Abstract:Time perception is an essential element of conscious and subconscious experience, coordinating our perception and interaction with the surrounding environment. In recent years, major technological advances in the field of neuroscience have helped foster new insights into the processing of temporal information, including extending our knowledge of the role of the cerebellum as one of the key nodes in the brain for this function. This consensus paper provides a state-of-the-art picture from the experts in the field of the cerebellar research on a variety of crucial issues related to temporal processing, drawing on recent anatomical, neurophysiological, behavioral, and clinical research.The cerebellar granular layer appears especially well-suited for timing operations required to confer millisecond precision for cerebellar computations. This may be most evident in the manner the cerebellum controls the duration of the timing of agonist-antagonist EMG bursts associated with fast goal-directed voluntary movements. In concert with adaptive processes, interactions within the cerebellar cortex are sufficient to support sub-second timing. However, supra-second timing seems to require cortical and basal ganglia networks, perhaps operating in concert with cerebellum. Additionally, sensory information such as an unexpected stimulus can be forwarded to the cerebellum via the climbing fiber system, providing a temporally constrained mechanism to adjust ongoing behavior and modify future processing. Patients with cerebellar disorders exhibit impairments on a range of tasks that require precise timing, and recent evidence suggest that timing problems observed in other neurological conditions such as Parkinson’s disease, essential tremor, and dystonia may reflect disrupted interactions between the basal ganglia and cerebellum.The complex concepts emerging from this consensus paper should provide a foundation for further discussion, helping identify basic research questions required to understand how the brain represents and utilizes time, as well as delineating ways in which this knowledge can help improve the lives of those with neurological conditions that disrupt this most elemental sense. The panel of experts agrees that timing control in the brain is a complex concept in whom cerebellar circuitry is deeply involved. The concept of a timing machine has now expanded to clinical disorders.

Abstract:A central question in understanding cognition and pathology-related cognitive changes is how we process time. However, time processing difficulties across several neurological and psychiatric conditions remain seldom investigated. The aim of this review is to develop a unifying taxonomy of time processing, and a neuropsychological perspective on temporal difficulties. Four main temporal judgments are discussed: duration processing, simultaneity and synchrony, passage of time, and mental time travel. We present an integrated theoretical framework of timing difficulties across psychiatric and neurological conditions based on selected patient populations. This framework provides new mechanistic insights on both (a) the processes involved in each temporal judgement, and (b) temporal difficulties across pathologies. By identifying underlying transdiagnostic time-processing mechanisms, this framework opens fruitful avenues for future research.

Abstract:Time cognition is an essential function of human life, and the impairment affects a variety of behavioral patterns. Neuropsychological approaches have been widely demonstrated that Parkinson's disease (PD) impairs time cognitive processing. Many researchers believe that time cognitive deficits are due to the basal ganglia, including the striatum or subthalamic nucleus, which is the pathomechanism of PD, and are considered to produce only transient recovery due to medication effects. In this perspective, we focus on a compensatory property of brain function based on the improved time cognition independent of basal ganglia recovery and an overlapping structure on the neural network based on an improved inhibitory system by time cognitive training, in patients with PD. This perspective may lead to restoring multiple functions through single function training.

Abstract:Purpose of review: New insights into the psychophysiological determinants of performance changes and brain plasticity associated with motor sequence learning have recently been gained through behavioral and imaging studies in healthy individuals. In addition, using a variety of motor sequential paradigms in groups of patients affected by a movement disorder, major advances have been achieved in our understanding of the pathophysiological mechanisms underlying Parkinson's and Huntington's diseases, as well as primary forms of dystonia. Recent findings: This review begins by describing the latest findings in normal participants with regards to the dynamic alterations in neural networks observed across the different phases of motor sequence learning. It then focuses on the hotly debated issue of motor memory consolidation, highlighting the results of novel studies that investigated the role of both day and night sleep, the neural substrates and the developmental evolution mediating this process. Finally, this paper addresses current work looking at motor sequence learning in movement disorders that helps to better comprehend the functional contribution of basal ganglia structures to this type of memory, to assess the impact of such diseases on related patterns of brain activation, as well as to identify the neuronal compensatory mechanisms educed by these basal ganglia disorders. Summary: Such advances have major implications, not only for optimizing ways to learn new skilled behaviors in real-life situations, but also for guiding therapeutic approaches in patients with movement disorders.
The Cumulative Effect of Transient Synchrony States on Motor Performance in Parkinson's Disease

Abstract:Bursts of beta frequency band activity in the basal ganglia of patients with Parkinson's disease (PD) are associated with impaired motor performance. Here we test in human adults whether small variations in the timing of movement relative to beta bursts have a critical effect on movement velocity and whether the cumulative effects of multiple beta bursts, both locally and across networks, matter. We recorded local field potentials from the subthalamic nucleus (STN) in 15 PD patients of both genders OFF-medication, during temporary lead externalization after deep brain stimulation surgery. Beta bursts were defined as periods exceeding the 75th percentile amplitude threshold. Subjects performed a visual cued joystick reaching task, with the visual cue being triggered in real time with different temporal relationships to bursts of STN beta activity. The velocity of actions made in response to cues prospectively triggered by STN beta bursts was slower than when responses were not time-locked to recent beta bursts. Importantly, slow movements were those that followed multiple bursts close to each other within a trial. In contrast, small differences in the delay between the last burst and movement onset had no significant impact on velocity. Moreover, when the overlap of bursts between the two STN was high, slowing was more pronounced. Our findings suggest that the cumulative, but recent, history of beta bursting, both locally and across basal ganglia networks, may impact on motor performance. SIGNIFICANCE STATEMENT Bursts of beta frequency band activity in the basal ganglia are associated with slowing of voluntary movement in patients with Parkinson's disease. We show that slow movements are those that follow multiple bursts close to each other and bursts that are coupled across regions. These results suggest that the cumulative, but recent, history of beta bursting, both locally and across basal ganglia networks, impacts on motor performance in this condition. The manipulation of burst dynamics may be a means of selectively improving motor impairment.

Differential Temporal Perception Abilities in Parkinson’s Disease Patients Based on Timing Magnitude

Abstract:Abstract Non-motor symptoms in Parkinson’s Disease (PD) predate motor symptoms and substantially decrease quality of life; however, detection, monitoring, and treatments are unavailable for many of these symptoms. Temporal perception abnormalities in PD are generally attributed to altered Basal Ganglia (BG) function. Present studies are confounded by motor control facilitating movements that are integrated into protocols assessing temporal perception. There is uncertainty regarding the BG’s influence on timing processes of different time scales and how PD therapies affect this perception. In this study, PD patients using Levodopa (n = 25), Deep Brain Stimulation (DBS; n = 6), de novo patients (n = 6), and healthy controls (n = 17) completed a visual temporal perception task in seconds and sub-section timing scales using a computer-generated graphical tool. For all patient groups, there were no impairments seen at the smaller tested magnitudes (using sub-second timing). However, all PD groups displayed significant impairments at the larger tested magnitudes (using interval timing). Neither Levodopa nor DBS therapy led to significant improvements in timing abilities. Levodopa resulted in a strong trend towards impairing timing processes and caused a deterioration in perceptual coherency according to Weber’s Law. It is shown that timing abnormalities in PD occur in the seconds range but do not extend to the sub-second range. Furthermore, observed timing deficits were shown to not be solely caused by motor deficiency. This provides evidence to support internal clock models involving the BG (among other neural regions) in interval timing, and cerebellar control of sub-second timing. This study also revealed significant temporal perception deficits in recently diagnosed PD patients; thus, temporal perception abnormalities might act as an early disease marker, with the graphical tool showing potential for disease monitoring.

Abstract:Tourette syndrome (TS) is a neurodevelopmental disorder characterized by motor and vocal tics. Co-occurrence of attention-deficit/hyperactivity disorder (ADHD) or obsessive–compulsive disorder (OCD) is very frequent in the pediatric population as well as the presence of an impairment of the executive functions. The aim of our study was to investigate motor timing, that is, the temporal organization of motor behavior, in a pediatric population of Tourette patients. Thirty-seven Tourette patients (divided in 22 "pure" Tourette patients and 15 with ADHD) were compared with 22 healthy age- and gender-matched subjects. All subjects underwent a neuropsychiatric screening and were tested for their planning and decision-making abilities by using a standardized test, such as Tower of London (ToL). Two experimental paradigms were adopted: finger-tapping test (FTT), a free motor tapping task, and synchronization–continuation task. An accuracy index was calculated as measure of ability of synchronization. We found that "pure" TS as well as TS+ADHD showed lower scores in the FTT for the dominant and non-dominant hands than controls. Moreover, in the synchronization and continuation test, we observed an overall lack of accuracy in both TS groups in the continuation phase for 2,000 ms (supra-second interval), interestingly, with opposite direction of accuracy index. Thus, "pure" TS patients were classified as "behind the beat," whereas, TS+ADHD as "ahead of the beat." The performance in the finger tapping was inversely correlated to ToL total scores and execution time, whereas we did not find any correlation with the accuracy index of the synchronization and continuation test. In conclusion, here, we explored motor timing ability in a childhood cohort of Tourette patients, confirming that patients exhibit an impaired temporal control of motor behavior and these findings may be explained by the common underlying neurobiology of TS and motor timing.

Abstract:Abstract This study investigated the effects of different types of neurological deficits on timing functions. The performance of Parkinson, cerebellar, cortical, and peripheral neuropathy patients was compared to age-matched control subjects on two separate measures of timing functions. The first task involved the production of timed intervals in which the subjects attempted to maintain a simple rhythm. The second task measured the subjects' perceptual ability to discriminate between small differences in the duration of two intervals. The primacy of the cerebellum in timing functions was demonstrated by the finding that these were the only patients who showed a deficit in both the production and perception of timing tasks. The cerebellar group was found to have increased variability in performing rhythmic tapping and they were less accurate than the other groups in making perceptual discriminations regarding small differences in duration. Critically, this perceptual deficit appears to be specific to the perception of time since the cerebellar patients were unaffected in a control task measuring the perception of loudness. It is argued that the operation of a timing mechanism can be conceptualized as an isolable component of the motor control system. Furthermore, the results suggest that the domain of the cerebellar timing process is not limited to the motor system, but is employed by other perceptual and cognitive systems when temporally predictive computations are needed.

Abstract:Background Psychomotor disturbances are observed across psychiatric disorders and often present as psychomotor slowing, agitation, disorganized behavior, or catatonia. Psychomotor function includes both cognitive and motor components, but the neural circuits driving these subprocesses and how they relate to symptoms have remained elusive for centuries. Methods We analyzed data from the Human Connectome Project for Early Psychosis, a multi-site study of 125 people with early psychosis and 58 healthy participants with resting-state fMRI and clinical characterization. Psychomotor function was assessed using the 9-Hole Pegboard Task, a timed motor task which engages mechanical and psychomotor components of action, and tasks assessing processing speed and task switching. We used multivariate pattern analysis of whole-connectome data to identify brain correlates of psychomotor function. Results We identified discrete brain circuits driving the cognitive and motor components of psychomotor function. In our combined sample of psychosis patients (n=89) and non-psychiatric controls (n=52), the strongest correlates of psychomotor function (pegboard performance) (p<.005) were between a midline cerebellar region and (1) left frontal region and (2) pre-supplementary motor area (preSMA). Psychomotor function was correlated with both cerebellar-frontal connectivity (r=0.33) and cerebellar-preSMA connectivity (r=0.27). However, the cognitive component of psychomotor performance (task-switching) was only correlated with cerebellar-frontal connectivity (r=0.19), whereas the motor component (processing speed) was only correlated with cerebellar-preSMA connectivity (r=0.15), suggesting distinct circuits driving unique subprocesses of psychomotor function. Conclusions We identified cerebellar-cortical circuits that drive distinct subprocesses of psychomotor function. Future studies should probe relationships between cerebellar connectivity and psychomotor performance using neuromodulation.

Self and time in individuals with schizophrenia: A motor component?

Abstract:Phenomenology suggests a disruption in the experience of time in individuals with schizophrenia, related to disorders of the sense of self. Patients themselves relate a fragmentation of their temporal experience and of their sense of self. Temporal expectations help relate the present moment to the future and we have previously shown that temporal expectations are fragile in patients, and relate to disorders of the self. Here, we investigate whether patients' performance is still impaired when the motor response to the expected event can be prepared in advance. In two different experiments participants (41 patients vs. 43 neurotypicals in total) responded to a visual target occurring at a variable interval (or "foreperiod") after an initial warning signal. Moreover, in Experiment 1 we measured the sense of self with the EASE scale. We observed the usual benefit of the passage of time: the longer the waiting period, the better the preparation, and the faster the responses. However, this effect also comprises sequential (surprise) effects, when a target occurs earlier than on the preceding trial. We evaluated the effect of the passage of time, by isolating trials that followed a trial with the same foreperiod. The benefit of long, versus short, foreperiods was still observed in controls but disappeared in patients. The results suggest that the benefit of the passage of time is diminished in patients and relates to self disorders, even when the task allows for motor preparation. The results suggest that a non-verbal impairment sub-tends disorders of the sense of self.

Abstract:Durations in the several seconds' range are cognitively accessible during active timing. Functional neuroimaging studies suggest the engagement of the basal ganglia (BG) and supplementary motor area (SMA). However, their functional relevance and arrangement remain unclear because non-timing cognitive processes temporally coincide with the active timing. To examine the potential contamination by parallel processes, we introduced a sensory control and a motor control to the duration-reproduction task. By comparing their hemodynamic functions, we decomposed the neural activities in multiple brain loci linked to different cognitive processes. Our results show a dissociation of two cortical neural circuits: the SMA for both active timing and motor preparation, followed by a prefrontal-parietal circuit related to duration working memory. We argue that these cortical processes represent duration as the content but at different levels of abstraction, while the subcortical structures, including the BG and thalamus, provide the logistic basis of timing by coordinating the temporal framework across brain structures.

Abstract:Bradykinesia is defined as a "complex" of motor alterations including decreased movement amplitude and/or speed and tendency to reduce them with movement repetition (sequence effect). This study aimed at investigating the neural and kinematic correlates of bradykinesia during hand-tapping in people with Parkinson's disease (pwPD) relative to healthy controls. Twenty-five pwPD and 25 age- and sex-matched healthy controls underwent brain functional MRI (fMRI) during a hand-tapping task: subjects alternatively opened and closed their right hand as fully and quickly as possible. Hand-tapping kinematic parameters were objectively measured during the fMRI task using an optical fibre glove. During the fMRI task, pwPD showed reduced hand-tapping amplitude (hypokinesia) and a greater sequence effect. PwPD relative to healthy controls showed a reduced activity of fronto-parietal areas, middle cingulum/supplementary motor area (SMA), parahippocampus, pallidum/thalamus and motor cerebellar areas. Moreover, pwPD showed an increased activity of brain cognitive areas such as superior temporal gyrus, posterior cingulum, and cerebellum crus I. The decreased activity of cerebellum IV-V-VI, vermis IV-V, inferior frontal gyrus, and cingulum/SMA correlated with hypokinesia and with the sequence effect. Interestingly, a reduced activity of areas involved in motor planning and timing correlated both with hypokinesia and with the sequence effect in pwPD. This study has the major strength of collecting objective motor parameters and brain activity simultaneously, providing a unique opportunity to investigate the neural correlates of the "bradykinesia complex".

Abstract:There is no sense organ specifically dedicated to time perception, as there is for other senses such as hearing and vision. However, this subjective sense of time is fundamental to our conception of reality and it creates the temporal course of events in our lives. Here, we explored neurobiological relations from the clinical perspective, examining timing ability in patients with different neurological and psychiatric conditions (e.g. Parkinson's disease, depression, bipolar disorder, anxiety disorders and schizophrenia). The neural bases of present distortions in time perception and temporal information processing still remain poorly understood. We reviewed: a) how the brain is capable of encoding time in different environments and multiple tasks, b) different models of interval timing, c) brain structures and neurotransmitters associated with time perception, d) the relationship between memory and time perception, e) neural mechanisms underlying different theories in neural and mental processes, and f) the relationship between different mental diseases and time perception. Bibliographic research was conducted based on publications over the past thirteen years written in English in the databases Scielo, Pubmed/MEDLINE, ISI Web of Knowledge. The time perceptions research are executed to evaluate time perception in mental diseases and can provide evidence for future clinical applications.

Abstract:Attention allows us to selectively process the vast amount of information with which we are confronted, prioritizing some aspects of information and ignoring others by focusing on a certain location or aspect of the visual scene. Selective attention is guided by two cognitive mechanisms: saliency of the image (bottom up) and endogenous mechanisms (top down). These two mechanisms interact to direct attention and plan eye movements; then, the movement profile is sent to the motor system, which must constantly update the command needed to produce the desired eye movement. A new approach is described here to study how the eye motor control could influence this selection mechanism in clinical behavior: two groups of patients (SCA2 and late onset cerebellar ataxia LOCA) with well-known problems of motor control were studied; patients performed a cognitively demanding task; the results were compared to a stochastic model based on Monte Carlo simulations and a group of healthy subjects. The analytical procedure evaluated some energy functions for understanding the process. The implemented model suggested that patients performed an optimal visual search, reducing intrinsic noise sources. Our findings theorize a strict correlation between the “optimal motor system” and the “optimal stimulus encoders.”

Abstract:Movement disorders, such as Parkinson's disease, essential tremor, and dystonia, are characterized by their predominant motor symptoms, yet diseases causing abnormal movement also encompass several other symptoms, including non-motor symptoms. Here we review recent advances from studies of brain lesions, neuroimaging, and neuromodulation that provide converging evidence on symptom-specific brain networks in movement disorders. Although movement disorders have traditionally been conceptualized as disorders of the basal ganglia, cumulative data from brain lesions causing parkinsonism, tremor and dystonia have now demonstrated that this view is incomplete. Several recent studies have shown that lesions causing a given movement disorder occur in heterogeneous brain locations, but disrupt common brain networks, which appear to be specific to each motor phenotype. In addition, findings from structural and functional neuroimaging in movement disorders have demonstrated that brain abnormalities extend far beyond the brain networks associated with the motor symptoms. In fact, neuroimaging findings in each movement disorder are strongly influenced by the constellation of patients' symptoms that also seem to map to specific networks rather than individual anatomical structures or single neurotransmitters. Finally, observations from deep brain stimulation have demonstrated that clinical changes, including both symptom improvement and side effects, are dependent on the modulation of large-scale networks instead of purely local effects of the neuromodulation. Combined, this multimodal evidence suggests that symptoms in movement disorders arise from distinct brain networks, encouraging multimodal imaging studies to better characterize the underlying symptom-specific mechanisms and individually tailor treatment approaches.

Abstract:Bradykinesia is a behavioral manifestation that contributes to functional dependencies in later life. However, the current state of bradykinesia indexing primarily relies on subjective, time-averaged categorizations of motor deficits, which often yield poor reliability. Herein, we used time-resolved analyses of accelerometer recordings during standardized movements, data-driven factor analyses, and linear mixed effects models (LMEs) to quantitatively characterize general, task- and therapy-specific indices of motor impairment in people with Parkinson's disease (PwP) currently undergoing treatment for bradykinesia. Our results demonstrate that single-trial, accelerometer-based features of finger-tapping and rotational hand movements were significantly modulated by divergent therapeutic regimens. Further, these features corresponded well to current gold standards for symptom monitoring, with more precise predictive capacities of bradykinesia-specific declines achieved when considering kinematic features from diverse movement types together, rather than in isolation. Herein, we report data-driven, sample-specific kinematic profiles of diverse movement types along a continuous spectrum of motor impairment, which importantly, preserves the temporal scale for which biomechanical fluctuations in motor deficits evolve in humans. Therefore, this approach may prove useful for tracking bradykinesia-induced motor decline in aging populations the future.

Abstract:From a review of the anatomical relationships and single unit activity in the components of the basal ganglia related to limb movement, it is concluded that the major outflow from basal ganglia circuits is via the motor cortex (area 4). Recent results of recording from area 4 neurons revealed that they preferentially "encode" the higher derivatives of movement, i.e. acceleration and jerk. In the parkinsonian (PK) patient and in the monkeys treated with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP), EMG responses to imposed loads show a markedly increased gain of the "M2" component which depends upon the integrity of area 4 and which correlates with the severity of PK rigidity. The above observations are considered, along with those of others (demonstrating prolonged movement times, a decreased "repertoire" of voluntary movements fractionation of voluntary movements, inability in tracking movements without visual input, and failure to improve performance in PK's) in relation to a model of the interactions between sensory input and motor programs. Using this model, it is hypothesized that the above PK movement deficits, as well as rigidity, can be accounted for by abnormal processing of the mechanoreceptor sensory input utilized in the generation and execution of movements. The MPTP treated monkey is suggested as a model in which to directly test the hypothesis.

Abstract:The ability to produce high movement speeds is a crucial factor in human motor performance, from the skilled athlete to someone avoiding a fall. Despite this relevance, there remains a lack of both an integrative brain-to-behavior analysis of these movements and applied studies linking the known dependence on open-loop, central control mechanisms of these movements to their real-world implications, whether in the sports, performance arts, or occupational setting. In this review, we cover factors associated with the planning and performance of fast limb movements, from the generation of the motor command in the brain to the observed motor output. At each level (supraspinal, peripheral, and motor output), the influencing factors are presented and the changes brought by training and fatigue are discussed. The existing evidence of more applied studies relevant to practical aspects of human performance is also discussed. Inconsistencies in the existing literature both in the definitions and findings are highlighted, along with suggestions for further studies on the topic of fast limb movement control. The current heterogeneity in what is considered a fast movement and in experimental protocols makes it difficult to compare findings in the existing literature. We identified the role of the cerebellum in movement prediction and of surround inhibition in motor slowing, as well as the effects of fatigue and training on central motor control, as possible avenues for further research, especially in performance-driven populations.
