Abstract:Oral squamous cell carcinoma (OSCC) is one of the most common types of human cancer in the world. Oral leukoplakia (OLK) is the most commonly diagnosed premalignant lesion in the oral cavity and it is also most associated with the development of OSCC. However, the mechanism of malignant transformation from OLK to OSCC is not completely understood. In this study, we attempted to evaluate expression of Aurora A and the main effectors in the Ras/Raf/MEK/ERK pathway in OSCC and OLK clinical tissues. Protein expression was analyzed by immunohistochemical technology in 25 tissue samples of OLK and 21 OSCC clinical tumor specimens. Upon analyzing the levels of Aurora A, phospho-MEK1/2 and phospho-ERK1/2, a remarked increase was observed in tumors compared to OLK tissues. No statistically significant difference between tumor and OLK tissues was found for pRaf-1 S259 and ERK1/2. These results suggest that the increase in Aurora A, phospho-MEK1/2 and phospho-ERK1/2 expression was closely correlated with malignant transformation of OLK. Furthermore, Aurora A, phospho-MEK1/2 and phospho-ERK1/2 might be coordinated for valuable prognostic indicators of malignant transformation of OLK. In addition, more than 50% pERK1/2 immunopositive cells appears to be a good cut off value to predict the malignant transformation of OLK.

Abstract:To investigate the expressions and significances of P57(kip2), p53 and hsp60 proteins in the dysplasia-carcinoma sequence of oral mucosa. A retrospective study was performed in 10 cases of normal oral mucosa, 79 cases of leukoplakia and 67 cases of squamous cell carcinoma (SCC). The expressions of P57(kip2), p53 and hsp60 proteins were detected in the tissue samples of these populations using immunohistochemical method. P57(kip2) expression was decreased in oral leukoplakia with moderate or severe dysplasia, and further decreased in oral SCC. Negative expression of P57(kip2) was significantly associated with advanced tumor size, the occurrence of lymph node metastasis and the advanced clinical stage in oral SCC. The overall 5-year survival rate in the P57(kip2) positive group was significantly higher than that in the P57(kip2) negative group. P57(kip2) expression was decreased in oral leukoplakia with moderate or severe dysplasia, and further decreased in oral SCC. It was a remarkable progressive and prognostic biomarker in oral SCC.
Expressions of P53 and Nm23 Proteins in Oral Squamous Cell Carcinoma

Abstract:Objective:To investigate the expressions of P53and nm23 proteins in cancer tissue of patients with oral squamous cell carcinoma.Methods:There were 57 males and 43 females,ranging in age from 35 to 75 years old.The clinical stages were T1 10,T2 29,T3 56 and T4 5;pathologic classifications were high differentiation in 74,medial and low differentiation in 26;lymph node metastasis in 56,no metastasis in 44.The expressions of P53 and nm23 proteins were detected by immunohistochemical technique.Results:The expressions rates of P53and nm23 proteins were 58.0%(58/100) and 40.0%(40/100) respectively (P＜0.05)in cancer tissue of patients with oral squamous cell carcinoma.The expressions rates of P53and nm23 were 71.5%(40/56)and 28.6%(16/56)in group of patients with node lymph node metastasis;40.9%(18/44)and 54.5%(24/44) in group of patients with negative metastasis respectively.Conclusion:Lymph node metastasis might be correlated with high level expression of P53 protein and low level expression of nm23 protein in patients of oral squamous cell carcinoma.
DNA Ploidy Property in Cancer Nests and Tissue of Incisional Edge of Oral Squamous Cell Carcinomas.

Abstract:Objective:To investigate DNA ploidy and S-phase fractio n of cancer nests ,the tissue of incisional edge and normal mucosa from patients with oral squamous cell carcinoma.Methods:DNA ploidy of oral squamous cell carcinoma,the tissue of incisional edge and oral mucosa obtained from 39 patients with oral squamous cell carcinoma were detected by FACS.S-phase fraction of them was calculated.Results:The aneuploidy rate of oral squamous cell carcinoma and the tissue of incisional edge was 38%(15/39) and 31%(12/39),respectively.There was no statistical difference between them in the aneuploidy rate.But the aneuploidy rate of two was significantly higher than those of oral mucosa 15%(6/39).The incidence of cervical lymph node metastasis was significantly (P0.025)higher in the aneuploidy cases(6/15) than in the diploid cases(5/24) from cancer nests.There was no significant difference for the incidence of cervical lymph node metastasis between the aneuploidy cases and the diploid cases from the tissue of incisional edge.S-phase fraction of cancer nests and the tissue of incisional edge were 23±8.6%?26±9.2%,respectively.S-phase fraction was significantly higher in them than in oral mucosa 12±4.9 %(P0.5).Conclusions:These results suggest a significant relationship between aneuploidy and incidence of the cervical lymph node metastasis.The tissue of incisional edge maybe have the potentiality of malignant transformation.

Abstract:Introduction: Despite advances in diagnostics and therapeutics, two-thirds of oral cancer patients present with advanced disease, which increases both the morbidity and mortality risk. Circulating tumour cells (CTCs) are released in the circulation by primary tumours and have been demonstrated to have significant correlations between their occurrence and disease progression. Objectives: To characterize the circulating tumour cells in subjects with histologically diagnosed oral squamous cell carcinoma (OSCC). Materials and Methods: This pilot study was undertaken with ten fresh blood samples (6 ml each). Five samples from apparently healthy individuals and five OSCC samples were cultured and subjected to flow cytometric analysis for CD44 expression. Immunostaining was done using CD44 and EpCAM markers. Result: Several cells in OSCC samples showed EpCAM and CD44 positivity following immunostaining. However, flow cytometry performed with CD44 alone was not specific for OSCC samples. Hence, proving that CD44 and EpCAM when used in conjunction can help to characterize CTCs. Conclusion: The findings of our study suggest that the demonstration of CTCs is feasible and helps in understanding of disease progression and metastatic risk. Sensitive detection of CTCs from blood samples can serve as an implicit tool in early cancer diagnosis and prognosis through liquid biopsy which in itself is minimally invasive and time-saving.
Increased levels of CK19 mRNA in oral squamous cell carcinoma tissue detected by relative quantification with real-time polymerase chain reaction.

Abstract:Oral squamous cell carcinoma (OSCC) is the most common malignant tumour in the oral and maxillofacial region and has a poor prognosis. Cytokeratin 19 (CK19) is a component of cytoskeleton protein. Previous studies have reported abnormal expression of CK19 protein in OSCC tissue. This study is to investigate the quantitative level of CK19 gene transcript in OSCC tissue as well as its clinical significance. Thirty-one OSCC patients (26 males and 5 females) took part in the present study, aged 34–78 years (mean 58.2 years). The level of CK19 mRNA was detected using fluorescent quantitative real-time reverse transcriptase polymerase chain reaction (RT-PCR) in cancerous and paracancerous tissues. The relative quantification in cancerous tissue compared with paracancerous tissue was calculated using the 2−ΔΔCt equation. The level of CK19 mRNA in cancerous tissue from OSCC patients was 2.21-fold higher than that in paracancerous tissue (P=0.020), and the amplicon was specific without genomic DNA contamination. The level of CK19 mRNA correlated significantly with the pathological differentiation grade of OSCC tissue (P=0.025), with poorer differentiation indicating a higher level of CK19 mRNA. These results suggest that fluorescent quantitative real-time RT-PCR is accurate and reliable for the detection of CK19 gene transcript levels in OSCC tissue. The level of CK19 mRNA was increased in OSCC tissue, and this was significantly correlated with the pathological differentiation grade.

Abstract:Background: Oral squamous cell carcinoma (OSCC) is one of the most prevalent cancers in the world. OSCC is a highly invasive lesion frequently having soaring morbidity as well as substantial mortality, attributed to resistance to therapy, metastasis, and recurrence driven by specific populations of cancer stem cells (CSC). The evidence of the association of expression of stem cell biomarker CD44 and metastatic potential of the tumor is inconclusive in OSCC and hence needs further evaluation. Objectives: To determine the immunohistochemical expression of CD44 in OSCC and to find its association with lymph node metastasis and TNM staging. Materials and methods: One hundred and five histologically proven cases of OSCC were studied. Histopathological parameters like depth of invasion, presence of lymph node metastasis, grading, and TNM staging were done according to the new AJCC staging criteria. Both intensity and proportion of CD44 expression were recorded. Results: The mean age observed in this study was 52.59 years with a male: female ratio of 1:3.76. Forty-nine cases (46.6%) showed a depth of invasion of more than 10 mm. Fifty-two out of one hundred and five cases (49%) had nodal involvement. TNM staging was 5.7%, 7.6%, 44.7%, and 42% for stages I, II, III, and IV, respectively. The majority of the cases (87.5%) showed CD44 expression in the tumor. There was a significant association between the CD44 expression and lymph node metastases (P < 0.001). Higher CD44 expression was seen in stages III and IV (P < 0.001). Conclusion: CD44, a stem cell biomarker is significantly associated with higher TNM stage and lymph node metastases. This may be useful in predicting the tumor behavior in the small biopsy.
Cyclin D1 Gene Numerical Imbalances in Laryngeal Squamous Cell Carcinoma: A Tissue Microarray Grid Based Analysis

Abstract:Background: Deregulation of critical proteins involved in cell cycle stability, such as cyclins, is a frequent genetic event in the development and progression of solid malignancies. Concerning laryngeal squamous cell carcinoma (LSCC), cyclin D1 oncogenic transformation leads to an aberrant protein expression and seems to affect the biological behaviour of the neoplasm. The aim of this study was to determine the correlation of cyclin D1 numerical imbalances with the corresponding protein expression levels in LSCCs. Material and method: Using tissue microarray (TMA) technology, fifty (n=50) histologically confirmed primary LSSCs were cored at a diameter of 1.5 mm. Immunohistochemistry (IHC) and chromogenic in situ hybridization (CISH) analyses were applied. Concerning the screening process in CISH slides, a novel real-time reference and calibration grid platform was implemented. Results: Protein overexpression was observed in 22/50 (44%) cases; whereas, gene amplification was seen in 13/50 (26%) cases (p=0.02). Combined protein/ gene deregulation was associated with the stage of malignancy (p= 0.0014, p=0.001), whereas overall protein expression was strongly correlated with the grade of tumour (p= 0.001). Conclusion: Cyclin D1 gene amplification led to aberrant protein expression in LSCCs and it was also correlated with an aggressive biological behaviour. To best of our knowledge, this study was the first described grid based CISH analysis under conventional bright field microscopy for detecting gene numerical imbalances while providing a novel and accurate description for screening-mapping process in the entire slide area.<br />.
Tumour Budding as a Predictive Factor for Lymph Node Metastases in Preoperative Oral Cancer Biopsies: A Retrospective Study

Abstract:Introduction: The enormous advancement in understanding of Oral Squamous Cell Carcinomas (OSCCs) has not been accompanied by a significant reduction in the high morbidity and mortality rates associated primarily with disease recurrence and lymph node metastases. Aim: To evaluate the tumour budding as an independent prognostic marker to predict lymph node metastasis in preoperative biopsies. Materials and Methods: The present retrospective study was conducted in the Department of Pathology, Guntur Medical College (tertiary care centre), Andhra Pradesh, India, from January 2018 to December 2021. Samples collected from 32 patients with preoperative diagnostic oral cavity biopsies, who also underwent resection with cervical lymph node dissection. The degree of differentiation, preoperative tumour budding, postoperative tumour budding, and intratumoural budding in preoperative biopsies were all assessed histologically in each case. The results were analysed using Chi-square test, KaplanMeier method and log-rank test. Results: Thirty two OSCC cases were examined by Haematoxylin and Eosin (H&E) stained slides. By using univariate analysis, the histological factors like depth of invasion (p-value=0.04), pattern of invasion (p-value=0.004), presence of preoperative tumour budding (p-value=0.008), postoperative tumour budding (p-value=0.004), intratumoural tumour budding (p-value=0.019), and sex (p-value=0.03) all significantly associated with risk of lymph node metastasis. Other clinical and histological factors, including age, the largest tumour size, histological grade, Lymphovascular Invasion (LVI), Perineural Invasion (PNI), and stromal response, did not significantly associate with the probability of lymph node metastasis. Conclusion: The potential of morphological features, such as Tumour Budding (TB) evaluated in OSCC diagnostic preoperative biopsies may aid in identifying patients who may benefit from more aggressive treatments.
Revelation of comprehensive cell profiling of primary and metastatic tumour ecosystems in oral squamous cell carcinoma by single-cell transcriptomic analysis

Abstract:Background: The analysis of single-cell transcriptome profiling of tumour tissue isolates helps to identify heterogeneous tumour cells, neighbouring stromal cells and immune cells. Local metastasis of lymph nodes is the most dominant and influential biological behaviors of oral squamous cell carcinoma (OSCC) in terms of treatment prognosis. Understanding metastasis initiation and progression is important for the discovery of new treatments for OSCC and prediction of clinical responses to immunotherapy. However, the identity of metastasis-initiating cells in human OSCC remains elusive, and whether metastases are hierarchically organized is unknown. Therefore, this study was conducted to understand the cellular origins and gene expression signature of OSCC at the single-cell level. Methods: Single-cell RNA sequencing (scRNA-seq) was used to analyze cells from tissue of para-carcinoma (PCA: adjacent normal tissue not less than 2 cm from the tumour), carcinoma (CA), lymph node metastasis (LNM) from patients with OSCC and PCA and CA tissue from patients with second primary OSCC (SPOSCC) after radiotherapy of nasopharyngeal carcinoma (NPC). The cell types and their underlying functions were classified. The comparisons were then conducted between the homology and heterogeneity from cell types and both conservative and heterogeneous aspects of evolution were identified. Immunohistochemistry was performed to verify the makers of cell clusters and the expression level of novel genes. Results: A single-cell transcriptomic map of OSCC was created, including 16 clusters of PCA cells, 17 clusters of CA cells, 14 clusters of left LNM cells, and 14 clusters of right LNM cells. We also discovered two novel types of cells including CD1C-CD141-dendritic cells and CD1C+_B dendritic cells. Most of the non-cancer cells are immune cells, with two distinct clusters of T lymphocytes, B lymphocytes, CD1C-CD141-dendritic cells+ and CD1C+_B dendritic cells. We also classified cells into 15 clusters for SPOSCC after radiotherapy of NPC. Determining the upregulated expression levels of IL1RN and C15orf48 as novel markers using immunohistochemistry facilitated the correct classification of OSCC including SPOSCC after radiotherapy of NPC and the prediction of their prognosis. Conclusions: The findings provided an unprecedented and valuable view of the functional states and heterogeneity of cell populations in LNM of OSCC and SPOSCC after radiotherapy of NPC at single-cell genomic resolution. Moreover, this transcriptomic map discovered new cell types in mouth, and novel tumour cell-specific markers/oncogene.
Immunohistochemical expression levels of cyclin D1 and CREPT reflect the course and prognosis in oral precancerous lesions and squamous cell carcinoma

Abstract:Cyclin D1 is the most essential progressive regulator of the cell cycle, and its transcription is enhanced by CREPT (cell cycle-related and expression-elevated protein in tumour). These molecules regulate cell growth, and their aberrant expression can cause malignant transformation. In this study, the expression of these molecules was explored to investigate the molecular alterations in oral precancerous lesions and squamous cell carcinoma. Cyclin D1 and CREPT expression was examined immunohistochemically in tissue specimens from 55 patients with oral epithelial precursor lesions (OEPLs) and 84 patients with oral squamous cell carcinoma (OSCC). Associations between the results and clinicopathological variables were examined. Cyclin D1 and CREPT expression levels were higher in OSCC than in OEPLs. Furthermore, there were statistically significant differences in cyclin D1 expression among the different grades of OEPLs and OSCC lesions. In OSCC, there were statistically significant differences in CREPT expression according to sex, T stage, and degree of differentiation. In addition, the expression of both molecules was significantly correlated with postoperative metastasis and modes of invasion. The expression of cyclin D1 and CREPT was found to depend upon the state of development and progression of the oral epithelial lesions, and clinicopathological behaviours might be affected by these molecules in OSCC.
Proteogenomic Characterization of Primary Oral Squamous Cell Carcinomas Unveils the Extracellular Matrix Remodeling and Immunosuppressive Microenvironment Linked with Lymph Node Metastasis

Abstract:Oral squamous cell carcinoma (OSCC) is an increasingly prevalent malignancy worldwide. This study aims to understand molecular alterations associated with lymph node metastasis of OSCC in order to improve treatment strategies. We analyzed a cohort of 46 patients with primary OSCC, including 10 with lymph node metastasis and 36 without. Using a comprehensive multi-omics approach—encompassing genomic, transcriptomic, proteomic, epigenetic, single-cell, and spatial analyses—we integrated data to delineate the molecular landscape of OSCC in the context of lymph node metastasis. Our genomic analysis identified significant mutations in key genes within the MAPK, TGF-β, and WNT signaling pathways, which are essential for tumor development. The proteogenomic analysis highlighted pathways critical for lymph node dissemination and factors contributing to an immunosuppressive tumor microenvironment. Elevated levels of POSTN were found to reorganize the extracellular matrix (ECM), interact with TGF-β, disrupt cell cycle regulation, and suppress the immune response by reducing VCAM1 activity. Integrated analyses of single-cell and spatial transcriptome data revealed that cancer-associated fibroblasts (CAFs) secrete TGF-β1/2, promoting cancer cell metastasis through epithelial-mesenchymal transition (EMT). Our integrated multi-omics analysis provides a detailed understanding of molecular mechanisms driving lymph node metastasis of OSCC. These insights could lead to more precise diagnostics and targeted treatments.

Immunohistochemical expression of Bcl-2 in oral epithelial dysplasia and oral squamous cell carcinoma

Abstract:Background: The B cell lymphoma-2 gene is a proto-oncogene whose protein product inhibits apoptosis. Its role is associated with keeping cells alive, but not by stimulating them to proliferation, as other proto-oncogenes do. Increased expression of protein product of Bcl-2 gene appears in the early phase of carcinogenesis leading to apoptosis impairment and in consequence to the progression of neoplastic changes. Objective: To evaluate and compare the expression of Bcl-2 protein in oral epithelial dysplasia and oral squamous cell carcinoma (OSCC). Materials and methods: Sixty cases of formalin-fixed paraffin-embedded archival specimens comprising of 30 cases of leukoplakia with oral epithelial dysplasia and 30 cases of OSCC were taken for immunohistochemical analysis using monoclonal antibody against anti-human Bcl-2 oncoprotein. Results: Immunostaining for Bcl-2 protein was identified in basal and parabasal layers as granular cytoplasmic staining in oral epithelial dysplasia. In OSCC, Bcl-2 immunoreactivity was most prominent in the peripheral cells of the infiltrating tumor islands which diminished toward the center in well-differentiated and moderately differentiated OSCC, whereas stronger and more diffuse expression of Bcl-2 oncoprotein was seen in poorly differentiated OSCC. Overall positivity of 26.7% (8/30) was observed in oral epithelial dysplasia and 30% (9/30) in OSCC in this study. Interpretation and conclusion: Altered expression of Bcl-2 oncoprotein may be an early molecular event which leads to prolonged cell survival, increased chances of accumulation of genetic alterations, and subsequent increase in malignant transformation potential.
Analysis of CyclinD1,p27 and PTEN Proteins Expression in Oral Squamous Cell Carcinoma

Abstract:Objective To investigate the expression of CyclinD1,p27 and PTEN protein in oral squamous cell carcinoma(OSCC) tissues and its correlation.Methods 50 patients with oral squamous cell carcinoma was the observation group;50 patients with normal oral mucosa was the control group.Protein expressions of CyclinD1,p27 and PTEN were detected by immunohistochemical SP method.Results Positive rates of CyclinD1,p27 and PTEN protein expression were 80.00%,82.00%,60.00% in the oral squamous cell carcinoma,positive rates of CyclinD1,p27 and PTEN protein expression in the normal oral mucosa were 0,10.00% and 100.00%.Positive rates of CyclinD1,p27 protein expression of the observation group were higher than those of the control group,while PTEN protein expression rate of the observation group were higher than those of the control group;CyclinD1,p27 and PTEN protein expression in oral squamous cell carcinoma tissue was related to the degree of differentiation,the poorer the differentiation,the lower the levels of p27 and PTEN protein expression,and the higher the level of CyclinD1 expression,P < 0.05.Conclusion CyclinD1,p27 and PTEN protein expression are closely related to OSCC differentiation.The poorer the differentiation,the lower the levels of p27 and PTEN protein expression,the higher the level of CyclinD1 protein expression.

Abstract:We use the Galerkin averaging method to construct a coordinate trans- formation putting a nonlinear PDE in Poincar´ e normal form up to finite order. We also give a rigorous estimate of the remainder showing that it is small as a dif- ferential operator of very high order. The abstract theorem is then applied to a quasilinear wave equation, to the water wave problem and to a nonlinear heat equation. The normal form is then used to construct approximate solutions whose difference from true solutions is estimated. In the case of hyperbolic equations we obtain an estimate of the error valid over time scales of order � −1 (� being the norm of the initial datum), as in averaging theorems. For parabolic equations we obtain an estimate of the error valid over infinite time.

Immunohistochemical quantification of partial-EMT in oral cavity squamous cell carcinoma primary tumors is associated with nodal metastasis

Abstract:Objectives: Quantify by immunohistochemistry (IHC) a partial epithelial-to-mesenchymal transition (p-EMT) population in oral cavity squamous cell carcinoma (OCSCC) and determine its predictive value for lymph node metastasis. Methods: Tissue microarrays (TMA) were created using 2 mm cores from 99 OCSCC patients (47 with low volume T2 disease, 52 with high volume T4 disease, and ∼50% in each group with nodal metastasis). IHC staining was performed for three validated p-EMT markers (PDPN, LAMB3, LAMC2) and one marker of well-differentiated epithelial cells (SPRR1B). Staining was quantified in a blinded manner by two reviewers. Tumors were classified as malignant basal subtype based on staining for the four markers. In this subset, the p-EMT score was computed as the average of p-EMT markers. Results: 84 tumors were classified as malignant basal. There was 87% inter-rater consistency in marker quantification. There were associations of p-EMT scores with higher grade (2.15 vs. 1.92, p = 0.04), PNI (2.13 vs. 1.83, p = 0.003), and node positivity (2.09 vs. 1.87, p = 0.02), including occult node positivity (56% vs. 19%, p = 0.005). P-EMT was independently associated with nodal metastasis in a multivariate analysis (OR 3.12, p = 0.039). Overall and disease free survival showed trends towards being diminished in the p-EMT high group. Conclusions: IHC quantification of p-EMT in OCSCC primary tumors is reliably associated with nodal metastasis, PNI, and high grade. With prospective validation, p-EMT biomarkers may aid in decision-making over whether to perform a neck dissection in the N0 neck and/or for adjuvant therapy planning.

Abstract:Objective．To study the relation of the oral squamous cell carcinoma chemotherapy and the index of the DNA content and the cell cycle.Methods．The 46 samples of 23 patients with OSCC were detected for the DNA content and index of cell cycleby folw cytometry ,and the relation with the result of the chemotherapy analyzed.Results．①DI and the rate of aneuploid of the untreated patients was 1.94 and 82.6% respectively,which had no relation with pathological classific ation and tumor size.②The DI and aneuploid rate after chemotherapy were lower than that of prechemotherapy significantly.③There was no relation between the DI and aneuploid rate with the result of chemotherapy.Conclusion．Chemotherapy can decrease the DNA content and aneuploid rate.the DI and an euploid rate cannot predict the result of chemotherapy,however,the FCM can provide some guide for chemotherapy throuht the cell cycle.
