Abstract:Aims: This study aims to compare the proportion of peripheral blood T lymphocyte subsets and related blood cell and bone marrow cytology indexes between patients with aplastic anemia (AA) and hypoplastic myelodysplastic syndrome (hypo-MDS), and investigate the clinical identification significance. Materials and methods: A total of 41 patients with AA and 46 patients with hypo-MDS were collected, and the proportions of peripheral blood T lymphocyte subsets, CD3-CD16/CD56+NK cells, CD3+CD57+T-LGL cells and CD19+B lymphocytes were detected by flow cytometry. Key findings: The CD4+/CD8+ ratio decreased in the AA and hypo-MDS groups, and the difference was statistically significant when compared with that in the control group (P<0.05). However, there was no significant difference between AA and hypo-MDS groups (P>0.05). The proportions of CD3-CD16/CD56+NK cells and CD3+CD57+T-LGL cells in the hypo-MDS group were distinctly higher than those in the AA group (P<0.05). However, the proportion of CD19+B lymphocyte was obviously lower than that in the AA group (P<0.05). Furthermore, the proportions of reticulocytes, bone marrow progenitor cells and immature red blood cells in the hypo-MDS group were significantly more than those in the AA group (P<0.05), and the proportion of mature lymphocytes in the hypo-MDS group was distinctly lower than that in the AA group (P<0.05). Significance: Changes of T lymphocyte subsets and the proportions of large granular lymphocytes and B lymphocytes can be utilized as indexes in the differential diagnosis between AA and hypo-MDS.

Abstract:Introduction Aplastic anemia (AA) and hypoplastic myelodysplastic syndrome (MDS‐h) are bone marrow failure disease and difficult to distinguish merely by morphological analysis. In this study, we investigated the value of flow cytometry (FCM) in the differential diagnosis of AA and MDS‐h. Methods We included 822 patients (626 control, 69 AA, 22 MDS‐h and 105 dilution patients) from January 2017 to December 2022 for a retrospective study. Bone marrow myeloid progenitor (MP) cell and mature lymphocytes proportions were analyzed by FCM. The ratio of MP cell proportion and mature lymphocytes proportion, MPLR, was calculated. Data were compared by Kruskal–Wallis test. Differential diagnostic efficacy was evaluated by receiver operating characteristic (ROC) curve. Cutoff value was determined by the maximum Youden index. Results Bone marrow MP cell proportion and MPLR of MDS‐h patients were higher than AA patients. Mature lymphocytes proportion of MDS‐h patients was lower than AA patients. Area under ROC curve (AUC of ROC) of MP cell proportion, MPLR and mature lymphocytes proportion to distinguish AA from MDS‐h were 0.992, 0.988, and 0.850, respectively. Moreover, MPLR of dilution patients was higher than AA patients but lower than MDS‐h patients. The AUC of ROC curves of MPLR to distinguish MDS‐h and AA from dilution were 0.854 and 0.871, respectively. Conclusion Bone marrow MP cell proportion and MPLR can effectively discriminate AA from MDS‐h with similar differential efficacy, which is higher than mature lymphocytes proportion. Moreover, MPLR can evaluate the quality of bone marrow aspirates, which would interfere with the differential diagnosis.

Abstract:Objective To study the biological characters of hypoplastic myelodysplastic syndrome (Hypo-MDS). Methods The incidence, clinical features, bone marrow aspiration, bone marrow biopsy, CD34 expression and karotype of patients with Hypo-MDS were studied and compared with those of normo/hypercellular MDS and aplastic anemia. Results There were significant differences in megakaryocyte, CD34 expression and abnormal karotype among Hypo-MDS, normo/hypercellular MDS and aplastic anemia. Conclusion Hypo-MDS has unique biological features and may be a new subtype of myolodysplastic sydrome.
Clinical and Biological Characteristics of Acute Myeloid Leukemia with 20-29% Blasts: a Retrospective Single-Center Study

Abstract:It is controversial whether acute myeloid leukemia (AML) patients with 20-29% bone marrow (BM) blasts should be considered AML or myelodysplastic syndromes (MDS). We retrospectively studied 382 patients, including 108 AML with 20-29% BM blasts (AML20-29), 210 AML with >= 30% BM blasts (AML >= 30), and 64 MDS with 10-19% BM blasts (MDS-EB2). We found that AML20-29 were more similar to MDS-EB2 in terms of advanced age, less blood count, the increased presence of poor-risk cytogenetics. The frequency of mutated genes in AML20-29 had both the characters of AML and MDS. Median overall survival of AML20-29 and MDS-EB2 were similar and shorter than those of AML >= 30 (p = .045). Multivariate analysis showed inferior survival with increased age, low platelet count and FLT3 mutations. Our findings suggest that AML20-29 have clinical features more similar to MDS than AML.

Abstract:Myelodysplastic syndrome (MDS) is a clonal stem cell disorder characterized by ineffective hematopoiesis and frequent leukemia progression. Cytogenetic abnormalities are presented in MDS patients and predict poor prognosis. To better define their relevance to clinical outcome in Chinese patients with primary MDS, cytogenetic analysis was performed in 508 cases and retrospectively compared with clinical data. The median age was 49 years which was 10 years younger than that of western patients. The median overall survival was 34 (range 1–161) months, with the 2-year and 5-year survival rate of 50.0% and 15.0%, respectively. Among 367 cases with evaluable karyotype in bone marrow mononuclear cells, cytogenetic abnormalities were found in 136 of them (37.1%), including 56 cases (15.3%) of numeric abnormality and 80 cases (21.8%) of structural abnormality. Higher incidence of cytogenetic abnormality was found in RAEB-T (43/86, 50.0%) and RAEB (44/106, 41.5%) patients than that of RA patients (38/143, 26.6%). RAEB and RAEB-T had more cases of non-hypodipoidy, double aberrations with translocation, and complex aberrations. In RA, the most frequent aberrations were duplication and numeric changes. This study also showed RAEB appears to be the course of MDS that genome turbulence was under the evolution pressure. In RAEB-T, the genome abnormalities tend to be uniform and behaved as special translocation, which is in accordance with the clinical behavior of AL. Poor prognosis and early leukemia transformation were observed in the patients with cytogenetic abnormalities, especially with complex cytogenetic aberrations and chromosome translocations. In conclusion, cytogenetic abnormalities possess strong predictive value on disease outcome in primary MDS patients.

Abstract:OBJECTIVE:To explore the implication of karyotype analysis in diagnosis and prognosis of myelodysplastic syndrome (MDS).METHODS:The chromosomes were prepared with direct method, brief culture of cells and R-banding techniques, and then the karyotypic analysis was performed.RESULT:Seventy-seven out of 283 patients (27.21%) had karyotypic abnormalities, including the numeral abnormalities of chromosomes and structural alterations. The most common chromosomal aberrations were +8, -20/20q-, -Y, translocation, -7/7q-, +9, -5/5q-. The rate of abnormal karyotype in refractory anemia with erythroblasts (RAEB) and refractory anemia erythroblasts-transformation (RAEB-t) was much higher than in refractory anemia (RA). Patients with abnormal karyotype or higher IPSS scores had a higher risk of transformation into acute leukemia than patients with normal karyotype or lower IPSS scores (P<0.05).CONCLUSION:MDS is a highly heterogenous disorder and karyotype analysis is helpful for its diagnosis and prognosis estimation.
Analysis of the Karyotype Abnormalities and Its Prognostic in 298 Patients with Myelodysplastic Syndrome

Abstract:OBJECTIVE:To investigate the relationship between cytogenetic markers with World Health Organization (WHO) classification, disease progress and prognosis in cases with primary myelodysplastic syndromes (MDS).METHODS:298 patients with de novo MDS from the first affiliated hospital of medical school, Zhejiang University were enrolled in the retrospective analysis of WHO classification, karyotype, and prognosis. Follow-up study was also conducted.RESULTS:The WHO classifications at first diagnosis were as follows: refractory cytopenia with unilineage dysplasia (RCUD), 18 cases; refractory anemia with ring sideroblasts (RARS), 8 cases; refractory cytopenia with multiline dysplasia (RCMD), 104 cases; refractory anemia with excess blasts-1, 76 cases; refractory anemia with excess blasts-2, 85 cases; MDS unclassified (MDS-U), 5 cases involved; and single del (5q), 2 cases. 39.6% of MDS patients carried karyotypic abnormalities. Among them, the frequency of numerical abnormalities, structural abnormalities and the existence of composite abnormalities were 45, 31, and 42, respectively. The composite abnormalities were unbalanced translocations and complex chromosomal abnormalities. The incidence of both karyotypic abnormalities and complex chromosomal abnormalities in RAEB group was higher than that in non-RAEB group (P<0. 05). An analysis based on IPSS-R Scoring System showed that advanced risk stratification (except the low-risk group) gradually enhanced the incidence of karyotypic abnormalities (P<0.05). In addition, the probability of evolution to leukemia increased with the higher IPSS-R score (P<0.05). In RAEB group, the cases with +8 chromosome, accounting for 19.5% of karyotypic abnormalities, had worse prognosis than those with normal chromosomes.CONCLUSION:Karyotype was identified with an independent risk factor in MDS patients. Therefore, the information on cytogenetic analysis was critical for diagnosis, prognosis and individual treatment. MDS patients presenting+8 chromosome, an intermediate risk factor, were associated with a poorer outcome compared to cases with normal chromosomes in RAEB group.

Abstract:Objective To explore the immunological pathogenesis of aplastic anemia (AA) in children. Methods To determine the CD3+?CD4+?CD8+ T-cell subgroups in peripheral blood of 32 children with aplastic anemia and 20 healthy children by flow cytometer (FCM).Results AA group had higher levels of CD8+ T-cell and a lower ratio of CD4+/CD8+ comparing to the control group (P0.05).Conclusion The increase of CD8+ levels and the decrease of CD4+/CD8+ ratio may be associated with the immunological pathogenesis of aplastic anemia in children.
Performance of Automatic Blood Cell Analysis Parameters for Differential Diagnosis of Myelodysplastic Syndromes and Aplastic Anemia

Abstract:Objective To evaluate the performance of automated blood cell analysis parameters for differential diagnosis of myelodysplastic syndromes (MDS) and aplastic anemia (AA).Methods Data of automatic blood cell analysis parameters at diagnosis of confirmed patients with MDS and AA from December 2002 to February 2011 in Peking University Shenzhen Hospital were retrospectively reviewed.Results 33 cases of MDS and 36 cases of AA were recruited in this study.Based on the evaluable data,mean corpuscular hemoglobin concentration (MCHC) (328.58 ± 17.24 g/L vs 342.47±18.75 g/L,n=33/36) was significantly lower (P=0.002 1),while monocyte percentage (MONO％) (11.48±9.99 vs 6.94±2.50,n=32/34),platelet distribution width (PDW％) (13.51±4.24 vs 10.62±3.68,n=20/22) and platelet hematocrit (PCT％)(0.11 ±0.10 vs 0.04±0.07,n=11/15) were markedly higer (all P＜0.05) in patients with MDS than that of AA.No significantly differences for other blood cell analysis parameters were seen between patients with MDS and AA.Under the condition of best cut-off value,areas under the ROC curve of MCHC,MONO％,PDW and PCT were 0.706 (95％ confidence interval:0.584～0.809),0.666 (0.540～0.778),0.668 (0.506～0.805) and 0.745 (0.538～0.894) respectively.MONO％ and MCHC had high specificities (97.06％ and 88.89％) and positive predictive values (93.3％ and 80.0％) for differential diagnosis of MDS from AA.Conclusion MONO％ and MCHC may be used as simple indicators for differential diagnosis of MDS and AA.
A flow cytometric approach to compare stem cell apoptosis in aplastic anemia and hypoplastic myelodysplastic syndrome

Abstract:Background and objectives: Aplastic anemia (AA) is a disease caused by bone marrow (BM) failure. There are many similarities between AA and hypoplastic myelodysplastic syndrome (MDS); hence, differentiating them could be problematic. The current study aimed to use the new technique of flow cytometry as a possible diagnostic tool for AA and hypoplastic MDS. Patients and methods: The BM mononuclear cell (BMMC) and blood samples from 44 participants (17 patients with AA, 13 with hypoplastic MDS, and 14 healthy controls) were collected. The flow cytometric analysis of the cluster of differentiation 34 (CD34) levels and cell apoptosis was performed for all sample types. Results: Patients with hypoplastic MDS showed a high percentage of CD34+ cells with low apoptosis, while those with AA showed a low percentage of CD34+ cells with high apoptosis. Conclusions: Despite the similarity in the clinical presentation of hypoplastic MDS and AA, they are biologically different disorders. Increased CD34+ cell numbers with high viability may provide a useful and accurate tool for the differential diagnosis of hypoplastic MDS from AA.
[Quantitative Analysis of Dendritic Cell Subsets in Bone Marrow of Patients with Acute Myeloid Leukemia].

Abstract:In order to study the quantity of dendritic cell (DC) subsets of bone marrow in patients with acute myeloid leukemia (AML), the bone marrow aspirate were collected from 77 newly diagnosed AML patients and from 30 healthy persons. The quantity of DC subsets (myeloid dendritic cells, mDC and plasmacytoid dendritic cells, pDC) were detected by flow cytometry and analysed by 3-color and 4-color cytometric gate. Based on the conventional 3-color panel, mDC were identified by Lin-HLA-DR+CD11c+ and pDC were identified by Lin-HLA-DR+CD123+. Based on the 4-color panel, mDC were identified by Lin-HLA-DR+CD11c+ BDCA-1+ and pDC were identified by Lin-HLA-DR+CD123+BDCA-2+. The results showed that a reduction of mDC was found in 74.0% (57/77) and 58.4% (45/77) patients, a reduction of pDC was found in 90.9% (70/77) and 46.8% (36/77) patients respectively by 3-color and 4-color cytometric analysis. Meanwhile an expansion of mDC was showed in 19.5% (15/77) and 22.1% (17/77) patients, an expansion of pDC was showed in 1.3% (1/77) and 27.3% (21/77) patients respectively by 3-color and 4-color cytometric analysis. In subtypes of AML-M2, AML-M3 or AML-M4/5, 81.4%, 100% and 42.1% patients showed mDC decrease and 88.4%, 100% and 89.5% patients showed pDC decrease respectively by 4-color cytometric analysis. It is concluded that the 4-color cytometric gate is better method for detection of mDC and pDC from bone marrow of newly diagnosed AML patients as compared with 3-color cytometric gate, the majority of AML patients showed reduction of mDC and pDC. The percentages of patients with mDC normal or mDC increase in AML-M4/5 subtypes are more than that in AML-M2/3 subtypes, while the pDC does not show difference between AML subtypes.
[Expressions of CD117 and CD11b in Patients with APL at Diagnosis and Post-Treatment].

Abstract:The aim of this study was to evaluate the value of CD117/CD11b phenotypic analysis to diagnosis and prognosis of acute promyelocytic leukemia (APL). Three- or four-color flow cytometry with a series of 22 monoclonal antibodies and CD45/Side Scatter (SSC) gating strategy were used to identify immunophenotypic characteristics of APL as compared to CML in chronic phase (CML-CP). PML/RAR alpha fusion gene was detected by using reverse-transcription polymerase chain reaction (RT-PCR) technique. The results showed that MPO, CD13 and CD33 were almost expressed in all patients with APL and CML-CP whereas HLA-DR and CD34, the hematopoietic progenitor cell markers, were rarely expressed. The positive rate of CD15 in APL was significantly lower than those in CML-CP (P < 0.01). CD117 was positive in 78.3% of the APL cases and in none of the cases of CML-CP. On the other hand, CD11b was almost positive in all cases of CML-CP, but only 16.9% of the APL cases were found positive for this antigen. The CD117+ CD11b- phenotype was present in 72.3% of APL cases while none of cases with CML-CP with this phenotype. Almost all of the cases with CML-CP had the phenotype of CD117- CD11b+. CD117- CD11b+ phenotype was detected in all patients recovering from APL with CD117+ CD11b- phenotype at diagnosis and after treatment with all-trans-retinoic acid (ARTA) for 2 months. PML/RAR alpha fusion gene was positive in 80.6% (25/31) of the APL cases, of which, 64% of the cases belonged to the type L while only 36% of the cases were showed type S for this fusion gene. The positive rates of CD117 were 87.5%, 44.4% and 33.3% in type L group, S group and negative group respectively. It is concluded that analysis of both CD117 and CD11b phenotype may be helpful to the diagnosis, therapy and prognosis of APL in children and adults and to differentiation of APL from recovering benign myeloid proliferation.
Analysis of Mean Corpuscular Volume and Red Cell Distribution Width in Patients with Aplastic Anemia

Abstract:To explore the characteristics of hemogram in patients with aplastic anemia (AA), especially mean corpuscular volume (MCV) and red cell distribution width (RDW). We examined the blood routine of 180 new-onset AA patients and used 166 patients with myelodysplastic syndrome (MDS) as controls. Among the 180 AA patients, 105 (58.3%) were diagnosed with severe AA (SAA), while 75 (41.7%) were diagnosed with non-severe AA (NSAA). Compared to MDS, patients with SAA generally had unfavorable hemogram, including significantly lower white blood cell (WBC), absolute neutrophil count (ANC), hemoglobin (Hb), platelet (PLT) and reticulocyte counts (RET). However, WBC, ANC and lymphocyte counts were higher in the NSAA group than in the MDS group; Hb and Ret were comparable between the two groups. 8.5% of SAA patients and 58.1% of NSAA patients presented with macrocytic anemia, whereas 25.7% of SAA and 64.0% of NSAA had a high RDW. In the MDS group, 54.7% of patients presented with macrocytic anemia, and 84.7% had increased RDW. WBC, ANC, PLT, and Ret in a high-RDW group (25.7% of SAA) were significantly higher than in a normal-RDW group (74.3% of SAA). Overall, most SAA patients exhibited normocytic–normochromic anemia, and their hemograms decreased more significantly; more than half of NSAA patients showed macrocytic-heterogeneous anemia, and their hemograms were similar to those of MDS. Patients with elevated RDW may have better residual bone marrow hematopoietic function than those with normal RDW but with more severe anemia.

Abstract:Objective To explore the significance of the number of circulating CD34+ cells in the peripheral blood from patients with myelodysplastic syndromes(MDS) on disease subtype and prognosis.Methods The percentage and absolute count of circulating CD34+ cells of the 16 healthy individuals and 40 patients with MDS were measured by flow cytometry.Forty patients with MDS were divided into RA/RARS/RCMD subgroup or RAEB Ⅰ/RAEB Ⅱ subgroup;favorable chromosomal subgroup,intermediate chromosomal subgroup and poor chromosomal subgroup;intermediate-risk Ⅰ subgroup,intermediate-risk Ⅱ subgroup and high-risk subgroup according to WHO classification,chromosomal abnormalities and international prognostic scoring system(IPSS).Results The percentage and the absolute count of circulating CD34+ cells in peripheral blood from patients with MDS were 0.67% and 17.24 cells/μl,while they were 0.03% and 1.63cells/μl in normal control(both P<0.01).In MDS group: the percentage and the absolute count of CD34+ cells in RA/RARS/RCMD subgroup were 0.05% and 3.50 cells/μl,which were lower than 3.09% and 81.95 cells/μl in RAEB Ⅰ/RAEB Ⅱ subgroup(both P<0.01).The percentage and the absolute count of CD34+ cells in favorable chromosomal subgroup were 0.05% and 3.50 cells/μl,while they were 1.29% and 18.23 cells/μl in intermediate subgroup and 3.09% and 133.10 cells/μl in poor chromosomal subgroup,respectively.With the emergence of chromosomal abnormalities,the percentage and absolute count were gradually increased(P<0.05).The percentage and the absolute count of CD34+ cells in intermediate-risk Ⅰ(0.05% and 3.50 cells/μl),intermediate-risk Ⅱ(1.57% and 35.55 cells/μl) and high-risk subgroup(8.15% and 192.05 cells/μl) were gradually increased with the progressive increase of IPSS score(all P<0.01).Conclusion Detection of the number of circulating CD34+ cells in peripheral blood cells may be a useful tool for subtyping and predicting the prognosis of MDS.
Reduced Regulatory Effects of Bone Marrow-Derived Mesenchymal Stem Cells on Activated T Lymphocytes and Th1/Th2 Cytokine Secretion in Children with Aplastic Anemia.

Abstract:Acquired aplastic anemia (AA) is a recognized immune-mediated disorder and abnormally activated T lymphocyte-mediated bone marrow destruction is considered to be its main pathogenesis. Whether abnormal activation of T lymphocytes would also damage bone marrow-derived MSCs remains to be further studied. The aim of this study was to analyze the extent of T lymphocyte activation and the levels of Th1/Th2 cytokines of AA patients, and to explore the immunomodulatory effects of BM-MSCs on IL-2-stimulated T lymphocyte activation and cytokine production in vitro by means of transwell co-culture assay and flow cytometry measurement. The intermediate (CD25+) activated T cells were dominant in peripheral blood, while the early (CD69+) and late (HLA-DR+) activated T cells were predominant in bone marrow. Severe AA patients have an obviously higher proportion of CD3+CD8+CD69+ T cells than NSAA cases. The levels of IL-2 and IL-6 in AA patients were slightly elevated and INF-γ was mildly decreased in comparison with normal individuals. BM-MSCs derived from AA could not effectively inhibit the IL-2-induced activation of T cells with higher proportions of CD25+CD3+CD4+, CD69+CD3+CD4+ and CD25+CD3+CD8+ T cells after co-culture, and they showed a decreased ability to balance the Th1/Th2 cytokine production. Moreover, they had less robust osteogenic differentiation and more prone to adipogenic differentiation. We concluded that abnormally excessive T cell activation accompanied by abnormal cytokine secretion may impair the function of BM-MSCs in children with aplastic anemia.

Abstract:OBJECTIVE:To evaluate the value of flow cytometry (FCM) for the differential diagnosis between myelodysplasia (MDS) subtype refractory cytopenia with multiple dysplasia (RCMD) and aplastic anemia (AA).METHODS:The flow cytometric data of bone marrow samples from 168 cases of RCMD and 77 cases of AA were analyzed retrospectively in blind, and its results were compared with gold standard to evaluate its diagnosis values.RESULTS:The specificity of abnormal of single immunophenotype in the surface of granulocytes and myeloblasts was high (range 75.3%-100%), but the sensitivity was very low (range 5.4%-50%). In parallel tests, the sensitivity and specificity of the combination of CD34+ cells≥1%, myeloblasts≥3%, abnormal expression of CD117 in granulocytes and loss of CD13 in myeloblasts or increased intensity of CD33 in granulocytes were higher than other combinations. The sensitivity and specificity of above combination were more than 62% and 92%, respectively. In the scoring method, different score was given to 8 markers according to different diagnostic value, which were CD34+ cells≥1%, myeloblasts≥3%, abnormal expression of CD117 in granulocytes, loss of CD13 in myeloblasts, increased intensity of CD33 in granulocytes, loss of CD13 in granulocytes, loss of CD10 in granulocytes, and decreased SSC in granulocytes. The sensitivity and specificity were both high if we defined that the total score≥1.5 was RCMD and the score<1.5 was AA.CONCLUSIONS:The value of abnormal of single immunophenotype for differential diagnosis between RCMD and AA is low. Parallel tests can increase the diagnostic sensitivity obviously and not decrease the specificity. CD34+ cells≥1%, myeloblasts≥3% and abnormal expression of CD117 in granulocytes were the most important markers. The scoring method is precise to distinguish RCMD from AA.

Abstract:Absolute lymphocyte count (ALC) after therapy has been reported to be an independent prognostic factor for clinical outcome in leukemia. This study mainly analyzed ALC at interim of therapy on day 22 (ALC-22) and the ratio of ALC-22 to ALC at diagnosis (ALC-0) on the impact of survival and the relation of ALC to lymphocyte subsets in 119 pediatric B-lineage acute lymphoblastic leukemia (B-ALL) patients. Univariate analysis revealed that ALC-22/ALC-0 ratio <10% was significantly associated with inferior overall survival (OS) (hazard ratio (HR) = 12.24, P=0.0014) and event-free survival (EFS) (HR = 3.3, P= 0.0046). In multivariate analysis, ALC-22/ALC-0 ratio remained an independent prognostic factor for OS (HR=6.92, P=0.0181) and EFS (HR=2.78, P=0.0329) after adjusting for age, white blood cell (WBC) count and minimal residual disease (MRD) status. A Spearman correlation test showed that CD3+ T cells had a negative correlation with ALC-0 (r=0.7204, P<0.0001) and a positive correlation with ALC-22 (r=0.5061, P= 0.0071). These data suggest that ALC-22/ALC-0 ratio may serve as a more effective biomarker to predict survival in pediatric B-ALL and ALC is mainly associated with CD3+ T cells. (C) 2014 Elsevier Ltd. Clinical analysis for thirty patients with myelodysplastic syndromes

Abstract:Objective To study the clinical characteristics of patients with myelodysplastic syndromes(MDS).Methods Thirty cases with MDS were analyzed retrospectively.Results(1) There were four cases with refractory anemia(RA),ten cases with refratctory cytopenia with multilineage dysplasia(RCMD),14 cases with refractory anemia with excess of blasts(MDS-RAEB) in which six cases developed into acute monocytic leukemia(AML-M5).(2) After androgen,ATRA,and Chinese medicine(Fufang Zao-fanwan) were used in six cases with MDS-RA,partial remission in one of them,hematological improvement in two of them,non-responses in three of them.(3) Five cases with MDS-RCMD combined with autoimmune diseases achieved hematological improvement after treated with low-dose Predisone(10～15 mg/d) as well as above three drugs.One case with MDS-RCMD achieved complete remission after being treated with Cyclosporine A 5 mg/kg in half an year.(4) One case with MDS-RAEB combined with hypoplasic red cell developed AML-M5 after being treated with Cyclosporine A in three months.Six cases with MDS-RAEB developed AML-M5 in half an year.Three of them gave up treatment and died of pneumonia at last.Two of them achieved non-response after being treated with two course of DA(idarubicin and cytrarabine) regimen,and died of pneumonia.One of them achieved partial remission after being treated with CAG(cytrarabine,aclarubicin and granulocyte colony-stimulating factor) regimen.Conclusion(1) MDS comprises a spectrum of heterogeneous,clonal disorder of hematopoiesis.20% cases of this study finally developed AML-M5.They have low response to chemotherapy and high fitality.Majority of them died of pneumonia.(2) MDS-RCMD combined with autoimmune diseases treated with low-dose Predisone as well as other drugs is improved to have curative effects.(3) Complete remission of patients with low-risk MDS treated with Cyclosporine A suggests that abnormal immune mechanism plays an important role in low-risk MDS patients.

Abstract:The photoaffinity labeling agent 5-azidoindole-3-acetic acid, an analog of the endogenous plant hormone indole-3-acetic acid (an auxin), was used to identify indole-3-acetic acid-binding proteins in maize. Two peptides with subunit molecular masses of 24 and 22 kilodaltons are specifically labeled in a saturable manner. Both peptides are slightly acidic and behave as dimers under nondenaturing conditions. The possibility that one of these peptides is the auxin receptor that mediates cell elongation in maize is discussed.

Abstract:Objective To evaluate peripheral blood circulating CD34+ cell count in patients with myelodysplastic syndrome(MDS)-refractory anemia(RA)/refractory anemia with ring siderblasts(RARS)/refractory cytopenia with multilineage dysplasia(RCMD).Methods A total of 52 MDS-RA/RARS/RCMD patients and 19 hematopoietic stem cell donors(healthy controls) were enrolled,and their peripheral blood circulating CD34+ cell percentages and counts were determined byflow cytometry. They were classified into low risk, intermediate-Ⅰ risk,intermediate-Ⅱ risk,high risk subgroups and very low risk,low risk, intermediate risk,high risk, very high risk subgroups according to the International Prognostic Scoring System(IPSS) and World Health Organization (WHO)-Based Prognostic Scoring System(WPSS),respectively. The correlation of peripheral blood circulating CD34+ cell count with the clinical characteristics of MDS-RA/RARS/RCMD patients was analyzed.Results MDS-RA/RARS/RCMD patients had higher percentage and count of peripheral blood circulating CD34+ cells compared with healthy controls(P<0.01). There was no correlation between peripheral blood circulating CD34+ cell count inMDS-RA/RARS/RCMD patients and patients' sex,age,cytopenia degree,white blood cell count decreasing and abnormal localization of immature precursor(ALIP)(r<0.625,P>0.05),and there was correlation between peripheral blood circulating CD34+ cell count and abnormal karyotype,IPSS score, WPSS score and myelofibrosis (r>0.995,P<0.01). There were 16 MDS-RA/RARS/RCMD patients with peripheral blood circulating CD34+ cell count >10.00×106/L. Among them, there were 12 patients with complex karyotype abnormalities(10 patients with chromosome 7 abnormalities). A total of 12 and 13 patients belonged to IPSS intermediate-Ⅱ risk and WPSS high risk subgroups,respectively. There were 9 patients with myelofibrosis. Among 36 MDS-RA/RARS/RCMD patients with peripheral blood circulating CD34+ cell count <10.00×106/L,no patient had complex karyotype abnormalities or chromosome 7 abnormalities. Except for 1 patient in WPSS high risk subgroup,other patients were in IPSS low,intermediate-Ⅰ risk subgroups and WPSS very low,low and intermediate risk subgroups. Only 1 patient had myelofibrosis.Conclusions The count of peripheral blood circulating CD34+ cells increases in MDS-RA/RARS/RCMD patients,which can be further used for the classification of MDS risk.
