Abstract:Purpose: Local recurrences of nasopharyngeal carcinoma (NPC) may be salvaged by reirradiation with conventional techniques, but with significant morbidity. Intensity-modulated radiation therapy (IMRT) may improve the therapeutic ratio by reducing doses to normal tissue. The aim of this study was to address the efficacy and toxicity profile of IMRT for a cohort of patients with locally recurrent NPC. Methods and materials: Between August 2003 and June 2009, 70 patients with radiologic or pathologically proven locally recurrent NPC were treated with IMRT. The median time to recurrence was 30 months after the completion of conventional radiation to definitive dose. Fifty-seven percent of the tumors were classified asrT3-4. The minimum planned doses were 59.4 to 60 Gy in 1.8- to 2-Gy fractions per day to the gross disease with margins, with or without chemotherapy. Results: The median dose to the recurrent tumor was 70 Gy (range, 50-77.4 Gy). Sixty-five patients received the planned radiation therapy; 5 patients received between 50 and 60 Gy because of acute side effects. With a median follow-up time of 25 months, the rates of 2-year locoregional recurrence-free survival, disease-free survival, and overall survival were 65.8%, 65.8%, and 67.4%, respectively. Moderate to severe late toxicities were noted in 25 patients (35.7%). Eleven patients (15.7%) had posterior nasal space ulceration, 17 (24.3%) experienced cranial nerve palsies, 12 (17.1%) had trismus, and 12 (17.1%) experienced deafness. Extended disease-free interval (relative risk 2.049) and advanced T classification (relative risk 3.895) at presentation were adverse prognostic factors. Conclusion: Reirradiation with IMRT provides reasonable long-term control in patients with locally recurrent NPC.
Long-term survival and late complications of intensity-modulated radiotherapy for recurrent nasopharyngeal carcinoma

Abstract:Background: To evaluate the effectiveness and toxicities of intensity-modulated radiotherapy (IMRT) for locally recurrent nasopharyngeal carcinoma (NPC). Methods: One hundred and eighty-four previously irradiated NPC patients with recurrent disease and re-irradiated by IMRT between February 2005 to May 2013 had been reviewed. The disease was re-staged I in 33, II in 27, III in 70 and IV in 54 patients. Seventy-five percent of the patients received cisplatin-based chemotherapy. Results: The median survival time was 33 months. The 3-year actuarial rates of local recurrence-free survival (LRFS), distant metastases-free survival (DMFS), and overall survival (OS) rates were 85.1, 91.1, and 46.0%, respectively. About 53% of the patients experienced Grade 3-4 late toxicities. Forty-four patients died of massive hemorrhage of the nasopharynx caused by radiation induced mucosal necrosis. Multivariate analysis indicated that chemotherapy and time interval between initial radiotherapy and re-irradiation were independent predictors for DMFS. Conclusion: IMRT is an effective method for patients with locally recurrent NPC. Massive hemorrhage of the nasopharynx is the major sever late complication and also the leading cause of death. Early recurrence is negative factor for DMFS. Combination of chemotherapy can improve DMFS, but not for OS. Optimal salvage treatment strategies focusing on improvement of survival and minimization of late toxicities are warranted.
Preliminary Results of Re-irradiation for Locally Recurrent Nasopharyngeal Carcinoma with Intensity Modulated Radiotherapy

Abstract:Background and purpose:Treatment for locally recurrent nasopharyngeal carcinoma(NPC) was difficult and complicated.This study aimed to study for the treatment outcome,radiation toxicity and the prognostic factors in patients with locally recurrent NPC treated with intensity-modulated radiation therapy(IMRT) in our cancer center.Methods:An analysis of 108 NPC patients with local recurrence who were re-irradiated with IMRT between 2006 and 2010 was conducted.According to the 2002 American Joint Committee on Cancer Stage Classification,the distribution of disease restaging was 31.4 % for stageⅠ,8.3% for stageⅡ,32.4% for stageⅢ and 27.7% for stageⅣ.Median time from initial radiotherapy to the nasopharyngeal recurrence was 51 months(range,8-276).All patients were treated with 6MV photon with 7-9 fields using IMRT.The toxicities were evaluated by Common Toxicity Criteria(CTC),version 3.0.Results:The 5 year overall survival,progression free survival,local recurrence-free survival and distant metastasis-free survival were 59%,56%,52.9% and 55.8%,respectively.44 patients died,19 patients died of nasopharynx hemorrhea,the others died of metastasis or exhaustion.Grade 3–4 toxicities were reported in 24 patients.In a univariate analysis,patient age,recurrent T,recurrent stage were significant prognostic factors for overall survival and local recurrence-free survival.Recurrent T also was the prognostic factors for progression free survival and distant metastasis-free survival.In a multivariate analysis,patient age and recurrent stage remained significant for overall survival.Conclusion:Re-irradiation using IMRT is available to improve local tumour control and to prolong patients' survival.High dose prescription may result in radio-necrosis of nasopharyngeal mucosa and bleeding.The hemorrhea was the main death cause.The incidence of serious toxicities was also serious;and affected the quality of life.Younger patient ages,lower T stage and recurrent stage are independent prognostic factors for overall survival and local recurrence free survival of recurrent NPC.

Abstract:The present study presents the first known use of the A-B Rating Scale, a measure of Type A behavior, with 276 female college students. The purpose of this research was to explore the factor structure of the A-B scale and especially to attempt a replication of the factor structure across samples. Analysis gave a three-factor solution (Drivenness, Hypomania, and Leadership). Congruence coefficients indicated that the scale possesses significant factor matching across some of the factors. Research aimed at assessing the criterion validity of the three factors is called for.

Abstract:More than 10% of nasopharyngeal cancer (NPC) patients develop local recurrence after definitive radiotherapy or chemoradiation. Re-irradiation (re-RT) with X-ray-based intensity-modulated radiotherapy (IMXT) is the most accepted treatment for salvaging locally recurrent NPC (LR-NPC) especially for those at locally advanced stages. However, the reported 2-year overall survival (OS) rates only ranged between 45∼60% for locally advanced LR-NPC after salvage IMXT largely due to treatment-induced toxicities. Due to its inherent physical and biological advantages, carbon-ion radiotherapy (CIRT) is feasible and potentially effective for LR-NPC. Here we report the 1-year outcomes of our phase I/II trial in terms of survival, disease control, and toxicities of LR-NPC patients treated at the maximal tolerable dose (MDT) of CIRT re-RT. This open-label single arm phase I/II trial enrolled pathologically and/or clinically confirmed LR-NPC (age 18∼70 years) diagnosed >12 months after the completion of initial IMXT. In the phase I study, the Time-to-Event Continual Reassessment Method was used to determine, via a dose-escalation scheme, the MTD of CIRT re-RT. The primary end point of the phase II part was patients' 2-year OS. Severe (i.e., Grade 4 or 5) acute and subacute toxicities were analyzed. All patents were treated at 3 Grey Equivalent (GyE) per daily fraction, 5 days a week, using intensity-modulated CIRT with raster-scanning technology. Neoadjuvant chemotherapy (cisplatin + docetaxel or gemcitabine) was recommended for patients with stage III-IVA/B LR-NPC, but concurrent chemotherapy was prohibited. Sixteen patients were accrued to the phase I dose escalating (range 51∼63GyE in 17∼21 fractions) protocol. No patient experienced grade 4 toxicities, thus the MTD was set at 63 GyE/21 fractions, the highest of the protocol dose scheme. Thirty-nine LR-NPC patients (AJCC stage I/II/III/IVA-B disease in 4/8/10/17 patients) were accrued to the phase II study. All patients completed planned 63GyE/21 daily fractions without break. With a median follow-up of 8.4 (range 2.0∼21.9) months, 1 patient died from massive hemorrhage subsequent to mucosal necrosis. The 1-year overall, local progression-free, regional recurrence-free, and distant metastasis free survival rates were 95.0%, 87.1%, 96.4%, and 100%, respectively. No patient experienced acute toxicity of ≥ grade 3. Grade 3 or above late toxicities observed included mucosal necrosis (5.1%), xerostomia (2.6%), hearing loss (5.1%), and temporal lobe necrosis (2.6%). This was the first prospective clinical trial to evaluate the efficacy and safety of CIRT for LR-NPC. Our results suggested that CIRT provided promising OS at 1 year with infrequent severe toxicities.

Abstract:We have developed a dual-topology/dual-coordinate free-energy simulation method for use with a QM/MM force field. By combining two parallel processes into one alchemical process, we are able to compute the double free-energy difference (delta deltaF) within a single simulation, which eliminates half of the expensive quantum-mechanical simulation in general. The method has been tested in computing the solvation free-energy differences of several molecular pairs and shows close agreement with experimental results.
Sequential Chemotherapy and Intensity-Modulated Radiation Therapy in the Management of Locoregionally Advanced Nasopharyngeal Carcinoma: Experience of 370 Consecutive Cases

Abstract:Abstract Introduction To investigate the outcome of locoregionally advanced nasopharyngeal carcinoma (NPC) treated with intensity-modulated radiation therapy (IMRT) after induction chemotherapy, with or without concomitant chemotherapy. Methods Between August 2003 and March 2007, 370 patients with locoregionally advanced NPC were treated with IMRT. Presenting stages were stage IIB in 62, stage III in 197, and stage IVA/B in 111 patients. All patients except for 36 patients with cervical lymphadenopathy of 4 cm or less in diameter received 2 cycles of cisplatin-based neoadjuvant chemotherapy. Forty-eight patients received cisplatin-based concurrent chemotherapy as well. Results With a median follow-up time of 31 months (range 5 to 61 months), the 3-year local control, regional control, metastasis-free survival (MFS), disease-free survival (DFS) and overall survival (OS) rates were 95%, 97%, 86%, 81% and 89%, respectively. Multivariate analyses revealed that both age (≤ 60 vs. >60) and N-classification are significant prognosticators for OS (P = 0.001, hazard ratio [HR] 2.395, 95% confidence interval [CI] 1.432-4.003; P = 0.012, hazard ratio [HR] 2.614, 95% confidence interval [CI] 1.235-5.533); And N-classification is the only significant predicative factor for MFS (P = 0.002, [HR] 1.99, 95% CI 1.279-3.098). T-classification and concurrent chemotherapy were not significant prognostic factors for local/regional control, MFS, DFS, or OS. Subgroup analysis revealed that concurrent chemotherapy provided no significant benefit to IMRT in locoregionally advanced NPC, but was responsible for higher rates of grade 3 or 4 acute toxicities (50% vs. 29.8%, P < 0.005). No grade 3 or 4 late toxicity including xerostomia was observed. However, two patients treated with IMRT and neoadjuvant but without concurrent and adjuvant chemotherapy died of treatment related complications. Conclusion IMRT following neoadjuvant chemotherapy produced a superb outcome in terms of local control, regional control, MFS, DFS, and OS rates in patients with stage IIB to IVB NPC. Effective treatment strategy is urgently needed for distant control in patients diagnosed with locoregionally advanced NPC.
Efficacy and Toxicity of Intensity Modulated Carbon Ion Radiation Therapy for Local Recurrent Nasopharyngeal Cancer

Abstract:To compare the short-term outcomes with regards to efficacy and toxicity after reirradiation using intensity-modulated carbon-ion radiation therapy (IMCT) or intensity-modulated radiation therapy (re-IMXT) in nasopharyngeal cancer (NPC) patients who developed local recurrence (LRNPC) after their initial, definitive IMXT course. The short-term outcomes of 14 patients retreated for LRNPC between May of 2015 and September of 2015, who were followed-up for at least 3 months after salvage IMCT, were compared to those of 47 similar patients retreated with repeat IMXT between 2012 and 2014. Dose of salvage IMCT and re-IMXT were 50∼60 GyE (2.0∼2.5 GyE/day) and 54∼66Gy (2.0∼2.1 Gy/day), respectively. More patients in the re-IMCT group had recurrent T3 or T4 diseases (71.4% vs. 44.7%, P = 0.079). Other characteristics of the patients and disease including: patients’ gender, age, stage at initial diagnosis, time interval between the initial IMXT and local recurrence, dose of initial IMXT, and the use of chemotherapy during initial treatment, and N-classification at recurrence were similar between the 2 groups. Complete response rates at 3-month follow-up were 64.3% vs. 78.7% between the salvaging IMCT and re-IMXT groups, respectively (P = 0.271). No patients developed local disease progression in either group, but 2 patients died after re-IMXT including 1 from massive nasopharyngeal hemorrhage. The probability of acute mucositis (31.9% vs. 0, P = 0.014) and mucosal necrosis/hemorrhage (29.8% vs. 0, P = 0.026) were significantly higher in patients who received re-IMXT as compared to those who received salvage IMCT. In addition, 2 patients developed symptomatic temporal lobe necrosis after re-IMXT, while none developed this in the IMCT group. When locally recurrent NPC after primary definitive IMXT is retreated with either IMCT or IMXT, similar response rates and overall survival times within 6 months after re-irradiation are observed. However, no patient who received salvage IMCT developed acute or subacute toxicities of grade 2 or above, whereas 29.8% patients experienced life-threatening mucosal necrosis/hemorrhage after re-IMXT. The long-term outcome of salvage IMCT in LRNPC needs further follow-up.
Dimerization-induced folding of MST1 SARAH and the influence of the intrinsically unstructured inhibitory domain: low thermodynamic stability of monomer.

Abstract:The serine/threonine mammalian sterile 20-like kinase (MST1) is involved in promotion of caspase-dependent and independent apoptosis. Phosphorylation and oligomerization are required for its activation. The oligomerization domain, denoted as SARAH domain, forms an antiparallel coiled coil dimer, and it is important for both MST1 autophosphorylation and interactions with other proteins like the Rassf proteins containing also a SARAH domain. Here we show that the monomeric state of SARAH is thermodynamically unstable and that homodimerization is coupled with folding. Moreover, the influence of the inhibitory domain on SARAH stability and affinity is addressed. By investigating the thermal denaturation using differential scanning calorimetry and circular dichroism, we have found that the SARAH domain dissociates and unfolds cooperatively, without a stable intermediate monomeric state. Combining the data with information from isothermal titration calorimetry, a low thermodynamic stability of the monomeric species is obtained. Thus, it is proposed that the transition from MST1 SARAH homodimer to some specific heterodimer implies a non-native monomer intermediate. The inhibitory domain is found to be highly flexible and intrinsically unfolded, not only in isolation but also in the dimeric state of the inhibitory-SARAH construct. The existence of two caspase recognition motifs within the inhibitory domain suggests that its structural flexibility might be important for activation of MST1 during apoptosis. Moreover, the inhibitory domain increases the thermodynamic stability of the SARAH dimer and the homodimer affinity, while having almost no effect on the SARAH domain in the monomeric state. These results emphasize the importance of flexibility and binding-induced folding for specificity, affinity, and the capacity to switch from one state to another.
Advantages of Intensity Modulated Radiotherapy in Recurrent T1-2 Nasopharyngeal Carcinoma: a Retrospective Study.

Abstract:BACKGROUND:Recurrent T1-2 Nasopharyngeal Carcinoma (rT1-2) may be salvaged by 3D - CRT (3D-Conformal Radiotherapy), IMRT (Intensity Modulated Radiotherapy), Brachytherapy (BT), BT with external radiotherapy. The purpose of this study is to address the efficacy and toxicity profile of aforementioned four modalities for rT1-2 NPC.METHODS:168 patients, median age 48 years (range 16-75 years) proven rT1-2 NPC were diagnosed and treated with four different irradiation modalities (3D-CRT, IMRT, BT, BT with external radiotherapy). Median time to recurrence was 30 months (range 1-180 months). The median follow-up time was 28 months (range, 4-135 months).RESULTS:161 patients completed a median dose of 6445 cGy (ranging 30 to 87 Gy). Seven patients prematurely terminated their treatment due to acute side-effects and received 30-49 Gy. The 1- and 3-year local regional recurrent free survival (LRRFS), distant free survival (DFS), and overall survival (OS) rates were 82.03% vs. 82.03% vs. 82.58%, 51.33% vs. 51.33% vs. 53.41, respectively. Gender and recurrence T-classification were the two significant adverse prognostic factors for LRRFS, DFS, and OS rates. Grade 3 or 4 toxicities were tolerable.CONCLUSION:3D-CRT, IMRT, BT, BT with external radiotherapy are feasible and efficacious for rT1-2 NPC. In toxicity 3D-CRT/IMRT group is lower than BT group. IMRT is superior for rT1-2 NPC.
Failure patterns and individualized treatment plans of reirradiation for inoperable locally recurrent nasopharyngeal carcinoma

Abstract:Re-irradiation with intensity-modulated radiotherapy (IMRT) remains the primary treatment modality for inoperable locally recurrent nasopharyngeal carcinoma (NPC). However, the rate of radiation-related late adverse effects is often substantially high. Therefore, we aimed to explore failure patterns and individualized treatment plans of re-irradiation for inoperable locally recurrent NPC. Ninety-seven patients who underwent IMRT were retrospectively analyzed. Sixty-two patients had clinical target volume of recurrence (rCTV) delineated, and thirty-five patients had only gross tumor volume of recurrence (rGTV) delineated. Twenty-nine patients developed second local failures after re-irradiation with IMRT (28 cases available). Among those patients, 64.3% (18/28) of patients and 35.7% (10/28) developed in-field or out-field, respectively. No statistical correlation was observed between target volume (rGTV or rCTV) and the local recurrence rate, local failure patterns, grade ≥ 3 toxicity, and survival. Multivariate analysis showed that recurrent T (rT) stage (HR 2.62, P = 0.019) and rGTV volume (HR 1.73, P = 0.037) were independent prognostic factors for overall survival (OS). Risk stratification based on rT stage and rGTV volume revealed that low risk group had a longer 3-year OS rate (66.7% vs. 23.4%), lower total grade ≥ 3 toxicity (P = 0.004), and lower re-radiation associated mortality rates (HR 0.45, P = 0.03) than high risk group. This study demonstrates that the delineation of rCTV may not be beneficial for re-irradiation using IMRT in locally recurrent NPC. Patients with low risk were most suitable for re-irradiation, with maximizing local salvage and minimizing radiation-related toxicities. More precise and individualized plans of re-irradiation are warranted.
Intensity-modulated radiotherapy for 69 locally recurrent nasopharyngeal carcinoma cases:analysis of efficacy and prognostic factors

Abstract:Objective To examine retrospectively the efficacy and prognostic factors of locally recurrent nasopharyngeal carcinoma(NPC)treated with intensity-modulated radiotherapy(IMRT).Methods Sixty nine locally recurrent NPC patients were performed IMRT,the second irradiation total prescription dose was 49.5～77.4Gy(median fractional dose 66Gy),and fractional dose was 1.862.5Gy(median fractional dose 2.1Gy).Forty eight patients were treated with 1～6 courses of cisplatin-based chemotherapy.Results The median follow-up time was 20 months.The rates of 1-year local progression-free survival,distant metastasis-free survival,and overall survival were 92.9%,81.8% and 81.8%,respectively.The rates of 2-year local progression-free survival,distant metastasis-free survival,and overall survival were 88.8%,65.5% and 65.5%,respectively.The result of univariate analysis showed that both the first RT modality(P=0.004)and the second irradiation dose(P=0.011)were positively related to the survival time,respectively.The result of multivariate analysis showed that both the first RT modality(P=0.004)and the second irradia-tion dose(P=0.004) influenced the prognosis of recurrent NPC after radiotherapy.The toxicity response during radiotherapy was well tolerated in patients.Conclusion IMRT is effective to locally recurrent NPC and can improve survival rate.Both the first RT modality and the second irradiation dose are independent prognostic factors of the overall survival for locally recurrent NPC patients.

Abstract:Objective: To investigate the patterns of local failure and prognosis in patients with locally recurrent nasopharyngeal carcinoma (rNPC) after primary intensity-modulated radiotherapy (IMRT). Methods: The data of 298 patients with locally rNPC after IMRT were retrospectively analyzed. Magnetic resonance images of the initial and recurrent tumors were reviewed and, for patients with extra-nasopharyngeal local recurrence, the gross tumor volume of local recurrence was transferred to the original IMRT plan for dosimetry analysis. Significant prognostic factors for overall survival (OS) were selected by multivariate Cox regression analysis. Results: The commonest recurrence sites were the nasopharynx (93%, 277/298) and skull base (53.7%, 160/298). Of the 21 patients with extra-nasopharyngeal recurrence (19 cases valid), 12 had in-field failures, 4 had marginal failures, and 3 had out-field failures. The ethmoid sinus (57.1%, 4/7) and nasal cavity (28.6%, 2/7) were the most frequent sites of marginal and out-field failures. After median follow-up of 37 months, the 3-year and estimated 5-year OS rates were 57.3% and 41.7%, respectively. Multivariate analysis showed that age, recurrence interval, plasma Epstein-Barr virus (EBV) DNA level, and recurrent T stage were independent prognostic factors for OS. Conclusions: Local failure after IMRT occurs most commonly in the nasopharynx and skull base. In patients with extra-nasopharyngeal recurrence, in-field failure remains the main failure pattern, and marginal and out-field failures mainly occur in the ethmoid sinus and nasal cavity. Elder age, shorter recurrence interval, detectable plasma EBV DNA, and advanced recurrent T stage are negative predictors of OS in patients with rNPC.
Preliminary Results of Nasopharyngeal Carcinoma Treated with Intensity-Modulated Radiotherapy: a Retrospective Study of 364 Patients

Abstract:The aim of the study was to evaluate the survival and toxicity of 364 patients with nasopharyngeal carcinoma (NPC) treated by intensity-modulated radiotherapy (IMRT). Cisplatin-based chemotherapy was given to patients with local-regionally advanced disease. The median follow-up was 26 months (range 3-62 months). The 2-year local failure-free survival, regional failure-free survival (RFFS), distant metastasis-free survival (DMFS) and overall survival (OS) were 97.6, 96.8, 89.1 and 93.5 %, respectively. Overall disease failures (at any site) were found in 60 patients. Eighteen patients experienced locoregional failures: seven were local only, seven were regional only and four were both local and regional. Forty-two patients developed distant metastases. Of these, 30 patients had single organ metastasis and 12 had multiple organ metastases. The most common acute toxicities were dermatitis, mucositis and xerostomia. Grade 0-2 dermatitis, mucositis and xerostomia occurred in 337 patients (92.6 %), 204 patients (56.1 %) and 364 patients (100 %), respectively. Grade 3 dermatitis, mucositis and xerostomia were seen in 27 patients (7.4 %), 160 patients (44 %) and 0 patients. No Grade 4 acute toxicities were observed. N stage was an independent prognostic factor for RFFS, DMFS and OS. Our preliminary results showed that IMRT provides excellent local-regional control for NPC, with acceptable acute toxicities. Distant metastasis remains the most difficult treatment challenge. More effective systemic chemotherapy should be explored.
Long-term Patterns of Regional Failure for Nasopharyngeal Carcinoma Following Intensity-Modulated Radiation Therapy

Abstract:Purpose: To analyze the long-term patterns of regional failure following intensity-modulated radiotherapy (IMRT) for nasopharyngeal carcinoma (NPC). Methods: From January 2005 to December 2010, 275 non-metastatic NPC patients treated with IMRT were retrospectively enrolled. Patients staged as II (lymph nodes measuring 4 or more cm in diameter), III or IV also received chemotherapy. Failures were assessed as in-field or out-field relative to the pretreatment planning computed tomography data sets. Univariate and multivariate analyses were performed with Cox proportional hazards model to analyze the effect of various prognostic factors on regional failure-free survival (RFFS) and overall survival (OS). Results: During a median follow-up of 71 months, the RFFS and OS rates were 94.3% and 83.9%, respectively. Seventeen patients developed regional failures, of which 16 were in-field; one patient showed an out-field failure in the parotid gland, and no recurrences were seen for level Ib. Failures in level II and in the retropharyngeal area accounted for 70.6% (12/17) and 52.9% (9/17) of all failures, respectively. The 5-year RFFS rates for patients with classifications of N0-1 and N2-3 were 98.5% and 90.2%, respectively (p = 0.001). Multivariate analysis showed that N stage was the only independent prognostic predictor of RFFS (HR 7.363, 95% CI 1.516-35.756, p = 0.013). Conclusions: The regional failure of NPC after treatment with IMRT is uncommon but is significantly higher in N2-3 patients than in N0-1 patients. In-field failures represent the main pattern of regional recurrence and are most often detected in level II and in the retropharyngeal area, while out-field failure is rare. Close attention should be directed to NPC patients with advanced N stages.

Abstract:Background Salvage endoscopic nasopharyngectomy has better survival prognosis and fewer complications in the management of early stage rNPC, compared to re-irradiation. However, the treatment modality of advanced recurrent nasopharyngeal carcinoma (rNPC) remains controversial. Thus, the purpose of this study was to investigate the demographics, clinical outcomes, and prognostic factors associated with salvage endoscopic nasopharyngectomy in advanced rNPC. Methods This study conducted a retrospective analysis of advanced rNPC patients who underwent salvage surgery betweenm January 2014 and December 2019. The overall survival (OS) and progression-free survival (PFS) were analyzed. Univariable and multivariable analyses of OS and PFS were performed using the Cox regression model. The predicted values of the parameters were determined by means of the receiver operating characteristic (ROC) curve analysis. Results Among the 120 patients included, there were 75 patients with rT3 stage and 45 patients with rT4 stage. With the median follow-up time of 18 months,the 3 -year OS and PFS were 55.2% and 29.4%, respectively. Multivariate analyses showed that the rNPC patients with older age, low BMI (Body Mass Index), rT4 stage, tumor necrosis, and tumor invasion into the ICA was predictive of worse OS, whereas low BMI and rT4 stage were associated with worse PFS. In addition, the rT stage was identified as a better predictor of OS (area under the ROC curve: 0.669; P =0.003) than the other clinical features. Conclusions Salvage treatment using endoscopic nasopharyngectomy appears to be an effective treatment in the management of patients with advanced rNPC. In addition, case matching studies and prospective studies with larger clinical samples are required to further evaluate the efficacy of endoscopic surgery compared with re-irradiation in advanced rNPC.

Abstract:[Abstra ct] Objective To investigate the long-term efficacy and adverse effects of intensity-modulated radiotherapy (IMRT) for nasopharyngeal carcinoma (NPC).Methods A total of 869 patients with biopsy-proven NPC without distant metastasis who underwent the whole course of IMRT from 2009 to 2010 were enrolled.Of all the patients, 84.8%received cisplatin-based chemotherapy.The prescribed dose to the primary lesion in the nasopharynx was 66-70Gy in 30-32 fractions, and the dose to the positive lymph nodes in the neck was 66 Gy in 30-32 fractions.The Kaplan-Meier method was used to calculate survival rates, the log-rank test was used for difference analysis and univariate prognostic analysis , and the Cox proportional hazards model was used for multivariate prognostic analysis .Rseu lts The 5-year overall survival( OS ) , local recurrence-free survival, regional recurrence-free survival, distant metastasis-free survival, and disease-free survival ( DFS ) were 84.0%, 89.7%, 94.5%, 85.6%, and 76.3%, respectively.In the patients with locally advanced NPC,concurrent chemotherapy tended to reduce distant metastasis (83.6%vs.75.7%, P=0.050) and improve OS (82.6%vs.77.0 %, P=0.082).Induction chemotherapy tended to improve OS ( 80.7% vs.71.4%, P=0.057 ) , and the induction chemotherapy containing docetaxel or gemcitabine tended to improve OS (83.3%vs.72.2%, P=0.058).The patients who received a boost after the initial radiotherapy had a significantly lower DFS rate than those who did not (52.2%vs.71.1%, P=0.004).The concurrent chemotherapy increased the incidence rates of long-term xerostomia and trismus, while a high dose of cisplatin increased the incidence rates of xerostomia and hearing impairment.Conclusions IMRT for NPC provides satisfactory long-term efficacy.Concurrent chemotherapy combined with IMRT tends to reduce the incidence of distant metastasis, and other values need further investigation.The boost therapy after radiotherapy may be associated with poor prognosis.Chemotherapy increases the incidence of long-term toxicities.

Abstract:Abstract OBJECTIVES To evaluate clinical characteristics and long-term outcomes of individualized re-treatment in patients with different locoregionally recurrent patterns of nasopharyngeal carcinoma (NPC). MATERIALS AND METHODS From January 2005 to December 2018, sixty-five locoregionally recurrent NPC patients were included in the study. The location of recurrence determined different treatments with radiotherapy (RT) and/or surgery. RESULTS The median age at the time of re-treatment was 51 years old. Twenty-nine (44.6%) patients had tumors classified as T3-4 and forty-four (67.7%) patients had tumors classified as N2-3 in the initial treatment. All patients were previously treated with intensity-modulated radiotherapy (IMRT) to a median dose of 66Gy (range, 66–70.4Gy). The median time between initial treatment and recurrence was 25 months (range: 10–105 months). Tumor relapse was classified as rT3-4 in 19 patients and rN2-3 in 8 patients. The median interval between the date of locoregional relapse to the date of death or last follow-up was 28 months (range: 0 to 112 months). The overall survival (OS) at 1-year, 3-years, and 5-years was 94.4%, 67.3%, and 47.2%, respectively. Age at the time of recurrence (> 45 years) altered the OS significantly (P = 0.007). CONCLUSIONS Locoregionally recurrent NPC should be individualized re-treatment. Therapy modality according to the recurrent site achieved relatively satisfactory long-term survival outcomes. Age at the time of recurrence (> 45 years) served as negatively prognostic factor of OS.

Abstract:Objectives: To identify the failure patterns and prognostic factors of nonmetastatic nasopharyngeal carcinoma (NPC) in the intensity-modulated radiotherapy (IMRT) era. Methods: Data on 847 patients with newly diagnosed, non-disseminated NPC treated by IMRT between 2012 and 2016 were retrospectively reviewed. Survival outcome, failure patterns and prognosis factors were analyzed. Results: The 5-year local relapse-free survival, nodal relapse-free survival, distant metastasis-free survival, disease-free survival, and overall survival rates were 94.3%, 95.3%, 84.8%, 76.5% and 85.7%, respectively. The major local recurrence sites were the nasopharynx (91.5%, 43/47) and skull base (68.1%, 32/47); 39 patients had in-field failures, four had marginal failures, and four had out-field failures. Level IIb (62.2%, 23/37) was the most frequent regional recurrence site, followed by IIa (35.1%, 13/37) and retropharyngeal region (32.4%, 12/37); 35 cases had in-field failure alone, one had out-field failure alone, and one had both in- and out-field failure. TNM stage was the most significant factor for prognosis prediction. 402 (47.5%) patients had acute adverse events of grade 3 or 4; leukopenia (31.5%) and mucositis (26.7%) was the most common hematological and non-hematological event, respectively. Late complications were slight or moderate damages; xerostomia (647/847, 76.4%) and hearing impairment (422/847, 49.8%) remained the most troublesome. Conclusion: NPC patients treated with IMRT obtained satisfactory survival outcomes. The key failure pattern was distant metastasis. The main pattern of local-regional failure was in-field failure. Screening high risk patients with distant metastases and optimizing radiotherapy targets should be studied.
