Abstract:Urothelial carcinoma (UC) is the most common type of cancer of the bladder and upper urinary tract, and is characterized by a high risk of recurrence and progression. Urine fluorescence in situ hybridization (FISH) is a technique that detects genetic aberrations in exfoliated cells in the urine, with specific probes for chromosomes 3, 7 and 17 and the p16 gene. To evaluate the diagnostic value of FISH in UC, 119 patients from November 2010 to June 2012 with suspected UC were recruited into a prospective, cross-sectional study and were followed up for 12-30 months. These patients received voided urine cytology and FISH tests, and underwent cystoscopy and/or ureteroscopy as a reference standard. The final diagnoses confirmed 73 patients with UC, located in the bladder, upper urinary tracts or the two. The sensitivity of FISH for detecting UC was superior to cytology, irrespective of tumor grade and stage: Overall, 80.8 vs. 32.9% (P<0.001); low grade, 75.8 vs. 12.1% (P<0.001); high grade, 85 vs. 50% (P<0.005); non-muscle-invasive, 81.1 vs. 28.3% (P<0.001) and muscle-invasive, 80 vs. 45% (P<0.05), respectively. The specificities of the two tests were similar; overall, the specificity was 89.1% for cytology vs. 100% for FISH, and no significant difference was observed between the methods. Notably, FISH exhibited 100% sensitivity for cytologically non-diagnostic UC, but 33.3% specificity. In conclusion, FISH is a reliable and non-invasive diagnostic tool for bladder and upper urinary tract UC, particularly in patients with low-grade or early stage tumors.

Abstract:. Progression of disease in the native coronary circulation was studied in 60 patients before and after aortocoronary saphenous vein bypass surgery. The mean interval between the first angiographic study and operation was 2.0 months and the mean interval from operation to second catheterization was 15.3 months. The results from a second arteriographic study after an interval of 12.3 months on 25 comparable patients, with no surgery in between, served as a control material for frequency and severity of the “natural” progression of the disease. Only vessels which were not occluded at the first examination were included in the re-study. In the 60 operated patients, 106 arteries were grafted and 74 ungrafted. Total graft patency at the time of re-study was 66%. Progression was found in segments proximal to the anastomosis in 42% of arteries with patent graft, and in 47% of arteries with occluded graft, whereas in segments distal to the site of anastomosis progression was found in 5% of the arteries with patent graft and in 55% of arteries with occluded graft. Progression of the disease was found in 11% of ungrafted arteries. In the control group, progression of the disease was found in 24%. The study thus shows that after aortocoronary bypass surgery, progression in segments proximal to the anastomosis is significantly higher than in nongrafted arteries. The progression was independent of patency or occlusion of the graf to the vessel. Progression in the segments distal to the anastomosis was not significantly different from the frequency of progression seen in nongrafted arteries if the graft was patent, but was much higher if the graft was occluded.
Visually imprinted maternal preference in ducklings is redirected by social interaction with siblings.

Abstract:This study examined the influence of social interaction with broodmates on a visually imprinted maternal preference in domestic mallard ducklings (Anas platyrhynchos) during the first three days of postnatal life. Twenty-four-hour-old ducklings were allowed to follow a stuffed mallard hen for 30 min. This experience resulted in a visually imprinted preference for the familiar stuffed hen over 4 stuffed ducklings in simultaneous choice tests at 48 and 72 hr after hatching, but only if the imprinted birds were kept in social isolation. Imprinted ducklings reared in a social group containing either same-age siblings or the stuffed mallard hen and same-age siblings displayed a visual preference for the stuffed ducklings over the familiar mallard hen at 48 and 72 hr tests. As a first step in the analysis of the features of the social exposure to siblings that are necessary for the redirection of the visually imprinted maternal preference, it was found that direct social interaction is essential to establish the preference for ducklings, but even mere visual exposure to siblings does away with the imprinted maternal preference. These results provide striking evidence of the ready redirection of a species-specific maternal preference in otherwise imprinted ducklings.
The Clinical Application of Fluorescence in Situ Hybridization in Diagnosing Urothelial Carcinoma

Abstract:Background: Chromosomal aberrations in exfoliated urothelial cells have been associated with the development of urothelial carcinoma. This study aimed to evaluate the efficacy of two kinds of fluorescence in situ hybridization (FISH) kit probes in diagnosing urothelial carcinoma (UC). Methods: From February 2009 through September 2014, urine specimens from 89 consecutive patients with urothelial carcinoma and 11 controls with benign disease were collected and analyzed by means of GP FISH and cytology. Urine samples from 50 consecutive patients with urothelial carcinoma and 68 patients with non-urothelial carcinoma with hematuria were also collected and analyzed by UroVysion FISH and cytology. The sensitivity and specificity of two kinds of FISH and cytology in different stages and grades of urothelial carcinoma were statistically analyzed via SPSS 17.0 and compared. Results: The overall sensitivity of GP FISH and cytology were 88.8% (79/89) and 55.1% (49/89), respectively (p < 0.001), and the overall specificity was 90.9% (10/11) and 100.0% (11/11), respectively (p > 0.05). The overall sensitivity of UroVysion FISH and cytology was 68.0% and 18.0%, respectively (p < 0.05), and the overall specificity was 91.2% and 98.5%, respectively (p > 0.05). The sensitivity of FISH and cytology in diagnosing UUT-UC were 88.9% (16/18) and 44.4%(8/18),respectively. The sensitivity of GP FISH and UroVysion FISH in diagnosing UUT-UC were 100% (10/10) and 75.0% (6/8) respectively. Conclusions: FISH is a non-invasive technique with higher sensitivity and similar specificity compared with urine exfoliated cell cytology in diagnosing urothelial carcinoma by detecting the aberrations of chromosome 3, 7, 17, and 9p21. FISH is significantly superior to cytology in diagnosing low stage and low grade urothelial carcinomas. FISH is a very valuable and promising technique in diagnosing urothelial carcinomas.
Comparison of urinary cytology and fluorescence in situ hybridization in the detection of urothelial neoplasia: An analysis of discordant results

Abstract:Background: We examine the performance of cytology and FISH in the detection of urothelial carcinoma (UC), and explore the reasons for discrepant results, and potential clinical implications. Methods: Urine samples from 89 patients were prospectively collected for simultaneous cytology and UroVysion FISH, and results correlated with concurrent biopsies and/or clinical or histologic follow-up data. Corresponding tissue biopsies, where available, were also evaluated by FISH. Results: Sensitivity and specificity of cytology and FISH for the detection of UC was 54.8% and 92% and 50% and 88%, respectively. Only one of seven false-positive urinary FISH results proved to be an "anticipatory positive" on extended follow-up. Five of eight (62.5%) high grade (HG) carcinomas with false-negative urinary FISH, were negative due to the absence/paucity of FISH-detectable changes in the tumor cells. In atypical cytology cases, the FISH result did not assist in identifying UC. There was no significant difference between an atypical cytology result and a positive FISH result, with respect to the identification of patients with UC. Conclusions: We found urinary cytology to be more sensitivity and specific than FISH in the detection of UC, though the difference was not statistically significant. Up to 24% of HG UCs are FISH negative due to an absence of FISH-detectable abnormalities in the tumor cells. Paucity of neoplastic cells in the urine also contributes to false-negative FISH results in both HG and low grade tumors. Negative urinary FISH cannot be taken alone as indicating the absence of significant disease in patients with atypical cytology.

Abstract:Objective To evaluate the possibility and validity of detecting numerical aberration of chromosomes 3,7,9 and 17 by fluorescence in situ hybridization(FISH)in order to diagnose urothelial carcinoma of bladder in Chinese.Methods A total of 140 volunteers were recruited as subjects,including 80 hematuria patients suffering from bladder urothelial carcinoma,30 hematuria patients with benign bladder diseases,and 30 healthy individuals.Exfoliated cells from fresh urinary specimen was collected and examined by FISH using centromeric probes of chromosome 3,7,17 and region probe of 9p21.Cytology and transabdominal ultrasonography(US)was applied at the same time and all of the patients were definitely diagnosed by cystoscopy and biopsy.The sensitivity and specificity of each method in the diagnosis of bladder cancer were determined by comparative analysis.Results Among the 80 subjects in tumor group which were verified with pathology,68 patients showed positive by FISH,28 were positive by urine cytology,and 71 were positive by US.In non-tumor control group of 30 cases,29 showed negative and 1 false-positive by FISH,all negative by urine cytology,23 negative and 7 false-positive by US.The sensitivity and specificity of FISH,urine cytology and US on detecting bladder cancer were 85.00%,32.50%,88.75%,and 96.67%,100.00%,76.67%,respectively.Conclusion The sensitivity and specificity of FISH detecting 3,7,9,17 chromosome aberrations are high in the diagnosis of bladder cancer.It can be used as an effective method for assistance diagnosis of bladder cancer.

Abstract:To assess the clinical utility of a fluorescence in situ hybridization (FISH) assay as a non-invasive method for diagnosing and monitoring urothelial carcinoma (UC) in the upper urinary tract (UUT).Urine specimens from 63 consecutive patients with UUT-UC and 69 controls with benign disease were analyzed by means of cytology and FISH. For FISH analysis, labeled probes specific for chromosomes 3, 7, and 17 and for the p16 (9p21) gene were used to assess chromosomal abnormalities indicative of malignancy.Sensitivity and specificity of both techniques were determined and compared. The frequency of chromosomal aberrations of malignant cells from UUT-UC was also determined.Of 63 patients with UUT-UC, FISH affords an overall sensitivity of 84.1% (53/63), the figure being 71.4% (20/28) for PTa and PT1 tumors,94.3% (33/35) for PT2-4 tumors. The sensitivities of urine cytology were 35.7% (10/28) for PTa and PT1 tumor,45.7% (16/35)for PT2-4 tumors,with an overall sensitivity of 41.3% (26/63). The sensitivities of the two methods for the low grade tumors were 80% (20/25) and 44% (11/25), and for high grade tumors were 86.8% (33/38) and 39.5% (15/38), respectively. Specificities for FISH and urine cytology were 91.3% (63/69) and 94.2% (65/69)respectively.According to the results,the sensitivity of FISH for the detection of UUT-UC is superior to that of urine cytology and the specificities of FISH and urine cytology are not significantly different. FISH can promote the diagnosis of UUT-UC, especially for the low stage and low grade cases,it may be a new promising non-invasive method for the diagnosis of UUT-UC.
Application of Double Fluorescence in Situ Hybridization Technology in the Diagnosis of Bladder Urothelial Carcinoma

Abstract:[Objective]To evaluated the performance of a dual-color FISH (fluorescence in situ hybridization) assay for urothelial carcinoma (UC) of the bladder, in comparison to urinary cytology[.Methods]Fluorescence in situ hybridization (FISH) was performed using centromeric probes of chromosome 7, 9, 17 and 9p21 region to examine chromosome aberration of exfoliated cells in 78 urine samples from patients with urothelial carcinoma (UC) of the bladder, 30 urine samples from healthy control. All of the samples were analyzed by urinary cytology at the same time[.Results]The sensitivity of urine cytology for Ta, T1 and T2-T4 tumors was 35.5% (11/31), 54.1 %(13/24) and 69.6% (16/ 23), respectively, for an overall sensitivity of 51.6%. The sensitivity of FISH for Ta, T1 and T2-T4 tumors was 64.5% (20/31), 87.5% (21/24) and 95.7% (22/23), respectively, for an overall sensitivity of 80.8%. FISH was significantly more sensitive than cytology for T1 (P =0.047), T2-T4 (P =0.0235), grade Ⅱ (P =0.004), grade Ⅲ(P =0. 002) and all tumors (P =0.001). The specificity of cytology and FISH was 98% and 96%, respectively (P =0.612)[.Conclusion] The sensitivity of FISH for the detection urothelial carcinoma (UC) of the bladder is superior to that of cytology, and the specificity of FISH and cytology for urinary bladder cancer are not significantly different. FISH can improve significantly the diagnosis of urinary bladder cancer and will be significant for screening of urinary bladder cancer.
Fluorescence in situ hybridization status of voided urine predicts invasive and high-grade upper tract urothelial carcinoma

Abstract:Here, we assessed the diagnostic accuracy of fluorescence in situ hybridization (FISH) for detecting aggressive upper tract urothelial carcinoma (UTUC). A total of 210 UTUC patients from a single hospital in China were enrolled in this retrospective study between 2012 and 2016. Patients were classified as FISH-positive or -negative based on FISH analysis of voided urine, and the clinicpathological characteristics of these two patient groups were compared. Patients with positive FISH results had more advanced tumor stages and higher tumor grades than those with negative results. The overall sensitivity of FISH for detecting advanced UTUC was 62.4% (131/210). The sensitivity and positive predictive values of positive FISH results were 76.5% and 59.5% for high-grade UTUC and 71.7% and 58.0% for muscle-invasive UTUC. These data suggest that voided urine FISH results accurately predict advanced UTUC and might help distinguish patients with aggressive disease from those who might benefit from conservative management.

Abstract:UroVysionTM is a fluorescence in situ hybridization assay that was developed for the detection of bladder cancer (UC accounted for 90%) in urine specimens. It consists of fluorescently labeled DNA probes to the pericentromeric regions of chromosomes 3, 7, 17 and to the 9p21 band location of the P16 tumor suppressor gene, which was approved by the Food and Drug Administration (FDA) in 2001 and 2005, respectively, for urine detection in patients with suspected bladder cancer and postoperative recurrence monitoring. Furthermore, recent studies also demonstrated that U-FISH was useful for assessing superficial bladder cancer patients' response to Bacillus Calmette-Guérin therapy and in detecting upper tract urothelial carcinoma. Therefore, positive U-FISH was well known to urologists as a molecular cytogenetic technique for the detection of UC. However, with the continuous enrichment of clinical studies at home and abroad, U-FISH has shown a broader application space in the detection of various urinary primary tumors and even metastatic tumors. This review focuses on summarizing the research status of U-FISH in UC, non-urothelial carcinoma and metastatic tumor, so as to strengthen urologists' more comprehensive understanding of the application value of U-FISH and better complete the accurate diagnosis and treatment of urological cancers.
A Modification of the von Haberer‐Finney Operation

Abstract:Introduction Von Haberer (1922) and Finney (192'1), after performing partial gastrectomy for gastric and/ or duodenal ulcer, anastomosed the entire open end of the gastric pouch to the side of the second portion of the duodenum after the duodenal stump had been securely closed and inturned. Finney was carrying out the same procedure in 1924 and at a time when he had no knowledge that von Haberer was performing this procedure for benign or malignant lesions of the stomach. Schoemaker (Ell)) was the first surgeon to extend and simplify gastric resection [or duodenal ulcer and [or benign or malignant lesions of the stomach by excising most of the lesser curvature before the performance of end-to-end gas trod uodenosrom y (Schoemaker-Bi llroth I) or end-to-side gastrojejunostomy (SchoemakerBillroth II). He himself had no trouble from leakage at the [ammercckc (critical angle) although some of his contemporaries reported a number of deaths from spreading peritonitis caused by the breaking down of this dangerous junction. At the present time the majority of surgeons have adopted and simplified Schoemnkers plan of "tubing" the gastric pouch before anastomosing its mouth to the duodenum or proximal jejunum. This technique ensures the maximum mobility of the pouch, facilitates the fashioning of the stoma and eliminates any abnormal upward projection or extension or gastrin-pro· clueing mucosa (Fig. I). In 1950 I modified the von Haberer-Finney operation as well as the Schoemaker-Billroth I procedure on the lines which arc clearly depicted in the illustrations. Utility of Urovysion FISH in Improving the Diagnostic Performance of Urine Cytology

Abstract:Introduction: The atypical urothelial cell (AUC) category in The Paris System (TPS) in urine cytology (UrCy) is a challenging area. This study aims to evaluate the UroVysion fluorescence in-situ hybridization assay(U-FISH) in predicting the outcome of AUC. Additionally, we explored the association of abnormal U-FISH results in high-grade urothelial carcinoma (HGUC) concerning muscularis propria invasion(MPI). Methods: This is a retrospective study, and U-FISH was done on archived Papanicolaou-stained smears. Four cohorts were included: AUC-NN (Non-Neoplastic), AUC-N (Neoplastic), HGUC-MI (muscle-invasive), and HGUC-MF (muscle-free) outcome on histopathology (HPE) and with clinical follow-up of 12 to 29 months. U-FISH was evaluated for diagnostic purposes, and MPI and tumor stage prediction by U-FISH score (UFS; high vs. low) based on copy number gain of chromosomes (Chr). Results: U-FISH was performed on 70 cases (20-AUC-NN, 20-AUC-N, 15-HGUC-MI, and 15-HGUC-MF) and was successful in 58/70 (82.85%) cases. All UC cases showed amplification of ≥2Chr, and all the AUC-NN cases reported non-neoplastic on HPE were negative for U-FISH. U-FISH picked up all carcinoma cases in the AUC-N cohort. Chr3 amplification was statistically significant in differentiating HGUC-MI from HGUC-MF and LGUC cases. Chr3 signals with a cut-off of 6 signals could identify MPI with a sensitivity of 80.95% and specificity of 41.94%. The UFS of the HGUC-MI group was significantly higher than HGUC-MF. Conclusions: U-FISH successfully identified all cases of AUC with neoplastic outcomes. In the HGUC group, there was a difference in cases with and without MPI, which requires further confirmation in a larger prospective cohort.

Abstract:Background: Positive UroVysion TM fluorescence in situ hybridization (FISH) is generally considered as urothelial carcinoma (UC). We clarify if UroVysion TM FISH can be positive in carcinoma of non-urothelial lineages (CNUL), and verify the consistency of urine FISH and histological FISH in CNUL. Methods: All CNUL subjects detected by urine FISH assay due to haematuria from Tongji Hospital were screened. Meanwhile, 2 glandular cystitis and 2 urothelial carcinoma were served as negative or positive control. Paraffin-embedded tissue sections of all subjects were sent to the pathology department for histological FISH detection. Results: A total of 27 patients were included in this study, including 9 with adenocarcinomas, 11 with squamous cell carcinomas, and 7 with other tumour types. The overall positive rate in urine FISH was 64.00% (16/25) in patients with CNUL, 77.78% (7/9) in those with adenocarcinoma and 54.55% (6/11) in those with squamous carcinoma. There was a significant difference in the GLP p16 gene deletion rate between UC and CNUL (100% vs. 8.00%, p = 0.017). Histological FISH results showed that the histological results of 19 patients were consistent with their urine FISH results, and only one patient with stage IIIa urachal carcinoma had inconsistent histological FISH results (positive) and urine FISH (negative) results. Conclusion: We demonstrated for the first time the application value of FISH in CNUL on urine samples. Positive urine FISH tests indicate not only UC, but also CNUL. UroVysion TM FISH possibly has a high positive rate in CNUL. CNUL and UC have different genetic changes shown by FISH.

Abstract:Bladder cancer is characterized by its frequent recurrence and progression. Effective treatment strategies need to be based on an accurate risk stratification, in which muscle invasiveness and tumor grade represent the two most important factors. Traditional imaging techniques provide preliminary information about muscle invasiveness but are lacking in terms of accuracy. Although as the gold standard, pathological biopsy is only available after the surgery and cannot be performed longitudinally for long-term surveillance. In this work, we developed a microfluidic approach that interrogates circulating tumor cells (CTCs) in the peripheral blood of bladder cancer patients to reflect the risk stratification of the disease. In a cohort of 48 bladder cancer patients comprising 33 non-muscle invasive bladder cancer (NMIBC) cases and 15 muscle invasive bladder cancer (MIBC) cases, the CTC count was found to be considerably higher in the MIBC group compared with the NMIBC group (4.67 vs. 1.88 CTCs/3 mL, P=0.019), and was significantly higher in high-grade bladder cancer patients verses low-grade bladder cancer patients (3.69 vs. 1.18 CTCs/3mL, P=0.024). This microfluidic assay of CTCs is believed to be a promising complementary tool for the risk stratification of bladder cancer.
Application of fluorescence in situ hybridization in the detection of bladder transitional-cell carcinoma: A multi-center clinical study based on Chinese population

Abstract:Objective: To evaluate the diagnostic value of fluorescence in situ hybridization (FISH) in bladder cancer. Methods: We enrolled healthy volunteers and patients who were clinically suspected to have bladder cancer and conducted FISH tests and cytology examinations from August 2007 to December 2008. Receiver operating characteristic (ROC) curve analysis was performed and the area under curve (AUC) values were calculated for both the FISH and urine cytology tests. Results: A cohort of 988 healthy volunteers was enrolled to establish a reference range for the normal population. A total of 4807 patients with hematuria were prospectively, randomly enrolled for the simultaneous analysis of urine cytology, FISH testing, and a final diagnosis as determined by the pathologic findings of a biopsy or a surgically-excised specimen. Overall, the sensitivity of FISH in detecting transitional-cell carcinoma was 82.7%, while that of cytology was 33.4% (p < 0.001). The sensitivity values of FISH for non-muscle invasive and muscle invasive bladder transitional-cell carcinoma were 81.7% and 89.6%, respectively (p = 0.004). The sensitivity values of FISH for low and high grade bladder cancer were 82.6% and 90.1%, respectively (p = 0.002). Conclusion: FISH is significantly more sensitive than voided urine cytology for detecting bladder cancer in patients evaluated for gross hematuria at all cancer grades and stages. Higher sensitivity using FISH was obtained in high grade and muscle invasive tumors.

Abstract:We demonstrate the temporal contrast enhancement in a petawatt-class Ti:sapphire chirped-pulse amplification (CPA) laser system. An extra saturable absorber, introduced downstream after a low-gain optical parametric chirped-pulse amplification (OPCPA) preamplifier, has improved the temporal contrast in the system to 1.4×10(12) on the subnanosecond time scale at 70 TW power level. We have achieved 28 J of uncompressed broadband output energy with this system, indicating the potential for reaching peak powers near 600 TW.

Abstract:Objectives: To evaluate the diagnostic values of urine liquid-based cytology (LBC), fluorescence in situ hybridization (FISH), and p16/Ki-67 dual immunostaining in the detection of upper tract urothelial carcinomas (UTUCs). Methods: Sixty-one patients with UTUCs were retrospectively analyzed. All patients were diagnosed both by urine cytology and by FISH, and histologically confirmed as UTUCs. Thirty-two patients had been stained with p16/Ki-67 dual labeling. Results: The sensitivities for low-grade UTUCs (LGUTUCs) and high-grade UTUCs (HGUTUCs) were 33.3% and 67.4% by LBC, 60% and 69.6% by FISH, and 12.5% and 66.7% by p16/Ki-67 dual labeling, respectively. Conclusions: Our results indicated that LBC was more suitable to identify HGUTUCs, FISH was highly valuable for predicting LGUTUCs, and p16/Ki-67 dual labeling was useful for distinguishing HGUTUCs from LGUTUCs. The combined application of these methods may improve the sensitivity or accuracy in the detection or diagnosis of UTUCs.
Diagnostic value of Xpert® BC Detection, Bladder Epicheck®, Urovysion® FISH and cytology in the detection of upper urinary tract urothelial carcinoma

Abstract:Purpose: Following the current guidelines, diagnosis and staging for upper urinary tract tumours (UTUC) can be performed with Computed Tomography, urography, ureterorenoscopy (URS) and selective cytology. The aim of the study was to evaluate the performance of the Xpert®-BC-Detection and the Bladder-Epicheck®-test in the detection of UTUC and compare it with cytology and the Urovysion®-FISH test using histology and URS as gold standard. Methods: A total of 97 analyses were collected through selective catheterization of the ureter before URS to test for cytology, Xpert®-BC-Detection, Bladder-Epicheck® and Urovysion®-FISH. Sensitivity, specificity, and predictive values were calculated using histology results/URS as reference. Results: Overall sensitivity was 100% for Xpert®-BC-Detection, 41.9% for cytology, 64.5% for Bladder-Epicheck® and 87.1% for Urovysion®-FISH. The sensitivity of Xpert®-BC-Detection was 100% in both, LG and HG tumours, sensitivity of cytology increased from 30.8% in LG to 100% in HG, for Bladder-Epicheck® from 57.7% in LG to 100% in HG and of Urovysion®-FISH from 84.6% in LG to 100% in HG tumours. Specificity was 4.5% for Xpert®-BC-Detection, 93.9% for cytology, 78.8% for Bladder-Epicheck® and 81.8% for Urovysion®-FISH. PPV was 33% for Xpert®-BC-Detection, 76.5% for cytology, 58.8% for Bladder-Epicheck® and 69.2% for Urovysion®FISH. NPV was 100% for Xpert®-BC-Detection, 77.5% for cytology, 82.5% for Bladder-Epicheck® and 93.1% for Urovysion®FISH. Conclusion: Bladder-Epicheck® and Urovysion®FISH along with cytology could be a helpful ancillary method in the diagnosis and follow-up of UTUC while due to its low specificity Xpert®-BC Detection seems to be of limited usefulness.
