Abstract:Rosacea is a non-infectious dermatological disease of middle age, requiring long-term and often lifelong treatment. The onset of the disease begins with minor clinical manifestations such as centrofacial transient erythema or conjunctivitis, which patients often do not consider as a need to consult a doctor. Early therapy prevents the chronization of the process and the development of severe forms, which include papulo-pustular rosacea, ophthalmosacea and phymes. One of the methods of rosacea treatment is physiotherapy, which includes laser light, cryotherapy, darsonvalization, electrophoresis, electrocoagulation, pulse therapy, inductothermy, phototherapy of vascular pathology, and others. This review presents the main mechanisms of laser light action on human skin and systematizes recent data on its use in various forms of rosacea. Despite the moderate success of laser therapy, further research will help to choose more effective treatment protocols for rosacea. The appearance of the first signs of vasodilation in the facial area, especially in people with a family history of rosacea, frequent eye diseases or the presence of fim, requires contacting a dermatologist.

Abstract:Although extremely computer literate, today's engineering students don't enter college with the same level of hands-on "tinkering" with hardware that prior generations exhibited. Gone are the days when students were ham radio operators, played with Erector sets and had tinkered extensively with electronic kits or simply taken things apart. As a result, students have less "gut intuition" than prior generations possessed when entering the job market. Lab experimentation provides a sense of where things deviate from theory, offering the opportunity to explore non-ideal conditions; while also giving students the chance to play with hardware and gain the experience that helps them support their subsequent design courses. Hands-on, intellectually engaging studio course delivery was a revolutionary idea that has improved the quality of education at Rensselaer over the past 12 years and has since been adopted, in various forms, by many other universities. Yet even with more engaging studio environments, student learning is still impeded by space constraints, insufficient time for laboratory activities (particularly to do the in-depth probing that leads to an intuitive feel for system design) and poorly designed equipment that takes up a great deal of space. Furthermore, the equipment sets can't be brought home for individual study, thus limiting the time for hands-on exploration that students need to grasp the "big ideas" in engineering

Abstract:Rosacea is a common inflammatory skin disease characterized by erythema, episodes of flushing and inflammatory lesions. It typically affects the face and is more prevalent among fair skin individuals affecting women more than men. Various treatments are available for rosacea with light‐based therapies commonly used in the management of erythema.The use of intradermal botulinum toxin type‐A has been reported to be beneficial in the treatment of rosacea‐associated erythema and flushing with good results and a low side‐effect profile. In this article we present our experience on the successful combination of both pulsed dye laser and intradermal botulinum toxin type‐A in erythema and flushing in 20 rosacea patients. In addition to subjective improvement we measured the degree of erythema using a 3D Antera™ camera in order to quantify our results.We demonstrated high efficacy and satisfaction rate with this combined approach and a low side‐effect profile. To our knowledge the combination of laser and intradermal botulinum toxin in the management of rosacea has not been previously reported.This article is protected by copyright. All rights reserved.

Abstract:Phototechnologies occupy a leading position in the treatment of patients with rosacea. The most effective technologies are laser: pulsed dye laser (585nm, 595nm) and neodymium laser (1064nm), however, issues of comparative analysis, including in patients with a combination of rosacea subtypes, require further study. Material and methods. We observed 64 patients with diagnosed rosacea with a combination of subtypes: erythematotelangiectatic and papulopustular. Among them, 48 (75%) were female and 16 (25%) were male. The age of the patients varied widely: from 31 to 57 years, the average age was 39.2±4.6 years. The average duration of the disease was 5.1±2.3 years. Depending on the therapy, patients were divided into 2 groups: group 1 received treatment with a neodymium laser, group 2 – treatment with a pulsed dye laser. Research results. A cumulative assessment taking into account clinical research methods with a sufficient degree of reliability revealed the advantages of a neodymium laser over a pulsed dye laser, regardless of the clinical picture of rosacea, which was confirmed by the dynamics of the IGA/PGA indices: reduction in group 1 was 79.5%/66.7%, in group 2–63.2%/51.3%, respectively. These data were confirmed by the dynamics of the DIQI index – 81.7% and 61.6%, respectively.

Abstract:Rosacea is a chronic cutaneous disorder affecting primarily the face, characterized by erythema, transient or persistent, telangiectasia, and inflammatory lesions including papulo-pustules and swelling. The essential component of the disease is the persistent erythema of facial skin. Episodes of flushing (acute-subacute intermittent vasodilation) are common. Swelling and erythema of the nose along with dilatation of the pilosebaceous poral orifices, known as rhinophyma, can be noted in chronic cases. Rosacea affects up to 10% of the world population and is especially noted in fair-skinned individuals aged 35-50. Women are affected more often than men. Several treatment modalities including topical medications, systemic drugs, lasers, and light-based therapies have been used for the management of rosacea with variable results. Topical medications such as azelaic acid, metronidazole, and sulfacetamide/sulfur, oral antibiotics such as tetracyclines, and oral retinoids alone or, most commonly, in combination form the mainstay of treatment. Light therapies such as intense pulsed light and pulsed dye laser are best used for the erythemato-telangiectatic type. Topical brimonidine, oxymetazoline, ivermectin, tacrolimus, pimecrolimus, low-dose modified-release tetracyclines and botulinum toxin are the new additions to the therapeutic armamentarium. This article provides a comprehensive review of the various therapies used for rosacea.
Comparative efficacy of short-pulsed intense pulsed light and pulsed dye laser to treat rosacea

Abstract:Background: Laser and light-based therapies have often been used successfully to treat rosacea. Recently, short-pulsed intense pulsed light (IPL) that emitted pulse durations down to 0.5 ms was found to be effective for rosacea treatment. Objective: This study evaluated the efficacy of short-pulsed IPL in the treatment of rosacea compared with pulsed dye laser (PDL) using same pulse duration and fluence. Materials and Methods: Nine patients with rosacea were enrolled in a randomized, split-face trial. Each treatment consisted of four sessions at three-week intervals and followed up until three weeks after the last treatment. Efficacy was assessed by erythema, melanin index, physician's subjective evaluation, and patient's satisfaction. Results: The mean change in erythema index was -4.93 ± 1.59 for the short-pulsed IPL group and -4.27 ± 1.23 for the PDL group. The mean change in melanin index was -2.52 ± 2.45 for the short-pulsed IPL group and -1.95 ± 1.41 for the PDL group. There was no significant difference in either melanin or erythema index between short-pulsed IPL and PDL treatments, and there were no noticeable adverse events. Conclusions: There was no significant difference between PDL and short-pulsed IPL treatment using the same energies and pulse. Both PDL and short-pulsed IPL were satisfactory and safe for rosacea treatment.

Abstract:Rosacea is one of the most common dermatoses of adults. In recent years many studies have contributed to a better understanding of the pathophysiology of rosacea. They suggest that an altered innate immune response is involved in the vascular and inflammatory manifestations seen in rosacea. A good understanding of the disease and its special features is necessary for the differential diagnosis of the many clinical subtypes and for a stage- and phase-specific treatment approach. Topical treatments that are widely accepted are metronidazole and azelaic acid; agents under investigation that show promise include permethrin, calcineurin inhibitors and sulfur compounds. For systemic therapy antibiotics (tetracyclines, macrolides) and recently doxycycline in anti-inflammatory rather than anti-microbial dosages are used, as well as isotretinoin in severe cases. Findings such as rhinophyma and telangiectases can be treated using different laser systems or dermabrasion. This article gives an overview regarding rosacea, a challenging condition with multiple therapeutic options.

Abstract:The formation of collagen fibrils from staggered repeats of individual molecules has become "accepted" wisdom. However, for over thirty years now, such a model has failed to resolve several structural and functional questions. In a novel approach, it was found, using atomic force microscopy, that tendon collagen fibrils are composed of subcomponents in a spiral disposition-that is, their structure is similar to that of macroscale ropes. Consequently, this arrangement was modeled and confirmed using elastic rod theory. This work provides new insight into collagen fibril structure and will have wide application-from the design of scaffolds for tissue engineering and a better understanding of pathogenesis of diseases of bone and tendon, to the conservation of irreplaceable parchment-based museum exhibits.

Abstract:Rosacea is a chronic inflammatory dermatosis mainly affecting the cheeks, nose, chin, and forehead. Rosacea is characterized by recurrent episodes of flushing or transient erythema, persistent erythema, phymatous changes, papules, pustules, and telangiectasia. The eyes may also be involved. Due to rosacea affecting the face, it has a profound negative impact on quality of life, self-esteem, and well-being. In addition to general skin care, there are several approved treatment options available for addressing these features, both topical and systemic. For some features, intense pulse light, laser, and surgery are of value. Recent advances in fundamental scientific research have underscored the roles of the innate and adaptive immune systems as well as neurovascular dysregulation underlying the spectrum of clinical features of rosacea. Endogenous and exogenous stimuli may initiate and aggravate several pathways in patients with rosacea. This review covers the new phenotype-based diagnosis and classification system reflecting pathophysiology, and new and emerging treatment options and approaches. We address new topical and systemic formulations, as well as recent evidence on treatment combinations. In addition, ongoing studies investigating novel therapeutic interventions will be summarized.

Abstract:The prevalence of rosacea in skin of color (SOC) populations is estimated to be as high as 10% in some countries. Traditionally, intense pulsed light (IPL) and pulsed dye laser (PDL) have been the laser and energy-based devices (EBDs) used to treat rosacea. However, not all laser and EBDs are safe for SOC (Fitzpatrick skin types IV-VI) due to increased absorption of energy in pigmented skin and increased risk of post-inflammatory hyperpigmentation and scarring. This review summarizes the use of the top seven laser and EBDs for treating rosacea in SOC.

Abstract:Background Rosacea, a chronic inflammatory skin condition, is marked by enduring redness, visible blood vessels, and inflammatory eruptions in facial areas. Managing rosacea remains a persistent challenge for dermatologists, especially in cases unresponsive to conventional treatments. Injectable poly‐d,l‐lactic acid (PDLLA) has shown promise in treating erythema and telangiectasia associated with rosacea in addition to age‐related concerns. Employing Mirajet, a laser‐induced microjet system, for administering PDLLA is a novel and promising treatment for rosacea. Aims We aimed to evaluate the efficacy and safety of injectable PDLLA delivered via a needle‐free microjet system for managing rosacea. Methods Four Korean women with persistent and refractory rosacea received five monthly sessions of PDLLA needle‐free injections. Clinical assessments were conducted using the Clinician's Erythema Assessment and Patient's Self‐Assessment (PSA) at baseline, 4 weeks post‐treatment, and 22 weeks post‐final treatment. Adverse events were monitored throughout the study period. Results At 4 weeks post‐treatment, both Clinician's Erythema Assessment and PSA scores indicated significant improvements in erythema that were sustained up to the 22‐week follow‐up. Patients reported high satisfaction with resolution of redness and improved skin texture. Mild swelling, redness, and petechiae were observed post‐treatment but resolved spontaneously. No product‐related adverse events were noted during the study period. Conclusion Injectable PDLLA delivered via laser‐induced microjet injection demonstrated promising efficacy in improving rosacea symptoms and skin quality for up to 22 weeks without significant adverse effects. Larger randomized controlled trials are needed to confirm these findings and evaluate long‐term safety and sustainability of outcomes.

Abstract:Previous clinical studies have shown that pulsed dye laser (PDL) and intense pulsed light (IPL) are effective for treating erythematotelangiectatic rosacea(ETR). This article aims to compare the efficacy and safety of PDL and IPL at three different wavelength bands (broad-band, single-narrow-band, and dual-narrow-band) in treating ETR. Sixty subjects with ETR were randomly categorized into four groups and received one of the following laser treatments: PDL (595 nm), IPL with Delicate Pulse Light (DPL, 500–600 nm), IPL with M22 590 (590–1200 nm), or IPL with M22 vascular filter (530–650 nm and 900–1200 nm). Four treatment sessions were administered at 4-week intervals, with one follow-up session 4 weeks after the final treatment. The efficacy of the four lasers was evaluated by comparing the clinical symptom score, total effective rate, VISIA red area absolute score, and RosaQoL score before and after treatment. The safety was evaluated by comparing adverse reactions such as pain, purpura, erythematous edema, and blister. All 60 subjects completed the study. Within-group effects showed that the clinical symptom score, VISIA red area absolute score, and RosaQoL score of all four groups were significantly reduced compared to before treatment ( p < 0.001). Between-group effects showed no statistically significant difference among the four laser groups. Safety analysis showed that all four lasers were safe, but the incidence of blister was higher in the M22 vascular group. Nonpurpurogenic PDL, DPL, M22 590, and M22 vascular were equally effective in treating ETR and were well-tolerated. ClinicalTrial.gov Identifier: NCT05360251.

Updates on the pathophysiology and management of acne rosacea

Abstract:There are many options for the treatment of acne rosacea, including topical and systemic therapies, laser and light-based therapies, and surgical procedures. A classification system for rosacea identifies 4 subtypes (ie, erythematotelangiectatic, papulopustular, phymatous, and ocular), which may help guide therapeutic decision making. Until recently, the pathophysiology of acne rosacea has been poorly understood and limited to descriptions of factors that exacerbate or improve this disorder. Recent molecular studies suggest that an altered innate immune response is involved in the pathogenesis of the vascular and inflammatory disease seen in patients with rosacea. These findings may help explain the benefits of current treatments and suggest new therapeutic strategies helpful for alleviating this disease. The goals of therapy include reduction of papules, pustules, erythema, physical discomfort, and an improvement in quality of life. Standard topical treatment agents include metronidazole, azelaic acid, and sodium sulfacetamide-sulfur. Second-line therapies include benzoyl peroxide, clindamycin, calcineurin inhibitors, and permethrin. There are also various systemic therapy options.
