Abstract:A method is reported for palladium-catalyzed N-quinolyl carboxamide-directed olefination of the unactivated C(sp(3))-H bonds of phthaloyl alanine with a broad range of vinyl iodides at room temperature. This reaction represents the first example of the stereoretentive installation of multisubstituted terminal and internal olefins onto unactivated C(sp(3))-H bonds. These methods enable access to a wide range of challenging β-vinyl α-amino acid products in a streamlined and controllable fashion, beginning from simple precursors.

Abstract:The scarcity of malignant Hodgkin and Reed-Sternberg cells hampers tissue-based comprehensive genomic profiling of classic Hodgkin lymphoma (cHL). By contrast, liquid biopsies show promise for molecular profiling of cHL due to relatively high circulating tumour DNA (ctDNA) levels1-4. Here we show that the plasma representation of mutations exceeds the bulk tumour representation in most cases, making cHL particularly amenable to noninvasive profiling. Leveraging single-cell transcriptional profiles of cHL tumours, we demonstrate Hodgkin and Reed-Sternberg ctDNA shedding to be shaped by DNASE1L3, whose increased tumour microenvironment-derived expression drives high ctDNA concentrations. Using this insight, we comprehensively profile 366 patients, revealing two distinct cHL genomic subtypes with characteristic clinical and prognostic correlates, as well as distinct transcriptional and immunological profiles. Furthermore, we identify a novel class of truncating IL4R mutations that are dependent on IL-13 signalling and therapeutically targetable with IL-4Rα-blocking antibodies. Finally, using PhasED-seq5, we demonstrate the clinical value of pretreatment and on-treatment ctDNA levels for longitudinally refining cHL risk prediction and for detection of radiographically occult minimal residual disease. Collectively, these results support the utility of noninvasive strategies for genotyping and dynamic monitoring of cHL, as well as capturing molecularly distinct subtypes with diagnostic, prognostic and therapeutic potential.
Mutational profiling of circulating tumor DNA and clinical characteristics in lymphoma: Based on next generation sequencing

Abstract:Liquid biopsy has been experimented with to identify the mutation of lymphoma based on next-generation sequencing (NGS). We applied NGS analysis to circulating tumor DNA (ctDNA) in 20 lymphoma patients. Then, we compared treatment outcomes, and clinical characteristics among these patients, then investigated mutational profiling. Two independent cohorts of 241 patients with mature B cell lymphoma in Mature B-cell malignancies data set (MBN) data set and 50 diffuse large B-cell lymphoma (DLBCL) patients in DLBCL data set, were used to examine the association between gene mutations and prognosis. We found ctDNA positive group had significantly more relapsed/PD (7/12, 58.3%) and less CR/PR patients (1/12, 8.3%) compared to negative group (0, 0%) (5/8, 62.5%) (p < 0.001). Somatic alterations were identified in 12 of 20 patients and the total 11 mutations were: Ataxia telangiectasia mutated (ATM), TP53, BCL2, BTG2, CD28, EP300, IDH2, IRF8, JAK3, NOTCH1, and NRAS. ATM (S2168L) was found in SLL and TLBL for the first time. BTG2 (c.292_293del), CD28 (P119T), IRF8 (E74D) and NOTCH1 (c.4348 G > A) were newly detected in DLBCL, angioimmunoblastic T-cell lymphoma, primary central nervous system lymphoma, and BCL for the first time respectively. We also disclosed an unreported mutation EP300 (c.1058_1059insC) in DLBCL. Our cases implied ctDNA detection consistent with the FISH of tissue samples to some extent, speculating new molecular subtypes of DLBCL, finding some potential drug-resistant mutations, and suggesting disease recurrence. Moreover, in MBN and DLBCL datasets, patients with TP53 mutation had a significantly shorter OS (all p < 0.05) in both circulating free DNA and tumor tissue. The mutations (no SNP) of NOTCH1 (all p < 0.05) significantly contributed to worse OS in the two cohorts.

Abstract:Introduction Seizing the full potential of lymphoma circulating tumor DNA (ctDNA) for scientific discoveries and accurate precision medicine tools requires diminished dependence on tumor tissues, enhanced detection of rare variants and harnessing non-random ctDNA fragmentation patterns for non-invasive profiling. Establishment of such tools is pivotal to overcome clinical challenges set by pervasive heterogeneity and different treatment responses in the patients with large B-cell lymphoma (LBCL). Methods Using our previous data on targeted and whole-genome sequencing (WGS) levels of lymphoma ctDNA (Meriranta, 2022), we constructed a novel targeted sequencing assay with B-cell lymphoma-relevant loci and used duplex barcoding to improve analytical sensitivity. We optimized fragment pattern conservation by comparison of different library preparation strategies and established the requirements for exhaustive and reproducible duplex sequencing with error-rate down to 2-4/10^6. We examined replicate samples and in vitro and in silico dilution series of ctDNA spike-in in healthy donor cell-free DNA (cfDNA) to determine the limits of tumor-naïve variant calling, ctDNA quantification and minimal residual disease (MRD) detection. We applied this assay to >500 plasma, tumor and whole-blood DNA samples from primary LBCL patients treated in biomarker driven Nordic phase II trial (Leppä, 2022) and correlated the results with biological and clinical data (Fig A). Results Ultra-sensitive charting of pretreatment ctDNA revealed profound spatial heterogeneity in LBCL patients at diagnosis. Plasma profiles were formed by contributions from multiple cell subpopulations with divergent hypermutations and driver events, some exhibiting convergent trajectories diverged even at early B-cell stages preceding germinal center entrance. The cell populations represented in the diagnostic biopsies contributed an unpredictable proportion of the overall ctDNA pool and majority (63%) of the subclonal ctDNA mutations were not detected in the matched tissues resulting in conflicting genotypes between different materials. Dominant but not subclonal TP53 mutations in the ctDNA associated with poor survival also uncovering patients with no TP53 aberrations in their diagnostic biopsies. On therapy, allele fractions of lymphoma mutations, regardless of their clonal status, declined with early responding patients being MRD negative after two treatment cycles with excellent survival. At the end-of-therapy, Duplex-MRD predicted progression with excellent specificity (98%) but with limited negative predictive value (90%) and sensitivity (47%). Fragmentomic analysis revealed that duplex-recovery was suboptimal for subnucleosomal cfDNA fragment lengths with the highest mutational content. Remarkably, harnessing cfDNA fragments with only single-strand recovery (i.e. non-duplex) for combined ‘Dual-MRD’ enhanced ctDNA detection, translated to better distinction of early responders and resolved most false-positive end-of-therapy FDG-PET scans (Fig A). Besides enriched for shorter fragment sizes, additional non-random structural features of ctDNA molecules extended to their distinct repertoire of fragment nucleotide end motifs (Fig B). Notably, mutant fragments carried less physiological DNAse1L3 nuclease end signatures than their reference counterparts or healthy donor cfDNA (Fig B). Distinct fragmentomic elements of ctDNA molecules contained unexploited utility for non-invasive disease phenotyping and detection that we spanned to shallow (~10x) WGS breadth in independent validation series of lymphoma and healthy donor samples. Conclusion By harnessing synergy between duplex technology, single-strand recovery and distinct fragmentomic features, we discovered novel characteristics in lymphoma ctDNA that correlate with disease biology and improve non-invasive disease profiling to overcome limitations of tumor biopsies and clinical estimates.

Abstract:Abnormalities of chromosome number, such as the autosomal trisomies and sex chromosome aneuploidies, are considered to be sporadic events with low and constant recurrence risk across populations. On the other hand, abnormalities of chromosome structure can be generated by environmental agents and also transmitted in families and therefore may accumulate in certain populations. Evidence from several geographically diverse newborn infant screening studies and from clinical cytogenetics laboratories (including our own) supports the hypothesis that the frequency of structural abnormalities varies among populations, whereas the frequency of numerical abnormalities remains relatively constant among populations. The data from our laboratory, based on 1,201 patients over a 6-year period, suggest a two- to nearly fourfold higher frequency of structural defects over that of other populations (8.8% vs 4.2% and 2.5% of samples tested). Some of the problems associated with making comparisons among the published data sets are discussed, along with alternative explanations for the variability in the frequency of structural defects reported in different populations.

Abstract:Noninvasive disease monitoring and risk stratification by circulating tumor DNA (ctDNA) profiling has become a potential novel strategy for patient management in B-cell lymphoma. Emerging innovative therapeutic options and an unprecedented growth in our understanding of biological and molecular factors underlying lymphoma heterogeneity have fundamentally increased the need for precision-based tools facilitating personalized and accurate disease profiling and quantification. By capturing the entire mutational landscape of tumors, ctDNA assessment has some decisive advantages over conventional tissue biopsies, which usually target only one single tumor site. Due to its non- or minimal-invasive nature, serial and repeated ctDNA profiling provides a real-time picture of the genetic composition and facilitates quantification of tumor burden any time during the course of the disease. In this review, we present a comprehensive overview of technologies used for ctDNA detection and genotyping in B-cell lymphoma, focusing on pre-analytical and technical requirements, the advantages and limitations of various approaches, and highlight recent advances around improving sensitivity and suppressing technical errors. We broadly review potential applications of ctDNA in clinical practice and for translational research by describing how ctDNA might enhance lymphoma subtype classification, treatment response assessment, outcome prediction, and monitoring of measurable residual disease. We finally discuss how ctDNA could be implemented in prospective clinical trials as a novel surrogate endpoint and be utilized as a decision-making tool to guide lymphoma treatment in the future.

Abstract:PURPOSE Current clinical challenges in Hodgkin lymphoma (HL) include difficult-to-treat relapsed/refractory disease and considerable long-term toxicities of treatment. Since clinical risk factors lack discriminatory power, intensity of therapy is mainly based on tumor burden. Exploring HL genetics and tumor microenvironment (TME) might provide valuable insights for improved risk stratification. MATERIALS AND METHODS In this study, we applied circulating tumor DNA sequencing to 243 patients obtained from pivotal German Hodgkin Study Group trials to identify subtypes of HL. Independent validation of the subtypes was performed in 96 patients treated in the EuroNet-PHL-C2 study. Outcome differences of subtypes were assessed in an event-enriched clinical validation cohort comprising 72 patients from the HD21 trial, using a refined, validated, and clinically feasible assay. RESULTS We propose a biologic classification of HL consisting of three distinct subtypes: inflammatory immune escape HL is characterized by frequent copy-number variations including immune escape variants such as high-level amplifications of the PD-L1 locus and an inflammatory TME. Virally-driven HL is associated with Epstein-Barr virus and/or human herpesvirus 6 and an inflammatory TME with neutrophils and macrophages, while the tumor mutational burden (TMB) is low. Oncogene-driven HL is defined by a high TMB, recurrent mutations in oncogenic drivers such as TNFAIP3 , ITPKB , and SOCS1 , and a cold TME. A refined and validated assay version aiming at clinically feasible risk stratification showed significant progression-free survival differences between subtypes. In addition, assessment of minimal residual disease (MRD) allowed for the detection of patients at very high risk of relapse within the subtypes. CONCLUSION We propose a clinically feasible, noninvasive method for individualized risk stratification and MRD monitoring in patients with HL on the basis of circulating tumor DNA sequencing.

Abstract:Introduction: Analyzing liquid biopsies for tumor-specific aberrations can facilitate detection of measurable residual disease (MRD) during treatment and at follow-up. In this study, we assessed the clinical potential of using whole-genome sequencing (WGS) of lymphomas at diagnosis to identify patient-specific structural (SVs) and single nucleotide variants (SNVs) to enable longitudinal, multi-targeted droplet digital PCR analysis (ddPCR) of cell-free DNA (cfDNA). Methods: In 9 patients with B-cell lymphoma (diffuse large B-cell lymphoma and follicular lymphoma), comprehensive genomic profiling at diagnosis was performed by 30X WGS of paired tumor and normal specimens. Patient-specific multiplex ddPCR (m-ddPCR) assays were designed for simultaneous detection of multiple SNVs, indels and/or SVs, with a detection sensitivity of 0.0025% for SV assays and 0.02% for SNVs/indel assays. M-ddPCR was applied to analyze cfDNA isolated from serially collected plasma at clinically critical timepoints during primary and/or relapse treatment and at follow-up. Results: A total of 164 SNVs/indels were identified by WGS including 30 variants known to be functionally relevant in lymphoma pathogenesis. The most frequently mutated genes included KMT2D, PIM1, SOCS1 and BCL2. WGS analysis further identified recurrent SVs including t(14;18)(q32;q21) (IGH::BCL2), and t(6;14)(p25;q32) (IGH::IRF4). Plasma analysis at diagnosis showed positive circulating tumor DNA (ctDNA) levels in 88% of patients and the ctDNA burden correlated with baseline clinical parameters (LDH and sedimentation rate, p-value <0.01). While clearance of ctDNA levels after primary treatment cycle 1 was observed in 3/6 patients, all patients analyzed at final evaluation of primary treatment showed negative ctDNA, hence correlating with PET-CT imaging. One patient with positive ctDNA at interim also displayed detectable ctDNA (average variant allele frequency (VAF) 6.9%) in the follow-up plasma sample collected 2 years after final evaluation of primary treatment and 25 weeks before clinical manifestation of relapse. Conclusion: In summary, we demonstrate that multi-targeted cfDNA analysis, using a combination of SNVs/indels and SVs candidates identified by WGS analysis, provides a sensitive tool for MRD monitoring and can detect lymphoma relapse earlier than clinical manifestation.

Abstract:Diffuse large B cell lymphoma (DLBCL), the most common lymphoid malignancy in adults, is a clinically and genetically heterogeneous disease that is further classified into transcriptionally defined activated B cell (ABC) and germinal center B cell (GCB) subtypes. We carried out a comprehensive genetic analysis of 304 primary DLBCLs and identified low-frequency alterations, captured recurrent mutations, somatic copy number alterations, and structural variants, and defined coordinate signatures in patients with available outcome data. We integrated these genetic drivers using consensus clustering and identified five robust DLBCL subsets, including a previously unrecognized group of low-risk ABC-DLBCLs of extrafollicular/marginal zone origin; two distinct subsets of GCB-DLBCLs with different outcomes and targetable alterations; and an ABC/GCB-independent group with biallelic inactivation of TP53, CDKN2A loss, and associated genomic instability. The genetic features of the newly characterized subsets, their mutational signatures, and the temporal ordering of identified alterations provide new insights into DLBCL pathogenesis. The coordinate genetic signatures also predict outcome independent of the clinical International Prognostic Index and suggest new combination treatment strategies. More broadly, our results provide a roadmap for an actionable DLBCL classification.
Tracking the evolution of untreated high-intermediate/high-risk diffuse large B-cell lymphoma by circulating tumour DNA

Abstract:Diffuse large B-cell lymphoma (DLBCL) is a highly heterogenous malignancy, early identification of patients for relapse remains challenging. The potential to non-invasively monitor tumour evolutionary dynamics of DLBCL needs to be further established. In the present study, 17 tumour biopsy and 38 plasma samples from 38 patients with high-intermediate/high-risk DLBCL were evaluated at baseline. Longitudinal blood samples were also collected during therapy. Circulating tumour DNA (ctDNA) was analysed using targeted sequencing based on a gene panel via a recently developed methodology, circulating single-molecule amplification and re-sequencing technology (cSMART). We found that the most frequently mutated genes were tumour protein p53 (TP53; 42·1%), histone-lysine N-methyltransferase 2D (KMT2D; 28·9%), caspase recruitment domain family member 11 (CARD11; 21·1%), cAMP response element-binding protein binding protein (CREBBP; 15·8%), β2 -microglobulin (B2M; 15·8%), and tumour necrosis factor alpha-induced protein 3 (TNFAIP3; 15·8%). The mutation profiles between ctDNA and matched tumour tissue showed good concordance; however, more mutation sites were detected in ctDNA samples. Either TP53 or B2M mutations before treatment predicted poor prognosis. Analysis of dynamic blood samples confirmed the utility of ctDNA for the real-time assessment of treatment response and revealed that the increases in ctDNA levels and changes in KMT2D mutation status could be useful predictors of disease progression. Our present results suggest that ctDNA is a promising method for the detection of mutation spectrum and serves as a biomarker for disease monitoring and predicting clinical recurrence.

Abstract:Diffuse large B-cell lymphoma (DLBCL) is the most common aggressive B-cell lymphoma with high clinical and biological heterogeneity. Despite current effective immunochemotherapy, up to 40% of patients do not respond or develop refractory disease. DLBCL is characterized by two major cell-of-origin (COO) subtypes, germinal center B cell-like (GCB) and activated B cell-like (ABC), with recent research uncovering additional molecular subgroups based on genomic alterations or tumor microenvironment (TME) features. However, current classifiers use bulk or deconvolution profiling which blurs intra-tumor heterogeneity, hindering the discovery of true functional cell states underlying clinical diversity. Here, we constructed a multimodal single-cell atlas of 62 DLBCL cases, integrating single-cell transcriptomes, immune repertoire and genetic features, to decrypt recurrent B cell states and TME ecosystems while identifying clinically relevant prognostic biomarkers (Fig.1). We performed simultaneous 5'-end single-cell RNA, B-cell receptor (BCR) and T-cell receptor sequencing on 63 DLBCL lymph node biopsies, recovering 109,294 cells after stringent quality controls. These biopsies were obtained from the real-world, clinically-annotated, multicentric CeVi collection and included 43 patient samples at diagnosis (ABC = 17, GCB = 11, Unclassified = 6, Not Evaluated = 9; 21 of which did not achieve event-free survival at 24 months (EFS24) after R-chemotherapy regimens), 11 relapsed/refractory DLBCL and 8 DLBCL transformed from indolent lymphoma. We distinguished malignant B-cells from TME using canonical markers and BCR sequence, exploring heterogeneity in both components. We developed an original bioinformatic approach to define malignant B cell transcriptional archetypes shared across patients. To assess whether subclonal genetics are associated with transcriptional states, we inferred copy number variations (CNV) and studied BCR intraclonal variations of malignant cells. We also integrated TME data (39,674 cells) to quantify subpopulations and clonally expanded T-cell subsets, and performed robust statistical analyses to identify clinically relevant features linked to patient outcomes. To circumvent interpatient heterogeneity in malignant B cells, we defined transcriptional cellular archetypes conserved across patients by combining 3 unsupervised methods: rank-biased overlap meta-clustering, variational autoencoder and canonical correlation analysis integration. We identified 5 cellular archetypes (Arch.1-5): Arch.1 was characterized by plasma cell identity, Arch.2 by MYC and NFKB pathways, Arch.3 by MYC pathway only, Arch.4 by quiescence markers, and Arch.5 by GC markers. Several archetypes co-existed in each tumor sample, and archetype identity was superimposed to the transcriptional profile conferred by COO molecular class as an additional layer of transcriptional heterogeneity. Cell state-specific CNV and BCR subclones were very rare, suggesting that subclonal genetic changes were not the main drivers of malignant B cell plasticity and archetypes. Kaplan-Meier analysis showed a significant negative association between proportion of Arch.4 cells and continuous EFS in our diagnosis cohort. Unsupervised analysis of archetype composition further distinguished ABC patients who achieved EFS24 (enriched in Arch.3 cells) from those with a particularly poor prognosis (enriched in Arch.4 cells). Integrated analysis of the TME revealed three major ecosystems, including one enriched for effector and cytotoxic T cells. In diagnosis samples, pseudo-temporal trajectory analysis revealed the enrichment of clonally-expanded hyperactivated CD8+ T cells expressing canonical exhaustion markers in patients achieving EFS24, suggesting a positive link between the extent of CD8+ T cell differentiation and response to R-CHOP based regimens. Shedding new light on DLBCL biological and clinical heterogeneity, our comprehensive single-cell atlas of DLBCL uncovered recurrent tumor cell states and TME features crucial for stratifying non-responsive ABC patients and guiding treatment decisions. Ongoing single-cell spatial transcriptomics analyses will be used to further validate our findings and elucidate archetype-TME interactions that may drive malignant B cell plasticity, with the potential to uncover novel therapeutic targets.

Thallium Lung-to-Heart Quantification: Three Methods of Evaluation

Abstract:201 TI stress test images, has been found useful in diagnosing cardiac dysfunction. We evaluated three methods of quantification in terms of inter- and intraobserver variability and reproducibility. Fifty anterior 201 TI stress images were quantified by each of the following methods: Method A (sum region), which involved one region of interest (ROI) in the measurement of pul monary activity relative to that of the myocardium; Method B (count density), which required two ROis, the lung-to-heart ratio being dependent on count density; and Method C (maximum pixel), which used the gray scale of the computer to determine the most intense pixels in the lung field and myocardium. Statistical evaluation has shown that the three methods assess clinical data equally well. Method C was found to be most reproducible in terms of inter- and intraobserver variability (p = 0.0016 and 0.0083), followed by Meth ods A (p = 0.0026 and 0.0129) and B (p = 0.0038 and 0.0135). Al though nearly equivalent in terms of statistics, the three methods possess inherent differences and therefore should not be used inter changeably without conversion factors. Thallium-201 stress test imaging is useful in detecting coronary artery disease (CAD); as an adjunct to this examination, the lung-to-heart ratio provides additional knowledge of cardiac function. Previous studies suggest that increased pulmonary 201 Tl activity immediately after stress correlates with an increase in CAD (1). Furthermore, the lung to-heart ratio is held to be very specific for advanced CAD because increased activity appears to be a function of increased pulmonary capillary wedge pressures, reflecting development of transient, exercise-induced left ventricular failure (2). Given the clinical significance of the lung-to-heart ratio, we have evaluated three methods of quantification in terms of inter and intraobserver variability and reproducibility.
Validation of Circulating Tumor DNA As an Alternative to Tumor Tissue Biopsy in Genotyping across Multiple Lymphoma Subtypes

Abstract:Introduction Genotyping plays an important role in the diagnosis, risk stratification and treatment response surveillance of lymphoma, a disease with considerable heterogeneity in clinical and pathological characteristics. To date, tumor tissue biopsy based genotyping is still considered as gold standard in lymphoma, but its application is hampered by the invasive nature and imperfect performance in capturing spatial and temporal tumor heterogeneity. Circulating tumor DNA (ctDNA), which is shed into peripheral blood by apoptotic or necrotic tumor cells, has shown the potential as a noninvasive alternative to tumor tissue DNA (tDNA) for molecular profiling. Many efforts have been made to investigate the feasibility of ctDNA based on next-generation sequencing (NGS) in lymphoma, but most of the studies were conducted in Caucasian population and limited to B-cell lymphoma patients. In the present study, we evaluated the performance of plasma-derived ctDNA against tDNA based genotyping across multiple lymphoma subtypes. Methods Between 4/2021 and 5/2023, a total of 110 patients were enrolled in this study. Matched tumor tissue biopsies and peripheral blood were collected at diagnosis. Both tDNA and ctDNA were extracted and subjected to a targeted capture NGS assay covering the entire coding region or recurrently mutated region of 196 genes. The mean coverage depth was 2431x and 2513x for tDNA and ctDNA, respectively. The limit of detection (LOD) was 1% and 0.5% for tDNA and ctDNA, respectively. Results The enrolled cohort comprised of 63 (57.3%) male and 47(42.7%) female patients, with a median age of 58 years old (range 1.25-89). Among the 110 patients, the most prevalent subtype is diffuse large B-cell lymphoma (DLBCL,n=45), following by angioimmunoblastic T-cell lymphoma (AITL, n=16), anaplastic large cell lymphoma (ALCL, n=13) , mantle cell lymphoma (MCL, n=10), follicular lymphoma (FL, n=9), primary mediastinal large B-cell lymphoma (PMBL, n=9), extranodal natural killer/T cell lymphoma (ENKTL, n=4), and peripheral T-cell lymphoma not otherwise specified (PTCL-NOS, n=4). For further analysis, DLBCL, MCL, FL and PMBL were categorized as B-cell lymphoma group (n=73), while AITL, ALCL, ENKTL and PTCL-NOS were categorized as T-cell lymphoma group (n=37). Mutations were detected in 109(99.10%)tDNA and 110(100%)ctDNA, respectively. A total of 1586 mutations involving 177 genes were detected in tDNA, and the median number of mutations per sample was 12 (range 0-62), with KMT2D (39.09%), KMT2C (30.90%), IGLL5 (30.00%), TET2 (25.45%) and TP53 (20.90%) being the top 5 mutated genes. For ctDNA, a total of 1750 mutations involving 178 genes were detected and the median number of mutations per sample was 14 (range 2-63), with KMT2D (37.27%), KMT2C (29.09%), IGLL5 (27.27%), TET2 (25.45%) and DNMT3A (27.27%) being the top 5 mutated genes. To determine the concordance of mutations detected in tDNA and ctDNA, sensitivity was calculated utilizing tDNA as gold standard. The overall sensitivity of ctDNA based genotyping was 85.39% (95%CI: 82.36%-88.41%), with B-cell lymphoma showing a slightly higher sensitivity than T-cell lymphoma (86.17%, 95%CI: 83.51%-88.83% vs. 83.84%, 95%CI: 76.30%-91.32%). In terms of subtypes, PTCL-NOS exhibited the highest sensitivity (89.58%, 95%CI: 56.43%-122.73%, while AITL exhibited the lowest sensitivity (78.53%, 95%CI: 64.09%-92.97%) (Figure 1A). In addition, the difference of mutational spectrum between B-cell lymphoma and T-cell lymphoma was precisely depicted by ctDNA based genotyping. We discovered mutations in TET2, RHOA, and PLCG1 were significantly present in T-cell lymphoma, while mutations in MYD88, CD79B, CARD11, CREBBP, MYC, PIM1, IGLL5, ATM, SOCS1 and HIST1HIE were significantly enriched in B-cell lymphoma (Figure 1B). Conclusion In the present study, recurrent mutations across a variety of lymphoma subtypes were identified by ctDNA based genotyping, which were in high concordance with tDNA based genotyping. These data demonstrated that ctDNA based genotyping is a reliable noninvasive alternative to tumor tissue biopsy for molecular profiling of lymphoma patients, especially for patients whose tissues are not accessible for genotyping.

Abstract:Plasma alpha- and beta-mannosidase activities were measured in goats with beta-mannosidosis, a recently discovered, inherited deficiency of the glycohydrolase beta-D-mannosidase. Plasma beta-mannosidase activity in the affected animals was zero at 1 and 4 weeks after they were born. Enzyme values of age-matched, unaffected goats were between 66 and 222 nmol/hr/ml of plasma, alpha-Mannosidase activities were similar in both affected and unaffected animals. This investigation indicates that deficiency of plasma beta-mannosidases activity may be diagnostic of beta-mannosidosis.

Abstract:Purpose: Clinical outcomes of patients with CNS lymphomas (CNSLs) are remarkably heterogeneous, yet identification of patients at high risk for treatment failure is challenging. Furthermore, CNSL diagnosis often remains unconfirmed because of contraindications for invasive stereotactic biopsies. Therefore, improved biomarkers are needed to better stratify patients into risk groups, predict treatment response, and noninvasively identify CNSL. Patients and methods: We explored the value of circulating tumor DNA (ctDNA) for early outcome prediction, measurable residual disease monitoring, and surgery-free CNSL identification by applying ultrasensitive targeted next-generation sequencing to a total of 306 tumor, plasma, and CSF specimens from 136 patients with brain cancers, including 92 patients with CNSL. Results: Before therapy, ctDNA was detectable in 78% of plasma and 100% of CSF samples. Patients with positive ctDNA in pretreatment plasma had significantly shorter progression-free survival (PFS, P < .0001, log-rank test) and overall survival (OS, P = .0001, log-rank test). In multivariate analyses including established clinical and radiographic risk factors, pretreatment plasma ctDNA concentrations were independently prognostic of clinical outcomes (PFS HR, 1.4; 95% CI, 1.0 to 1.9; P = .03; OS HR, 1.6; 95% CI, 1.1 to 2.2; P = .006). Moreover, measurable residual disease detection by plasma ctDNA monitoring during treatment identified patients with particularly poor prognosis following curative-intent immunochemotherapy (PFS, P = .0002; OS, P = .004, log-rank test). Finally, we developed a proof-of-principle machine learning approach for biopsy-free CNSL identification from ctDNA, showing sensitivities of 59% (CSF) and 25% (plasma) with high positive predictive value. Conclusion: We demonstrate robust and ultrasensitive detection of ctDNA at various disease milestones in CNSL. Our findings highlight the role of ctDNA as a noninvasive biomarker and its potential value for personalized risk stratification and treatment guidance in patients with CNSL. [Media: see text].

Abstract:The analysis of circulating tumor DNA (ctDNA) in liquid biopsy specimens has an established role for the detection of predictive molecular alterations and acquired resistance mutations in several tumors. The low-invasiveness of this approach allows for repeated sampling and dynamic monitoring of disease evolution. Originating from the entire body tumor bulk, plasma-derived ctDNA reflects intra- and interlesional genetic heterogeneity. In the management of lymphoma patients, ctDNA quantification at various timepoints of the patient's clinical history is emerging as a complementary tool that may improve risk stratification, assessment of treatment response and early relapse detection during follow-up, most prominently in patients with diffuse large B-cell lymphoma or classic Hodgkin lymphoma. While liquid biopsies have not yet entered standard-of-care treatment protocols in these settings, several trials have provided evidence that at least a subset of lymphoma patients may benefit from the introduction of liquid biopsies into daily clinical care. In parallel, continuous technological developments have enabled highly sensitive ctDNA assessment methods, which span from locus-specific techniques identifying single hotspot mutations, to sequencing panels and genome-wide approaches that explore broader genetic and epigenetic alterations. Here, we provide an overview of current methods and ongoing technical developments for ctDNA evaluation. We also summarize the most important data from a selection of clinical studies that have explored the clinical use of ctDNA in several lymphoma entities.

Abstract:Abstract Recent technological advances in the detection and characterization of cell free circulating tumor DNA (ctDNA) are providing new opportunities for treatment tailoring based on real-time monitoring of clonal evolution. Circulating biomarkers can provide a "liquid biopsy" alternative to tissue biopsies allowing noninvasive tumor genotyping and the serial analysis of genomic changes from a simple blood test. The emerging clinical benefits of ctDNA analysis in the management of solid tumours has led to an expansion of opportunities in the use of ctDNA for disease monitoring in haematological malignancies. Research in this area has rapidly expanded exploring the role of ctDNA for risk stratification, monitoring tumour burden and identifying the early emergence of treatment resistance. In the management of lymphoma, comprehensive analysis of somatic genomic alterations in plasma DNA has emerged as an important clinical tool. The use of ctDNA analysis to follow tumour specific genomic alterations identified on an individual basis can be used to help identify minimal residual disease post treatment and guide therapeutic decisions. Moreover, ctDNA analysis can been utilized to identify distinct biological subtypes of lymphoma and provide insights into patterns of genomic evolution following treatment. The role of ctDNA as a minimally invasive strategy for disease monitoring has also recently been demonstrated in chronic lymphocytic leukemia (CLL) showing additional potential of this technique in haematological malignancies with a circulating disease component. In CLL, ctDNA analysis can reveal changes in the disease across tissue compartments, including the bone marrow and lymph nodes, providing additional information to that which can be assessed by monitoring the circulating disease alone. Serial ctDNA analysis in CLL can allow clonal dynamics to be monitored over time and can identify the emergence of genomic changes associated with Richter's syndrome. Here, I will focus on the use of ctDNA technology to follow disease burden and track genomic changes in patients receiving novel therapies for lymphoid malignancies, as well as the promises and challenges in developing these tools for clinical application. Abstract:Diffuse large B cell lymphoma (DLBCL), the most common non-Hodgkin lymphoma (NHL), is a clinically and molecularly heterogeneous malignant lymphoproliferative disease. In the era of personalized medicine, genetic information is critical to early diagnosis, aiding risk stratification, directing therapeutic option, and monitoring disease relapse. However, lacking a circulating disease with most DLBCL cases hampers the acquisition of tumor genomic landscapes and disease dynamics. Circulating tumor DNA (ctDNA) is a novel noninvasive, real-time, and tumor-specific biomarker, reliably reflecting the comprehensive tumor genetic profiles, thus holds great promise in individualized medicine, including precise diagnosis and prognosis, response monitoring, and relapse detection of DLBCL. Here, we reviewed the recent advances of ctDNA in DLBCL and discussed its clinical values at different time points during the disease courses by comparing with the current routine methods in clinical practice. Collectively, we anticipated that ctDNA will ultimately be integrated into the management of DLBCL to facilitate precision medicine.
