Abstract:Catch crop candidates (corn, guinea grass) for recovering nutrients from farm soil and aquatic plants (water caltrop, water hyacinth) were utilized to produce l-lactic acid. The efficiencies of pre-treatment methods for enzymatic saccharification and l-lactate production of two fermentation processes, thermophilic simultaneous saccharification and fermentation (SSF), as well as separate saccharification and fermentation, were compared. Conditions were set at 55°C and pH 5.5 for non-sterile fermentation. Alkaline/peroxide pre-treatment proved the most effective for saccharification in pre-treated corn, guinea grass, water caltrop and water hyacinth with glucose yields of 0.23, 0.20, 0.11 and 0.14 g/g-dry native biomass (24-hour incubation period), respectively. Examination of the two types of thermophilic l-lactate fermentation employed following alkaline/peroxide pre-treatment and saccharification demonstrated that the l-lactate yield obtained using SSF (0.15 g/g in the case of corn) was lower than that obtained using separate saccharification and fermentation (0.28 g/g in the case of corn). The lower yield obtained from SSF is likely to have resulted from the saccharification conditions used in the present study, as the possibility of cellulase deactivation during SSF by thermophilic l-lactate producing bacteria existed. A cellulase that retains high activity levels under non-sterile conditions and a l-lactate producer without cellulose hydrolysis activity would be required in order for SSF to serve as an effective method of l-lactate production.

Abstract:BackgroundIrreversible electroporation (IRE), a nonthermal injury ablation technique, has been shown to be effective and safe in various organs, such as in the kidney, liver, prostate, or in pancreas. In contrast to radiofrequency or microwave ablation, IRE is also effective in the neighborhood of major vessels. Many human cancers reveal lymphatic spread. The present study aimed to evaluate technical feasibility and safety of IRE in lymphatic tissue. To our knowledge, this is the first report showing successful IRE of lymph nodes in a standardized porcine survival model.MethodsA total of ten pigs were divided into two study groups. Five animals received ECG-gated IRE of mesenteric lymph nodes of the small bowel and were sacrificed 2 h after ablation. Another five animals were followed up for 7 days. Clinical parameters, laboratory and abdominal imaging by contrast-enhanced computed tomography, as well as histology were obtained from all animals at different time points.ResultsDuring and after IRE ablation, no cardiocirculatory side effects were noted in any of the animals. In the acute phase experiments, no damage to adjacent organs and no thermal injuries were seen following IRE. One hundred twenty minutes after ablation, no significant laboratory changes were observed. In the survival group, all animals recovered quickly and showed normal activity and feeding habits indicating a minimal pain level. Seven days after IRE ablation, a significant increase in white blood cell count was observed, while creatinine, urea, or hemoglobin remained unchanged. Computed tomography revealed a hypodense lesion following IRE already at 2 h. Histopathology showed coagulation necrosis of the treated lymph nodes with preservation of the lymph node capsule.ConclusionsThis porcine survival model shows that IRE can safely and effectively be performed in lymph nodes. Thus, IRE might display a novel approach for therapy of lymph node metastasis. Further clinical studies are needed to evaluate the oncologic outcome of IRE ablation in lymph node metastasis.

Abstract:OBJECTIVES The study explores whether conduct-disordered children differ from healthy children in reference to their moral development and to what extent cognitive and education factors, respectively psychosocial stress factors mediate the level of socio-moral development. METHODS Sixteen boys aged nine to fourteen years with an ICD-10 diagnosis of Conduct Disorder were compared to sixteen age-matched healthy controls. The level of socio-moral maturity was assessed by means of the German version of the Sociomoral Reflection Measure (Gibbs et al., 1992). RESULTS Results show that conduct-disordered boys tended to differ from their healthy counterparts in terms of the level of socio-moral maturity of judgement. According to the theory of Gibbs and co-workers, the moral judgement of healthy children (characterized by a pro-social and mutually moral attitude) is more mature than that of conduct-disordered boys. While the latter remain at an interim level between immature and mature socio-moral reasoning, the former adhere to more rational, exchange-oriented morals. The factors "intelligence" and "maternal support" exercise a decisive influence on socio-moral development. CONCLUSIONS It should be investigated whether or not the current results can be generalized in a larger sample.
Feasibility of Computed Tomography-Guided Irreversible Electroporation for Porcine Kidney Ablation.

Abstract:OBJECTIVE:The objective was to evaluate the feasibility and safety of computed tomography (CT)-guided percutaneous irreversible electroporation (IRE) in porcine kidneys.MATERIALS AND METHODS:Under CT guidance, two monopole probes were used to precisely puncture through the renal parenchyma into the renal hilum in nine anesthetized adult Bama miniature pigs. After which, IRE ablation was performed. Biochemical and pathological examinations were carried out 2 h, 2, 7, and 14 days after the procedure.RESULTS:All procedures were performed successfully without any serious complications such as bleeding, infection, or death. All pigs survived until the end of the study. Pathological examinations showed that cells in the ablation area were dead within 2 days after the procedure, whereas the vascular endothelium showed only slight damage. After 2 days, endothelialization ensued and regrowth of smooth muscle cells was observed after 14 days. Hemogram tests indicated a transient increase but gradually returned to baseline levels 14 days after the procedure.CONCLUSION:IRE was essentially safe, however further studies on tumor ablation using several different animal models are needed.
Safety of Irreversible Electroporation Ablation of the Pancreas

Abstract:OBJECTIVES: To evaluate the safety of irreversible electroporation (IRE) on swine pancreatic tissue including its effects on peripancreatic vessels, bile ducts, and bowel.METHODS: Eighteen Yorkshire pigs underwent IRE ablation of the pancreas successfully and without clinical complications. Contrast-enhanced computed tomography angiography and laboratory studies before the IRE ablation with follow-up computed tomography angiography, laboratory testing, and pathological examination up to 4 weeks postablation were performed.RESULTS: In a subset of cases, anatomic peripancreatic vessel narrowing was seen by 1 week postablation, persisting at 4 weeks postablation, without apparent functional impairment of blood flow. Laboratory studies revealed elevated amylase and lipase at 24 hours post-IRE, suggestive of acute pancreatitis, which normalized by 4 weeks post-IRE. There was extensive pancreatic tissue damage 24 hours after IRE with infiltration of immune cells, which was gradually replaced by fibrotic tissue. Ductal regeneration without loss of pancreatic acinar tissue and glandular function was observed at 1 and 4 weeks postablation.CONCLUSIONS: In our study, we demonstrated and confirmed the safety and minimal complications of IRE ablation in the pancreas and its surrounding vital structures. These results show the potential of IRE as an alternative treatment modality in patients with pancreatic cancer, especially those with locally advanced disease.

Abstract:Irreversible electroporation (IRE) with microsecond-pulsed electric fields (μsPEFs) can effectively ablate hepatocellular carcinomas in animal models. This preclinical study evaluates the feasibility and safety of IRE on porcine livers. Altogether, 10 pigs were included. Computed tomography (CT) was used to guide two-needle electrodes that were inserted near the hilus hepatis and gall bladder. Animals were followed-up at 2 hours and at 2, 7 and 14 days post-treatment. During and after μsPEF ablation, electrocardiographs found no cardiovascular events, and contrast CT found no portal vein thrombosis. There was necrosis in the ablation zone. Mild cystic oedema around the gall bladder was found 2 hours post-treatment. Pathological studies showed extensive cell death. There was no large vessel damage, but there was mild endothelial damage in some small vessels. Follow-up liver function tests and routine blood tests showed immediate liver function damage and recovery from the damage, which correlated to the pathological changes. These results indicate that μsPEF ablation affects liver tissue and is less effective in vessels, which enable μsPEFs to ablate central tumour lesions close to the hilus hepatis and near large vessels and bile ducts, removing some of the limitations and contraindications of conventional thermal ablation.

Abstract:BACKGROUND Irreversible electroporation (IRE) is a local non-thermal ablative technique which has been suggested as a potential cancer therapy. However, the specific anatomic characteristics of the pancreatic head make it challenging to perform any local ablation in this region. Therefore, the safety and feasibility of IRE in the pancreatic head region should be further explored. AIM To evaluate the safety of IRE in pancreatic head region including its effects on pancreatic ducts, vessels, and adjacent gastrointestinal organs. METHODS Eight landrace miniature pigs underwent IRE of pancreatic head tissue successfully, with a total of 16 lesions created. Laboratory testing including white blood cell (WBC) count and serum amylase before IRE with follow-up laboratory analysis and pathological examination at 1, 7, 14, and 28 d postablation were performed. RESULTS All pigs tolerated the ablation procedure without serious perioperative complications. Transiently elevated WBC count and amylase were observed at 24 h post-IRE, suggesting an acute pancreatic tissue damage which was confirmed by pathological observations. Vascular endothelial cells and pancreatic duct epithelial cells in ablation zone were also positive in terminal deoxynucleotidyl transferase dUTP nick end labeling staining. There was extensive duodenum mucosa damage with local hemorrhage 24 h after ablation, while regeneration of new villous structures were observed at 7 and 28 d post-IRE. Masson's trichromatic staining showed that the extracellular matrix was still intact in vessels and pancreatic ducts, and even in the duodenum. CONCLUSION IRE ablation to the pancreatic head may be safe and feasible without long-term damage to the surrounding vital structures. However, risks of stress injuries in acute phase should be taken into consideration to prevent severe perioperative complications. Abstract:OBJECTIVES: Use of thermal tumor ablation in the pancreatic parenchyma is limited because of the risk of pancreatitis, pancreatic fistula, or hemorrhage. This study aimed to evaluate the feasibility and safety of irreversible electroporation (IRE) in a porcine model.METHODS: Ten pigs were divided into 2 study groups. In the first group, animals received IRE of the pancreatic tail and were killed after 60 minutes. In the second group, animals received IRE at the head of the pancreas and were followed up for 7 days. Clinical parameters, computed tomography imaging, laboratory results, and histology were obtained.RESULTS: All animals survived IRE ablation, and no cardiac adverse effects were noted. Sixty minutes after IRE, a hypodense lesion on computed tomography imaging indicated the ablation zone. None of the animals developed clinical signs of acute pancreatitis. Only small amounts of ascites fluid, with a transient increase in amylase and lipase levels, were observed, indicating that no pancreatic fistula occurred.CONCLUSIONS: This porcine model shows that IRE is feasible and safe in the pancreatic parenchyma. Computed tomography imaging reveals significant changes at 60 minutes after IRE and therefore might serve as an early indicator of therapeutic success. Clinical studies are needed to evaluate the efficacy of IRE in pancreatic cancer.

Different Schedules of Granulocyte Growth Factor Support for Patients With Breast Cancer Receiving Adjuvant Dose-Dense Chemotherapy: A Prospective Nonrandomized Study

Abstract:PurposeThis prospective, nonrandomized study evaluates 4 schedules of granulocyte colony-stimulating factor (G-CSF) for patients with breast cancer receiving adjuvant dose-dense chemotherapy regarding febrile neutropenia, treatment delays, and costs. Patients and MethodsTwo hundred and thirty-one patients were enrolled to receive adjuvant dose-dense chemotherapy with 4 G-CSF schedules: filgrastim (300 mcg) days 3 to 10 [n = 84 (36.4%) group A]; days 3 to 7 [n = 26 (11.3%) group B]; days 5, 7, 9, and 11 [n = 64 (27.7%) group C], or pegfilgrastim (6 mg) on day 2 [n=57 (24.6%) group D]. ResultsThirteen patients were hospitalized due to 14 episodes of febrile neutropenia; 3 in group A, 3 in group B, 1 in group C, and 6 in group D. No statistically significant difference was observed among the 4 groups. Fewer febrile neutropenic events were observed in group C than in group D (P=0.041). No statistically significant differences were observed in treatment delays or other hematological toxicities. Average overall G-CSF cost per patient in groups A and D was $8500 versus $4400 in groups B and C. ConclusionsWe found a trend in favor of the shorter G-CSF schedule. A larger, prospective randomized trial should be carried out to evaluate shorter versus standard filgrastim and pegfilgrastim schedules with regard to clinical outcomes, hematological and nonhematological toxicities, and impact in costs.
Percutaneous irreversible electroporation for breast tissue and breast cancer: safety, feasibility, skin effects and radiologic–pathologic correlation in an animal study

Abstract:Background To study the safety, feasibility and skin effects of irreversible electroporation (IRE) for breast tissue and breast cancer in animal models. Methods Eight pigs were used in this study. IRE was performed on the left breasts of the pigs with different skin–electrode distances, and the right breasts were used as controls. The electrodes were placed 1–8 mm away from the skin, with an electrode spacing of 1.5–2 cm. Imaging and pathological examinations were performed at specific time points for follow-up evaluation. Vital signs, skin damage, breast tissue changes and ablation efficacy were also closely observed. Eight rabbit models with or without VX2 breast tumor implantations were used to further assess the damage caused by and the repair of thin skin after IRE treatment for breast cancer. Contrast-enhanced ultrasound and elastosonography were used to investigate ablation efficacy and safety. Results During IRE, the color of the pig breast skin reversibly changed. When the skin–electrode distance was 3 mm, the breast skin clearly changed, becoming white in the center and purple in the surrounding region during IRE. One small purulent skin lesion was detected several days after IRE. When the skin–electrode distance was 5–8 mm, the breast skin became red during IRE. However, the skin architecture was normal when evaluated using gross pathology and hematoxylin-eosin staining. When the skin–electrode distance was 1 mm, skin atrophy and yellow glabrescence occurred in the rabbit breasts after IRE. When the skin–electrode distance was ≥5 mm, there was no skin damage in the rabbit model regardless of breast cancer implantation. After IRE, complete ablation of the targeted breast tissue or cancer was confirmed, and apoptosis was detected in the target tissue and outermost epidermal layer. In the ablated breasts of the surviving animals, complete mammary regeneration with normal skin and hair was observed. Furthermore, no massive fibrosis or mass formation were detected on ultrasound or through hematoxylin–eosin staining. Conclusions After IRE, the skin architecture was well preserved when the skin–electrode distance was ≥5 mm. Moreover, breast regeneration occurred without mass formation or obvious fibrosis.
Integrative Omics Analyses Reveal Molecular Changes of Irreversible Electroporation on Pancreatic cancer in Mice

Abstract:Abstract Purpose Irreversible electroporation (IRE) is a local ablative technique with encouraging results in treatment of pancreatic cancer. However, the high recurrence rates make adjuvant therapy necessary. To develop effective adjuvant therapies with IRE, we aimed to analyze the molecular changes of pancreatic cancer after IRE.. Methods KPC cell line was used to set subcutaneous or orthotopic pancreatic cancer mouse models. Both models were randomized to either sham surgery and IRE, and named control group or IRE group, respectively. The subcutaneous tumor models were used to evaluate IRE efficacy by recording the tumor volume and survival time. Orthotopic tumor models were employed to analyze the molecular changes of tumor tissues using transcriptomics, proteomics, and single-cell RNA sequencing (scRNA-seq) a day after IRE. Results IRE transiently inhibited tumor growth and prolonged the survival time of subcutaneous tumor models. Transcriptomics and proteomics analyses showed that T helper 1 (Th1) and T helper 2 (Th2) cell differentiation as well as T helper 17 (Th17) cell differentiation pathways were most significantly downregulated in the IRE group compared with the control group, indicating that IRE significantly affected CD4 + T cells. Furthermore, scRNA-seq revealed that the proportion of CD4 + effector T cell of CD4 + T cells decreased significantly, whereas the proportions of CD4 + naive T cells and regulatory T cell (Tregs) increased significantly in the IRE group. Gene set enrichment analysis revealed that tumor necrosis factor (TNF) and interleukin (IL)-17 signaling pathways of CD4 + effector T cells, and the IL-17 signaling pathway of CD4 + naive cells were significantly downregulated after IRE. Conclusions IRE can effectively treat pancreatic cancer in mice and significantly altered the distribution and functions of CD4 + T cells in pancreatic cancer.

Irreversible electroporation (IRE) is a non-thermal ablation technique that could allow ablation in the pancreas while preserving vital surrounding blood vessels. The aim of this study was to assess the safety and ablation volume of IRE in porcine pancreatic tissue.METHODS: IRE of swine pancreases was performed using an open technique and ultrasound guidance and the animals were followed for 72 hr, 7 days, and 14 days post-IRE.RESULTS: All pigs underwent IRE, with one pig was unsuccessful after three attempts using at 3,000 V due to inability to achieve a stable current. All animals recovered well and revealed only mild adhesions, no ascites, and no pancreatic necrosis. All animals had transient increases in amylase and lipase that normalized on post-IRE day 3. Pathologic analysis revealed that ablation defects (median size 3 cm × 2.8 cm) were seen in electroporated areas with significant destruction of the pancreatic tissue with patent vascular structures.CONCLUSIONS: This animal model demonstrates that IRE of the pancreas performed at an optimal voltage is well tolerated, with rapid resolution of pancreatic inflammation and preservation of vascular structures.

Abstract:Irreversible electroporation (IRE) is a method of non-thermal focal tissue ablation characterized by irreversibly permeabilizing the cell membranes while preserving the extracellular matrix. This study aimed to investigate tissue remodeling after IRE in a porcine model, especially focusing on the extracellular matrix and hepatic stellate cells. IRE ablation was performed on 11 female pigs at 2,000 V/cm electric field strength using a versatile high-voltage generator and 3 cm diameter parallel-plate electrodes. The treated lobes were removed during surgery at 1, 3, 7, 14, and 21 days after IRE. Tissue remodeling and regeneration were assessed by histopathology and immunohistochemistry. Throughout the treated area, IRE led to extensive necrosis with intact collagenous structures evident until day 1. From then on, the necrosis progressively diminished while reparative tissue gradually increased. During this process, the reticulin framework and the septal fibrillar collagen remained in the necrotic foci until they were invaded by the reparative tissue. The reparative tissue was characterized by a massive proliferation of myofibroblast-like cells accompanied by a complete disorganization of the extracellular matrix with the disappearance of hepatic architecture. Hepatic stellate cell markers were associated with the proliferation of myofibroblast-like cells and the reorganization of the extracellular matrix. Between 2 and 3 weeks after IRE, the lobular architecture was almost completely regenerated. The events described in the present study show that IRE may be a valid model to study the mechanisms underlying liver regeneration after extensive acute injury.

Percutaneous ultrasound-guided irreversible electroporation: A goat liver study

Abstract:Irreversible electroporation (IRE) is a new tumor ablation technique. Pulsed electric fields (PEFs) with permanent duration (100 μsec) permanently permeabilize the cell membrane, causing the formation of innumerable permanent nanopores in the cell membrane and leading to cell death. In this study, percutaneous IRE was performed on 24 goat livers under the guidance of ultrasonography (US). IRE-exposed tissues were pathologically examined and glucose-6-phosphatase (G-6-P) and succinodehydrogenase (SDH) staining were used to detect the activity and function of the endoplasmic reticulum and mitochondria of liver tissues at 0 and 24 h after IRE ablation. Tissue ablation responses were monitored in real-time with US in the percutaneous IRE group; the largest diameter of the ablation zones was measured immediately (D1) and after 24 h (D2). Following D2 measurement, the animals were sacrificed and the gross sections (D3) were measured. The pathological examination results showed complete tissue necrosis after 24 h instead of immediately following IRE. The largest long diameters measured by intraprocedural US immediately after IRE (D1, 39.58±2.13 mm) were larger than those measured by US after 24 h (D2, 37.07±3.51 mm) and in gross section measurements (D3, 36.44±2.04 mm; P<0.05). D1 showed a good linear correlation with D3 (r=0.949). We conclude from these studies that IRE is not an acute ablating effect which leads to cell death. If US-guided percutaneous IRE focused on the target liver areas accurately, physicians would be able to assess the extent of necrosis through the regression equation during the IRE ablation procedure, and evaluate whether sufficient electric field energy had been applied to the desired tissue. Assisted with US guiding and monitoring, the minimally invasive IRE procedure in intraperitoneal lesions may become an important tumor ablation technique.
Histological analysis of human pancreatic carcinoma following irreversible electroporation in a nude mouse model

Abstract:AIM: To determine changes in the morphology and function of pancreatic cancer cells after irreversible electroporation (IRE) treatment, and to explore the clinical significance of IRE treatment for pancreatic cancer providing an experimental basis for the clinical application of IRE treatment.METHODS: IRE was carried out in an athymic nude mouse model of pancreatic carcinoma generated with human pancreatic cancer cells 1. In therapy groups, IRE electrodes were inserted with 90 pulses per second at 800 V/cm applied to ablate the targeted tumor tissues. Histological assessment of the affected tissue was performed by hematoxylin and eosin staining (HE). Quantification of cell proliferation and apoptosis was performed by evaluating Ki67 and caspase-3 levels, respectively. Flow cytometry was used to assess cell apoptosis. Ultrasound imaging was carried out to evaluate IRE treatment results. Pathological correlation studies showed IRE is effective for the targeted ablation of pancreatic tumors in an orthotopic mouse model.RESULTS: IRE was efficacious in removing tumors in the orthotopic mouse model. The IRE-ablated zone displays characteristics of nude mouse models at different time-points as assessed by hematoxylin and eosin staining. Immunohistochemical analysis of samples from the pancreatic cancer models showed significantly enhanced caspase-3 cleavage and Ki67. Flow cytometry data corroborated the above findings that apoptosis in tumor cells was observed immediately on the first postoperative day, and with time the middle and late stages of apoptosis were observed. For ultrasound imaging studies, the IRE ablation zone became a hyperechoic area due to increasing inflammatory and immunologic cellular contents.CONCLUSION: IRE is a promising new approach for pancreatic cancer, with many potential advantages over conventional ablation techniques.

Abstract:Irreversible electroporation is a local, non-thermal ablation method, where short electrical pulses of high voltage lead to changes in cell membrane permeability and cell death. Recent experimental studies have shown that it does not lead to damage of blood vessels, nerves, bile duct or ureters. The aim of our experimental study was to evaluate the negative effect of irreversible electroporation regarding damage to the vascular wall and porcine pancreatic tissue. Irreversible electroporation of the pancreas was performed in 6 pigs after medial laparotomy. Irreversible electroporation was applied to each pig to the splenic lobe of the pancreas in order to assess damage to the pancreatic tissue and to the duodenal lobe of the pancreas to assess damage to the vascular structure of the pancreatic tissue. Higher ablation electric intensity (minimum 500 V/cm – maximum 1,750 V/cm, step 250 V/cm) in 90 μs pulses was utilized on each pig. After 7 days, macroscopic and microscopic evaluations of en bloc resected specimen (pancreas with duodenum) were performed. During 7 post-ablation days, no deaths or clinical worsening occurred in any of the pigs. Necrotic changes in the pancreatic tissue were recorded at an electric intensity of 750 V/cm. Changes in the outer layers of the wall of the arteries and veins occurred at 1,000 V/cm. Transmural vascular wall damage was not recorded in any case. Irreversible electroporation allows for relatively efficient cell death in the target tissues. Our independent experimental work confirms the safety of this method towards vascular structures located in the ablation zone.
