Abstract:The objective of this study was to compare the efficacy and safety of levofloxacin 750 mg for 5 days versus 500 mg for 7-14 days intravenous (IV) in the treatment of community-acquired pneumonia (CAP). This clinical trial was the first of its kind conducted in Chinese people and also in Asian population. A total of 241 were enrolled and randomized to 750 mg group (n = 121) or 500 mg (n = 120) group from 10 study centers. The median treatment duration was 5.0 days in 750 mg and 9.0 days in 500 mg group. The median total dose was 3750 mg in 750 mg and 4500 mg in 500 mg group. The bacterial eradication rate was 100% in both groups. The overall efficacy rate in 750 mg group was 86.2% (94/109), and 84.7% (94/111), in 500 mg group of full analysis set visit 4, 95% confidence interval of 1.6% (-7.8-10.9%); the statistical results showed that 750 mg group was non-inferior to 500 mg group. The most common clinical adverse drug reactions were injection site adverse reactions in both 750 mg group and 500 mg group; the other common adverse drug reactions were insomnia, nausea, skin rash, etc. The most common drug-related laboratory abnormalities were neutrophil percentage decreased, decreased white blood cell count, alanine aminotransferase, and aspartate aminotransferase elevation in both 750 mg group and 500 mg group. Most of adverse drug reactions were mild in severity and well-tolerated. In summary, the regimen of levofloxacin 750 mg IV for 5 days was at least as effective and well tolerated as 500 mg IV for 7-14 days for the treatment of CAP.

Abstract:ObjectivesTo evaluate the efficacy and safety of oral sitafloxacin versus oral moxifloxacin in the treatment of Chinese adults with community-acquired pneumonia (CAP).Patients and methodsThis is a multicenter, randomized, open-label, positive-controlled clinical trial (chinadrugtrials.org.cn identifier: CTR20130046). CAP patients received sitafloxacin tablets 100 mg once daily (qd) or 100 mg twice daily (bid) to compare with moxifloxacin tablets 400 mg qd, for 7-10 days. The primary outcome was non-inferiority of sitafloxacin to moxifloxacin in clinical cure rate at test of cure (TOC) visit in per-protocol set (PPS).ResultsA total of 343 patients were randomized (sitafloxacin 100 mg qd, n = 117; sitafloxacin 100 mg bid, n = 116; moxifloxacin, n = 110), 291 patients were included in the PPS (sitafloxacin 100 mg qd, n = 96; sitafloxacin 100 mg bid, n = 94; moxifloxacin, n = 101). The clinical cure rate was 94.8% in the sitafloxacin 100 mg qd group, 96.8% in the sitafloxacin 100 mg bid group and 95.0% in the moxifloxacin group. At the TOC visit, the microbiological success rate was 97.0% (32/33) in the sitafloxacin 100 mg qd group, 97.1% (34/35) in the sitafloxacin 100 mg bid group and 94.9% (37/39) in the moxifloxacin group in the microbiological evaluable set (MES). The incidence of study-drug-related adverse events (AEs) was 23.3% (27/116) in the sitafloxacin 100 mg qd group, 29.8% (34/114) in the sitafloxacin 100 mg bid group and 28.2% (31/110) in the moxifloxacin group (p > .05). The common AEs related to study drug were dizziness, nausea, diarrhea, increased platelet count and alanine transaminase (ALT) elevation. All the AEs resolved completely after discontinuation of study drug.ConclusionSitafloxacin 100 mg qd or 100 mg bid for 7-10 days is not inferior to moxifloxacin 400 mg qd for 7-10 days in clinical efficacy for adult CAP patients. Sitafloxacin provides a safety profile comparable to moxifloxacin.

Taking fC(max) /MIC ≥5 and fAUC(24 h) /MIC ≥30 as a target, the cumulative fraction of response (CFR) of levofloxacin 750 mg for treatment of community-acquired pneumonia (CAP) was calculated using Monte Carlo simulation. The probability of target attainment (PTA) of levofloxacin at various minimal inhibitory concentrations (MICs) was also evaluated.RESULTS AND DISCUSSION: The results of PK study showed that the C(max) and AUC(0-∞) of levofloxacin were 14·94 μg/mL and 80·14 μg h/mL following single-dose infusion of levofloxacin. The half-life and average cumulative urine excretion ratio within 72 h post-dosing were 7·75 h and 86·95%, respectively. The mean C(ss,max), C(ss,min) and AUC(0-τ) of levofloxacin at steady state following multiple doses were 13·31 μg/mL, 0·031 μg/mL and 103·7 μg h/mL, respectively. The accumulation coefficient was 1·22. PK/PD analysis revealed that the CFR value of levofloxacin 750-mg regimen against Streptococcus pneumoniae was 96·2% and 95·4%, respectively, in terms of fC(max) /MIC and fAUC/MIC targets.WHAT IS NEW AND CONCLUSION: The regimen of 750-mg levofloxacin once daily provides a satisfactory PK/PD profile against the main pathogenic bacteria of CAP, which implies promising clinical and bacteriological efficacy for patients with CAP. A large-scale clinical study is warranted to confirm these results.

Abstract:Objective To compare curative effects between β-lactam plus macrolides and fluoroquinolone alone in treatment of community acquired pneumonia.Methods Retrospective Study was performed with patients diagnosed as CAP from Jaunuary 2010 to June 2011.Results Total of 89 CAP patients were included,among which 56 patients were prescribed fluoroquinolone and 33 patients were β-lactam plus macrolide.There were no significant differences in ages,genders,complications,previous antibiotics usage,antipyetics,physical signs and results of laboratory examination between these two groups(P>0.05).There were no statistical differences in curative effects such as time of fever continuing,time of hospitalization,time of CRP 80% decrease between two groups(P>0.05).There was also no significant difference by PSI subgroup analysis.Conclusion The curative effects of β-lactam plus macrolides and fluoroquinolone alone were similar in treatment of community acquired pneumonia.

Abstract:Background: Nemonoxacin malate is a novel non-fluorinated quinolone for oral and intravenous (IV) administration. This phase 3, multicentre, randomised, double-blind, double-dummy, parallel-controlled clinical trial (NCT02205112) evaluated the efficacy and safety of IV nemonoxacin vs. levofloxacin for the treatment of community-acquired pneumonia (CAP) in adult patients. Methods: Eligible patients were randomised to receive 500 mg nemonoxacin or levofloxacin via IV infusion, once daily for 7-14 days. The primary endpoint was the clinical cure rate at the test-of-cure (TOC) visit in the modified intent-to-treat (mITT) population. Secondary efficacy and safety were also compared between nemonoxacin and levofloxacin. Results: Overall, 525 patients were randomised and treated with nemonoxacin ( n = 349) or levofloxacin ( n = 176). The clinical cure rate was 91.8% (279/304) for nemonoxacin and 85.7% (138/161) for levofloxacin in the mITT population ( P > 0.05). The clinical efficacy of nemonoxacin was non-inferior to levofloxacin for treatment of CAP. Microbiological success rate with nemonoxacin was 88.8% (95/107) and with levofloxacin was 87.8% (43/49) ( P > 0.05) at the TOC visit in the bacteriological mITT population. The incidence of drug-related adverse events (AEs) was 37.1% in the nemonoxacin group and 22.2% in the levofloxacin group. These AEs were mostly local reactions at the infusion site, nausea, elevated alanine aminotransferase/aspartate aminotransferase (ALT/AST), and QT interval prolongation. The nemonoxacinrelated AEs were mostly mild and resolved after discontinuation of nemonoxacin. Conclusions: Nemonoxacin 500 mg IV once daily for 7-14 days is effective and safe and non-inferior to levofloxacin for treating CAP in adult patients. (c) 2024 The Authors. Published by Elsevier Ltd. Randomized Controlled Clinical Trial of Moxifloxacin Versus Levofloxacin Injection in the Treatment of Community Acquired Pneumonia

Abstract:Objective To evaluate the efficacy and safety of moxifloxacin for the treatment on the patients with community acquired pneumonia.Methods A randomized controlled,parallel,block,open clinical trial was conducted.Patients were randomized into each group in the study.Trial group patients received moxifloxacin 400 mg once daily,and control group patients received levofloxacin 200 mg twice daily by intravenous infusion.The duration was 7~14 days in both groups.Ninety-two cases in the ITT analysis,46 patients in trial group received moxifloxacin treatment,and 46 in control group received levofloxacin treatment;73 cases in the PP analysis,37 patients in moxifloxacin group,and 66 patients in levofloxacin group;73 cases in PP/MBE,34 patients in moxifloxacin group and 39 patients in levofloxacin group;62 cases in the PP/MBE,31 patients in each group.The overall efficacy rates of each group in ITT were 76.1%.Results The overall efficacy rates in PP were 89.2% and 88.9%,respectively.The bacterial eradication rates of each group in ITT/MBE were 88.2% and 82.1%,and in PP/MBE were 90.3% and 90.6%,respectively.The adverse drug reaction was evaluated in 92 cases.The adverse drug reaction rates,which related to study drug of each group were 10.3% and 8.7%,respectively.The results showed that there were no statistical differences between these two groups(P0.05).Conclusion Moxifloxacin was effective and safe as levofloxacin in the treatment of community acquired pneumonia.

Abstract:Background/Purpose: To compare the clinical efficacy and safety of nemonoxacin with levofloxacin in treating community-acquired pneumonia (CAP) in a Phase II clinical trial. Methods: One hundred ninety-two patients with CAP were randomized to receive oral nemonoxacin (500 mg or 750 mg) or levofloxacin (500 mg) once daily for 7-10 days. Clinical and bacteriological responses were determined at the test of cure (TOC) visit in the full analysis set (FAS). Results: The clinical cure rate of nemonoxacin (500 mg), nemonoxacin (750 mg), and levofloxacin (500 mg) was 93.3%, 87.3%, and 88.5%, respectively, in the FAS (n = 168), and 93.0%, 93.9%, and 88.9%, respectively in the per protocol set (n = 152). At the TOC visit, nemonoxacin at 500 mg and 750 mg was proven to be noninferior to levofloxacin at 500 mg in the FAS in terms of clinical efficacy. The overall bacteriological success rate was 83.3% in both nemonoxacin groups and 80.0% in the levofloxacin 500 mg group in the bacteriological FAS. The comprehensive efficacy rate was comparable among the three groups (87.5% for the nemonoxacin 500 mg group, 93.8% for the nemonoxacin 750 mg group, and 81.3% for the levofloxacin 500 mg group). Most drug-related adverse events were mild and transient, mainly gastrointestinal symptoms such as nausea and vomiting, transient neutropenia, and elevated liver enzymes. No drug-related serious adverse events occurred. Conclusion: Either 500 mg or 750 mg of oral nemonoxacin taken once daily for 7-10 days demonstrated high clinical and bacteriological success rates in Chinese adult patients with CAP. Abstract:Levofloxacin (LVFX), a fluoroquinolone agent, has a broad spectrum that covers Gram-positive and -negative bacteria and atypical pathogens. It demonstrates good clinical efficacy in the treatment of various infections, including lower respiratory tract infections (LRTIs) and urinary tract infections (UTIs). To evaluate the efficacy and safety of oral LVFX 500 mg once daily, a large open-label clinical trial was conducted in 1266 patients (899 with LRTIs and 367 with UTIs) at 32 centers in China. In the per-protocol population, the clinical efficacy rate (cure or improvement) at 7 to 14 days after the end of treatment was 96.4% (666/691) for LRTIs and 95.7% (267/279) for UTIs. In 53 patients diagnosed with atypical pneumonia the treatment was effective. The bacteriological efficacy rate was 96.6% (256/265) for LRTIs and 93.3% (126/135) for UTIs. The eradication rate of the causative pathogens was 100% (33/33) for Haemophilus influenzae and 96.0% (24/25) for Streptococcus pneumoniae in LRTIs, and 94.1% (80/85) for Escherichia coli in UTIs. The overall efficacy rates were 89.3% (617/691) for LRTIs and 87.8% (245/279) for UTIs. The incidence of drug-related adverse events (ADRs) was 17.3% (215/1245), and the incidence of drug-related laboratory abnormalities was 15.7% (191/1213). Common ADRs were dizziness, nausea, and insomnia. Common laboratory abnormalities included “WBC decreased”, “alanine aminotransferase (ALT) increased”, “aspartate aminotransferase (AST) increased”, and “lactate dehydrogenase (LDH) increased”. All of these events were mentioned in the package inserts of fluoroquinolones including LVFX, and most events were mild and transient. Thirty-four patients (2.7%) were withdrawn from the study because of the ADRs. No new ADRs were found. This study concluded that the dosage regimen of LVFX 500 mg once daily was effective and tolerable for the treatment of LRTIs and UTIs.

Abstract:ObjectiveTo evaluate the clinical efficacy and safety of domestic levofloxacin for injection in the treatment of acute bacterial infections.MethodsA randomized multicenter clinical study was conducted to compare domestic levofloxacin for injection with imported levofloxacin(500 mg/100 ml).Both drugs were administered intravenously at a dose of 500 mg once daily for 7 to 14 days.ResultsA total of 160 patients were enrolled in the study,157 patients were eligible for clinical efficacy analysis,78 in treatment group(domestic levofloxacin),and 79 in control group(imported levofloxacin).The cure rates and effective rates were 67.6% and 89.7% in domestic levofloxacin group,and 50.0% and 90.0% in control group respectively.The bacterial eradication rates were 98.2%(53/54) and 100.0%(50/50) respectively.There was no statistically significant difference between the two groups(P>0.05).The incidence of adverse reactions was 13.8%(12/80) and 16.3%(13/80) in the two groups,mainly mild to moderate nausea,gastric discomfort,insomnia,skin rash,phlebitis and elevated transaminases.There was no statistically significant difference between the two groups(P>0.05).ConclusionsDomestic levofloxacin for injection is effective and safe for the treatment of acute bacterial infections of respiratory and urinary tract.

Abstract:This study aimed to explore the pharmacokinetic features of levofloxacin (LVFX) in Chinese patients with infections and to confirm oral LVFX 500 mg once daily as an optimal treatment regimen based on pharmacokinetic-pharmacodynamic (PK-PD) analysis. A total of 1052 plasma samples from 164 Chinese adult patients with communityacquired lower respiratory tract infections (CALRTIs) and 18 healthy volunteers were used for population PK analysis. LVFX 500-mg tablets were given once daily. A nonlinear mixed effects model (NONMEM) program was used for population PK model-building and a two-compartment model with first-order absorption process was established. Creatinine clearance (CL cr ) and body weight were identified as intrinsic factors which significantly affected oral clearance (CL t /F) and the apparent volume of distribution of the central compartment (V1/F), respectively. The final model is described as follows: CL t /F (l/h) = (8.97 + 0.917 × (CL cr (ml/min) − 100.92) × 60/1000) × exp (η CLt/F ). V1/F (l) = (85.3 + 1.22 × (weight (kg) − 60.75)) × exp (η V1/F ). Q/F (l/h) = 0.351. V2/F (l) = 6.81. k a (h −1 ) = 1.44 × exp(η ka ). Based on the population PK model, mean C max and AUC 0–24h in CALRTI patients were estimated as 5.13 µg/ml and 58.98 µg·h/ml, respectively. A subgroup analysis showed that patients with mild renal dysfunction (50 ml/min ≤ CL cr < 80 ml/min) had 34% higher AUC 0–24h values compared to patients with normal renal function (CL cr ≥ 80 ml/min). Postmodeling simulation using final population PK estimates also showed that C max and AUC 0–24h increased markedly in patients with severe renal dysfunction. The results indicate that LVFX dosage adjustment should be individualized on the basis of the CL cr , especially in those with CL cr less than 50 ml/min. None of the PK parameters had any correlation with the occurrence of adverse events. PK-PD analysis indicated that, in patients treated with LVFX 500 mg once daily, the AUC 0–24h /MIC ratio exceeded the target for those major CALRTI pathogens isolated. In addition, the C max /MIC ratio reached 5 for Streptococcus pneumoniae , indicating that the emergence of LVFX-resistant S. pneumoniae could be prevented during the therapy with this dosage regimen. These results demonstrate that oral LVFX 500 mg once daily has favorable PK parameters and PK-PD features in patients with CALRTIs, and the results strongly support this dosage regimen for the treatment of CALRTI.

Abstract:OBJECTIVE To evaluate the dosing regimens of levofloxacin. METHODS The drug concentrations in serum and urine were assayed by HPLC method and the pharmacokinetic parameters were calculated after intravenous infusion of a single dose of 200 mg, 300 mg and 500 mg levofloxacin to healthy volunteers. The in vitro activity MIC of levofloxacin against 823 clinical isolates were determined and compared with other antimicrobial agents. Based on the above results, the PK/PD parameters C(max)/MIC and AUC/MIC were calculated and the dosing regimens of levofloxacin were proposed for infections caused by different pathogens. RESULTS The results of clinical pharmacokinetic study showed that the C(max) of levofloxacin was 3.4 mg/L +/- 0.8 mg/L, 4.8 mg/L +/- 1.4 mg/L and 7.6 mg/L +/- 1.1 mg/L respectively, AUC(0-infinity) was 14.4 mg.h/L +/- 2.5 mg.h/L, 21.9 mg.h/L +/- 4.5 mg.h/L and 38.3 mg.h/L +/- 4.9 mg.h/L respectively, T(1)/2 beta was 6.2 h +/- 0.4 h, 6.4 h +/- 0.9 h and 6.5 h +/- 0.6 h respectively, and 69% +/- 5%, 69% +/- 6%, and 65% +/- 4% of the doses were excreted in urine within 24 h after intravenous infusion of a single dose of levofloxacin 200 mg, 300 mg and 500 mg. The in vitro pharmacodynamic study showed that levofloxacin was highly active against Hemolytic streptococcus and Moraxella catarrhalis. It was active against Streptococcus pneumoniae (including penicillin nonsusceptible strains), Hemophilus influenzae, methicillin-sensitive Staphylococcus aureus (MSSA), Klebsiella pneumoniae and Stenotrophomonas maltophilia. Levofloxacin also had good activity against Pseudomonas aeruginosa and K.pneumoniae. However, most of isolates of enterococcal spp. were resistant to levofloxacin. Above 50% of Escherichia coli isolates were resistant to levofloxacin. Based on the results of PK/PD parameters, the adequate dosing regimens of levofloxacin should be once daily 200 mg once daily of levofloxacin was expected to be effective for the treatment of infections caused by M. catarrhalis. The regimen of 300 mg once daily could be effective for the treatment of infections caused by Hemolytic streptococcus. 500 mg once daily of levofloxacin was expected to be effective for the treatment of respiratory tract infections caused by S. pneumoniae or H. influenzae. For treatment of respiratory tract infections and urinary tract infections caused by levofloxacin-susceptible organisms including K. pneumoniae, E. coli, P. aeruginosa, and S.maltophilia, 500 mg once daily of levofloxacin was needed to obtain good clinical efficacy. But PK/PD parameters predicted that 500 mg daily of levofloxacin was not effective for infections caused by methicillin-resistant Staphylococcus aureus (MRSA) and Enterococci. CONCLUSION The proposed dosing regimens of levofloxacin based on PK/PD concepts are expected to provide good efficacy in clinical practice.

Abstract:Objective To evaluate the efficacy and safety of oral levofloxacin 500 mg once daily in the treatment of patients with community acquired lower respiratory tract infections (LRTIs) or urinary tract infections (UTIs),a prospective,uncontrolled,open-label,multi-center study was conducted.Methods Patients with community acquired pneumonia (CAP),acute exacerbation of chronic bronchitis (AECB),acute uncomplicated lower urinary tract infection (LUTI),acute pyelonephritis (APN),recurrent urinary tract infection (UTI) or complicated UTI were treated by levofloxacin 500 mg tablet once daily.Results Among the 1 266 patients enrolled,1 245 were included in full analysis set (FAS) and 1 082 were included in the per protocol set (PPS).The safety analysis population was identical to the FAS population.At test of cure (day 7-14 after the end of treatment) among the PPS population,the clinical efficacy rates of CAP,AECB,LUTI,APN,recurrent UTI and complicated UTI were 96.8% (306/316),96.0% (360/375),93.7% (74/79),98.7% (76/77),94.9% (93/98) and 96.0% (24/25),respectively.The bacteriological efficacy rates were 98.4% (120/122),95.1% (136/143),97.6% (41/42),94.1% (48/51),92.3% (24/26) and 81.3% (13/16),respectively.The overall efficacy rates were 91.7% (289/315),87.2% (328/376),93.5% (72/77),89.7% (70/78),86.7% (85/98) and 69.2% (18/26),respectively.The incidence of drug-related adverse reactions (ADRs) was 17.3% (215/1 245).The most common ADRs were dizziness,nausea and insomnia.The incidence of drug-related laboratory abnormalities was 15.7% (191/1 213) with WBC decreased,ALT increased and AST increased being the most frequent.Most events were mild and transient.Only 2.7% of the patients withdrew from the study due to ADRs.No drug-related serious adverse events (SAE) were found.Conclusions The dosing regimen of levofloxacin 500 mg once daily is effective and tolerable for the treatment of LRTIs and UTIs.

Abstract:OBJECTIVE To evaluate the efficacy and safety of pazufloxacin and levofloxacin in the treatment of bacterial infections.METHODS A randomized multicenter clinical trial was conducted for the treatment of patients with respiratory tract and urinary tract infection,While an opened trial was conducted for skin soft-tissue infections.RESULTS In randomized study,243 patients were enrolled in PerProtocol analyse,The results showed that the overall cure rates were 65.25% for pazufloxacin group and 66.40% for levofloxacin group,the efficacy rates were 93.22% and 96.00%,respectively.The incidence of adverse reaction was 10.32% and 7.63%,respectively.The results showed that there was no statistical difference between the two groups(P>0.05);In opened study,29 patients were enrolled in Per-Protocol analyse,The overall cure rates and efficacy rates were 51.72% and 86.21%.The incidence of adverse reaction was 3.33%.CONCLUSION Pazufloxacin injection is effective and safe for the treatment of bacterial infections.
Sequential moxifloxacin therapy in hospitalized patients with community-acquired pneumonia

Abstract:Objective To evaluate the efficacy and safety of sequential moxifloxacin therapy in treatment of hospitalized patients with community-acquired pneumonia(CAP).Methods The 186 patients with community-acquired pneumonia who were hospitalized during January 2009 and December 2010 were randomized to receive two different moxifloxacin regimens.The patients in the control group received moxifloxacin by intravenous infusion,400 mg once daily for 7-10 days.The patients in sequential therapy group received moxifloxacin by intravenous infusion 400 mg once daily for 3-5 days,followed by oral administration at the same dose.The total treatment duration was also 7-14 days.Results Eighty-six patients in sequential group and 88 patients in control group completed the study.No serious adverse drug reaction was reported in either group.The safety profile did not show any significant difference between the two groups(P>0.05).The clinical efficacy rate was 87.2%in sequential group, and 89.8%in the control group.The bacterial eradication rate was 92.1%in sequential group and 95.7%in the control group, respectively.Comparison between groups did not show any significant difference(P>0.05).Conclusions Moxifloxacin is effective and safe in the treatment of local hospitalized CAP patients.The sequential therapy is similar to full intravenous therapy. It may be a useful alternative therapy for those CAP patients who can take the drug orally.
Pharmacokinetics of multiple intravenous instillation of levofloxacin in Chinese healthy subjects.

Abstract:To study the pharmacokinetics of multiple doses intravenous infusion of levofloxacin instillation in Chinese healthy volunteers.Intravenous infusion of levofloxacin instillation 200 mg within 60 min was given to 10 male healthy volunteers for 7 d, on d 1 and d 7, once-daily, from 2-6 d twice-daily dosing. The concentrations of levofloxacin in serum and urine were assayed by HPLC.The main pharmacokinetic parameters af ter the first dosing were as follows: Cmax was (2.4+/-0.4) mg/L; AUC0- was (16.1 +/- 1.4) mg . h . L-1; T1/2beta was (6.3 +/- 0.3) h. The concentration in serum reached steady state within 72 h. The main parameters after the last dosing were as follows: Cssmax was (2.9 +/- 0.4) mg/L; Cssmin was (0.71 +/- 0.19) mg/L; Cav was (1.40 +/- 0.29) mg/L; AUCss0-12 was (17 +/- 3) mg . h . L-1; T1/2beta was (6.2 +/- 0.8) h. The 24-h cumulative urinary excretion rate was (88 +/- 5) %. From the calculation, the cumulative rate was 1.20; the fluctuation index was 1.30. The difference of T1/2beta and AUC between the first dosing and the last dosing was not significant, and the elimination rate of levofloxacin was not changed after multiple dosing. No clear adverse events were noted during this study.There was no accumulation of drug after the repeated intravenous infusion with 200 mg levofloxacin instillation for 7 d.

Abstract:Objectives: Levofloxacin is widely used because of its broad-spectrum antimicrobial activity and convenient dosing schedule. However, the relevance of its use in children remains to be investigated. The purpose of this study is to investigate the efficacy and safety of levofloxacin use in children with severe infections. Methods: We conducted a retrospective observational study of patients <18 years of age who received levofloxacin intravenously in the Pediatric Intensive Care Unit (PICU) of our hospital during the period between 2021 and 2022. Patient demographics, course characteristics, clinical effectiveness, and adverse event correlations were extracted through a retrospective tabular review. Results: We included 25 patients treated with 28 courses of levofloxacin. The mean age of these children treated with levofloxacin was 4.41 years. Conversion of pathogenic microbiological test results to negative after levofloxacin treatment was detected in 11 courses (39.29%). A decrease in inflammatory markers, white blood cell or C-reactive protein counts, was detected in 18 courses (64.29%). A total of 57 adverse events occurred during the treatment period, of which 21 were possibly related to levofloxacin and no adverse events were probably related to levofloxacin. Conclusion: The effectiveness of levofloxacin use in children with serious infections is promising, especially for the treatment of multidrug-resistant bacteria. Adverse events occurring during the initiation of levofloxacin therapy in children are reported to be relatively common, but in this study, only a small percentage of them were possibly related to levofloxacin, and none of them were highly possibly related to levofloxacin.

Abstract:Levofloxacin is a fluoroquinolones with a broad spectrum of antibacterial activity. A randomized controlled clinical study of levofloxacin was conducted campared with ciprofloxacin. The objective of the study is to evaluate the safety and efficacy of levofloxacin in the treatment of bacterial infections. A total of 52 patiens was enrolled in the study. 25 patients received levofloxacin and 27 patients received ciprofloxacin were assessable for clinical efficacy. The result showed that the overall efficacy rates were 88.00% for the levofloxacin group and 85. 19% for the ciprofloxacin group, whereas the bacterial clearance rates were 95.65% and 89.47%, respectively. The adverse ding reaction rate were 4.00% and 7.41 %, respectively. The results showed that levofloxacin was effective and safe for the treatment of bacterial infections. There were no statistical differences between the two groups (P0.05).

Abstract:Background/Purpose Nemonoxacin is a novel nonfluorinated quinolone with excellent in vitro activity against most pathogens in community-acquired pneumonia (CAP), especially Gram-positive isolates. The purpose of this study was to assess the efficacy and safety of nemonoxacin compared with levofloxacin in patients with CAP. Methods A phase 3, multicenter, randomized (2:1) controlled trial was conducted in adult CAP patients receiving nemonoxacin 500 mg or levofloxacin 500 mg orally once daily for 7–10 days. Clinical, microbiological response and adverse events were assessed. Non-inferiority was determined in terms of clinical cure rate of nemonoxacin compared with that of levofloxacin in a modified intention-to-treat (mITT) population. NCT registration number: NCT01529476. Results A total of 527 patients were randomized and treated with nemonoxacin (n = 356) or levofloxacin (n = 171). The clinical cure rate at test-of-cure visit was 94.3% (300/318) for nemonoxacin and 93.5% (143/153) for levofloxacin in the mITT population [difference (95% CI), 0.9% (−3.8%, 5.5%)]. The microbiological success rate was 92.1% (105/114) for nemonoxacin and 91.7% (55/60) for levofloxacin in the bacteriological mITT population [difference (95% CI), 0.4% (−8.1%, 9.0%)]. The incidence of adverse events (AEs) was comparable between nemonoxacin (33.1%, 118/356) and levofloxacin (33.3%, 57/171) (P > 0.05). Conclusion Nemonoxacin 500 mg once daily for 7–10 days is as effective and safe as levofloxacin for treating adult CAP patients in terms of clinical cure rates, microbiological success rates, and safety profile.ClinicalTrials.gov identifier: NCT01529476.

Abstract:OBJECTIVE:To evaluate the clinical efficacy and safety of antofloxacin hydrochloride tablet for the treatment of acute bacterial infections.METHODS:A multi-center randomized control, double blind and double dummy clinical trial was conducted; levofloxacin tablet was closed as controlled drug. The duration of treatment was 7-14 days in both groups.RESULTS:A total of 719 patients were enrolled in the study, in which 359 patients treated with antofloxacin and 360 patients treated with levofloxacin were included. Three hundred and thirty and 337 patients completed the study and met with all the criteria for per-protocol analysis, respectively. By the end of chemotherapy, the cured rates in per protocol set (PPS) population were 79.7% and 77.4%, the effective rates were 95.2% and 96.7%, and the bacterial clearance were 96.7% and 97.5% for the treating and control group, respectively. The clinical and bacterial efficacy of antofloxacin and levofloxacin was comparable by the analysis of infectious sites. Three hundred and fifty-seven and 356 patients in antofloxacin and levofloxacin groups were evaluated the safety. The drug adverse events occurred both in 10.1%, and drug adverse reactions occurred in 7.8% and 7.9% patients in the two groups. The most common drug adverse reactions were mild gastroenteric symptoms. No QTc prolongation was detected in all the patients. One patient in each group had mild blood glucose increase at the end of therapy, but the glucose returned to normal level without any intervention. No statistic significant difference between the two groups in clinical efficacy and safety was detected (P > 0.05).CONCLUSIONS:Antofloxacin hydrochloride tablet was effective and safe for the treatment of acute bacterial infections.
