Abstract:Background: Although non-alcoholic fatty liver disease (NAFLD) has been shown to be a multisystem disease complicated with extrahepatic malignancies, there is limited data on the association between NAFLD and breast cancer. Here, we conducted a retrospective cohort study to investigate the relationship between NAFLD, and the perioperative and short-term outcomes of breast cancer in Chinese population. Methods: A total of 140 patients with breast cancer who underwent mastectomy at our center in 2016 were enrolled, including 70 patients with NAFLD and 70 patients without NAFLD. NAFLD was diagnosed mainly on the basis of ultrasonography, and the severity was graded by noninvasive indicators. Breast cancer was diagnosed by postoperative pathology; its perioperative outcomes were assessed based on the duration of hospitalization, cost, surgery-related factors, and perioperative complications, while the short-term prognosis was assessed based on short-term metastasis or recurrence. Results: Among patients with breast cancer who underwent mastectomy, those with NAFLD had a longer duration of hospitalization (6.0 vs. 5.0 days, p = 0.014) and incurred higher medicine cost (4470.0 vs. 3202.2 yuan, p = 0.011) than those of patients without NAFLD. Additionally, patients with NAFLD had a longer duration of operation (2.0 vs. 1.5 hours, p = 0.002) and experienced more blood loss (20.0 vs. 20.0 mL, p = 0.015) than patients without NAFLD did. There was no significant difference in the perioperative complications and short-term metastasis or recurrence between the groups. The effect of NAFLD was significant in smaller tumors, infiltrating ductal and lobular carcinomas, and moderately differentiated tumors. The perioperative outcomes worsened with increase in the severity of NAFLD.
