Abstract:Abstract Measles is a vaccine preventable illness. Nevertheless, in recent years, measles is still endemic and epidemic in both the developed world and the developing world. The public perception of measles in the past was that it was not a big deal. However, measles is associated with a number of complications which can be places in three categories which are: acute(diarrhea, otitis media, pneumonia, encephalitis, seizures, and death) and delayed-subacute sclerosing panencephalitis (SSPE) and post measles immune amnesia. Contrary to the beliefs of the anti-vaccine lobby, measles is bad. In acute measles, the death rate is 1-3 per 1000 and the risk of encephalitis is 1 per 1000. Relatively recent investigations indicate that SSPE is considerably more common than previously believed. The worldwide contribution of post measles immune amnesia to morbidity and mortality is likely to be huge. In exposure situations two doses of measles vaccine will prevent 99% of cases. Presently in the United States the first dose is given at 12 through 15 months of age. The second dose is most often administered at 4 through 6 years of age. In my opinion, the second dose of measles vaccine should be given 4 to 6 weeks after the first dose rather than at 4 to 6 years of age. Children who don’t have antibody to measles should not travel to risk areas.

Guarding the gatekeepers: a comprehensive approach to control nosocomial measles

Abstract:Purpose Despite substantial vaccination progress, persistent measles outbreaks challenge global elimination efforts, particularly within healthcare settings. In this paper, we critically review the factors contributing to measles outbreak and effective control measures for nosocomial transmission of measles. Methods We systematically searched electronic databases for articles up to 17th May, 2023. This was performed by two independent reviewers, with any disagreements resolved by a third reviewer. We also searched governmental and international health agencies for relevant studies. Results Forty relevant articles were systematically reviewed, revealing key factors fuelling measles outbreak in healthcare settings, including high transmissibility capability; high intensity exposure; delayed care; failure to use protective equipment and implement control measures; vaccine failure; unclear immunisation history and lack of registries; and lacking recommendation on healthcare workers' (HCWs) measles vaccination. To combat these challenges, successful control strategies were identified which include early notification of outbreak and contact tracing; triaging all cases and setting up dedicated isolation unit; strengthening protective equipment use and physical measures; improving case detection; determining immunity status of HCWs; establishing policy for measles vaccination for HCWs; management of exposed personnel; and developing a pre-incident response plan. Conclusion A coordinated and comprehensive approach is essential to promptly identify and manage measles cases within healthcare settings, necessitating multifactorial strategies tailored to individual settings. These findings provide a valuable foundation for refining strategies to achieve and maintain measles elimination status in healthcare environments.

Abstract:Background Measles remains a significant contributor to illness and death among young children, despite the presence of a safe and efficacious vaccine. One of the most highly transmissible viruses affecting humans is measles, which can be almost entirely prevented through vaccination. The recent surge in measles cases worldwide has been particularly notable in India. A total of 172 confirmed outbreaks of measles were recorded between October 2021 and September 2022, resulting in 12,589 cases. Measles, being highly contagious, is transmitted through the respiratory route. The disease affects 75 to 90% of susceptible household contacts, reaching peak infectivity three days before the rash emerges. The recent measles outbreak observed since January 2023 holds significance, as Bangalore district alone has reported around 500 cases. Therefore, the current study aims to evaluate the clinical and sociodemographic characteristics of measles in pediatric patients. Methods This was a retrospective study conducted over seven months. Data pertaining to demographics such as age, gender, parental occupation, parental educational background, and housing situation were gathered. Details regarding the child's immunization status, the onset of fever, and the onset of rash were also documented. Descriptive data was represented using mean, standard deviation, and percentage or proportion. Results Over the study period of seven months, 53 children were admitted due to measles. The majority of the individuals were female, accounting for 28 cases (52.8%). The typical clinical manifestations of measles such as cough, coryza, and conjunctivitis were documented in 45 (84.90%), 43 (81.13%), and 32 (60.37%) children, respectively. Specifically, two (3.77%), 14 (26.41%), 20 (37.73%), and 17 (32.07%) individuals were categorized as belonging to the upper middle, lower middle, upper lower, and lower socioeconomic strata. Six children in the nine- to 12-month age range had not received the measles-rubella (MR) vaccination. Thirteen (54.16%) out of 24 children aged one to five years had not received a single dose of the vaccine, while the children who had received at least one dose did not experience any complications. Among children aged less than nine months, nine to 12 months, and more than 12 months, three (100%), two (66.6%), and three (75%) respectively, experienced complications. All the patients recovered completely and were discharged from the hospital. Conclusion Measles is re-emerging as an infectious disease among children, as evidenced by the current outbreak. Study indicates that measles tends to impact infants and under five children more frequently. Through prompt and suitable medical intervention, all affected children experienced full recovery. It was observed that immunized children had fewer complications in comparison to their non-immunized counterparts. Acquiring a thorough understanding of the clinical profile of the illness is crucial for policymakers in developing essential guidelines for measles immunization coverage.
The effect of time since measles vaccination and age at first dose on measles vaccine effectiveness - A systematic review

Abstract:Background: In settings where measles has been eliminated, vaccine-derived immunity may in theory wane more rapidly due to a lack of immune boosting by circulating measles virus. We aimed to assess whether measles vaccine effectiveness (VE) waned over time, and if so, whether differentially in measles-eliminated and measles-endemic settings. Methods: We performed a systematic literature review of studies that reported VE and time since vaccination with measles-containing vaccine (MCV). We extracted information on case definition (clinical symptoms and/or laboratory diagnosis), method of vaccination status ascertainment (medical record or vaccine registry), as well as any biases which may have arisen from cold chain issues and a lack of an age at first dose of MCV. We then used linear regression to evaluate VE as a function of age at first dose of MCV and time since MCV. Results: After screening 14,782 citations, we identified three full-text articles from measles-eliminated settings and 33 articles from measles-endemic settings. In elimination settings, two-dose VE estimates increased as age at first dose of MCV increased and decreased as time since MCV increased; however, the small number of studies available limited interpretation. In measles-endemic settings, one-dose VE increased by 1.5% (95% CI 0.5, 2.5) for every month increase in age at first dose of MCV. We found no evidence of waning VE in endemic settings. Conclusions: The paucity of data from measles-eliminated settings indicates that additional studies and approaches (such as studies using proxies including laboratory correlates of protection) are needed to answer the question of whether VE in measles-eliminated settings wanes. Age at first dose of MCV was the most important factor in determining VE. More VE studies need to be conducted in elimination settings, and standards should be developed for information collected and reported in such studies.

Abstract:We analyze stability of the planar orientational structure in a nematic-liquid-crystal cell with planar anchoring conditions at both substrates. Specifically, we study the instabilities of the ground state caused by surface elasticity with the saddle-splay elastic constant K24 violating the Ericksen inequalities. We express the surface part of static correlation functions as a functional integral over the fluctuation field induced by director fluctuations at confining walls and derive the stability conditions for the planar structure with respect to the fluctuation modes characterized by the in-plane wave numbers and by the parity. These conditions are analyzed in the cell thickness-fluctuation wavelength plane through the parametrization for the boundary curve of the instability region. For relatively small K24, the fluctuation mode of the critical wavelength is found to render the structure unstable when the thickness of the cell is below its critical value. The parity of the critical mode changes as the twist-splay ratio K(2)/K(1) is passing through unity. Further increase of K24 beyond the second threshold value, 4K(1)K(2)/(K1+K2), leads to the instability with respect to short wavelength fluctuations regardless of the cell thickness. We compute the critical thickness and the critical wavelength as a function of K24, the twist-splay ratio, and the azimuthal anchoring strength.

Abstract:Despite high vaccine-effectiveness, wild-type measles can occur in previously vaccinated persons. We compared the clinical presentation and disease severity of measles by vaccination status and age in the post-elimination era in the United States. We included U.S. measles cases reported from 2001–2022. Breakthrough measles was defined as cases with ≥1 documented dose of measles-containing vaccine, classic measles as the presence of rash, fever, and ≥1 symptoms (cough, coryza, or conjunctivitis), and severe disease as the presence of pneumonia, encephalitis, hospitalization, or death. Vaccinated cases with low and high avidity IgG were classified as primary (PVF) and secondary (SVF) vaccine failures, respectively. Among 4,056 confirmed measles cases, 2,799 (69%) were unvaccinated, 475 (12%) were breakthrough infections, and 782 (19%) had unknown vaccination; 1,526 (38%), 1,174 (29%), and 1,355 (33%) were aged <5, 5–19, and ≥20 years, respectively. We observed a general decline in classic presentation and severe disease with an increase in the number of doses, and less complications among children aged 5–19 years compared to other age-groups. Among 93 breakthrough cases with avidity results, 11 (12%) and 76 (82%) were classified as PVF and SVF, respectively, with a higher proportion of PVFs having a classic measles presentation and severe disease than SVFs. Breakthrough measles cases tended to have milder disease with less complications. A small proportion of breakthrough infections were due to PVF than SVF. It is critical to maintain high MMR vaccination coverage in the United States to prevent serious measles illnesses.

Abstract:There are presented data characterizing measles (ICD-10 B05) as the most contagious viral infection, its contagiousness is more than 90%. RNA virus of the genus morbilliviruses of the paramyxovirus family is the causative agent of measles. The incidence of measles is recorded among children and among adults, mainly among unvaccinated against this infection. The source of infection is only a measles-sick person, who is dangerous to others already 3-5 days before the rash appears. The features of the epidemic process of measles in connection with immunization and the gain in collective immunity are described. From 2012 to the present, the measles epidemic situation has been shown to be complicated, which has been the reason for the repeated postponement of the deadlines for its liquidation, proclaimed by WHO. Sufficient conditions for achieving the elimination of measles are formulated. First of all, it is necessary to maintain a 95% level of immunity in the prescribed age and professional groups of the population.

Abstract:Measles virus is a highly infectious RNA virus belonging to the genus Morbillivirus in the family Paramyxoviridae. It causes measles, a respiratory disease that is one of the leading causes of childhood mortality worldwide. The virus has a pleomorphic structure and a genome consisting of a single strand of negative-sense RNA. The genome encodes six structural proteins, including the nucleocapsid (N), phosphoprotein (P), matrix (M), fusion (F), hemagglutinin (H), and the large polymerase (L) proteins. Measles virus enters host cells through the interaction of the viral H protein with cellular receptors, followed by fusion of the viral envelope with the host cell membrane. The virus then undergoes replication and assembly, leading to the release of new virus particles. The viral life cycle is complex and involves numerous host factors. Diagnosis of measles virus is typically made by detecting the virus in clinical specimens using laboratory tests such as RT-PCR or serological assays. Clinical symptoms such as fever, cough, runny nose, and rash can also aid in the diagnosis. Complications of measles virus infection include pneumonia, encephalitis, and death. Prevention of measles virus involves vaccination, maintaining high population immunity, and early identification and isolation of infected individuals. Measles virus remains a significant public health threat, particularly in areas with low vaccination coverage. The virus is highly contagious and can cause severe disease and mortality. The global incidence of measles has increased in recent years due to outbreaks in various regions. Treatment for measles virus infection is primarily supportive, with no specific antiviral therapies currently available.
Trends of the Global, Regional, and National Incidence of Measles, Vaccine Coverage, and Risk Factors in 204 Countries From 1990 to 2019

Abstract:Background: Measles is a highly contagious disease that has caused global morbidity and mortality. Though great progress has been made in measles elimination, the resurgence of measles has been observed in recent years. As extant literature were mainly confined to data of local area, we conducted a systematic analysis to explore the trends of the incidence of measles, coverage rate, and the risk factors from 1990 to 2019 on global, regional, and national levels. Methods: Data on cases of incidence, age standardized rate (ASR), vaccine coverage, and risk factors of measles were retrieved from the Global Burden of Disease (GBD) Study 2019 database. Estimated averaged percentage change (EAPC) of ASR was calculated to quantify the trends of measles incidence. Pearson correlation was applied to assess the association of EAPC and measles-containing vaccine coverage rate with socio-demographic index (SDI) in 2019, and the correlation between ASR and measles-containing vaccine coverage rate. Results: Globally, there was a significant decrease in the number (84.18%) and ASR (6.13%, 95% CI: 5.41-6.84%) of measles incidence from 1990 to 2019. More than 80% of incidence cases were attributed to children under 5 and the proportion was highest in low SDI region. Countries and territories with low ASRs are mostly clustered in North America, Southern Latin America, and Western Europe, whereas those with high ASRs are mainly clustered in Africa, East Asia, and South Asia. Lower SDI was associated with higher ASR and lower vaccine coverage rate. The more the SDI value was further away from 0.6, the smaller the absolute value did EAPC have. Child wasting accounted for the most measles-related death cases, followed by child underweight, child stunting, and vitamin A deficiency. Conclusions: Measles eradication is feasible, but urgently demands political commitment, financial support, and public engagement. In the context of COVID-19 pandemic and the global resurgence of measles, surveillance systems and routine immunization programs should be improved, while vaccine hesitancy needs to be addressed.

Abstract:The worldwide elimination of measles and rubella is feasible, but not without overcoming the substantial challenge of vaccine hesitancy. This challenge is complicated by the spread of misinformation and disinformation fueled by rapidly progressing technologies and evolving forms of online communication. The recent COVID-19 pandemic has only added further complexity to this challenge. However, considerable progress has been made in understanding the scope of the problem and the complex factors that influence vaccine hesitancy. Our understanding of evidence-based strategies for addressing vaccine hesitancy has grown significantly, including evidence for effective communication and behavioral interventions. In this article, we review measles and rubella vaccines and vaccine hesitancy. We then provide an overview of evidence-based strategies for addressing vaccine hesitancy, including communication strategies and behavioral interventions. This article is relevant to healthcare professionals, health system leaders, public health professionals, policymakers, community leaders, and any individuals who have a role in addressing vaccine hesitancy in their communities. Finally, we review future directions and major areas of research need.

Review of the measles epidemic in children from Central Eastern Europe in the third millennium

Abstract:Measles is an extremely contagious viral disease. Even though a safe vaccine exists for this disease, it remains one of the leading causes of mortality and morbidity in infants and young children. We aimed to create a retrospective descriptive study in which to analyze the evolution of the measles epidemic at the European level. The documentation was carried out using European Centre for Disease Prevention and Control (ECDC) and World Health Organization (WHO) statistics. At the same time, we present the epidemic's evolution in Romania, using data provided by the Romanian National Institute of Public Health and Ministry of Health. European statistical data indicate a high number of patients diagnosed with measles both among children and adults. All European countries benefit from the measles vaccination in the form of the measles-mumps-rubella (MMR) trivaccine included in their National Vaccination Programme. The vaccination schedule varies from country to country. In the vaccination scheme, most European countries include two doses of the MMR vaccine among the pediatric population. Romania registered a limited number of cases of measles between 1980 and 2015 following the introduction of the measles vaccination in the National Vaccination Programme. Since 2005, the Romanian vaccination schedule includes two doses of MMR trivaccine, administered at 12 months and at 6-7 years. After 2015, as a result of a significant decrease in the number of vaccinated children, Romania is facing a major increase in the cases of measles. Consecutively, a measles epidemic was declared in 2016 and an additional dose of mandatory MMR trivaccine was introduced at the age of 9 months. After 4 years of this schedule of administration, starting with August 2020, the additional dose of MMR administered during infancy has been discontinued. We propose an analysis of the factors that influenced the downward evolution of the measles epidemic in Romania at the beginning of the third millennium. Issues related to the limitation of interpersonal contact in the context of the social distancing imposed by the Sars-CoV-2 virus pandemic are discussed. We consider necessary a detailed documentation of the percentage of new disease cases that will appear in the pediatric population in the near future, in the context of the resumption of daily activity after the reopening of nurseries, kindergartens and schools.

Abstract:A woman in her 40s known to have systemic lupus erythematosus presented with a maculopapular rash on her face, neck and chest following measles exposure. She had received a single-dose measles vaccine as a child in the 1970s and was therefore presumed to be immune, and thus not infectious. As a result, she was initially managed in an open bay. Measles virus IgM antibody in serum was undetectable; however, measles virus RNA was subsequently detected in throat swab by PCR, which is consistent with current infection. Measles is one of the most transmissible diseases in the world and cases are rising both in the UK and globally. Our case and literature review highlight the risk of vaccine failure in measles, especially in people who have not received two doses of the measles, mumps and rubella vaccine. It also highlights the challenges in diagnosing measles in previously vaccinated individuals.
Measles Vaccine Coverage and Disease Outbreaks: A Systematic Review of the Early Impact of COVID-19 in Low and Lower-Middle Income Countries

Abstract:Objectives: We aimed to evaluate changes to measles-containing vaccine (MCV) provision and subsequent measles disease cases in low- and lower-middle income countries (LICs, LMICs) in relation to the COVID-19 pandemic. Methods: A systematic search was conducted of MEDLINE, OVID EMBASE and PubMed records. Primary quantitative and qualitative research studies published from January 2020 were included if they reported on COVID-19 impact on MCV provision and/or measles outbreak rates within LICs and LMICs. Results: 45 studies were included. The change in MCV1 vaccination coverage in national and international regions ranged −13% to +44.4% from pre-COVID time periods. In local regions, the median MCV1 and overall EPI rate changed by −23.3% and −28.5% respectively. Median MCV2 rate was disproportionally impacted in local areas during COVID-interruption time-periods (−48.2%) with ongoing disruption in early-recovery time-periods (−17.7%). 8.9% of studies reported on vaccination status of confirmed measles cases; from these, 71%–91% had received no MCV dose. Conclusion: MCV vaccination coverage experienced ongoing disruption during the recovery periods after initial COVID-19 disruption. Vaccination in local area datasets notably experienced longer-term disruption compared to nationally reported figures.

Abstract:Objective Measles, predominantly impacting the pediatric population, manifests symptoms encompassing fever, rash, rhinorrhea, cough, conjunctivitis, accompanied by infrequent yet plausible life-threatening complications. Methods This retrospective study conducted between January 2022 and July 2023 at a 1,105-bed tertiary hospital in Istanbul, Turkey, was focused on children diagnosed with measles and aged <18 years. Cases with measles-specific immunoglobulin M antibodies or with measles virus RNA detected by polymerase chain reaction were defined as definite measles cases. Results A total of 78 pediatric patients were diagnosed with measles, of whom 40 were evaluated as definite (51.3%) cases. Among the definite cases, 29 (72.5%) and 11 (27.5%) were girls and boys, respectively. Their median age was 78 (42–158) months. It was observed that 28 (70%) patients were unvaccinated against measles, whereas data on the vaccination status of 6 (15%) patients could not be obtained. As for the symptoms, rash was observed in all diagnosed patients (100%), fever in 37 (92.5%), cough in 27 (67.5%). In addition, pneumonia was observed in 20 patients (50%). Vitamin A was administered to 5 (12.5%) patients below 2 years old. Of the patients, 29 (72.5%) were hospitalized. Three patients required intensive care unit follow-up. No mortality was recorded. When outpatients and inpatients were compared, the incidence rate of pneumonia was statistically higher in inpatients ( p = 0.013). Conclusions Recent World Health Organization data reveal a surge in childhood measles instances, underscoring the imperative of bolstering vaccination rates to fortify societal immunity and safeguard individuals against potential complications. Abstract:Relevance . Measles infection still does not lose its relevance, as experts everywhere register outbreaks of the disease. The aim of the study is characterization of the measles epidemic process in the context of different strategies for its vaccination in a large industrial city to make adequate management decisions. Materials and methods. The authors used the data of statistical reports on measles incidence in Yekaterinburg in 1950–2017 as the study materials. The authors analyzed data for six periods: pre-vaccination (1950–1961), selective immunization (1962–1965), routine vaccination of children under 8 years (1966–1972), vaccination of children under 14 years (1973–1986), the introduction of mass revaccination of children (1987–2001) and the period of universal vaccination at the stage of elimination of infection (2002–2017). Results. In the pre-vaccination period, the average annual incidence rate was 1381.7 ± 162.9 per 100 ths population, the seasonal increase in the incidence was in december–may, in the structure of age groups dominated by children. During the period of selective immunization, at the stage of testing of the domestic vaccine, there was a slight decrease in the incidence to 1082.8 ± 189.1 per 100 ths population, intra-annual dynamics and age distribution of patients remained virtually unchanged. During routine vaccination of children up to 8 years of age, there was a significant reduction in the incidence to the level of 219.8 ± 110.8 per 100 ths population , which was observed in almost all age groups, with the exception of children 10–14 years and adults. Seasonal manifestations of the epidemic process were similar to previous periods. With an increase in the cohort for vaccination at the expense of children up to 14 years, there was a further decrease in the incidence to 89.9 ± 39.1 per 100 ths population, but in some years measles outbreaks were registered, with an active spread of infection among schoolchildren in educational institutions. The decision to introduce a second vaccination for 6-year olds before school made it possible to achieve a sporadic level of morbidity and to change the main parameters characterizing the epidemic process in all previous periods. The epidemic process has become intermittent. Periodic rises disappeared. When the infection was not spread, the population immunity and vaccination rates were consistent with the recommendations of who experts. However, against this background, 2016 was registered a major outbreak of measles. Conclusion. The use of different tactics of vaccination led to significant changes in the parameters of the epidemic process of measles. However, at the present stage immunization schemes do not control the situation. In this regard, it is necessary to raise the issue of introduction of revaccination against adult measles every 10 years, as well as changes in the antigenic composition of the live measles vaccine, taking into account data on circulating strains of the virus of genetic lines D and H.
Case Ascertainment of Measles during a Large Outbreak—Laboratory Compared to Epidemiological Confirmation

Abstract:Measles is a highly contagious viral disease, and hence, sufficient herd immunity is obligatory to prevent infection transmission. Measles is still a cause of considerable disease burden globally, mainly in children. During a national measles outbreak in Israel in 2018–2019, the peak incidence rates occurred in the Jerusalem district. Most measles cases in the Jerusalem district (75.5%, 1702) were observed in children younger than 15 years of age, 49.2% (1109) were in children under 5 years of age, and 18.9% (425) were in infants under 1 year of age. The routine measles vaccination schedule includes two doses at 1 and 6 years of age. Most cases (1828, 81.1%) were unvaccinated (zero measles vaccine doses). These cases comprised the 425 affected infants under 1 year of age, who were ineligible for vaccination, along with the 1403 children over 1 year of age, who were otherwise unvaccinated. This study aimed to describe the epidemiologic and laboratory features of this measles outbreak, and to investigate case ascertainment (laboratory confirmed compared to epidemiologically confirmed cases). The study population included 2254 measles cases notified during the period spanning June 2018 to May 2019 in the Jerusalem district (incidence rate 176 per 10,000 population). Of the 2254 cases, 716 (31.8%) were laboratory confirmed, and 1538 (68.2%) were confirmed as epidemiologically linked. Most laboratory confirmed cases (420, 58.7%) underwent real-time PCR tests. Serological tests (measles IgM and IgG) were used in 189 (26.4%) cases, and a combination of RT-PCR and serology was used in 107 (14.9%) cases. In a multivariate model analysis, the variables significantly associated (after adjustment) with higher odds for laboratory confirmation included month of disease onset (late), additional measles cases in the household (single case), place of medical treatment (hospital; either emergency department, or hospitalization) and vaccination status (at least one prior vaccine dose). The measles outbreak described demonstrates the urgency of addressing vaccination gaps with appropriate outbreak prevention programs. The road to measles elimination needs to be paved with robust public health infrastructure, excellent field epidemiology for outbreak surveillance, investigation, and control, and laboratory proficiency.

On 19 January the UK Health Security Agency (UKHSA) declared a national incident in England amid spiralling cases and a drop in vaccine uptake.Andrew Wakefield's fraudulent and long discredited research linking the measles, mumps, and rubella (MMR) vaccine with autism has cast a long shadow. It remains part of the picture but the reasons behind falling vaccine rates are now more complex, GPs and public health doctors say.Last year only 84.5% of 5 year olds in England had had both doses of the MMR vaccine, a figure that hides pockets of particularly low coverage. In Birmingham, the epicentre of the current measles outbreak, uptake was 75%. The World Health Organization established measles elimination as a goal for 2020 but, unfortunately, this objective has not been achieved yet and outbreaks still occur. Herd immunity, ie, a population immunity higher than 95%, is required to stop the measles virus transmission. Communication plays a crucial role in immunization strategy to obtain high coverage levels, as it helps to fight barriers against vaccination. Delay and refusal of measles vaccination have become widespread due to misinformation, fake news and barriers to effective communication. This phenomenon has been defined as "vaccine hesitancy" and is considered as one of the top ten risks for global health. The alleged association between measles vaccination and autism has caused a sharp decline in vaccination rates. In this current situation, mass communication integrated into public health policies is fundamental to sway people's positive attitudes toward vaccination. Digital communication strategies based on social media and other internet platforms may represent useful tools to promote immunization and discourage skepticism and complement information provided by health-care professionals who have been considered as the most credible source on risk/benefits on vaccines for families. Digital communication strategies that may help supporting the measles elimination strategy include monitoring information needs online, integrating digital communication into immunization programs, involving a multidisciplinary group in communication, developing content that balances facts with positive messaging, using multiple communication channels. Abstract:Significant outbreaks of measles are occurring throughout the world, including the United States, the United Kingdom, India, and other areas with significant rates of organ transplantation, fed in part by vaccine reluctance and lower rates of childhood vaccination. 1 , 2 Measles is transmitted via airborne spread or respiratory droplets and is among the most contagious of pathogens with infection occurring in up to 90% of nonimmune close contacts 3 ; a single case of infection is estimated to cause 12–18 secondary cases in susceptible individuals 1 , 4 and may be even more contagious in susceptible immunocompromised populations. Measles typically presents with fever, maculopapular rash, conjunctivitis, cough, and coryza, although such symptoms may be muted in transplant recipients, who may present without a rash. 5 , 6 Serious manifestations of the disease include pneumonia due to measles virus or superimposed secondary infection and acute disseminated encephalomyelitis, a demyelinating disease of the central nervous system that develops in 1:1000 cases. 5 There is no effective antiviral treatment for measles and the management is supportive. 5 Beyond the acute mortality, long-term sequelae also contribute to morbidity and delayed mortality: disability related to permanent brain damage from acute encephalomyelitis; measles inclusion body encephalitis, which occurs typically in immunocompromised individuals 6–12 mo after infection; and subacute sclerosing panencephalitis, a fatal progressive neurodegenerative disease that occurs years later in roughly 1:10 000 of cases. 1 , 5 , 7 Rates and outcomes in immunocompromised patients are largely unstudied; as with so many other infections, they are likely at higher risk for severe outcomes. 6
