Abstract:According to the current urban workers' basic medical insurance system,the workers who participate in medical insurance and their units do not need to continue to pay the insurance premium after their retirement.Affected by aging of the population,early retirement and other factors,this security system must present the aging tendency.That is to say,in this system,the crowd who pay the premium will reduce relatively and the crowd who enjoy the treatment will expand relatively.Applying the combing method of life insurance actuarial and non-life insurance actuarial and establishing the model,this article reveals this trend.Based on the real data of some cities,the article discusses the effect of aging on the long-term balance of medical funds,studies the balanced premium rate and provides scientific basis for making policy.On this basis,the article puts forward some policy proposal on reducing the systematic aging degree of urban workers' basic medical insurance and achieving a sustained and healthy run.

Abstract:The government has set out its plans1 to widen the options available in the NHS pension scheme to encourage the most highly paid clinicians not to reduce their hours for tax reasons. A consultation2 launched on 11 September will allow staff to cut their pension contributions in any given year to avoid breaching Treasury limits and to eliminate “spikes” in contributions caused by promotions or increased earnings, spreading them instead over several years to avoid extra charges. The Department of Health and Social Care admits that the proposed changes have been forced on it to protect services. Abstract:Objectives: To explore and portray the perspectives of National Health Service Ambulance personnel related to the latest rise in the National Health Service occupational pension age. Methods: Data gathering took the form of 35 in-depth interviews. A thematic analysis was used to characterise and articulate key concepts and meanings. The analysis applied interpretive techniques, as views expressed were from personal experiences, and allowed for an in-depth analysis of shared meanings. Results: The themes reported captured the desire of many Ambulance personnel to exit their employment well in advance of their retirement age, despite satisfaction gained from patient care. This early exit is being driven by increased worry that the work demands of the job are unsustainable, especially for older workers, as clinical responsibilities increase and their social support diminishes. Also, Ambulance personnel feel betrayed by their employers, because their retirement is being delayed further by another change in their pensionable age. Conclusion: There is an increased orientation for ‘living for today’ and indications of a willingness to sacrifice salary and pension income in order to protect their health in older age, which has implications for long-term financial and general well-being in retirement.
Is being in paid work beyond state pension age beneficial for health? Evidence from England using a life-course approach

Abstract:Background: Given the current policy emphasis in many Western societies on extending working lives, we investigated the health effects of being in paid work beyond state pension age (SPA). Until now, work has largely focused on the health of those who exit the labour force early. Methods: Our data come from waves 2-4 of the English Longitudinal Study of Ageing, including the life history interview at wave 3. Using logistic and linear regression models, we assessed the longitudinal associations between being in paid work beyond SPA and 3 measures of health (depression, a latent measure of somatic health and sleep disturbance) among men aged 65-74 and women aged 60-69. Our analyses controlled for baseline health and socioeconomic characteristics, as well as for work histories and health in adulthood and childhood. Results: Approximately a quarter of women and 15% of men were in paid work beyond SPA. Descriptive bivariate analyses suggested that men and women in paid work were more likely to report better health at follow-up. However, once baseline socioeconomic characteristics as well as adulthood and baseline health and labour market histories were accounted for, the health benefits of working beyond SPA were no longer significant. Conclusions: Potential health benefits of working beyond SPA need to be considered in the light of the fact that those who report good health and are more socioeconomically advantaged are more likely to be working beyond SPA to begin with.
Facilitators and barriers for working beyond statutory pension age: A prospective cohort study across 26 European countries

Abstract:Objective The aging population of European countries highlights the need for extended working lives. This study aims to investigate facilitators and barriers for working beyond the statutory pension age (SPA). Methods Using data from waves 1, 2, 4–9 of the Survey of Health, Ageing and Retirement in Europe (SHARE) (2004–2022), we followed 9131 workers with a mean age of 56.9 [standard deviation (SD) 3.5] years from 26 European countries until they surpassed the SPA for their respective country, sex and year of participation. Using robust Poisson regression, we modelled the prospective association of work factors, lifestyle, health, and demographics at baseline with working at least one year beyond the SPA. Results Participants were followed for 9.5 (SD 3.9) years. After surpassing the SPA by at least one year, 18% were still working. Among the work factors, opportunities for skill development [risk ratio (RR) 1.20, 95% confidence interval (CI) 1.07–1.34] and recognition at work (RR 1.13, 95% CI 1.01–1.26) facilitated working beyond SPA, while time pressure (RR 0.89, 95% CI 0.81–0.97) and poor prospects for job advancement (RR 0.76, 95% CI 0.70–0.83) were barriers. For the other factors, smoking was negatively associated with working beyond the SPA, while living in the northern part of Europe, higher level of education, and being divorced or separated were positively associated with working beyond the SPA. Conclusion This prospective cohort study across 26 European countries identified four modifiable work factors that influenced working beyond the SPA. Nurses aged over 50 years and their experiences of shift work

Abstract:Aim: The Late Career Nurse project examined views and characteristics of nurses working in New Zealand who were born before 1960. This paper focuses on the experiences of such nurses who undertake shift work. Background: The mean age of registered nurses in New Zealand has been rising steadily, and 40% are now aged 50 years or over. While there is substantial literature on the phenomenon and consequences of the ageing nursing workforce, little is known of the particular experiences of nurses aged over 50 years who work shifts. Method: An anonymous online survey was emailed to eligible nurse New Zealand Nurses Organisation members aged over 50 years in February 2012. Quantitative and qualitative analyses of the 3273 responses received were undertaken. Result: Over 45% of respondents worked shifts or flexible hours. While shift work suited many, others noted deleterious effects on family and social relationships, physical and mental health (notably sleep patterns and fatigue), and decreasing tolerance for shift work as they age. Poor scheduling practices were particularly detrimental. Conclusion and implications for nursing management: Worldwide, workforce ageing means strategies are required to retain older nurses in the workforce. Improved scheduling practices including increasing access to flexible and part time work hours, and development of resources on coping with shift work are recommended.
Factors associated with the self-perceived ability of nursing staff to remain working until retirement: a questionnaire survey

Abstract:BackgroundIt is important to learn how employers in European countries can prevent nursing staff from changing occupation or taking early retirement in order to counteract expected nursing shortages. However, to date research on nursing staff’s ability to remain working until retirement age has been limited. The purpose of this study was to gain insight into the associations between different job and organisational characteristics, job satisfaction, occupational commitment and the self-perceived ability to continue working in the current line of work until the official retirement age.MethodsThe questionnaire-based, cross-sectional study included 730 nursing staff members employed in Dutch hospitals, nursing homes, organisations for psychiatric care, homes for the elderly, care organisations for disabled people and home care organisations (mean age: 48; 89 % female). Linear and logistic regression analyses and mediation analyses were applied to test hypothesised associations.ResultsReducing work pressure and increasing appreciation by senior management in particular have positive consequences for nursing staff’s self-perceived ability to continue working until the official retirement age. The job and organisational characteristics of autonomy, work pressure, supportive leadership, educational opportunities, communication within the organisation and appreciation of nursing staff by senior management together have substantial impact on nursing staff’s job satisfaction. Job satisfaction in turn is related to the self-perceived ability to continue working until the retirement age. However, job satisfaction mainly summarises the joint effect of job and organisational characteristics and has no supplementary effect on the self-perceived ability to continue working.ConclusionEmployers should primarily focus on work pressure and the appreciation of nursing staff by senior management in order to retain nursing staff even as they get older.

Abstract:Older Americans have been retiring later for a number of reasons, including jobs that are becoming less physically demanding, the shift from defined benefit to defined contribution pensions, and changes in Social Security’s incentives. What are the implications of working longer for workers’ mortality and health? Answering this question is complicated, because work and health are jointly determined – healthy people with lower mortality tend to work longer. Previous studies looking at the causal effect of work on mortality and health have found mixed results and have tended to focus on the effects of early retirement, not delayed retirement. A simple assumption would be that the relationship between them is symmetric. But it is unclear that that assumption is correct – after all, people who decide to keep working are likely a healthier group than those who stop early. This paper uses administrative data from the Netherlands and exploits policy variation designed to delay retirement to explore the links between work and health outcomes.
The perceptions of older nurses regarding continuing to work in a nursing career after retirement: A qualitative study in two Chinese hospitals of different levels

Abstract:Background: There is a shortage of nurses worldwide, and a large number of older nurses will be entering retirement in the near future. Older nurses have rich nursing experience, and their retirement is a major loss of resources for the nursing profession. Returning to work after retirement is becoming increasingly popular among retirees today, but there is limited knowledge of the perceptions of older nurses regarding continuing to work in a nursing career after retirement. Objectives: To explore older nurses' perceptions of continuing to work in a nursing career after retirement. Design and settings: We conducted a descriptive, qualitative study of older nurses from two public hospitals of different levels in China in 2018. Participants: A total of 27 older nurses aged 50-60 years were recruited, 15 from a tertiary hospital and 12 from a secondary hospital. Methods: Individual semi-structured interviews were conducted with older nurses. An interview guide was developed to explore nurses' perceptions of continuing to work in a nursing career after retirement. Thematic analysis was used for the data analysis. We developed an interview guide and analysed the data based on the three essential conditions of the framework for understanding behaviour. Findings: Based on the definition of the three essential conditions of the framework for understanding behaviour, 14 subthemes emerged from the data were categorised under this framework. The three main themes were as follows: the nurses' perceptions of their capability, motivation, and opportunity related to engaging in a nursing career after retirement. A number of older nurses with rich clinical experience have the ability and motivation to engage in nursing a career after retirement. However, discrimination against nurses, burnout, and the effects on young people's employment are barriers to older nurses considering working in a nursing career after retirement. Regarding re-employment after retirement, older nurses also expressed concerns about their physical health, family responsibilities, and salary as well as lack of managerial and policy support. Conclusions: These themes confirmed previous findings and offered new perspectives on older Chinese nurses' perceptions of working in a nursing career after retirement. These findings will influence future policy development and research directions. Policies for facilitating a prolonged working life should be developed. Future research should focus on how to address the challenges confronted by older nurses.

In 2012 the rate of workers aged 55–64 in employment was at 69/55 (male/female) per cent compared with 47/28 per cent in 1998 (the current EU-27 average is 56/42; cf. Noll & Weick, 2013, p. 11). However, participation drops sharply after the age of 60, although this is the age group with the largest rise in employment (Nowossadeck & Vogel, 2013, p. 7).

Abstract:Abstract Background Demographic changes in all industrialized countries have led to a keen interest in extending working lives for older workers. To achieve this goal, it is essential to understand the patterns of retirement and specifically what characterizes individuals who continue to work beyond retirement age. Thus, the aim of this paper was to contribute to the international body of empirical knowledge about individuals who continue in the workforce after retirement age. We present evidence from Denmark and examine what characterizes individuals who continue in the workforce after retirement age and investigate the likelihood of continued work after retirement age while controlling for a set of socio-economic and lifestyle factors. Methods The study population consisted of 5,474 respondents to the Copenhagen Aging and Midlife Biobank (CAMB) 2021 survey, divided into two groups. The first group included subjects ( n = 1,293) who stayed longer in the workforce even though they had the possibility to retire. The second group consisted of subjects who had retired full-time at the time of the survey ( n = 4,181). Survey data was linked to register data to provide a broader dataset. In order to investigate the heterogeneity between the two groups in terms of important socio-economic, work-related and health-related variables, t-test, Mann-Whitney U (Wilcoxon Rank) test, and chi-square tests were employed. Further, to examine the probability of an individual working after retirement age a logit model with step-wise inclusion was utilized. Results Overall, individuals who continue to work even though they could retire tend to be wealthier, healthier, and males compared to individuals who are retired full-time. Further, there are more older workers who have partners and are co-habitants than retirees. The likelihood of continuing in the workforce past retirement age is affected by several work-related factors as well as life-style factors. The likelihood of working past retirement age decreases by years spent in the workforce (marginal effect of -0.003), if you have a partner (-0.080) and if your partner is outside of the workforce (marginal effect of -0.106). The likelihood increases by health (marginal effect of -0.044 of moving from excellent/very good health to good health or to fair/poor health, physical working capability (marginal effect of -0.083 of moving from no/some problems to severe problems or cannot work at all) and income (marginal effect of 0.083 from moving from the lowest income-quantile to higher quantiles). Conclusion These results are in line with the previous literature and suggest the importance of designing retirement policies that tailor the transition toward retirement according to specific characteristics of both the individual and the segment of occupation.

Abstract:Abstract Accelerating population aging is raising concern in many countries now about the availability of workers for essential work roles and responsibilities. A scoping research literature review was done to identify factors currently associated with early retirement and contemporary strategies to encourage and support longer working lives. Among 53 relevant articles, seven early retirement factors were revealed: Ill health, good health, workplace issues, the work itself, ageism, social norms, and having achieved personal financial or pension requirement criteria. Six solutions, none of which had been proven effective, were identified: Occupational health programs, workplace enhancements, work adjustments, addressing ageism, changing social norms, and pension changes. The evidence base on early retirement prevention is not strong, with qualitative research studies needed to gain a more in-depth understanding of early retirement influences and also mixed-methods studies needed to test early retirement prevention solutions for their effects, and particularly in the healthcare sector as healthcare needs typically rise with advanced ageing. Until more evidence is available, every healthcare and other organization should perform an early retirement risk assessment and identify current versus needed policies and programs to encourage and to enable more middle-aged and older people to work longer in life.
Have later state pensions made people less connected? The effects of older female pension ages in England on social capital

Abstract:Abstract Many countries have increased their state pension ages (SPAs) to offset demographic ageing and to increase numbers of older people remaining in paid work. Most research attention has focused on the economic outcomes whereas little attention has been paid to impacts on social capital, including rates of volunteering and care-giving. Prior research suggests that these are sensitive to retirement decisions. Using nine waves of individual longitudinal data from the English Longitudinal Study of Ageing, this study examines the effects of increased SPA on volunteering, group membership and providing care. Our results show that higher SPAs have a negative impact on rates of volunteering and providing care, particularly for women in couples. Single women are not affected, and there are not significant effects on group membership and childcare provision. The more work-like aspects of social capital are affected by the increased SPA.

Abstract:Using a standard macro model with overlapping generations in the manner of Blanchard–Yaari, we show that by broadening the labour supply on the extensive margin through longevity adjustment of the statutory retirement age, labour supply is likely to decrease on the intensive margin. However, this backlash effect depends on the specific design of the pension system. It is significantly higher under a pay-as-you-go scheme with fixed benefits compared to a pay-as-you-go scheme with fixed contributions and a fully funded scheme based on voluntary savings. The findings have implications for responses to the fiscal and macroeconomic challenges raised by an ageing population.

Abstract:Abstract Differences in life expectancy across socioeconomic groups create a serious problem of inequality within the public part of the pension system. This article considers two actuarially sound ways of addressing longevity inequality. The first is to allow low life-expectancy workers to retire earlier and delay the retirement of the high life-expectancy workers so that the two groups receive the same amount, equal to the expected discounted value of future pension benefits received by the average worker under the current system. The second, and more radical, is to delegate to occupational pension funds the task of paying out the public pension benefits to each retiree, based on a lump-sum transfer from the government to the pension funds of an amount for each retiree equivalent to the payout in the first scenario. Abstract:Older people are facing a changed set of expectations regarding work and retirement. Until quite recently early retirement was being encouraged. Today's older workers, however, are contemplating longer working lives amid policy concerns about the costs of social welfare associated with an aging population. While working longer is, almost universally, being promoted as a social and individual good this article argues that this policy shift is unlikely to change the situation of many older workers who would have needed to continue working anyway. It also argues that the emphasis on prolonged employment undervalues engagement in activities beyond employment. As well, it argues that the emphasis on prolonging employment neglects to account for the attendant risks for many of continuing to work or having to seek work and the potential health- and well-being-enhancing effects of retirement.

Elderly Workers in Japan: The Need for a New Deal **

Abstract:IntroductionEvidence suggests that Japan has been successful in social and economic terms in the management of older workers (Sueki, 2013). This national tendency has been translated systematically into the national system for human resource management (HRM) in Japan. Historically, policy-makers in Japan tend to encourage the unemployed or inactive back into jobs or urge those in employment to delay retiring in order to avoid a long disconnection from the labour market. As a result older employees tend to stay in the labour market in larger numbers and for longer time than in most European countries, including Germany (ILO, 2013).Correspondingly, policy at the national level has given the opportunity to large companies to utilize dedicated human resources at reasonable cost and to the public authorities the possibility to slow down the rising deficit of the social security system. However, there was always little attempt to optimize the talent of elderly people after mandatory retirement in teaching them new skills and to allow them to shift gradually to complete retirement in good conditions. Little public money has been devoted to training institutions for all types of workers, including the older workers until the last decade (Johns & Urasawa, 2011). Many of them occupy under-qualified jobs. They are also not given the opportunity to acquire new skills and they are not paid fairly. Other older workers perform qualified jobs and were reasonably rewarded so far because financial incentives complementing public pension could be added to their wage.However, these traditional practices appear increasingly unsustainable under the impact of demographics, the socio-cultural context, the regulatory environment and the subsequent need for changes in the social security system. Japan cannot afford not to continue to train older workers in order that they have marketable skills, even age sixty and above. The raising of the age of eligibility for receiving public pension means that the income of older workers would decline significantly without other kinds of public support, something that is very difficult in view of the public deficit, or companies' acceptance to pay higher wage, something that the large majority of companies declare to be beyond their means and economically unrealistic. Thus, it may be time to examine how the traditional practices of managing elderly employment can evolve, what kinds of new HRM approaches can be tried, what form of new deals might be offered to employees, and how these might be implemented.ContextLow fertility rates and rising life expectancy are causing the ratio of Japanese workers to retirees to shrink. Between 2010 and 2040, the share of Japan' working-age population will fall by more than 10 per cent (World Bank Group, 2015). Japan went from having one of the lowest dependency ratios in 1960 to one of the highest in the world nowadays. It is bound to escalate further and projected to more than double in the following twenty years (ILO, 2013).In Japan as in many countries ageing is still often correlated with less dynamism and entrepreneurial spirit, and lower productivity. However, efforts to prolong or sustain working life are increasingly understood in a broader and deeper perspective. It is increasingly thought that no upper limit should be decided in setting the threshold be- tween working age and retirement age. Many people are not only able to work longer, but prefer and choose to do so. Surveys suggest that Japanese people recognize the need to work longer and the desirability in terms of economic benefits and strengthened social engagement (World Bank Group, 2015). Research also suggests that the productivity of elderly workers may be maintained for some types of tasks: for example, through innovative technologies in shop floors and in offices. Abstract:Abstract Background The proportion of people aged 60 years or over is growing faster than other age groups. Traditionally, retirement has been considered as both a loss to the labour market and an additional economic burden on the nation. More recently, it is widely accepted that retired people can still contribute to society in many ways, though the extent of their contributions will depend heavily on their state of health. In this context, a significant practical issue is how to encourage older people to use the health services they need. This study aims to evaluate the effects of pensions on older adults’ health service utilization, and estimate the level of pension required to influence such utilization. Methods Using data from a nationally representative sample survey, the China Health and Retirement Longitudinal Study, we adopted a fuzzy regression discontinuity design and undertook segmented regression analysis. Results It was found that a pension did encourage low-income people to use both outpatient (OR = 1.219, 95% 1.018–1.460) and inpatient services (OR = 1.269, 95% 1.020–1.579); but also encouraged both low- and high-income people to choose self-treatment, specifically over-the-counter (OR = 1.208, 95% 1.037–1.407; OR = 1.206, 95% 1.024–1.419; respectively) and traditional Chinese medicines (OR = 1.452, 95% 1.094–1.932; OR = 1.456, 95% 1.079–1.955; respectively). However, receiving a pension had no effect on the frequency of outpatient and inpatient service use. Breakpoints for a pension to promote health service utilization were mainly located in the range 55–95 CNY (7.1–12.3 EUR or 8.0–13.8 USD). Conclusions A pension was found to have mixed effects on health service utilization for different income groups. Our study enriches existing evidence on the impact of pensions on healthcare-seeking behaviour and can be helpful in policy design and the formulation of improved models relating to pensions and healthcare utilisation.

Abstract:Delayed retirement initiative proposed in China attaches greater importance to the sustainability of pension systems and the labour shortage, but less to the health status of older people. The existing social health insurance and pension system are not well established to match this initiative. This study investigates the policy mix of delayed retirement, employment-based social health insurance, social pension participation for health status of older people. Results of the data from the China Health and Retirement Longitudinal Study (CHARLS-2018) show that late retirement could benefit health status among older adults. Moreover, such effect of late retirement appears more salient for those uninsured by employment-based social health insurance and those still in the pension contribution phase upon reaching the statutory retirement age. Hence, in countries with inadequate health insurance and pension systems, such as China, delayed retirement may serve as an important alternative to social security for the health of older people.
