Abstract:Introduction: an effective health information system (HIS) ensures the production, analysis, dissemination and use of reliable and up-to-date information on the determinants of health. However, it can encounter obstacles that hinder its functioning, such as armed conflicts, which limit access and quality of healthcare services. The purpose of our study was to help improve data management for routine health information system in the health district of Timbuktu during a security crisis. Methods: we conducted a descriptive cross-sectional study, among health information management professionals in the Timbuktu Health District from 15 April to 08 September 2023. Data obtained from a survey questionnaire were analyzed using Epi Info version 7.2.2. and processed using Microsoft Word and Excel 2016. Results: a total of 6 health facilities were surveyed. Data collection, analysis and feedback were very poor. Data quality was 100% complete, 92.40% prompt and 68.11% accurate. The major constraints were: low involvement of health workers in the SIS (22.22%), insufficient training on the SISR (29.63%), supervision (47.06%), internet inaccessibility (66.67%), feeling of insecurity (37.04%) and fear (61.76%) in health facilities. Conclusion: our results show low-level processes, poor network coverage, shortage of qualified health information management professionals and increasing insecurity. A broader mixed-methods research would provide a better understanding.

Abstract:Background Rapid, integrated information exchange between stakeholders is critical for effective emergency preparedness and response. However, many low- and middle-income countries face barriers to seamless data sharing. While information accessibility is recognized as important for evidence-based decision-making and resource allocation in Ethiopia, factors influencing current health information sharing practices among stakeholders involved in public health emergency management programs are unclear. This study aims to examine multi-sectoral stakeholders' perspectives and experiences with health data sharing during emergencies in Ethiopia, to identify opportunities and challenges influencing practices to strengthen the national public health emergency response system. Methods A mixed-methods study was conducted between June and August 2023, involving a survey of 169 stakeholders actively involved in PHEM programs in Ethiopia as well as 23 in-depth interviews with key informants in senior leadership or advisory roles. The data was analyzed using descriptive statistics in SPSS and thematic analysis of qualitative transcripts. Results During emergencies, it was observed that data sharing between different entities occurred. Quantitative findings showed the predominant types of health data shared between stakeholders during emergencies included hospital data (109, 64.5 %), clinical case information, and laboratory results. Challenges limiting effective coordination included issues like limited functionality of digital health systems (75, 44 %), incompatible data formats (13, 34 %), and financial constraints (83, 49 %) and socio-cultural barriers constrain current practices in Ethiopia. Qualitative interviews identified five themes around risk communication and inclusive alert systems. Experts emphasized tailored, multichannel outreach but noted infrastructure gaps and digital divides currently limit poorer communities' engagement. Conclusion While collaborative health information exchange during emergencies is recognized as important, systemic, financial, and socio-cultural barriers constrain current practices in Ethiopia. Targeted strategies including capacity building, investment in integrated data infrastructure, economic optimization through innovative financing models, trust-based relationship development, and locally relevant communication channels informed by stakeholder perspectives can optimize information accessibility, coordination, quality, and equity of healthcare services during public health emergencies.

Abstract:This work involved diagnosing the Beninese health system based on secondary data from several sources (statistical directories, surveys, censuses, and study reports). The descriptive analysis of the evolution of health indicators in Benin between 2002 and 2021 and in-depth interviews with resource people in the health sector made it possible to assess the efforts and progress made by those in power and system stakeholders. There have been many tangible efforts to improve accessibility to health services (the rates of health coverage and attendance at health facilities have increased from 86% to 96% and from 35% to 56% between 2002 and 2021). These efforts have significantly reduced morbidity and mortality rates. The results also indicate a drop in deaths due to malaria (141 deaths to 86 per 100,000 inhabitants between 2003 and 2021) and a drop in the prevalence of HIV AIDS up to 0.8% from 2019 ahead of several countries—neighbors of the sub-region. Early neonatal mortality fell from 10.1‰ to 4.7‰ as well as deaths of children under 5 years old, thus improving life expectancy at birth from 59.6 years in 2002 to 63.8 years in 2013. Despite these efforts, many challenges remain, adding to the strong demographic growth in the country, which clearly expresses the threats weighing on the daily state of health of the Beninese population and calling for a new way of thinking for a sustainable health system. The study ends with the prioritization of challenges and a proposal for strategies to have an efficient and resilient health system, capable of producing quality, healthy, and productive human capital.

Ecology of emergency care in lower-tier healthcare providers in Ghana: an empirical data-driven Bayesian network analytical approach

Abstract:The healthcare landscape in Ghana is primarily composed of lower-tier providers, which serve as the initial point of contact for most medical emergencies. This study aimed to assess the emergency care preparedness and readiness of primary healthcare providers using a robust evaluation approach. A multicentre retrospective cross-sectional study was conducted on 460 healthcare facilities using the standardised Health Facilities Emergency Preparedness Assessment Tool (HeFEPAT). Data were analysed via Bayesian Belief network. Emergency preparedness was associated with facility location, type, ownership, and in-charge personnel. Over 70% of facilities lacked specialised emergency/critical care personnel. Although 65% of in-charges reported protocol knowledge, only 7.8% could execute cardiopulmonary resuscitation. 90% of facilities lacked onsite defibrillators, and over 80% had no cerebrovascular accident medications. Road traffic accident protocols were largely unavailable, with an estimated 53% probability of lacking such protocols. Private-owned facilities were more likely to lack protocols for road traffic accidents (76% vs 20% probability) and general acute care (62% vs 32%) compared to government-owned facilities. Significant gaps in emergency preparedness were identified across the studied health facilities, indicating limited capacity to manage critical situations effectively. Urgent investments in emergency medicine training, essential resources, and evidence-based protocols are needed. Standardised emergency preparedness assessments should be implemented for accreditation and quality improvement. Further research can inform the development of national guidelines and targeted interventions to strengthen emergency response capacities.

Hospital Waste Management: An Overview of the Situation in Health Districts outside Yaounde in the Centre Region, Cameroon

Abstract:Background: Hospital waste management is an important process that must be handled with care. This is because of the potential risks it represents for human health and the environment. A good understanding of the processes and legislation involved is an important asset, especially in developing countries. In order to assess the level of application of regulations, this study describes the quality of hospital waste management in health districts outside Yaounde in the Centre region. Methodology: A descriptive cross-sectional study was conducted in public health facilities of 24 Health Districts in the Centre Region from February 1 to March 31, 2024. Sampling was consecutive and non-exhaustive. Data were collected using a Google form from health facility managers and/or staff in charge of hospital hygiene. The data was coded and analyzed using R Statistics version 4.3.1. Results: A total of 61 participants coming from 20 Health Districts of the Central Region were enrolled in the study. With a sex ratio (M/F) of 0.9, most of the participants were paramedical staff. Less than 10% of the participants were Medical-sanitary technician. Most of the respondents reported that waste was sorted at source. However, nearly half (45%) carried out regular checks at this stage. More than two-thirds of responded (78%) reported that household waste was packed in buckets and black bags. More than half of the infectious waste was packaged in bin buckets (57%). More than one-third (39%) of the waste was collected daily. The vast majority (93%) reported that they disposed of waste on site. Nearly 5% reported disposing of waste in a public landfill. A small proportion (7.3%) of health facilities had an incinerator. Conclusion: Despite the efforts made, there are still significant shortcomings in the hospital waste management strategy of the health facilities visited. Stakeholders need technical, financial and material support if they are to meet international standards.

Abstract:Pharmacovigilance is the science and activities related to the detection, evaluation, understanding and prevention of adverse drug reactions or any other possible drug-related problems. In our tropics, this discipline is in an embryonic state. The availability of a management system capable of responding to pharmacovigilance activities is the main objective of our study. The coding was done on the DJANGO Framework. Signal detection was done using the ROR method. We designed three modules which are the notification module, the analysis module and the statistics module. This study has allowed us to launch the basis for a computerization of the pharmacovigilance information system and partly meets our objective. However, it could lead to the integration of the dictionary of adverse effects such as MedDRA as well as the International Classification of Medicines (ATC, EphMRA).

Abstract:Objective Hypertensive crisis is an increasingly frequent medical condition in our context. Its management in medical emergencies is a real challenge for physicians. Few data on hypertensive crisis are available in Chad. The aim of this study was to investigate the epidemiological, clinical and prognostic characteristics of hypertensive crisis in the medical emergency department of Reference National Teaching Hospital in N'Djamena. Patient and methods This was a prospective cohort study running from 1er March 2020 to October 31 2020. Patients presenting with a sudden and severe rise in blood pressure (systolic ≥ 180 mmHg and/or diastolic ≥ 110 mmHg) with or without acute target-organs damage, had been consecutively included and followed up over a period of one (01) month. Epidemic and clinical characteristics on admission, and morbidity and mortality parameters during the course of the disease were collected. The Kaplan-Meier method and the Cox model were used to analyze survival and factors associated with death, with a significance level of p<0.05. Results Of the 3978 hypertensive patients admitted to medical emergencies, 252 had a hypertensive crisis, i.e. a prevalence of 6.3%. Two hundred and seventeen (217) patients were included in the study, divided into 149 cases (69%) of hypertensive emergency and 67 cases (31%) of hypertensive hypertensive urgencies. The mean age of the patients was 55.2 ± 14 years (20 and 80 years) and 67% were male. Hypertension was known in 138 patients (64%). At least one complication was present on admission in 69% of patients. Complications were classified as cardiac (50.7%), neurological (38.2%), kidney impairment (46.5%) and ocular (46.1%). The average number of antihypertensive drugs used was 2 ± 0.83 1, 4. Calcium antagonists (86.5%), diuretics (35.5%), converting enzyme inhibitors or angiotensin II receptor antagonists (33.3%) and betablockers (18%) were the pharmacological classes prescribed. Good compliance during follow-up was observed in 124 patients. One-month survival was 84% for all patients, with a 16% mortality rate. Factors associated with death were the duration of hypertension, and the occurrence of cardiovascular, renal dysfunction and ocular disease (p < 0.05). Conclusion Hypertensive crisis is a frequent pathology in sub-Saharan Africa, with high morbidity and mortality. Prevention requires early detection and effective management of hypertension.

Abstract:Objectives: Achieving the Sustainable Development Goals will require data-driven public health action. There are limited publications on national health information systems that continuously generate health data. Given the need to develop these systems, we summarised their current status in low-income and middle-income countries. Setting: The survey team jointly developed a questionnaire covering policy, planning, legislation and organisation of case reporting, patient monitoring and civil registration and vital statistics (CRVS) systems. From January until May 2017, we administered the questionnaire to key informants in 51 Centers for Disease Control country offices. Countries were aggregated for descriptive analyses in Microsoft Excel. Results: Key informants in 15 countries responded to the questionnaire. Several key informants did not answer all questions, leading to different denominators across questions. The Ministry of Health coordinated case reporting, patient monitoring and CRVS systems in 93% (14/15), 93% (13/14) and 53% (8/15) of responding countries, respectively. Domestic financing supported case reporting, patient monitoring and CRVS systems in 86% (12/14), 75% (9/12) and 92% (11/12) of responding countries, respectively. The most common uses for system-generated data were to guide programme response in 100% (15/15) of countries for case reporting, to calculate service coverage in 92% (12/13) of countries for patient monitoring and to estimate the national burden of disease in 83% (10/12) of countries for CRVS. Systems with an electronic component were being used for case reporting, patient monitoring, birth registration and death registration in 87% (13/15), 92% (11/12), 77% (10/13) and 64% (7/11) of responding countries, respectively. Conclusions: Most responding countries have a solid foundation for policy, planning, legislation and organisation of health information systems. Further evaluation is needed to assess the quality of data generated from systems. Periodic evaluations may be useful in monitoring progress in strengthening and harmonising these systems over time.

Abstract:Background: The epidemiology of emergency care globally is largely unknown, especially in developing countries, because of a lack of common taxonomy, an absence of robust reporting mechanisms, and because emergency medicine is not always recognised as a subspecialty in its own right. Other disease-specific and specialty-specific research networks have addressed these barriers and brought about improved health of populations as a result. The University of Michigan's Department of Emergency Medicine is developing an international emergency-department research network to study the epidemiology of emergency visits in developed and developing countries. Our objectives are: to describe the first steps taken in creation of an international emergency-department research network; to discuss lessons learned; and propose a feasibility pilot study to determine epidemiology of emergency department visits across ten emergency departments in eight countries. Methods: We contacted emergency departments that have a training programme for emergency medicine and have an existing collaboration with the University of Michigan's Global Reach programme in China, India, Ghana, Brazil, Taiwan, Austria, Finland, and the USA. We conducted a survey of research-related resources and electronic data access to determine whether sites were suitable for inclusion in the network. Finally, we proposed a feasibility study to measure interest in the project at sites as well as their ability to participate. Findings: We noticed substantial variation between sites with respect to: the emergency department triage process; access to consultants; definitions of levels of severity of illness; management of resuscitation; and discharge disposition, even for the same condition. We developed a manual of operations, standard taxonomies, and are working on Institutional Review Board approval and data transfer agreements. We will be enrolling patients in prospective and retrospective studies with a sparse simultaneous-sampling design over a period of 6 months. The data collected will have ongoing quality checks to confirm validity and reliability. Interpretation: Despite some substantial barriers, development of an infrastructure to study emergency care at a global level is feasible. The sustainability plan is to create a robust network that nurtures future investigators, identifies funding sources, and works on collaborative research. We anticipate challenges in collecting baseline data on emergency department presentations, so have designed two pathways to collect data: a retrospective Electronic Health Record query; and prospective data collection in real-time. We are planning an in-person meeting to establish standard processes for research priorities and resource allocation. Funding: None.

Abstract:Background: The Ministry of Health in Côte d'Ivoire and the International Training and Education Center for Health at the University of Washington, funded by the United States President's Emergency Plan for AIDS Relief, have been collaborating to develop and implement the Open-Source Enterprise-Level Laboratory Information System (OpenELIS). The system is designed to improve HIV-related laboratory data management and strengthen quality management and capacity at clinical laboratories across the nation. Objective: This evaluation aimed to quantify the effects of implementing OpenELIS on data quality for laboratory tests related to HIV care and treatment. Methods: This evaluation used a quasi-experimental design to perform an interrupted time-series analysis to estimate the changes in the level and slope of 3 data quality indicators (timeliness, completeness, and validity) after OpenELIS implementation. We collected paper and electronic records on clusters of differentiation 4 (CD4) testing for 48 weeks before OpenELIS adoption until 72 weeks after. Data collection took place at 21 laboratories in 13 health regions that started using OpenELIS between 2014 and 2020. We analyzed the data at the laboratory level. We estimated odds ratios (ORs) by comparing the observed outcomes with modeled counterfactual ones when the laboratories did not adopt OpenELIS. Results: There was an immediate 5-fold increase in timeliness (OR 5.27, 95% CI 4.33-6.41; P<.001) and an immediate 3.6-fold increase in completeness (OR 3.59, 95% CI 2.40-5.37; P<.001). These immediate improvements were observed starting after OpenELIS installation and then maintained until 72 weeks after OpenELIS adoption. The weekly improvement in the postimplementation trend of completeness was significant (OR 1.03, 95% CI 1.02-1.05; P<.001). The improvement in validity was not statistically significant (OR 1.34, 95% CI 0.69-2.60; P=.38), but validity did not fall below pre-OpenELIS levels. Conclusions: These results demonstrate the value of electronic laboratory information systems in improving laboratory data quality and supporting evidence-based decision-making in health care. These findings highlight the importance of OpenELIS in Côte d'Ivoire and the potential for adoption in other low- and middle-income countries with similar health systems.
Challenges for work in healthcare: comparative study on University Hospitals in Algeria, Brazil and France

Abstract:This study compares the challenges for work in emergency services of publicuniversity hospitals in Algeria, Brazil and France. The description and analysis are organized in three topics: context and trajectory of the health systems; hospitals and emergency services; and the challenges that are faced. The research carried out interviews, surveys, observation and "groupes de rencontre du travail" / GRT. The data analysis was done using participatory appraisal techniques associated to triangulation of sources and data. The main challenges found were: workforce deficit; lack of hospitals beds in inpatient units; deficit of infrastructure and materials; excess of "chronophagic activities"; generational transition; and violence by patients and families.Despite their particularities, the countries coincide regarding the challenges. Measures to rationalize and restrain spending have a greater impact on Algeria and Brazil due to the low level of public funding, but they also occur in France. The hospital management cannot be dissociated from healthcare system planningconsidering the increasing pressures of the demographic and epidemiological transition. In medium term, measures that may mitigate "chronophagic activities", materials deficit and the violence should be considered to improve work in emergencies.
A Cross-sectional Study of Self-Perceived Educational Needs of Emergency Nurses in Two Tertiary Hospitals in Nairobi, Kenya

Abstract:Background: Many low- and middle-income countries lack resources for well-functioning emergency care systems. Emergency nurses interact with injured and critically ill patients as the first contact in many health care settings. However, insufficient training limits nurses from providing ideal emergency care. The purpose of this research was to highlight educational needs specific to nurses working in 2 emergency departments in Nairobi, Kenya. Methods: A descriptive cross-sectional study involving emergency units of 2 of the largest referral and teaching hospitals (Aga Khan University Hospital, Nairobi, and Kenyatta National Hospital) in Nairobi, Kenya, was conducted. Data were collected by using an adapted structured, self-administered questionnaire. The data were analyzed using descriptive statistics. The skills and competencies of the participants were assessed. In addition, the educational gaps and needs of the participants around emergency care such as trauma, cardiovascular diseases, and respiratory and neurological illnesses were described. Results were presented in frequencies and percentages. Results: The questionnaire response rate was 63.6% (n = 84). Most of the respondents held associate degrees in nursing (72.6%), whereas 19% had a bachelor's degree in nursing. Most respondents (84.5%) perceived themselves as being highly competent in basic skills such as performing cardiopulmonary resuscitation and assessment of body systems. Less than half of the respondents (48.8%) perceived themselves as being highly competent in intermediate skills such as assisting with endotracheal intubation. In advanced competencies, such as analyzing electrocardiograms and administering thrombolytic medications, only 16.7% perceived themselves as highly competent. Conclusion: The results of this study suggest there is a knowledge gap and educational needs among emergency nurses in Nairobi, Kenya. It identified injuries/trauma; cardiovascular, respiratory, and neurological disease; and other emergencies as topics of focus areas with a high need. To address these knowledge and skills needs, a future specialty training in emergency nursing is recommended and this could be achieved through continuing professional development and short courses or postgraduate-level training.
Emergency preparedness capacity of a university hospital in Ghana: a cross-sectional study

Abstract:Background: The health and safety of people are often endangered during emergencies and disasters. Efficient emergency management systems ensure that mitigation, preparedness, response, and recovery actions exist to preserve the health, safety, and welfare of the public. Failure to carry out appropriate responses can have adverse consequences for both emergency responders and casualties; hence, the need for emergency preparedness. This study sought to assess the state of emergency preparedness capacity of the Kwame Nkrumah University of Science and Technology hospital in Ghana. Methods: A facility-based cross-sectional study was conducted between December 2018 and February 2019 using three guidelines developed respectively by the World Health Organization, the Ministry of Health-Ghana, and the Ghana Health Service. The hospital's emergency preparedness was assessed regarding the emergency policies, plan, protocol, equipment, and medications. Results: Overall, the hospital's emergency preparedness level was weak (57.36%). Findings revealed that the hospital had inadequate emergency equipment, and supplies for emergency care delivery, especially during upsurge. It also did not have an emergency planning committee. There were noticeable deficiencies in some emergency resources such as chest tubes, basic airway supplies, and many emergency drugs. Other vital emergency tools such as pulse oximeter, thermometer, and emergency medications were inadequate. The hospital had a strong emergency plan and policies on assessment (77.8% and 78%) respectively. Conclusion: The Kwame Nkrumah University of Science and Technology hospital is not prepared sufficiently for an emergency surge, and this poses a major health challenge. Emergency items must be made available, and the organization and planning of emergency service provisions must be improved to avoid preventable deaths during an emergency surge.

Abstract:For ensuring success of gene and cell based therapies it is of prime importance to develop technology for non-invasive monitoring of the location and duration of gene expression, distribution and targeting of therapeutically engineered cells and vector particles in vivo. A number of advances have been achieved in high resolution, in vivo imaging methods, such as bioluminescence imaging, Magnetic resonance imaging (MRI), positron emission tomography (PET) and various fluorescence imaging techniques, including fluorescence mediated tomography (FMT) and near infrared fluorescence (NIRF) reflectance imaging. In this review we will discuss different imaging modalities that have found applications in gene and cell therapies of cancer.

Abstract:The management of craniocerebral trauma begins at the site of the accident. This is one of the diseases for which pre-hospital care is well codified. The objective of this study is to report the experience of the Emergency Aid Medical Service (EMAS) of BENIN in the pre-hospital management of craniocerebral trauma. 146 head injuries were cared for. 116 were male. The mean age was 32 years with extremes ranging from 2 to 77 years. Etiology in 68% of patients was a public road accident. And the request for the use of the EMAS was made by highway users for 41% of the injuries. About 5.5% were severe cranio-brain injuries, more than 75% of which had died in intensive care. In 77% of the EMAS interventions, there was an intensive care physician in the team. 34 wounded had been transported in a vacuum mattress. The rigid cervical collar was used in 8 major traumas. All the wounded were transported by ambulance. The duration of hospitalization varied from a few hours in the emergency department to 90 days in the hospital ward. 9 patients died. One of them, who died in an ambulance, was a woman. The prehospital medical care of head trauma in BENIN is still progressing.

Effect of Electrodeposition Conditions on Kirkendall Void Formation between Electrodeposited Cu Film and Sn-3.5Ag Solder

Abstract:Sn-3.5Ag solder on Cu foil and electrodeposited films were reflowed at 260degC for 1 min. After reflow process, all specimens showed typical scallop shape of Cu6Sn5 at the Cu/solder interface. After thermal aging at 150degC, the formation and growth of Kirkendall void and two IMCs, Cu3Sn and Cu6Sn5 showed quite differently on Cu foil and electrodeposited Cu films. In the case of Kirkendall voids, the electrodeposited Cu film showed much more at the IMCs than OFHC Cu foil. The voids in electrodeposited Cu with an additive were clearly much more than that without additive. And also, most voids of electrodeposited Cu with an additive were distributed at the interface between Cu3Sn and Cu, while the voids of electrodeposited Cu without additive were randomly distributed in Cu3Sn layer. For the growth kinetics of Cu6Sn5 and Cu3Sn layer, total thickness and relative fractions of IMCs were measured after thermal aging at 150degC. The ratio of Cu3Sn layer to total IMC was about 50% except for specimen of electrodeposited Cu film with additive. For the electrodeposited Cu film with additive, ratio of Cu3Sn to total IMC decreased with the aging time. It is clear that electrodeposition condition of Cu film influences interfacial reaction and void formation behavior between Cu and solder. The drop impact test showed that electrodeposited Cu film with additive degraded drastically with aging time. Fracture occurred at the Cu/Cu3Sn interface, where a lot of voids existed. In contrast, Cu foil showed much more reliable although brittle fracture occurred at the Cu6Sn5/Cu3Sn interface. Therefore, voids occupied at the Cu/Cu3Sn interface are shown to be degraded seriously drop reliabilities of solder joints.
Advancing access to care: An assessment of the prehospital system in Senegal

Abstract:Background Most low‐ and middle‐income countries do not have a mature prehospital system limiting access to definitive care. This study sought to describe the current state of the prehospital system in Senegal and offer recommendations aimed at improving system capacity and population access to definitive care. Methods Structured interviews were conducted with key informants in various regions throughout the country using qualitative and quantitative techniques. A standardized questionnaire was generated using needs assessment forms and system frameworks. Descriptive statistics were performed for quantitative data analysis, and qualitative data was consolidated and presented using ATLAS.ti. Results Two (20%) of the studied regions, Dakar and Saint‐Louis, had a mature prehospital system in place, including dispatch centers and teams of trained personnel utilizing equipped ambulances. 80% of the studied regions lacked an established prehospital system. The vast majority of the population relied on the fire department for transport to a healthcare facility. The ambulances in rural regions were not part of a formal prehospital system, were not equipped with life‐support supplies, and were limited to inter‐facility transfers. Conclusions While Dakar and Saint‐Louis have mature prehospital systems, the rest of the country is served by the fire department. There are significant opportunities to further strengthen the prehospital system in rural Senegal by training the fire department in basic life support and first aid, maintaining cost efficiency, and building on existing national resources. This has the potential to significantly improve access to definitive care and outcomes of emergent illness in the Senegalese community.

Abstract:Background: Integrating the principles of humanized care into healthcare systems is critical to promoting optimal well-being throughout the patient care process. In a context of limited resources, improving the quality of care and health services means identifying priority sites to optimize cost-effectiveness. The objective was to measure key indicators of quality and safety of care in health facilities (HFs) and to map priority health districts (HDs) for effective implementation of high-impact interventions in the Centre Region. Methods: We conducted a descriptive cross-sectional analysis using retrospective data from 2018 to 2022 from 32 HDs comprising the Centre Region were extracted in April 2023. Data were retrieved from the national database using DHIS 2 and checked for completeness. The health indicators assessed were those defined in the Cameroon Health Sector Strategy for 2020-2030. Results: The density of the qualified HCWs was 13.7 per 10 000 inhabitants in 2022. The density of doctors in rural areas has fallen sharply over the five-year study period, with a decline of almost 20% by 2020 in favor of urban areas. The Odza HD had the highest inpatient mortality rate in the center over the five-year study period (49%). In rural areas, the Esse HD had the highest mortality rate (32.4%). Esse (18.8%) and Odza (6.7%) HDs had the highest perioperative mortality rates in rural and urban areas, respectively. Most of the urban HDs in the Centre Region (5/8=62.5%) had a neonatal mortality rate higher than the regional median (RM=3.1). Half of the rural HDs had a maternal mortality ratio higher than the regional median (118 per 1000 live births). The HDs of Mbandjock (357/1,000) and Mvog-Ada (285/1,000) were the most affected in rural and urban areas respectively. More than two thirds of urban HDs recorded a proportion of free malaria treatment cases below the regional median (14%). Conclusions: Efforts to humanize healthcare in the Centre Region are hindered by persistent disparities in care quality and inconsistent policy implementation. Addressing these challenges requires timely implementation of a National Strategic Plan to Improve the Quality of Healthcare and Service Delivery, emphasizing on capacity building through training and supervision.

Abstract:Background and objective: Most countries in sub-Saharan Africa need to catch up in integrating information and communication technologies (ICT) into their health systems. This is mainly because of the need for more infrastructure that allows for reasonable use of the technologies. To support the actions of the Ministry of Health of Burkina Faso, a Non-governmental Organization (NGO) has implemented the integrated electronic diagnostic approach (IeDA) Project. The project includes the deployment of an electronic consultation register (ECR). This article aims to explore the perceptions of healthcare providers on the benefits and disadvantages of using the ECR. Methods: We conducted a qualitative, descriptive study through individual semi-structured interviews with healthcare providers. Data were collected in the Toma health district in December 2021. In addition, a thematic analysis was performed using NVivo software. Results: Thirty-five healthcare workers were interviewed (19 nurses, 7 midwives, 6 mobile community health and hygiene workers, and 3 birth attendants). Two main themes emerged from our analyses, which are the advantages and disadvantages perceived by ECR users. Our data suggest that using the ECR had many benefits ranging from improving healthcare providers' knowledge and performance in terms of patients' care, assisting and helping in patient diagnosis and treatment and improving patient satisfaction. However, the participants also shared their negative perceptions about the ECR, mentioning that it increased their workload. They also reported lengthened consultation time and work duplication as the tool was still in its trial phase and was used along with the paper consultation register. Conclusion and global health implications: The ECR is an effective tool for diagnosis and management, which has several advantages and reasonably satisfies patients. However, disadvantages, including increased workload and lack of fluidity and stability of the system, must be considered to ensure better usability.
