneal stroma is avascular and has a relatively low keratocyte density, it's likely that the corneal BM is different in composition from the BMs in other tissues. BMs are composed of a diverse assemblage of extracellular molecules, some of which are likely specific to the tissue where they function; but in general they are composed of four primary components-collagens, laminins, heparan sulfate proteoglycans, and nidogens-in addition to other components such as thrombospondin-1, matrilin-2, and matrilin-4 and fibronectin. Severe injuries to the cornea, including infection, surgery, and trauma, may trigger the development of myofibroblasts and fibrosis in the normally transparent connective tissue stroma. Ultrastructural studies have demonstrated that defective epithelial basement membrane (EBM) regeneration after injury to the cornea underlies the development of myofibroblasts from both bone marrow- and keratocyte-derived precursor cells. Defective EBM permits epithelium-derived and tear-derived transforming growth factor beta (TGF-β), platelet-derived growth factor (PDGF), and possibly other modulators, to penetrate the stroma at sustained levels necessary to drive the development and persistence of vimentin + alpha-smooth muscle actin + desmin+ (V + A + D+) mature myofibroblasts. A recent discovery that has contributed to our understanding of haze development is that keratocytes and corneal fibroblasts produce critical EBM components, such as nidogen-1, nidogen-2 and perlecan, that are essential for complete regeneration of a normal EBM once laminin secreted by epithelial cells self-polymerizes into a nascent EBM. Mature myofibroblasts that become established in the anterior stroma are a barrier to keratocyte/corneal fibroblast contributions to the nascent EBM. These myofibroblasts, and the opacity they produce, often persist for months or years after the injury. Transparency is subsequently restored if the EBM is fully regenerated, myofibroblasts are deprived of TGF-β and undergo apoptosis, and keratocytes reoccupy the anterior stroma and reabsorb the disordered extracellular matrix.
The corneal fibrosis response to epithelial-stromal injury

Abstract:The corneal wound healing response, including the development of stromal opacity in some eyes, is a process that often leads to scarring that occurs after injury, surgery or infection to the cornea. Immediately after epithelial and stromal injury, a complex sequence of processes contributes to wound repair and regeneration of normal corneal structure and function. In some corneas, however, often depending on the type and extent of injury, the response may also lead to the development of mature vimentin+ α-smooth muscle actin+ desmin+ myofibroblasts. Myofibroblasts are specialized fibroblastic cells generated in the cornea from keratocyte-derived or bone marrow-derived precursor cells. The disorganized extracellular matrix components secreted by myofibroblasts, in addition to decreased expression of corneal crystallins in these cells, are central biological processes that result in corneal stromal fibrosis associated with opacity or "haze". Several factors are associated with myofibroblast generation and haze development after PRK surgery in rabbits, a reproducible model of scarring, including the amount of tissue ablated, which may relate to the extent of keratocyte apoptosis in the early response to injury, irregularity of stromal surface after surgery, and changes in corneal stromal proteoglycans, but normal regeneration of the epithelial basement membrane (EBM) appears to be a critical factor determining whether a cornea heals with relative transparency or vision-limiting stromal opacity. Structural and functional abnormalities of the regenerated EBM facilitate prolonged entry of epithelium-derived growth factors such as transforming growth factor β (TGF-β) and platelet-derived growth factor (PDGF) into the stroma that both drive development of mature myofibroblasts from precursor cells and lead to persistence of the cells in the anterior stroma. A major discovery that has contributed to our understanding of haze development is that keratocytes and corneal fibroblasts produce critical EBM components, such as nidogen-1, nidogen-2 and perlecan, that are essential for complete regeneration of a normal EBM once laminin secreted by epithelial cells self-polymerizes into a nascent EBM. Abstract:OBJECTIVE:To investigate the relationship between the haze formation and the change of extracellular matrix (ECM) in the corneal tissue after photorefractive keratectomy (PRK).METHODS:Twenty-four adult New Zealand White rabbits received PRK in the right eye for minus sign10.00 D ablation. According to the time of rabbits sacrifice, they were divided into 8 groups, and 3 rabbits were in each group. They were immediate, 24 hour, 1 week, 2 week, 1 month, 3 month, 6 month and 12 month groups. After surgery, the corneas were examined periodically by slip-lamp microscopy for haze formation. The right eyes were enucleated, and the corneas were studied with immunohistochemical evaluation for extracellular matrix, including collagen type I, type III, type VI, cellular fibronectin, tenascin and laminin.RESULTS:The incidence of haze was 100% after PRK. The formation of haze had a tendency from mild to severe, and along with the time prolongation, it gradually decreased and disappeared. The newly synthesized collagen type I, type III, type VI, cellular fibronectin, tenascin and laminin were deposited in the anterior corneal stroma.CONCLUSION:The formation of haze is closely related to the deposition of ECM elements in the anterior corneal stroma in the surgical area.
Effects of amniotic membrane on epithelial wound healing and stromal remodelling after excimer laser keratectomy in rabbit cornea

Abstract:Aims: To investigate if the amniotic membrane (AM) promotes epithelial migration while inhibiting stromal remodelling associated with corneal haze after excimer laser keratectomy. Methods: A wound 150 microm in depth and 6.0 mm in diameter was produced in 40 rabbits using an excimer laser. One eye was randomly chosen to be covered by the AM while the other eye served as a control. Epithelial wound healing was evaluated, together with any morphological changes of the anterior stroma connected with corneal haze. These morphological changes were histopathologically analysed using dichlortriazinyl aminofluorescein (DTAF), Masson trichrome staining, and an image analyser. Results: The AM group had a short latent phase followed by fast epithelial healing (p<0.001) during the early wound healing period and a significant decrease in the inflammatory response, together with a smaller change in the number of keratocytes than the control group. The mean thickness of the regenerated stroma was significantly thinner in the AM group than in the control group at 8 weeks (p<0.0001). The AM group had a more regular architecture of regenerated stromal lamella at 8 weeks and significantly less haze after 4 weeks than the control group (p<0.05). Conclusion: Use of the AM as a dressing on a corneal wound created by excimer laser surgery, in which severe haze is expected, may induce rapid epithelial healing with less inflammatory response. The AM may inhibit the irregular synthesis of stromal collagen that is associated with corneal haze.
Late open conversion after endovascular aneurysm repair.

Abstract:OBJECTIVES Endovascular treatment of the infrarenal abdominal aorta (endovascular repair, EVAR) has emerged as an alternative to open surgery. However, a small subset of patients exists who undergo conversion either in the first 30 postoperative days or later during the course of postoperative surveillance. In the present study, we review our experience with late conversion operations. METHODS Between December 2004 and August 2012, 411 EVARs were performed. During the same time interval, nine patients (males) with a mean age of 71 years (range, 59-79 years) required late open conversion. The median interval between EVAR and the conversion operation was 34 months (range 14-60 months). RESULTS The indications for late conversion included persistent proximal type I endoleak (n = 2), type II endoleak with sac enlargement (n = 1), aneurysm rupture (n = 1), endotension (n = 2), stent-graft thrombosis (n = 1) and stent-graft infection (n = 2). Complete stent-graft explantation was performed in five patients. Eight patients underwent elective conversion. One patient presenting with rupture had an emergency operation. The 30-day mortality rate was 0%. CONCLUSIONS Late open conversion after EVAR can be performed safely and successfully. Complete stent-graft explantation may be our preferred treatment option, but it is not always necessary, except in cases presenting with graft infection.

Abstract:OBJECTIVE:To observed the effect of amniotic membrane transplantation (AMT) on expressions of transforming growth factor-beta 1(TGF-beta 1), collagens I, III and fibronectin (FN) in rabbit corneal healing after photorefractive keratectomy (PRK), and investigate the anti-scarring formation mechanism of AMT.METHODS:Ten rabbits underwent bilateral PRK to correct 8 diopters of myopia. One eye was randomly transplanted with preserved human amniotic membrane, and the other eye served as the control. The expressions of TGF-beta 1, collagens I, III and FN were studied by immunohistochemistry at 4 weeks after operation.RESULTS:The expression of TGF-beta 1 in corneal epithelium and keratocytes and collegans in anterior stroma was significantly less in AMT group than in the control group. The expressions of collagens III and FN in anterior stroma of ablation area were also significantly less in AMT group than in the control group. The staining of collagen I had no significant difference between the AMT group and the control group.CONCLUSION:The expressions of TGF-beta 1, collagens III and FN can be suppressed by amniotic membrane after PRK. These data suggest that part of the anti-scarring effect of AMT may be mediated through the suppression of TGF-beta 1, collagens III and FN expression.
Corneal tissue repairing after laser in situ keratomileusis

Abstract:Objective: To investigate the corneal tissue repairing after laser in situ keratomileusis(LASIK). Methods: Twenty-seven adult New Zealand White rabbits were divided into 8 groups. The left eyes of all ralbits received LASIK for -10.00 D ablation. The average depth of corneal flaps were (93.6 ± 20.3) μm. The depth of ablation was 107 μm in every eye. At 0,24 h,1 and 2 week and at 1,3,6 and 12 months after surgery, corneas were examined periodically by slit-lamp microscopy and were studies with light and transmission electron microscopy. Results: The central part of corneas were transparent and had no haze after LASIK by slitlamp examination in every group for the entire study period. Light microscopy revealed the epithelium had no apparent changes in the central part of corneas. Space existed between the flap and the anterior ablated stroma in the early stage (1 week to 1 month after operation) and disappears at the later period (3 months later). The number of fibroblasts in the anterior ablated stroma increased for a short time (24 h to 1 month after operation). Through transmission electron microscopy examination, basement membrane of corneal epithelium was continuous. The density of hemidesmosomes was normal. The collagen fibrils were organized, the diameter of fibrils and the space between the fibrils were regular in the anterior stroma. The morphology of the fibroblasts had no apparent change compared to that of control eyes. Conclusion: The transparency in the central part of corneas after LASIK was correlated with integrity of corneal epithelium and basement membrane.
[The Expression of Ki-67, TGF-beta(2) and Alpha-Sma after the Femtosecond Laser-Assisted Intrastromal Scanning in Rabbit Cornea].

Abstract:OBJECTIVE:To study how the cornea would express Ki-67, TGF-beta(2) and alpha-SMA and how the cornea would heal when it is scanned intrastromally. METHODS:Twenty New Zealand white rabbits were randomly numbered and used for the experiments. The 60 kHz Intralase femtosecond laser delivery system was used to scan 35 rabbit corneas (of these corneas, 5 right corneas were from 5 rabbits). The unoperated 5 left eyes were used as control. The laser settings were: spot/line separation, 10 microm; diameter, 8.5 mm; energy for scanning the stroma, 1.3 microJ; scanning depth, 135 microm, no edge cuts were performed. Slit lamp was used to observe the cornea at 0.5 h, 1 h, 2 h, and 3 months after surgery. On day 1, 3, 7, 30, 90 after surgery, 6 and 1 scanned corneas at each time point were taken for Western blot and immunocytochemical detection respectively to detect the expression of TNF-alpha, Ki-67, TGF-beta(2) and alpha-SMA. RESULTS:Microbubbles were found in corneal stroma after surgery. The number of microbubbles tended to decrease significantly with time. At 2 h, the cornea turned to be transparent again. During 3-month follow-up, the cornea was always transparent and no haze appeared. Western blot analysis and immunocytochemical detection demonstrated that the expression of Ki-67 began to increase at day 1(0.0670 + or - 0.0008) and reached the peak at day 3 after surgery (0.6923 + or - 0.005). While there were significant difference with the control group at day 90 after surgery (t = 24.12, 57.22, 43.26, 39.78, 18.35;P < 0.05), a gradual decrease in expression of ki-67 could be detected from day 3 to 90. There were no significant change in the expression of TGF-beta(2) (t = 0.933, 0.856, 0.934, 0.970, 1.132) and alpha-SMA (t = 1.126, 1.235, 0.993, 1.175, 1.211) at all time points after surgery (P > 0.05). CONCLUSIONS:After intrastromal scanning, the expression of Ki-67 increases and the keratocyte is activated and proliferated. However, due to no epithelium injured and TGF-beta(2) confined to the epithelium, the activated and proliferated keratocytes could not be transformed into myofibroblasts.
[Stromal Cells Change after Epipolis Laser in Situ Keratomileusis and Laser-Assisted Subepithelial Keratectomy in Rabbits].

Abstract:OBJECTIVE:To compare the effects of epipolis laser in situ keratomileusis (Epi-LASIK) and laser-assisted subepithelial keratectomy (LASEK) on the corneal stromal cells, and to evaluate their effects on corneal haze histopathologically.METHODS:Forty-eight New Zealand white rabbits (96 eyes) were randomly treated with Epi-LASIK in one eye and LASEK in the other, and 2 rabbits (4 eyes) without any treatment were used as control. Cornea stromal cell apoptosis was evaluated by terminal deoxyribonucleotidyl transferase-mediated deoxynuridine triphosphate nick end labeling (TUNEL) assay. Stromal cells proliferation and myofibroblasts generation were evaluated by immunocytochemical analyses the expression of Ki-67 and alpha-smooth muscle actin (alpha-SMA) respectively. Myofibroblast generation was further testified by Western blot analysis of alpha-SMA. Both number of TUNEL, Ki-67 and alpha-SMA positive cells and expression of alpha-SMA were semi quantitatively analyzed to explore their effects on corneal haze.RESULTS:Many TUNEL-positive cells appeared in the central anterior stromal in early stages after both LASEK and Epi-LASIK, and the number of TUNEL-positive cells reached a peak 24 hours after either LASEK or Epi-LASIK. There were more TUNEL positive cells found in LASEK group than that in Epi-LASIK group in 1 week postoperatively (t = 3.63, 7.80, 4.34, 2.95, all P < 0.01). There were a lot of Ki-67-positive cells in anterior corneal stroma after both LASEK and Epi-LASIK, but the peak appeared at 72 hours after the treatments. The significant difference was also found in the number of Ki-67-positive cell between the two treatments in 1 week postoperatively (t = 3.81, 5.85, 5.09, 5.59, all P < 0.01). alpha-SMA-positive cells started to appear apparently at 1 week after both LASEK and Epi-LASIK, and both the peaks appeared at 1 month after the treatments, and there were still a lot of alpha-SMA-positive cells in corneal stroma at 3 months after LASEK. There were more alpha-SMA-positive cells found after 1 week in LASEK group than that in Epi-LASIK group (t = 2.97, 7.19, 6.73, all P < 0.01).CONCLUSION:There are less stromal cell apoptosis, proliferation and myofibroblast generation in Epi-LASIK than that in LASEK. Therefore, comparing with LASEK, Epi-LASIK induces less corneal haze response in correcting myopia.
Comparative Analysis of Corneal Wound Healing: Differential Molecular Responses in Tears Following PRK, FS-LASIK, and SMILE Procedures

Abstract:Background/Objectives: In this study, we aimed to analyze the changes in the expression profiles of selected messenger RNAs (mRNAs) and their encoded proteins in the tears of patients undergoing photorefractive keratectomy (PRK), femtosecond-assisted laser in situ keratomileusis (FS-LASIK), and small-incision lenticule extraction (SMILE) procedures. Methods: A total of 120 patients were divided into three groups based on the laser vision correction (LVC) procedure: PRK, FS-LASIK, or SMILE. Tear samples were collected preoperatively and at 1, 7, 30, and 180 days postoperatively. The expression levels of selected messenger RNAs (mRNAs) and proteins were analyzed by using reverse transcription quantitative polymerase chain reaction (RT-qPCR) and enzyme-linked immunosorbent assay (ELISA), respectively. Results: PRK and FS-LASIK elicited significantly stronger biological responses than SMILE. Interleukin-15 (IL-15) expression increased notably in the PRK and FS-LASIK groups, with mRNA levels reaching fold changes of 4.65 ± 0.65 and 4.99 ± 0.28, respectively, on day 1, compared with only 2.09 ± 0.23 in the SMILE group. Vascular endothelial growth factor A (VEGFA) levels were also elevated in the PRK (2.98 ± 0.23 fold change) and FS-LASIK groups (3.45 ± 1.09 fold change) on day 1, while the SMILE group showed minimal fluctuations. The protein concentration analysis based on the ELISA confirmed these trends, with IL-15 levels peaking at 54.2 ± 2.5 pg/mL in the PRK group and 52.8 ± 3.1 pg/mL in the FS-LASIK group, compared with 32.4 ± 1.9 pg/mL in the SMILE group on day 1. Similarly, VEGFA protein concentrations were the highest in the PRK (72.4 ± 4.1 pg/mL) and FS-LASIK patients (69.5 ± 3.8 pg/mL) on day 1 but remained low in the SMILE patients (45.6 ± 2.3 pg/mL). By day 180, gene expression and protein levels in all groups had stabilized, returning to near-preoperative values. Conclusions: PRK and FS-LASIK induced more pronounced molecular and protein-level changes during corneal wound healing than the less invasive SMILE procedure, indicating stronger biological responses. These findings suggest that tailored postoperative care based on the specific procedure could optimize healing and patient outcomes. However, further research with larger sample sizes and longer follow-ups is needed to confirm these observations and develop personalized treatment strategies.

Corneal Epithelial Remodeling Induced by Photorefractive Keratectomy Enhancement After Small-Incision Lenticule Extraction

Abstract:Purpose: To assess changes in corneal epithelial thickness (ET) within 9-mm diameter cornea after photorefractive keratectomy (PRK) retreatment after small-incision lenticule extraction (SMILE).Methods: A total of 28 eyes of 19 patients with mean spherical equivalent of -1.30 ± 0.60 D who underwent retreatment after SMILE were included in this retrospective study. ET mapped across a 9-mm diameter area was obtained using wide-field optical coherence tomography (OCT) before and at one, three, and six months after surgery. The ET changes were compared between the different time points and analyzed zones.Results: Before enhancement, the ET were 63.64 ± 6.01 µm and 61.25 ± 4.32 µm in central and paracentral zones, respectively. The ET of central and paracentral zones significantly decreased at one month and subsequently increased until six months. Six months after surgery, significant epithelial thickening occurred in 2- to 9-mm diameter cornea (all P < 0.05), whereas no significant change was observed in central 2-mm diameter cornea (P = 0.460). There was no significant difference in the ET between the central and paracentral zones (P = 1.00). The degree of myopic correction significantly correlated with the average ET in the central (P = 0.046) and paracentral (P = 0.033) zones at six months after PRK enhancement. No significant correlation was detected between the average ET of all zones and the postoperative spherical equivalent at six months after surgery (all P > 0.05).Conclusions: PRK enhancement did not alter the overall trend of corneal epithelial remodeling induced by SMILE. An asymmetric and flatter lenticule-like pattern of epithelial remodeling was observed six months after surgery, which did not affect the refractive outcomes.Translational Relevance: An asymmetric and centrally flattened lenticule-like pattern of epithelial remodeling was observed after PRK enhancement. Surgeons should consider expanding the intended optical zones for enhancement surgery after SMILE.

Abstract:Abstract Background The corneal biomechanical properties with the prolongation of time after corneal refractive surgery are important for providing a mechanical basis for the occurrence of clinical phenomena such as iatrogenic keratectasia and refractive regression. The aim of this study was to explore the changes of corneal elastic modulus, and stress relaxation properties from the 6-month follow-up observations of rabbits after a removal of anterior corneal tissue in simulation to corneal refractive surgery. Methods The anterior corneal tissue, 6 mm in diameter and 30–50% of the original corneal thickness, the left eye of the rabbit was removed, and the right eye was kept as the control. The rabbits were normally raised and nursed for 6 months, during which corneal morphology data, and both of corneal hysteresis (CH) and corneal resistance factor (CRF) were gathered. Uniaxial tensile tests of corneal strips were performed at months 1, 3, and 6 from 7 animals, and corneal collagen fibrils were observed at months 1, 3, and 6 from 1 rabbit, respectively. Results Compared with the control group, there were statistical differences in the curvature radius at week 2 and month 3, and both CH and CRF at months 1, 2, and 6 in experiment group; there were statistical differences in elastic modulus at 1, 3, and month 6, and stress relaxation degree at month 3 in experiment group. The differences in corneal elastic modulus, stress relaxation degree and the total number of collagen fibrils between experiment and control groups varied gradually with time, and showed significant changes at the 3rd month after the treatment. Conclusions Corneas after a removal of anterior corneal tissue undergo dynamic changes in corneal morphology and biomechanical properties. The first 3 months after treatment could be a critical period. The variation of corneal biomechanical properties is worth considering in predicting corneal deformation after a removal of anterior corneal tissue.

Abstract:Corneal opacification or scarring is one of the leading causes of blindness worldwide. Human limbus-derived stromal/mesenchymal stem cells (hLMSCs) have the potential of clearing corneal scarring. In the current preclinical studies, we aimed to determine their ability to heal the scarred corneas, in a murine model of corneal scar, and examined their ocular and systemic toxicity after topical administration to rabbit eyes. The hLMSCs were derived from human donor corneas and were cultivated in a clean room facility in compliance with the current good manufacturing practices (cGMP). Before the administration, the hLMSCs were analyzed for their characteristic properties including immunostaining, and were further subjected to sterility and stability analysis. The corneas (right eye) of C57BL/6 mice (n = 56) were stripped of their central epithelium and superficial anterior stroma using a rotary burr (Alger Brush® II). Few mice were left untreated (n = 8), while few (n = 24) were treated immediately with hLMSCs after debridement (prophylaxis group). The rest (n = 24, scar group) were allowed to develop corneal scarring for 2 weeks and then treated with hLMSCs. In both groups, the treatment modalities included encapsulated (En+) and non-encapsulated (En-) hLMSCs and sham (vehicle) treatment. The follow-up (4 weeks) after the treatment or debridement included clinical photography, fluorescein staining, and optical coherence tomography at regular intervals. All the images and scans were analyzed using ImageJ software to assess the changes in corneal haze, scar area, and the reflectivity ratio of the epithelium to the stroma. The scar area and the scar intensity were found to be decreased in the groups that received hLMSCs. The reflectivity of the stroma was found to be normalized to the baseline levels before the debridement in the eyes that were treated with hLMSCs, relative to the untreated. In the safety study, the central corneas of the left eye of 18 New Zealand rabbits were scraped with a needle and then treated with En+ hLMSCs, En- hLMSCs, and the sham (n = 6 each). Rabbits were then followed up for 4 weeks, during which blood and tear samples were collected at regular intervals. These rabbits were then assessed for changes in the quantities of inflammatory markers (TNF-α, IL-6, and IgE) in the sera and tears, changes in the ocular surface observations such as intraocular pressure (IOP), and the hematological and clinical chemistry parameters. Four weeks later, the rabbits were euthanized and examined histopathologically. No significant changes in conjunctival congestion, corneal clarity, or IOP were noticed during the ophthalmic examination. The level of inflammatory molecules (TNF-α and IL-6 TNF-α) and the hematological parameters were similar in all groups without any significant changes. Histological examination of the internal organs and ocular tissues did not reveal any abnormalities. The results of these studies summarize that the En+ and En- hLMSCs are not harmful to the recipient and potentially restore the transparency of debrided or scarred corneas, indicating that hLMSCs can be assessed for clinical use in humans.
Adhesion of Laser in Situ Keratomileusis–like Flaps in the Cornea: Effects of Crosslinking, Stromal Fibroblasts, and Cytokine Treatment

Abstract:PURPOSE: To evaluate 3 approaches, both cellular and acellular, to improve the healing of laser in situ keratomileusis flaps in bovine corneas.SETTING: School of Optometry and Vision Sciences and Cardiff Institute of Tissue Engineering and Repair, Cardiff University, Cardiff, United Kingdom.DESIGN: Experimental study.METHODS: Laser in situ keratomileusis like flaps were created in bovine corneas, and the flap bed was treated with tumor necrosis factor-alpha, interleukin-1 alpha, Fas ligand, transforming growth factor-beta(1), or activated stromal fibroblasts. In separate experiments, flaps were created and repositioned. The corneas were then crosslinked using ultraviolet-A (UVA) light. All samples were then placed in organ culture for up to 4 weeks. Untreated samples acted as controls.RESULTS: All treatments increased the adherence of the stromal flap. This was achieved at the expense of corneal clarity except in the case of crosslinking (CXL). In this case, the flap adhesion force immediately increased while the cornea remained clear. The force then decreased gradually during organ culture, although it remained at twice the level of the control corneas after 3 weeks in culture.CONCLUSIONS: The results suggest that riboflavin UVA CXL is a hopeful approach for increasing the adherence strength of corneal flaps while keeping the cornea clear. Further studies are necessary to confirm the durability of the strengthening effect and to exclude serious late complications.
Morphologic and Histopathologic Changes in the Rabbit Cornea Produced by Femtosecond Laser-Assisted Multilayer Intrastromal Ablation.

Abstract:PURPOSETo observe morphologic and histopathologic changes in the midperiphery of the rabbit cornea produced by femtosecond laser-assisted multilayer intrastromal ablation, determine whether this method may be used to correct myopia, and study how the cornea heals when the epithelium is not injured.METHODSThe right eyes of 10 New Zealand White rabbits were used for the experiments. A 60-kHz femtosecond laser delivery system was used, and three lamellar layers of laser pulses were focused starting at a corneal depth of 180 microm and ending at 90 microm from the surface, with each successive layer placed 45 microm anterior to the previous layer. In the interface of the applanation contact lens cone, a 6-mm diameter aluminum circle was placed at the center to block the laser, and ablation was limited to the midperiphery of the cornea. The laser settings were spot/line separation, 10 microm; diameter, 8.5 mm; energy for ablating the stroma, 1.3 microJ. Topography examination was used to document changes in corneal power. Light microscopy, transmission electron microscopy (TEM), and confocal microscopy in vivo were applied to observe changes in the cornea.RESULTSThere was significant change in mean corneal power between baseline and postoperative month 3 (n = 8; P = 0.0001), with a decrease from 46.82 D to 44.42 D. There was no haze formation or refractive regression throughout the follow-up. There were no corneal structural abnormalities under light microscopy. Activated keratocytes and necrotic debris were visible under confocal microscopy. Fibroblasts were observed, and no myofibroblasts appeared under TEM.CONCLUSIONSMultilayer intrastromal ablation by the femtosecond laser with intact epithelium in the midperiphery of the corneal stroma can flatten the cornea without causing haze formation or refractive regression. This procedure allows the cornea to heal differently than when traditional corneal refractive surgery is performed and the epithelium is damaged.
