Abstract:Background: A number of studies have investigated the prognostic impact of the neutrophil-to-lymphocyte ratio (NLR) in patients with melanoma but the results were controversial. Therefore, we conducted a meta-analysis to explore the prognostic value of NLR in melanoma. Methods: The databases of PubMed, Embase, and Web of Science were thoroughly searched. Associations between NLR and overall survival (OS) and progression-free survival (PFS) were investigated by pooling hazard ratio (HR) and 95% confidence interval (CI). Results: A total of 12 studies comprising 3207 patients were finally included in the meta-analysis. The results showed that a high NLR was associated with poor OS (HR = 2.23, 95% CI = 1.64–3.04, P < .001, random-effects model) and PFS (HR = 2.19, 95% CI = 1.78–2.69, P < .001, fixed-effects model). Subgroup analyses demonstrated that NLR was still associated with poor OS and PFS for patients in Western countries who were treated with ipilimumab. No significant publication bias was found in this meta-analysis. Conclusion: This meta-analysis demonstrated that a high NLR was predictive of poor OS and PFS in patients with melanoma.
A Single Center Retrospective Study Assessing the Prognostic Significance of Pre-Treatment Neutrophil/lymphocyte Ratio in Locally Advanced Nasopharyngeal Carcinoma

Abstract:Background: In light of the growing evidence suggesting the impact of inflammatory parameters on the survival of individuals with cancer, this research assessed the prognostic significance of the neutrophil-tolymphocyte ratio (NLR) in individuals diagnosed with locally advanced nasopharyngeal carcinoma (NPC) prior to undergoing intensity-modulated radiation therapy (IMRT).Methods: A total of 163 individuals diagnosed with locally advanced NPC treated with IMRT at our hospital between January 2012 and December 2017 were included in this research. For each patient, the absolute counts of neutrophils and lymphocytes were recorded, and the NLR was calculated at the first diagnosis. To determine the optimal cut-off values for NLR, receiver operating characteristic (ROC) curve analysis was conducted. The effects of the determined cut-off value on local failure-free survival (LFFS), overall survival (OS), progression-free survival (PFS), and distant failure-free survival (DFFS) were evaluated employing the Cox regression model.Results: The median follow-up duration for the individuals in this study was 15 months (ranging from 6 to 79 months). According to the determined NLR cut-off value of 3.27, individuals were classified into two groups (high NLR and low NLR). Individuals in the high-NLR group had remarkably poorer 3-year OS (62.8% vs. 91.7%), PFS (51.4% vs. 82.4%), and DFFS (70.7% vs. 89.6%) compared to the low-NLR group. Furthermore, the outcomes of univariate and multivariate survival analyses revealed that NLR served as an independent predictor of DFFS (HR: 2.81, 95% CI: 1.195-6.608, P=0.018), OS (HR: 3.1, 95% CI: 1.211- 7.935, P=0.018), and PFS (HR: 2.21, 95% CI: 1.133-4.292, P=0.02).Conclusions: Elevated NLR exhibited a significant correlation with reduced OS, DFFS, and PFS. These findings suggest that NLR holds promise as a cost-effective and reliable marker for the prediction of clinical outcomes among patients with locoregionally advanced nasopharyngeal carcinoma (LANPC). Furthermore, incorporating NLR into clinical decision-making regarding LANPC treatment strategies may contribute to a more targeted approach aimed at reducing the risk of distant failure.
Prognostic Value of Post-Radiotherapy Neutrophil-to-lymphocyte Ratio in Locally Advanced Nasopharyngeal Carcinoma

Abstract:To explore the temporal profile of the peripheral neutrophil-to-lymphocyte ratio (NLR) in patients with locally advanced nasopharyngeal carcinoma (LANPC) and the potential prognostic value of its dynamic changes. Complete blood count of 112 patients from a previous phase II study were retrospectively collected at the timepoints of the initiation of induction chemotherapy (pre-IC), within 1 week before radiotherapy started (pre-RT), and within 1 week after radiotherapy finished (post-RT). Data of 103 patients were fully recorded and Cox regression analysis was used to analyze the correlations of potential risk factors with 5‑year overall survival (OS). The performance of the prognostic factor was validated in another independent cohort of 103 matched (by T and N stage) patients selected from 236 consecutive NPC patients treated with IC and concurrent chemoradiation. Multivariate analysis (MVA) identified patient age >50 years old (hazard ratio [HR] = 3.4, p = 0.02), weight loss during RT >7.5% (HR = 3.2, p = 0.03), and post-RT peripheral NLR >7.05 (vs. NLR ≤7.05, HR = 2.5, p = 0.04, 5‑year OS 71.4% vs. 87.8%) as unfavorable prognostic factors for OS. There was also a non-significant trend in the MVA that patients with post-RT peripheral NLR >7.05 showed worse progression-free survival (PFS; HR = 1.9, p = 0.06, 5‑year PFS 64.1% vs. 81.8%). Post-RT NLR had a good prognostic performance in the validation cohort (concordance index = 0.73, standard error 0.10; p = 0.02, Wilcoxon test). Post-RT NLR is an independent prognostic factor for OS in LANPC patients. The dynamic change of the routinely tested inflammatory variable could help selection of appropriate treatment options and follow-up strategies.
Combined pretreatment neutrophil-lymphocyte ratio and platelet-lymphocyte ratio predicts survival and prognosis in patients with non-metastatic nasopharyngeal carcinoma: a retrospective study

Abstract:The clinical significance of the combination of neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio (PLR) is unclear. This study investigated the predictive value of pretreatment NLR (pre-NLR) combined with pretreatment PLR (pre-PLR) for the survival and prognosis of nasopharyngeal carcinoma (NPC). A total of 765 patients with non-metastatic NPC from two hospitals were retrospectively analyzed. The pre-NLR-PLR groups were as follows: HRG, high pre-NLR and high pre-PLR. MRG, high pre-NLR and low pre-PLR or low pre-NLR and high pre-PLR. LRG, neither high pre-NLR nor high pre-PLR. Receiver operating characteristic (ROC) curves were used to identify the cutoff-value and discriminant performance of the model. We compared survival rates and factors affecting the prognosis among different groups. The 5-year overall survival (OS), local regional recurrence-free survival (LRRFS) and distant metastasis-free survival (DMFS) of NPC patients in HRG were significantly poorer than those in MRG and LRG. The pre-NLR-PLR score was positively correlated with T stage, clinical stage, ECOG, and pathological classification. Multivariate cox regression analysis showed that pre-NLR-PLR scoring system, ECOG, pre-ALB, pre-CRP and pre-LMR were independent risk factors affecting 5-year OS, LRRFS and DMFS. The ROC curve showed that area under the curve (AUC) values of pre-NLR-PLR of 5-year OS, LRRFS and DMFS were higher than those of pre-NLR and pre-PLR. pre-NLR-PLR is an independent risk factor for the prognosis of NPC. The pre-NLR-PLR scoring system can be used as an individualized clinical assessment tool to predict the prognosis of patients with non-metastatic NPC more accurately and easily.

Abstract:Circular dichroism (CD) is one of the few sensitive indicators of molecular configuration. CD spectroscopy has therefore long been used for structure elucidation. Recent trends show, however, an enormous progress of CD spectroscopy in analytical chemistry, either as an independent method, or as a detector in hyphenated separation techniques. Its uses became especially widespread in pharmaceutical analyses, since enantiomeric purity is a crucial criterion of drug quality. Objectives of the present Ph.D. work included fundamental research in CD and related methodologies, but also, its applications in pharmaceutical analysis. A technical prerequisite of our research work was the instrumental hyphenation of separation and chiroptical detection techniques, which has been achieved by the combination of high performance liquid chromatographic and circular dichroism spectroscopic devices. Our home-built apparatus was the first HPLC-CD instrument in Hungary. Its uses allowed the elaboration of a new method for the determination of peak homogeneity, which applies simultaneous, dual CD/UV detection, and a subsequent ratio-formation. An improved version of this method resulted in the deconvolution of overlapping chromatographic peaks. Another type of peak homogeneity control method was elaborated by rapid, on-line recording of CD spectra in the progress of elution, in order to analyse apparently homogeneous peaks. Circular dichroism has also been applied to quantitate enantiomeric purity in composite samples without any separation of the analytes, and also, to determine equilibrium constants of protonating chiral compounds.

Abstract: The aim of this retrospective study was to evaluate the prognostic significance of pretreatment Neutrophil-to-Lymphocyte Ratio(NLR) in locally advanced non-small cell lung cancer(NSCLC) patients treated with thoracic radiotherapy. We retrospectively analyze 420 patients who received thoracic radiotherapy alone, sequential chemoraiotherapy or concurrent chemoradiotherapy for locally advanced stage NSCLC from January 2007 to December 2010 of our hospital. The patients were divided into two groups (high NLR group and low NLR group) with appropriate cutoff point using the receiver operating characteristic (ROC) curve method. The survival curve was established by Kaplan-Meier method. The Log-rank test was used to compare the survival of the two NLR groups and the multivariate analysis was carried out by Cox regression model. Among the 420 patients, 99 received radiotherapy alone, 139 received sequential chemoradiotherapy and 182 received concurrent chemoradiotherapy. 345 patients died and 75 were still alive. The median follow-up time was 5.2 years and the median overall survival was 22 months. The cut-off value of pretreatment NLR was 2.1. The 5-year PFS and OS rates in high NLR group and low NLR group were 10.6% vs 15.7% (=0.033) and 15.5% vs 22.7% (=0.012). Multivariate analysis confirmed that pretreatment NLR (hazard ratio 1.06, =0.041) was independent prognostic factor of OS. Our study revealed that the pretreatment NLR is the independent prognostic factor of OS in patients with locally advanced stage NSCLC treated with thoracic radiotherapy. However, NLR is still greatly influenced by patient's condition and treatment which needs further research.

Abstract:e18082 Background: Neutrophil to lymphocyte ratio (NLR) has been shown to be a prognostic indicator for different types of cancer. We aimed to investigate the prognostic significance of NLR in advanced non-small cell lung cancer (NSCLC) patients. Methods: From January 2007 to December 2010, total 201 patients who were histologically diagnosed NSCLC, staged for the TNM stage III or IV and chemotherapy-naïve were included. All patients were followed up until August 1st 2011. NLR was calculated from lymphocyte and neutrophil count on routine blood tests taken before diagnosis or first-line therapy. Results: The median survival time of all included patients was 354 days and mean value of NLR was 4.06. Cases were distributed into two groups at NLR value of 4.06. Multivariate analysis demonstrated that NLR was an independent and significant prognostic factor in overall survival (OS) (Hazard Ratio, HR=1.659, 95% Confidence Interval, CI=1.075-2.559, p=0.022). Conclusions: The results suggested that the elevated NLR predicted poor OS for advanced NSCLC patients. It may be a potential biomarker to stratify higher risk of death in advanced NSCLC patients, and this kind of patients may be supposed to more aggressive treatment to pursuit longer survival.

Abstract:Background The neutrophil-lymphocyte ratio (NLR) has been proposed as an indicator of systemic inflammatory response and may predict the clinical outcome in some cancers, such as head and neck cancer and gastric cancer. However, the value of this ratio is variable in different cancers. Studies of the relationship between NLR and both survival and response to chemoradiation have been limited with respect to locally advanced rectal cancer. Methods and materials From 2006 to 2011, 199 consecutive locally advanced rectal cancer patients who were treated with neoadjuvant chemoradiation in the Shanghai Cancer Center were enrolled and analysed retrospectively. Tumor response was evaluated by pathological findings. The baseline total white blood cell count (WBC) and the neutrophil, lymphocyte, platelet counts were recorded. The neutrophil-lymphocyte ratio (NLR) and the relationship with clinical outcomes such as overall survival (OS) and disease-free survival (DFS) was analyzed. Results With ROC analysis, the baseline NLR value was found to significantly predict prognosis in terms of OS well in locally advanced rectal cancer patients. A multivariate analysis identified that a cut-off value of NLR ≥ 2.8 could be used as an independent factor to indicate decreased OS (HR, 2.123; 95% CI, 1.140-3.954; P = 0.018). NLR ≥ 2.8 was also associated with worse DFS in univariate analysis (HR, 1.662; 95% CI, 1.037-2.664; P = 0.035), though it was not significant in the multivariate analysis (HR, 1.363; 95% CI, 0.840-2.214; P = 0.210). There was no observed significant correlation of mean value of NLR to the response to neoadjuvant chemoradiation. The mean NLR in the ypT0-2 N0 group was 2.68 ± 1.38, and it was 2.77 ± 1.38 in the ypT3-4/N+ group, with no statistical significance (P = 0.703). The mean NLR in the TRG 0–1 group was 2.68 ± 1.42, and it was 2.82 ± 1.33 in the TRG 2–3 group with no statistical significance (P = 0.873). Conclusions An elevated baseline NLR is a valuable and easily available prognostic factor for OS in addition to tumor response after neoadjuvant therapy. Baseline NLR could be a useful candidate factor for stratifying patients and making treatment decisions in locally advanced rectal cancer.
Prognostic role of the neutrophil-to-lymphocyte ratio in patients with primary central nervous system lymphoma

Abstract:Neutrophil-to-lymphocyte ratio (NLR) is one of the parameters of a complete blood cell count (CBC) test and has been reported to be an easily accessible prognostic marker in aggressive cancer, including non-Hodgkin lymphoma (NHL). Primary central nervous system lymphoma (PCNSL) is an extranodal NHL with highly aggressive features. However, the importance of the NLR has never been assessed in PCNSL. This retrospective study enrolled 62 biopsy-proven patients whose baseline NLR was available, and reviewed their medical records to compare both high (≥2.0) and low NLR (<2.0) groups, in terms of clinical characteristics and outcomes. The low NLR group showed significantly better response rates to induction chemotherapy compared to the high NLR group (p=0.041). At a median follow-up of 41.5 months, the high NLR group revealed a significantly worse 3-year overall survival (OS) (42.5 vs. 71.2%; p=0.031) and a worse 3-year progression-free survival (PFS) (37.3 vs. 60.1%; p=0.028). Univariable Cox analysis results showed that a high NLR at diagnosis was a poor prognostic factor for both 3-year OS (HR 2.64, 95% CI 1.06-6.60; p=0.038) and 3-year PFS (HR 2.41, 95% CI 1.07-5.42; p=0.034). However, multivariable analyses adjusting for International Extranodal Lymphoma Study Group (IELSG) score and induction chemotherapy regimen with rituximab, which were strongly prognostic in this study, showed no statistical significance even with the high NLR group's tendency towards a worse 3-year OS (HR 2.36, 95% CI 0.84-6.62, p=0.102) and a worse 3-year PFS (HR 2.28, 95% CI 0.93-5.63, p=0.073). In conclusion, given that NLR is simple and easily obtainable, it might play a potentially prognostic role in PCNSL from early disease onset.
Prognostic Value of Lymph Node Necrosis at Different N Stages in Patients with Nasopharyngeal Carcinoma

Abstract:Background: Lymph node necrosis (LNN), including retropharyngeal nodal necrosis and cervical nodal necrosis, which is related to radiotherapy/ chemotherapy resistance, is a common phenomenon in nasopharyngeal carcinoma (NPC). This study was to assess the prognostic value of LNN at different N stages in NPC patients. Materials and Methods: In total, 1,665 newly diagnosed NPC patients at stage TxN1-3M0 from two centers were enrolled. Univariate and multivariate models were constructed to assess the association between LNN and long-term survival outcomes. The propensity score matching method was performed to balance treatment groups for baseline characteristics. Results: Of the 1,665, 540 patients (540/1665, 32.4%) were diagnosed with LNN, of which 54.1% (292/540) patients were at stage N1, 31.3% (169/540) at stage N2, and 14.6% (79/540) at stage N3. Univariate and multivariate analyses indicated LNN as an independent predictor for progression‑free survival (PFS), overall survival (OS), distant metastasis-free survival (DMFS), and locoregional relapse-free survival (LRRFS) in stage N1-3 patients (all P<0.001). When patients were analyzed according to stage, similar findings were observed for N1 patients (all P<0.001); for N2 patients, LNN independently predicted PFS (P=0.003), OS (P=0.011), and DMFS (P=0.004), and for stage N3, LNN only independently predicted LRRFS (P=0.019). 123 pairs of patients who received induction chemotherapy plus concurrent chemoradiotherapy or only concurrent chemoradiotherapy were matched, adding induction chemotherapy improved 5-year OS, PFS and LRFFS, but the results were not statistically significant. Conclusions: In NPC patients, LNN could independently predict poor prognosis at all N1-3 stages and at each N stage (N1 to N3). The value of adding induction chemotherapy to concurrent chemoradiotherapy in patients with LNN still requires further prospective studies.

The Prognostic Role of Pre-Treatment Neutrophil-to-Lymphocyte Ratio in an Asian Cohort of Patients with Oropharyngeal Squamous Cell Carcinoma

Abstract:Purpose: The neutrophil-to-lymphocyte ratio is a simple biomarker that reflects the balance between the systemic inflammatory and immunity status. Here we investigate the prognostic role of pre-treatment neutrophil-to-lymphocyte ratio (NLR) in an Asian cohort of oropharyngeal squamous cell carcinoma (OPSCC) patients. Methods: A retrospective review of OPSCC patients from a tertiary institution was conducted. The NLR was calculated from the haematological specimen taken within a month before treatment. Survival rates were estimated via the Kaplan–Meier method, and Cox proportional hazards regression was performed for univariable and multivariable analyses. The NLR cutpoint was determined using maximally selected log-rank statistics. Results: In a cohort of 148 OPSCC patients, 43% were p16-positive and 44% were p16-negative, with a median follow-up of 24 months. The p16-positive patients were younger (median age 62 vs. 67 years) and exhibited a lower prevalence of heavy smoking (47% vs. 69%). The p16-negative cases frequently presented at an advanced disease stage (74% vs. 41%), with a history of previous radiotherapy (26% vs. 3%). The p16-negative patients displayed a higher median NLR (2.91 vs. 2.49). The 3-year disease-specific survival (DSS) in p16-positive was higher compared to p16-negative patients (89.9% vs. 41.6%). The optimal NLR cutpoint was determined as 3.56 and predicted for decreased DSS (hazard ratio [HR] 2.59, p = 0.004). Multivariable analysis revealed smoking, high NLR ≥ 3.56, and p16-negativity as independent variables associated with poorer DSS and overall survival (OS) across the cohort. Conclusion: A high NLR is independently prognostic of poorer DSS in OPSCC, independent of p16 and smoking status. A NLR of more than 3.56 was highly prognostic for poorer survival and warrants further validation in larger cohorts of OPSCC.

Abstract:Neutrophil-to-lymphocyte ratio (NLR) was reported as an independent prognostic factor in many studies, but its cutoff point was not yet concluded. We set forth to prove and validate cutoff point of NLR as a poor prognostic factor for overall survival (OS) in nonmetastatic nasopharyngeal carcinoma (NPC) patients.Retrospective cohort of nonmetastatic NPC adult patients treated with intensity-modulated radiotherapy with curative aim at Siriraj hospital during 2007 to 2014 was enrolled. NLR was defined as absolute neutrophil count divided by absolute lymphocyte count. OS was the primary outcome. We explored our cutoff value by maximum concordance index (C-index) method, and we validated our cutoff and previously reported cutoff values by categorizing patients as NLR ≤ 3 or >3. Internal validation was done by bootstrapping method.Four hundred sixty-three patients were included. The median follow-up time was 70.8 months. By the end of June 2019, 211 patients had died. In univariable analysis of OS by Cox model, an NLR value of 3 showed the highest C-index (0.548) with an HR of 1.43 (95% CI: 1.08-1.89). After adjustment for body mass index, overall staging, age, gender, and histology in multivariable analysis, an NLR >3 was still an independent prognostic factor of poor OS (HR = 1.34, 95% CI = 1.01-1.79). After internal validation, the resampling method shows no overfitting condition and corrected C-index was 0.547 for univariable analysis.A cutoff point of NLR of 3 from routine blood test was found to be an independent poor prognostic factor among patients with nonmetastatic NPC. This prognostic factor could be included in clinical prediction model of NPC and this further prediction model would select high risk patients for intensive treatment.

Abstract:The present study aimed to clarify the prognostic role of the pre‐treatment neutrophil‐to‐lymphocyte ratio (NLR) for the response to neoadjuvant chemotherapy (NAC) in locally advanced breast cancer (LABC). Due to conflicting results in currently available data, the specific focus of the present study was on evaluating the associations between the pre‐treatment NLR and the rate of achieving a pathological complete response (pCR) and survival outcomes. For the present study, data from a cohort of 465 consecutive patients with LABC who underwent NAC at King Feisal Specialist Hospital and Research Center (Riyadh, Saudi Arabia) between 2005 and 2014 were obtained from a prospective BC database and analyzed. Patients were stratified into two groups based on an optimal NLR cut‐off determined using the receiver operating characteristic curve. Logistic regression analyses were conducted to assess variables associated with pCR, and Cox regression analyses were used to assess variables associated with survival outcomes. The low pre‐treatment NLR group (≤2.2) was found to exhibit a higher likelihood of achieving a pCR (odds ratio, 2.59; 95% CI, 1.52‐4.38; P 2.2). Sub‐group analysis revealed that the observed significance in survival outcomes was driven by the triple‐negative BC (TNBC) subgroup. Patients with residual TNBC disease and a high pre‐treatment NLR were observed to have lower 5‐year DFS (44.4 vs. 75.0%; P=0.02) and 5‐year OS (55.9 vs. 84.5%; P=0.055) rates compared with those with residual TNBC disease and a low NLR. To conclude, data from the present study suggest that the pre‐treatment NLR can serve as a viable independent prognostic factor for pCR following NAC in patients with LABC and for survival outcomes, particularly for patients with TNBC.

Abstract:Purpose/Objective(s) Efficacy of immune checkpoint inhibitors (ICI) in curative head and neck squamous cell carcinoma (HNSCC) may be hampered by therapeutic ablation of tumor-draining lymphatics with standard chemoradiation (CRT). Recent emergence of neoadjuvant immunotherapy in HNSCC highlights the need for biomarkers to optimize anti-tumor immunity while eliminating locoregional microscopic disease. An elevated neutrophil-to-lymphocyte ratio (NLR) may reflect an unfavorable balance between pro-tumor inflammation and anti-tumor immunity and is associated with poor immunotherapy outcomes, yet its role as a biomarker of neoadjuvant HNSCC immunotherapy is unknown. We studied the association between NLR dynamics and prognosis in HNSCC patients treated with neoadjuvant immuno-chemotherapy followed by dose and volume response-stratified CRT. Materials/Methods 106 (71 HPV positive and 35 HPV negative) locoregionally advanced HNSCC patients prospectively enrolled in two clinical trials (NCT03107182; NCT039449150) were treated with neoadjuvant nivolumab and chemotherapy followed by dose and volume response-stratified treatment with reduced or standard CRT (including elimination of elective nodal radiation). NLRs for each patient were calculated at baseline, after neoadjuvant therapy, and 1, 3, 6, 9, and 12 months after treatment completion. NLRs at baseline and after neoadjuvant therapy were divided into quartiles, with high NLRs being the highest quartile (>3.66 and >5.63, respectively). NLR differences between reduced and standard CRT groups were analyzed by Wilcoxon ranked sum test. Overall survival (OS) and progression free survival (PFS) were analyzed by Kaplan-Meier and Cox regression methods. Results Among HPV positive patients, 37% were Stage II/III; among HPV negative patients, 97% were Stage IV (AJCC 8th ed.). 77% of all patients received reduced CRT. Median follow-up was 26.7 months. High NLRs after neoadjuvant nivolumab/chemotherapy were associated with worse OS and PFS (p=0.006 and p=0.03, respectively), while baseline high NLRs demonstrated a non-significant trend toward worse OS and PFS (p=0.16 and p=0.10, respectively). In multivariate analysis controlling for tobacco use, ECOG, stage/HPV status, treatment stratification, and PD-L1 CPS, high NLRs after neoadjuvant nivolumab/chemotherapy remained independently associated with worse OS (HR: 5.58, 95% CI: 1.51-20.56, p=0.01) and PFS (HR: 3.09, 95% CI: 1.10-8.67, p=0.03). During follow-up, NLRs were significantly lower in patients receiving reduced CRT compared to those receiving standard CRT at all time points after treatment (p<0.05). Conclusion High NLRs after neoadjuvant immuno-chemotherapy associated with worse survival in locoregionally advanced HNSCC patients. Reduced CRT led to more favorable NLRs which may enhance anti-tumor immunity after neoadjuvant immunotherapy. Further work characterizing dynamic changes in immune phenotype with neoadjuvant HNSCC immunotherapy is warranted.

Abstract:Former FCC Chairman William Kennard recently characterized the Internet network neutrality debate as nothing but a battle between the “extremely wealthy” and the “merely rich,” suggesting it was a distraction from truly important telecom-related issues. Kennard misses the point. The outcome of this controversy will affect everybody who comes into contact with the Internet, and as usual the interests of ordinary consumers are being left in the lurch. Anti-neutrality forces (primarily the increasingly conglomerated telecommunications provider giants) have been manipulating this controversy to their own advantage, and to the detriment of nearly everyone else. Starting from Defense Department research origins, the Internet and its ancestors have thrived by providing essentially neutral channels of communications, with the networks themselves not imposing unreasonable constraints on applications using their facilities, from email to video streaming, and everything in between. But to the telephone companies and their ilk, neutral transmission isn’t an adequate profit center. They want a cut of everybody’s action, as exemplified by AT&T’s CEO Edward Whitacre infamously swiping at Google and other major Internet services, claiming they were using “his pipes” for free. His assertion is an utter fallacy, and the anti-neutrality folks know it. We all already pay for our Internet access. Google pays for its connectivity—undoubtedly not petty cash either. Every DSL or Internet cable hookup is feeding money into telecom company coffers. Even if we choose to use VoIP phone services, we’re still paying a phone company or cable TV firm for the underlying Internet circuits. Much of the anti-neutrality argument is simple greed in action. The telecom providers have watched their traditional business models decay around them and are looking for new ways to strangle any real competition, in league with public relations and lobbying spin aimed at obscuring this fundamental fact. If that sounds too strong, let’s remember that the telecom landscape is littered with the broken promises and unfair tactics of the dominant telephone companies in particular—promised broadband rollouts never delivered, cherry-picking lucrative neighborhoods for advanced services deployments, major rate hikes when regulatory scrutiny is lifted, and so on. These are textbook examples of predatory practices. No wonder it’s so difficult to believe these telecom firms now, or why so many observers feel that laws mandating neutrality— enacted today, before neutrality slips away—are the only practical approach to maintaining Internet fairness. The telecom providers’ historical behaviors, increasingly restrictive ISP Terms of Service requirements, and the generally lackadaisical or even consumer-hostile attitudes of the FCC and most other related regulators are a clear warning. Outrageously skewed bandwidth pricing, demands for profit participation, blocking or throttling of services that are viewed as competitive, and other punitive telecom provider actions are likely to occur if it’s possible to get away with them. Anti-neutrality proponents have incorrectly suggested that pro-neutrality arguments are invalidated by a large and powerful firm such as Google taking a strong stance in favor of network neutrality. Google does have a financial interest in the outcome, but so do the rest of us. In the non-neutral Internet world envisioned by the telecom providers—with only after the fact, glacially slow antitrust suits as the main possible redress for abuses—it’s doubtful that Google, Vonage, eBay, or many other successful Internet companies could have afforded to really get off the ground in the first place. A non-neutral Net would likely be a death knell for an entire future of competitive Internet entrepreneurs who might otherwise have brought us a vast range of useful new services, especially start-ups and other initially small businesses. Neutrality is an aspect of the Internet that is so taken for granted that it seems invisible and intrinsic, but it has been critical to the Internet’s success to date. It’s unfortunate the network neutrality controversy has escalated to an emotional level, which sometimes obscures underlying facts. But most Internet users simply don’t realize how drastically and negatively they could be affected if anti-neutrality arguments hold sway. Getting true network neutrality back after it’s been lost is likely to be effectively impossible. Except for the anti-neutrality camp itself, we’d all be worse off with a non-neutral Internet, and that’s a risk we simply must not accept.
Prognostic value of neutrophil-to-lymphocyte ratio change in patients with locally advanced non-small cell lung cancer treated with thoracic radiotherapy

Abstract:In prior investigations, a correlation was established between patient outcomes in locally advanced non-small cell lung cancer (LA-NSCLC) following thoracic irradiation and parameters, such as pre/post-treatment neutrophil-to-lymphocyte ratio (NLR) and NLR change (ΔNLR). However, these parameters could potentially be influenced by radiation-related variables, such as gross tumor volume (GTV). The primary aim of this study was to elucidate the factors impacting post-treatment NLR and ΔNLR and to further assess their prognostic relevance. In this retrospective study, a cohort of 188 LA-NSCLC patients who underwent thoracic radiation between 2012 and 2017 was assessed. The calculation of pre/post-treatment NLR involved the use of absolute neutrophil and lymphocyte counts. ΔNLR was defined as the difference between post- and pre-treatment NLR values. To assess the relationships between various variables and overall survival (OS), local progression-free survival (LPFS), and distant metastasis-free survival (DMFS), the Kaplan–Meier technique and Cox proportional hazards regression were employed. Additionally, Spearman's rank correlation analysis was carried out to investigate correlations between the variables. The analysis revealed that both post-treatment NLR (r = 0.315, P < 0.001) and ΔNLR (r = 0.156, P = 0.032) were associated with GTV. However, OS, LPFS, and DMFS were not independently correlated with pre/post-treatment NLR. ΔNLR, on the other hand, exhibited independent associations with OS and DMFS (HR = 1.054, P = 0.020, and P = 0.046, respectively). Elevated ΔNLR values were linked to poorer OS ( P = 0.023) and DMFS ( P = 0.018) in the Kaplan–Meier analysis. Furthermore, when stratifying by GTV, a higher ΔNLR remained to be associated with worse OS and DMFS ( P = 0.047 and P = 0.035, respectively) in the GTV ≤ 67.41 cm 3 group, and in the GTV > 67.41 cm 3 group ( P = 0.028 and P = 0.042, respectively), highlighting ΔNLR as the sole independent predictive factor for survival and metastasis, irrespective of GTV.

Abstract:BACKGROUND In recent studies, accumulating evidence has revealed a strong association between the inflammatory response and the prognosis of many tumors. There is a certain correlation of neutrophil-to-lymphocyte ratio (NLR) with the prognosis in gastric cancer (GC) patients undergoing neoadjuvant chemotherapy (NAC). However, the existing research results have remained controversial. AIM To explore the relationship between NLR ratio and prognosis of GC patients receiving NAC. METHODS A thorough systematic search was performed in databases such as PubMed, Embase, Web of Science, and Cochrane Library, the search is available until February 29, 2024, and studies exploring the interaction of NLR with clinical outcomes were collected. Relevant studies meeting pre-defined inclusion and exclusion criteria were carefully chosen. The outcomes included progression-free survival (PFS), relapse-free survival, disease-free survival (DFS), and overall survival (OS). The hazard ratio (HR) and its corresponding 95% confidence interval (CI) were utilized for estimation. RESULTS Our analysis encompassed 852 patients and incorporated data from 12 cohort studies. The comprehensive analysis revealed a significant association of high NLR with reduced OS (HR = 1.76; 95%CI: 1.22-2.54, P = 0.003), relapse-free survival (HR = 3.73; 95%CI: 1.74-7.96, P = 0.0007), and PFS (HR = 2.32; 95%CI: 1.42-3.81, P = 0.0008) in patients. However, this correlation in disease-free survival was not significant. NLR demonstrated its crucial role in effectively predicting the OS of GC patients undergoing NAC at different detection times, ages, regions, and NLR thresholds. CONCLUSION In GC patients receiving NAC, an elevated NLR is strongly associated with reduced OS and PFS. NLR has become an effective biomarker for patient prognosis evaluation, providing valuable insights for the treatment strategies of NAC in GC patients.

Abstract:Objective. To research the impact of neutrophil-lymphocyte ratio (NLR) as a prognostic parameter in non-small-cell lung cancer (NSCLC) patients treated with epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs). Methods. We searched the databases such as the American Society of Clinical Oncology (ASCO), EMBASE, PubMed, the European Society of Medical Oncology (ESMO), Wanfang, and CNKI for articles illustrating the impact of pretreatment NLR on survival data in NSCLC patients undergoing EGFR-TKIs treatment. We did a meta-analysis for overall survival (OS) and progression-free survival (PFS). Results. We recruited 10 studies in our meta-analysis. Our study suggested that patients with low NLR had better PFS (hazard ratio (HR) = 1.67, 95% confidence interval (CI) = (1.16–2.39), and P value = 0.005) and OS (HR = 1.66, 95% CI = (1.08–2.55), and P value = 0.02) in comparison to patients with high NLR. Conclusion. In conclusion, our meta-analysis revealed that lower NLR predicted a better survival (PFS and OS) in patients receiving the treatment of EGFR-TKIs.

Abstract:The clinical influence of the neutrophil-to-lymphocyte ratio (NLR) in predicting outcomes in patients with locally advanced rectal cancer (LARC) who achieve a pathological complete response (pCR) to neoadjuvant chemoradiotherapy (NACRT) has seldom been investigated. We retrospectively recruited 102 patients with LARC who achieved a pCR to NACRT and the association of NLR status with survival and tumor recurrence in the patients was analyzed. Thirteen patients (12.7%) developed tumor recurrence. A high NLR (≥3.2) was significantly associated with tumor recurrence (p = 0.039). The 5-year OS rates in patients with a low NLR and patients with a high NLR were 95.1% and 77.7%, respectively (p = 0.014); the 5-year DFS rates in patients with low NLR and patients with a high NLR were 90.6% and 71.3%, respectively (p = 0.031). The Cox proportional hazards model indicated that an NLR of ≥3.2 was an independent poor prognostic factor for DFS (hazard ratio [HR] = 3.12, 95% confidence interval [CI] = 1.06-9.46, p = 0.048) and OS (HR = 6.96, 95% CI = 1.53-35.51, p = 0.013). A pretreatment high NLR (≥3.2) was a promising predictor of reduced OS and DFS in patients with LARC who achieved a pCR to NACRT.
