Abstract:Objectives: The aim of the study was to evaluate the effect of high-flux (HF) hemodialysis (HD) on delayed protective hepatitis B virus (HBV) antibody seroconversion in HD patients who had no response to the classic third dose of HBV vaccination. Methods: We performed a prospective cohort study. Forty-two patients who did not respond within 6 months after the third dose of the vaccination were enrolled in the study. The patients were randomized to either an HF HD treatment group (n = 19) or a conventional HD treatment group (n = 23). Patients' serum hepatitis B surface antibody levels were followed monthly. Results: After 6 months of follow-up, there were 15 (78.9%) patients in HF group and 7 (30.4%) patients in the control group showing delayed HBV vaccination response. The level of antibody titer of HBV vaccination responders was 103.6 +/- 48.3 mIU/mL in the HF group and 23.5 +/- 15.7 mIU/mL in the control group. The antibody titer did not correlate with sex, age, serum albumin, or hemoglobin in either group. Four patients in control group lost HBV vaccination response > 2 times after 3 months of follow-up, whereas no patients in the HF group lost HBV vaccination response. Conclusion: Hemodialysis patients who do not respond to the classic third dose of HBV vaccination could reobtain a delayed higher protective HBV antibody seroconversion rate by HF HD without other intervention.
Monitoring the Efficacy of Infant Hepatitis B Vaccination and Revaccination in 0- to 8-Year-old Children: Protective Anti-Hbs Levels and Cellular Immune Responses.

Abstract:Vaccination against hepatitis B virus (HBV) is recommended worldwide. The aim of this study was to assess the efficacy of infant hepatitis B vaccination and revaccination in 0- to 8-year-old children in the context of protective anti-HBs levels and cellular immune responses. Using a random questionnaire survey, 1695 pre-school children were recruited as research subjects during January 2015 to June 2017. Blood samples were obtained to measure HBV serological markers as well as peripheral immunocytes. The children were divided into non-, low- and hyper- responsive groups (NR, LR, and HR) based on the vaccination efficacy. Additionally, the effect of revaccination on the NR group was evaluated at 1 month after completion of the vaccination course. Among a total of 1695 children, 1591 (93.86%) were infants who were followed while undergoing their primary course of hepatitis B vaccination at the 0-1-6 month schedule, and 1249 (79.30%) of them developed antibodies against HBsAg (anti-HBs) titers greater than 10 IU/L. The results of immunocyte studies indicated that the CD8+ T cells, CD4+CD45RO+ T cells, CD8+CD45RA+ T cells, and T follicular helper (Tfh) cells increased significantly in NR compared with HR. However, lymphocytes, CD4+ T cells, and CD4+CD45RA+ T cells in NR were lower than that in HR. 96 of the non-response cases showed seroprotection after revaccination among 103 cases. Therefore, most of the preschool children who received hepatitis B vaccine in infancy achieved significant seroprotection. Seroconversion rates of individuals revaccinated after initial vaccination failure were significantly higher than those after primary vaccination. Different vaccination efficacy groups showed significant changes in circulating immunocytes, which might be a factor affecting the recombinant HBV vaccine's immune effectiveness.

Abstract:BackgroundIn hemodialysis patients Hepatitis B virus (HBV) infection is one of the problems. Because of HBV vaccine response is lower than in the general population, in this study it is aimed to determine the factors that may cause inadequate HBV vaccine response in hemodialysis patients.MethodsIn study, HBsAg, anti-HBs, anti-HBc IgG data belonging to 278 patients were obtained from file and computer records. It was seen that seronegative cases had been given recombinant HBV vaccine. Anti-HBs titers were monitored 1 month after vaccination was completed. According to this, the patients are divided into two groups. Those with anti-HBs < 10 IU/mL were identified as non-responders and with anti-HBs ≥ 10 IU/mL as responders. Factors such as age, serum albumin and urea reduction rate which may affect inadequate response to HBV vaccine were evaluated. As statistical examination, Chi-square test was used for the analysis of the data determined by counting, and logistic regression was used for statistically significant independent variables in chi-square test. p value of < 0.05 was considered statistically significant (Confidence interval: 95%).ResultsOut of 278 patients, according to exclusion criteria 81 patients were excluded. 13.2%(26/197) of HBV vaccinated patients had insufficient response. The inadequate response rate to HBV vaccination was found to be higher in patients with age ≥ 65 (p = 0.039), serum albumin < 3.5 g/dL (p = 0.024) and urea reduction rate ≤ 65 (p = 0.028). No statistically significant relationship was found between inadequate response to HBV vaccine and anti-HCV positivity, presence of diabetes mellitus, anemia status, vitamin D therapy and vascular access pathway variability.ConclusionWe conclude that relatively high patient age, low albumin level and insufficient urea reduction rate may cause inadequate HBV vaccine response. Taking these factors into consideration may provide a useful insight for an adequate response to vaccination.

Quality Control Measures for Lowering the Seroconversion Rate of Hemodialysis Patients with Hepatitis B or C Virus.

Abstract:BACKGROUND: Hemodialysis (HD) patients are at high risk of infection by hepatitis B virus (HBV) or hepatitis C virus (HCV). The present study was designed to determine the impact of quality control measures on the prevention of transmission of blood-borne viruses.METHODS: A total of 6182 adult maintenance HD patients from all HD units in Zhejiang Province were recruited on January 1, 2007. The baseline demographic and clinical characteristics were recorded and all patients were followed up until death or survival at 4 years later. The Quality Control Standards of Hemodialysis were gradually implemented in HD units. The HBV or HCV seroconversion rates of the recruited patients were calculated and compared every year during the observation period.RESULTS: The prevalence of HBV was 8.3% at the beginning of the study, and 6.6% for HCV. With the implementation of the HD quality control measures, the HBV seroconversion rate tended to decrease year by year (chi(2)=6.620, P=0.085), and the HCV seroconversion rate decreased significantly (chi(2)=10.41, P=0.015). Compared with the data in 2007, the HBV seroconversion rate (chi(2)=4.204, P=0.040, relative risk ratio 0.393, 95% CI 0.156-0.991) and the HCV seroconversion rate (chi(2)=7.373, P=0.007, relative risk ratio 0.386,95% CI 0.189-0.787) decreased significantly in 2010.CONCLUSION: Quality control measures for HD decreased the seroconversion rates of HBV or HCV in HD patients, showing that updated quality control measures reduce the risk for transmission of blood-borne viruses in the HD population.

Abstract:Over the last few decades, increasing attention has been focused on the use of electrostimulation to aid osseous healing. Since the pioneering work of Yasuda in the 1950s focusing on the concept of piezoelectric fields associated with bone, intense scientific research has analyzed the application of electrostimulation technology. “Piezoelectricity” is the ability of certain crystals to produce a voltage when subjected to mechanical stress. The word is derived from the Greek “piezein,” which means to squeeze or press. In a piezoelectric crystal, the positive and negative electrical charges are separated, but symmetrically distributed, so that the crystal overall is electrically neutral. When a stress is applied, this symmetry is disturbed, and the charge asymmetry generates a voltage. Bone, which is rich in calcium phosphate crystals also produces similar potentials if stressed. The compression side has a negative potential, while the tension side has a positive potential. Another important form of electric potential associated with bone, especially traumatized bone, is transmembrane (bioelectric) potentials, which are generated by cellular metabolism and depend on cell viability. Areas of greatest cellular activity demonstrate the greatest negative potential. The phenomena of piezoelectric and bioelectric potentials are important in modifying a variety of processes in both the early callus and later remodeling phases of fracture healing. Hence, the application of exogenous electrical potentials either as an adjunct to accelerate osseous healing or as an alternative to operative intervention (for nonunion) may facilitate bony union by positively influencing the fracture-healing cascade.

Abstract:BACKGROUND: To evaluate the immunogenicity, antibody persistence, and safety of the 60 µg hepatitis B vaccine in hemodialysis patients in China.METHODS: We conducted a multicenter, randomized, double-blind, parallel-controlled trial including 352 hemodialysis patients who were centrally randomized in a ratio of 1:1 to receive a 20 µg (IM20 group) or 60 µg (IM60 group) recombinant hepatitis B vaccine at months 0, 1, and 6.RESULTS: The vaccine-elicited antibody responses peaked at month 7, and declined at month 12. At month 7, the IM60 group had stronger GMC of anti-HBs, and a higher proportion of seroconversion and high-level response than the IM20 group did (P < 0.05). Better immune responses were observed in the IM60 group, especially for those aged or in the high-frequency hemodialysis population.CONCLUSION: The high dose 60 µg recombinant hepatitis B vaccines elicited stronger immune responses than the 20 µg hepatitis B vaccine did among hemodialysis patients.CLINICAL TRIAL REGISTRATION: ClinicalTrials.gov, number NCT02963714.

Abstract:Hemodialysis (HD) patients represent a high-risk group for hepatitis B infection. It is crucial to administer hepatitis B vaccination and stimulate higher and more sustained levels of anti-HBs. Our aim is to enhance the immunogenicity and persistence by implementing high-dose and prolonged hepatitis B vaccine schedule regimen in HD patients. We conducted this multicenter, randomized, parallel-controlled trial between July 2020 and February 2023 at 11 hospitals in Shanxi province, China. A total of 504 HD patients were enrolled. All participants randomly allocated in a ratio of 1:1:1 to receive recombinant HBV vaccine of 3 standard doses (20 μg) at 0-1-6 months (IM20×3 group), 4 standard doses at 0-1-2-6 months (IM20×4 group), or 4 triple doses (60 μg) at 0-1-2-6 months (IM60×4 group). The vaccine-elicited antibody response peaked at month 7. The follow-up outcomes ranging from month 7 to 30 revealed that the response rates of anti-HBs decreased from 85.9% (134/156) to 33.0% (33/100) in IM20×3 group, from 92.5% (135/146) to 53.9% (56/104) in IM20×4 group and from 95.4% (145/152) to 57.3% (55/96) in IM60×4 group. The duration of vaccine-induced response with 75% of patients maintained protective antibody were 21.0 months in IM20×3 group, 25.7 months in IM20×4 group ( vs. IM20×3 group, P =0.056) and 29.2 months in IM60×4 group ( vs. IM20×3 group, P =0.034). All the adverse reactions were mild. The four-triple-dose hepatitis B vaccination regimens could enhance the immunogenicity and 2-year duration in HD patients. Abstract:Introduction Hepatitis B virus (HBV) vaccination is crucial for seronegative advanced chronic kidney disease (CKD) patients, for protection during dialysis while preparing for transplantation. A standard regimen for HBV vaccination requires 24 weeks to be completed. An accelerated HBV vaccination regimen completed within 8 weeks has shown early effective seroconversion in healthcare workers. However, data in advanced CKD patients are limited. Methods A randomized controlled trial was conducted in advanced CKD (eGFR<30 mL/min/1.73 m 2 ) and dialysis patients. The patients were randomly assigned to either a standard HBV vaccination regimen (Engerix B; 40 μg at 0, 4, 8, and 24 weeks) or an accelerated regimen (40 μg at 0, 1, 2, 8 weeks). The anti-HBs antibody were measured at 12, 28, and 52 weeks. Seroconversion were defined as anti-HBs ≥10 IU/L. Results At 12 weeks, among the intention-to-treat population of 133 participants (65 in the accelerated and 68 in the standard groups), the accelerated group demonstrated significantly higher rates of seroconversion (83.08% vs 63.24%, p=0.01). In the per-protocol analysis of 125 patients (62 in the standard and 63 in the accelerated groups), the accelerated group exhibited higher seroconversion rate compared with the standard group (85.71% vs. 69.35%, p=0.03). At 28 and 52 weeks, the seroconversion rates were similar between the two groups. Conclusions In patients with advanced CKD, the accelerated HBV vaccination regimen demonstrated a significantly higher seroconversion rate at 12 weeks of vaccination. This finding suggests that the accelerated regimen is an effective option to achieve rapid seroconversion before initiating hemodialysis or undergoing kidney transplantation.

Abstract:Background The effects of COVID‐19 vaccines on immunocompromised people such as hemodialysis (HD) patients are an important topic that should be addressed. This study reports an observation of the effect of the third dose of the Sinopharm vaccine (SphV3) on the level of hepatitis B surface antibody (anti‐HBs) in HD patients, and the differences between anti‐HBs titers before and after SphV3 were analytically evaluated. Methods This single‐center observational study involved all HD patients presented to Shariati Hospital, Tehran, Iran, from February 2021 to March 2022. All patients received three doses of the Sinopharm vaccine over 8 months. The anti‐HBs level is measured every 6 months as the routine evaluation against HBV infection for all HD patients. Three months before (anti‐HBs‐B3) and 3 months after (anti‐HBs‐A3) SphV3 were the routine times to measure the anti‐HBs titer during this study. Results Twenty‐five HD patients were enrolled. Overall, the anti‐HBs‐A3 was significantly higher than anti‐HBs‐B3 (p = 0.001). The anti‐HBs levels before and after SphV3 were not statistically remarkable in patients with diabetes and ischemic heart disease. The patients with a history of kidney transplant and those with a history of COVID‐19 had significant differences between anti‐HBs‐B3 and anti‐HBs‐A3 (p = 0.002, p = 0.003, respectively). Conclusion Our findings revealed that inactivated COVID‐19 vaccine may be involved in the humoral immune response to hepatitis B in HD patients. It may be novel and have significant implications for the vaccination protocol for immunocompromised patients, including those undergoing HD and transplant recipients.

The duration of hepatitis B vaccine immunity in pediatric dialysis patients

Abstract:BackgroundDialysis patients are at risk for hepatitis B infection, a serious but preventable disease. Long-term hepatitis B protection has not been defined in pediatric patients with chronic kidney disease stage 5 on dialysis (CKD 5D) who were vaccinated as infants or children.MethodsAnnual hepatitis B antibody surveillance data were collected retrospectively on a cohort of pediatric CKD 5D patients (n = 202) at a single institution and analyzed by survival analysis to assess hepatitis B immunity duration.ResultsMedian duration of immunity by Kaplan–Meier analysis since primary vaccination was 106.3 [95 % confidence interval (CI) 93.9, 124.4] months. After the initiation of dialysis, the median duration of hepatitis B immunity was 37.1 (95 % CI 24.2, 72.3) months. Multivariate adjusted analysis showed that there was a significant difference in the duration of hepatitis immunity based on the timing of hepatitis B vaccination (p < 0.001). Patients immunized after starting dialysis had a hazard ratio of 6.13 (95 % CI 2.87, 13.08) for hepatitis B immunity loss compared to patients immunized as infants (p < 0.001).ConclusionsAfter dialysis initiation, protective hepatitis B antibody levels wane rapidly, with a shortened duration of immunity. In our cohort of pediatric patients with CKD 5D, this decline was more pronounced in children who were immunized after starting dialysis than in those who received hepatitis B immunizations during childhood. Both groups of patients should be monitored with serial antibody titers.

Abstract:Introduction: Among hemodialysis patients, hepatitis B virus (HBV) and hepatitis C virus (HCV) infections contribute significantly to mortality and morbidity. Infection with these hepatotropic viruses in hemodialysis patients is due to their increased contact with blood and its derivatives. Additionally, not following the proper protocol for infection control, contaminated devices, and untrained personnel contribute to the nosocomial transmission of these infections. This cross-sectional study was planned to estimate the seroprevalence of hepatitis B surface antigen (HBsAg) and anti-hepatitis C virus antibodies in patients to know the seroconversion rate for hepatitis B and hepatitis C and to evaluate the risk factors that contribute to seroconversion in patients undergoing hemodialysis at our center. Materials and methods: This study included 185 patients with chronic kidney disease undergoing hemodialysis in our center. After giving informed consent, a blood sample from each patient was collected for testing for HBsAg and anti-HCV antibodies initially and then every month. Results: Of the total 185 patients, five participants tested positive for HBV (2.7%), and 29 individuals tested positive for HCV (15.67%). During the study period, seroconversion for hepatitis C was observed in three patients (1.62%), and seroconversion for hepatitis B was observed in one patient (0.54%). An evaluation of the potential risk factors revealed that dialysis conducted outside the facility contributed significantly to HCV infection. Conclusion: Our study shows lower HBV rates but higher HCV rates. The demographic data of the patients and the duration of dialysis are related to the risk of infection. Dialysis within the same healthcare facilities reduces the transmission risk.
Overt and occult hepatitis B virus infection detected among chronic kidney disease patients on haemodialysis at a Tertiary Hospital in Ghana

Abstract:Hepatitis B virus (HBV) infection is endemic in Ghana and chronic kidney disease patients on haemodialysis are a high-risk group for HBV infection. We determined the prevalence of overt and occult HBV infection among haemodialysis patients at the Korle Bu Teaching Hospital in Ghana. 104 consenting End Stage Renal Disease patients on long-term haemodialysis were recruited for the study and their socio-demographic, clinical and laboratory information were obtained using structured questionnaire. All the participants were tested for the hepatitis B surface antigen (HBsAg). The HBsAg-negative participants were re-tested for hepatitis B surface antibody (HBsAb), hepatitis B core antibody (HBcAb) and HBV DNA using chemiluminescence and Roche COBAS Ampli-Prep/TaqMan analyser and real-time polymerase chain reaction. Eight (7.7%) of the total participants were positive for HBsAg. Among the 96 HBsAg-negative participants, 12.5% (12) were HBcAb-positive, 7.3% (7) had detectable HBV DNA (mean = 98.7±53.5 IU/mL) and 40.6% (39) were positive for HBsAb. Five out of the 7 HBV DNA-positive participants were males and only one participant was negative for HBcAb. Seventy-three out of the 96 HBsAg-negative participants were vaccinated and 37 of these vaccinated individuals had significant HBsAb titres (mean = 423.21± 380.72 IU/mL). Our data demonstrated that the prevalence of overt and occult HBV infection among the haemodialysis (HD) patients was 7.7% and 7.3%, respectively, and only 50.7% of those who showed proof of vaccination were protected from HBV infection.

Abstract:Background and objectives: Hepatitis B virus (HBV) infection is common in people with chronic kidney diseases (CKD). The guidelines recommend four doses, 2.0 mL each, of HBV vaccine, given at zero, one, two and six months in these patients. However, real-life data on the effectiveness of this schedule are limited. We retrospectively reviewed the HBV vaccine response in the CKD population. Methods: The study included adult (≥ 18 years) patients with glomerular filtration rate < 60 mL/min, if they had received four doses (each of 2.0 mL volume) of HBV vaccine and anti-HBs titer was measured at ≥ 1 month of the last dose of vaccine. Participants with hepatitis C or human immunodeficiency virus (HIV) coinfection, organ transplant recipients, active or remote malignancy or use of immunosuppressive medication were excluded. Anti-HBs antibody was measured with two different assays with their limits of detection up to 500 mIU/mL and 1000 mIU/mL. The presence of detectable anti-HBs antibody and anti-HBs titer ≥ 10 mIU/mL defined seroconversion and seroprotection, respectively. Results: The study included 208 patients (71.9% males; age 44 [33-55] years; CKD stage II/III/IV/V in 1.4%/7.2%/26.4%/64.9%; 46% on maintenance hemodialysis [MHD]). Overall, seroconversion and seroprotection were achieved in 174 (83.7%) and 161 (77.4%) participants and anti-HBs titer, measured three (2-8) months after the fourth dose, was 124 (12-500) mIU/mL. The median anti-HBs antibody levels at ≤ 6, 7-12, 13-24 and 24 months after the fourth doses were 116, 478, 43 and 70 mIU/mL, respectively. Age, body mass index, stage of CKD, serum albumin and dialysis status were not associated with seroprotection (p < 0.05). Conclusion: A standard vaccination schedule of four 2.0 mL doses of HBV vaccine in CKD patients induces reasonably good and sustained seroprotection.

Abstract:Abstract Background: The hepatitis B virus (HBV) is responsible for all forms of hepatitis (occult HBV infection [OBI]) endangering the health of the public. The growth, division, and activity of immune cells are governed by chemical mediators called cytokines. Evidence suggests that inadequate immune responses contribute to the persistence of HBV. Objectives: The goal of this study was to determine the prevalence of hepatitis B e antigen (HBeAg) among HBV surface antigen-positive (HBsAg+) persons by analyzing the association between age and gender and the severity of HBV infection. Materials and Methods: Seventy-two individuals from August 2022 to November 2022 were hired: a total of 35 healthy participants and 37 persons with acute or chronic HBV infection. Healthy controls and research participants ranged from 20 to 80 years old, and all of them were analyzed using serum samples (3 mL). The levels of HBV, tumor necrosis factor-alpha (TNF-α), and HBeAg in the blood were determined using enzyme-linked immunosorbent assay (ELISA). Results: 37 affirmatives out of 72 Using a double-antibody sandwich ELISA, we determined that our HBV participants met the inclusion criteria. The findings of the HBsAg ELISA Kit indicated that the prevalence of HBsAg was greatest in those 35–49 years old (32.5%) and lowest in those 20–34 years old (21.6%) and 50–64 years old (21.6%). The HBsAg ELISA Kit result showed that the 37 patients who tested positive for HBsAg, 22 were female (59.5%), and 15 were male (40.5%). This suggests that the prevalence of HBsAg infection is higher in females than in males. Dialysis patients have been shown to have increased levels of HBeAg and TNF-α. Conclusions: Patients in Babylon province with chronic HBV had significantly higher than average levels of HBeAg and TNF.
Discrepancy between serological and virological analysis of viral hepatitis in hemodialysis patients

Abstract:Background and aim: Viral hepatitis is a health threat for hemodialysis (HD) patients and it may be transmitted during treatment. Some patients categorized to have viral hepatitis were found to be non-viremic. To clarify the discrepancy between the serological tests in HD patients, we conducted the study. Methods: A total of 1681 HD patients was included. Blood samples were analyzed for hepatitis B surface antigen (HBsAg) and anti-hepatitis C antibody (anti-HCV). Detection of hepatitis B virus (HBV) DNA and hepatitis C virus (HCV) RNA were performed in either HBsAg (+) or anti-HCV (+) samples. HBV DNA/HCV RNA was also measured in a subset of HBsAg (-) and anti-HCV (-) patients. Liver function tests were analyzed and compared with the serological and virological tests. Results: The serological tests showed that 230 patients (13.7%) were HBsAg (+) and 290 (17.3%) were anti-HCV (+). We were unable to detect HBV DNA in 97 of 230 (42.2%) HBsAg (+) patients, and HCV RNA could not be found in 76 of 290 (26.2%) anti-HCV (+) patients. In 167 HBsAg (-) patients, only one showed a trace amount of HBV DNA. None of 151 anti-HCV (-) patients showed detectable HCV RNA. The prevalence rate of viral hepatitis remains high in Taiwanese HD patients: 13.7% for HBV and 17.3% for HCV. However, virological analysis showed 42.2% non-viremic rate for HBsAg and 26.2% non-viremic rate for anti-HCV. Conclusions: The findings might challenge the presently suggested principles of bed and machine dedication and the diagnosis of viral hepatitis in HD patients.
The Effectiveness and Sero-Immunity of Hepatitis B Vaccination in People Who Use Drugs: A Systematic Review and Meta-Analysis

Abstract:Hepatitis B virus (HBV) vaccination has been available for over four decades. However, a synthesis of the evidence regarding the effectiveness of this strategy on preventing hepatitis B infection in people who use drugs (PWUD) is lacking. A systematic search of the MEDLINE (via PubMed), SCOPUS, Web of Science, and Cochrane Library databases was conducted up to June 2024. Eight studies reported on the effectiveness of HBV vaccination, defined as a positive result for HBsAg or anti-Hbc in vaccinated versus non-vaccinated PWUD, with a pooled effect size of 52% (95% CI: 28.2–67.9) for HBsAg and 31.89% (95% CI: 14.8–45.5) for anti-Hbc. For sero-immunity, defined as the proportion of vaccinated PWUD with levels of anti-HBs ≥ 10 mIU/mL, we found that 66.2% (95% CI: 0.61–0.71; I2 = 94%) had protective levels of anti-HBs. The results of this meta-analysis indicate that the incidence of HBV infection is lower in vaccinated PWUD than in those who did not receive the vaccine. However, the effectiveness is lower than that observed in the general population. This highlights the need for a thorough review of the factors influencing the prevention of HBV infection in PWUD.

Abstract:Patients with chronic kidney disease (CKD) on dialysis have a higher mortality rate associated with SARS-CoV-2 infection. Although vaccines are now available, the protective response rates and determinants of humoral response to the vaccine are poorly described in patients on peritoneal dialysis. This was a prospective observational study describing the response rates of detectable and standardized protective antibody titers one month after each mRNA vaccine dose in a cohort of 88 patients on peritoneal dialysis. We found that the vast majority of patients produced protective levels of antibodies (73%) one month after the second vaccine dose. In the multivariate analysis, the single determinant for an adequate humoral response was the weekly Kt/V, a surrogate of dialysis dose. The response rate was higher, but not significantly, with the mRNA-1273 than with the BNT162b2 vaccine one month after the second dose (78.7 vs. 46.2%, respectively, p = 0.02). We found that patients on peritoneal dialysis had a satisfactory humoral response rate, which was much higher than in transplant recipients. PD patients with a poor humoral response, particularly those with a low wKT/V, may benefit from an additional dose of vaccine.

Abstract:Background & Aims Although HBV vaccine has a 95% seroconversion rate in the general population, patients with chronic liver disease have reduced seroconversion rates (16-79%). The aim of the study was to describe seroconversion rates with HBV vaccines in patients with chronic liver disease. Approach & Results Retrospective chart review was performed among 652 patients who received a complete HBV vaccine series in the hepatology clinic. Of those, 126 patients that were included, 111 received a single dose series, and 15 patients received a double dose series. The seroconversion rate was overall low at 35%, and stayed the same at 35% with double dose and at 33% with single dose. Patients who received a single dose series were further analyzed to review risk factors for seroconversion. Overall, 65% of patients had cirrhosis. Patients were more likely to seroconvert if no cirrhosis (51% vs 72%, P=.04), higher aminotransferase levels, intermediate anti-HBs (2.5-11.9 mIU/mL) at baseline (87.5% vs 14%) Conclusion Patients with chronic liver disease had a low rate (35%) of response to HBV vaccination. The response rates did not improve in patients that received double dose series. Patients with cirrhosis, lower aminotransferase levels and with a lower baseline anti-HBs had decreased response rates.
Humoral Response after SARS-CoV-2 mRNA Vaccination in a Cohort of Hemodialysis Patients and Kidney Transplant Recipients

Abstract:Background Kidney transplant recipients and patients receiving hemodialysis are immunocompromised populations that are prioritized for COVID-19 vaccination but were excluded from clinical trials of SARS-CoV-2 mRNA vaccines. Antibody titers and rates of seroconversion after vaccination are lower among patients with CKD and those taking immunosuppressants compared with controls. Data are lacking regarding their humoral response to vaccination to prevent COVID-19. Methods This investigation of early serological response after COVID-19 vaccination with the Pfizer/BioNTech (BNT162b2) mRNA vaccine included 78 patients undergoing hemodialysis, 74 kidney transplant recipients, and seven healthy controls. We recorded data from the medical file for various clinical parameters, including response to hepatitis B vaccination, and measured antibody titers against SARS-CoV-2 at 0, 14, 28, 36, and 58 days after the first injection. Results In controls, we detected antibodies at a positive level (>13 arbitrary units per ml; AU/ml) at day 14 postinjection, which increased progressively to peak at day 36 (1082 AU/ml; interquartile range [IQR], 735.0–1662.0). Patients undergoing hemodialysis had lower titers that peaked at day 58 (276 AU/ml; IQR, 83.4–526.0). We detected a positive antibody level in only three transplant recipients at day 36. In patients on hemodialysis, those aged 75 years, and serum albumin and Kt/V were positively correlated with serological response ( P <0.04 and P <0.0, respectively); nonresponders to HBV vaccine had the lowest anti–SARS-CoV-2 antibody titers. Conclusions Our results suggest that the postvaccination humoral response is strongly inhibited by immunosuppressant therapy in kidney transplant recipients, and is reduced by the uremic condition in patients undergoing hemodialysis. Significance Statement Data are lacking on the humoral response to COVID-19 vaccination in kidney transplant recipients and patients on hemodialysis.
