Abstract:OBJECTIVES: Anemia and changes in platelets (PLT) are common in inflammatory bowel disease (IBD). In our study, we aimed to verify whether PLT count can independently reflect the severity of IBD. METHODS: In our hospital, 137 Crohn's Disease (CD), 69 Ulcerative colitis (UC) patients, and 412 healthy controls were included to compare the differences in PLT count. In addition, the effect of anemia, C-reactive protein (CRP), age, CD activity index (CDAI) or Mayo on PLTs was also analyzed. If PLTs independently affected CD or UC, we used the receiver operating characteristic (ROC) curve to verify the diagnostic value and obtain the cut-off value of PLT. RESULTS: CD and UC patients had higher PLT than controls (p < 0.001, p < 0.001; respectively). In CD patients, the results showed that patients with anemia (P<0.01), Iron Deficiency Anemia (IDA) (p<0.001), CRP >= 8 mg/L (p=0.046), and CDAI >= 150 (p < 0.001) had higher PLT, while in UC patients, those with anemia (p=0.018), CRP >= 8 mg/L (p=0.045), and Mayo >= 3 (p=0.029) had higher PLT. Univariate analysis showed that CDAI was positively correlated with PLT count (p<0.001), while hemoglobin (p=0.001) and age (p<0.001) were negatively correlated with PLT in CD. In UC patients, Mayo (p=0.001) and CRP (p <0.001) were positively correlated with PLT, while hemoglobin (p=0.002) was negatively correlated. Finally, by linear stepwise multivariate analysis, we clarified the positive relationship between PLT and CD (p<0.001) by eliminating the interference of hemoglobin, and determined the cut-off value of PLT as 298 x 109/L. For UC, we did not obtain similar results. CONCLUSIONS: PLT can be an indicator of disease severity in CD, while there is a lack of evidence regarding this finding in UC.

Abstract:Aim: To determine the type and frequency of pulmonary involvement in patients with UC and to determine the distribution of pulmonary dysfunction in patients with UC. Methods: A total of 39 cases of UC, were enrolled consecutively in this study from the Department of Gastroenterology. The age, sex, family history, duration of disease, biochemical parameters (CRP, ESR, blood routine test, etc.), lung function, chest X-ray, diagnoses, and treatments were collected. Patients and controls underwent standard pulmonary function testing. Comparisons of rates among multiple groups were done with Fisher exact test, continuous variables were compared with one-way ANOVA. Results: According to the true love index, the severity of active UC was moderate and severe among 36.7% and 63.3% of subjects respectively. The pulmonary function test “FCV” showed reduced mean FCV among subjects with active UC (2.86±0.77 litres) and inactive UC (3.91±0.62 litres) as compared to controls (4.04±1.14 litres). Among the patients with deranged PFT, 1 (7.6%) had obstructive, 7 (53.8%) had small airway disease and 5 (38.6%) had a restrictive pattern of lung involvement. Conclusion: Subclinical pulmonary dysfunction is frequent in UC and dependent on disease activity. Some alterations were sub-clinical and some of the patients showed troublesome pulmonary symptoms.
Differences in Lung Dysfunction between Inflammatory Bowel Disease and Ir-ritable Bowel Syndrome Based on Interaction of Lung and Large Intestine

Abstract:Objective To compare the differences in lung function indexes and pulmonary manifes-tations among the patients of inflammatory bowel disease( IBD)and irritable bowel syndrome( IBS)as well as healthy group so as to sum up the occurrence rule of bronchial damage in IBD patients and seek the clinical evidence on“interaction of lung and large intestine”. Methods Two hundred and nineteen IBD patients were in accordance with the inclusive criteria,of them,183 cases had ulcerative colitis and 36 cases Crohn′s disease( CD). Thirty patients were collected,suffered from diarrhea IBS,and at same time 20 healthy volun-teers were chosen in the control group. The general data and lung symptoms were collected. Serum liver and kidney functions were detected. X-ray test of chest and lung function examinations were given. Results A-mong IBD patients,71. 2% of them had lung dysfunction,19. 6% of them had abnormal indications on the chest X-ray film,18. 7% of them had lung symptoms such as cough and short breath and 30. 1% of them had parenteral symptoms except lung such as recurrent dental ulcer. Among those diarrhea IBS patients, 33. 3% of them had lung dysfunction,3. 3% of them had lung symptoms. Among the healthy volunteers,20%of them had lung dysfunction and no case had lung symptom. The results of VC,FEV 1. 0%,MMEF,FEF 75%,DLCO and TLC in CD patients were reduced apparently as compared with those diarrhea IBS patients and healthy volunteers. The results of FEV 1. 0%,FEF 75%,MMEF and DLCO in UC patients were reduced apparently as compared with those diarrhea IBS patients and healthy volunteers. Conclusion Compared with diarrhea IBS patients and healthy volunteers,IBD patients have lung dysfunction more likely. The lung damage in CD patients is obstructive ventilation impairment and restrictive ventilation impairment and that in UC pa-tient is obstructive or mixed ventilation impairment;and that in IBD patients is much serious than the other parenteral manifestation. All of those prove the specificity of the disorders between lung and large intestine.

Abstract:Background Current biomarkers have been routinely non-invasive methods for assessing disease activity of inflammatory bowel disease (IBD), but none of them are specific. This study was aimed to determine the performance of the serological biomarkers for detecting disease activity in patients with IBD. Methods A prospective study that included 73 ulcerative disease (UC) subjects, 141 Crohn's disease (CD) subjects, and 30 of them were complicated with C. difficile infection (CDI), randomly selected from a single-institution IBD database. Disease activity was assessed using by Truelove and Witts criteria for UC and Harvey Bradshaw Simple Index for CD. Serological inflammatory biomarkers were compared in different severity groups. Receiver operator curve analyses assessed the performance of each biomarker in discriminating disease states. Results For UC patients, elevated monocyte counts, C reactive protein (CRP) and decreased lymphocyte counts, lymphocyte/monocyte ratio (LMR) significantly differed between subjects with active UC and UC in remission, and LMR performed better than the other leukocyte profiles. LMR of 3.1 was 76% sensitive and had a specificity of 67% for active UC (figure 1). CRP and fibrinogen were significantly elevated in UC with CDI compared to inactive UC without CDI patients. For CD patients, higher values of white blood cells, neutrophils, monocytes, neutrophil/lymphocyte ratio, CRP fibrinogen, and lowe values of LMR and hemoglobin were significantly different between subjects with active CD and CD in remission. None of the biomarkers included had a good correlation with disease activity (Area Under the ROC curve [AUC] < 0.70) (figure 2). CD with CDI patients compared with CD patients in remission showed significantly higher CRP. Conclusions A low LMR represents an inexpensive, readily available test with a promising value to identify disease activity in UC patients. Whereas none of the inflammatory biomarkers showed a discriminative value in disease activity of CD.
The association between pulmonary function impairment and colon inflammation in ulcerative colitis patients: A scientific basis for exterior-interior correlation between lung and large intestine

Abstract:ObjectiveTo investigated the involvement of pulmonary function impairment in ulcerative colitis (UC), to explore a scientific basis for the Chinese medicine (CM) theory of exterior-interior correlation between Lung (Fei) and Large intestine (Dachang).MethodsTotally 120 patients with a diagnosis of UC were recruited and the demographics, clinical data, and blood samples were collected. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) concentrations were measured. Every patient accepted pulmonary function test and took chest radiograph (CXR).>ResultsPulmonary function abnormalities were present in 72 of 120 patients. The median (interquartile range) vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), carbon monoxide diffusion capacity (DLCO) of lung, total lung capacity (TLC) and functional residual volume (FRV) were decreased in distal UC and pancolitis compared with ulcerative prochitis (P <0.0005). Male patients had increased VC, FEV1/FVC, and residual volume (RV)/TLC compared with female (P <0.0005), but decreased DLCO and carbon monoxide iffusion capacity (KCO) of lung/alveolar ventilation (P <0.0005). Age was strongly correlated with RV (Spearman rank correlation coefficient (rs)=−0.57,P <0.0001), and RV/TLC (rs=0.48,P<0.0001). Age was also correlated with FEV1/FVC (rs=−0.29, P=0.001), forced expiratory flow in 75% vital capacity (FEF75%, rs=−0.20, P=0.03), DLCO (rs=−0.21, P=0.02), TLC (rs=−0.25, P=0.006), and FRV (rs=−0.28, P=0.002). The course of disease was correlated with FEF75% (rs=−0.18, P=0.049) and KCO (rs=−0.19, P=0.036). Chest radiograph abnormalities were presented in 38 of 120. Pulmonary symptoms were presented in 10 of 120. Other extraintestinal complications were presented in 21 of 120.ConclusionsPulmonary function impairment was more frequently than other extraintestinal complications in UC patients, which may be affected by sex, age, extent and course of disease. These results may be a scientific basis for the theory of exterior-interior correlation between Lung and Large intestine.

P467 Perception Gap Between Patients with Inflammatory Bowel Disease (IBD) and Physicians in China: A Cross-sectional Study

Abstract:Abstract Background The aim of the study was to assess the perception of inflammatory bowel disease (IBD) among Chinese patients and physicians. Methods Data in this cross-sectional study was collected from 6/2020 to 8/2020, which covered 24 provinces/cities in China and based on two surveys that was designed for collecting information from patients with Crohn’s disease (CD)/ulcerative colitis (UC) and the physicians. Results Surveys were completed by 1058 patients (601 CD and 457 UC patients) and 351 physicians (181 treating CD and 170 treating UC). For the control of disease symptoms over the past 12 months, physicians believed that 65.19% and 57.65% of CD and UC patients, respectively, were managed without affecting their quality of life. Meanwhile, 56.07% and 49.46% of CD and UC patients, respectively, reported that disease control did not affect their quality of life. Physicians estimated that 87.85% of CD and 88.24% of UC patients were satisfied with treatment results. And 86.36% of CD and 79.43% of UC patients were satisfied with the current medications. Physicians prescribed biologics for 69.52% of IBD patients, and 48.3% of IBD patients actually used them. Repeated medication (30.95%), diarrhea (27.95%) and abdominal pain (25.29%) were most frequently ranked as most bothersome symptoms by CD patients. For physicians, abdominal pain (42.54%) was the most bothersome symptoms, followed by repeated medication (23.76%) and diarrhea (17.13%). Regarding UC symptoms, 51.18% of physicians believed that bloody stool was the most bothersome symptom for patients, and 41.58% of patients held this view. The sense of urgency was concerned as one bothersome symptom by 19.69% patients, while only 4.12% of physicians held the same views (Fig 1). Regarding the efficacy evaluation, in view of the CD physicians, 42.5% of CD patients were believed achieving completely or mostly controlled, while from the patients’ perception, only 32.6% of CD patients agreed. Similarly, the proportion of UC patients who was believed by physicians that achieving completely or mostly controlled (40.0%) was also higher than that of UC patients who believed by themselves (Fig 2). Conclusion Most of the IBD patients and physicians were satisfied with the treatment results and the current medications. The disease control expectation of the patients was slightly more than that of the physicians. Further studies are needed to explore factors that associated with the treatment expectation of the patients.

Abstract:Summary: Introduction: Inflammatory bowel diseases (IBDs) are a group of diseases including Crohn’s disease, ulcerative colitis and unclassified IBD. They are chronic, immune-mediated inflammatory diseases of the gastrointestinal tract. Due to their extraintestinal manifestation, they are considered to be systemic diseases. IBD has a negative impact on health-related quality of life. The treatment is life-long and may be accompanied by side effects. Low-intensity activity of moderate duration is sufficient to elicit improvements in fitness, decrease stress, and improve symptoms. In this study, the relationship between physical activity and IBD development and course is described. Aims: To assess the relationship between physical activity and exercise capacity and the severity and course of IBD. Methods: Probands with IBD and healthy controls underwent pulmonary function testing (PFT) and cardiopulmonary exercise testing (CPET). The results were compared between these two groups; in the IBD group, the results were correlated with disease activity parameters as well. In the IBD group, the probands were encouraged to perform regular physical activity. After 0.5–1 year, they underwent another CPET. Their exercise capacity development was objectively assessed and it was correlated with their IBD development. Results: Forty-one children were included in the study. Twenty of them were healthy controls, while 21 comprised the IBD patient group. These probands underwent the PFT and CPET. When comparing healthy controls and IBD patients, significant differences were found in peak oxygen uptake and peak work rate. In the IBD group, a negative correlation was found between peak work rate and C-reactive protein concentration. In faecal calprotectin, a significant correlation was found only in the subgroup with ulcerative colitis – the negative correlation with peak oxygen uptake and partial pressure of CO2 in exhaled air in the peak of the exercise. PCDAI correlated positively with ventilatory index, peak dead space and tidal volume ratio but negatively with peak work rate, work rate at the time of anaerobic threshold, peak oxygen uptake and oxygen uptake efficacy slope. In PUCAI, we found a positive correlation with CO2 ventilatory uptake at the time of anaerobic threshold and negative correlations with peak oxygen uptake and oxygen uptake efficacy slope. In further CPET in the IBD group, a significant decrease was found in faecal calprotectin and PCDAI or PUCAI; there were also positive correlations between PCDAI/PUCAI changes and resting heart rate changes. The negative correlation was found in the correlation with the changes in peak oxygen uptake, metabolic index and ventilatory equivalent for CO2 at the time of anaerobic threshold. Conclusion: These results suggest that regular recreational physical exercise positively impacts IBD. When adhering to treatment recommendations, doing sport professionally is also possible. Abstract:BACKGROUND:Inflammatory bowel disease (IBD) is a complex chronic disorder characterized by systemic inflammation, which may cause abnormal state of coagulation, resulting in cardiac events. This study aimed to investigate the incidences and risks of cardiac events in patients with IBD in China.METHODS:A retrospective cohort study was performed comprising 1435 patients with IBD from 12 IBD centers in China. Cases were matched with 1588 eligible participants without IBD from 12 medical centers according to age, sex, and laboratory parameters.RESULTS:Patients with IBD in China exhibited significantly higher incidences of ischemic heart disease (IHD; coronary heart disease included) but lower frequencies of right bundle branch block and premature contraction than those of matched controls. The risk of IHD increased in patients with IBD, peaking at the age of 18-35 years. Female patients with IBD were more likely to experience IHD than male patients. The C-reactive protein (CRP) levels and neutrophil count in the peripheral blood were positively related with the risk of IHD among patients with Crohn's disease, whereas plasma fibrinogen levels were negatively related with the risk of IHD both in patients with Crohn's disease and ulcerative colitis.CONCLUSIONS:The risk of IHD is increased in patients with IBD, especially in young female patients with IBD when compared with matched non-IBD subjects. The CRP and plasma fibrinogen levels and neutrophil count in the peripheral blood may be potential predictors associated with the occurrence of IHD in patients with IBD. The study's findings have significant implications for the management and prevention of cardiac events in patients with IBD.

Abstract:Abstract Background Moderate physical activity (PA) appears to be beneficial for inflammatory bowel disease (IBD), improving the symptoms of the disease and promoting the maintenance of remission. However, little is known about the impact of different types of PA on IBD activity. Methods IBD patients with stable treatment without steroids for 4 months were prospectively included and randomized into 3 groups: muscle strengthening exercises, aerobic exercises, and a control group. The impact of a 10-week training period with 2 sessions per week was evaluated. Following parameters were collected: clinical activity of the disease (Mayo score for ulcerative colitis or UC, Harvey-Bradshaw index or HBI for Crohn’s disease or CD, patient reported outcome or PRO for both), Godin Leisure Time Exercise Questionnaire or GLTEQ (assessing PA level), Metabolic Equivalent Task or MET (assessing sedentarity level), barriers to PA, quality of life assessment (by EuroQol 5 dimensions or EQ5D and Short health scale or SHS), Inflammatory Bowel Disease Fatigue (IBD-F), physical abilities according to exercise stress test and maximum strength test. Results Between January 2021 and September 2022, a total 33 patients were enrolled in the program. Of these, 24 patients (13 women, 17 CD, median age of 31.6 [IQR, 29.4- 46.4]) completed the program and were included in the analysis (dropout rate of 18.2%). Table 1 shows the characteristics of these patients at inclusion. Eight, six and ten patients were respectively randomized in the muscle strengthening, aerobic exercise and control groups. After the training period, 50% of patients in the muscle strengthening group and 66.7% of patients in the aerobic exercise group showed a clinical improvement (according Mayo score and HBI). Strengthening was associated with significant reduction in PRO rectal bleeding in UC patients (p<0.0001) and aerobic exercise was associated with a significant lowering of HBI (p=0.039). Among the 24 patients, 2 CD patients relapsed in the muscle strengthening group during the training period. Aerobic exercise significantly reduced the barriers to PA (p=0.037). In neither group did we find any positive or negative impact of PA on quality of life or fatigue. Strengthening exercise and aerobic exercise significantly improved VO2max (maximal oxygen consumption) and Pmax in the exercise stress test, respectively. Most of the maximum strength tests were improved in the muscle strengthening group. Conclusion PA was well tolerated and associated with an improvement of disease activity in the majority of the IBD patients. There may be a variable impact of different types of PA in CD and UC, which should be explored further in larger cohorts.

Relationship between Exhaled Nitric Oxide and Lung Function

Abstract:Ulcerative colitis (UC) is characterized by immune system dysregulation with frequent extraintestinal manifestations, including airway involvement. A reduction in CO diffusing capacity and functional alterations in small airways have been described. An extended analysis of fractional exhaled nitric oxide (FeNO) may distinguish the sites of production, and the presence of small airway inflammation may be a useful, non-invasive marker for patient follow-up. The aim of our study was to compare the PFTs as well as FeNO and CANO values of UC patients with different clinical disease activities and healthy subjects to reveal lung function abnormalities and the presence of subclinical airway inflammation. We enrolled 42 adult outpatients at different clinical activity stages of UC (39 ± 13 years) and a healthy control group of 41 subjects (29 ± 3 years). C-reactive protein (CRP) and FeNO values at different flows (50,100, and 200 mL/s) were collected. All patients performed pulmonary function tests (PFTs) with static volumes and diffusing capacity (DLCO). FeNO and CANO values were significantly increased in UC patients when compared with controls (p = 0.0008 and p < 0.0001, respectively) and were proportional to disease activity (FeNO class 3: 28.1 ppb vs. classes 1–2: 7.7 ppb; CANO values class 3: 8.6 ppb vs. classes 1–2: 2.7 ppb (p < 0.0001)). TLC and DLCO were significantly reduced in severe (Mayo 3) UC patients (p = 0.010 and p = 0.003, respectively). The results of this study show significant lung functional abnormalities in UC patients and suggest the presence of airway inflammation directly correlated with disease activity, suggesting the need for an integrated approach in routine assessment.

Abstract:The authors describe two cases of false aneurysm of the right subclavian artery. One was due to a penetrating cervical injury and the other to blunt thoracic trauma. Both patients were treated surgically. Early diagnosis by aortography and surgical treatment are important. The operation is difficult because of the cervicothoracic course of the subclavian artery. Because of this a wide approach should be made and reconstruction is mandatory.

Abstract:Background The inflammatory bowel disease (IBD), comprising Crohn's disease (CD) and ulcerative colitis (UC) is a complex disease with multifactorial etiology. The intestinal dysbiosis have been investigated to play an important role in IBD pathogenesis and disease activity. The aim of our study was to analyze the intestinal microbiota composition in IBD across different severity levels and the impact of biologic therapy on microbiota modulation. Methods In this study, 27 IBD patients were recruited, including 14 patients undergoing biologic therapy for moderate to severe disease activity and 13 controls with inactive disease. The gut microbial composition was determined by 16S ribosomal RNA gene sequencing of stool samples. Results Biologic therapy led to significant clinical improvement in IBD disease activity after 48 weeks. About species richness, community alpha diversity was significant lower in active CD patients and enriched gradually after biologic therapy. The beta-diversity regard to the difference of bacterial community composition showed significant difference between patients in biologic and control group. A decrease in Firmicutes and increase in Bacteroidetes abundance were observed in patients with active disease, both in CD and UC. Biologic treatment induced shifts in gut microbiota, with increased Firmicutes and decreased Bacteroidetes , as well as improved F/B ratio gradually after treatment, correlating with disease activity. Conclusions Our study suggested that gut microbiota differences changed after biologic therapies among IBD with different disease activity, and a rising Firmicutes/Bacteroidetes ratio could be a potential predictor for disease activity and treatment response monitoring.

Abstract:Abstract Background Cross-sectional evidence suggests a higher burden of chronic respiratory diseases in people with inflammatory bowel disease (IBD), but there is a lack of prospective evidence to clarify the direction of their associations. This study aimed to investigate the association of IBD with the risk of two major chronic respiratory diseases, chronic obstructive pulmonary disease (COPD) and asthma. Methods We included 430,414 participants from UK Biobank and followed them from recruitment (2006-2010) to 2021. COPD and asthma cases were obtained from inpatient data and death register. Using Cox proportional hazards models, we estimated the multivariable-adjusted hazard ratios (HR) with 95% confidence intervals (CIs) of developing COPD and asthma in participants with IBD compared with IBD-free groups. We also investigated the association among Crohn's disease (CD) and ulcerative colitis (UC) with the risk of COPD and asthma. Results Over a median follow-up of 143 months, there were 11,196 incident COPD and 9831 asthma cases. The adjusted HRs of developing COPD (HR=1.54, 95% CI: 1.33-1.79) and asthma (HR=1.52, 95% CI: 1.29-1.79) were higher for those with IBD when compared with IBD-free participants. Participants with CD and UC were also found to have a higher risk of COPD (CD: HR=1.71; 95% CI: 1.36-2.15; UC: HR=1.45; 95% CI: 1.20-1.75) and asthma (CD: HR=1.73; 95%CI: 1.33-2.25; UC: HR=1.41; 95%CI: 1.15-1.73) when compared with those free of IBD (Table 1). Conclusion This study suggested that individuals with IBD have a higher risk of developing COPD and asthma, highlighting the importance of preventing chronic respiratory diseases among IBD patients.

Abstract:Inflammation serves a key role in chronic obstructive pulmonary disease (COPD). However, changes in the immune profiles of patients with COPD remain unclear. The present prospective observational study aimed to determine the expression profiles of immune cells and inflammatory factors of patients with COPD and healthy controls, and to analyze the relationship between immune profiles and smoking history. A total of 140 subjects were enrolled in the present study between September 2018 and April 2019 at West China Hospital of Sichuan University (Chengdu, China). These included 87 patients with stable COPD and 24 patients with acute exacerbated COPD, as well as 29 healthy controls. Baseline characteristics were recorded during the screening period, and levels of immune cells were examined using flow cytometry. In addition, levels of inflammatory factors were measured using ELISAs. The results revealed increased expression of the CD64+/CD14+ mononuclear phagocyte system (MPS) and CD16+CD66+ neutrophils, and decreased expression of CD3+CD4+ T cells and CD3+ CD8+ T cells (all P<0.05) in the peripheral blood of patients with COPD and smokers relative to non‑smoking controls. In addition, significant differences were observed in protein levels of IL‑6, IL‑1β, TNF‑α, TGF‑α, IFN‑γ, IL‑8, IL‑17A and CRP among the three groups (all P<0.05). Furthermore, the IL‑17A, TNF and NF‑κB signaling pathways were found to serve a key role in the inflammatory network of COPD. Pearson's correlation analysis revealed a positive correlation between CD3+T lymphocyte counts and pulmonary function, and a negative correlation with smoking and exacerbations. Furthermore, neutrophils and MPS were negatively associated with pulmonary function, and IL‑8 was positively associated with cough. There was also a negative association between CRP and IL‑17A with pulmonary function but a positive correlation with symptoms and exacerbation. Club cell secretory protein was also negatively associated with emphysema parameters. In conclusion, the present findings revealed significant differences in profiles of immune factors among patients with COPD, smokers and non‑smoking controls and their association with clinical characteristics. [Correlation Between Pulmonary Function Impairment and Levels of Alpha1-Antitrypsin in Serum and Colon of Ulcerative Colitis Patients: a Clinical Research].

Abstract:OBJECTIVE:To explore the mechanism of pulmonary involvement in ulcerative colitis (UC) patients by observing the correlation between pulmonary functions and levels of alpha1-antitrypsin (A1AT) in serum and colon tissue in UC patients.METHODS:Totally 90 patients with confirmed UC were assigned to different groups according to the extent of disease, the disease activity, the staging of severity, and course of disease. The serum level of A1AT in UC patients with different extent of disease, the disease activity, the staging of severity, and course of disease were compared. And 30 healthy volunteers were recruited as the control group. The serum renal and hepatic functions, pulmonary functions, and serum levels of A1AT were detected in the UC group and the control group. The correlation between A1AT and each pulmonary function index in UC patients was analyzed. The A1AT content in the colon tissue was detected with immunohistochemical assay in 20 UC patients as well as in 10 healthy volunteers.RESULTS:Of the 90 UC patients, 54 patients were accompanied with pulmonary function abnormality (60.0%), and 24 with extraintestinal manifestations (26.7%). Compared with the control group, the serum level of A1AT was significantly lower in the UC group (P < 0.05). The serum level of A1AT was significantly higher in those with proctitis than in those with distal colonitis and pancolitis (P < 0.05). The serum level of A1AT was lower in patients with the course of disease 5 years and more than 5 years than in those with the course of disease less than 5 years (P < 0.05). Vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in one second (FEV1.0), total lung capacity (TLC), function residual volume (FRV), and the ratio of diffusion capacity for carbon monoxide of lung (DLCO) were much lower in those with proctitis than in those with distal colonitis and pancolitis (P < 0.05). The ratio of FVC was negatively linear correlated with the course of disease (r = -0.23, P = 0.018). There was a positive correlation between the serum level of A1AT and peak expiratory flow (PEF) (r = 0.22, P = 0.03). The level of A1AT in the colon tissue was obviously lower in the UC patients than in those of the control group (P < 0.05). Mild and moderate UC patients had increased levels of A1AT in the colon tissue, when compared with severe UC patients (P < 0.05). The level of A1AT in the colon tissue was higher in those with proctitis than in those with distal colonitis and pancolitis (P < 0.05).CONCLUSIONS:The prevalence of pulmonary function impairment was higher than other extraintestinal manifestations in UC patients. The pulmonary function test was helpful to screen the pulmonary impairment of UC patients. The A1AT level in the serum and the colon tissue obviously decreased in UC patients, indicating the pulmonary function impairment of UC patients might manifest as decreased A1AT levels correlated chronic airway inflammation, remodeling of airway, and obstructive changes.
Increased Proportions of Bifidobacterium and the Lactobacillus Group and Loss of Butyrate-Producing Bacteria in Inflammatory Bowel Disease

Abstract:ABSTRACT Dysbiosis in the intestinal microbiota of persons with inflammatory bowel disease (IBD) has been described, but there are still varied reports on changes in the abundance of Bifidobacterium and Lactobacillus organisms in patients with IBD. The aim of this investigation was to compare the compositions of mucosa-associated and fecal bacteria in patients with IBD and in healthy controls (HCs). Fecal and biopsy samples from 21 HCs, 21 and 15 Crohn's disease (CD) patients, and 34 and 29 ulcerative colitis (UC) patients, respectively, were analyzed by quantitative real-time PCR targeting the 16S rRNA gene. The bacterial numbers were transformed into relative percentages for statistical analysis. The proportions of bacteria were uniformly distributed along the colon regardless of the disease state. Bifidobacterium was significantly increased in the biopsy specimens of active UC patients compared to those in the HCs (4.6% versus 2.1%, P = 0.001), and the proportion of Bifidobacterium was significantly higher in the biopsy specimens than in the fecal samples in active CD patients (2.7% versus 2.0%, P = 0.012). The Lactobacillus group was significantly increased in the biopsy specimens of active CD patients compared to those in the HCs (3.4% versus 2.3%, P = 0.036). Compared to the HCs, Faecalibacterium prausnitzii was sharply decreased in both the fecal and biopsy specimens of the active CD patients (0.3% versus 14.0%, P < 0.0001 for fecal samples; 0.8% versus 11.4%, P < 0.0001 for biopsy specimens) and the active UC patients (4.3% versus 14.0%, P = 0.001 for fecal samples; 2.8% versus 11.4%, P < 0.0001 for biopsy specimens). In conclusion, Bifidobacterium and the Lactobacillus group were increased in active IBD patients and should be used more cautiously as probiotics during the active phase of IBD. Butyrate-producing bacteria might be important to gut homeostasis.

Leveraging Recurrent Phrase Structure in Large-scale Ontology Translation

Abstract:This paper presents a process for leveraging structural relationships and reusable phrases when translating large-scale ontologies. Digital libraries are becoming more and more prevalent. An important step in providing universal access to such material is to provide multi-lingual access to the underlying principles of organization via ontologies, thesauri, and controlled vocabularies. Machine translation of these resources requires high accuracy and a deep vocabulary. Human input is often required, but full manual translation can be slow and expensive. We report on a cost-effective approach to ontology translation. We describe our technique of prioritization, our process of collecting aligned translations and generating a new lexicon, and the resulting improvement to translation system output. Our preliminary evaluation indicates that this technique provides significant cost savings for human-assisted translation. The process we developed can be applied to ontologies in other domains and is easily incorporated into other translation systems.

Abstract:There are only scarce recent reports about the role of growth differentiation factor 15 (GDF-15) and some more data about interleukin-6 (IL-6) in inflammatory bowel diseases (IBD). We assessed GDF-15 and IL-6 serum levels in patients with IBD and associations with their characteristics. We included 122 and 71 stored samples from patients with Crohn's disease (CD) and ulcerative colitis (UC), respectively, and regular follow-up and 44 samples from healthy controls. Data regarding epidemiologic and disease characteristics were recorded. In CD, both GDF-15 and IL-6 levels were higher in active disease or all patients than controls ( P ≤ 0.020) as well as patients with elevated CRP ( P ≤ 0.008), endoscopically active disease ( P ≤ 0.017), age ≥ 40 years ( P ≤ 0.005) and active smokers ( P ≤ 0.050) and were positively correlated with hospitalization numbers ( P ≤ 0.019). GDF-15 levels were also positively correlated with flares within year-1 ( P < 0.001). In UC, both GDF-15 and IL-6 levels were higher in clinically active or all patients than controls ( P < 0.001), but they shared no other association with patient characteristics except for positive correlation with CRP. Only IL-6 levels were higher in active than inactive UC either clinically ( P = 0.047) or endoscopically ( P < 0.001) and were positively correlated with stool calprotectin ( P = 0.021). GDF-15 was positively correlated to IL-6 levels only in UC (r s =0.591, P < 0.001) but not in CD. In conclusion, in CD, GDF-15 and IL-6 levels could constitute indexes of activity and even offer a prognostic index of disease progression. In UC, IL-6 could also represent an activity index, but the role of GDF-15 needs further evaluation.

Abstract:Background and Aim: To investigate the incidence of small intestinal bacterial overgrowth (SIBO) and low-grade systemic inflammation in patients with inflammatory bowel disease (IBD). Methods: From August 2018 to December 2019, 71 patients with inflammatory bowel disease were recruited, and 30 healthy people who came to our hospital for physical examinations at the same time were selected as the control group. The lactulose hydrogen breath test (LBT) and methane LBT were used to detect the prevalence of SIBO. The incidence of low-grade systemic inflammation in IBD patients was determined by the fractional exhaled nitric oxide (FeNO) breath test. The chi-squared test was used for statistical analysis. Results: The positive rate of SIBO in the IBD group was 33.8 %, which was significantly higher than that in the healthy control group (6.6 %) (P<0.01). The incidence of SIBO between UC and CD groups was not significantly different (P = 0.551). The SIBO positive rate was significantly increased in CD patients with no ileocecal valve, ileocecal valve malformation, or intestinal stenosis (P<0.05). The SIBO negative rate of IBD patients treated with infliximab (IFX) was significantly higher than that of IBD patients treated with 5-aminosalicylic acid (5-ASA) (P = 0.006). The FeNO positive rate in IBD patients with SIBO was higher (17/24) than that of IBD patients without SIBO (6/47), and the difference was statistically significant (P<0.01). Conclusion: SIBO in IBD patients was significantly higher than in healthy people, but there was no significant difference between UC and CD groups. The combination of the absence of the ileocecal valve, ileocecal valve malformation, and intestinal lumen stenosis was associated with CD combined with SIBO. IBD patients using IFX were less likely to incorporate SIBO than IBD patients using 5ASA. SIBO was associated with a low inflammatory response in IBD patients.
