Abstract:This paper proposes a method to improve the process model estimation based on limited experimental data by making use of abundant production data and to achieve the optimal process adjustment based on the improved process model. The proposed method is called an Estimation‐adjustment (EA) method. Furthermore, this paper proves three properties associated with the EA, which guarantee the feasibility and effectiveness of using EA for integrating production and experimental data for optimal process adjustment. Also, the paper develops a sequential hypothesis testing procedure for implementing the EA. The properties and implementation of the EA are demonstrated in a cotton spinning process. The Contest between Life-Oriented and Specialization : A Study on the Self-treatment Phenomenon in Ming and Qing Dynasties in China

Abstract:It is natural for people to think that the increasing popularity of seeking medical treatment and the continuous development of medicine are the important embodiment of the progress of human civilization. Generally speaking, the progress of medical technology and the abundance of medical resources mean that the phenomenon of self-treatment is reduced. However, this study of Chinese medical history found that it had always been a common choice to seek self-treatment for people in all social classes, from literati and bureaucrats to ordinary people in remote mountainous areas, especially during the Ming and Qing Dynasties in China. Such records can be seen especially when looking through historical materials such as medical books, local chronicles, genealogies, notes, and anthologies. Although medical skills in Ming-Qing era were more developed than previous eras and the local medical resources were more abundant, from ordinary people in remote areas to elite groups such as literati and officials, people often regarded self-treatment as a basic choice when they or their family members fell ill, no matter whether they had the ability or the opportunity to delay medical treatment. This article aims to explore the logic and root behind this seemingly contradictory phenomenon. Therefore, this research first raises the question: what accounted for this seemingly contradictory phenomenon? In response to this question, this study confirms the following: Firstly, low barriers to entry and high rewards drove people of varying qualifications into the medical profession, and good doctors were hard to find; thus, self-treatment was no worse than the alternatives. Secondly, in the Ming-Qing dynasties, Confucian physicians tended to write more practical prescriptions, which gave many people the opportunity to improve their own medical knowledge and provided great convenience for self-treatment. Thirdly, since the mid-Ming Dynasty, the high medical cost had caused serious difficulties for ordinary people to seek professional medical care, so they tried their best to save themselves when they fell ill. On this basis, this study further found that the deeper reason for this phenomenon lies in the incoordination between the high degree of daily life style of traditional medical treatment and the corresponding low degree of medical specialization and professionalism. Then, this leads to another question: is this incongruity a historical norm or a product of a certain historical stage? This article argues that during the Ming-Qing era, in the macro trend, the two showed a competitive relationship. However, in the context of a broader and more microscopic time and space, is this competitive relationship valid? Especially at present, chronic non-communicable diseases are becoming the biggest threat to human health. With the professional development of medical technology, the trend of medical life-style has also become a reasonable way to fill the gap from highly authoritative medical treatment and take on the responsibilities of their own health. This kind of medical specialization and life-style seems to be developing in parallel. Therefore, it can be concluded that the dissonance between the two is only the product of a certain historical period. Finally, this study raises another relevant question: what are the social and cultural implications behind the parallel development of medical life and specialization? This is a question to be answered in future research on the topic.
[Development of integrated traditional Chinese and western medicine and change of medical policy in China]

Abstract:"Sect of Integration of Chinese and Western Medicine" came into the world four hundred years ago when Traditional Chinese Medicine (TCM) contacted Western Medicine (WM) at the beginning of the 17th century. It collected historical experiences showing that the cooperation of TCM and WM is more efficient for the cure and prevention of disease than each of them separately. Now the recognition that the cooperation of eastern and western medicine is more efficient to cure disease is spreading widely. This study will help Korean eastern and western medicine to find their directions. First, the concept of "sect of Integration of Chinese and Western Medicine" which was established between the beginning of the 17th century and the middle of the 20th century, and Integration of Traditional Chinese and Western Medicine (ITCWM) which was formed after the middle of the 20th century will be discussed. The relationship of "sect of Integration of Chinese and Western Medicine" and ITCWM and political consideration for the establishment of ITCWM will also be discussed. Finally, the current status of ITCWM in China will be discussed. New trends of thought appeared in Chinese medicine, owing to the cultural background of modern China, the development of WM, and the academic background of the intellectual class. "Sect of Integration of Chinese and Western Medicine" and ITCWM are different in historical and social background. However, purpose, foundation of thoughts and logical idea are fundamentally the same. It can be said that "sect of Integration of Chinese and Western Medicine" provided academic mood to open the way for ITCWM and ITCWM is a succession of "sect of Integration of Chinese and Western Medicine". The concept of ITCWM has many ways of explanation. However, it can be said to build up the foundation of new medical area including Chinese special way of medical treatment and new methods of modern medicine, succeeding a legacy of TCM. ITCWM began before the establishment of People's Republic of China. Mao Ze-dong (1893-1976), a powerful politician, and Li Ding-ming (1881-1947) who had many experiences and insight for TCM and WM played important roles at this stage. The period from the New China to the Great Proletarian Cultural Revolution (1966-1975) is the term for the establishment of the shape of ITCWM. "The effort of research and development on TCM-WM integration" was adapted as one of hygienic policies for curing of epidemic disease and succession and development of the heritage of TCM to establish new medical area. TCM class for western medical doctors was opened and mass media was used to spread out ITCWM throughout China. During the period of the Great Proletarian Cultural Revolution, ITCWM had to be stepped back and stagnant. Only the TCM class of western medical doctors and some clinical applications were barely kept moving on and alive. From the period of the Great Proletarian Cultural Revolution to the end of the 1980s, there are the movement of re-preparation of ITCWM, education of successors, and the establishment of the Institute of ITCWM. Hospitals began to establish department of ITCWM. Furthermore, it was clearly indicated in the constitutional law that "We not only have to develop modern medicine but also traditional medicine". The equality of TCM and WM was legally established in this time. From the 1990s, "equality of TCM and WM" was adapted as one of the hygienic policies, and department of ITCWM was opened in traditional Chinese medical school and western medical school. ITCWM has been settled down as a new academic field through education, training, research, academic activity, and publishing text books. In conclusion, the motive of the development of ITCWM was the policy such as "the effort of research and development on TCM-WM integration" abd "equality of TCM and WM" aimed at the development of Chinese medical area. It is no doubt helpful to organize systems and policy-making for the cooperation of eastern and western medicine in Korea.

Abstract:China is the world's most populous country with total population of more than 1.3 billion by the end of 2006 in Mainland China (Table 1). Over recent years its economic development has been remarkable. In tandem with this and as a result of both this increasing prosperity and the one-child policy, its population is getting generally older, has more chronic disorders, and has higher expectations for better health care. Increasing demand for health care and economic liberalism has, in the most prosperous eastern provinces, led to an explosion in acute care. For example Hangzhou City (Eastern of China) with a population of about 7 million has around 60 hospitals with 25 000 beds. In the longer term, focusing healthcare resources on acute hospitals is an expensive option and is likely to exclude the less advantaged from formal health care. View this table: Table 1 Basic heath statistics in Mainland China by end of 2006.5 Family medicine is organisationally suited to the management of chronic disorders; it is also cheaper. For example, in Hangzhou City a family medicine consultation for diabetes costs 40 renminbi (RMB) compared with a typical hospital appointment that costs 120 RMB (average salary is 4000 RMB/month, 1 RMB = 1/15 pound sterling). In this article the development of family medicine in China, it current status, and future prospects are reviewed. Abstract:After the Russo-Japanese War of 1905,Japan ambitiously promoted public health services in the major cities along the railway and in ports in South Manchuria.At the same time,local Chinese authorities also actively established public health facilities.The two sides competed fiercely over leadership in public health services,and to some extent their endeavors improved the sanitary conditions in certain areas.However,since this was an expansion/resistance development model,additional political considerations were put into it beyond the treatment and prevention of disease,and the result was that the medical and sanitary conditions in urban slum areas and rural areas did not get significant attention or considerable investment.For a long time,many infectious diseases continued prevail in those areas.In this sense,the public health services promoted by Japan were naturally planned with an eye to colonial expansion and imperialist governance,while the "self-strengthening public health" and "patriotic medicine" carried out by local Chinese governments and local society can perhaps be regarded as efforts pursued under duress.

Abstract:China is the only country in the world where Western medicine and traditional medicine are practised alongside each other at every level of the healthcare system. Traditional Chinese medicine has a unique theoretical and practical approach to the treatment of disease, which has developed over thousands of years. Traditional treatments include herbal remedies, acupuncture, acupressure and massage, and moxibustion. They account for around 40% of all health care delivered in China. The current government policy of expansion of traditional facilities and manpower is being questioned because many hospitals using traditional Chinese medicine are already underutilized and depend on government subsidies for survival. Research priorities include randomised controlled trials of common treatments and analysis of the active agents in herbal remedies. As more studies show the clinical effectiveness of traditional Chinese medicine, an integrated approach to disease using a combination of Western medicine and traditional approaches becomes a possibility for the future.

[Unification of Oriental and Western medicine with study on Oriental and Western medicine]

Abstract:In Korea the system of medical management has been bifurcated into two parts since the restoration of nation from Japanese after World War II. One is 'Western Medicine' and the other is 'Oriental Medicine' like Chinese but not like Japanese. The authors attempted to study on the unification of both medicines to prevent the confusion of medicare for people. The major part of medical care is accomplished by Western medicine among people in Korea and China. However, Oriental medicine is still present as well as college of Oriental medicine in small number. There have been long struggles between two parts of medicine because of each assertions for the theories. The ancient medicine has been likely to have its characteristics either in Western or Oriental medicine although in many countries were succeeded by Western medicine except a few Asian countries such as Korea and China. In Japan since the license of Oriental(herb) medicine was ceased about one hundred years ago, the Western medicine has been authorized by law until today and the herb is the secondary medicine as a kind of folk treatment as well as continuous and enormous study for scientification. In only China and Korea this herb medicine has been kept to use as a part of medicine by law though China has developed combined medicine to use both Western and Oriental medicine by one physician since previous prime minister Chou En-Lai around the year 1950 who made a recommendation to the two medical societies, Western and Oriental. This fact has a big sense to establish the unification of two medicines in near future in Korea as well as China. For this accomplishment of medical unification both parts of medicine require sincere and enormous efforts to study the theory and practice of herb medicine even from now on. This unification of two medicines will provide the best medicare in most convenient way to the people in the world creating new world medicine like the third medicine including alternative medicine. The acupuncture is changing to a new method in Western medicine especially applying into anesthesia and pain clinic in even USA and Western countries at this time. In conclusion the unification of the Western and Oriental medicine will provide the most reasonable and practical medicare including alternative medicine to people in the world. Because the Oriental medicine will be used at least separately to supplement the Western medicine by one physician.
The Spread of Hygiene Concepts and the Making of Public Health Discourse in Modern East Asia

Abstract:If public health can be defined as "all activities to ensure universal medical use of the people and protect and promote health," it can be said that public health emerged in the process of developing the concept of hygiene in East Asia. While traditional hygiene emphasized individual curing and longevity, modern hygiene was the state in charge of individual body and discipline. East Asian countries had to practice modern tasks in the field of hygiene and medical care in line with the construction of modern countries, and it was considered legitimate for modern countries to intervene in individual bodies. As the demand for modern national construction became stronger, interest in public health rather than personal hygiene increased. In East Asia, a new interpretation of the concept of hygiene began in Japan. Sensai Nagayo(1838-1902) newly defined the concept of 'sanitation' to justify the physical intervention of the modern state in Meiji period. The concept of 'public health' began to be used in earnest in 1890, when Ogai Mori(1862-1922) translated Western-style health protection measures for the public as public health. Since then, public health has evolved into a universal social discourse in Japan. Japan's public health expanded to colonial Joseon, Taiwan, and China. Japan's victory in the Sino-Japanese War led East Asian countries to believe that hygiene was the root of the Japanese nation's power. In the early 20th century, the government of the Republic of China began to imitate the case of Japan while promoting modern education reform and institutional reform. Japanese-style 'public health' was transplanted into various hygiene laws and sanitary equipment. In Korea, modern hygiene was introduced and spread from the end of the 19th century to the first half of the 20th century, and the concept of 'public health' in Japan was mainly spreading. Public health in Japan was vaguely defined as an activity to protect and promote the health of the people, but in practice, it was focused on improving quarantine and environmental infrastructure. In response, the concept of American-style public health, which values prevention and treatment at the same time, has already begun to emerge under the Japanese colonial rule. In East Asia in the 1920s and 1930s, Japanese-style public health and American-style public health discourse competed, and measures to solve medical inequality were discussed in earnest. Interestingly, in common in East Asian countries, Actual Medical Expenses Campaigns to improve medical access at low cost and social medicine to universally provide prevention and treatment to the people have drawn attention. This was also a phenomenon caused by intensifying medical inequality as rapid urbanization and industrialization progressed in East Asian countries in the first half of the 20th century. Although it was impossible to resolve social contradictions or move toward fundamental reform of the national medical system due to the nature of the private movement, the actual medical movement further imprinted the need for public health care in the country and society. Social medicine studied the effects and relationships of the social environment on diseases and health, and studied ways to promote public health by using preventive medicine and therapeutic medicine. If social medicine was supported by state power, it was possible to go forward with practice such as State Medicine like China, otherwise it would only be a civilian movement such as the People's Health Movement, as in colonial Korea. Liberation and the Korean War were a dramatic turning point in American-style health that led to Japanese-style hygiene. Immediately after Liberation, there was a discussion between the left and right camps over medical nationalization to enhance the publicity of medical care. The medical community was sympathetic to the nationalization of medical care, but due to the lack of medical personnel and financial resources, specific alternatives could not be proposed. As American-style health studies gradually expanded their influence after the Korean War, American-style public health, which emphasized prevention and treatment activities, became established, and efforts were made to establish a health center system.

Abstract:Wisconsin officials during the 1990s seemed poised to enact innovative and comprehensive health care reform. During that era, an ambitious, popular, and reform-minded governor led the state. The state had an unusually professional legislature. The state's economy was strong. Even with these advantages, however, the report card on the state's efforts is mixed. The state enacted a fairly modest set of reforms that were financed largely by the federal government and subject to extensive federal oversight. The Wisconsin story thus seems to be about the politics of incrementalism. But while critics of incrementalist politics point out that the number of uninsured continues to grow, the catalytic federalism witnessed in Wisconsin in the 1990s may well be the best model for implementing health care reform.

Abstract:The Confucian culture, rich in its contents and great in its significance, exerted on the thinking, culture and political life of ancient China immense influences, unparalleled by any other school of thought or culture. Confucian theories on morality and ethics, with 'goodness' as the core and 'rites' as the norm, served as the 'key notes' of the traditional medical ethics of China. The viewpoints of Confucianism on benevolence and material interests, on good and evil, on kindheartedness, and on character cultivation were all inherited by the medical workers and thus became prominent in Chinese traditional medical ethics. Hence, it is clear that the medical profession and Confucianism have long shared common goals in terms of ethics. Influenced by the excellent Confucian thinking and culture, a rather highly-developed system of Chinese traditional medical ethics emerged with a well-defined basic content, and the system has been followed and amended by medical professionals of all generations throughout Chinese history. This system, just to mention briefly, contains concepts such as the need: to attach great importance to the value of life; to do one's best to rescue the dying and to heal the wounded; to show concern to those who suffer from diseases; to practise medicine with honesty; to study medical skills painstakingly; to oppose a careless style of work; to comfort oneself in a dignified manner; to respect local customs and to be polite; to treat patients, noble or humble, equally, and to respect the academic achievements of others, etc. Of course, at the same time, Confucian culture has its own historical and class limitations, which exerted negative influences on traditional medical ethics. Now, if we are to keep up with the development of modern medicine, a serious topic must be addressed. That is how to retain the essence of our traditional medical ethics so as to maintain historic continuity and yet, at the same time, add on the new contents of medical ethics so as to incorporate modern features into our system. Therefore, when trying to reform medical practice in China, we are faced with the urgent need to inherit and promulgate the essence of Confucian ethics--discarding its obsolete concepts--while at the same time building up the new medical ethics that can meet the requirements of the outside world and the future.

Abstract:Honed by clinical wisdom accumulated over thousands of years, a time-tested system encompassing theory, system, structure, education, examination, and practice has evolved for traditional Chinese Medicine. Although unassuming, as characteristic of Oriental tradition and heritage, Chinese Medicine nonetheless has faithfully and steadfastly served to protect the health of the Chinese and neighboring populace over thousands of years. In Mainland China, Chinese Medicine has never been relegated to the status of alternative medicine. Instead, the government has spearheaded the modernization of Chinese Medicine since 1950. Instead of pitting the two major health protection systems, viz. Chinese Medicine and the Western system against each other in confrontations and competitions, recent efforts have been directed at integrating the complementary aspects of the two systems into a comprehensive system, integrative medicine. Young doctors, known as East-West Integrative Medical doctors (“New Generation” Chinese Medicine medical doctors in Taiwan) are trained in both disciplines to assume primary and secondary medical care in the community. The results are increasingly recognized and accepted by Western nations.

[Birth and succession of a current of learning in Korean medicine: the supporting yang current of learning]

Abstract:In this study, I aim to reveal how Lee Gyoojoons medicine has given birth to a current of learning, the supporting yang current of learning, and describe its historical significance. Before anything, I'd like to throw the question of whether if there were any currents within the traditional Korean medicine. There are no records of medical currents being widely discussed until now in medical history of Korea; however, the current of Lee Jema's sasang medicine is the most noticeable one. Among the contemporaries of Lee Jema, during the late Chosun, there was also another famed medical practitioner called Lee Gyoojoon. Lee Gyoojoon mainly practiced his medicine within Pohang, Gyeongsangbuk-do area, his apprentices have formed a group and have succeeded his medical practice. Based on the analyses of Lee Gyoojoon's apprentices and the Somun Oriental Medical Society, which is known as a successor group to Lee Gyoojoon's medicine today, they are fully satisfying the five requirements to establish a medical current: first, they held Lee Gyoojoon as the first and foremost, representative practitioner of their current; second, they advocate the supporting yang theory suggested by Lee Gyoojoon, which is originated from his theory of Mind; third, books such as the Major Essentials of Huangdi's Internal Classic Plain Questions, and the Double Grinded Medical Mirror, were being used as the main textbooks to educate their students or to practice medicine. Fourth, Lee Gyoojoon's medical ideas were being transcended quite clearly within his group of apprentices, including Seo Byungoh, Lee Wonse, and the Somun Oriental Medical Society. Fifth, Lee Gyoojoon's apprentices were first produced through the Sukgok School, however, nowadays they are being produced through medical groups formed by Lee Wonse, the Somun Oriental Medical Society, regarding the propagation of medical theories, compilation of textbooks, publication of academic journals, etc. Then, what do the existence of the supporting yang current have their significances in history? First of all, Heo Joon, the great medical practitioner in 16th century Chosun, have revealed through his book the Treasured Mirror of Eastern Medicine (TMEM), that the essence of Eastern medicine differentiated from South and North medicine of China is being transcended in Korean medicine. However, we have not got a clear conclusion on what his views of the essence of Eastern medicine is. The TMEM is the legacy of Neo-confucianism, dominant in the Chosun at the time, and is considered the reference which covers from Taoism to Korean Medicine, that is practical as well as systematical in categorizing illnesses, their respective prescriptions, and herbs. Maybe, it seems that such characteristics of the TMEM naturally led the medical practitioners and Confucian scholars, Lee Jema and Lee Gyoojoon to adopt its principles, and furthermore, possibly contributed in materializing the tradition of Eastern Medicine. Secondly, both currents appeared in the late period of Chosun dynasty. Then, weren't there any preceding medical currents before them? The bureaucratic and centralized government of the Chosun dynasty demanded and supplied talents through a nationwide examination system. However, since the late-16th century, a few family from the Chungin class have come to dominate the important medical positions as inheritance doctors , bringing about the expansion of the private medical sector, as well as growth in the number of medical practitioners. This naturally brought about fierce competition among the practitioners, and it is probable that the competition sparked the need for standardized groups and societies that follow a single medical doctrine or theory, to differentiate from the others. Probably, the birth of current of learning, which succeeded to Lee Jema and Lee Gyoojoon's medical theory, exists as an extension of this social background. The major changes in systems to build a new Chosun in 1894 brought about the abolitions of old and antique institutions. Inheritance doctors naturally collapsed, and every medical practitioners had to compete in an open market. However, Lee Jema and Lee Gyoojoon, as a medical practitioner and Confucian scholar, weren't from medical families; instead, they have successfully established and led their medical groups. The Sasang medicine current, which first began in the Hamhung area, had creative medical theories and excellent practices, naturally led the discourses traditional medicine in the center areas of the Korean peninsula. In contrast, the supporting yang current, more popular in the Youngnam area at one time, struggled to keep their current during the period of Korean War, National Industrialization and Modernization. And it was only Lee Wonse's personal dedication to the current that made it survive through the times. It was not until the late 1990s, when the apprentices have gathered Lee Gyoojoon's accomplishments, that formed the Somun Oriental Medical Society as well as the supporting yang current. In summary, the birth and the succession of the supporting yang current clearly depicts how the various traditional medical groups and societies on the periphery have survived and transcended through difficult times. And at the same time, they can provide chance to ruminate the historical flow of traditional medicine in Korea.

Abstract:Effects of prolonged darkness on nitrogenase activity in vivo, nitrogenase activity in vitro, and the amounts of nitrogenase proteins were studied in symbiotic Frankia. Plants of Alnus incana (L.) Moench in symbiosis with a local source of Frankia were grown for 9 to 10 weeks in an 18/6 hour light/darkness cycle. After 12 hours of a light period, the plants were exposed to darkness for up to 40 hours. Nitrogenase activity (acetylene reduction activity) of intact plants was measured repeatedly. Frankia vesicle clusters were prepared from the nodules with an anaerobic homogenization and filtration technique and were used for measurements of in vitro nitrogenase activity and for measurements of the amounts of nitrogenase proteins on Western blots. Antisera made against dinitrogenase reductase (Fe-protein) of Rhodospirillum rubrum and against dinitrogenase (MoFe-protein) of Azotobacter vinelandii were used. Western blots were made transparent and nitrogenase proteins were quantified spectrophotometrically. Nitrogenase activity both in vivo and in vitro decreased after about 23 hours of darkness and continued to decrease to about 25% and 16% of initial activity, respectively, after 40 hours. The amount of Fe-protein and MoFe-protein in Frankia of the same plants decreased to 60% and 35%, respectively, after 40 hours of darkness. Loss of nitrogenase activity thus appeared to be largely explained by loss of MoFe-protein.
[Changes and establishment of the principle of "Unity of traditional Chinese medicine and western medicine"]

Abstract:"Unity of traditional Chinese and western medicine" was one of the three major health work principles at the early founding of the People's Republic of China. It was not only a choice of political strategy in line with the lack of contemporary health-care resources, but also a part of the revolutionary heritage for more than 20 years in the process of the Chinese Communist Party from the preliminary "application of both traditional and western medicine" to the making of the principles of both "cooperation of traditional Chinese and western medicine" and "unity of traditional Chinese and western medicine". All this was closely related to the concrete environment of the Chinese health work of various stages of Yan'an period with strong revolutionary atmosphere and was not the professional demand of the health workers, rather, this principle set up in 1950 was the result of the careful consideration of Mao Zedong and an adjustment guided under the revolutionary framework of Neo-democracy set up under the guidance of the Common Principle which was a bettered adaptation to the contemporary national condition.

Abstract:Chengdu Traditional Chinese Medicine College is one of the first four TCM colleges in the People’s Republic of China founded in 1956. The central government made the establishment decision and Chengdu face the southwest of China to enroll. The College’s training objectives, educational system, and student number were all uniformly stipulated by the top. The College planned to train senior practitioners with Marxist and Leninist ideas and mastery of Chinese medicine academic knowledge and medical technology for Socialist Construction and people’s healthcare. The founding of the Chengdu College of TCM is on the track of modern Chinese medical history. Chinese medicine has become an issue to debate since modern times, in the face of the abolition crisis and scientific discussions, Chinese medicine education is learning from the West to promote. The close connection between traditional medicine and the mass reflects its cultural connotation. The rehabilitation of Chinese medicine as a medical discipline is inseparable from the ruling party’s support. Chinese medicine is a tool to consolidate the regime of the Chinese Communist Party. Founding the TCM colleges is beneficial for training many practitioners to serve the state power.
Hypercalcemia in Patients with Disseminated Coccidioidomycosis

Abstract:Objective: Granulomatous disorders may be associated with hypercalcemia. In sarcoidosis, the pathogenesis of hypercalcemia has been clarified, whereas in other granulomatous disorders, such as coccidioidomycosis, the mechanism is unclear. We present 13 patients with coccidioidomycosis and hypercalcemia to illustrate the clinical course and the mechanism of hypercalcemia. Methods: We retrospectively reviewed all patients admitted to Kern Medical Center, a 270‐bed public hospital, from 1990 through 1997 with coccidioidomycosis and a serum calcium level of greater than 10.5 mg/dL on at least 3 occasions. In addition, no other causes for hypercalcemia were identified. Results: The mean highest serum calcium level was 12.7 ± 1.8 mg/dL. All patients had disseminated disease. Six patients were nonambulatory and 4 had bone involvement. Of the 9 patients in whom parathyroid hormone was measured, it was normal in 6 and suppressed in 3. Of the 9 patients in whom 25‐hydroxyvitamin D was measured, it was normal in 6, suppressed in 2, and elevated in 1. Of the 7 patients in whom 1,25‐dihydroxyvitamin D was measured, it was normal in 3 and suppressed in 4. Urinary calcium was elevated in 2 patients, both of whom were ambulatory. Nonambulatory patients had significantly higher serum calcium levels (14.3 ± 1.0 mg/dL) than ambulatory patients (11.3 ± 0.46 mg/dL) (P < 0.001). Conclusions: The mechanism of hypercalcemia in coccidioidomycosis is unrelated to increased production of 1,25‐dihydroxyvitamin D. Nonambulatory status is associated with higher mean serum calcium.

Abstract:Medicine News, first published in 1880, was the earliest western medicine journal founded in China, playing an important role in the history of Chinese modern journals and the history of western medicine communication. Today, no original copies of this journal survive in China. Quotations, citations and comments of some newspapers and periodicals on it at the time, reveal that the publication aim of this journal was to disseminate knowledge of western medicine to the Chinese people, that its editorial policy was "showing western medicine is superior to traditional Chinese medicine" , and that its communication of western medicine knowledge was also based on this policy. Medicine News reported on an international medical academic conference held in London for the first time, and it is worth mentioning that thousands of people attended the meeting. Medicine News is known to have promoted western medicine communication in China to an advanced and more up-to-date level. At the same time, its editorial policy and strategy had a great influence on the editing and publication of western medicine journals in the late Qing Dynasty and the Republic of China.

The written debate of both sides was really another important yet sharp form of struggle between them. The debate, lasting several years, dealt with many aspects of TCM, including yin and yang, medical education, abolishment of traditional Chinese medicine, and national Medicine issue. This debate involved not only the medical circles itself, but also the mass, and even the political circles.
