Abstract:Aim Evaluate the relationship between anti-Xa activity and anticoagulant effect, and ascertain whether accumulation of low-molecular-weight heparins (LMWH) occurs during haemodialysis. Methods There was an observational, single-centre study among participants who received the LMWH dalteparin, enoxaparin or nadroparin. A standard haemodialysis session lasted 4 hours. All included participants had anti-Xa activity measures at 0.5 and 4 hours. Extracorporeal circuit (ECC) clotting was evaluated by visual inspection of the haemodialyser and bubble trap after each haemodialysis session. The same person was tested at three consecutive haemodialysis sessions. Results Overall, 90 participants were enrolled and 259 haemodialysis sessions assessed. There was no significant difference in the mean anti-Xa activity at 0.5 and 4 hours for three consecutive sessions, so LMWH accumulation did not occur. There were 69 (26.6%) sessions in which, ECC clotting was visible. Compared with the group where circuit clotting did not occur, the LMWH dose and anti-Xa activity in the group where circuit clotting occurred were significantly lower. At 0.5 hour, anti-Xa <0.88 IU/mL had significantly higher odds of ECC clotting than that at >= 0.88 IU/mL. At 4 hours, anti-Xa <0.35 IU/mL had significantly higher odds of ECC clotting than that at >= 0.35 IU/mL. Conclusion We found that over three haemodialysis sessions, no significant accumulation of LMWH was evident in subjects receiving a LMWH dose of between 2000 and 5000 IU for regular. Anti-Xa activity measurement can be used to adjust the dosage of LMWH and predict the anticoagulant effect during haemodialysis.

Abstract:Objective To investigate the clinical effect of a new type of disposable heparin freedialysis tube in uremic patients without heparin dialysis. Methods A total of 110 patients withoutheparin dialysis were divided into control group and observation group by random digits table method with55 cases each. Based on the same pre dialysis and dialysis for heparin saline flushing of discontinuoussaline on the control group using the traditional dialysis pipe for hemodialysis without heparin, theobservation group used disposable non heparin dialysis pipe for hemodialysis without heparin. The numberof patients with venous catheter blockage, blood loss and completion of dialysis time in two groups wasrecorded. Results The number of patients with venous catheter blockage, blood loss and completion ofdialysis time was 6, 6, 47 cases in control group and 0, 0, 54 cases in observation group. The differences were statistically significant (χ2=4.407, 4.407, 4.356, P<0.05). Conclusions The new disposableheparin free dialysis tube can reduce the loss of blood and prolong the treatment time, and the operation issimple, safe and effective, and has good application value and application prospect.
Efficacy and safety of low molecular weight heparin compared to unfractionated heparin for chronic outpatient hemodialysis in end stage renal disease: systematic review and meta-analysis

Abstract:Background. Low molecular weight heparin (LMWH) is an effective anti-coagulant for thrombotic events. However, due to its predominant renal clearance, there are concerns that it might be associated with increased bleeding in patients with renal disease. Objectives. We systematically evaluated the efficacy and safety of LMWH compared to unfractionated heparin (UH) in end stage renal disease (ESRD) patients. Search Methods. Pubmed, Embase and cochrane central were searched for eligible citations. Selection Criteria. Randomized controlled trials, comparing LMWH and UH, involving adult (age > 18 years), ESRD patients receiving outpatient, chronic, intermittent hemodialysis were included. Data Collection and Analysis. Two independent reviewers performed independent data abstraction. I2 statistic was used to assess heterogeneity. Random effects model was used for meta-analysis. Results. Nineteen studies were included for systematic review and 4 were included for meta-analysis. There were no significant differences between LMWH and UFH for extracorporeal circuit thrombosis [risk ratio: 1 (95% CI [0.62-1.62])] and bleeding complications [risk ratio: 1.16 (95% CI [0.62-2.15])]. Conclusions. LMWH is as safe and effective as UFH. Considering the poor quality of studies included for the review, larger well conducted RCTs are required before conclusions can be drawn.

Dosing practice of low molecular weight heparins and its efficacy and safety in cardiovascular inpatients: a retrospective study in a Chinese teaching hospital

Abstract:Background: Low-molecular-weight heparins (LMWHs) are safe and effective anticoagulant options for cardiovascular patients when applied as body weight-adjusted doses. However, there are some barriers that make it difficult to implement weight-adjusted doses in clinical practice. Therefore, it is vital to learn the dosing practices of LMWH and its efficacy and safety in clinical practice. Methods: A retrospective study was conducted in cardiovascular inpatients who had received at least one dose of LMWH during a 6-month period. Appropriateness of LMWH dosing was determined and major clinical outcomes (major adverse vascular events and major bleeding) during hospitalization were evaluated. Results: A total of 376 admissions representing 364 patients received LMWH treatment. Of these, 17.0% (64/376) of admissions did not have body weight records. Of the 312 admissions included for the outcome study, only 34 cases (10.9%) received the recommended doses of LMWH, while 51 cases (16.3%) received mild underdoses, 223 cases (71.5%) received major underdoses and 4 (1.3%) received excess doses. There were 10 major adverse vascular events, which occurred more often in patients receiving excess doses of LMWH than in patients receiving recommended, mild or major underdoses (50%, 2.9%, 2.0% and 2.7%, respectively, P < 0.001). After multivariable analysis, severe renal insufficiency was an independent risk factor for major adverse vascular events [odds ratio (OR), 31.93; 95% confidence interval (CI), 5.99-170.30; P < 0.001]. No major bleeding was recorded. Conclusions: Underdose of LMWH is commonly used in cardiovascular inpatients, which was suboptimal according to guidelines. Using LMWH at a fixed, low dose for treatment purposes in patients without severe renal insufficiency was not associated with a higher risk of adverse vascular events in the current study, though larger studies with extended follow-ups are required to fully assess the long-term consequences of LMWH underdosing.
Evaluation of Optimal Dosage of Heparin in Hemodialysis Patients by Thromboelastograph

Abstract:OBJECTIVE:To assess the blood coagulation function and investigate the appropriate dose of unfractionated heparin by thromboelastograph in maintenance hemodialysis (MHD) patients.METHODS:Thirty MHD patients were enrolled in this study and divided into two groups. The total dose of unfractionated heparin was below 80 u/kg in the low-dose group (LH, n=16), while it exceeded 80 u/kg in the high-dose group (HH, n=14). Blood routine tests and conventional coagulation examinations were measured before hemodialysis. TEG and activated partial thromboplastin time (APTT) were examined at the beginning and the end of hemodialysis at the arterial circuit, and the second hour (h 2) at the venous circuit.RESULTS:The initial bolus dose of unfractionated heparin for LH and HH groups were (26.6±6.2) u/kg vs. (42.3±8.2) u/kg and the repeated maintenance dose for both the groups were (13.7±5.1) u/kg/h vs. (18.2±4.3) u/kg/h. No significant difference was noticed in results from blood routine tests and conventional coagulation parameters between the two groups. In LH group, the increase of APTT at h 2 of hemodialysis was significant compared with the baseline, while it recovered partly at the end of hemodialysis. R value prolonged at h 2 and the end of hemodialysis. CI value was more negative at the end of hemodialysis. In HH group, APTT obviously prolonged at h 2 and the end of hemodialysis. R value also obviously prolonged at h 2 of hemodialysis. At the end of hemodialysis, R and K values prolonged, MA value reduced, and CI value was more negative. APTT was significantly different between the two groups at h 2 of hemodialysis. At the end of hemodialysis, APTT was still extended in HH group, but there was no significant difference. R value at h 2, and R, K, MA, CI values at the end of hemodialysis were significantly different between the two groups. R values at the end of hemodialysis had a direct correlation with the dose of unfractionated heparin (r=0.403, P=0.041), but APTT had not. There was no significant difference in transmembrane pressure, venous pressure and filter clotting between the two groups.CONCLUSION:Low-dose heparin is effective and safe as anticoagulant in hemodialysis. TEG shows that the blood coagulation function is more sensitive than conventional coagulation parameters and is useful to anticoagulant therapy in MHD patients.
Comparison of the injection of low‐molecular weight heparin in the arterial vs. venous blood line for preventing extracorporeal circuit clotting during hemodialysis

Abstract:Low-molecular weight heparins (LMWH) are widely used for preventing clotting during hemodialysis (HD). Although injection in the venous blood line is recommended to avoid initial loss of LMWH through the dialyzer, LMWH is still frequently administered in the arterial blood line at the start of dialysis. This study aimed to compare the safety and efficacy of the same enoxaparin dose administered through the venous blood line or arterial blood line. We also evaluated antifactor Xa (aXa) activity according to the injection route and dialysis modalities: high-flux (HF) HD, medium cut-off (MCO) HD, and online hemodiafiltration (OL-HDF). Forty-three patients were studied over 18 consecutive dialysis sessions using a fixed enoxaparin dose (20 or 40 mg), first administered through the arterial blood line bolus and then through the venous blood line for another 18 sessions. Compared to arterial blood line administration, venous blood line bolus resulted in a significant increase in median post-dialysis aXa activity: 0.16 (0.1-0.6) IU/ml versus 0.31 (0.1-1.3) IU/ml, respectively, p = 0.006. After arterial blood line bolus of 40 mg enoxaparin, median post-dialysis aXa activity was significantly lower with OL-HDF compared to HF-HD: 0.14 (0.1-0.35) versus 0.32 (0.15-0.49), p = 0.02. A trend for lower clotting within lines and bubble trap using venous blood line bolus was observed. In conclusion, venous blood line enoxaparin injection is safe in OL-HDF patients. However, in HF-HD and MCO-HD, venous blood line injection of 40 mg enoxaparin may increase overdosing risk. Thus, aXa activity should be monitored in HF-HD and MCO-HD patients at risk of bleeding and/or on vitamin K antagonists and careful surveillance is required when administering a 40 mg enoxaparin dose through the venous blood line route.

Using nafamostat mesilate anticoagulation for continuous renal replacement therapy in high bleeding risk patients: a systematic review and meta-analysis

Abstract:Objectives: The purpose of this systematic review is to evaluate the safety and efficacy of nafamostat mesilate (NM) for continuous renal replacement therapy in high bleeding risk patients. Methods: We performed a comprehensive search on PubMed, Embase, CINAHL, Web of Science, the Cochrane Library, and CNIK from inception to May 1, 2022. Two reviewers independently screened, selected, and extracted data.A meta-analysis was conducted using RevMan 5.3 software. The systematic review protocol was registered. Results: 7 studies with 639 high bleeding risk patients were included for review including 4RCTs and 3 retrospective cohort studies. The anticoagulant in the control group of all included studies was anticoagulation-free. Compared with the baseline data, the activated partial thromboplastin time of the NM group was shortened at the end of observation (MD -12.16, 95% CI -23.70 to -0.62, p= 0.04). NM for CRRTsignificantly extended filter life (MD 9.65, 95% CI 5.60 to 13.71, p< 0.00001)and reduced mortality compared with anticoagulation-free treatment (RR 0.76, 95% CI 0.64 to 0.90, p= 0.002). No significant difference was observed in the incidence of bleeding complications (RR 1.16, 95% CI 0.59 to 2.26, p= 0.67) and total duration of CRRT (MD 12.65, 95% CI -6.79 to 32.09, p= 0.20) between the groups. Subgroup analysis showed the same results except for the mortality outcome. Conclusions: For patients at high bleeding risk undergoing CRRT, NM is a better anticoagulation option. More multicenter RCTs with large samples are required to validate and compare the effects of NM anticoagulants.
Nocturnal hemodialysis: three-year experience

Abstract:There is evidence that high frequency, as well as long duration, hemodialysis provides better clinical outcomes. We developed nocturnal hemodialysis, a new innovative form of renal replacement therapy, which is performed six to seven nights per week for 8 to 10 h during sleep at home. Blood flow was set at 300 ml/min and dialysate flow at 100 ml/min. An internal jugular catheter was used as the vascular access. Special precautions were taken to prevent accidental disconnection during sleep, as well as air embolization. Dialysis functions from the patient's home were monitored continuously via a modem at the nocturnal hemodialysis center. Twelve patients have completed training and have been successfully performing nocturnal hemodialysis for up to 34 mo. This study represents 170 patient months of experience accumulated over 3 yr. There was hemodynamic stability and significant subjective improvement in patient well being. Nightly Kt/V was 0.99. Weekly removal of phosphate was twice as high and beta2 microglobulin 4 times as high as conventional hemodialysis. All patients have discontinued their phosphate binders and have increased dietary phosphate and protein intake. BP control was achieved with fewer medications. Dialyzer reuse has decreased the operating costs to the level of the other form of home dialysis. Complications were infrequent and were related primarily to the dialysis access. Nocturnal hemodialysis represents the most efficient form of dialysis at low cost and should be considered as an option for patients who can be trained for home hemodialysis.

Abstract:Summary Unfractionated heparin (UFH) and low-molecular-weight heparins (LMWHs) are commonly used to prevent clotting of the hemodialysis extracorporeal circuit and optimize hemodialysis adequacy. There is no consensus on the optimal dosing for UFH and LMWHs during hemodialysis. In clinical practice, semiquantitative clotting scoring of the dialyzer and venous chamber may help to guide UFH and LMWH dose adjustment. Laboratory monitoring has not been shown to improve clinical outcomes and is therefore not routinely indicated in most hemodialysis patients. It might, however, be considered in select patients, such as those with extremes of body weight or history of repeated clotting or bleeding. Methods for laboratory monitoring include the activated partial thromboplastin time, activated clotting time, and antifactor Xa assays for UFH and antifactor Xa assay for LMWHs. Target ranges for anticoagulation in hemodialysis have been suggested but not clearly defined. When utilizing these tests, issues such as availability, standardization, interfering factors, and interpretation must be considered. In this narrative review, we discuss the rationale and methods of monitoring anticoagulation in hemodialysis.

Radiculopathy and Myelopathy in Patients with Primary Cervical Dystonia

Abstract:In a prospective series of 34 incident patients with primary cervical dystonia (CD), 6 showed clinical or radiological signs of cervical radiculopathy (RP) or myelopathy (MP) during the course of their movement disorder. Age at onset in these patients was in the range reported for pure spondylotic cervical RP without an accompanying movement disorder. Radiologically, spondylosis was mild in 1 case and absent in 2 cases. The intervals between onset of CD and RP were shorter than in literature reports of RP/MP in dystonic-athetotic patients of cerebral paresis. Clinically, RP/MP in patients with CD mostly appears at mid-cervical levels, whereas cases with cerebral paresis are more frequently affected at higher cervical segments. We propose that RP/MP does not occur more frequently in CD than in pure spondylotic cervical RP.
Low-dose Versus High-Dose Heparin Locks for Hemodialysis Catheters: a Systematic Review and Meta-Analysis

Abstract:BACKGROUND:Low-dose heparin lock has been suggested as an effective and safe catheter lock in hemodialysis. However, whether a low-dose lock is superior to a high-dose heparin lock in preventing catheter-related infections and maintaining catheter patency is inconclusive.STUDY DESIGN:A systematic review and meta-analysis was performed by searching in NGC (National Guideline Clearinghouse), Cochrane Library, Joanna Briggs Institute Library, MEDLINE, EMbase, RNAO (Registered Nurses' Association of Ontario), Nursing Consult, PubMed, OVID, China Biology Medicine disc (CBMdisc), China National Knowledge Infrastructure (CNKI), Chongqing VIP (CQVIP), Wanfang Data Knowledge Service Platform, and major nephrology journals.SETTING AND POPULATION:Patients receiving hemodialysis with central venous catheters.SELECTION CRITERIA FOR STUDIES:Randomized controlled trials (RCT), controlled clinical trials (CCT), and cohort studies comparing low-dose heparin lock with high-dose heparin lock in hemodialysis patients with central venous catheters. Intervention (exposure): Locking central venous catheters with low-dose heparin lock.OUTCOMES:Primary outcomes include bleeding-related complications and catheter-related infections. Secondary outcomes include catheter retention time, catheter thrombosis/occlusion incidence, and catheter dysfunction.RESULTS:Five RCTs and three CCTs were eventually identified for the meta-analysis, with 916 patients in total. Meta-analysis showed that, compared with the high-dose heparin group (≥ 5,000 U/mL), the low-dose (< 5,000 U/mL) heparin lock could significantly reduce the incidences of bleeding- related complications RR = 3.29, 95% CI (2.19, 4.94), p < 0.00001) and catheterrelated infections (RR = 1.66, 95% CI (1.01, 2.73), p = 0.04). However, no significant differences were observed in the catheter retention time MD = 9.32, 95% CI (-6.70, 25.35), p = 0.25), catheter thrombosis/occlusion incidence (RR = 0.68, 95% CI (0.28, 1.65), p = 0.39), or catheter dysfunction (RR = 1.07, 95% CI (0.75, 1.53), p = 0.71) between the high- and low-dose heparin groups.CONCLUSION:Low-dose heparin lock could decrease the incidences of catheter-related infections and bleeding-related complications without influencing the catheter retention time or the incidence of catheter thrombosis/occlusion or catheter dysfunction. Low-dose heparin lock solution should be recommended for the hemodialysis patients using central venous catheter in clinic.
Effects of Changing Anticoagulation with Unfractionated Heparin into Enoxaparin in Hemodialysis Patients:Prospective Non Randomized Unicentre Study

Abstract:Background: Maintaining the patency of the extracorporeal circuit (ECC) is required. The cost and long-term effect of anticoagulants on hemodialysis (HD) patients' parameters are still matters of concern. We aimed to study the effect of shifting to enoxaparin instead of unfractionated heparin (UFH) during HD and evaluate its efficacy, safety, and benefits. Design and methodology: This study was conducted at Arar hospital, Kingdom of Saudi Arabia (KSA), and included 86 end-stage renal disease (ESRD) patients on HD for six months starting from October 2017. All patients had been on UFH as an anticoagulant during hemodialysis sessions before they were shifted to equivalent doses of enoxaparin sodium (Clexan®) as a therapeutic upgrade to start with. The patients’ files were examined retrospectively for the period of UFH therapy and then all patients were observed prospectively for at least 6 months while on Clexane. The occurrence of clotting in the dialysis sets were recorded and the need for urgent HD for each patient and the reasons. Results: There was a reduction of filter clots even with the reduction of enoxaparin dose. There was a significant reduction of mean cholesterol, calcium, phosphorus, sodium, chloride, and serum osmolality. We found a significant reduction of pre-dialysis potassium among those who presented with a pre-dialysis potassium of higher than 5.5 mg/dl during the enoxaparin phase. Patients, who had thrombocytopenia while on UFH; showed improvement in platelet count after the shift to enoxaparin. However, patients who had platelet count lowered than 150000/mm3 6 months after having been shifted to enoxaparin; many of them received HD through a permanent catheter. No patients had a three-fold increase of normal values (3ULN) of liver function tests whether being on heparin or enoxaparin. Conclusion: Low molecular weight heparin (LMWH) could be more efficacious and safe than UFH with the improvement of some variables in HD patients.
The Effect of Low Molecular Weight Heparin on Hemodialysis and Anticoagulation Related Nursing Aspect

Abstract:To observe the anticoagulation effect of low molecular weight heparin on hemodialysis and to study its related nursing aspect.Method:APTT of 26 patients evaluated before and during hemodialysis,complications and coagulation of dialyzer of standard heparin and low molecular weight heparin were observed.Result:APTT was prolonged in the patients with standard heparin and more hemorrage complications were observed.No hemorrage was found in low molecular weight heparin group.Conclusion:Low molecular weight heparin has good and safe anticoagulation effect than standard heparin in hemodialysis.

Abstract:Objective:To compare the safety and efficacy of regional citrate plus low dose of low molecular weight heparins(LMWH) with only regional citrate or normal dose of,LMWH for different anticoagulation methods in patients with continuous veno-venuous hemofiltration(CVVH).Methodology:Fifty seven critically ill patients who requiring CVVH without anticoagulation contraindications were enrolled in this study.They were randomly divided into 3 groups adopting different anticoagulation protocols as following;regional citrate anticoagulation in group A,systemic LMWH in group B [loading dose of LMWH 40 IU/kg,maintenance dose 4 lU/(kg·h)],and regional citrate plus low dose of LMWH in group C[loading dose 20 IU/kg,maintenance dose 2 IU/(kg·h)].The primary outcome was the filter survival time,and the secondary outcome was the change of hemoglobin(Hb),platelet counts(PLT),and anticoagulation-related side effects. Results:Fifty-three patients completed the study and entered into data analysis,15 in group A,19 in group B and 19 in group C.The mean APACHEII scores in each group were 16.2±3.65,17.11±3.5 and 17.01±4.79,respectively(P 0.05).The filter survival time were(21.2±3.48 )h in group A,(25.1±5.50) h in group B,and(40.4±7.08) h in group C(P 0.01).There were 3 patients in each group of A(20%) and B(15.8%) switching to the anticoagulation protocol of group C due to fdter life span less than 8 hrs,4 patients in group B(21%) switching to group C due to bleeding,3 patients in group A(20%) and 4 patients in group C(21.1%) switching to group B due to citrate related complications. The percentage of patients with reduction of Hb and platelet counts levels was more than 30%,while no differences was found among 3 groups(P 0.05 ).There were no differences in the CV VH duration,hospital days and ICU days and giving up or death rates between 3 groups.Conclusion:Compared with anticoagulation using only regional citrate or LMWH, regional citrate plus low dose of LMWH protocol was more efficacy in prolonging filter life span,without significant increase of anticoagulation-related complications.

Abstract:The nephrotic syndrome (NS) carries one of the highest risks of thrombotic complications. Consequently, over the last 15 years, some nephrologists have treated patients risk (i.e. those with albuminemia < 20 g/l and membranous nephropathy) with anticoagulants: either subcutaneous heparin (Kakkar protocol) or antivitamin K. Low-molecular-weight heparin (LMWH) has a longer plasma half-life and better bioavailability than standard heparin and can thus be administered as a single daily injection. LMWH also carries a lower risk of hemorrhage. We prospectively studied the safety and efficacy of the LMWH Enoxaparin for preventive anticoagulation in NS. In a preliminary study, 10 adult nephrotic patients with biological markers of thrombosis risk (severe hypoalbuminemia and/or anomalies of the fibrinolytic pathway and/or deficiency in coagulation inhibitors) were given 40 mg (4,000 U) of Enoxaparin daily for at least 3 months; 3 patients were treated for 3 months, 1 for 6 months and 6 for 12 months. Patients were assessed for silent thrombosis, using renal vein Doppler ultrasonography, lower leg vein Doppler ultrasonography and lung ventilation-perfusion scintigraphy, before entry to the trial and subsequently at 3-month intervals. As LWMH caused no obvious side effects and no thrombosis was observed during the pilot study, we then placed 55 adult nephrotic patients free of thrombosis on the same treatment. Patients were seen according to the usual calendar required by their individual illnesses. At each examination, patients were assessed for clinical signs and symptoms of thrombosis and side effects; plasma D-dimer and urinary fibrin-fibrinogen degradation products were also measured at each visit.(ABSTRACT TRUNCATED AT 250 WORDS)
The effect of low molecular weight heparin as an antioxidant in chronic kidney disease

Abstract:Objective To identify the anti-oxidative effect of low molecular weight heparin (LMWH) in chronic kidney disease (CKD). Method Forty CKD patients at stage 3-5 were randomly divided into two groups, glutathione (GSH) group (0.9% NaCl 250ml + Glutathione 1.8g/d, intravenous injection for 10 days) and LMWH group (Fraxiparine 0.4ml/d, subcutaneous injection for 10 days). Plasma malondialdehyde (MDA), advanced oxidation protein product (AOPP) and glutathione peroxidase activity (GSH-PX) were measured at the beginning and the end of glutathione or LMWH administration. Results Plasma AOPP decreased and plasma GSH-PX increased significantly after the treatment in both LMWH and GSH groups (P0.05). The decrease of plasma AOPP was more significant in GSH group than in LMWH group (2.86±0.82 vs. 4.89±0.66μmol/l, P0.05). After the treatment, plasma MDA did not change significantly in LMWH group, but decreased (5.09±1.44 vs. 4.61±1.30nmol/ml, P0.05) in GSH group. There were no apparent side effects in both groups. Conclusion LMWH ameliorates oxidative stress state without apparent side effects. The anti-oxidative effect of LMWH is slightly lower than that of GSH.

Primary Clinical Experience in High Risk Uremic Patients Converted from Conventional to Short Daily Hemodialysis

Abstract:Objective To observe the effect of short daily hemodialysis (DHD) on high risk uremic patients converted from conventional HD . Methods Sixteen high risk patients who were not be able to tolerate or had no response to conventional hemodialysis were converted to 2 hours daily hemofiltration (HF) or hemodiafiltration (HDF) , 6 sessions per week. The hypotension during the course of treatment, and the change of body weight, the weekly Kt/V, the serum phosphorus, the serum albumin, the hemoglobin, the cardiothoracic ratio, (CTR), the left ventricular mass index (LVMI )and SF-36 quality of life scales were compared between pre-DHD and post-DHD, conventional HD and DHD. Results Compared with conventional hemodialysis, in DHD patients, weekly Kt/V increased from 4.36±0.62 to 4.88±0.41 (P=0.009), blood pressure decreased almost to normal, occurrence of hypotension during the session reduced significantly, serum albumin and hemoglobin enhanced significantly, CTR and LVMI were improved. The scores of 6 scales in SF-36, such as physical function (PF ), role physical (RP), bodily pain(BP), general health (GH) ,vitality (VT) , mental health (MH) were significantly improved as well (P 0.05). Conclusions High risk uremic patients can tolerate the short daily hemodialysis. DHD can keep the blood pressure stable, maintain hemodialysis adequacy,and improve the nutritional status and quality of life.

Abstract:Objective To compare the anticoagulation safety and efficacy of the two intravenous methods for low molecular weight heparin administration in continuous renal replacement therapy (CRRT).Methods Seventeen patients with severe acute renal failure with or without multiple organ failure and undergoing CRRT using Fragmin for anticoagulation were enrolled in this study.They were assigned to 2 groups:group A using continuous administration of Dalteparin/Fragmin,and group B using intermittent administration of Dalteparin/Fragmin.AntiXa factor activity,coagulation indexes,serum creatinine,and blood urea nitrogen were measured before and after CRRT.Results Anti-Xa factor activity was significantly higher in group A than in group B after continuous venovenous hemofiltration (CVVH) for 2 hours,but did not differ between the 2 groups at other time points.Activated partial thromboplastin time (APTT) was significantly increased at the end of CVVH in both groups.Conclusion Either continuous or intermittent administration of Dalteparin/Fragmin is safe and effective,but intermittent administration may cause greater variation in anticoagulation status.

Abstract:Some high-risk patients with end-stage renal disease (ESRD) cannot tolerate or do not respond to conventional haemodialysis three times per week. Sixteen high-risk ESRD patients were converted from conventional haemodialysis to short daily haemodialysis (sDHD). This consisted of 2 h daily haemofiltration or haemodiafiltration, six times per week. The dialysis response, blood pressure change, weight variations, weekly Kt/V (a measure of the adequacy of dialysis), serum phosphorus, serum albumin, haemoglobin, cardiothoracic ratio (CTR), left ventricular mass index (LVMI) and Short Form Health Survey (SF-36) quality-of-life scales were compared before and after conversion to sDHD. sDHD improved many clinical and biological variables. Weekly Kt/V increased from 4.36 +/- 0.62 on conventional haemodialysis to 4.88 +/- 0.41 after switching to sDHD. Blood pressure normalized, there were significant decreases in episodes of hypotension during haemodialysis, serum phosphorus concentration, CTR and LVMI, and there were significant increases in levels of serum albumin and haemoglobin. Calcium and phosphorus metabolism, and nutritional status were improved. Physical function, physical role, bodily pain, general health, vitality and mental health improved significantly.
Effectiveness and Safety of Nadroparin in Acute Coronary Syndrome

Abstract:OBJECTIVE:To evaluate the effectiveness and safety of subcutaneous low molecular weight heparin (LMWH) used in acute management of patients with non-ST segment elevation acute coronary syndrome (ACS).METHODS:A total of 102 patients with non-ST segment elevation ACS were treated for at least 48 hours ( > or =5 times) with subcutaneous nadroparin (1 mg/kg each 12 hours). All 102 patients underwent coronary angiographies (CAG) within 8 hours after LMWH injection, followed by immediate percutaneous coronary intervention (PCI).RESULTS:Anti-Xa activity at the time of catheterization was (0.62 +/- 0.18) IU/ml, and 90% of the patients had anti-Xa activity > 0.5 IU/ml. No death, myocardial infarction relapse or emergent revascularization occurred after PCI. Thrombosis and/or embolism occurred in 2 patients (3.5%) during PCI. Mild hemorrhage was observed in 4 patients (3.9%) of PCI group and in 2 patients (4.4%) in CAG group. No major hemorrhage occurred.CONCLUSION:PCI within 8-12 hours of the last dose after > or =48 hours nadroparin subcutaneous injection seems to be effective and safe.
