Abstract:Gabexate mesilate (GM) is a trypsin inhibitor, and mainly used for treatment of various acute pancreatitis, including traumatic pancreatitis (TP), edematous pancreatitis, and acute necrotizing pancreatitis. However, due to the characteristics of pharmacokinetics, the clinical application of GM still needs frequently intravenous administration to keep the blood drug concentration, which is difficult to manage. Specially, when the blood supply of pancreas is directly damaged, intravenous administration is difficult to exert the optimum therapy effect. To address it, a novel thermosensitive in-situ gel of gabexate mesilate (GMTI) was developed, and the optimum formulation of GMTI containing 20.6% (w/w) P-407 and 5.79% (w/w) P188 with different concentrations of GM was used as a gelling solvent. The effective drug concentration on trypsin inhibition was examined after treatment with different concentrations of GMTI in vitro, and GM served as a positive control. The security of GMTI was evaluated by hematoxylin-eosin (HE) staining, and its curative effect on grade II pancreas injury was also evaluated by testing amylase (AMS), C-reactive protein (CRP) and trypsinogen activation peptide (TAP), and pathological analysis of the pancreas. The trypsin activity was slightly inhibited at 1.0 and 5.0 mg/mL in GM group and GMTI group, respectively (P<0.05 vs. P-407), and completely inhibited at 10.0 and 20.0 mg/mL (P<0.01 vs. P-407). After local injection of 10 mg/mL GMTI to rat leg muscular tissue, muscle fiber texture was normal, and there were no obvious red blood cells and infiltration of inflammatory cells. Furthermore, the expression of AMS, CRP and TAP was significantly increased in TP group as compared with control group (P<0.01), and significantly decreased in GM group as compared with TP group (P<0.01), and also slightly inhibited after 1.0 and 5.0 mg/mL GMTI treatment as compared with TP group (P<0.05), and significantly inhibited after 10.0 and 20.0 mg/mL GMTI treatment as compared with TP group (P<0.01). HE staining results demonstrated that pancreas cells were uniformly distributed in control group, and they were loosely arranged, partially dissolved, with deeply stained nuclei in TP group. Expectedly, after gradient GMTI treatment, pancreas cells were gradually restored to tight distribution, with slightly stained nuclei. This preliminary study indicated that GMTI could effectively inhibit pancreatic enzymes, and alleviate the severity of trauma-induced pancreatitis, and had a potential drug developing and clinic application value.
Gabexate Mesylate-Poloxamer 407 Conjugate Alleviates Sodium Taurocholate-Induced Severe Acute Pancreatitis in an Optimized Rat Model

Abstract:Background and aims: We have previously shown that gabexate mesylate-poloxamer 407 conjugate (GMTI) alleviates traumatic pancreatitis in rats. In this study, we evaluated the therapeutic effect of GMTI on sodium taurocholate-induced severe acute pancreatitis (SAP) in an optimized rat model. Methods: An SAP rat model was established via microinjection of 3.5% sodium taurocholate and retention in the bile duct for 1 min. SAP rats were administered GMTI via tail vein injection (i.v.) or tail vein injection + intraperitoneal injection (i.v. + i.p.). All rats were sacrificed at 12 h after treatment. Biochemical approach and enzyme-linked immunosorbent assay were performed to measure the serum levels of amylase (AMY), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6). Hematoxylin and eosin staining and TUNEL assay were conducted to examine histopathology and acinar cell apoptosis in the rat pancreas. Results: SAP was successfully induced in all model rats, as evidenced by progressively aggravating SAP symptoms and signs, pancreatic histopathological abnormalities, as well as elevated serum levels of TNF-α, IL-6, and AMY. The mortality rates at 1 h, 6 h, and 12 h were 0%, 0%, and 25%, respectively. GMTI therapy via i.v. or i.v. + i.p. significantly reduced pancreatic wet weights, ascites amounts, pathological scores, and circulating levels of TNF-α and IL-6 while promoting acinar cell apoptosis in SAP rats. GMTI therapy via i.v. + i.p. outperformed i.v. in improving pancreatic histology and reducing TNF-α and IL-6 serum levels in SAP rats. Conclusions: Our optimized SAP rat model is reliable and reproducible. GMTI therapy is a promising approach against SAP.
Elimination of Intracellular Ca2+ Overload by BAPTA‑AM Liposome Nanoparticles: A Promising Treatment for Acute Pancreatitis

Abstract:Calcium overload, a notable instigator of acute pancreatitis (AP), induces oxidative stress and an inflammatory cascade, subsequently activating both endogenous and exogenous apoptotic pathways. However, there is currently lack of available pharmaceutical interventions to alleviate AP by addressing calcium overload. In the present study, the potential clinical application of liposome nanoparticles (LNs) loaded with 1,2-bis(2-aminophenoxy)ethane-N,N,N',N'-tetraacetic acid tetrakis (acetoxymethyl ester) (BAPTA-AM), a cell-permeant calcium chelator, was investigated as a thera-peutic approach for the management of AP. To establish the experimental models in vitro, AR42J cells were exposed to high glucose/sodium oleate (HGO) to induce necrosis, and in vivo, intra-ductal taurocholate (TC) infusion was used to induce AP. The findings of the present study indicated that the use of BAPTA-AM-loaded LN (BLN) effectively and rapidly eliminated excessive Ca2+ and reactive oxygen species, suppressed mononuclear macrophage activation and the release of inflammatory cytokines, and mitigated pancreatic acinar cell apoptosis and necrosis induced by HGO. Furthermore, the systemic administration of BLN demonstrated promising therapeutic potential in the rat model of AP. Notably, BLN significantly enhanced the survival rates of rats subjected to the TC challenge, increasing from 37.5 to 75%. This improvement was attributed to the restoration of pancreatic function, as indicated by improved blood biochemistry indices and alleviation of pancreatic lesions. The potential therapeutic efficacy of BLN in rescuing patients with AP is likely attributed to its capacity to inhibit oxidative stress, prevent premature activation of zymogens and downregulate the expression of TNF-alpha, IL-6 and cathepsin B. Thus, BLN demonstrated promising value as a novel therapeutic approach for promptly alleviating the burden of intracellular Ca2+ overload in patients with AP.
Early Local Drug Therapy for Pancreatic Contusion and Laceration.

Abstract:Objectives: To study the therapeutic effect of early local drug therapy on pancreatic contusion and laceration. Methods: Twenty pigs were divided into 4 groups: model(PL), 1 ml of saline; medical protein glue (EC), 1 ml of medical protein glue; ulinastatin (UL), 50000U of ulinastatin; combined treatment (UE), 1 ml of medical protein glue and 50000U of ulinastatin. 30 min after model establishment, different groups received different local drug treatments. The pancreatic function, peritoneal effusion and pancreatic pathology were observed. Results: The UE group got the best therapeutic effect. The changes of pancreatic function and the peritoneal effusion were compared with PL group as follows. 0-6h: amylase (p < 0.01), lipase (p > 0.05), effusion (p < 0.01); 6-12h: amylase (p > 0.05), lipase (p < 0.01), effusion (p < 0.01); 12-24h: amylase (p < 0.01), lipase (p < 0.01), effusion (p < 0.01). Conclusions: Early local drug therapy in pancreatic contusion and laceration could effectively control the development of the disease and improve the prognosis. (C) 2019 IAP and EPC. Study of the Protective Effects of Dexamethasone on Ileum Mucosa Injury in Rats with Severe Acute Pancreatitis.

Abstract:Objectives: To study the protecting effects of dexamethasone on ileum mucosa injury of rats with severe acute pancreatilis (SAP).Methods: The SAP rats were prepared by improved Aho's methods. The plasma endotoxin and inflammatory mediators in serum were determined. The rat mortality, pathological changes of terminal ileum, nuclear factor kappa B (NF-kappa B), apoptotic indexes, and apoptotic related protein expression were observed.Results: The plasma endotoxin, inflammatory mediators, and NF-kappa B protein expression as well as pathological scores of the treatment group of ileum mucosa were lower than those of the model group at different time points. P selectin in model group significantly exceeded the dexamethasone treatment group at 3 and 6 hours (P < 0.01, P < 0.05). Caspase-3 protein expression in dexamethasone treatment group significantly exceeded the model group at 3 and 6 hours (P < 0.05), and apoptotic indexes were higher than those of the model group at 6 hours (P < 0.05), but Bax protein has shown no marked difference among groups.Conclusions: Dexamethasone can reduce the endotoxin level and inflammatory mediators and down-regulate NF-kappa B protein expression of ileum mucosa, and ileum mucosa epithelial cell apoptosis induction was involved as well. The tissue microarrays technique is of advantage in SAP study.
Glyceryl Trinitrate For Prevention Of Post-Ercp Pancreatitis And Improve The Rate Of Cannulation: A Meta-Analysis Of Prospective, Randomized, Controlled Trials

Abstract:Background: Acute pancreatitis is the most common complication of diagnostic and therapeutic endoscopic retrograde cholangiopancreatography (ERCP). Several clinical trials used glyceryl trinitrate (GTN) to prevent the incidence of post-ERCP pancreatitis (PEP). However, the results were still controversial.Objective: To conduct a meta-analysis of published, full-length, randomized controlled trials evaluating the effect of prophylactic GTN on the prevention of PEP, improve the rate of cannulation and the prevention of hyperamylasemia.Methods: Literature searches were conducted using PubMed, EMBASE, The Cochrane Library and Web of Knowledge databases, using keywords "post-ERCP" and "pancreatitis" and limited in randomized controlled trials.Results: Twelve RCTs involving 2649 patients were included. Eleven RCTs compared GTN with placebo for PEP prevention. Meta-analysis showed the overall incidence of PEP was significantly reduced by GTN treatment (RR 0.67; 95% CI, 0.52-0.87). Nevertheless, GTN administration did not decrease the incidence of moderate to severe PEP (RR 0.70; 95% CI, 0.42-1.15). Subgroup analyses revealed that GTN administered by sublingual was more effective than transdermal and topical in reducing the incidence of PEP. Besides, the prophylactic effect of GTN was far more obvious in the group of high PEP incidence than in the group of low PEP incidence. Additionally, the incidence of hyperamylasemia was significantly reduced by GTN treatment (RR 0.69; 95% CI, 0.54-0.90). No differences of the successful cannulation rate of bile ducts (RR 1.03; 95% CI, 0.99-1.06) attributable to GTN were observed.Conclusion: Prophylactic use of GTN reduced the overall incidence of PEP and hyperamylasemia. However, GTN was not helpful for the severity of PEP and the rate of cannulation.
Protective Effects and Mechanisms of Baicalin and Octreotide on Renal Injury of Rats with Severe Acute Pancreatitis.

Abstract:AIM: To investigate the protective effects and mechanisms of Baicalin and octreotide on renal injury of rats with severe acute pancreatitis (SAP).METHODS: One hundred and eighty SD rats were randomly assigned to the model group, Baicalin-treated group, octreotide-treated group and sham operation group. The mortality, plasma endotoxin level, contents of blood urea nitrogen (BUN), creatinine (CREA), phospholipase A(2) (PLA(2)), nitrogen monoxide (NO), tumor necrosis factor (TNF)-alpha, IL-6 and enclothelin-1 (ET-1) in serum, expression levels of renal Bax and Bcl-2 protein, apoptotic indexes and pathological changes of kidney were observed at 3, 6 and 12 h after operation.RESULTS: The renal pathological changes were milder in treated group than in model group. The survival at 12 h and renal apoptotic indexes at 6 h were significantly (P < 0.05) higher in treated group than in model group [66.67% vs 100%; 0.00 (0.02)% and 0.00 (0.04)% vs 0.00 (0.00)%, respectively]. The serum CREA content was markedly lower in octreotide-treated group than in model group at 3 h and 6 h (P < 0.01, 29.200 5.710 mu mol/L- vs 38.400 +/- 11.344 mu mol/L-; P < 0.05, 33.533 +/- 10.106 mu mol/L vs 45.154 +/- 17.435 mu mol/L, respectively). The expression level of renal Bax protein was not significantly different between model group and treated groups at all time points. The expression level of renal Bcl-2 protein was lower in Baicalin-treated group than in model group at 6 h [P < 0.001, 0.00 (0.00) grade score vs 3.00 (3.00) grade score]. The Bcl-2 expression level was lower in octreotide-treated group than in model group at 6 h and 12 h [P < 0.05, 0.00 (0.00) grade score vs 3.00 (3.00) grade score; 0.00 (0.00) grade score vs 0.00 (1.25) grade score, respectively]. The serum NO contents were lower in treated groups than in model group at 3 h and 12 h [P < 0.05, 57.50 (22.50) and 52.50 (15.00) mu mol/L vs 65.00 (7.50) mu mol/L; P < 0.01, 57.50 (27.50) and 45.00 (12.50) mu mol/L vs 74.10 (26.15) mu mol/L, respectively]. The plasma endotoxin content and serum BUN content (at 6 h and 12 h) were lower in treated groups than in model group. The contents of IL-6, ET-1, TNF-alpha (at 6 h) and PLA(2) (at 6 h and 12 h) were lower in treated groups than in model group [P < 0.001, 3.031 (0.870) and 2.646 (1.373) pg/mL vs 5.437 (1.025) pg/mL; 2.882 (1.392) and 3.076 (1.205) pg/mL v5 6.817 (0.810) pg/mL; 2.832 (0.597) and 2.462 (1.353) pg/mL vs 5.356 (0.747) pg/mL; 16.226 (3.174) and 14.855 (5.747) pg/mL vs 25.625 (7.973) pg/mL; 18.625 (5.780) and 15.185 (1.761) pg/mL vs 24.725 (3.759) pg/mL; 65.10 (27.51) and 47.60 (16.50) pg/mL v5 92.15 (23.12) pg/mL; 67.91 +/- 20.61 and 66.86 +/- 22.10 U/mL, 63.13 +/- 26.31 and 53.63 +/- 12.28 U/mL vs 101.46 14.67 and 105.33 +/- 18.10 U/mL, respectively].CONCLUSION: Both Baicalin and octreotide can protect the kidney of rats with severe acute pancreatitis. The therapeutic mechanisms of Baicalin and octreotide might be related to their inhibition of inflammatory mediators and induction of apoptosis. Triple-drug therapy to prevent pancreatic fistula after pancreatectomy in a rat model

Abstract:Background: Pancreatic fistula (PF) is one of post-operative complications in pancreatic surgery, but there is no consensus about the optimal treatment for PF. Our group has established a rat model of PF, and we conducted the present investigation to determine the efficacy of the triple-drug therapy (somatostatin analogue, gabexate mesilate, and imipenem/cilastatin) against PF using our rat model. Methods: In the PF rat model, the triple-drug therapy was administered to the treated (T) group (n = 4), and we compared the results with those of a control (C) group (n = 4). The rats were sacrificed on postoperative day 3 (POD 3) and the levels of amylase and lipase in serum and ascites were measured. The intra-abdominal adhesion was scored. Each pancreas was evaluated pathologically, and inflammation was scored. Results: The ascitic amylase levels on POD 3 were 1982 (1738-2249) IU/L in the C group and significantly lower at 136 (101-198) IU/L in the T group (p = 0.02). The ascitic lipase levels on POD 3 were 406 (265-478) U/L in the C group and significantly lower at 13 (7-17) U/L in the T group (p = 0.02). The intra-abdominal adhesion score on POD 3 was 2 (1-2) in the C group and significantly lower at 0 (0-1) in the T group (p = 0.02). The histological evaluation showed that the average of pancreatic inflammatory score was 8.5 (8-9) in the C group and significantly milder at 5 (5-7) in the T group (p = 0.01). Conclusion: Our findings suggest that the triple-drug therapy could be useful as a treatment for PF in clinical settings.
Effects Of Baicalin On Inflammatory Mediators And Pancreatic Acinar Cell Apoptosis In Rats With Sever Acute Pancreatitis

Abstract:BACKGROUND: To investigate the effects of Baicalin and Octreotide on inflammatory mediators and pancreatic acinar cells apoptosis of rats with severe acute pancreatitis (SAP).METHODS: SD rats were randomly divided into sham operated group (I group), model control group (II group), Baicalin treated group (III group) and Octreotide treated group (IV group). Each group was also divided into subgroup of 3, 6 and 12 h (n = 15). The mortality rate, ascites/body weight ratio as well as the level of endotoxin, NO and ET-I in blood were measured. The pathological severity score of pancreas, apoptotic indexes, and expression levels of Bax and Bcl-2 proteins in each group were investigated.RESULTS: The survival rate of III and IV group has a significant difference compared with II group (P-12 h < 0.05). The ascites volume, contents of inflammatory mediators in blood and pathological severity score of pancreas of III and TV group declined at different degrees compared to II group (P < 0.055 P < 0.01 or P < 0.001). Apoptotic index in III group was significantly higher than that in II group at 3 and 6 h (P-3,P- 6 h < 0.05). Apoptotic index in TV group was significantly higher than that in II group at pancreatic tail at 6 h (P-6 h < 0.05). Expression level of Bax in III group was significantly higher than that in II group (pancreatic head P-3 h,P-6 h < 0.01, pancreatic tail P-3 h < 0.0001).CONCLUSIONS: Compared with Octreotide in the treatment of SAP, the protective mechanisms of Baicalin include reducing the excessive inflammatory mediators' release, inducing the pancreatic acinar cells apoptosis.
Effect of Baicalin and Octreotide on the Expression Levels of P‐selectin Protein in Multiple Organs of Rats with Severe Acute Pancreatitis

Abstract:Aim:To investigate the effect of baicalin and octreotide on the expression levels of P-selectin protein in multiple organs of rats with severe acute pancreatitis and explore the underlying mechanism.Methods:Rats were randomly divided into sham-operated, model control, baicalin-treated and octreotide-treated groups. At 3, 6 and 12 h after operation, the mortality rates of rats, the contents of plasma endotoxin as well as serum NO and ET-1, the pathological changes in multiple organs, and the expression levels of P-selectin protein in each group were observed.Results:At 12 h after operation, the mortality rates of rats in treated groups were significantly lower than that in the model control group (P < 0.05), and the pathological severity scores in multiple organs in treated groups were also significantly lower than those in the model control group (P < 0.05). The contents of plasma endotoxin, serum PLA(2) (at 6 and 12 h after operation), ET-1 and NO (at 3 and 12 h after operation) in treated groups were significantly lower than those in the model control group (P < 0.05, P < 0.01 or P < 0.001). In the baicalin-treated group, the expression levels of P-selectin protein in liver (at 3 h after operation), kidney (at 3 and 6 h after operation), pancreas, lung and spleen were significantly lower than those in the model control group (P < 0.01). In the octreotide-treated group, the expression levels of this protein in lung, intestinal mucosa (at 6 and 12 h after operation), lymph nodes (at 3 and 6 h after operation), spleen and thymus were significantly lower than those in the model control group (P < 0.05). Additionally, the products of the staining intensity and positive rate of P-selectin protein in pancreas, spleen (at 3 h after operation), intestinal mucosa (at 6 h after operation), thymus (at 6 h after operation) and lung (at 6 h after operation) in treated groups were significantly lower than those in the model control group (P < 0.05).Conclusion:Both baicalin and octreotide can exert some protective effects on multiple organs and the former is superior to the latter in protecting pancreas. Furthermore, decreasing the expression levels of P-selectin protein in these organs is one of the possible mechanisms.
Early Peritoneal Lavage with Ulinastatin Improves Outcome and Enhances Multi-Organ Protection in a Model of Severe Acute Pancreatitis.

Abstract:The aim of this study was to investigate the effect of early peritoneal lavage with ulinastatin on the outcome of a rat model of severe acute pancreatitis (SAP). A total of 80 male Wistar rats were randomly divided into the following groups: Sham-operated (C), SAP model (M), saline lavage (SL), intravenous ulinastatin (IU), early ulinastatin lavage (EUL) and late ulinastatin lavage (LUL). Intraperitoneal lavage or injection were performed immediately subsequent to the establishment of the SAP model in groups SL, IU and EUL and 3 h later in group LUL. Intraperitoneal lavage with or without ulinastatin was performed for 3 h. The survival time of the rats in groups C, M, EUL and LUL was recorded over a 12-h period and the median survival time was calculated. At 3 h after the induction of SAP, histopathological analyses were performed and the biochemical parameters of groups C, M, SL, IU and EUL were assessed. Groups EUL and LUL exhibited an increased median survival time compared with Group M, with the survival time of the rats in group EUL markedly longer than that in the group LUL rats. Group SL, IU and EUL rats were found to have reduced plasma activities of amylase, lipase, aspartate transaminase and alanine transaminase, with the biggest change observed in the group EUL rats. Furthermore, the intervention in groups SL and EUL was more effective at reducing creatinine and urea levels than that in group IU. Rats in group EUL exhibited a greater inhibition of the SAP-induced increase in troponin T levels than rats in groups SL and IU. The pathological severity scores of the pancreas, liver, kidney and lung in group EUL were significantly lower than those in groups M and better than those in groups SL and IU. In conclusion, early intraperitoneal lavage with ulinastatin significantly improves the median survival time and protects multi-organ function in an SAP model.
Protective Effect of Dexamethasone on the Kidneys of Rats with Severe Acute Pancreatitis and its Mechanism.

Abstract:Objective To investigate the protective effect of dexamethasone on the kidneys of rats with severe acute pancreatitis and explore its mechanism.Methods The SAP rat models were established and divided into three groups: model control group,dexamethasone treatment group and sham operation group.At 3,6 and 12 h after the operation,the survival rate of rats,as well as pathological changes,expression levels of Bax and Bcl-2 proteins,and changes in apoptosis index in the kidneys were detected in each group.The contents of NO,MDA,ET-1,BUN and CREA in serum were also determined.Results The survival rate at 12 hours in the treatment group was higher than that in the model group,but it showed no significant difference between the two groups(P> 0.05).The contents and pathological scores of each serum indexes at all time points after operation in the treatment group were significantly lower than those in the model group(P<0.01,P<0.05,or P<0.001).The expression level of Bcl-2 protein and apoptosis index in the kidneys at all time points after operation in the treatment group were significantly higher than those in the model group(P<0.05,P<0.01),the expression of Bax protein was negative in each group(P>0.05).Conclusion The protective effect of dexamethasone on the kidney is achieved mainly through lowering the contents of inflammatory mediators in blood.Additionally,dexamethasone-induced renal cell apoptosis plays a supporting role during this process.
Potential of Sivelestat in Protection Against Severe Acute Pancreatitis-Associated Lung Injury in Rats

Abstract:The incidence of acute pancreatitis has been rising worldwide in the past few decades. Despite extensive research efforts, the population-based mortality from acute pancreatitis remains high. Since dysfunction of multiple vital organs, most importantly the lungs, is the major cause of early death in acute pancreatitis patients, developing effective strategies to manage lung injury has become one of the focuses of recent research efforts aiming at improving the outcome of patients with acute pancreatitis. In this study, we attempted to create a rat model of acute pancreatitis through intraductal infusion of taurocholate and to evaluate the potential of sivelestat, a synthetic neutrophil elastase inhibitor, in protection against acute pancreatitis-associated lung injury using this rat model. The results demonstrated that: (1) 5% sodium taurocholate successfully induced histopathologic and biochemical abnormalities in the circulation, lung and pancreas characteristic of human acute pancreatitis, including an increase in amylase concentration and a decrease in partial arterial oxygen pressure (PaO2) in the blood, increases in activities of myeloperoxidase (MPO) (a lung injury marker) and neutrophil elastase (a quantitative indicator of neutrophil infiltration), and levels of malondialdehyde (an indicator of lipid peroxidation) and tumor necrosis factor-alpha (a major inflammatory mediator) in the lung; (2) intravenous administration of sivelestat effectively attenuated the taurocholate-induced abnormalities in all parameters analyzed except for serum amylase concentration. Our findings have validated the taurocholate model of acute pancreatitis and demonstrated great therapeutic potential for sivelestat in managing acute pancreatitis-associated lung injury.

Abstract:Purpose. To study the protecting e. ffects of dexamethasone on thymus of rats with severe acute pancreatitis (SAP). Methods. The SAP rats were randomly assigned to the model group and dexamethasone-treated group, the other normal healthy rats were assigned to the sham operation group. The rat survival, thymus pathological changes, apoptotic index, as well as expression levels of NF-kappa B, P-selectin, Bax, Bcl-2, and Caspase-3 protein of all groups were observed, respectively, at 3 hours, 6 hours, and 12 hours. The contents of amylase and endotoxin in plasma as well as the contents of TNF-alpha, PLA(2), and NO in serum were determined. Results. There was no marked di. fference between the model group and treated group in survival. The contents of di. fferent indexes in blood of treated group were lower than those of the model group to various degrees at di. fferent time points. The thymus pathological score was lower in treated group than in model group at 12 hours. The treated group in Caspase-3 protein expression of thymus significantly exceeded the model group at 12 hours. The apoptotic index was significantly higher in treated group than in model group. Conclusion. Dexamethasone has protecting e. ffects on thymus of SAP rats. Copyright (c) 2007.
Influence of exogenic prostaglandin E1 on metabolic changes of eicosanoids in SD rats with experimental acute pancreatitis

Abstract:Objective To study the serum changes of endogenous prostaglandin E2(PGE2), 6-keto-prostaglandin F1 alpha (PGF1α), and thromboxane B2(TXB2) in SD rats with tauracholate-induced necrohemorrhagic pancreatitis and to probe into the relationship between the change and the scores of pancreatic histological damages. Moreover, the influence of exogenic prostaglandin E1 on the metabolic changes of the PGE2, PGF1α, and TXB2 was studied. Methods Ninety SD rats were evenly randomized into control group, necrohemorrhagic pancreatitis group (model group), and PGE1 therapy group. Rats in control group were administered with 0.9% saline at low pressure into the pancreatic duct; rats in model group of were induced with introductal administration of 3.5% sodium tauracolate(0.1 ml/100 g body weight); rats in PGE1 therapy group were treated with peritoneal PGE1(3.0 μg/kg body weight) 1 h before and after pancreatitis induction. The serum and histological samples were harvested at 3, 6, and 12 hours after pancreatitis induction in each group. Results Compared with control group, model group had significantly increased serum levels of amylase (AML) and TXB2(P < 0.01), but decreased levels of PGE2(P < 0.015)and PGF1α(P > 0.05). Injection of PGE1 significantly lowered the enhanced levels of AML and TXB2 in pancreatitis models at different periods after injection(P < 0.01); but increased the levels of PGE2 and PGF1αcompared with those in group ANP(P > 0.05). The histological evaluation scores of pancreatic necrosis and haemorrhage in PGE1 therapy group were significantly lower than those in model group; the difference of pancreatic edema and leucocyte infiltration was not significant between the 2 groups. The scores of pancreatic necrosis and haemorrhage were closely related with serum levels of amylase and TXB2. Conclusions During experimental necrohemorrhagic pancreatitis, the serum TXB2 level increases significantly, serum PGE2 level decreases markedly, serum PGF1αlevel decreases but without significance. Exogenous PGE1 can reverse the above changes to a certain extent. The scores of pancreatic necrosis and haemorrhage are positively related with serum levels of amylase and TXB2. It is indicates that abnormal metabolism of eicosanoids may have a key role in the development and prognosis of necrohemorrhagic pancreatitis; exogenous PGE1 may influence the development of pancreatitis through regulating this abnormal metabolism of eicosanoids.

Abstract:The intravenous administration of d‐lysergic acid diethylamide (LSD) to pregnant female rabbits induced hyperthermia as well as disaggregation of polysomes in fetal organs and maternal brain. The LSD‐induced polysome shift in maternal brain and fetal organs was not due to an activation of ribonuclease or associated with alterations in the levels of free amino acids. Pretreatment with the receptor blocking agents haloperidol and pizotyline blocked both LSD‐induced polysome shift in maternal brain and fetal organs and LSD‐induced hyperthermia. Fine dissection of adult rabbit brain showed that the extent of LSD‐induced disaggregation of polysomes does not exhibit any marked regional variation.
Effects of glutamine on the evolution of severe acute pancreatitis in rats

Abstract:Purpose To investigate the effects of glutamine on the evolution of severe acute pancreatitis. Methods Eighty SD rats were randomly allocated to 3 groups: Gln group,SAP group and sham operation group.The rats in Gln and SAP groups were submitted to injection of sodium taurocholate under the pancreatic envelope to induce severe acute pancreatitis and jejunostomy was performed.The Gln group received glutamine and SAP group received saline. While the group had sham operation only. Twenty-four rats in each group were sacrificed 12, 24, 36 hours after the induction of SAP. The histological severity were assessed by Schmidts method. Serum amylase and TNF-α level were measured, and ascites were determined. The apoptosis of pancreatic acinar cells was detected by TUNEL method. Results Necrosis hemorrhage and neutrophil infiltration were observed in the pancreas, the lesions were enhanced in SAP group and were ameliorated in Gln group. Serum amylase increased after the induction of acute panreatitis,Gln group: (6 944.5±798.4) IU/L,SAP group (12 153±2 257) IU/L, the level of TNF-α were lower in Gln group than that in SAP group. Gln group:(176.5±17.65) ng/L, SAP Group:(297.83±36.06) ng/L. The apoptotic rate of pancreatic acinar in the experimental groups were much higher than that in the control group (P<0.01),Gln group were higher (13.07±0.62) than SAP group (6.88±0.52) (P<0.01). Conclusions Glutamine was shown to be able to ameliorate the pathologic changes in severe acute pancreatitis.

Abstract:Objective To investigate the effect of recombinant human growth hormone (rhGH) on rat liver injury with severe acute pancreatitis.Methods Severe acute pancreatitis model was established by injection of sodium taurocholate into the pancreatobiliary duct in 40 male Sprague-Dawley rats.Rats were randomly divided into experiment group (n =20) and control group (n =20).Another 20 male SD rats injected saline served as negative control group.The experiment group were treated with subcutaneously injected rhGH for 3 days,1 U·kg-1 ·d-1.12 h and 24 h after operation,the level of ALT,AST,TNF-α,IL-1 β,SOD,MDA,endotoxin and D-lactate was detected respectively;the degree of live cell apoptosis and pathological score of pancreatic tissue were compared among these groups.Results In comparison with negative control group,the level of ALT,AST,TNF-α,IL-1β,MDA,endotoxin and D-lactate,the liver cell apoptosis index and pathological score of pancreatic tissuc were significantly higher in the control group and experiment group at 12 h and 24 h after operation (P ＜ 0.05).ALT,AST,TNF-α,endotoxin and D-lactate at 12 h and 24 h were significantly lower in the experiment group (P ＜ 0.05).MDA level significantly declined at 12 h and 24 h after operation in experiment group(P ＜ 0.05).Liver cell apoptosis index of the control group was higher than experiment group (P =0.003).Conclusion rhGH pretreatment relieves liver injury in rat with severe acute pancreatitis.

Abstract:BACKGROUND/AIMS: Acute pancreatitis which is characterized by pancreatic inflammation can sometimes be difficult to treat because of limited therapeutic options. The purpose of the study was to assess the effects of agmatine in the acute pancreatitis experimental rat model.MATERIALS AND METHODS: An acute pancreatitis model was created with the administration of cerulein in 40 female Sprague-Dawley rats. Agmatine was administered as a protective agent at 5 mg/kg (low dose) and 10 mg/kg (high dose). The rats were divided into 5 groups, each with 8 rats: group 1 (acute pancreatitis); group 2 (acute pancreatitis+low-dose agmatine 5 mg/kg); group 3 (acute pancreatitis+high-dose agmatine 10 mg/kg); group 4 (placebo, acute pancreatitis+saline); and group 5 (sham and saline infusion). All rats were sacrificed 24 hours after the last injection, and the levels of superoxide dismutase, interleukin-1 beta, and tumor necrosis factor-alpha were assessed in blood samples collected via cardiac puncture. Histopathological examination was performed by a pathologist, who was blind to the groups, according to the Schoenberg's pancreatitis scoring index.RESULTS: The amylase (16.67 and 37.89 U/L), glutathione peroxidase (13.62 and 18.44 ng/mL), tumor necrosis factor-α (39.68 and 64 ng/mL), interleukin-1 (484.73 and 561.83 pg/mL), and transforming growth factor-β (110.52 and 126.34 ng/L) levels were significantly lower and superoxide dismutase (1.29 and 0.98 ng/L) and malondialdehyde (0.99 and 0.96 nmol/mL) levels were significantly higher in group 3 compared to group 1 (P < .05). Moreover glutathione peroxidase, tumor necrosis factor-α, and transforming growth factor-β levels were lower, and malondialdehyde levels were higher in the group 3 compared to group 2 (P < .05). Although the Schoenberg's pancreatitis scoring index was not significantly different between the high- and low-dose treatment groups, rats who received high-dose treatment had significantly lower scores compared to those with acute pancreatitis group.CONCLUSION: This is the first study that evaluated the efficacy of agmatine in an experimental model of acute pancreatitis. Agmatine, an anti-inflammatory and antioxidant agent, had a protective effect in an experimental rat model of acute pancreatitis.

[Prevention and treatment of post-traumatic pancreatic necrosis in patients with blunt abdominal trauma]

Abstract:Introduction: High incidence of necrotic and suppurative complications is feature of acute post-traumatic pancreatitis. Severe trauma of the pancreas and post-traumatic pancreatitis lead to depressurization of ductal system that requires adequate drainage of damaged area and retroperitoneal fat. Material and methods: 95 patients in group 1 received standardized treatment. The victims of the 2nd group (44 patients) were treated using immunoreactive therapy (roncoleukin) and octreotide (the dose depended on the severity of pancreatitis) at early stages. The efficacy of treatment was assessed based on clinical, laboratory and instrumental parameters. Results: Regardless severity of pancreatic injury overall mortality in groups 1 and 2 was 41% and 20.5% respectively. The main causes of adverse outcomes are severe destructive pancreatitis, postnecrotic suppurative complications. Conclusion: Adequacy rather radicalism of surgery should be preferred for blunt pancreatic trauma management. Minimally invasive surgical techniques and new methods of biological hemostasis may be applied. Timely use of anti-enzymatic and immunoactive therapy reduces the risk of severe post-traumatic pancreatitis, suppurative complications and improves outcomes in patients with blunt pancreatic trauma.
